2. Factors associated with medication non-adherence among patients with heart failure

Abstract Objective: to identify the factors contributing to medication non-adherence among patients with heart failure. Method: cross-sectional and analytical study using the Medida de Adesão ao Tratamento [Treatment Adherence Measure] scale to assess medication non-adherence. Independent variables were collected using the European Heart Failure Self-care Behavior Scale and an instrument developed by the authors based on a previous study. Statistical tests were implemented to analyze data with p≤0.05 statistical significance. Results: the sample comprised 340 patients, with 9.4% considered non-adherent. The multiple analysis results showed that one unit increase in an individual’s self-care score led to an 8% increase in the prevalence of non-adherence; patients with a family income above three times the minimum wage presented a prevalence of non-adherence equal to 3.5% of the prevalence of those with up to one times the minimum wage; individuals consuming alcohol or with depression presented 3.49 and 3.69 times higher prevalence of non-adherence, respectively, than individuals not presenting such history. Conclusion: medication non-adherence was associated with self-care, family income, depression, and alcohol consumption.

ABSTRACT Objectives: to identify in the literature the main nursing diagnoses according to the NANDA-I diagnostic classification for people hospitalized with heart failure. Methods: an integrative literature review, carried out in February 2019 and updated in July 2023, in the MEDLINE via PubMed, LILACS, SciELO and CINAHL databases. Given the use of acronym PEO, studies without a time cut in Portuguese, English and Spanish were included. Descriptive analysis was carried out to present the identified information. Results: analysis of 27 articles identified 24 nursing diagnoses, with emphasis on Decreased Cardiac Output, Excessive Fluid Volume, Decreased Activity Tolerance and Fatigue. Final Considerations: evidence can contribute to better diagnostic decisions centered on people with heart failure in search of more assertive health results and have the potential to support future studies on a possible syndromic pattern in this population.

5. Psychometric evaluation of the Functional Health Pattern Assessment Screening Tool – Modified Brazilian Version

Objective: to test the factorial structure, reliability and convergent validity of the Functional Health Pattern Assessment Screening Tool – Modified Brazilian Version. Method: this was a psychometric evaluation of the Functional Health Pattern Assessment Screening Tool – Modified Brazilian Version. Seven hundred and seventeen participants answered the data collection instrument consisting of two parts. Part I included a structured questionnaire to collect sociodemographic data and the participants’ perceptions and satisfaction with their current health status. Part II consisted of the tool being tested. The internal structure was assessed using Confirmatory Factor Analysis. Convergent validity was evaluated by the correlation of the tool scores with the rates corresponding to self-perception and satisfaction with current health status. Reliability was assessed using Cronbach’s alpha. Results: the Confirmatory Factor Analysis confirmed a three-factor solution. The factor loadings were significant and varied from 0.16 to 0.75; the fit indices suggested moderate fit of the model. Internal consistency for all three components varied between 0.779 and 0.919. Conclusion: the findings suggest that the tool is valid and reliable to be used in the Brazilian population, although caution is recommended when interpreting the results due to the moderate fit of the model.

ABSTRACT Objective: To analyze the evidence of content validity of telephone messages regarding preventive measures against COVID-19. Method: Methodological study, in which messages containing text and image were developed through a narrative literature review and whose content was evaluated by eight judges in terms of clarity, practical relevance, theoretical relevance and vocabulary. The content validity index (CVI) was calculated, and messages that reached levels above 90% were considered to have adequate evidence of validity. Results: Eighteen text messages/images were developed containing information about COVID-19, hand hygiene, use and handling of masks and the importance of social distancing. After second round of evaluation, a content validity index above 90% was obtained in all evaluated indicators. Conclusion: The telephone messages were developed and showedadequate evidence of content validity.

ABSTRACT Objectives: to analyze the concept of cardiovascular risk to support nursing practice. Methods: this is an analysis to define the concept of cardiovascular risk, through the use of eight steps of Walker’s and Avant’s framework, using a literature review in indexed scientific journals. Results: cardiovascular risk is defined in a broad and original way as a context of health and nursing care that makes it possible to identify modifiable (cardiometabolic, behavioral, psychosocial, cultural and occupational) and non-modifiable (biological) risk factors for cardiovascular diseases that act as early and interrelated markers, of multiple and heterogeneous etiology, predisposing to cardiovascular vulnerability. Conclusions: with the analysis and definition of the concept of cardiovascular risk, we realized that it will be possible to base nursing practice, with implications in clinical practice for identifying and reducing risk factors for cardiovascular diseases, with nursing relevance in the care of these subjects.

Resumo Objetivo avaliar o efeito do Guia Autoinstrucional para Raciocínio Clínico na acurácia diagnóstica de estudantes de Bacharelado em Enfermagem. Método ensaio clínico randomizado, paralelo, duplo cego (pesquisadores e avaliadores dos desfechos), realizado com graduandos de Enfermagem. Aplicaram-se, em duas fases, estudos de caso validados para a identificação de diagnóstico de enfermagem/problema do paciente, etiologia e pistas, sendo utilizado o Guia com o grupo intervenção na segunda fase. Os desfechos - acurácia diagnóstica, etiológica e número de pistas - foram avaliados por meio de rubricas validadas. Para análise, utilizou-se estatística descritiva para dados demográficos; teste exato de Fisher para similaridades na educação prévia e confiança; teste de Mann-Whitney para idade; teste ANOVA não paramétrico na avaliação da hipótese de diferenças no desempenho. Resultados amostra final composta por 24 estudantes no grupo controle e 27 no intervenção; sem diferença quanto ao sexo, idade e educação. Houve diferença para acurácia diagnóstica (p=0,041) e etiológica (p=0,0351) no grupo intervenção, mostrando efeito negativo da utilização do Guia. Conclusão a autoinstrução implementada uma única vez não foi efetiva em impactar a acurácia diagnóstica de estudantes solucionando estudos de caso. A aplicação repetida do Guia como ferramenta didática pode ser efetiva para melhorar tal desfecho. REBEC: RBR-4bhr78.

Methods: An observational and cross-sectional study including family members of patients hospitalized in the coronary intensive care unit for acute coronary syndrome in Killip I or II. Objetivo: Identificar a satisfação dos pacientes acompanhados em um Ambulatório de Educação em Saúde e avaliar o efeito das variáveis antecedentes com o grau de satisfação dos pacientes. Métodos: Trata-se de um estudo de corte transversal, realizado em um Ambulatório de Educação em Saúde. A amostra foi composta por todos os pacientes cardiopatas atendidos neste ambulatório (17 pacientes). A satisfação dos pacientes foi avaliada por meio do instrumento Satisfação do Paciente (ISP). Resultados: Observou-se que todos os pacientes relataram um alto nível de satisfação e os domínios que obtiveram maiores e menores pontuações foram o profissional e o educacional, respectivamente. Não houve correlação significativa do nível de satisfação com nenhuma das variáveis antecedentes. Conclusão: Os pacientes apresentaram alto nível de satisfação e nenhuma variável se relacionou com a satisfação.

