2. Association Between Sleep Microstructure and Incident Hypertension in a Population-Based Sample: The HypnoLaus Study

Background Poor sleep quality is associated with increased incident hypertension. However, few studies have investigated the impact of objective sleep structure parameters on hypertension. This study investigated the association between sleep macrostructural and microstructural parameters and incident hypertension in a middle- to older-aged sample. Methods and Results Participants from the HypnoLaus population-based cohort without hypertension at baseline were included. Participants had at-home polysomnography at baseline, allowing assessment of sleep macrostructure (nonrapid eye movement sleep stages 1, 2, and 3; rapid eye movement sleep stages; and total sleep time) and microstructure including power spectral density of electroencephalogram in nonrapid eye movement sleep and spindles characteristics (density, duration, frequency, amplitude) in nonrapid eye movement sleep stage 2. Associations between sleep macrostructure and microstructure parameters at baseline and incident clinical hypertension over a mean follow-up of 5.2 years were assessed with multiple-adjusted logistic regression. A total of 1172 participants (42% men; age 55±10 years) were included. Of these, 198 (17%) developed hypertension. After adjustment for confounders, no sleep macrostructure features were associated with incident hypertension. However, low absolute delta and sigma power were significantly associated with incident hypertension where participants in the lowest quartile of delta and sigma had a 1.69-fold (95% CI, 1.00-2.89) and 1.72-fold (95% CI, 1.05-2.82) increased risk of incident hypertension, respectively, versus those in the highest quartile. Lower spindle density (odds ratio, 0.87; 95% CI, 0.76-0.99) and amplitude (odds ratio, 0.98; 95% CI, 0.95-1.00) were also associated with higher incident hypertension. Conclusions Sleep microstructure is associated with incident hypertension. Slow-wave activity and sleep spindles, 2 hallmarks of objective sleep continuity and quality, were inversely and consistently associated with incident hypertension. This supports the protective role of sleep continuity in the development of hypertension.

- Abstract
Background\ud \ud frailty is a dynamic condition for which a range of interventions are available. Health state utilities are values that represent the strength of an individual’s preference for specific health states, and are used in economic evaluation. This is a topic yet to be examined in detail for frailty. Likewise, little has been reported on minimally important difference (MID), the extent of change in frailty status that individuals consider to be important.\ud \ud \ud Objectives\ud \ud to examine the relationship between frailty status, for both the frailty phenotype (FP) and frailty index (FI), and utility (preference-based health state), and to determine a MID for both frailty measures.\ud \ud \ud Design and setting\ud \ud population-based cohort of community-dwelling Australians.\ud \ud \ud Participant\ud \ud in total, 874 adults aged ≥65 years (54% female), mean age 74.4 (6.2) years.\ud \ud \ud Measurements\ud \ud frailty was measured using the FP and FI. Utilities were calculated using the short-form 6D health survey, with Australian and UK weighting applied. From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions. © Copyright.

The presence of antibodies against cardiolipin, a normal component of tissues, has been found in a number of different conditions that have a tendency towards thrombosis (the formation of blood clots). Antibodies to the phospholipid compound cardiolipin were assessed in 110 patients who had experienced either a full stroke or a transient ischemic attack (TIA; in which the cerebral blood flow is momentarily interrupted, but not long enough to result in a stroke). Antibodies of both the IgG class and IgM class were assayed. Nine of 110 patients were found to have IgG anticardiolipin antibodies, and 10 of 110 were found to have the IgM class of antibodies against this phospholipid. Of 122 control subjects tested, 8 had IgM anticardiolipin antibodies; the difference between the patient group and the controls was not significant for IgM antibodies. However, the increased presence of IgG antibodies in the patient group was statistically significant. Three of the nine patients with IgG anticardiolipin antibodies were 40 years old or younger. These patients were found to have a constellation of other symptoms that are consistent with the thrombotic syndromes associated with antiphospholipid antibodies. The results suggest that widespread screening of stroke patients for the presence of antiphospholipid antibodies is not warranted. However, such screening might be appropriate if limited only to patients who are at risk for recurrent events and, particularly, especially young patients who have thrombotic syndromes, such as collagen diseases. Background: Change in the prevalence of raised blood pressure could be due to both shifts in the entire distribution of blood pressure (representing the combined effects of public health interventions and secular trends) and changes in its high-blood-pressure tail (representing successful clinical interventions to control blood pressure in the hypertensive population). Our aim was to quantify the contributions of these two phenomena to the worldwide trends in the prevalence of raised blood pressure. Methods: We pooled 1018 population-based studies with blood pressure measurements on 88.6 million participants from 1985 to 2016. We first calculated mean systolic blood pressure (SBP), mean diastolic blood pressure (DBP) and prevalence of raised blood pressure by sex and 10-year age group from 20-29 years to 70-79 years in each study, taking into account complex survey design and survey sample weights, where relevant. We used a linear mixed effect model to quantify the association between (probittransformed) prevalence of raised blood pressure and age-group- and sex-specific mean blood pressure. We calculated the contributions of change in mean SBP and DBP, and of change in the prevalence-mean association, to the change in prevalence of raised blood pressure. Results: In 2005-16, at the same level of population mean SBP and DBP, men and women in South Asia and in Central Asia, the Middle East and North Africa would have the highest prevalence of raised blood pressure, and men and women in the highincome Asia Pacific and high-income Western regions would have the lowest. In most region-sex-age groups where the prevalence of raised blood pressure declined, one half or more of the decline was due to the decline in mean blood pressure. Where prevalence of raised blood pressure has increased, the change was entirely driven by increasing mean blood pressure, offset partly by the change in the prevalence-mean association. Conclusions: Change in mean blood pressure is the main driver of the worldwide change in the prevalence of raised blood pressure, but change in the high-blood-pressure tail of the distribution has also contributed to the change in prevalence, especially in older age groups. © The Author(s) 2018.

