Abstract

Marine temperature extremes are anomalous ocean temperature events, often persisting over several weeks or longer, with potential impacts on physical and ecological processes that often encompass socio-economic implications. In recent years, a considerable effort has been directed at the development of metrics allowing an objective characterization of both marine heatwaves (MHWs) and marine cold spells (MCSs). However, the majority of these metrics do not consider explicitly the spatial extent of the events. Here, we rank and evaluate the relative importance of marine temperature extreme events thanks to a metric, called activity, that combines the number of events, duration, intensity and spatial extent. According to this definition, in the Mediterranean basin between 1982 and 2021, summer 2018 experienced slightly more MHW activity than summer 2003, documented as an exceptional extreme event. Besides, MHW activities were higher in the last two decades while winter MCS activities were higher in the 1980s-1990s. The highest MHW activities occurred preferentially in the western Mediterranean while the strongest MCS activities took place preferentially in the eastern Mediterranean. Moreover, the duration, mean intensity, and activity of the three strongest MHWs are twice as high as those of the three strongest MCSs. The long-term tendency of extreme events activity shows an accelerated increase for summer MHWs (about +150°C.days.10⁶km²) and a linear decrease for winter MCSs in the Mediterranean (about -60°C.days.10⁶km²) over the last four decades.

Abstract

Marine heat waves (MHWs) and cold spells (MCSs) are anomalous ocean temperature events that occur in all oceans and seas with great ecological and economic impacts. The quantification of the relative importance of marine temperature extreme events is often done through the calculation of local metrics, the majority of them not considering explicitly the spatial extent of the events. Here, we propose a ranking methodology to evaluate the relative importance of marine temperature extreme events between 1982 and 2021 within the Mediterranean basin. We introduce a metric, generically termed activity, combining the number of events, duration, intensity and spatial extent of: i) summer MHWs and ii) winter MCSs. Results at the entire Mediterranean scale show that the former dominate in the last two decades while the latter are prevalent in the 1980s and 1990s. Summers with the highest MHW activity were 2018, 2003 and 2015 and winters with the strongest MCS activity took place in 1992, 1984 and 1983. The highest MHW activity occurred in the Gulf of Lion while the highest MCS activity took place preferably in the Aegean basin. According to our proposed definition, the three strongest MHWs almost double the duration, mean intensity, and activity of the three strongest MCSs. The long-term tendency of activity shows a rapid increase for summer MHWs and a linear decrease for winter MCSs in the Mediterranean over the last four decades. Abstract

The aim of this study was to estimate the incidence of COVID-19 disease in the French national population of dialysis patients, their course of illness and to identify the risk factors associated with mortality. Our study included all patients on dialysis recorded in the French REIN Registry in April 2020. Clinical characteristics at last follow-up and the evolution of COVID-19 illness severity over time were recorded for diagnosed cases (either suspicious clinical symptoms, characteristic signs on the chest scan or a positive reverse transcription polymerase chain reaction) for SARS-CoV-2. A total of 1,621 infected patients were reported on the REIN registry from March 16th, 2020 to May 4th, 2020. Of these, 344 died. The prevalence of COVID-19 patients varied from less than 1% to 10% between regions. The probability of being a case was higher in males, patients with diabetes, those in need of assistance for transfer or treated at a self-care unit. Dialysis at home was associated with a lower probability of being infected as was being a smoker, a former smoker, having an active malignancy, or peripheral vascular disease. Mortality in diagnosed cases (21%) was associated with the same causes as in the general population. Higher age, hypoalbuminemia and the presence of an ischemic heart disease were statistically independently associated with a higher risk of death. Being treated at a selfcare unit was associated with a lower risk. 5. Long-term warming and interannual variability contributions’ to marine heatwaves in the Mediterranean