ABSTRACT Objective: To discuss the contribution of nursing theories in the conceptual expansion of good practices in the area for the construction of the unified health system (Sistema Único de Saúde – SUS). Method: Theoretical production of reflection, based on the neomodern framework of philosophy of science. Results: The knowledge and political, social and epistemological commitments of nursing are discussed, highlighting the relationship of the theories with good practices for the construction and application of knowledge in the area in the context of the Brazilian health care system. Reflection on how middle-range theories can help in solving dichotomies between discipline, theory, practice, producers and consumers of knowledge is made. Conclusion: Original reflections on the contributions of disciplinary and professional knowledge of nursing theories to the creation of SUS are presented, defending a creation through the academy-service partnership, in which the theories start being incorporated into a new concept of nursing practices aligned to neomodern principles of science.

Objective: develop and validate a video on bed bathing directed to nursing professionals and students. Method: the video was based on the literature and presents the definition of bed bath, indications for its performance, steps to perform it, and potential complications. Nursing professors and nurses validated it. They assessed the pertinence of content, clarity, and language of the video scenes. The Delphi Technique was used in this phase. After recording, three nursing professors, along with undergraduate nursing students from a public university, assessed the educational video. The professors first watched the video and suggested changes, and then the students watched the video after the changes were implemented. Results: six rounds were needed for experts to validate the video script using the Delphi Technique. After the video recording, undergraduate students considered the video of easy understanding. Conclusion: the video script was composed of four topics and was validated by experts after six rounds. The video was assessed by the professors and nursing undergraduate students, who considered the topics and the video as a whole as apprehensible. This study is expected to contribute to professional training and improvement of the knowledge and skills of nursing students.

ABSTRACT Objectives: To evaluate the effectiveness of NIC interventions “Teaching: Disease Process”, “Health Education” and “Cardiac Care” in the improvement of NOC results in individuals with Heart Failure (HF), and the Nursing Diagnosis (ND) “Ineffective Health Control”. Methods: Retrospective cohort, conducted at a Health Education in Nursing outpatient clinic. Fourteen patients with HF had follow-up for one year in six bimonthly consultations. The effectiveness of interventions was verified through the change of the result scores through the Friedman’s Non-Parametric Test and Analysis of Non-Parametric Variance for repeated measures. Abstract OBJECTIVE To identify predictors of red blood cell transfusion (RBCT) after cardiac surgery. METHOD A prospective cohort study performed with 323 adults after cardiac surgery, from April to December of 2013. A data collection instrument was constructed by the researchers containing factors associated with excessive bleeding after cardiac surgery, as found in the literature, for investigation in the immediate postoperative period. The relationship between risk factors and the outcome was assessed by univariate analysis and logistic regression. RESUMOObjetivo:Avaliar se o consumo alimentar de pacientes com síndrome coronariana aguda (SCA) segue as recomendações nacionais e internacionais e se existe relação com níveis de estresseMétodo:Estudo transversal com 150 pacientes com SCA que foram entrevistados utilizando-se os Questionários de Frequência Alimentar e Escala de Estresse Percebido-10.Resultados:Consumo diário acima das recomendações: colesterol (92%), fi bras (42,7%) e proteínas (68%); consumo abaixo das recomendações: potássio (88%) e carboidratos (68,7%); consumo conforme recomendações: sódio (53,3%) e lípides (53,3%). A maioria (54%) com consumo alimentar inadequado era estressada ou altamente estressada. Houve associação estatisticamente signifi cante entre menor nível de estresse e maior consumo de fibras.Conclusão:Em pacientes com SCA, o consumo alimentar esteve fora do recomendado pelas diretrizes e o menor consumo de fibras ocorre concomitantemente com maior estresse. Esforços educativos podem instrumentalizar os pacientes na adequação do consumo alimentar e no controle do estresse.

OBJECTIVES: The Millennium Development Goals are centered around combatting poverty and other social evils all over the world. Thus, this study seeks to identify the Millennium Development Goals as an object of study in theses from Postgraduate Nursing Programs in Brazil scoring 5 (national excellence) and 6 or 7 (international excellence), and evaluate the association between the score for the program and achieving the Millennium Development Goals.METHOD: Exploratory descriptive document research. Data were collected from the Notes on Indicators/Coordination for Higher Education Personnel Improvement for the 15 Postgraduate Nursing Courses scoring between 5 and 7 in the three-year-period of 2010/2012.RESULTS: of the 8 Millennium Development Objectives, 6 were dealt with in the theses. There was an association (Fisher's exact test p-value=0.0059) between the distribution of the theses and the program scores in relation to the Millennium Development Objectives (p-valor=0.0347)CONCLUSION: the doctoral theses were slightly related to the Millennium Development Objectives, covering the population's economic development, health conditions and quality of life. It is recommended that Postgraduate Programs in Nursing pay closer attention to the Millennium Development Objectives..

RESUMO A doença arterial coronariana é a principal causa de morbimortalidade no mundo e compromete o desempenho individual e familiar. O objetivo deste estudo foi identificar os fatores de risco da doença arterial coronariana e associá-los ao perfil sociodemográfico dos familiares conviventes de pacientes com síndrome coronariana aguda. Trata-se de estudo transversal realizado em um hospital universitário. A amostra foi composta de 50 famílias distintas e 158 familiares conviventes de pacientes com síndrome coronária aguda. Os fatores de risco foram avaliados mediante entrevista com instrumento elaborado pelos pesquisadores e por meio de instrumentos previamente validados. Os dados foram submetidos à estatística descritiva e para a avaliação das relações entre as variáveis foram utilizados testes de associação. A maioria das famílias apresentava baixa renda, baixo nível de escolaridade e elevado número de conviventes e dependentes. A hipertensão arterial, diabetes mellitus, dislipidemia, obesidade, sedentarismo, etilismo e tabagismo foram os fatores de risco mais encontrados. As associações registradas foram: idade acima dos 51 anos com diabetes mellitus, hipertensão arterial, dislipidemia; idade entre 20 e 50 com alcoolismo e tabagismo; baixa escolaridade com diabetes mellitus, obesidade e hipertensão arterial; estado civil com diabetes mellitus, hipertensão arterial, dislipidemia e alcoolismo; ocupação com alcoolismo e tabagismo; baixa renda com alcoolismo; e o alcoolismo com tabagismo. O estilo de vida inadequado associado aos fatores sociodemográficos e às comorbidades já existentes demonstram a necessidade de educação em saúde também para esses familiares conviventes.

