Background: Coronavirus disease 2019 (COVID-19) may lead to immunosuppression, leaving patients vulnerable to secondary invasive fungal infection like mucormycosis. The present study aimed to determine whether there are any risk factors associated with mortality in mucormycosis among COVID-19 patients. Patients and Methods: Patients with COVID-19 diagnosed with mucormycosis who received treatment at University Hospitals were included in the study. Complete blood count (CBC), glycated hemoglobin (HBA1c), C-reactive protein (CRP), serum albumin level, creatinine, ferritin levels, lactate dehydrogenase (LDH), D-dimer and histopathological observations were performed for all participants’ specimens. Results: The number (N) of patients included in the study was 46. About 85 % (39/46) of patients had post-COVID-19 syndrome and the other 7 cases were in the active phase of the disease. The subsurface structural geometries of the United Arab Emirates (UAE) fold-and-thrust belt (FTB) and foreland basins are interpreted from seismic, well data, and gravity and magnetic data, integrated with surface geology. We also determined the basement depths, as well as the relation between shallow sedimentary structures and deep basement features. Miocene to Lower Jurassic sequences were interpreted and mapped. Additionally, we outlined subsurface extent of Sumeini and Hawasina allochthonous nappes. The tectonic subsidence curves suggest that the final major passive margin rifting event occurred in the early Aalenian and lasted till Oxfordian. Loading of the Semail ophiolite thrust sheet and accompanying allochthonous thrust sheets resulted in uplift at ca. 95 Ma and rapid subsidence at ca. 83 Ma, indicating the transition of the Arabian margin from a rifted passive margin to a foreland basin. The region witnessed an accelerated subsidence during the late Oligocene-Miocene, attributed to the initial collision of the Central Iran and Arabian plates. The Permian-Jurassic NW-SE oriented rift faults were reactivated as thrust faults during the Late Cretaceous ophiolite obduction and late Oligocene-Miocene continental collision. Two different tectonic regimes are identified in the FTB. Based on the seismic reflection profiles and derivative maps of gravity and magnetic data, the northern regime has NNW–SSE striking thrusts, backthrusts and folds. It is characterized by major inversion of the rift faults with up to 3,700 m thrusting throw. Four major west-verging and east-dipping thrusts, which cross the northern area, form fault-propagation folds and dissect the entire stratigraphy. Whereas the southern regime around Jabal Hafit is dominated by the Tarabat backthrust, which cuts across the Upper Cretaceous and Cenozoic successions. Moreover, the Mesozoic platform carbonates are dissected by inverted faults (pop-up structure) in this regime. The Hawasina décollement, together with the inverted basement structures formed the Jabal Hafit anticline as a backthrust structure. On the other hand, results of the 3D gravity inversion indicate that the basement depths range from 11.5 km along uplifted areas of the FTB to 18.8 km within the deeper parts of the foreland basin. Significant northward increases in both basement depth and the thickness of the late Oligocene-early Miocene successions indicate a northward increase in the severity of thrusting and crustal loading along the main Khusub thrust. The basement uplifts are found to be correlated with shallower anticlinal features, indicating a direct relationship between basement uplifts and shallow sedimentary structures and hence indicating reactivation of basement structures during the Zagros collision.

Background: There are 170 million people in the globe living with hepatitis C virus (HCV) infection, which causes inflammation and hepatic fibrosis in various degrees. A portion of these patients will develop cirrhosis and other end-stage liver disease problems over the course of twenty to forty years. Low levels of serum ceramide 24 (Cer24) are linked to severe liver fibrosis and poor response to antiviral therapy in those with chronic HCV infection. Objective: This study was designed to assess level of ceramide 24 in chronic HCV and normal individuals. Subjects and Methods: The study was carried out as a case control study at Tropical Medicine Department, Clinical Pathology Department in Zagazig University Hospitals and at Viral Hepatitis Treatment Unit in Al-Ahrar Teaching Hospital. The study included 60 individuals who were divided into 2 groups. Group 1 included 30 case and group 2 was 30 healthy subjects as control. All patients were clinically evaluated, had routine laboratory investigations and measurements of circulating levels of ceramide 24. Abdominal ultrasonography was done. Results: Serum ceramide 24 level in HCV patients (cases) is 15.16 ± 6.93 while its level in normal individuals (control) is 65.01 ± 65.84. Conclusion: It was found that serum ceramide 24 level was significantly reduced in case group.

