5. How do expatriates and local professionals work in auditing firms? : a Weberian analysis of perseverance and adjustment in the Saudi Arabian workplace

Abstract

This study examines expatriates and local professionals work in auditing firms in a traditional society (Saudi Arabia). Previous studies have concentrated on the functionalist perspective in professional development such as how auditors become professionals whilst ignoring other factors such as cultural and societal influences and the interaction of professionals within workplaces. In addition, the studies overlooked the micro-level in auditing sector in developing countries. These gaps motivated the exploration of professionalism in auditing firms in Saudi Arabia, drawing upon the Weberian framework of accounting to analyse multiple levels of the auditing sector. In addition, the Theory of Work Adjustment (TWA) is employed to analyse professionals' perseverance in adjusting to their work environments at the firm-level. A qualitative research design is adopted with data collected through semi-structured interviews with 37 participants from auditing firms, the Ministry of Human Resource and Saudi Organization for Certified Public Accountants (SOCPA). The data are analysed using thematic analysis. Although expatriate professionals are sought by auditing firms for their expertise and exercise of audit judgments, their continued role and influence is heavily mediated by their hierarchical relationship with the audit firm owners/managers. They are compelled to obey to the ultimate ends of the owners/managers of auditing firms. This role is based on a substantive rational requirement rather than formal rational requirements, thereby enhancing the dominance of owners/managers on labour market. Micro-level analysis confirms that expatriates were more persevered to adjust to work environment requirements than local professionals. Consequently, the localisation agenda implementation has been delayed. The current study contributes to the literature of auditing profession development in developing countries as well as job localisation in the auditing sector. Tension analysis of macro- and micro-levels with professionals' perseverance analysis provides further knowledge of the Weberian framework with TWA about rationalities, domination, resistance and perseverance.

11. Inpatient Satisfaction on Non-Pharmacological Interventions for Acute Settings: A Systematic Review.

Abstract

This review aimed at systematically mapping and summarizing the evidence on non-pharmacological interventions that targeted patient satisfaction in inpatient acute settings. Methods: Three electronic databases were searched, including PubMed, EBSCO, and ScienceDirect. The inclusion criteria were: (1) studies of non-pharmacological interventions to improve patients' satisfaction and targeting inpatients between the ages of 19 and 65 years old; (2) studies written in English and published in the last 10 years, starting from 2017. The search results were imported and screened for eligibility on Covidence. The data was then extracted, using a tool entered in Covidence's Extraction 2.0. The extraction tool included domains on both intervention impact and delivery processes. Results: A total of 11 articles met the inclusion criteria. Randomized control trials represented the most among the group; seven studies were included given that the others were quasi-experimental studies. Those studies were conducted on the different types of services offered in acute care departments. These studies did not use a standardized questionnaire to evaluate their respective trial outcomes or to implement various adapted or adopted modules of intervention. Of note, the intervention was effective in enhancing patient satisfaction in only some of the studies. Conclusion: Different types of intervention modules have been effective in improving acute care patient satisfaction. However, further studies are needed to evaluate the effectiveness of an intervention among all patients in different acute care departments at the same time. [ABSTRACT FROM AUTHOR]

24. Comparison of Salivary IGF-1, IGFBP-3, and CTX with Periodontal Status among Patients Belonging to Various Skeletal Maturity Groups.

Abstract

Purpose: To compare the levels of salivary IGF-1, IGFBP-3, and CTX with periodontal status among patients belonging to various skeletal maturity groups. Materials and Methods: This cross-sectional study was conducted on 80 participants 6 to 25 years of age. Based on skeletal maturity, the participants were categorised into 3 different stages: prepubertal, pubertal, and post-pubertal stages. The periodontal status of the participants was assessed using the simplified oral hygiene index (OHI-S), bleeding on probing (BOP), probing pocket depth (PPD), clinical attachment loss (CAL), and community periodontal index (CPI). The saliva samples were examined for IGF-1, IGFBP-3, and CTX using the respective ELISA kits. One-way ANOVA was used to determine statistically significant differences of means across the study groups for continuous variables. Results: The study demonstrated statistically significant differences for the parameters OHI-S, bleeding on probing, PPD, CPI, and CAL (p < 0.05) depending on skeletal maturity stage. ANOVA test showed a statistically significant difference by stage in IGF-1, IGFPB3, and CTX (p < 0.01). Conclusion: An association exists between periodontal status and levels of salivary IGF-1, IGFBP-3, and CTX in patients belonging to various skeletal maturity groups. [ABSTRACT FROM AUTHOR]