RESUMO A alta incidência e prevalência da doença coronariana é evidente no Brasil e no mundo. A concepção de saúde e enfermidade influencia nas atitudes e condutas dos indivíduos diante do processo saúde-doença e afeta o desempenho e o desenvolvimento do núcleo familiar. O objetivo foi analisar crenças, valores e atitudes dos familiares conviventes com pacientes com síndrome coronariana aguda sobre a doença coronariana. Trata-se de estudo transversal com abordagem qualitativa realizado em um Hospital Universitário. A amostra foi composta de nove familiares, mediante uma entrevista semiestruturada. Os dados foram coletados por meio de instrumentos com questões elaboradas tendo como base as premissas do referencial teórico e avaliados segundo o Modelo de Crença em Saúde. Os familiares tinham entre 26 e 66 anos. As categorias suscetibilidade, severidade, benefícios e barreiras percebidas derivadas do Modelo de Crença em Saúde, assim como a categoria conscientização e mudança, mostraram que os familiares possuem conhecimento sobre os aspectos preventivos e de controle das consequências da doença coronariana, porém as crenças e valores adquiridos ao longo de suas vidas, influenciados pelo contexto cultural, social, econômico, emocional e principalmente familiar, interferem nos comportamentos preventivos da doença e promotores de saúde. A despeito do conhecimento que os familiares conviventes possuem sobre a doença, o enfermeiro precisa adequar o processo educacional para minimizar os conflitos decorrentes de crenças, valores e contexto do ambiente familiar, facilitar e promover a incorporação de novos hábitos.

OBJECTIVE: to identify the relationship between different presentations of acute coronary syndrome and cardiovascular risk factors among hospitalized individuals.METHOD: cross-sectional study performed in a teaching hospital in São Paulo, in the State of São Paulo (SP). Socio-demographic, clinical and anthropometric data of 150 individuals hospitalized due to acute coronary syndrome were collected through interviews and review of clinical charts. Association between these data and the presentation of the syndrome were investigated.RESULTS: there was a predominance of ST segment elevation acute myocardial infarction. There was significant association of systemic hypertension with unstable angina and high values of low density lipoprotein with infarction, without influence from socio-demographic characteristics.CONCLUSION: arterial hypertension and high levels of low-density lipoprotein were associated with different presentations of coronary syndrome. The results can provide support for health professionals for secondary prevention programs aimed at behavioural changing.

Trata-se de ensaio clínico randomizado, em que se objetivou comparar a frequência e intensidade de sintomas de ansiedade de pacientes em pré-operatório de cirurgia cardíaca que receberam acolhimento do enfermeiro ou do familiar ou os que não receberam nenhum tipo específico de acolhimento. A amostra foi constituída de 66 pacientes em pré-operatório de cirurgia cardíaca, que foram alocados em três grupos: acolhimento pelo enfermeiro, sem acolhimento específico e acolhimento pelo familiar. A ansiedade foi avaliada em dois momentos: antes e após a intervenção. O instrumento utilizado foi o construído e validado por Suriano, composto por 19 características definidoras do diagnóstico de enfermagem ansiedade. Observou-se que a redução dos sintomas de ansiedade foi maior no grupo que recebeu acolhimento dos familiares quando comparado aos outros dois grupos. Os resultados sugerem que o incentivo à participação de familiares pode contribuir para a redução de sintomas ansiosos em pacientes no pré-operatório de cirurgias cardíacas.

This was a descriptive, retrospective study, with a quantitative method, with the aim of analyzing the nursing diagnoses contained in the records of children of 0 to 36 months of age who attended infant health nursing consults. A documentary analysis and the cross-mapping technique were used. One hundred eighty-eight different nursing diagnoses were encountered, of which 33 (58.9%) corresponded to diagnoses contained in the Nomenclature of Nursing Diagnoses and Interventions and 23 (41.1%) were derived from ICNP® Version 1.0. Of the 56 nursing diagnoses, 43 (76.8%) were considered to be deviations from normalcy. It was concluded that the infant health nursing consults enabled the identification of situations of normalcy and abnormality, with an emphasis on the diagnoses of deviations from normalcy. Standardized language favors nursing documentation, contributing to the care of the patient and facilitating communication between nurses and other health professionals.

A principal tabela de áreas em uso pelo CNPq está reconhecidamente desatualizada, necessitando de revisão ou reclassificação. O objetivo deste artigo é contribuir para o atual debate, com base em resultados prévios de participação em trabalho de revisão da tabela realizado na década de 1990 e de resultados de pesquisas na área da organização e representação do conhecimento no contexto da Ciência da Informação. Apresenta uma proposta de reformulação da Área de Conhecimento da Enfermagem e algumas reflexões sobre as possibilidades deste processo em curso.

OBJECTIVES: to validate the concept "risk for decreased cardiac output". METHOD: Six of the eight steps suggested in the technique developed by Walker & Avant were adopted to analyze the concept of the phenomenon under study and the proposal made by Hoskins was used for content validation, taking into account agreement achieved among five experts. RESULTS: the concept "decreased cardiac output" was found in the nursing and medical fields and refers to the heart's pumping capacity while the concept "risk" is found in a large number of disciplines. In regard to the defining attributes, "impaired pumping capacity" was the main attribute of decreased cardiac output and "probability" was the main attribute of risk. The uses and defining attributes of the concepts "decreased cardiac output" and "risk" were analyzed as well as their antecedent and consequent events in order to establish the definition of "risk for decreased cardiac output", which was validated by 100% of the experts. CONCLUSION: The obtained data indicate that the risk for decreased cardiac output phenomenon can be a nursing diagnosis and refining it can contribute to the advancement of nursing classifications in this context.OBJETIVO: Validar el concepto riesgo del débito cardíaco disminuido. MÉTODO: Fue adoptada la técnica de Walker & Avant para analizar la definición del fenómeno enfocado, utilizando seis de las ocho etapas sugeridas y la propuesta de Hoskins para validar el contenido, considerándose la conformidad entre cinco expertos. RESULTADOS: el concepto de debito cardíaco disminuido se encuentra en las áreas enfermería y médica y su atención se centra en la capacidad de bombeo del corazón. Sin embargo, el concepto de riesgo es presente en un gran número de áreas. Las características definitorias de la disminución del gasto cardíaco mostraron como principal atributo deficiencia de la bomba cardiaca y, para el riesgo, el atributo de probabilidad. Analizados los usos y características definidoras de los conceptos "débito cardíaco disminuido" y "riesgo" con los hechos anteriores y subsiguientes, fue posible elaborar la definición de "riesgo de débito cardíaco disminuido", la cual fue validada por 100% de los expertos. CONCLUSIÓN: Los resultados encontrados permitieron concluir que el fenómeno de riesgo de débito cardíaco disminuido puede constituir un diagnóstico de enfermería y su refinamiento puede contribuir al avance de las clasificaciones de enfermería.OBJETIVO: validar o conceito de risco de débito cardíaco diminuído. RESULTADOS: o conceito de débito cardíaco diminuído foi encontrado nas áreas de enfermagem e médica e está centrado na capacidade de bombeamento do coração, embora também esteja presente em um grande número de disciplinas. Em relação aos atributos definidores, o débito cardíaco diminuído teve como atributo principal a deficiência de bombeamento cardíaco e o risco, o atributo de probabilidade. Analisados os usos e os atributos definidores dos conceitos de "débito cardíaco diminuído" e "risco", seus eventos antecedentes e consequentes, construiu-se a definição de risco de débito cardíaco diminuído, que foi validada por 100% dos especialistas. CONCLUSÃO: os dados obtidos permitiram inferir que o fenômeno risco de débito cardíaco diminuído pode se constituir num diagnóstico de enfermagem e, por meio do seu refinamento, contribuir para o avanço das classificações de enfermagem nesse contexto.