Abstract

In the past 40 years, marine heatwaves (MHWs) have experienced a worldwide increase in duration, intensity, frequency and spatial extent. This trend has been particularly evident in the Mediterranean, where exceptional events were observed during the summers of 2022, 2018 and 2003. This study proposes a twofold analysis of MHWs in the Mediterranean, focusing on their statistical characteristics and physical causes. A satellite dataset is utilized to analyze MHWs via an index, called activity, which aggregates the occurrence, duration, intensity and spatial extent of events. Our results show that the trend toward more active summers for MHWs is strongest in the western Mediterranean basin and long-term warming is the main driver in the whole Mediterranean basin. We also show that in the western and Adriatic Mediterranean region, the increase of SST variability contributes about a third to the MHW activity long-term trend whereas in the central, eastern and Aegean basins, the variability of SST mostly acts to diminish this trend. Through principal component analysis (PCA) of MHW activity, we found that the three most severe summer MHW events in the Mediterranean occur at the same location where the overall trend is highest. Interannual variability increased MHW activity in 2022 around the Balearic Sea, in 2018 in the eastern basins and in 2003 in the central basins. A joint PCA revealed that the long-term trend in MHW activity co-varies with a positive geopotential height anomaly over the Mediterranean, which is consistent with the generation of atmospheric-driven MHWs and which, at the North Atlantic scale, resembles the positive phase of the summer East Atlantic. The additional interannual variability contribution to these three severe summers was associated with western warming and projected onto the positive phase of the summer North Atlantic Oscillation. The increase in MHW over the last 40 years is also associated in the western, central and Adriatic regions with increased downward short-wave radiation and in the eastern Mediterranean with decreased upward long-wave radiation. Increased upward latent heat flux partly compensated for the MHW long-term increase over the whole Mediterranean basin. The interannual variability of MHW activity is related in the western, central and Adriatic basins to increased downward sensible and decreased upward latent heat flux possibly due to warm and humid air intrusion.

Objectives Pentraxin 3 (PTX3), in common with myeloperoxidase and proteinase 3, is stored in human neutrophil granules and is expressed on apoptotic neutrophil surface. We therefore investigated the presence of anti-PTX3 autoantibodies (aAbs) in the sera of antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis (AAV) patients. Methods Presence of anti-PTX3 autoantibodies was analysed by a specific enzyme-linked immunosorbent assay in sera from 150 patients with microscopic polyangiitis (MPA), granulomatosis with polyangiitis (GPA), and eosinophilic granulomatosis with polyangiitis (EGPA), and in sera of 227 healthy subjects (HS), 40 systemic sclerosis (SSc) patients, and 25 giant cell arteritis patients (GCA). Using indirect immunofluorescence on fixed human neutrophils, we also analyzed the staining pattern associated with the presence of anti-PTX3 aAbs. Abstract

Drug rash with eosinophilia and systemic symptoms (DRESS syndrome) is a rare and severe drug-induced hypersensitivity syndrome characterized by haematological abnormalities (hypereosinophilia and/or mononucleosis) and multiorgan involvement. Renal failure has been rarely described. We report the case of a 77-year-old female with sulphasalazine-induced DRESS syndrome who improved rapidly on corticosteroid treatment. After prednisone withdrawal, the patient developed renal failure that necessitated a session of haemodialysis. A kidney biopsy showed acute tubulointerstitial nephritis with an intense lymphocytic infiltrate and tubular necrosis. Kidney function normalized after a further 2 weeks of corticosteroid treatment. This is the first histologically proven case of acute tubulointerstitial nephritis in the setting of sulphasalazine-induced DRESS syndrome.

Abstract

Rapid changes in the Arctic have been observed and are expected to be amplified in future. However, the influence of Arctic sea-ice decline on global climate remains under debate. We focus on a modeling analysis of the ocean-atmosphere responses to a near-term Arctic sea-ice loss (corresponding to the period 2035-2055 under the RCP8.5 scenario). With the coupled model IPSL-CM5A2, two different methods to melt Arctic sea-ice are implemented to estimate the robustness of the protocol. We will show that modifying the albedo or changing the thermal conductivity of the Arctic sea-ice show similar pattern responses but with different magnitude. It is also found that, at the 10% significance level, Arctic sea-ice decline leads to large-scale circulation responses such as a negative North Atlantic Oscillation (NAO) associated with southward shift of the westerlies; an expansion north of the North Atlantic current; a weaker Atlantic Meridional Overturning Circulation (AMOC) linked to a shallower mix layer depth at subpolar North Atlantic deep-convection site; a southward shift of the Intertropical Convergence Zone (ITCZ); a tropical Pacific cooling and a positive Southern Annular Mode (SAM).