Microangiopathy is implicated in the pathogenesis of diabetic vascular complications. Nail fold videocapillaroscopy (NVC) is an easy non-invasive tool of microvasculature assessment. This study compares the NVC changes in adolescents with Type1 diabetes (T1D) to healthy controls and correlates them to diabetic vascular complications.Hundred thirty-five adolescents with T1D (disease duration 5 years) were compared to 135 matched controls. Diabetes duration, insulin therapy, fundus, and Toronto clinical scoring system (TCSS) were assessed. Fasting lipids, fraction-C of glycosylated hemoglobin (HbA1C), urinary albumin creatinine ratio (UACR), nerve conduction velocity, and NVC were performed.NVC changes were found in 120 adolescents with T1D (88.8%). These changes were significantly higher in adolescents with T1D than controls (p .001). Significant positive relation was found between NVC changes and TCSS (p = .006), diabetes duration (p = .001), HbA1C (0.008), cholesterol (p = .011), LDL (0.016), UACR (p .001), and nerve conduction velocity (p .001). Multivariate logistic regression study revealed that diabetic nephropathy and neuropathy were independently associated with NVC changes (p .001 and p = .007, respectively).Adolescents with T1D have significantly higher NVC changes than controls. These changes were more evident in those having vascular complications than those without. Thus, NVC can be a potential non-invasive tool for early assessment and follow-up of the microvasculature among adolescents with T1D.

The seismic stratigraphy and sedimentary facies of the Cretaceous and Paleogene sedimentary successions in north-eastern offshore part of Taranaki Basin, New Zealand have been investigated in order to unravel their depositional evolution and identify the potential hydrocarbon plays. Interpretation of regional seismic lines covering the entire shelf-slope and deepwater regions as well as integrating seismic and sedimentary facies allows the identification of several seismic-stratigraphic sequences within the studied successions. Early Cretaceous syn-rift successions (C1 sequence) were deposited in the structural lows near the present-day slope as swamp and marsh facies changing basinwards into turbidites and marine shales. The post-rift Cretaceous sequences (C2, C3 sequences) started with the progradation of the Taranaki delta (C2A-C2D units) followed by sedimentation of the transgressive C3A-E facies accumulated in response to thermal subsidence and high-rates of clastic supply. Extensive shelf conditions prevailed during the deposition of C2 and C3 Cretaceous sequences continued during the Paleogene with deposition of uniform marine shales throughout the basin. Rates of sediment supply substantially decreased during Oligocene prompting the deposition of marine micrite-rich carbonate. Variation in fossil content confirms the occurrence of several cycles of sea level fluctuations and episodic variations in terrigenous input. Organic-rich facies could be associated with the prograding clinoforms of the C2B unit and probably contain, basinward, large amount of gas and oil prone kerogen. The transgressive facies of C2D unit may also contain organic-rich shales basinward and coal near the present day Taranaki shelf. Potential reservoirs are hosted on the C3 transgressive estuarine sandstones. Paleogene mudstones are excellent regional seals for the hydrocarbons generated and trapped in the underlying Late Cretaceous facies.

Lactoferrin may play a role in the treatment of chronic tonsillitis due to its direct antimicrobial activity.Objective: To assess the possible role of lactoferrin in reduction of raised serum Anti-Streptolysin O Titer (ASOT) in cases of chronic tonsillopharyngitis in comparison to long acting penicillin.Methods: This study included 117 children with tonsillopharyngitis with high ASOT randomly divided into three groups; group 1 treated with lactoferrin, group 2 treated with long acting penicillin and group 3 treated with both drugs. For all patients ASOT was measured after three and six months of starting treatment.Results: This study included 60 males and 57 females with the mean age (8.5 ± 2.4). There is statistically significant reduction in ASOT in all groups after three months of treatment. ASOT after 3 months was significantly lower in group1 (370± 440) and group 3 (350± 450) in comparison to group 2 (420± 560) with p value 0.02, 0.004, respectively, with no significant difference in comparing group 1 to group 3 p value 0.4. Also, ASO titre after 6 months was significantly lower in group1 (350± 420) and group 3 (340± 440) in comparison to group 2 (420± 550) with p value 0.02, 0.007, respectively, with no significant difference in comparing group 1 to group 3 p value 0.5. In comparing ASOT at three months and six months of treatment in the three studied groups; it decreased by 2% in group 1, and 1.6% in group 3 and no change in group 2.Conclusion: Lactoferrin alone or in combination with long acting penicillin is safe and more effective than long acting penicillin alone in reducing ASOT. In late 2019, a novel coronavirus, now designated SARS-CoV-2, emerged and was identified as the cause of an outbreak of acute respiratory illness in Wuhan, a city in China, named as COVID-19. Since then the waves of the virus exponentially hit many countries around the globe with high rates of spread associated with variable degrees of morbidity and mortality. The WHO announced the pandemic state of the infection in March 2020 and by June 1st 2020 more than 6 million individuals and more than 370 thousands case fatalities were documented worldwide. In this article, we discussed many aspects regarding this emerged infection based on the available evidence aiming to help clinician to improve not only their knowledge but also their practices toward this infection.