This integrative review investigates how nurses plan the hospital discharge of patients with Congestive Heart Failure (CHF) since an inadequate discharge plan and patients’ subsequent non-adherence to instruction provided upon discharge are indicated as potential factors for re-hospitalization. A total of 24 papers were found in a search carried out in the LILACS and MEDLINE databases between 2004 and 2008, which given the inclusion criteria, were reduced to 14 papers. The papers were analyzed and categorized into ‘Health Education’, and ‘Nursing Care’. The synthesis of results indicates that the discharge plan devised by nurses is based on two categories. The actions of nurses to promote health education can enable patients with CHF to improve self-care.Artículo de revisión integradora que tiene por objetivo identificar como las enfermeras han planificado el alta de los pacientes con Insuficiencia Cardíaca Congestiva, ya que el plan de alta inadecuado y el no seguimiento de las orientaciones dadas son apuntados como posibles factores de rehospitalización. A través de la búsqueda en las bases de datos LILACS y MEDLINE, abarcando el período de 2.004 a 2.008, fueron encontrados 24 artículos, que por el criterio de selección de la muestra resultaron en 14. Los artículos fueron analizados y categorizados en Educación en Salud y Cuidado de Enfermería. Con eso fue posible sintetizar los resultados e identificar que el plan de alta realizado por los enfermeros está basado en estas dos categorías, ya que a través de las conductas del enfermero para promover la educación en salud será posible proporcionar al paciente con Insuficiencia Cardíaca Congestiva una mejoría en el autocuidado.Trata-se de artigo de revisão integrativa que teve por objetivo identificar como as enfermeiras têm planejado a alta dos pacientes com insuficiência cardíaca congestiva, pois o inadequado plano de alta e o não seguimento das orientações dadas são apontados como possíveis fatores de re-hospitalização. Através da busca nas bases de dados LILACS e MEDLINE, abrangendo o período de 2004 a 2008, foram encontrados 24 artigos que, pelo critério de seleção da amostra, resultaram em 14. Os artigos foram analisados e categorizados em Educação em Saúde e Cuidado de Enfermagem. Com isso, foi possível sintetizar os resultados e identificar que o plano de alta realizado pelos enfermeiros está baseado nessas duas categorias, pois as condutas do enfermeiro para promover a educação em saúde podem proporcionar ao paciente com insuficiência cardíaca congestiva melhora no autocuidado.

This cross-sectional study was carried out at a university hospital to describe the nursing interventions most frequently performed in the clinical practice of an intensive care unit, based on nursing care prescriptions, and to investigate their similarity to the Nursing Interventions Classification (NIC). The sample consisted of 991 hospitalizations of patients. Data were retrospectively collected from the computer database and analyzed through descriptive statistics and cross-mapping. A total of 57 different NIC interventions frequently used in the unit were identified; most of them in the complex (42%) and basic physiological (37%) domains, in the classes ‘respiratory management’ and ‘self-care facilitation’. Similarity between the nursing care prescribed and nursing interventions/NIC was found in 97.2% of the cases. The conclusion is that the interventions/NIC used in the clinical practice of this intensive care unit reflects the level of complexity of nursing care, which is mainly directed at the regulation of the body’s physical and homeostatic functioning.Se trata de un estudio descriptivo, transversal realizado en un hospital universitario con los objetivos de describir las intervenciones de enfermería más utilizadas en la práctica clínica de una unidad de terapia intensiva, con base en las prescripciones de enfermería y, analizar si son similares a las Nursing Interventions Clasification (NIC). La muestra constó de 991 internaciones de pacientes. Los datos fueron recolectados retrospectivamente, en base informatizada, y analizados por la estadística descriptiva y diseño cruzado. Se identificó 57 diferentes intervenciones/NIC frecuentemente utilizadas en la unidad; siendo la mayoría en el dominio fisiológico complejo (42%) y fisiológico básico (37%), en las clases de control respiratorio y facilitación del autocuidado. En 97,2% de los casos se encontraron similares entre las prescripciones de enfermería de la unidad y las intervenciones/NIC. This study compared the levels of anxiety presented by patients with acute myocardial infarction in bed and shower baths and the influence of antecedent variables: age, gender, medications, previous hospitalization and/or bed bath, patients' preference regarding the professional's gender, risk factors and anxiety-traits. This crossover study was conducted between February and August 2007 in coronary units. The sample was composed of 71 patients with acute myocardial infarction. The State-Trait Anxiety Inventory (STAI) was applied before the baths (bed and shower baths), immediately after the baths and twenty minutes after the second evaluation. Results revealed that patients were more anxious in the bed bath than in the shower in the three assessments (p Este estudio tuvo como objetivo comparar los niveles de ansiedad, generados en el baño en la cama y en el de aspersión, en pacientes con infarto agudo del miocardio, así como la influencia de las variables antecedentes: edad, sexo, medicaciones, internación y/o baño en la cama previo, preferencia del paciente en cuanto al sexo del profesional, factores de riesgo y la ansiedad de trazo. Esta investigación es un estudio cross-over, realizado entre febrero y agosto de 2007, en Unidades Coronarias. La muestra fue constituida por 71 pacientes con infarto agudo del miocardio. El instrumento utilizado fue el IDATE-estado, siendo aplicado antes de los baños (cama y aspersión), inmediatamente después de los baños y veinte minutos después de la segunda evaluación. A identificação dos diagnósticos de enfermagem mais comumente relatados na literatura no perioperatório de cirurgia cardíaca pode auxiliar enfermeiros nesse contexto da prática clínica a planejar cuidados de enfermagem adequados às necessidades individuais de cada paciente, colaborando para a implementação de ações rápidas e eficazes para a resolução dos problemas identificados. Desenvolveu-se uma revisão integrativa de literatura com o objetivo de identificar os diagnósticos de enfermagem no perioperatório de cirurgia cardíaca. A busca aconteceu nas bases de dados LILACS e SCIELO com os descritores "diagnóstico de enfermagem" ou "nursing diagnosis", "cuidados de enfermagem" ou "nursing care" e "cirurgia cardíaca" ou "thoracic surgery". Foram incluídos 13 trabalhos, os quais identificaram 17 principais diagnósticos no pré-operatório, 10 principais diagnósticos no transoperatório e 28 principais diagnósticos no pós-operatório. Diferentes estudos frequentemente encontraram os mesmos diagnósticos em cada período, o que reforça um perfil com o qual os enfermeiros podem esperar lidar e sobre o qual devem intervir para obter melhores resultados. Estudos adicionais sobre a acurácia diagnóstica e a validade de tais diagnósticos são esperados.

ABSTRACT Objective: to characterize the development of research on nursing diagnoses in nursing graduate programs and verify the application of the results of these studies according to the perception of academic advisors. Methods: documental research on dissertations and theses available from the databases of Brazilian universities with graduate programs in nursing, and exploratory survey on the practical application of their results. Results: 216 dissertations and theses were analyzed. Most of them were descriptive studies, and the most widely used diagnostic taxonomy was NANDA International. Little practical use of the knowledge produced by the dissertations and theses was identified. Conclusion: Although there has been a considerable increase in research on nursing diagnoses, the survey stage revealed scarce practical application of their results, prompting the need for further studies in order to identify the barriers in the way of transposing theory to practice.

