Objectives: The main goal of the study was to investigate how pregabalin (PGB) affects proinflammatory cytokine release in patients with fibromyalgia syndrome (FMS). Patients and methods: This experimental research study was conducted with 85 female participants (mean age: 49.6±10.1 years; range, 30 to 73 years) between April 2020 and November 2020. Of the participants, 30 were FMS patients using PGB 150 mg/day for at least three months, 30 were FMS patients not using PGB, and 25 were healthy individuals. The detection of FMS was carried out according to the 2010 American College of Rheumatology diagnostic criteria. Levels of proinflammatory cytokines (interleukin [IL]-2, IL-6, IL-12, IL-17, interferon-gamma, and tumor necrosis factor-alpha) were measured by enzyme-linked immunosorbent assay. Results: Serum concentrations of proinflammatory cytokines were remarkably decreased in FMS patients using PGB (p<0.001) and were higher in patients with FMS not using PGB than in healthy subjects (p<0.001). The highest values of proinflammatory cytokines were found in the group of FMS patients not using PGB (p<0.001). Conclusion: These results indicate that PGB inhibits the release of proinflammatory cytokines, suggesting that it can be used as an anti-inflammatory agent in inflammatory cases. - Abstract
Objectives: This study aimed to investigate the physical and emotional effects of the coronavirus disease 2019 pandemic in patients with fibromyalgia syndrome (FMS) and chronic low back pain (CLBP) patients. Patients and methods: The cross-sectional controlled study was performed with 1,360 participants (332 males, 1,028 females; mean age: 42.3±12.5 years; range, 18 to 65 years) between September 2020 and February 2021. The participants were evaluated in three groups: the FMS group (n=465), the CLBP group (n=455), and the healthy control group (n=440). Physical activity, pain levels, and general health status before and during the pandemic were evaluated in all participants. Stress levels were analyzed with the perceived stress scale (PSS) in all groups, and disease activity was analyzed with the fibromyalgia impact questionnaire (FIQ) in patients with FMS. Results: Patients with FMS had worsened general health status and pain levels during the pandemic compared to the other groups (p<0.01). The FMS group showed significantly higher PSS scores than those in other groups (p<0.01). There was a weak-positive correlation between FIQ and PSS parameters in patients with FMS (p<0.05, r=0.385). Conclusion: The general health status, pain, and stress levels of the patients with FMS and CLBP tended to worsen during the pandemic. Aim Aim of the study is to search the effects of whole body vibration (WBV) in fibromyalgia patients. Materials and Methods In this single blind study 20 fibromyalgia patients were diagnosed according to 2010 American College of Rheumatology criteria whom participated in the study with the mean age of 57 ± 7 (years). Participants were randomized into 2 equal groups. In the intervention group (n = 10), 6 different types of exercises were combined with 30 hz frequency, 2 mm amplitude WBV for 4 weeks, The control group (n = 10) performed the same exercises on the same platform without vibration. The patients were assessed at baseline, at the 3rd and 6th months by Visual Analogue Scale (VAS), Fibromyalgia Impact Questionnaire (FIQ) and Beck Depression Inventory (BDI). Results Only FIQ score was significantly better in the treatment group at the 6th month evaluation (p = 0.043). Conclusion WBV is found to be effective in reducing symptoms of fibromyalgia.

Objective: To evaluate the relationship between ultrasonographic femoral cartilage thickness and presence of concomitant osteoporosis in a group of female patients with knee osteoarthritis (OA). Materials and Methods: This study included 118 women with knee OA who visited our outpatient clinic. Demographic data were collected, radiologic grading using Kellgren Lawrence (K-L) scale, ultrasonographic femoral cartilage thickness (FCT) evaluation, pain intensity evaluation, disability evaluation using OA index [Western Ontario and McMaster Universities Osteoarthritis index (WOMAC)], quality of life measurement using Short Form-36 (SF-36) and bone density measurement using dual-energy X-ray absorptiometry (DXA) were conducted for each patient. Results: We found that 58 patients (median age: 64.5 years, range: 50-75) had osteoporosis (group 1) and 60 patients (median age: 62 years, range: 51-75) did not have osteporosis (group 2). Group 2 had higher body mass index (BMI) in addition to lower WOMAC, SF-36 physical function, physical role limitation, pain and social function scores. The severity of osteoporosis and K-L staging were negatively correlated. The DXA femoral neck and total lumbar T-scores were higher in the advanced stages of OA. FCT had no significant correlation with age, WOMAC index and SF-36 scores. Moreover, the left knee FCT was negatively correlated with BMI. Conclusion: Radiologic staging of OA had a negative correlation with osteoporosis but no significant correlation with the quantitative measurement of FCT using ultrasonography. [ABSTRACT FROM AUTHOR]

The aim of the study is to investigate the fracture risk and related factors of 341 female in patients over 50 years of age between 01.01.2013 and 31.12.2013. Patient information on tea, bisphosphonate intake and conservative clothing were obtained and patients were subjected to 3 meters walking test. Turkey Fracture Risk Assessment Tool (FRAX) was used to calculate the 10 years major osteoporotic fracture (OF) and hip fracture risks of patients with and without BMD by Dual Energy X-Ray Absorptiometry. OF and hip fracture risks of patients with BMD (n=85) and without BMD (n=256) were 1,17%, 16,4 % and 2,3%, 16% respectively. In conclusion, fracture risk assessment in female pa-tients with or without BMD were similar and fracture risk assesment without BMD had significant relationship between risk factors such as; physical activity (p=0.009, r=0.16) and tea intake (OF p=0.030, hip fracture p=0.004)., Bu çalışmanın amacı, kliniğimizde yatarak tedavi gören 50 yaş üzeri kadınlarda kırık riski ve ilgili faktörlerin araştırılmasıdır. Çalışmaya 1 Ocak 2013-31 Aralık 2013 tarihleri arasında yatarak tedavi gören 50 yaş üstü 341 kadın hasta dahil edilmiştir. Hastalar Türkiye kırık Riski Değerlendirme Anketi (FRAX) ile ve günlük çay tüketimi, kapalı giyim, bifosfonat kullanımı açısından sorgulanmış, hastalara 3 metre kalk yürü testi uygulanmıştır. Dual energy X-ray Absorptiometry ile ölçülmüş kemik mineral yoğunluğu (KMY) olan ve olmayan tüm hastaların major osteoporotik kırık (OK) ve kalça kırığı riski hesaplanmıştır. KMY ölçümü olan (n=85) ve olmayanlarda (n=256) 10 yıllık OK ve kalça kırık riskleri sırasıyla %1,17, %16,4 ve %2,3, %16 idi. Sonuç olarak, çalışmamızda KMY'si olan ve olmayan 50 yaş üzeri kadınlarda kırık riski tahmini benzer olup, KMYsi olmayanlarda fiziksel aktivite düzeyi (p=0.009, r=0.16) ve çay tüketiminden (OK p=0.030, kalça kırığı p=0.004) etkilenmektedir.