International audience; Several end-to-end encryption technologies for emails such as PGP and S/MIME exist since decades. However, end-to-end encryption is barely applied. To understand why users hesitate to secure their email communication and which usability issues they face with PGP, S/MIME as well as with pEp (Pretty Easy Privacy), a fairly new technology, we conducted an online survey and user testing. We found that more than 60% of e-mail users are unaware of the existence of such encryption technologies and never tried to use one. We observed that above all, users are overwhelmed with the management of public keys and struggle with the setup of encryption technology in their mail software. Even though users struggle to put email encryption into practice, we experienced roughly the same number of users being aware of the importance of email encryption. Particularly, we found that users are very concerned about identity theft, as 78% want to make sure that no other person is able to write email in their name.

Two main reservoirs are producing in Komombo Basin: the first one belongs to the C Member of the Six Hills Formation, and the second belongs to the Albian/Cenomanian cycle. The C Member reservoir lacks detailed studies. Therefore, a detailed study of this reservoir is needed. 3D geological reservoir modeling of the C Member reservoir can be a pertinent part of an overall strategy for the development of hydrocarbon fields in Komombo Basin. Five boreholes, three vertical seismic profiles and twenty 2D seismic reflection sections are integrated in Petrel™ modeling software for building 3D structural, facies and petrophysical models for the C Member reservoir. The constructed 3D structural model reveals the presence of two normal faults, in NW–SE and NE–SW directions. A detailed petrophysical evaluation was performed for the available wells. The resulted facies/petrophysical parameters are then used as input in the processes of facies and petrophysical modeling. The C Member reservoir exhibits thickness values ranging from about 91.5 to 426.5 m. The constructed 3D facies model of the studied reservoir depicts that the shale beds have the large probability distribution in the study area with the comparison of the sandstone and siltstone beds. The created 3D petrophysical models reveal that the C Member reservoir has a fair reservoir quality. This reservoir exhibits, generally, high water saturation values in most parts of the study area, while the hydrocarbon saturation is restricted to the depocenter of the basin.

The Albian/Cenomanian reservoir is one of the two main reservoirs composing the petroleum system in the Komombo Basin. However, these reservoirs have not previously been studied in detail. Therefore, a detailed study of the reservoirs is performed. Six boreholes, three vertical seismic profiles and twenty 2D seismic lines are integrated in the Petrel™ Schlumberger modeling software to build a 3D structural model for the Albian/Cenomanian reservoir. This model represents a framework for the subsequent 3D facies and petrophysical models of the studied reservoir. The interpretation of the created maps and 3D models shows two normal fault directions: NW–SE and NE–SW, where the NW–SE direction is dominant, creating a major half-graben system. Conventional well log analyses are performed for the available wells. The results are used in subsequent facies and petrophysical modeling. The thickness of the Albian/Cenomanian reservoir ranges from about 137 to 411.5 m. Petrographic analyses were conducted on two cores retrieved from the Albian/Cenomanian cycles covering the depth interval of 1323–1352 m in well AB-5. Most of the samples consist of quartz arenite and feldspathic arenite. The 3D facies model of the reservoir shows that the sandstone has a large probability distribution in the study area compared to the shale and siltstone. The 3D petrophysical models imply that the Albian/Cenomanian reservoir has good to very good reservoir quality. The main issue for the reservoir is the high water saturation values. In contrast, the hydrocarbon saturation is restricted to some parts of the basin.