30. Impact of anxiety and depression on morbidity and mortality of patients with coronary syndrome

ABSTRACT Objective: Evaluate the impact of anxiety and depression on morbidity and mortality of patients with acute coronary syndrome. Method: Retrospective cohort study, with follow-up of two years, conducted with 94 patients. The morbidity and mortality (readmission, myocardial revascularization, and death) was evaluated immediately after discharge and after one and two years. Anxiety and depression were evaluated by the State-Trait Anxiety Inventory and by Beck’s Depression Inventory. The Kaplan-Meier estimator and the Logrank test were used. The significance level adopted was 0.05. Results: We observed that 76.6% of the patients did not present symptoms of depression or had mild signs, while 78.8% had low to moderate anxiety. The symptoms of depression and anxiety were not related to morbidity (need for MR p=0.098 and 0.56, respectively; readmission p=0.962 and 0.369, respectively) and mortality (p=0.434 and 0.077, respectively). Conclusion: No relationship was found between levels of anxiety and depression with the morbidity and mortality of patients.

ABSTRACT Objective: to describe faculty member training as a strategy for the development of leaders and the promotion of continuous academic progress in teaching, research, and extension. Method: this reflection article presents a history of the highest academic degree obtained in Brazil, describing the characteristics of the competitions and highlighting aspects and activities that show, nowadays, intellectual and scientific maturation. Results: aspiring professors submit their academic trajectory to analysis and judgment by peers, based on strict assessment criteria, to receive a professor habilitation. Its purpose is to promote a moment of analysis of their academic career and to identify the remarkable contribution to the advancement of the university and the country through consolidated and impactful activities. Final considerations: pursuing this highest degree in the Brazilian academic career can be a moment of individual and collective advancement, contributing to the consolidation of areas of knowledge.

32. The Healthcare Environment Survey: A multicountry psychometric evaluation of nurses’ job satisfaction

To evaluate the properties of a reduced-item Healthcare Environment Survey measuring nurses' job satisfaction across eight countries.There is currently no rigorously tested international measure of nurses' job satisfaction that can be used internationally to improve the nurse work environment.Nursing staff from 11 hospitals in eight countries participated in this study. The original 57-item, 11-facet Healthcare Environment Survey was evaluated for reliability, validity, and measurement invariance: Cronbach's alpha was used to test for reliability; construct, discriminate, and convergent testing were used to test validity; and invariance testing including configural, metric, and scalar tests were used to study measurement invariance between the countries.2,046 nursing staff completed the survey. Reliability was established for all six subscales and the combined composite score. Both validity and measurement invariance were supported in every test conducted. An excellent model fit was found for the final 19-item, 6-facet Healthcare Environment Survey that explained 82% of the variance of nurses' job satisfaction.Findings suggest the instrument is an efficient measure of nurses' job satisfaction across multiple countries. Longitudinal testing for invariance will be needed to ensure the model remains a good fit. Testing more countries will also verify model fit.The instrument can be used to measure nurse job satisfaction globally.The instrument can be used to assess interventions to improve the social (patient, unit manager, and coworker) and technical (professional rewards, autonomy, and professional growth) aspects of nurse job satisfaction.

Resumo Objetivo Verificar a prevalência da ansiedade e sua associação com os fatores sociodemográficos e clínicos em mulheres com hipertensão arterial sistêmica. Métodos Estudo transversal com 258 mulheres com hipertensão arterial sistêmica diagnosticada há pelo menos seis meses e que eram atendidas no ambulatório de Hipertensão Arterial de uma instituição pública voltada ao ensino, pesquisa e assistência na cidade de São Paulo. O instrumento continha dados sociodemográficos, clínicos e de hábitos de vida e foi preenchido por meio de uma entrevista. A ansiedade foi avaliada pelo inventário de Ansiedade Traço e classificada em baixa, moderada, elevada e muito elevada. A avaliação da associação entre os fatores sociodemográficos e clínicos com o nível de ansiedade foi realizada pelos testes de associação e regressão logística simples multinomial, considerando o nível de significância de 5%. Resultados Identificou-se que 70,5% apresentavam ansiedade moderada e 19,4% elevada. Na regressão logística multinomial simples identificou-se que quanto maior a idade maior a chance de ansiedade elevada (p=0,01; Odds Ratio =1,09), as mulheres sem companheiro tinham maiores chances de ansiedade elevada (p=0,02, Odds Ratio =3,19) e com o aumento da renda mensal familiar menor foi a chance de ansiedade elevada (p=0,04, Odds Ratio =0,99). Conclusão Houve alta prevalência de ansiedade moderada na população estudada e a ausência de companheiro foi o fator que melhor explicou o fenômeno de ansiedade. Os enfermeiros devem propor intervenções, principalmente para estas pessoas, com o intuito de reduzir tal sentimento. Resumen Ocurrencias Verificar la prevalencia de la ansiedad y su relación con los factores sociodemográficos y clínicos en mujeres con hipertensión arterial sistémica. Métodos Estudio transversal con 258 mujeres con hipertensión arterial sistémica diagnosticadas hace seis meses por lo menos y que habían sido atendidas en consultorios externos de Hipertensión Arterial de una institución pública orientada a la educación, investigación y atención en la ciudad de São Paulo. El instrumento contenía datos sociodemográficos, clínicos y de hábitos de vida y fue completado mediante una encuesta. La ansiedad fue evaluada mediante el inventario de rasgos de ansiedad y clasificada como baja, moderada, alta o muy alta. La evaluación de la relación entre los factores sociodemográficos y clínicos con el nivel de ansiedad fue realizada con la prueba de asociación y regresión logística simple multinominal, con un nivel de significación de 5 %. Resultados Se identificó que el 70,5 % presentó ansiedad moderada y el 19,4 % alta. En la regresión logística multinomial simple se identificó que, cuanto mayor era la edad, mayor era la probabilidad de ansiedad alta (p=0,01; Odds Ratio =1,09), las mujeres sin compañero tenían mayores probabilidades de ansiedad alta (p=0,02, Odds Ratio =3,19) y con el aumento de los ingresos familiares mensuales, la probabilidad de ansiedad alta fue menor (p=0,04, Odds Ratio =0,99). Conclusión Se observó una alta prevalencia de ansiedad moderada en la población estudiada y la ausencia de compañero fue el factor que mejor explicó el fenómeno de ansiedad. Los enfermeros deben proponer intervenciones, principalmente para estas personas, con el objetivo de reducir ese sentimiento. Abstract Objective To verify the prevalence of anxiety and its association with sociodemographic and clinical factors in women with hypertension. Method This is a cross-sectional study with 258 women with hypertension diagnosed for at least six months and who were treated at the hypertension outpatient clinic of a public institution dedicated to teaching, research and care in the city of São Paulo. The instrument contained sociodemographic, clinical and lifestyle data and was completed through an interview. Anxiety was assessed by the State-Trait Anxiety inventory and classified as low, moderate, high and very high. The assessment of the association between sociodemographic and clinical factors with anxiety level was performed using association tests and simple multinomial logistic regression, considering a significance level of 5%. Results It was found that 70.5% had moderate anxiety and 19.4% had high anxiety. In the simple multinomial logistic regression, it was identified that the older the age, the greater the chance of high anxiety (p=0.01; Odds Ratio =1.09), women without a partner were more likely to have high anxiety (p=0.02, Odds Ratio =3.19), and with increasing monthly family income, the chance of high anxiety was lower (p=0.04, Odds Ratio =0.99). Conclusion There was a high prevalence of moderate anxiety in the population studied and the absence of a partner was the factor that best explained the anxiety phenomenon. Nurses should propose interventions, especially for these people, in order to reduce this feeling.