- Abstract
Amaç: Romatoid artrit (RA) hastalarında yeni bir enflamatuvar belirteç olan sistemik immün enflamasyon indeksi [SII (Platelet sayısı×nötrofil sayısı/lenfosit sayısı)] ile hastalık aktivitesi arasındaki ilişkiyi değerlendirmeyi amaçladık. Yöntem: Tek merkezli kesitsel bir çalışma olarak planladığımız çalışmamıza 18-65 yaş arası 104 RA hastası ve 43 sağlıklı gönüllü olmak üzere toplam 147 katılımcı dahil edildi. Katılımcıların klinik, laboratuvar ve demografik özellikleri kaydedildi. RA hastalarını, hastalık aktivite skoru 28-eritrosit sedimentasyon hızına (DAS28-ESR) göre <2,6 olanları remisyonda RA (n=51) ve >2,6 olanları aktif RA (n=53) olmak üzere iki gruba ayırdık. SII'nin C-reaktif protein (CRP), eritrosit sedimentasyon hızı (ESR), DAS28-ESR, basitleştirilmiş hastalık aktivite indeksi (SDAI) ve klinik hastalık aktivite indeksi (CDAI) ile korelasyonlarını belirlemek için Spearman korelasyon analizini kullandık. Aktif RA grubu ile remisyon RA grubu karşılaştırıldığında, SII ve diğer değişkenlerin hastalık aktivitesini değerlendirmedeki etkinliğini belirlemek için ROC (alıcı işlem karakteristikleri, receiver operating characteristic) eğrisi analizi kullanıldı. Bulgular: RA grubunda sağlıklı kontrollere göre nötrofil düzeyleri daha yüksek, lenfosit düzeyleri ise daha düşüktü (her ikisi için p<0,05). Platelet (PLT) değerleri açısından gruplar arasında anlamlı fark yoktu (p=0,095). Aktif RA grubunda nötrofil, PLT, CRP, ESR, DAS28-ESR, SDAI ve CDAI değerleri remisyondaki RA hastalarına göre daha yüksekti (p<0,05). Lenfosit değerleri açısından ise gruplar arasında anlamlı farklılık saptanmadı (p=0,089). SII, hem RA grubunda sağlıklı kontrollere göre, hem de aktif RA grubunda remisyondaki RA grubuna göre daha yüksek ve istatistiksel olarak anlamlıydı (her ikisi için p<0,001). SII değeri, CRP (rs: 0,627, p<0,001), ESR (rs: 0,383, p<0,001), DAS28-ESR (rs: 0,775, p<0,001), SDAI (rs: 0,796, p<0,001) ve CDAI (rs: 0,798, p<0,001) ile pozitif korelasyon gösterdi. RA hastalık aktivitesini belirlemek için optimal kesme noktası 479,36 (×109/L) [eğri altındaki alan (EAA): 0.968, %95 güven aralığı (GA): 0,914-0,993, duyarlılık: %92,45, özgüllük: %86,27]. The objective is to evaluate clinical efficacy of balneotherapy. 48 Ankylosing Spondylitis (AS) patients, selected according to the Modified New York diagnostic criteria, were randomized into 2 treatment groups; (in every group n=24) Balneotherapy (Group I) and Heated Tap Water Therapy (Group II) combined with exercises. Patients in Group I (N = 24) were treated with hot spring water (38ºC). The outcome measures were; Nottingham Health Profile (NHP), Bath AS Functional Index (BASFI), Bath AS Disease Activity Index (BASDAI), Dougados Functional Index (DFI), global well-being of the patient (GWB), erytrocyte sedimentation rate, morning stiffness, Modified Schober Test (Sch), fingertip-fibula head distance (FFD), chest expansion (CE), chin-manubrium distance and occiput-wall distance. The evaluations were done before treatment end at the 3rd month by double blinding procedure and ITT principle. Improvements were observed in BASDAI (p = 0.002); BASFI (p = 0.04); DFI (p = 0.03); NHP Pain (p= 0.01); GWB (p = 0.02); CE (p = 0.000); Sch (p = 0.04); and FFD (p = 0.03) were determined to be superior in Group I at the end of the 3rd month. The data presented reveals the short-term effectiveness of BT on pain intensity, disease activity, functional capacity, spinal motion and global well being of the AS patients when compared with heated tap water therapy., Çalışmanın amacı, balneoterapinin klinik etkinliğini araştırmaktır. 48 Ankilozan Spondilitli (AS) hasta, Modifiye New York tanı kriterlerine göre belirlendi ve egzersizle kombine Balneoterapi (Grup I) ve ısıtılmış çeşme suyu tedavisi (Grup II) olmak üzere (her biri n=24) 2 gruba randomize edilmiştir. Değerlendirme parametreleri; Notthingham Sağlık Profili (NSP), Bath AS Fonksiyonel İndeks (BASFI), Bath AS Hastalık Aktivitesi İndeksi (BASDAI), Dougados Fonksiyonel İndeks (DFI), Hastanın genel değerlendirmesi (HGD), eritrosit sedimentasyon hızı, sabah tutukluğu, modifiye Schober testi (Sch), parmak ucu fibula mesafesi (PFM), göğüs ekspansiyon (GE), çene-manubrium mesafesi ve oksiput-duvar mesafesidir. Değerlendirmeler tedavi öncesi ve tedavi sonrası 3.ay olmak üzere 2 kez çift kör ve ITT prensibine göre yapılmıştır. 3.ayda BASDAI'de (p=0,002), BASFI'de (p=0,04); DFI'de (p=0,03); NSP Ağrıda (p=0,01); HGD'de (p=0,02); GE'de (p=0,00); Sch'de (p=0,04) ve PFM'de (p=0,03) Grup I lehine anlamlı düzelmeler gözlendi. BT'nin kısa süreli izlemde ağrı, hastalık aktivitesi, fonksiyonel kapasite, spinal flexibilite ve hastanın genel değerlendirmesinde etkili olduğu gösterilmiştir.