Background and study aim: Baveno VI consensus recommended the use of non- invasive predictors of EV to avoid unnecessary endoscopies. Von Willbrand factor (VWF) and VITRO score, (VWF/ platelet count), are both correlated to liver cirrhosis and fibrosis. The aim of this study was to evaluate the role of VWF and VITRO score as predictors of esophageal varices and their bleeding. Patients and Methods: Seventy seven patients were included in this study. They were randomly selected from cirrhotic patients admitted to endoscopy unit for the first time. They were allocated into two groups;group I:patients who have esophageal varices,group II:patients with no esophageal varices. Abu Gharadig Field lies in the central portion of Abu Gharadig Basin of the northern part of the Western Desert of Egypt, about 256 km west of Cairo. Abu Gharadig Basin is considered as an E-W trending intracratonic basin . The Upper Bahariya Member is a clayey reservoir and represents one of the Late Cretaceous units. There is a need for identifying whether the petrophysical parameters of this member are related to the facies changes or the present structures. Therefore, detailed structural and petrophysical studies are required. The structural configuration of the Upper Bahariya Member is studied in the present case and the results of this study will form the basis for future petrophysical studies. The present study includes the delineation of the main structural elements of the studied member within the study area. The aim of the present study was achieved through building a 3D structural model using Petrel™ Schlumberger Modeling software by integrating twenty 2D seismic sections with four digital well logs and a check-shot survey. The interpretation of the resulted model shows that the Upper Bahariya Member was folded into doubly plunging anticline with a gentle plunge to SW and a relatively steep plunge to NE. The fold was dissected by a series of normal faults trending E-W to NW-SE. The anticline is asymmetric and exhibits sigmoidal shape. The axis has mainly a NE-SW orientation and exhibits changing of this orientation in the NE part to be ENE-WSW and in the SW part to be also ENE-WSW, relating to the effect of a rotational stress. Growth faulting and thinning of the studied member at the crest of the anticline suggest the syn-sedimentary tectonics. Thus, these structures are related to the Late Cretaceous right-lateral shear which led to the opening of the North Atlantic.

The Upper Bahariya Member represents an argillaceous sandstone reservoir and it is one the Late Cretaceous units. There is a need for understanding the detailed facies and petrophysics of this member and identifying whether its petrophysical parameters are related to the facies changes or the present structures. Therefore, the present study aims at determining the facies and petrophysical characteristics of the Upper Bahariya Member and their distribution in the Abu Gharadig field. This field is located in the central portion of Abu Gharadig basin in the northern part of the Western Desert of Egypt. The available data for the present study includes four digital well logs, twenty 2D seismic sections and a checkshot survey. This is achieved through building the 3D facies and petrophysical models using Petrel™ Schlumberger Modeling software. These models were built on a predefined 3D stratigraphic and structural framework. The interpreted lithology of the Upper Bahariya Member is represented by sandstone cemented by argillaceous and/or calcareous materials, siltstone , shale and minor limestone inter-beds. A new lithofacies subdivision for the Upper Bahariya is introduced in the present study through the study area. This subdivision is represented by three lithofacies (an upper shale-silt-carbonate lithofacies, a middle sand-silt lithofacies and a lower shale-silt lithofacies). The Upper Bahariya Member is evaluated as a good quality reservoir and it exhibits rapid variations in the conditions of deposition, from continental fluvial to marine passing through the tidal flood plain conditions. The sandstone source is mainly from the southeast and southwest directions. The vertical and lateral changes of the different petrophysical parameters are directly related to vertical and lateral facies changes.

Despite our understanding of the different mechanisms of man-made earthquakes, their short-term prediction and prevention is yet to be attained. In the present study, we propose an integrated four-step approach to predict and prevent man-made earthquakes or reduce their chance of occurrence. Our four-step approach includes locating the highly anomalous zones of microseismic emission (MSE) that result from the stress-deformed state inside a geological formation and often represents the “seismic nuclei” for impending earthquakes, monitoring the variations and dynamics of the anomalous MSE zones over a period of one lunar month, inducing a creep-discharging of the MSE zones using a vibroseis seismic source at the ground surface, and monitoring the same MSE zones following the creep-discharge to determine whether the stress-deformed state was released and the chance of potential earthquake occurrence has been eliminated or reduced. The proposed full four-step approach has never implemented at one single location. Nevertheless, these steps have been tested separately at different sites and have proven successful. We propose conducting the full four-step approach at various locations of potential man-made earthquake activities around the world including the state of Oklahoma in the USA.