PURPOSE To assess the validity of a Written Clinical Reasoning Prompt (WCRP) to help nursing students' clinical reasoning (CR). METHOD This is a methodological study of translation and cross-cultural adaptation into Brazilian Portuguese of instruments that aim to promote CR improvement, composed of (1) WCRP; (2) two case studies; (3) a questionnaire about students' perceptions during decision-making in case studies; (4) a scoring rubric for correcting case studies. For translation and cross-cultural adaptation, stages 1-8 of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) Consortium Network were adopted. Agreement values among experts >80% and content validity coefficient (CVC) > 0.8 were considered satisfactory. For the pretest, a randomized clinical trial was carried out with 24 nursing students (intervention group, n = 14, using the WCRP to solve case studies; control group, n = 10, without using the WCRP). FINDINGS The WCRP was translated and adapted into Brazilian Portuguese, requiring minimal adjustments to obtain agreement among the judges above 80% and CVC above 0.80. Regarding face validation, an adequate agreement was obtained in the assessment by students. There was no difference in the accuracy of nursing diagnoses between the intervention and control groups in the pretest. CONCLUSION The WCRP was translated and adapted into Brazilian Portuguese and had adequate face and content validity estimates; however, there was no association with improving nursing students' diagnostic accuracy. IMPLICATIONS FOR NURSING PRACTICE The translated and adapted versions of all instruments into Brazilian Portuguese had adequate evidence of content and face validity. The use of WCRP was not associated with a significant improvement in nursing students' diagnostic accuracy. New studies with larger samples, a sample power of at least 80%, and a level of significance of 5% are needed.

Background In Brazil, there are no instruments available to measure the presence, frequency, severity and distress related to heart failure (HF) symptoms. Aims To adapt the Symptom Status Questionnaire - HF (SSQ-HF) into Brazilian Portuguese and to examine the content validity of the adapted version. Methods The instrument was translated, back-translated and evaluated by an expert committee for semantic, idiomatic, cultural, and conceptual equivalences. An agreement ≥80% was considered adequate. The adapted version was evaluated by both an expert committee (n = 9) for clarity, theoretical relevance and practical relevance (acceptable content validity coefficient (CVC): ≥0.70), and by a patient committee regarding understanding (n = 40). Results The adapted version obtained 100% agreement regarding the equivalences. The total instrument CVC was 0.99. All patients understood the items. Conclusion The SSQ-HF-Brazilian version has satisfactory evidence of equivalence and content validity. Additional psychometric tests are deemed to confirm that the instrument can be used in Brazil.

Background Patients with acute coronary syndrome (ACS) are often admitted to intensive cardiac care units (ICCUs) to have their vital signs and ventricular function monitored. In most cases, they are conscious and bedbound, causing high stress and anxiety levels. Aims and objectives To assess the stressors affecting patients admitted to ICCUs. Design This is a cross-sectional study performed in a public hospital in Sao Paulo, Brazil. Methods A sample of 100 patients with ACS admitted to an ICCU, after 24 hours of hospitalization, was recruited. Sociodemographic and clinical data were collected, and the Intensive Care Unit Stressor Assessment Scale (ICESS) was applied to investigate stressors. The ICESS' internal consistency was estimated by Cronbach's alpha, and the stressors were analysed by descriptive statistics, association tests, and correlation tests. Results Most patients were male, married, and aged 51 to 60 years. The ICESS showed an excellent internal consistency. The mean score identified in patients pointed to a non-stressed to moderately stressed condition. The indicators considered to be more stressful were as follows: "Not knowing ICU length of stay," "Being unable to fulfil family roles," "Missing husband or wife," and "Loss of self-control." Conclusions Stress levels were considered to be non-stressful to moderately stressful. The main stressors were related to psychological distress and physical discomfort dimensions. Relevance to clinical practice Mapping the main stressors of patients with coronary artery disease will allow health professionals to implement strategies to reduce them and consequently reduce their anxiety levels.

Objectives: to reflect on the global understanding of the Nursing Process concept, with emphasis on the Brazilian context. Methods: a reflection article, aligned with the vision and expertise of researchers who are members of the Nursing Process Research Network. Results: the reflection is presented in two main topics: The evolution of Systematization of Nursing Care X Nursing Process concepts and its consonance with national and international practices, and Brazilian legislation; The Nursing Process concept realignment in Brazilian legislation in line with current care, teaching and research practices. Final Considerations: the reflections were oriented to the Nursing Process’ conceptual, normative and legal issues, including elements of its historical evolution, and, with that, pointed to the need to modify the Brazilian regulation on the Nursing Process. RESUMEN Objetivos: reflexionar sobre la comprensión global del concepto de Proceso de Enfermería, con énfasis en el contexto brasileño. Métodos: ensayo reflexivo, alineado con la visión y experiencia de investigadores integrantes de la Red de Investigación del Proceso de Enfermería. Resultados: la reflexión se presenta en dos temas principales: La evolución de los conceptos de Sistematización de la Atención de Enfermería X Proceso de Enfermería y su consonancia con las prácticas nacionales e internacionales, y la legislación brasileña; La realineación del concepto de Proceso de Enfermería en la legislación brasileña de acuerdo con las prácticas actuales de atención, enseñanza e investigación. Consideraciones Finales: las reflexiones se orientaron a las cuestiones conceptuales, normativas y legales del Proceso de Enfermería, incluyendo elementos de su evolución histórica, y, con eso, apuntaron para la necesidad de cambiar la regulación brasileña sobre el Proceso de Enfermería. ABSTRACT Objectives: to build and validate software for the care of people with cardiovascular risk. Methods: a methodological study, applied to software development, anchored in a nursing theory and classification system, in three stages: 1) requirements engineering; 2) software architecture and coding; and 3) testing and content validity by 12 experts in computer science, with a Content Validity Ratio score. Results: called e-TEORISC, in software format, for nursing care for people with cardiovascular risk, online and offline, containing a database linked to the Nursing Process stages. Experts considered that the attributes of functional suitability, performance efficiency, reliability, maintainability, usability, safety and portability obtained desirable scores. Conclusions: e-TEORISC has evidence of validity to instrumentalize care for people at cardiovascular risk, with potential for technology transfer to the Brazilian Health System.