Çalışmanın amacı osteoporoz hastalarında demografik özellikler, vücut kitle indeksi (VKI), kalsitriol, parathormon (PTH), alkalen fosfataz (ALF) düzeyleri ile dual-enerji-X ray absorpsiyometri (DXA) değerleri ve kırık oluşumu arasındaki ilişkiyi araştırmaktır. Osteoporoz tanı kriterlerine uyan 310 hasta değerlendirildi. Hastaların serum kalsiyum, kalsitriol, PTH, ALF düzeyleri ve VKİ'leri tespit edildi. DXA lomber total (LT), femur boyun (FB) ve femur total (FT) değerleri ölçüldü. Torakal, lomber yan, pelvis grafileri çekildi. Kadınlarda FT değerleri ve VKI, LT ile FB ve FT; FB ile FT arasında pozitif korelasyon; PTH ile kalsitriol arasında negatif korelasyon saptandı. Erkeklerde LT ile PTH arasında pozitif korelasyon saptandı. 113 (%36.5) hastada kırık saptandı. Bu hastalarda PTH düzeyi kırığı olmayanlara göre daha yüksekti. Kırığı olan kadın hastaların VKİ değerlerinin daha yüksek olduğu görüldü. VKI ile kırık ve erkeklerde LT DXA ile PTH düzeyleri arasında pozitif korelasyon saptanmış olması dışında verilerimiz literatürle uyumludur. Literatürle çelişkili sonuçlar hasta sayımızın azlığıyla ilişkili olabilir., The aim of this study is to assess the relations between demographic properties, body mass indexes (BMI); calcitriol, parathormone (PTH), alkaline phosphatase (ALP) levels, bone fractures and dual-energy-X ray absorptiometry (DXA) T scores of osteoporotic patients. 310 patients with osteoporosis were assessed. Serum calcium, calcitriol, PTH, ALP levels and BMI of patients were determined. DXA lumbar total (LT), femur neck (FN), femur total (FT) values were measured. Thoracal, lumbar lateral, pelvic radiographs were obtained. In women, FT values were positive correlating with BMI, FN and FT were positively correlated with LT; FN and FT were correlated positively with each other. There was a negative correlation between PTH and calcitriol levels. In men, LT and PTH were positively correlated. 113 patients had bone fractures. These patients' PTH levels were higher than patients who were not having fractures. Women with bone fractures had higher BMI values. Except BMI and fractures; in men, LT and PTH were positively correlated. Our results were similar with literature. Exceptional results may be associated with our small patient population.

Bu çalışmanın amacı semptomatik primer diz osteoartritli (OA) hastalarda osteoartritin radyolojik şiddeti ile bireylere ait özelliklerin, klinik bulguların şiddetinin, alt ekstremite dizilim düzgünlüğünün değerlendirilmesinde kullanılan Q açısının, ultrasonografi ile ölçülen patellofemoral kıkırdak kalınlığının, yaşam kalitesi ile disabilite düzeyinin ve genel sağlık algısı arasındaki ilişkisininin değerlendirilmesidir.Çalışmaya Amerikan Romatoloji Cemiyeti (ACR) kriterlerine göre her iki dizinde OA bulunan, dahil edilme ve dışlama kriterlerini karşılayan 100 hasta (82 kadın, 18 erkek) dahil edildi. Bu çalışma için özel değerlendirme formu oluşturuldu. Bu formla hastaların demografik özellikleri, eşlik eden hastalıkları, ailede OA varlığı, semptom süresi, dominant ekstremitesi kaydedildi. Hastaların boy ve kilo ölçümleri yapılarak vücut kitle indeksleri (VKİ) hesaplandı. Diz grafilerinin değerlendirilmesi için Kellgren-Lawrence (K-L) radyolojik evreleme skalası kullanıldı. Hastaların ağrısının değerlendirmesi için Vizüel Analog Skala (VAS) kullanıldı. Ağrı, tutukluk ve fiziksel fonksiyonları değerlendirmek için Western Ontario ve McMaster Üniversiteleri Osteoartrit İndeksi (WOMAC), genel sağlık durumunu değerlendirmek için de Kısa form-36 (KF-36) kullanıldı. Dijital gonyometre ile Q açısı ölçümleri yapıldı. Hastaların ultrasonografik incelemesinde femoral kıkırdak kalınlığı ölçümü yapıldı. Bu çalışmanın amacı diz osteoartriti (OA) olan kadın hastalarda osteoporozu (OP) olan ve olmayanların femoral kıkırdak kalınlıklarını ultrason yardımıyla karşılaştırarak OP varlığı ile femoral kıkırdak kalınlığı arasında herhangi bir ilişki olup olmadığının değerlendirilmesi, OA-OP ilişkisinin araştırılması ve OA'nın kliniği ile femoral kıkırdak kalınlığı arasındaki ilişkinin değerlendirilmesidir.Çalışmaya Amerikan Romatoloji Cemiyeti (ACR) kriterlerine göre her iki dizinde osteoartrit bulunan 118 kadın hasta dahil edildi. Hastaların Dünya Sağlık Örgütü (WHO) OP tanı kriterlerine göre 58'inde OP mevcut iken (Grup-1), 60'ında OP yoktu (Grup-2). Hastaların yaş, menopoz yaşları, çay tüketimi ve OP tedavi durumu sorgulandı. Boy ve kilo ölçümleri yapılarak vücut kitle indeksleri (VKİ) hesaplandı. Hastalarda Ağrı değerlendirmesi için Vizüel Analog Skala (VAS), fonksiyon ve disabilitenin değerlendirilmesi için Western Ontario ve McMaster Üniversiteleri Osteoartrit İndeksi (WOMAC), yaşam kalitesi değerlendirmesi için genel sağlık ölçütü Kısa form-36 (KF-36), iki yönlü diz grafilerinin değerlendirilmesi için Kellgren-Lawrence (K-L) radyolojik evreleme skalası, Kemik Mineral Yoğunluğu (KMY) ölçümü için DEXA kullanıldı. Hastaların ultrasonografik incelemesinde femoral kıkırdak kalınlığı ölçümü yapıldı.Gruplar kilo ve VKİ açısından karşılaştırıldığında OP olmayan grupta kilo ve VKİ değerleri osteoporoz olanlara göre daha yüksek saptandı (p=0,000). Gruplar K-L evreleme açısından karşılaştırıldığında sağ ve sol diz için ileri evrede OP görülme oranının azaldığı saptandı. DEXA ile değerlendirilen Kalça ve Lomber T skor ölçümleri ile K-L diz OA evreleme arasındaki ilişkiyi değerlendirdiğimizde sağ ve sol dizde ileri evre OA'da Femur boyun (FN), Lomber total (LT) ölçümleri yüksek bulundu. K-L evreleme ile VKİ arasında pozitif yönde ilişki saptandı. K-L evreleme ile ultrasonografik femoral kıkırdak kalınlığı ölçümü arasında anlamlı bir ilişki bulunmadı. Gruplar ultrasonografik femoral kıkırdak kalınlığı ölçümü açısından değerlendirildiğinde gruplar arasında fark saptanmadı. Gruplar WOMAC ve KF-36 skorları açısından karşılaştırıldığında OP olmayan grupta istatistiksel olarak WOMAC ağrı, tutukluk, fonksiyon ve total skorları daha yüksek; KF-36 fiziksel fonksiyon, fiziksel rol kısıtlanması, ağrı, sosyal fonksiyon skorları daha düşük bulundu. Femoral kıkırdak kalınlığı ile VAS, WOMAC ve KF-36 skorları arasında ilişki saptanmadı. Hastalar OP tedavi açısından değerlendirildiğinde antirezorptif tedavi alan grupta sağ diz (p=0,036) ve sol dizde (p=0,021) ultrasonografik femoral kıkırdak kalınlığı daha yüksek saptandı.Sonuç olarak radyografik olarak değerlendirildiğinde, OA-OP arasında negatif bir ilişki saptanmasına rağmen bu hastalarda ultrasonografik femoral kıkırdak kalınlığı açısından anlamlı bir ilişki gözlenmedi. OA'nın kliniği ile femoral kıkırdak kalınlığı arasında herhangi bir ilişki saptanmadı. OA'yı radyografik olarak değerlendirmenin kişisel yoruma açık ve değişikliklere duyarlılığının az olması nedeniyle ultrason gibi kantitatif ölçümlerin daha güvenilir sonuçlar sağlayacağını düşünmekteyiz. The aim of this study is to compare ultrasonographic femoral cartilage thickness of female individuals with knee osteoarthritis (OA) which have or don't have osteoporosis (OP) to understand whether there is a relationship between OP and femoral cartilage thickness and evaluate radiographic relationship between OA-OP and clinical features of OA with femoral cartilage thickness. 118 women with bilateral knee osteoarthritis are included in the study which are diagnosed according to ACR (American College of Rheumatology) criteria. 58 of them had OP according to World Health Organization's (WHO) diagnosis criteria (Group 1) while the other 60 didn't have (Group 2). Age of the patients, their menopause age, tea consumption and their OP treatment history were examined. Body mass index (BMI) was calculated for each of them as kg/m². Visual Analogue Scale (VAS), The Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Short form-36 (SF-36) were used to assess pain, functionality-disability and quality of life, respectively. Kellgren-Lawrence (K-L) classification was used to grade knee osteoarthritis radiographically. DEXA was used to measure of bone mineral density (BMD). Femoral cartilage thickness measurements were done during the ultrasonographic evaluation.After comparing the groups according to weight and BMI, patients without OP (Group 2) had the higher weight and BMI values when compared with the group of OP patients (Group 1) (p=0,000). When the groups were compared according to radiographic K-L grades for both of the knees, rate of OP was lower as the K-L grades increased. When DEXA and radiographic K-L grades were evaluated, Femoral Neck (FN) and Lumbar Total (LT) measurements were found to be higher in further grades of OA both at right and left knees. There was also a positive relationship found between radiographic K-L grade and BMI. There was no significant correlation between radiographic K-L grading and ultrasonographic femoral cartilage thickness measurements. When compared according to ultrasonographic measurements, no difference was found between two groups. When the groups are compared regarding to WOMAC and SF-36, the group with no OP had higher scores in WOMAC pain, stiffness, function sub scores and total score and, lower score in SF-36 physical function, physical role limitation, pain and social function. Femoral cartilage thickness, VAS, WOMAC and SF-36 scores showed no significant difference either. When the patients were evaluated according to their OP treatment history, patients who were having antiresorptive treatment had higher femoral cartilage thickness at right knee (p=0,036) and left knee (p=0,021) with regard to others.In conclusion, although there was a negative correlation between OA and OP in the radiographic assesment of the knee OA, no significant correlation was observed between these patients in terms of ultrasonographic femoral cartilage thickness. No relationship was found between clinical signs of OA and femoral cartilage thickness. Due to the low sensitivity on deviations and dependence on subjective interpretation of radiographic OA examination, it is considered that quantitative measurements like ultrasound will provide more reliable results. 72