The Lower Cenomanian siliciclastic Bahariya Formation represents one of the main producing reservoirs in the north Western Egyptian Desert. A few previous subsurface studies exist that integrated core, petrographic, scanning electron microscope (SEM), and X-ray diffraction (XRD) analyses to study the reservoir quality of this formation. Consequently, the present study is carried out to fill this gap and outline the sedimentological, petrophysical, and digenetic characteristics of this siliciclastic reservoir in addition to the impact of these characteristics on reservoir quality. Detailed microscopic examination, XRD, SEM, and conventional core analyses were applied on the available cores. Five distinct microfacies types were petrographically identified, belonging to the arenite and wacke facies . These microfacies are represented by quartz arenite, subarkose, argillaceous quartz wacke, calcareous quartz wacke, and calcareous feldspathic greywacke . The formation was subjected to compaction and was affected by four types of cement; silica, calcite , kaolinite , and pyrite cementation . It has three types of porosities: primary intergranular and secondary porosity (microporosity and dissolution). The studied core samples of the Bahariya Formation are discriminated into two reservoir rock types (RRTs) and exhibit extreme heterogeneity. RRT1 comprises mainly quartz arenite, whereas RRT2 comprises quartz wacke. RRT1 has a reservoir quality of tight to poor due to porosity/permeability occluding by the different cementations. In contrast, RRT2 has a reservoir quality of tight to fair, despite the presence of the clay matrix. The tight to fair quality is possibly attributed to the dissolution of feldspars and authigenic clays. The Lower Cenomanian reservoir is considered a second cycle deposition and represents a tight to a fair quality reservoir in the studied wells.

Since remote cerebellar hemorrhage, with intracerebral hemorrhage after supratentorial neurosurgery, is rare, its pathophysiology remains elusive. Here, we report a 64-year-old man who had severe bilateral symmetric remote cerebellar hemorrhage with frontal lobe hemorrhage following burr-hole evacuation for supratentorial chronic subdural hematoma. Computed tomography venography showed undeveloped left internal jugular vein and sigmoid sinus. He received 3 weeks of conservative treatment and fully recovered. Overdrainage of cerebrospinal fluid and head rotation with undeveloped internal jugular vein may have resulted in this complication. This case is the first case in the literature with this event sequence and has some significance for revealing the mechanism of remote cerebellar hemorrhage occurrence after other supratentorial surgeries.

Background: Burn is a serious health problem globally. Every year more than 300, 000 people die from fires only and millions of people are disabled and disfigured by severe burns. Survivors of burns often lead to a life complicated not only by the physical consequences of the burns, but also by stigma and discrimination related to the disability and disfigurement. The aim of the study was to assess the predisposing factors for joint contractures among patients with burn. The research question: what are the predisposing factors for joint contractures among patient with burn? Research design: A descriptive/exploratory research design was utilized to achieve the purpose of this study. Subjects: A convenience sample of 176 adult male and female burn patients diagnosed post burn contracture as a result of 2nd and / or 3rd degree burn injury admitted to the surgical wards and burn unit at one of the governmental university hospitals was recruited in the current study as the burn cases admission increase increased in the following three years (2010, 2011, and 2012) as (405, 543, 487) respectively. The study sample calculated using a power analysis Tool: Burn assessment data sheet consists of two parts: 1- Demographic data sheet and 2- Predisposing factor assessment sheet through direct interview with patients. Results and conclusion: The study results concluded that the highest percent of the predisposing factors for joint contractures among patients with burn injury are (wound not healed on discharge, presence of signs of infection, no physical exercise after discharge, male, accidental burn injury, hospital not near, Rt.arm as a region of burn joint involved and no skin plastic surgery). Middle percent of them are (no health teaching about type of food, Lt. arm region of burn involved, home as a place of burn, married, rural area, limited movement for the affected joint, no application of joint support, educational level as diplome and no health teaching about correct joint positioning) followed by the lowest percent which are (2nd and 3rd degree burn injury, 20- as a percentage of burn injury, refuse to do physical exercise, no movement of the affected joint, Age (18˂ 30), flame burn and late starting of physical exercise. Recommendations: Developing a comprehensive treatment and rehabilitation program for those patients. All burn patients should receive adequate care to avoid post burn complication.

