Recommender systems are widely used to help people find items that are tailored to their interests. These interests are often influenced by social networks, making it important to use social network information effectively in recommender systems. This is especially true for demographic groups with interests that differ from the majority. This paper introduces STUDY, a Socially-aware Temporally caUsal Decoder recommender sYstem. STUDY introduces a new socially-aware recommender system architecture that is significantly more efficient to learn and train than existing methods. STUDY performs joint inference over socially connected groups in a single forward pass of a modified transformer decoder network. We demonstrate the benefits of STUDY in the recommendation of books for students who are dyslexic, or struggling readers. Dyslexic students often have difficulty engaging with reading material, making it critical to recommend books that are tailored to their interests. We worked with our non-profit partner Learning Ally to evaluate STUDY on a dataset of struggling readers. STUDY was able to generate recommendations that more accurately predicted student engagement, when compared with existing methods., Comment: 15 pages, 5 figures

3. Parameter Estimation of Proton Exchange Membrane Fuel Cells Using Chaotic Newton-Raphson-Based Optimizer

This paper presents a novel improved metaheuristic algorithm, Chaotic Newton-Raphson-Based Optimizer (CNRBO), for the Proton Exchange Membrane Fuel Cells (PEMFCs) parameter extraction problem. In this study, ten distinct chaotic maps are integrated within the Newton-Raphson-based optimizer (NRBO) to improve its performance. The chaotic maps are employed to define the probability of the trap avoidance operator (TAO). The proposed CNRBO algorithm is validated using standard benchmark optimization problems. Results proved its advantages over the standard NRBO algorithm in terms of accuracy, robustness, and convergence speed. For PEMFC parameters extraction using the proposed CNRBO algorithm, the objective function to be minimized is the sum of squared errors (SSE) between estimated and measured stack voltages across various data points. To validate the effectiveness and reliability of the proposed CNRBO, its performance is compared with recent algorithms on eight different PEMFC stack models including NedStackPS6, BCS 500W, Horizon 500W, 250W stack, Avista SR-12 500W, Temasek 1KW, Ballard Mark V 5KW and Horizon H-1000XP stack. Statistical measures are employed to assess the superiority and robustness of the proposed CNRBO. Statistical analyses demonstrate that the proposed CNRBO algorithm outperforms existing algorithms in terms of accuracy, search capability, and convergence speed, solidifying its position as a powerful tool for PEMFC parameter estimation.

4. Improved particle swarm optimization for fractional order PID control design in robotic manipulator system: A performance analysis

This research seeks to promote the field via the design and implementation optimized robotic manipulator control systems, recognizing control techniques' vital role in current engineering applications. This study introduces an improved particle swarm optimization (IPSO) technique that maximizes the efficiency of a Fractional Order Proportional-Integral-Derivative (FOPID) controller by optimally adjusting FOPID gains in robotic manipulator systems. The controller has undergone refinement and enhancement using state-of-the-art particle swarm optimization (PSO) techniques incorporating a cost function and a representative bio-inspired algorithm. The IPSO algorithm enhances global search efficiency by preventing premature convergence and local minima trapping through chaos-based initialization and adaptive mutation strategies. The performance of IPSO-tuned FOPID controllers is benchmarked against conventional PSO-tuned FOPID controllers using various objective functions. The stabilizing fractional order PID controllers demonstrated a higher stability margin than traditional PID controllers. Numerical simulations support the developed strategy by analyzing the step and sinusoidal responses of the closed-loop system within the stability region. The results indicate that IPSO outperforms PSO with improvements of approximately 50% for 10 iterations, about 12% for 50 iterations, and around 20% for 100 iterations across ITSE, ITAE, and ITAE metrics, respectively. Furthermore the statistical analysis based on Wilcoxon sign rank test proof that the IPSO algorithm significantly improves convergence speed, controller accuracy, and overall performance, thereby enhancing the effectiveness of the IPSO technique such as in case of 10 iteration the confidence intervals do not include zeros, which indicates that IPSO outperformed the traditional POS in all scenarios.

