Abstract

Transcutaneous electrical nerve stimulation (TENS) has proven effective in treating pain in many experimental and clinical studies. In addition to the analgesic effect, direct TENS of peripheral nerves had anti-inflammatory and regenerative effects in the treatment of distal polyneuropathy and spinal cord injury. This work demonstrates the experience of using direct TENS in the treatment of a 52-year-old patient with post-COVID-19 Guillain–Barré (GBS) and acute transverse myelitis (ATM) overlap syndrome. Direct TENS of peripheral nerves showed high efficiency in enhancing the therapeutic effect of combined plasma exchange and pharmacotherapy by 89.5% with a significant reduction in neuropathic pain, motor and sensory deficits, bladder and bowel disorders and regression of neurophysiological changes. We suggest that direct TENS of peripheral nerves can be a promising option for combined therapy of GBS and ATM overlap syndrome and other diseases with the simultaneous development of distal polyneuropathy and spinal cord injury. Further trial studies are required.

Abstract

Background. Rising prevalence of allergic diseases is a major medical and social problem.Objective. The aim of the study is to study the prevalence of allergy symptoms in schoolchildren of Chechen Republic according to the ISAAC (International Study of Asthma and Allergy in Childhood) questionnaire.Methods. We have surveyed 3,398 schoolchildren from 26 randomly chosen schools located in 2 urban and 2 rural areas of Chechen Republic. The first age group (7–8 years old) included 1331 first graders, the second group (13-14 years old) — 2067 eighth graders.Results. The prevalence of allergic diseases symptoms according to the questionnaire was the following: bronchial asthma (BA) — 18.4% (625 children), allergic rhinitis (AR) — 16.6% (565 children), atopic dermatitis (AD) — 4.7% (160 children), while medically verified diagnosis was established in dramatically less cases. Analysis of BA, AR, AD symptoms prevalence has revealed significant correlation between studied parameters and age. Symptoms of BA, AR prevailed in the older age group, AD in the younger one. There were no gender differences identified. Higher prevalence of BA and AR symptoms was recorded in urban residents, while the prevalence of AD in urban and rural areas was quite the same. Moreover, estimation of allergy symptoms prevalence revealed mild course of these diseases in schoolchildren of Chechen Republic.Conclusion. The study, conducted with ISAAC questionnaire, demonstrates high prevalence of allergy symptoms in schoolchildren of Chechen Republic, exceeding the official statistics. It requires particular attention from the healthcare system and improved diagnostic methods for allergies.

Abstract Background The aim of this study was to ascertain the prevalence of burnout in Kazakhstan firefighters with regard to position and to identify predictors of faster burnout in order to plan future preventive strategies. Methods Data on demographics, lifestyle, fatigue (Fatigue Severity Scale (FSS)), SF-8 health-related quality of life (HRQL) and Maslach Burnout Inventory (MBI) emotional exhaustion (EX), cynicism (CY) and professional efficacy (PE) were obtained from 604 (94% men, median age 27 (interquartile range (IQR) 12) years) firefighters from all 18 fire departments of the city of Almaty. Associations between predictors and burnout EX, CY and PE dimensions were tested using multivariate logistic regression analyses. Results Burnout scores were low in this sample, including EX (0.6; IQR 1.55), CY (1.2; IQR 1.8) and PE (4.8; IQR 2.4). Abstract

BackgroundHidradenitis suppurativa (HS) is associated with comorbidities that contribute to poor health, impaired life quality, and mortality risk.ObjectiveTo provide evidence-based screening recommendations for comorbidities linked to HS.MethodsSystematic reviews were performed to summarize evidence on the prevalence and incidence of 30 comorbidities in patients with HS relative to the general population. The screening recommendation for each comorbidity was informed by the consistency and quality of existing studies, disease prevalence, and magnitude of association, as well as benefits, harms, and feasibility of screening. The level of evidence and strength of corresponding screening recommendation were graded by using the Strength of Recommendation Taxonomy (SORT) criteria.ResultsScreening is recommended for the following comorbidities: acne, dissecting cellulitis of the scalp, pilonidal disease, pyoderma gangrenosum, depression, generalized anxiety disorder, suicide, smoking, substance use disorder, polycystic ovary syndrome, obesity, dyslipidemia, diabetes mellitus, metabolic syndrome, hypertension, cardiovascular disease, inflammatory bowel disease, spondyloarthritis, and sexual dysfunction. It is also recommended to screen patients with Down syndrome for HS. The decision to screen for specific comorbidities may vary with patient risk factors. The role of the dermatologist in screening varies according to comorbidity.LimitationsScreening recommendations represent one component of a comprehensive care strategy.ConclusionsDermatologists should support screening efforts to identify comorbid conditions in HS.