Objectives: Intra‐oral bone grafting relies on three‐dimensional understanding of mandibular anatomy. This study assessed the bone volume at the two most common intra‐oral bone harvesting sites, the retromandibular and symphyseal regions, and assessed the impact of age and sex on the available bone at these sites. Materials and methods: Demographic and anatomical data were collected from cone beam computer tomographs (CBCT's) of 200 randomly selected, fully dentate participants (100 male/100 female) between the ages of 24 and 86 years. Statistical analysis was conducted with SPSS V25, using ANalysis of COVAriance (ANCOVA) to determine the effects of age and sex on the measurements at the donor sites. Results: At retromandibular sites, women have a broader alveolar crest with a narrower mandible at the level of the IDC. There is a statistically significant difference, between the sexes, in bone width from the buccal cortex to the IDC. Men have a significantly greater distance from the outer buccal plate to the IDC. There is no difference in any measured dimension at the symphyseal region. There is a statistically significant reduction in bone volume with increasing age at both mandibular sites of 0.03–0.05 mm annually, irrespective of tooth loss. Conclusion: Anatomical variability due to sex and bone reduction with age are both important findings in dental implantology, which must be considered when treatment planning and selecting bone grafting sites in the mandible. This study reinforces the importance of pre‐operative CBCT in planning bone grafting procedures. [ABSTRACT FROM AUTHOR]

- Abstract
Convergence and extension movements elongate tissues during development. Drosophila germ-band extension (GBE) is one example, which requires active cell rearrangements driven by Myosin II planar polarisation. A combinatorial code of Toll receptors downstream of pair-rule genes contributes to this polarization via local cell-cell interactions. We developed novel computational methods to analyse the spatiotemporal dynamics of Myosin II. We show that initial Myosin II bipolar cell polarization gives way to unipolar enrichment at parasegmental boundaries and two further boundaries within each parasegment, concomitant with a doubling of cell number as the tissue elongates. These boundaries are the primary sites of cell intercalation, behaving as mechanical barriers and providing a mechanism for how cells remain ordered during GBE. Enrichment at parasegment boundaries during GBE is independent of Wingless signaling, suggesting pair-rule gene control. We propose an updated cell-cell interaction model for Myosin II polarization that we tested in a vertex-based simulation.

The movements of cells that underlie the generation of form during vertebrate development are in most cases poorly understood. The zebrafish provides a model system in which we can address this problem in vivo. Imaging methods now allow us to see the movements of many hundreds of cells but still the details of the motion are not easy to interpret by eye. Computer methods have been developed to allow the movements of each of these cells to be traced over time. Algorithms have been developed to highlight aspects of cell behaviour, relative cell movements and local reorganisations from these data and to display them in ways that reveal their underlying pattern. Given this exhaustive analysis of morphogenesis, we can address in great detail questions such as where and how cells move, if cells move as individuals or as members of larger groups. Kinetic analyses give us insight into the origin of force production. A measure of changes in cell neighbours tells us about cell mixing and rearrangement over time and the orientation of such changes. We will present analyses of early cell movements during gastrulation.