Bu çalışmada Miyofasyal ağrı sendromu (MAS) tedavisindetranskutanöz elektriksel nöral stimülasyon (TENS) ve kuru iğnelemetedavilerinin etkinliğinin karşılaştırılması amaçlandı.Çalışmaya 18-60 yaş arası en az 3 aydır üst trapezius kasında MASolan 50 hasta alındı. Hastalar numaralandırılmış zarf çekim usulü ilerandomize edilerek, 25'er kişilik 2 gruba ayrıldı. 1. gruba haftada 5 gün, 2hafta konvansiyonel TENS (50-100 Hz, 100 μsn) uygulandı. 2. grubaunilateral 2 toplamda 4 tetik noktaya 1 hafta arayla 2 defa kuru iğnelemeenjeksiyonu uygulandı. Her iki gruba da ev programı olarak üst trapeziuskasına yönelik germe egzersizi eğitimi verildi. Hastalar Vizüel Analog Skala,algometri cihazı ile ağrı basınç eşiği ölçümü, palpabl kas spazm derecesi,Nothingham Sağlık Profili (NSP) ile yaşam kalitesi ve goniometre ile servikallateral fleksiyon ve rotasyon dereceleri ile değerlendirildi.Gruplar arasında Vizüel Analog Skala (VAS), algometrik ölçümler,palpabl kas spazm derecesi (PKSD), NSP alt grupları ve servikal lateralfleksiyon ve rotasyon derecelerinde istatistiksel olarak anlamlı bir farklılıkyoktu. Tedavi sonrası değerlendirmelerde 1. hafta ve 1. ay VAS skorları,algometrik ölçümlerin yüksekliği ve NSP alt gruplarından duygusalreaksiyonlarda iyileşme açılarından kuru iğneleme grubu lehine istatistikselolarak anlamlı farklılık saptandı.Sonuç olarak TENS ve kuru iğneleme tedavileri ağrının azalmasındaetkindir. This study aimed to compare effectiveness DRY needling withconventional Transcutaneous Electrical Nerve Stimulation (TENS) treatmentsof myofascial pain syndrome.This study comprised 50 patients who had at last for 3 month triggerpoint in their upper trapezius muscle with ages between 18-60 years anddiagnosed with myofascial pain syndrome (MPS). The patients wererandomized by method of choosing numbered and closed envelopes. Anddivided in two groups which consist of 25 patients. İn grup 1, the patientswere treated 5 day a week for 2 week with conventional TENS (50-100 Hz,100 μsn). İn grup 2, the patients were treated once a week for two week withdry needling to upper trazius muscle (unilateral 2 total 4 trigger point).Stretching exercises for upper trapezius muscle were taught to each groupand they were asked to exercise at home. The patients were evaluated withVisual analog scale (VAS) for pain, pain pressure threshold (PPT) measuredwith an algometer, palpable muscle spasm degree, Nothinghan Health Profilequality of life measurements (NHP) and cervical lateral flexion and rotationdegrees measured with a goniometer.There is no statistical significant difference between groups, in VAS,pain pressure threshold, palpable muscle spasm degree, all NHP subgroupsscores and cervical lateral flexion and rotation degrees at pre treatment. Posttreatment evaluations demonstrated a significant effective in dry needlinggroup that decrease in 1 week and 1 month after treatment VAS scores,increase algometric measurements and decrease emotional reaction in oneof subgroup of NHP than conventional TENS group. Both of dry neddling and conventional TENS treatments are effectiverelieving the pain. Dry needling is much more effective than conventionalTENS compare with relieving pain, increases of algometric measurementsand decreases emotional reaction score which one of subgroup NHP. 51