Abstract

This study aims to understand subaltern cosmopolitanism and governance dynamics in low-income neighbourhoods in Colombo, Sri Lanka. Subaltern cosmopolitanism is a relatively new underrepresentation of cosmopolitanism that draws attention to the life of various marginal groups in society. Low-income neighbourhoods in Colombo (known locally as watta) are key areas that bring together diverse ethnic, religious, and linguistic groups, especially when contrasted to the main societies in Sri Lanka. Despite the fact that economically and socially disadvantaged group of residents illustrates fundamental characteristics of cosmopolitanism resulting from daily interactions with difference, these elements have not been adequately investigated within the cosmopolitanism framework. Ethnography was the key approach used in this study. Participant observation, in-depth interviews and archival records were used to investigate and understand the present and past life of watta residents. To analyse the collected data, narrative analysis and thick description, were used. This study found that the watta culture is distinct in that it allows different groups to engage in everyday life. Religion is an essential element of watta residents through which they have interacted with different groups of people for a long time. However, a small group is involved in governance of the watta. This requires outsiders' involvement and brings in party politics, welfare assistance, and development projects which have exacerbated ethnic and religious differences among the residents of the watta. Hence, now the watta is changing like the majority society, the division in ethnicity and religion arises in the community. This thesis concludes that subaltern cosmopolitanism exists in the low-income neighbourhoods in Colombo, but it has had a significant impact due to the influence of internal and external stakeholders.

Abstract

Hereditary Angioedema (HAE) is a rare and disabling disease for which early diagnosis and effective therapy are critical. This revision and update of the global WAO/EAACI guideline on the diagnosis and management of HAE provides up-to-date guidance for the management of HAE. For this update and revision of the guideline, an international panel of experts reviewed the existing evidence, developed 28 recommendations, and established consensus by an online DELPHI process. The goal of these recommendations and guideline is to help physicians and their patients in making rational decisions in the management of HAE with deficient C1-inhibitor (type 1) and HAE with dysfunctional C1-inhibitor (type 2), by providing guidance on common and important clinical issues, such as: 1) How should HAE be diagnosed? 2) When should HAE patients receive prophylactic on top of on-demand treatment and what treatments should be used? 3) What are the goals of treatment? 4) Should HAE management be different for special HAE patient groups such as children or pregnant/breast feeding women? 5) How should HAE patients monitor their disease activity, impact, and control? It is also the intention of this guideline to help establish global standards for the management of HAE and to encourage and facilitate the use of recommended diagnostics and therapies for all patients.

Abstract

We identify amino acid variants within dominant SARS-CoV-2 T cell epitopes by interrogating global sequence data. Several variants within nucleocapsid and ORF3a epitopes have arisen independently in multiple lineages and result in loss of recognition by epitope-specific T cells assessed by IFN-γ and cytotoxic killing assays. Complete loss of T cell responsiveness was seen due to Q213K in the A∗01:01-restricted CD8+ ORF3a epitope FTSDYYQLY207-215; due to P13L, P13S, and P13T in the B∗27:05-restricted CD8+ nucleocapsid epitope QRNAPRITF9-17; and due to T362I and P365S in the A∗03:01/A∗11:01-restricted CD8+ nucleocapsid epitope KTFPPTEPK361-369. CD8+ T cell lines unable to recognize variant epitopes have diverse T cell receptor repertoires. These data demonstrate the potential for T cell evasion and highlight the need for ongoing surveillance for variants capable of escaping T cell as well as humoral immunity.