6. Comparative Analysis: Fractional PID vs. PID Controllers for Robotic Arm Using Genetic Algorithm Optimization

This paper presents a comparative analysis of a fractional-order proportional–integral–derivative (FO-PID) controller against the standard proportional–integral–derivative (PID) controller, applied to a nonlinear robotic arm manipulator systems. The genetic algorithm (GA) optimization method was implemented to tune the gain parameters of the FO-PID and PID controllers. The performance of the FO-PID and PID controllers were evaluated though different cost functions, including integral of squared error (ISE), integral of absolute error (IAE), integral of time-weighted absolute error (ITAE), and integral of time-weighted squared error (ITSE). The performance of these controllers was examined via extensive simulations by using MATLAB/SIMULINK for different operating scenarios of the robotic arm manipulator system. Based on the obtained results, a comparative performance matrix is proposed, wherein cost functions ISE, IAE, ITAE, and ITSE are represented as columns while characteristic parameters (overshoot, rising time, and settling time) are represented as rows. The proposed performance matrix facilitates the selection between the PID and FO-PID controllers.

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

The OMNeT++ simulator is well-suited for the simulation of randomized user behavior in communication networks. However, there are scenarios, where such a random model is unsuited to evaluate a communication system, and this paper attempts to highlight such a case. Using this example of ad-hoc communication between aircraft mid-flight, a tutorial-style description is attempted that shall show how the OMNeT++ simulator can be used when a wealth of real-world trace data is available. In particular, it is described how mobility trace files can be directly used within OMNeT++, and how to link the generation of data messages to this mobility data. This is explained via an example simulation that evaluates a communication network in which an aircraft notifies the ground control when it enters or leaves a specific geographic region. Additionally, a novel trace-based application has been developed to achieve this link between mobility and message generation. Furthermore, a new TDMA-based medium access protocol for decentralized communication networks is presented, which is oracle-based and thus allows a TDMA-like behavior of medium access without causing any overhead; it can be useful when upper-layer protocols should be evaluated under the assumption of TDMA-like behavior, but isolated from the effects of a full-fledged TDMA protocol. Finally, physical layer behavior is often either overly simplistic or overly computationally expensive. For the latter case, when a detailed channel model is available but its evaluation requires prohibitive computational effort, then averaging its behavior into trace data can find a middle ground between efficient evaluation and realistic representation. Hence, a novel trace-based radio model has been developed that makes use of an SNR to PER mapping. In this work we explore an auxiliary loss useful for reinforcement learning in environments where strong performing agents are required to be able to navigate a spatial environment. The auxiliary loss proposed is to minimize the classification error of a neural network classifier that predicts whether or not a pair of states sampled from the agents current episode trajectory are in order. The classifier takes as input a pair of states as well as the agent's memory. The motivation for this auxiliary loss is that there is a strong correlation with which of a pair of states is more recent in the agents episode trajectory and which of the two states is spatially closer to the agent. Our hypothesis is that learning features to answer this question encourages the agent to learn and internalize in memory representations of states that facilitate spatial reasoning. We tested this auxiliary loss on a navigation task in a gridworld and achieved 9.6% increase in accumulative episode reward compared to a strong baseline approach.

A common approach to solving physical reasoning tasks is to train a value learner on example tasks. A limitation of such an approach is that it requires learning about object dynamics solely from reward values assigned to the final state of a rollout of the environment. This study aims to address this limitation by augmenting the reward value with self-supervised signals about object dynamics. Specifically, we train the model to characterize the similarity of two environment rollouts, jointly with predicting the outcome of the reasoning task. This similarity can be defined as a distance measure between the trajectory of objects in the two rollouts, or learned directly from pixels using a contrastive formulation. The paper introduces a robust adaptive fault-tolerant control system for the six-degree-of-freedom (six-DOF) dynamics of quadrotor unmanned aerial vehicles (UAVs), incorporating disturbances and abrupt actuator faults to represent real-world conditions. The proposed control scheme employs robust control terms to manage unknown disturbances. However, robust control performance may degrade due to sudden fault impacts. To handle this issue, we introduce adaptive laws to ensure continuous adaptation. The control architecture ensures the tracking system’s stability by combining robust control using sliding-mode control (SMC) with adaptive control developed using the certainty equivalence principle. The sliding-surface error limits the adaptive laws, in which the convergence of estimated parameters to the actual unknown variables is not required as they fully rely on the convergence of the tracking error. We provide rigorous mathematics to validate the proposed control design. Furthermore, we conduct numerical simulations for a quadrotor UAV to showcase the effectiveness of the proposed scheme. The results demonstrate the efficacy of the proposed design in handling external disturbances and abrupt actuator faults.