Bu çalışmanın amacı subakromiyal sıkışma sendromu olan hastalar ilesağlıklı kontrollerin omuz ultrasonografi bulgularının karşılaştırılması vesubakromiyal sıkışma sendromu olgularında ultrasonografi bulguları ile ağrıve fonksiyonel durum arasındaki ilişkinin incelenmesidir.Çalışmaya; anamnez ve fizik muayene ile subakromiyal sıkışmasendromu tanısı konulan 40 hasta ve omuz ağrısı şikayeti olmayan 25sağlıklı kadın gönüllü alındı. Ultrasonografi ile biseps uzun başı tendonuetrafında sıvı artışı, akromiyoklavikular eklem dejenerasyonu vesubakromiyal subdeltoid bursit olup olmadığı incelendi; akromiyohumeralmesafe ve supraspinatus kas kalınlık ölçümleri yapıldı. Subakromiyal sıkışmasendromu olgularında ağrı şiddeti Vizüel Analog Skala (VAS), omuzağrılarının günlük yaşam aktivitelerine etkisi Kol, Omuz, El Sorunları Anketi[The Disabilities of the Arm, Shoulder and Hand (DASH)] ile değerlendirildi.Subakromiyal sıkışma sendromu ve kontrol grubunun ultrasonografibulgularının karşılaştırılmasında sadece biseps tenosinoviti varlığı açısındananlamlı farklılık saptandı. SSS olgularında ultrasonografi ile elde edilenveriler ile ağrı ve fonksiyonel durum arasında ilişki bulunmadı. Ultrasonographic Findings and Their Correlation with Functional Statusin Patients with Subacromial Impingement SyndromeThe aim of the present study is to compare the ultrasonographicfindings in patients with subacromial impingement syndrome with controlhealthy patients and evaluate the relation between sonographic findings andpain-functional status.40 patiens with a diagnosis of subacromial impingement syndromebased on patient history and physical examination, and 25 control patientswith no complain of shoulder pain were included in the study.Ultrasonography was performed to evaluate the presence of biceps tendonsheath effusion, acromioclavicular joint degeneration and subacromialsubdeltoidbursitis. In addition, acromiohumeral distance and supraspinatusmuscle thickness were measured. In patients with subacromial impingementsyndrome, pain and effect of shoulder pain on daily living were evaluatedwith Visual Analog Scale (VAS) and The Disabilities of the Arm, Shoulderand Hand (DASH), respectively.Biceps tenosynovitis was the only sonographic feature with astatistically significant difference between subacromial impingementsyndrome patients and control patients. No correlation was found betweenthe sonographic findings in subacromial impingement syndrome with painand functional status. Since asymptomatic patients may have pathologicalfindings and no correlation between ultrasonographic features and patientsymptoms were found, patients symptoms rather than the sonographicfindings should be the initial target of therapy. 62

Objective: This study is based on whether the self-management program choices For Better Bone Health is effective to promote behavioral strategies for improving bone health, life quality, pain perception, physical function, and balance in osteoporotic subjects. Design: In this single-blind, randomized controlled study, a total of 50 sedentary women with postmenopausal and idiopathic osteoporosis were selected from the outpatients of AtatUrk Balneotherapy and Rehabilitation Center according to their physical activity level and T scores of dual-energy x-ray absorptiometry as the inclusion criteria. Fifty sedentary women with BM D T scores of -2.5 or lower were randomized into two groups (self-management group: group 1; and control group: group 2) and enrolled in a 6-mo study. Participants attended self-management class once a week for 5 wks. Evaluations were done at baseline, at the end of the fifth week, and at the sixth month. Pain-intensity evaluation by Visual Analogue Scale (VAS), life-quality assessments by SF-36, balance testing by Sensitized Romberg Test (SRT), and functional assessment by Timed Sit to Stand test (TSS) and a simple questionnaire were the outcome measures. Results: When the groups were compared by change scores and percentages of change, improvements observed in pain intensity by VAS (P < 0.001), SF-36 Physical Function (P < 0.001), SF-36 Physical Role Limitations (P < 0.001), SF-36 Social Function (P < 0.001), SF-36 Mental Health (P < 0.001), SF-36 Vitality (P < 0.01), SF-36 Pain (P < 0.001), SF-36 General Health Perceptions (P < 0.05), SF-36 Emotional Role Limitations (P < 0.01), SIRT eyes open (P < 0.001), SRT eyes closed (P < 0.001), and TSS (P < 0.001) were determined to be superior in group 1 at the end of the sixth month. Seventy-four percent of patients in group 1 engaged in regular physical activities, and 92% of them declared that they understood the purpose and benefits of medications and dietary calcium intake. Fifty-seven percent of them formed personal plans for preventing traumas, whereas 8% of the subjects in group 2 experienced new falls but no fractures. Conclusion: It is determined that the self-management class led to improvements in functional, balance, and life-quality outcomes and to reductions in pain perception.