AimsTo evaluate the effect of postoperative forced-air warming (FAW) on the incidence of excessive bleeding (ExB), arrhythmia, acute myocardial infarction (AMI), and blood product transfusion in hypothermic patients following on-pump CABG and compare temperatures associated with the use of FAW and warming with a sheet and wool blanket.Methods and resultsA randomized clinical trial conducted with 200 patients undergoing isolated on-pump CABG from January to November 2018. Patients were randomly assigned into an Intervention Group (IG, FAW, n = 100) and Control Group (CG, sheet and blanket, n = 100). The tympanic temperature of all patients was measured over a 24-h period. ExB was the primary outcome, while arrhythmia, AMI, and blood product transfusion were secondary outcomes. The effect of the interventions on the outcomes was investigated through using bivariate logistic regression, with a level of significance of 5%. The IG was 79% less likely to experience bleeding than the CG [odds ratio (OR) = 0.21, confidence interval (CI) 95% 0.12–0.39, P ConclusionsThese findings show that FAW can be used following CABG until patients reach normothermia to avoid undesirable clinical outcomes.Trial registrationREBeC RBR-5t582g.

Objectives To summarize evidence in the literature on the predictors of insomnia in adults and to determine correspondences with diagnostic indicators of the NANDA-I diagnosis Insomnia. Methods An integrative review performed in Pubmed, Virtual Health Library and CINAHL. Forty-eight articles published in Portuguese, English or Spanish from 2011 to 2018 were included. An analysis of correspondence between the predictors and the NANDA-I related factors and associated conditions for Insomnia was performed. Results There was a correspondence of the predictors found in this review with NANDA-I related factors and associated conditions, except for grieving and frequent naps during the day. Smoking, caffeine intake, dysfunctional sleep beliefs, obesity and caregiver role strain are possible new related factors; chronic illness is a possible new associated condition and individuals going through changes in marital status, economically disadvantaged, female gender, increasing age and night shift worker are possible new at-risk populations. Conclusion The predictors of insomnia that had a correspondence with the NANDA-I elements can support the evidence base of the nursing diagnosis. The predictors found without a correspondence with the diagnosis can be considered for inclusion in the NANDA-I classification, thereby supporting the clinical reasoning of nurses and students.

RESUMO Objetivos: analisar o conceito de risco cardiovascular para fundamentar a prática de enfermagem. Métodos: trata-se de uma análise para definição do conceito de risco cardiovascular, por meio da utilização de oito etapas do referencial de Walker e Avant, utilizando uma revisão da literatura em periódicos científicos indexados. Resultados: define-se o risco cardiovascular de forma ampla e original como contexto de saúde e de cuidado de enfermagem que permite identificar fatores de risco para doenças cardiovasculares modificáveis (cardiometabólicos, comportamentais, psicossociais, culturais e laborais) e não modificáveis (biológicos), que atuam como marcadores precoces e inter-relacionados, de etiologia múltipla e heterogênea, predispondo à vulnerabilidade cardiovascular. Conclusões: com a análise e definição do conceito de risco cardiovascular, percebemos que será possível fundamentar a prática de enfermagem, com implicações na prática clínica para identificação e redução dos fatores de risco para doenças cardiovasculares, com protagonismo da enfermagem no cuidado desses sujeitos.

Objectives: to analyze the concept of cardiovascular risk to support nursing practice. Methods: this is an analysis to define the concept of cardiovascular risk, through the use of eight steps of Walker’s and Avant’s framework, using a literature review in indexed scientific journals. Results: cardiovascular risk is defined in a broad and original way as a context of health and nursing care that makes it possible to identify modifiable (cardiometabolic, behavioral, psychosocial, cultural and occupational) and non-modifiable (biological) risk factors for cardiovascular diseases that act as early and interrelated markers, of multiple and heterogeneous etiology, predisposing to cardiovascular vulnerability. Conclusions: with the analysis and definition of the concept of cardiovascular risk, we realized that it will be possible to base nursing practice, with implications in clinical practice for identifying and reducing risk factors for cardiovascular diseases, with nursing relevance in the care of these subjects. RESUMEN Objetivos: analizar el concepto de riesgo cardiovascular para apoyar la práctica de enfermería. Métodos: se trata de un análisis para definir el concepto de riesgo cardiovascular, mediante el uso de los ocho pasos del marco Walker y Avant, utilizando una revisión bibliográfica en revistas científicas indexadas. Resultados: el riesgo cardiovascular se define de manera amplia y original como un contexto de salud y cuidado de enfermería que permite identificar factores de riesgo modificables (cardiometabólicos, conductuales, psicosociales, culturales y laborales) y no modificables (biológicos) de enfermedades cardiovasculares, que actúan como marcadores tempranos e interrelacionados, de etiología múltiple y heterogénea, que predisponen a la vulnerabilidad cardiovascular. Conclusiones: con el análisis y definición del concepto de riesgo cardiovascular, percibimos que será posible fundamentar la práctica de enfermería, con implicaciones en la práctica clínica para la identificación y reducción de los factores de riesgo de enfermedades cardiovasculares, con protagonismo de enfermería en el cuidado de estas asignaturas. Métodos: trata-se de uma análise para definição do conceito de risco cardiovascular, por meio da utilização de oito etapas do referencial de Walker e Avant, utilizando uma revisão da literatura em periódicos científicos indexados. Resultados: define-se o risco cardiovascular de forma ampla e original como contexto de saúde e de cuidado de enfermagem que permite identificar fatores de risco para doenças cardiovasculares modificáveis (cardiometabólicos, comportamentais, psicossociais, culturais e laborais) e não modificáveis (biológicos), que atuam como marcadores precoces e inter-relacionados, de etiologia múltipla e heterogênea, predispondo à vulnerabilidade cardiovascular. Resumo Objetivo Investigar as evidências científicas sobre as úlceras de pernas como sequela da hanseníase. Métodos Revisão Integrativa da Literatura (RIL). Para identificação do tema e questão de pesquisa, utilizou-se a estratégia PICo, na sequencia procedeu-se a busca nas bases de dados: Index to Nursing and Allied Health Literature (Cinahl), Literatura Latino-americana e do Caribe em Ciências da Saúde (Lilacs), Banco de dados em Enfermagem (BDENF), EMBASE (Elsevier), PUBMED (National Library of Medicine), SCOPUS (Elsevier), Web of Science (Clarivate Analytics), selecionados os descritores específicos para cada uma delas e elaboradas as respectivas estratégias de busca. Os idiomas considerados foram: inglês, português e espanhol. O período da RIL foi indeterminado. Para a seleção dos estudos, utilizou-se a ferramenta Ryyan que permitiu dois dos autores independentemente incluir, excluir, ou ficar indeciso, as discordâncias foram solucionadas por um terceiro. Os artigos selecionados foram classificados de acordo com a Agency for Healthcare Research and Quality (AHRQ). E para a interpretação dos resultados os artigos foram agrupados por semelhança e categorizados da seguinte maneira: autor, ano país, nível de evidência, objetivos e resultados, conclusão e recomendação. Resultados Identificou-se 415 estudos e 10 foram incluídos na revisão. O resultado principal obtido foi o tratamento tópico da úlcera, as vulnerabilidades e os determinantes que assolam os acometidos pela hanseníase não foram estudados. Conclusão Não há fortes evidências sobre as terapias para o tratamento de úlceras de pernas decorrentes da hanseníase que apoiem a diminuição de sequelas incapacitantes ocasionadas pela doença perpetuando o estigma e a desigualdade social.