Abstract

This article from the Arab Journal of Interventional Radiology explores the use of preoperative botulinum toxin injections as a treatment option for complex abdominal wall hernias. The study involved 8 patients with an average age of 53.38 years. The results showed a statistically significant decrease in the size of the hernia defect and loss of domain after the Botox injection. Primary fascial closure was achieved in 87.5% of the individuals, and there were no significant side effects. Abstract

Background: Transcobalamin II (TCN2) defect is a rare metabolic disorder associated with a range of neurological manifestations, including mild developmental delay, severe intellectual disability, ataxia, and, in some cases, seizures. Cobalamin, an essential nutrient, plays a crucial role in central nervous system myelination. Clinical Presentation: We present a family with an index patient who exhibited progressive neurodevelopmental regression starting at 9 months of age, accompanied by myoclonic seizures, ataxia, and tremor. No significant hematological abnormalities were observed. Exome sequencing analysis identified a novel homozygous mutation, c.3G>A – P(Met1I), affecting the acceptor site of intron 4 of the TCN2 gene (chromosome 22: 31003321, NM_000355.4), leading to likely pathogenic variant potentially affecting translation. Following treatment with hydroxocobalamin, the patient demonstrated partial clinical improvement. He has a sibling with overt hematological abnormalities and subtle neurological abnormalities who is homozygous to the same mutation. Both parents are heterozygous for the same mutation. Conclusions: In infants presenting with unexplained non‐specific neurological symptoms, irrespective of classical signs of vitamin B12 deficiency, evaluation for TCN2 defect should be considered. Abstract

Brucellosis is an endemic zoonotic disease caused by intracellular gram-negative coccobacilli called Brucella. This infectious disease may implicate many farm animals and is transmissible to exposed humans. Brucellosis is potentially life-threatening and may lead to granulomatous multi-organ involvement with tendency to chronicity and recurrence. The treatment of brucellosis requires combined and protracted antimicrobial therapies to eliminate the disease and to avoid its relapse. Genitourinary brucellosis is common among infected humans in endemic areas and is considered the second-most affected focal site, which commonly manifests as epididymo-orchitis. Testicular abscess, however, is an extremely rare complication of brucellosis. To the best of our knowledge, in the literature, there are to date only 16 previously published case reports, including 22 patients of brucellar testicular abscesses, emphasizing the extreme rarity of this condition. Most of these cases harbored small abscesses, which were treated conservatively using antibiotics therapy only, or with added drainage of the abscesses. Larger abscesses were reported to necessitate orchiectomy. In some cases, the abscesses were mimicking tumors, and surgeries for orchiectomy were done accordingly. A summary of the previously reported cases in the literature is presented. Here, we present the 17th case report of a 34-year-old man with a right-side huge multilocular brucellar testicular abscess apparently replacing the entire testicle, who was successfully treated with organ-sparing management by incision-drainage of the large abscess with antibiotics, to eventually preserve his testis. In conclusion, brucellosis should be considered among the differential diagnoses of any testicular swelling, especially in endemic areas. Drainage of brucellar testicular abscess with appropriate medical treatment is feasible and may preserve the testicle, even with large abscess apparently replacing the entire testicle. Abstract

Aim: Temporary anchorage devices (TADs) have become the routine treatment tools for orthodontic treatment. The aim of this study was to assess the knowledge about TADs among the undergraduate students of Saudi Arabia. Materials and Methods: A cross- sectional web-based questionnaire was sent to 301 undergraduate dental students across different dental colleges in Saudi Arabia from third year to internship. This prevalidated 21-item questionnaire was sent through Google Form to different dental student groups. The questionnaire included details related to demography, knowledge related to TADs, and the role of continuing dental education in building the knowledge related to TADs. Descriptive statistical analysis and the chi-square test were used to evaluate the difference in the response to the questions among the different years of undergraduate students. Results: Approximately 88% of the interns could recognize that TADs are the Implants for skeletal anchorage and 57.8% defined TADs as “Implants for skeletal anchoring. Approximately 51.8% of the respondents felt that their knowledge was obtained from their undergraduate orthodontic studies. Sixty- one percent of the respondents stated their inability to diagnose cases that would benefit from orthodontic TADs. Most of the dental students from different years preferred the introduction of the TADs in the fifth-year curriculum. There existed a significant difference between the responses given by different-year dental students (P < 0.05). Conclusion: Internship students had comparatively better knowledge regarding TADs than the undergraduate students. This warrants the need to incorporate the new techniques and treatment modalities in the undergraduate orthodontic curriculum. [ABSTRACT FROM AUTHOR]