Bu çalışmanın amacı, kliniğimizde yatarak tedavi gören 50 yaş üzeri kadınlarda kırık riski ve ilgili faktörlerin araştırılmasıdır. Çalışmaya 1 Ocak 2013-31 Aralık 2013 tarihleri arasında yatarak tedavi gören 50 yaş üstü 341 kadın hasta dahil edilmiştir. Hastalar Türkiye kırık Riski Değerlendirme Anketi (FRAX) ile ve günlük çay tüketimi, kapalı giyim, bifosfonat kullanımı açısından sorgulanmış, hastalara 3 metre kalk yürü testi uygulanmıştır. Dual energy X-ray Absorptiometry ile ölçülmüş kemik mineral yoğunluğu (KMY) olan ve olmayan tüm hastaların major osteoporotik kırık (OK) ve kalça kırığı riski hesaplanmıştır. KMY ölçümü olan (n=85) ve olmayanlarda (n=256) 10 yıllık OK ve kalça kırık riskleri sırasıyla %1,17, %16,4 ve %2,3, %16 idi. Sonuç olarak, çalışmamızda KMY'si olan ve olmayan 50 yaş üzeri kadınlarda kırık riski tahmini benzer olup, KMY’si olmayanlarda fiziksel aktivite düzeyi (p=0.009, r=0.16) ve çay tüketiminden (OK p=0.030, kalça kırığı p=0.004) etkilenmektedir The aim of the study is to investigate the fracture risk and related factors of 341 female in patients over 50 years of age between 01.01.2013 and 31.12.2013. Patient information on tea, bisphosphonate intake and conservative clothing were obtained and patients were subjected to 3 meters walking test. Turkey Fracture Risk Assessment Tool (FRAX) was used to calculate the 10 years major osteoporotic fracture (OF) and hip fracture risks of patients with and without BMD by Dual Energy X-Ray Absorptiometry. OF and hip fracture risks of patients with BMD (n=85) and without BMD (n=256) were 1,17%, 16,4 % and 2,3%, 16% respectively. In conclusion, fracture risk assessment in female patients with or without BMD were similar and fracture risk assesment without BMD had significant relationship between risk factors such as; physical activity (p=0.009, r=0.16) and tea intake (OF p=0.030, hip fracture p=0.004)

Bu çalışmada Karpal Tünel Sendromu (KTS) tedavisinde karpal kemik mobilizasyonu uygulamasının etkinliğinin değerlendirilmesi amaçlanmıştır.Prospektif randomize kontrollü çalışmaya hafif-orta şiddetli KTS tanısı alan 40 hasta alındı. 1. Gruba (n=20) haftada 3 kez toplam 10 seans karpal kemik mobilizasyonu uygulandı ve hastalar 3 hafta boyunca geceleri nötral volar el bileği splinti kullandı. 2. Grup (n=20) 3 hafta boyunca geceleri nötral volar el bileği splinti kullandı. Tüm gruplar başlangıçta demografik veriler, klinik ve elektrofizyolojik parametrelerle değerlendirildi. Klinik parametreler Tinnel testi, Phalen testi, vizüel analog skala (VAS) ile gece ve gündüz ağrısı, el kavrama gücü, başparmak gücü, Boston fonksiyonel durum ölçeği ve ortalaması, Boston semptom şiddeti skoru ve ortalamasıydı. Elektrofizyolojik parametreler duyusal ileti hızı, duyusal latans, duyusal amplitüd, motor ileti hızı, motor latans, motor amplitüddü. Klinik parametreler 3. haftada ve hem klinik ve hem de elektrofizyolojik parametreler 3. ayda değerlendirildi.Mobilizasyon+splint grubunda tedavi sonrası (3. haftada) el kavrama gücü, Boston semptom şiddeti ve fonksiyonel durum ölçekleri, Tinnel testi ve gece ağrısında; 3. ayda tüm klinik parametrelerde ve duyusal amplitüd, duyusal latans değerlerinde anlamlı düzelme oldu. Splint uygulanan grupta tedavi sonrası (3. haftada) ve 3. ay kontrolde Boston semptom şiddeti ölçeği, Tinnel testi, Phalen testi, gece ve gündüz VAS değerlerinde anlamlı düzelme oldu, ancak elektrofizyolojik parametrelerde değişiklik olmadı. 3. hafta gece VAS değeri ile 3. ay başparmak gücü ve Boston fonksiyonel durum ölçeğinde gözlenen düzelme Grup 1'de Grup 2'ye göre üstün bulundu.Sonuçta splintle kombine edildiğinde karpal kemik mobilizasyonu KTS'de invaziv olmayan, etkili bir tedavi seçeneğidir. The aim of this study was to investigate the effectiveness of carpal bone mobilisation in the treatment of Carpal Tunnel Syndrome (CTS).A total of 40 patients with mild to moderate CTS were included in this prospective randomised controlled trial. In Group 1 (n=20), patients received carpal bone mobilisation 3 times a week, a total of 10 times, and used neutral volar wrist splint at night for 3 weeks. Patients in Group 2 (n=20) only used neutral volar wrist splint at night for 3 weeks. All patients were assessed at baseline with demographic parameters, clinical and electrophysiologic measurements. Clinical parameters were Tinnel and Phalen tests, day and night pain by visual analogue scale (VAS), grip and pinch strength, Boston Carpal Tunnel Questionnarie (BTCQ) for functional status and symptom severity. Electrophysiologic measurements were median nevre sensory and motor conduction velocities, distal sensory latencies, motor latencies, sensory and motor amplitude. Clinical measurements were done at the 3rd week and clinical and electrophysiological measurements both were done at the 3rd month.In mobilisation and splint group, grip strength, BTCQ symptom severity and functional status, Tinnel tests and night pain at the 3rd week, and clinical parameters, sensory amplitude and latencies at the 3rd month showed significant improvement. In splint group BTCQ symptom severity, Tinnel tests, Phalen tests, night and day pain improved at the 3rd week and at the 3rd month, but no improvement was observed in electrophysiological parameters. İmprovement observed in night pain at the 3rd week and pinch strength, BTCQ functional status at the 3rd month were superior in Group 1 compared to Group 2.In conclusion, when combined with splinting, carpal bone mobilisation is a non-invasive, effective alternative treatment modality in CTS. 61