The area of study is located at Wadi El-Nakheil to the west of Quseir district along the Egyptian Red Sea Coast. The present work is a case study of applying the 2D homogeneous function method. This function automatically inverts the first arrival refractions to derive a 2D velocity distribution involving the seismic boundaries. The present study aims at investigating the near-surface geological conditions of Wadi El-Nakheil. These conditions include the following: the different stratigraphic units, structural configuration, and the groundwater occurrence. Authors achieved this aim by carrying out shallow seismic refraction measurements along three profiles using direct and reverse shooting. A newly developed field methodology is used in the data acquisition. The interpretation of the seismic data is performed using GODOGRAF software which represents a 2D refraction tomographic inversion algorithm. The interpretation revealed the presence of seven geo-seismic zones, overlain by low-velocity surface layer, with a velocity range of less than 800 to 6800 m/s. The detected zones are dissected by several normal faults with NE and SW dipping directions and maximum throw of about 20 m. An angular unconformity is noticed between the overlying horizontal layers and the dipping (towards SW) underlying layers. One of the interpreted zones is identified as the water-bearing zone where the velocity is between 2200–2400 and 2600 m/s. This was emphasized by the correlation with the nearest borehole in the area. The water-bearing zone is interpreted as the Nakheil Formation. The thickness of the water-bearing zone ranges from 20 to 55 m, while its depth ranges from 35 to 65 m.

The present study aims at characterizing the sedimentological and petrophysical properties of the Middle-Late Miocene Mt Messenger reservoir in Taranaki Basin, New Zealand. It is a detailed study based on availability of sedimentological (petrography, SEM and XRD), well log data (gamma ray, neutron, density, sonic, shallow and deep resistivity, PEF and RFT logs), in addition to conventional and special core analysis data for Mt Messenger reservoir. The Mt Messenger Formation is considered onshore fan units of low resistivity clastic rocks with high percentage of metamorphic lithic fragments. From the RFT logs, it is composed of two or three compartments partially separated from each other by some tight zones. The reservoir zones are represented by thin beds of well sorted very fine to silty-grained litharenite; sometimes is feldspathic, argillaceous and/or micaceous with very good intergranular porosity of average reaches to 25%. Petrographically and petrophysically four reservoir rock types (RRTs) or hydraulic flow units (HFUs) can be assigned, each with its diagnostic petrophysical properties and storage and flow capacities. The reservoir quality increases to the south at Ngatoro-2 well, where a total of gas- and oil-bearing 5 prospective thin beds are recognized, which are mostly composed of litharenite with very good porosity (21–27.8%) and permeability (63–368 md). Its reservoir quality index (RQI) and flow zone indicator (FZI) indicates fair to good reservoir quality (0.544 = RQI ≤ 1.151 μm, and 1.972 = FZI ≤ 3.049 μm). Presence of argillaceous and/or micaceous materials in addition to the silt-sized grains decreased the reservoir quality from RRT1 samples which are characterized by fair to good reservoir properties (homogeneous pore size distribution, 0.18 = R35 ≤ 1.32 μm) to the RRT4 samples which are considered slightly tight reservoir (slightly heterogeneous pore size distribution, 4.44 = R35 ≤ 9.97 μm).

Background and study aim: Liver Cirrhosis is a strong and a common known risk factor for Cholelithiasis. Cholelithiasis is a multifactorial disease, based on a complex interaction of environmental and genetic factors. The primary aim of this study is to determine the frequency of cholelithiasis in chronic liver disease (CLD) patients admitted to Zagazig university hospitals. The secondary aim is to determine the risk factors and their association with the underlying etiology and severity of liver disease. Patients and Methods: We conducted a hospital based study including 131 patients with chronic liver disease based on clinical, laboratory and Ultrasonographic findings. Demographic, clinical and etiological data were recorded, using a pre-coded questionnaire. A number of laboratory tests as fasting plasma glucose, total cholesterol, triglyceride, aspartate aminotransferase (AST), alanine amino-transferase (ALT), alkaline phosphatase (ALP), hepatitis B surface antigen (HBsAg), and antibody to hepatitis C virus (HCV-Ab) were analyzed. Results: The number of registered cases was 131 with age (52.9±11.7).There were 55 (42%) males and 76 (58%) females. Hepatitis C (HCV) was present in 101 (77%) cases. The prevalence of cholelithiasis was 50.4%% (66 of 131 patients). Most of cholelithiasis patients presented with child C stage (68.2%), followed by child B (21.2%) and the least one was Child A. Hepatitis C (10.6%) was found to be associated with cholelithiasis (75.8%), followed by hepatitis B (13.6%). Auto-immune disease, diabetes mellitus, contra-ceptive pills and obesity are considered risk factors for cholelithiasis. Conclusion: Cholelithiasis tends to occur more frequently in patients with decompensated CLD. The higher incidence of cholelithiasis in CLD appears to be associated with HCV infection. This is an important parameter to be considered in a country with high prevalence of HCV as Egypt.