Background: Transcatheter implantation of the Edwards Sapien 3 valve (Edwards Lifesciences, Irvine CA) within the bioprosthetic mitral valve (MV) is an established method of treatment in adults. However, it has not been well studied in the pediatric age group. Methods: Transcatheter mitral valve-in-valve implantation was attempted in 4 symptomatic pediatric patients with a dysfunctional MV bioprosthesis implanted at an earlier stage due to severe MV stenosis or regurgitation. We reviewed our experience with MV implantation in this cohort. Results: The mean age and weight of the patients at the time of the procedure were 11.4 years (range: 10-14 years) and 36 kg (range: 31-44 kg), respectively. The transmitral mean gradient dropped from a mean of 19.75 mm Hg (range: 15-22 mm Hg) to a mean of 1 mm Hg (range: 0-3 mm Hg) after the procedure. The mean fluoroscopy time was 55.25 minutes (range: 40-72 minutes), and the mean hospital length of stay was 4 days (range: 3-7 days). The patients’ functional class improved from New York Heart Association class IV to class I during the follow-up period. Conclusions: Transcatheter mitral valve-in-valve implantation can be performed safely for dysfunctional bioprosthetic MVs in the pediatric age group with favorable early and midterm outcomes. This procedure offers a viable alternative in patients who have high surgical risk or are deemed unfit for conventional surgery. However, we still recommend a long-term study of this approach in a large cohort, multicentre study. Résumé: Introduction: L’implantation de la prothèse valvulaire Edwards Sapien 3 (Edwards Lifesciences, Irvine, CA) par cathéter dans la bioprothèse valvulaire mitrale (VM) est une méthode de traitement établie chez les adultes. Toutefois, cette méthode n’a pas fait l’objet d’études approfondies auprès d’enfants. Méthodes: Une tentative d’implantation valvulaire mitrale de type valve-in-valve par cathéter a été réalisée chez quatre enfants symptomatiques qui avaient une bioprothèse VM dysfonctionnelle implantée antérieurement en raison d’une sténose VM ou d’une régurgitation grave. Nous avons passé en revue notre expérience d’implantation VM auprès de cette cohorte. Résultats: L’âge et le poids moyens des patients au moment de l’intervention étaient respectivement de 11,4 ans (étendue : 10-14 ans) et de 36 kg (étendue : 31-44 kg). 19. Enhancement of shear strength by using nanosilica reactive powder concrete.

The current work includes an experimental investigation into the shear strength of nanosilica reactive concrete beams. The variable studied was the amount of the nansilica, which ranges from 0% to 3% by weight of cement. The mechanical parameters of the concrete mixture, such as splitting, compressive, and flexural strength, were also studied. Four nanosilica reactive powder concrete beams were subjected to a two-point load and evaluated. Estimating shear behavior was used to failure load, crack patterns and failure mechanisms, load-deflection capacity, and strain distribution. The findings indicate that adding nanosilica to the mix improved the mechanical qualities of the cured concrete. The most significant increase in compressive, splitting, and flexural strengths was observed when nanosilica equaled 3 percent, with increases of 31.3 percent, 45.3 percent, and 50 percent, respectively. Adding NS to reactive powder concrete beams improves shear strength by raising the ultimate shear capacity to 25.1% of the reference beam. In addition, the nanoparticles bridge the cementitious materials, improve beams' ductility, and limit fracture development, and delayed failures. [ABSTRACT FROM AUTHOR]