Çalışmanın amacı, balneoterapinin klinik etkinliğini araştırmaktır. 48 Ankilozan Spondilitli (AS) hasta, Modifiye New York tanı kriterlerine göre belirlendi ve egzersizle kombine Balneoterapi (Grup I) ve ısıtılmış çeşme suyu tedavisi (Grup II) olmak üzere (her biri n=24) 2 gruba randomize edilmiştir. Değerlendirme parametreleri; Notthingham Sağlık Profili (NSP), Bath AS Fonksiyonel İndeks (BASFI), Bath AS Hastalık Aktivitesi İndeksi (BASDAI), Dougados Fonksiyonel İndeks (DFI), Hastanın genel değerlendirmesi (HGD), eritrosit sedimentasyon hızı, sabah tutukluğu, modifiye Schober testi (Sch), parmak ucu fibula mesafesi (PFM), göğüs ekspansiyon (GE), çene-manubrium mesafesi ve oksiput-duvar mesafesidir. Değerlendirmeler tedavi öncesi ve tedavi sonrası 3.ay olmak üzere 2 kez çift kör ve ITT prensibine göre yapılmıştır. 3.ayda BASDAI’de (p=0,002), BASFI’de (p=0,04); DFI’de (p=0,03); NSP Ağrıda (p=0,01); HGD’de (p=0,02); GE’de (p=0,00); Sch’de (p=0,04) ve PFM’de (p=0,03) Grup I lehine anlamlı düzelmeler gözlendi. BT’nin kısa süreli izlemde ağrı, hastalık aktivitesi, fonksiyonel kapasite, spinal flexibilite ve hastanın genel değerlendirmesinde etkili olduğu gösterilmiştir. The objective is to evaluate clinical efficacy of balneotherapy. 48 Ankylosing Spondylitis (AS) patients, selected according to the Modified New York diagnostic criteria, were randomized into 2 treatment groups; (in every group n=24) Balneotherapy (Group I) and Heated Tap Water Therapy (Group II) combined with exercises. Patients in Group I (N = 24) were treated with hot spring water (38ºC). The outcome measures were; Nottingham Health Profile (NHP), Bath AS Functional Index (BASFI), Bath AS Disease Activity Index (BASDAI), Dougados Functional Index (DFI), global well-being of the patient (GWB), erytrocyte sedimentation rate, morning stiffness, Modified Schober Test (Sch), fingertip-fibula head distance (FFD), chest expansion (CE), chin-manubrium distance and occiput-wall distance. The evaluations were done before treatment end at the 3rd month by double blinding procedure and ITT principle. Improvements were observed in BASDAI (p = 0.002); BASFI (p = 0.04); DFI (p = 0.03); NHP Pain (p= 0.01); GWB (p = 0.02); CE (p = 0.000); Sch (p = 0.04); and FFD (p = 0.03) were determined to be superior in Group I at the end of the 3rd month. The data presented reveals the short-term effectiveness of BT on pain intensity, disease activity, functional capacity, spinal motion and global well being of the AS patients when compared with heated tap water therapy.

Amacımız 3 aydan uzun idiyopatik Karpal Tünel Sendromu (KTS) ile ilgili şikayetleri olan bayanlarda, girişimsel olmayan, etkili bir yöntem olduğunu düşündüğümüz kortikosteroid fonoforezin etkinliğini araştırmaktı.Çalışma Uludağ Üniversitesi Atatürk Rehabilitasyon Uygulama Araştırma Merkezinde Nisan 2008 ? Şubat 2009 arasında idiyopatik KTS tanısı konmuş, yaşları 32?71 arasında değişen (52.1 ? 1.0), 60 bayan hastanın 102 eliyle yapıldı. KTS tanısı öykü, Tinel ve Phalen testleri ve sinir ileti çalışmaları ile konuldu. Çalışma randomize, kontrollü, tek kör olarak biçimlendirildi. 1. gruba ultrason (US) jeli ile 1 MHz, 1 w/cm2 sürekli akım terapötik US, 2. gruba %0.025'lik beklometazon dipropionat ile 1 MHz, 1 w/cm2 sürekli akım fonoforez, 3. gruba 0 MHz, 0 w/cm2 plasebo US her seansta 5 dakika, toplam 10 seans uygulandı. Gruplar başlangıçta demografik veriler, klinik parametreler ve sinir ileti parametreleriyle, 10 seans tedavi sonrası klinik ve tedaviden 8 hafta sonra klinik ile birlikte sinir ileti parametreleriyle değerlendirildi.Fonoforez grubunda tedavi öncesine göre her iki el Tinel ve sol el Phalen testinde, fonksiyonel durum ölçeğinde, sol el motor ileti amplitüd hariç tüm motor ileti parametrelerinde ve tüm duyusal ileti parametrelerinde anlamlı iyileşme bulundu. US grubunda ise sağ ve sol motor ileti hızı, sağ motor amplitüd, sağ ve sol duyusal ileti amplitüd ve latanslarında anlamlı iyileşme bulundu. Plasebo US grubunda anlamlı değişiklik bulunmadı. Fonoforez motor ve duyusal ileti parametreleri açısından US ve plaseboya üstün bulunduSonuç olarak, KTS tedavisinde US ve fonoforez etkili bulunurken, fonoforezin hem klinik parametrelerde hem de sinir ileti çalışmalarında iyileşme açısından US ve plaseboya üstün olduğu gözlendi. Our aim was to study the efficacy of corticosteroid phonophoresis which is an uninvasive and effective method on women who have idiopatic Carpal Tunnel Syndrome (CTS) for at least three months.The study was conducted at Uludağ University Atatürk Rehabilitation and Resarch Center between April 2008 ? February 2009 on 60 women? 102 hands, between ages 32?71 (52.1 ? 1.0). CTS diagnosis was made by patient history, Tinel and Phalen tests and nerve conduction studies. Study was shaped as randomised, controlled and single blinded. The 1st group was treated with 1 MHz, 1 w/cm2 continuous therapotic ultrasound (US) with US gel, the 2nd group was treated with 1 MHz, 1 w/cm2 continuous phonophoresis with 0,025% beclomethasone dipropionate, the 3rd group was treated with 0 MHz, 0 w/cm2 placebo US, all for 5 minutes, totally 10 sessions applied. The groups were assessed with demographic, clinical parameters and nerve conduction studies at the beginning, at the end of 10 sessions and after 8 weeks.In Phonophoresis group, left hand Phalen, both hand Tinel test, functional status, all motor parameters except left hand motor conduction amplitude and all sensory conduction paremeters showed significant improvement. In US group right and left hands motor conduction velocities, right hand motor amplitude, right and left hands sensory conduction amplitudes and latency prameters significantly improved. Phonophoresis was superior to US and plasebo groups for the right and left hand motor and sensory conduction parameters. No changes were observed in placebo group.In conclusion we found that in CTS, phonophoresis and US are effective methods while phonohoresis was more effective at clinical and nerve conduction study parameters. 59