to evaluate the effect of the Self-Instructional Guide for Clinical Reasoning on the diagnostic accuracy of undergraduate Nursing students.a randomized, parallel and double-blind (researchers and outcome evaluators) clinical trial, carried out with undergraduate Nursing students. Validated case studies were applied in two phases to identify the patient's Nursing diagnosis/problem, etiology and clues, using the Guide with the intervention group in the second phase. The outcomes - diagnostic and etiological accuracy and number of clues - were evaluated using validated rubrics. Descriptive statistics were used to analyze demographic data; Fisher's exact test for similarities in prior education and confidence; Mann-Whitney's test for age; and non-parametric ANOVA test in the evaluation of the hypothesis of differences in performance.final sample composed of 24 students in the control group and 27 in the intervention group; no difference as to gender, age and schooling. There was a difference in diagnostic (p=0.041) and etiological (p=0.0351) accuracy in the intervention group, showing a negative effect of using the Guide.the one-time self-instruction was not effective in impacting the diagnostic accuracy of students solving case studies. Repeated application of the Guide as a teaching tool can be effective in improving such outcome. ABSTRACT Objectives: to develop a middle-range nursing theory for care in the context of cardiovascular risk. Methods: a theoretical development study, through induction through research and ICNP® practice standards, carried out in six stages: concept analysis; ICNP® terminological subset structuring; theory contextualization and purpose; proposition construction; modeling; and assumption construction. Results: the Theory of Care in the Context of Cardiovascular Risk has a middle-range scope, describing care, prescribing actions to promote health and reduce cardiovascular risk. Thirteen propositions were constructed in three categories (nursing metaparadigm, central and factorial concepts), two models and 16 assumptions. Final Considerations: the theory contributes to the construction of knowledge arising from the nursing process for care in the context of cardiovascular risk, generating propositions, assumptions and modeling, which will enable theoretical testing.

Objective: to describe faculty member training as a strategy for the development of leaders and the promotion of continuous academic progress in teaching, research, and extension. Method: this reflection article presents a history of the highest academic degree obtained in Brazil, describing the characteristics of the competitions and highlighting aspects and activities that show, nowadays, intellectual and scientific maturation. Results: aspiring professors submit their academic trajectory to analysis and judgment by peers, based on strict assessment criteria, to receive a professor habilitation. Its purpose is to promote a moment of analysis of their academic career and to identify the remarkable contribution to the advancement of the university and the country through consolidated and impactful activities. Final considerations: pursuing this highest degree in the Brazilian academic career can be a moment of individual and collective advancement, contributing to the consolidation of areas of knowledge. RESUMEN Objetivo: describir la formación de profesores universitarios como una estrategia para el desarrollo de líderes y la promoción del progreso académico continuo en la docencia, investigación y extensión. Método: este artículo de reflexión presenta una historia del más alto grado académico obtenido en Brasil, describiendo características de las competencias y destacando aspectos y actividades que muestran, hoy, maduración intelectual y científica. Resultados: los aspirantes a habilitación a la enseñanza someten su trayectoria académica al análisis y juicio de pares, con base en estrictos criterios de evaluación. Su propósito es promover un momento de análisis de la carrera académica e identificar el aporte notable al avance de la universidad y del país, a través de actividades consolidadas e impactantes. Consideraciones finales: la búsqueda de este grado más alto en la carrera académica brasileña puede ser un momento de avance individual y colectivo, contribuyendo a la consolidación de áreas de conocimiento. RESUMO Objetivo: descrever a formação do Livre-Docente como estratégia para o desenvolvimento de lideranças e a promoção de contínuo avanço acadêmico no ensino, na pesquisa e na extensão. Método: este artigo de reflexão apresenta histórico da titulação acadêmica mais alta obtida no Brasil, descrevendo características dos concursos e destacando aspectos e atividades que evidenciam, nos dias de hoje, amadurecimento intelectual e científico. Resultados: professores aspirantes à Livre-Docência submetem sua trajetória acadêmica à análise e ao julgamento realizado por pares, com base em rigorosos critérios de avaliação. Tem por finalidade promover um momento de análise da carreira acadêmica e identificar a contribuição marcante para o avanço da universidade e do país, por meio de atividades consolidadas e de impacto. Considerações finais: a busca por esta mais alta titulação da carreira acadêmica brasileira pode constituir momento de avanço individual e coletivo, contribuindo para a consolidação de áreas de conhecimento.

The objective of this study was to develop and validate an educational video on nasopharyngeal and oropharyngeal suctioning.The use of videos in nursing education can improve students' skills in performing procedures.This was a methodological study.This study was performed in five steps: (1) development of the script for an educational video on nasopharyngeal and oropharyngeal suctioning; (2) content validation of the script by 10 nurse specialists; (3) development of the video; (4) content validation of the video by six nurse specialists; (5) cognitive testing by 51 nursing students regarding the understanding of the items, until the following requirements were met: (1) mean and median scores ≥4, with significant inter-rater agreement, according to the Wilcoxon test; (2) 95% confidence intervals80 for the proportion of maximum scores, according to the binomial distribution. p values0.05 were considered significant.After four rounds of evaluation by the specialists, the script was considered validated. The video was considered validated after two rounds of evaluation by specialists and students (p 0.001). The video addressed the following topics: concept, indications, contraindications, required materials, appropriate technique, nursing notes and complications.The video script was created by using the Storyboard technique and validated by specialist nurses using the Delphi technique. Nursing students watched, analyzed and understood the video which may support them to improve their technical skills of this procedure.

Objective: To identify the need of family members of patients hospitalized in a coronary intensive care unit and their degree of satisfaction with the care provided. Methods: An observational and cross-sectional study including family members of patients hospitalized in the coronary intensive care unit for acute coronary syndrome in Killip I or II. After the second visit of the same family member to the patient in the unit, a 43-item inventory of needs and stressors of family members was applied. Family members assessed each need for its importance and satisfaction using a four-point Likert scale. METHODS: an experience report of the joint activity of the Brazilian Nursing Process Research Network (Rede de Pesquisa em Processo de Enfermagem) composed of Higher Education and Health Institution researchers in Brazil. RESULTS: five instruments were organized collectively, involving the elements of nursing practice (nursing diagnoses, outcomes and interventions) in assistance for community; for patients (with suspected or mild, moderate, and critical COVID-19 and residents in Nursing Homes); for nursing workers' health support, also subsidizing registration and documentation during the COVID-19 pandemic. FINAL CONSIDERATIONS: valuing the phenomena manifested by families/communities, patients and health professionals is essential for early detection, intervention, and prevention of diseases.