20. Medication take-back programs in Qatar: Parental perceptions

Purpose: To identify parental perception of a take-back program for medications. Materials and Methods: A cross-sectional study using a questionnaire was conducted at Hamad Medical Corporation, the only tertiary pediatric hospital in the State of Qatar at the time of the study. Qatar is a rapidly developing country with limited national data on the awareness of medication misuse among adults living with children at home and on the safety practices regarding medication disposal. Results: 305 questionnaires were completed (response rate = 90%). More than 80% of parents were in between 20 and 39 years of age, 70% of them were females, and 80% were college graduates. Approximately 90% of participants have immediate relatives who were taking medications for chronic diseases. Almost 60% of parents stated that they keep unused medications at home, whereas 10% were not aware of the fate of the left over medications. Approximately 95% of the parents dispose the expired medications. In terms of the mode of disposing the medications, 66% of caregivers dispose the medication bottle or package in the trash can, whereas 14% remove the medications from the bottles or packages and throw them in the trash, and 15% put them through the drain. When asked if participants read disposal measures in the medication pamphlet, only 10% answered “always,” whereas 26% answered “sometimes.” Participants were asked if they have heard of any medications take-back programs, 75% answered no, whereas 14% were not sure. However, almost 60% of them will use the take-back program if available and 18% were not sure. Conclusion: Parents residing in the State of Qatar have deficiencies in knowledge about medication disposal. Parent's attitudes and perceptions are considered indispensable targets for community health intervention. Our next step is to share our data with the ministry of health to spread awareness about the proper disposal of medicines and take-back programs in Qatar.

21. Parental perception of medications safe storage in the State of Qatar

Purpose: The purpose of this study is to identify parental perception of household medication storage. Methods: A prospective cross-sectional study utilizing a questionnaire was carried out at Hamad Medical Corporation, the solely tertiary pediatric hospital in the State of Qatar at the time of the study. Qatar is a young developing country with limited data on the awareness of medication storage among adults with children at home and on the safety practices regarding medication storage. Results: Three hundred and five questionnaires were completed. The vast majority of parents were married, one-third of them were males, and more than three quarters were college graduates and younger than 40 years of age. Almost 80% of the parents had more than three children but less than seven. In addition, 23% of participants were health-care workers. Almost 90% of the participants stored medications in a place that is easy to reach. However, the same percentage stated that those medications were stored in a locked place and that children did not have access to them. Approximately 10% of caregivers store multiple medications in one bottle, and the same percentage of participants do not check the expiration date on the medication labels. In terms of the most common medications stored at home, antihypertensives were on top of the list. Our study has shown that parental education and being a health-care worker were each associated with the difficulty in reaching medications (P = 0.006 and P = 0.011, respectively). Moreover, the percentage of participants who shared medications was significantly higher among those who were not working in the health-care section compared to those who were (P = 0.004). In addition, being a female parent and a college graduate was associated with the possibility of keeping excess or leftover medications at home (P = 0.025). Conclusion: Parents residing in the State of Qatar have some deficiencies in knowledge about medication storage. Parent's attitudes and perceptions are deemed vital objectives for population's health intervention.

Background: Sign language is an essential means of communication for hearing-impaired individuals. Objective: We aimed to develop an American sign language recognition dataset and use it in the deep learning model which depends on neural networks to interpret gestures of sign language and hand poses to natural language. Methods: In this study, we developed a dataset and a Convolutional Neural Network-based sign language interface system to interpret gestures of sign language and hand poses to natural language. The neural network developed in this study is a Convolutional Neural Network (CNN) which enhances the predictability of the American Sign Language alphabet (ASLA). This research establishes a new dataset of the American Sign Language alphabet which takes into consideration various conditions such as lighting and distance. Results: The dataset created in this study is a new addition in the field of sign language recognition (SLR). This dataset may be used to develop SLR systems. Furthermore, our research compares the results of our dataset with two different datasets from other studies. The other datasets have invariant scene conditions, but our suggested CNN model demonstrated high accuracy for all the tested datasets. Despite the different conditions and volume of the new dataset, it achieved 99.38% accuracy with excellent prediction and small loss (0.0250). Conclusions: The proposed system may be considered a promising solution in medical applications that use deep learning with superior accuracy. Moreover, our dataset was created under variable conditions which increases the number of contributions, comparisons, results and conclusions in the field of SLR and may enhance such systems.