The Western Passive Continental Margin of the Indian Plate (WCMI) serves as an elegant laboratory for understanding the syn-sedimentary rift-drift tectonics preserving a variety of episodic structural deformations and complex sedimentary infill. Here we present a multidisciplinary study on Offshore Indus Basin (OIB) that is less investigated compared to adjacent marginal basins along WCMI. Reflection seismic, gravity, borehole, and biostratigraphic data have been used to reconstruct the tectono-sedimentary history of the Meso-Cenozoic succession in OIB. Seismic based dynamic stratigraphy helped in mapping seven different depositional sequences grounded on morpho-tectonic considerations. The seismic expression of these sequences provided an idea of the depositional styles with different tectonic episodes since rifting in Jurassic to basin inversion in Miocene continued to present time. The OIB initially filled from the north with the sediments rapidly prograding southwards. In general, the basin deepens in the central and northwestern part where the formations achieve maximum depths. The analysis of 3D structural models revealed that the region experienced poly-phase deformation from the Late Cretaceous to recent times that include steeply dipping normal faults, horst, and graben, half-graben, half-graben with growth faulting, flower structures, anticline, syncline, and uplift with an angular unconformity. The most remarkable tectonic activity happened at the Early Miocene and resulted in the reactivation of extensional faults into strike-slip faults giving rise to a spectacular structural inversion due to transform movement between Indian and Arabian Plates that continues to present day. The most predominant faults are oriented in S20°-50°E with azimuth as 120°–160° and the maximum dip angles are >80° in the southwest flank of the basin. The main fault trend is S20°E-N20°W and dip angles ranging between 40° and 80° and above. A thick crust and the Ocean-Continent Transition (OCT) is imaged with massive felsic, mafic, and ultramafic intrusions by the help of forward gravity modeling. An initial onset of syn-rift tectonic subsidence between 90 and 68 Ma with a post-rift general increase between 68 and 54 Ma is comprehended while two uplift events at ~40 Ma and ~18 Ma are documented through backstripping analysis. It is inferred that the region is rapidly under complex structural inversion with growing folding, faulting, and flowering structures. This study could help in explaining the unanswered reasons of structural complexities, depositional styles, and basin inversion along worldwide passive margins.

Twenty two-year-old house wife from Sharkeya, Egypt presented to the outpatient clinic of Tropical Medicine Department with fatigue and bilateral heaviness and swelling in her lower limbs as well as her left upper limb that started at the dorsum of her feet and progressed gradually to affect both limbs asymmetrically (the left lower limb was swollen more than the right) up to the knee and then appeared in the left upper limb starting from the hand and gradually progressing to reach level of the elbow. The patient had normal general examination.Examination of the affected limbs revealed normal color, temperature and hair distribution. No visible or dilated veins were noted. The examination revealed no ulceration or any skin lesions in the affected limbs. The oedema was partially pitting with spongy sensation (stage 2). The circumferences of the affected limbs were measured to observe the response to therapy. The oedema level was up to knee in both lower limbs and up to elbow in the upper limb (grade 1). The pulsation of dorsalis pedis, posterior tibial, popliteal, and femoral arteries in both lower limbs were intact as well as radial, ulnar, brachial and axillary arteries pulsations in the affected upper limb. There were no palpable inguinal or axillary lymph nodes. The patient was admitted to Tropical Medicine Department, Zagazig University Hospitals for evaluation of her condition. The patient received broad spectrum antibiotic and diuretic therapy and was advised to elevate her lower limbs and wear an elastic band over the affected upper limb. The previous measures gave minimal improvement of the swelling. The patient's routine laboratory investigations were normal. The patient performed Doppler evaluation for the venous systems in the affected limbs which was normal. The nocturnal peripheral blood film was negative for W. bancrofti microfilaria. The lymphiscintigraphy for lower limbs revealed patent lymphatics.