25. A critical evaluation on the innovative performance of smartphone industry : a case study on Micromax Informatics in India

There is evidence which suggest that smartphone industry is an active platform experiencing steady growth and demand. The industry is contented with tough competition from different Smartphone companies and thus innovations are considered as buzz word in the industry. Smartphone manufacturers are very keen on assessing the upcoming trends in the market and thereby establishing the updates in the technology in to their existing and new products. The main purpose of the current research study was therefore to evaluate the innovation performances of Indian Smartphone sector using the case of Micromax Informatics as an example. This research sought to understand the impact of innovation performance of Micromax from the perspective of the various stakeholders. This included surveys and interviews with the aim of gaining first hand in-depth information about the Smartphone product. This research led DBA project also sought to identify the challenges, constraints and lessons learned on innovation performance from the experiences of diverse stakeholders in India smartphone industry’s in the context of developing nations. The sample size considered for the interview and survey was 10 managers and 100 customers respectively. By conducting survey with the customers, the researcher gathered a wealth of evidence about the performances of Micromax products. The user’s opinion about the Micromax smartphones was identified by conducting survey with the customers and further interview were held with managers regarding the performances of their product in line with the trends in the industry. The results suggest that, despite the interest and enthusiasm of Micromax for introducing Smartphones, the views of different authors are much more mixed regarding the performance of Micromax which is further exacerbated by the challenges coming from the competitors. Furthermore, there was evidence of the lack of innovative ideas and products of Micromax including not introducing the 4G when in line with the competitors. The main challenges observed when introducing Smartphones were a lack of detailed analysis of competitors and the rival.

Abstract

Hundreds of millions of network cameras have been installed throughout the world. Each is capable of providing a vast amount of real-time data. Analyzing the massive data generated by these cameras requires significant computational resources and the demands may vary over time. Cloud computing shows the most promise to provide the needed resources on demand. In this article, we investigate how to allocate cloud resources when analyzing real-time data streams from network cameras. A resource manager considers many factors that affect its decisions, including the types of analysis, the number of data streams, and the locations of the cameras. The manager then selects the most cost-efficient types of cloud instances (e.g. CPU vs. GPGPU) to meet the computational demands for analyzing streams. 34. A case of new-onset vitiligo in a patient on tofacitinib and brief review of paradoxical presentations with other novel targeted therapies

Abstract

With recent advancements in the understanding of vitiligo pathogenesis, Janus kinase (JAK) inhibitors have emerged as a promising new treatment modality, but their effects remain incompletely elucidated. Tofacitinib, an oral JAK 1/3 inhibitor approved for the treatment of rheumatoid arthritis, has previously been shown to induce significant re-pigmentation in vitiligo. However, as with other novel targeted therapies, cutaneous adverse effects have been observed. We report a 36-year-old woman with a history of rheumatoid arthritis, refractory to multiple pharmacotherapies, who was initiated on tofacitinib and subsequently developed progressive depigmented patches consistent with new-onset vitiligo. Although definitive causation cannot be established in this case without additional studies, it is important to note that many targeted therapies have the potential to induce paradoxical effects, that is, the occurrence or exacerbation of pathologic conditions that have been shown to respond to these medications. Paradoxical findings with other targeted therapies include the occurrence of melanoma during treatment with BRAF inhibitors, keratoacanthomas with PD-1 inhibitors, vitiligo and psoriasis with TNF-alpha inhibitors, and hidradenitis suppurativa with various biologic agents. Although JAK inhibitors hold therapeutic promise in the treatment of inflammatory skin disorders, further research is warranted to more fully comprehend their effects.

Abstract

Hidradenitis suppurativa is a severe and debilitating dermatologic disease. Clinical management is challenging and consists of both medical and surgical approaches, which must often be combined for best outcomes. Therapeutic approaches have evolved rapidly in the last decade and include the use of topical therapies, systemic antibiotics, hormonal therapies, and a wide range of immunomodulating medications. An evidence-based guideline is presented to support health care practitioners as they select optimal medical management strategies and is reviewed in this second part of the management guidelines. A therapeutic algorithm informed by the evidence available at the time of the review is provided.

Abstract

Hidradenitis suppurativa is a chronic inflammatory disorder affecting hair follicles, with profoundly negative impact on patient quality of life. Evidence informing ideal evaluation and management of patients with hidradenitis suppurativa is still sparse in many areas, but it has grown substantially in the last decade. Part I of this evidence-based guideline is presented to support health care practitioners as they select optimal management strategies, including diagnostic testing, comorbidity screening, and both complementary and procedural treatment options. Recommendations and evidence grading based on the evidence available at the time of the review are provided.