Modernizing communication systems is crucial for the future of aviation, and the
-band Digital Aeronautical Communications System (LDACS) in Air-Ground (A/G) and Air-Air (A/A) modes is being developed by European and American organizations to meet this need. In this study, we explore possible channel access schemes for aeronautical ad-hoc networks. We propose Multi-Channel Self-Organized TDMA (MCSOTDMA), a novel variant of randomized, reservation-based slotted ALOHA for LDACS A/A. In particular, we prove that it can achieve an optimal delay by following the$L$ -rule of traditional slotted ALOHA's throughput. This new variant allows users to distributively converge towards an optimal channel access strategy with no a-priori knowledge of their neighborhood. It scales to any number of users and always guarantees a freely chosen, average packet collision probability. We evaluate the performance on the busiest oceanic airspace, and compare it to medium access schemes used in maritime and other aeronautical networks. We demonstrate that arbitrary collision rates can be achieved at the cost of proportional packet delay, establish communication links with deterministic performance, and implement broadcast voice messaging among pilots. After more than four decades of post-approval, cisplatin is still an important treatment for numerous cancers. However, acute kidney injury (AKI), defined as significant impairment of renal filtration as discussed below, is the major limiting side effect of cisplatin, occurring in approximately 30% of patients (25–33% after the first course). Cisplatin also damages the kidneys’ ability to reabsorb magnesium in 40–100% of patients, with collateral health risks due to subsequent hypomagnesemia. Multiple methods and drugs have been proposed for preventing cisplatin-induced AKI, including saline infusion with or without mannitol, which has not always prevented AKI and has been found to activate a cellular stress response in renal tubular cells. While numerous reports and trials, as well as the National Comprehensive Cancer Network (NCCN), support premedication with magnesium and hydration, this practice has not been universally accepted. Many clinics administer intravenous magnesium (IV) only after identification of hypomagnesemia post-cisplatin treatment, thus placing patients at risk for AKI and chronic renal loss of magnesium. We present the following case report and additional supporting evidence identifying the immediate effect of IV magnesium prior to intraperitoneal cisplatin for cycle 4 because of documented hypomagnesemia resulting in normalization of oliguria, which had been experienced for the first three cycles. The patient subsequently requested and received IV magnesium before cisplatin for the next two cycles with continuation of normal urinary output. The effect of pretreatment with IV magnesium on urine output following cisplatin has not been previously reported and further supports pre-cisplatin administration. In addition, two recent meta-analyses of clinical trials and pre-clinical research are reviewed that demonstrate effectiveness of magnesium pretreatment to preventing AKI without reducing its chemotherapeutic efficacy. This case report with additional evidence supports the adoption of administration of 1–3 g IV magnesium before cisplatin as best practice to prevent cisplatin induced AKI and hypomagnesemia regardless of patient baseline serum magnesium levels.

Background: Little information is available about breast cancer (BC) in Sudan. Therefore, the present study aimed to provide baseline information about the demographic features and tumor characteristics, and also to investigate the associations between demographic variables and presentation stage in BC patients attending the National Cancer Institute-University of Gezira (NCI-UG), Sudan. Methods: In this cross-sectional study, we included all BC patients treated at the NCI-UG from January to December 2013. Patients' demographic, clinical and pathological data were retrieved from the hospital records and analyzed using SPSS version 20, and associations between these factors were tested as well. Results: A total of 232 cases were included in this research. The majority (97.8%) of subjects were females and 2.2% were males. The median age at diagnosis was 50 years (range, 22-90). The mean time between identification of symptoms and diagnosis was 13 months (SD=16.1). Clinical stages I, II, III, and IV represented 6.9%, 37.0%, 40.9% and 15.2%, respectively. Advanced stage at diagnosis was associated with longer duration between identifying the symptoms to diagnosis (P=0.006). Also the level of education of BC patients was significantly associated with clinical stage at presentation (P = 0.01). Conclusions: Sudanese patients with BC present at a younger age and with more advanced stage at diagnosis than those in developed countries. Patients' education level and duration from identification of BC symptoms to diagnosis significantly impact the stage at the time of presentation. In limited resource setting, early diagnosis of symptomatic BC is crucial in reducing the disease burden.

Recently, a considerable amount of research has been conducted on multiple GNSS receivers using the Direct RF sampling concept. This approach greatly reduces the hardware requirements of the traditional receiver by placing the analog-todigital converter as close as possible to the antenna, eliminating the mixer stages. Furthermore, directly sampling the RF signal using the ADC allows for a more efficient design. In this paper, a Multi-GNSS Direct Radio Frequency Sampling Method is designed, optimized, and its performance is presented for several GNSS signals, including GPS L1, GPS L2, and GPS L5. The implemented method is used to find the ranges of valid sampling frequencies in single and multiple band RF signal cases. The algorithm presented results in an easier way to find valid sampling frequencies and a straightforward implementation without the need for complicated mathematical calculations. We consider aliasing issues and computational problems, providing a full theoretical and practical analysis compared to previously presented algorithms in the literature. This paper also examines the relationship between the sampling frequency, noise folding, and its effect on the signal-to-noise ratio (SNR), thus allowing for the selection of the optimal sampling frequency.