Bu çalışma poster formatında 25-29 Nisan 2007 5.Osteoporoz-Osteoartrit Antalya kongresinde sunulmuş ve Prof. Dr. Mülazım Yıldırım en iyi araştırma ödülü almıştır. Çalışmanın amacı osteoporoz hastalarında demografik özellikler, vücut kitle indeksi (VKI), kalsitriol, parathormon (PTH), alkalen fosfataz (ALF) düzeyleri ile dual-enerji-X ray absorpsiyometri (DXA) değerleri ve kırık oluşumu arasındaki ilişkiyi araştırmaktır. Osteoporoz tanı kriterlerine uyan 310 hasta değerlendirildi. Hastaların serum kalsiyum, kalsitriol, PTH, ALF düzeyleri ve VKİ'leri tespit edildi. DXA lomber total (LT), femur boyun (FB) ve femur total (FT) değerleri ölçüldü. Torakal, lomber yan, pelvis grafileri çekildi. Kadınlarda FT değerleri ve VKI, LT ile FB ve FT; FB ile FT arasında pozitif korelasyon; PTH ile kalsitriol arasında negatif korelasyon saptandı. Erkeklerde LT ile PTH arasında pozitif korelasyon saptandı. 113 (%36.5) hastada kırık saptandı. Bu hastalarda PTH düzeyi kırığı olmayanlara göre daha yüksekti. Kırığı olan kadın hastaların VKİ değerlerinin daha yüksek olduğu görüldü. VKI ile kırık ve erkeklerde LT DXA ile PTH düzeyleri arasında pozitif korelasyon saptanmış olması dışında verilerimiz literatürle uyumludur. Literatürle çelişkili sonuçlar hasta sayımızın azlığıyla ilişkili olabilir. The aim of this study is to assess the relations between demographic properties, body mass indexes (BMI); calcitriol, parathormone (PTH), alkaline phosphatase (ALP) levels, bone fractures and dual-energy-X ray absorptiometry (DXA) T scores of osteoporotic patients. 310 patients with osteoporosis were assessed. Serum calcium, calcitriol, PTH, ALP levels and BMI of patients were determined. DXA lumbar total (LT), femur neck (FN), femur total (FT) values were measured. Thoracal, lumbar lateral, pelvic radiographs were obtained. In women, FT values were positive correlating with BMI, FN and FT were positively correlated with LT; FN and FT were correlated positively with each other. There was a negative correlation between PTH and calcitriol levels. In men, LT and PTH were positively correlated. 113 patients had bone fractures. These patients’ PTH levels were higher than patients who were not having fractures. Women with bone fractures had higher BMI values. Except BMI and fractures; in men, LT and PTH were positively correlated. Our results were similar with literature. Exceptional results may be associated with our small patient population.

Aims To study the clinical hand findings in Behcet's disease (BD) and to observe scintigraphic changes of these areas. Methods Fifty-seven randomly selected BD patients and the patients in the control group ( N = 40) were evaluated by two blind rheumatologists. The hands were examined for the presence of pain, tenderness, swelling, effusion, erythema, warmth, range of motion and limitation of motion, deformities and muscle atrophy. Then scintigraphic examination of the hands was performed. Control hand scintigrams were obtained from 40 age- and sex-matched patients and were examined by the same two observers. Results Thirty-two of the 57 patients (56.1%) showed Behcet's clinical hand findings. Terminal phalangeal pulp atrophy was observed in 17 (29.81%), rheumatoid-like hand findings were observed in 16 (28.1%), dorsal interosseos atrophy was observed in 12 (20.05%) and erythema over the digits was observed in 12 (20.05%). Twenty-four patients (42.1%) had scintigraphic involvement. The disease duration was observed to be an important factor for hand findings ( P = 0.040) and scintigraphic involvement ( P = 0.011). Conclusion High prevalence of hand involvement in BD and its relationship with disease duration is demonstrated. Hand involvement tends to be overlooked and careful examination is required in the evaluation of BD. The scintigraphic involvement detected in hands requires special consideration, too.

To investigate if spa water is superior to tap water (TW) in relieving the symptoms of pain, joint motion, life quality in knee osteoarthritis (KOA) patients. In this randomized placebo-controlled trial, 52 patients with KOA were followed in two groups. In group I (n = 27), patients were treated in the pool full of spa water at 37 degrees C for 20 min a day, 5 days a week, for a period of 2 weeks. In group II (n = 25), the same protocol was used but spa water was replaced by TW heated to 37 degrees C. Patients in both groups were given a home-based standardized exercise program. Evaluation parameters were pain (pVAS), tenderness score (TS), 50-ft walking duration, quadriceps muscle strength (QMS), active flexion degree (AFD), WOMAC OA index, and Nottingham Health Profile (NHP). The first evaluation was done after the informed consent was obtained. Second and third evaluations were done at the 2nd and 12th week. PVAS, 50-ft walking duration, AFD, TS, WOMAC, and NHP variables improved in group I. Same variables except QMS improved also in group II. Comparison of the groups just after treatment showed that only pVAS (P = 0.015), NHP pain score (P = 0.020), and TS (P = 0.002) differed significantly in favor of group I at the 2nd or 12th week. Both of the thermal treatment modalities were found to be effective in the management of the clinical symptoms and quality of life in KOA patients. However, pain and tenderness improved statistically better with balneotherapy. There were no significant differences between the groups for the other variables.

Summary Aim To evaluate the effects of a yoga-based exercise program on pain, fatigue, sleep disturbance, and biochemical markers in hemodialysis patients. Materials and methods In 2004 a randomized controlled trial was carried out in the outpatient hemodialysis unit of the Nephrology Department, Uludag University Faculty of Medicine. Clinically stable hemodialysis patients ( n = 37) were included and followed in two groups: the modified yoga-based exercise group ( n = 19) and the control group ( n = 18). Yoga-based exercises were done in groups for 30 min/day twice a week for 3 months. All of the patients in the yoga and control groups were given an active range of motion exercises to do for 10 min at home. The main outcome measures were pain intensity (measured by the visual analogue scale, VAS), fatigue (VAS), sleep disturbance (VAS), and grip strength (mmHg); biochemical variables – urea, creatinine, calcium, alkaline phosphatase, phosphorus, cholesterol, HDL-cholesterol, triglyceride, erythrocyte, hematocrit – were evaluated. Results After a 12-week intervention, significant improvements were seen in the variables: pain −37%, fatigue −55%, sleep disturbance −25%, grip strength +15%, urea −29%, creatinine −14%, alkaline phosphatase −15%, cholesterol −15%, erythrocyte +11%, and hematocrit count +13%; no side-effects were seen. Improvement of the variables in the yoga-based exercise program was found to be superior to that in the control group for all the variables except calcium, phosphorus, HDL-cholesterol and triglyceride levels. Conclusion A simplified yoga-based rehabilitation program is a complementary, safe and effective clinical treatment modality in patients with end-stage renal disease.