Background: Neonatal admission generally refers to the admission of newborns under 29 days old into a health facility for medical care. Because neonates are fragile and yet to develop competent immune system, they are prone to infections, and most of the illnesses they acquire usually require critical care, hence their admission to the neonatal intensive care units (NICUs). High global neonatal death have triggered improve fatality-based care. Methods: this hospital based retrospective study was conducted in the NICU of Atbara teaching hospital in River Nile State, Sudan. Data was collected from medical records of 300 neonate. Results: 68.7% of the neonates were term, while 18.0% were early preterm, 8.3% were late preterm, 4.0% were post-date, and 1.0% were post-term. Moreover, the most common morbidity among them were sepsis, preterm births, HIE, meconium aspiration syndrome, N-jaundice respiratory distress syndrome, and the mortality rate was 15.4% Conclusions: Majority of causes of neonatal morbidity and mortality in our study were preventable diseases. Therefore, interventions to improve services in the NICU are highly needed to improve the outcomes. [ABSTRACT FROM AUTHOR]

- Abstract
This study focuses on the theme of fear in the tale of la Peur by Guy de Maupassant. La Peur contains three stories with a common theme of fear. We focus on the second and third stories. The first story takes place in Algeria and the second story takes place in France. In order to carry out our study, we use both thematic analysis to analyze the theme of fear, and psychology to define the conception of fear. The importance of our study comes from its double psychological and literary value. Maupassant makes a careful study of fear, highlighting a picture of real life dominated by excessive fear. We aim, through this type of study, to always note the importance of literature as a point of intersection between the human sciences on the one hand and humanity on the other hand. We end up with results, the most important of which is that pathological fear is an enemy within us. Pathological fear is not like normal fear constituting an alarm bell against dangers; on the contrary it destroys our life and contaminates our surroundings. We must face it instead of escaping from. This type of story and study is one of the means of developing our knowledge of this destructive emotion at the depth of our souls. Key words: Desert, Forester, Fear, Drum, Poacher.

Background: Transcatheter implantation of the Edwards Sapien 3 valve (Edwards Lifesciences, Irvine CA) within the bioprosthetic mitral valve (MV) is an established method of treatment in adults. However, it has not been well studied in the pediatric age group. Methods: Transcatheter mitral valve-in-valve implantation was attempted in 4 symptomatic pediatric patients with a dysfunctional MV bioprosthesis implanted at an earlier stage due to severe MV stenosis or regurgitation. We reviewed our experience with MV implantation in this cohort. Results: The mean age and weight of the patients at the time of the procedure were 11.4 years (range: 10-14 years) and 36 kg (range: 31-44 kg), respectively. The transmitral mean gradient dropped from a mean of 19.75 mm Hg (range: 15-22 mm Hg) to a mean of 1 mm Hg (range: 0-3 mm Hg) after the procedure. The mean fluoroscopy time was 55.25 minutes (range: 40-72 minutes), and the mean hospital length of stay was 4 days (range: 3-7 days). The patients’ functional class improved from New York Heart Association class IV to class I during the follow-up period. Conclusions: Transcatheter mitral valve-in-valve implantation can be performed safely for dysfunctional bioprosthetic MVs in the pediatric age group with favorable early and midterm outcomes. This procedure offers a viable alternative in patients who have high surgical risk or are deemed unfit for conventional surgery. However, we still recommend a long-term study of this approach in a large cohort, multicentre study. Résumé: Introduction: L’implantation de la prothèse valvulaire Edwards Sapien 3 (Edwards Lifesciences, Irvine, CA) par cathéter dans la bioprothèse valvulaire mitrale (VM) est une méthode de traitement établie chez les adultes. Toutefois, cette méthode n’a pas fait l’objet d’études approfondies auprès d’enfants. Méthodes: Une tentative d’implantation valvulaire mitrale de type valve-in-valve par cathéter a été réalisée chez quatre enfants symptomatiques qui avaient une bioprothèse VM dysfonctionnelle implantée antérieurement en raison d’une sténose VM ou d’une régurgitation grave. Nous avons passé en revue notre expérience d’implantation VM auprès de cette cohorte. Résultats: L’âge et le poids moyens des patients au moment de l’intervention étaient respectivement de 11,4 ans (étendue : 10-14 ans) et de 36 kg (étendue : 31-44 kg). La moyenne du gradient moyen transmitral a baissé. Elle est passée de 19,75 mmHg (étendue : 15-22 mmHg) à 1 mmHg (étendue : 0-3 mmHg) après l’intervention. La durée moyenne de la fluoroscopie était de 55,25 minutes (étendue : 40-72 minutes), et la durée moyenne du séjour à l'hôpital était de quatre jours (fourchette : 3-7 jours). Conclusions: L’implantation valvulaire mitrale de type valve-in-valve par cathéter peut être pratiquée de façon sûre chez les enfants porteurs d’une bioprothèse VM dysfonctionnelle dont les issues à court ou à moyen terme sont favorables. Cette intervention est une alternative viable pour ces patients dont le risque lié à l’intervention chirurgicale est élevé ou considérés inaptes à subir une intervention chirurgicale traditionnelle. Toutefois, nous recommandons encore une étude à long terme sur cette approche, voire une vaste étude multicentrique de cohorte.

Since the outbreak of the coronavirus disease 2019 (COVID-19) pandemic, over 103214008 cases have been reported, with more than 2231158 deaths as of January 31, 2021. Although the gold standard for diagnosis of this disease remains the reverse-transcription polymerase chain reaction of nasopharyngeal and oropharyngeal swabs, its false-negative rates have ignited the use of medical imaging as an important adjunct or alternative. Medical imaging assists in identifying the pathogenesis, the degree of pulmonary damage, and the characteristic features in each imaging modality. This literature review collates the characteristic radiographic findings of COVID-19 in various imaging modalities while keeping the preliminary focus on chest radiography, computed tomography (CT), and ultrasound scans. Given the higher sensitivity and greater proficiency in detecting characteristic findings during the early stages, CT scans are more reliable in diagnosis and serve as a practical method in following up the disease time course. As research rapidly expands, we have emphasized the CO-RADS classification system as a tool to aid in communicating the likelihood of COVID-19 suspicion among healthcare workers. Additionally, the utilization of other scoring systems such as MuLBSTA, Radiological Assessment of Lung Edema, and Brixia in this pandemic are reviewed as they integrate the radiographic findings into an objective scoring system to risk stratify the patients and predict the severity of disease. Furthermore, current progress in the utilization of artificial intelligence via radiomics is evaluated. Lastly, the lesson from the first wave and preparation for the second wave from the point of view of radiology are summarized.

Thirty two farms biosecurity tests were used, biosecurity practices were evaluated in poultry farms using the checklist and organized questionnaires administered to commercial poultry farmers. Simple descriptive statistics were used (frequencies and percentages) to summarize and display the results. The results showed that 62.5% of the farms used the floor brooding system of the birds; 59.4% open barns; The domestic stock in access to the farm site 68.8%; Pets or wildlife birds have access to inside of barns or feed storage 71.9%; yearly water quality is not checked by a reputable laboratory 96.9%; employees have routine contact with other poultry species 84.4%; regular cleaning and disinfection of feeders and drinkers washing is done mainly by soap in addition to water. 43.8%; employees and their families with other people on poultry housing site 100%; poultry production, feed consumption, and mortality records are kept, but not reviewed daily 18.8%; there is untrimmed vegetation and debris within barn(s) 81.2%; vehicle flow designated way traffic onto premises and off premises 100% and perimeter fencing and gate present but not always locked or guarded or fence not completed 56.25%. The study concluded that there is a severe weakness in the application of biosecurity practices in its three branches isolation, cleaning disinfecting and traffic control. The adopted hygiene and traffic control measurement were all within the high and medium biosecurity risk limits whereas mismanagement resembled 60.4% the highest risk of biosecurity practices among the studies in commercial poultry farms of ElFashir Locality. Therefore, it is strongly recommended for local authorities to intervene with qualified teams of poultry professional and experts to train poultry farmers, managers and attendees on importance of implementing sound biosecurity practices.

- Abstract
An atrial septal aneurysm (ASA) is saccular defect of the atrial septum, and it may be associated with congenital heart disease. Cryptogenic strokes are strokes of unknown origins, and ASA may be one of these causes. In this case report, we described a cryptogenic stroke in an adult Sudanese female with interatrial septal aneurysm.
