This paper presents a unified formulation for the combination of the finite element method (FEM) and the boundary element method (BEM) in 3D frictional contact problems that is based on the use of localized Lagrange multipliers (LLMs). Resolution methods for the contact problem between non-matching meshes have traditionally been based on a direct coupling of the contacting solids using classical Lagrange multipliers. These methods tend to generate strongly coupled systems that require a deep knowledge of the discretization characteristics on each side of the contact zone complicating the process of mixing different numerical techniques. In this work a displacementcontact frameis inserted between the FE and BE interface meshes, discretized and finally connected to the contacting substructures using LLMs collocated at the msh-interface nodes. This methodology will provide a partitioned formulation which preserves software modularity and facilitates the connection of non-matching FE and BE meshes. &nbsp

This work presents a non symmetric Finite-Boundary Element Tearing and Interconnecting (FE-BETI) formulation for acoustic FSI problems, where the finite element method is used to model the structure, while the acoustical fluid domain is represented by the boundary element method. The method interconnects fluid and structure domains using the localized Lagrange multipliers, allowing the use of non-matching meshes on the interfaces. Furthermore, the methodology proposes a preconditioned projected biconjugate gradient solver, that presents good scalability properties in the solution of large problems.

Objetivo: El actual standar en el diagnóstico y seguimiento del tumor vesical es la cistoscopia con luz blanca. Recientes estudios sugieren que la cistoscopia con fluorescencia a través de derivados de las porfirinas puede mejorar la detección de lesiones vesicales. Hemos analizado los resultados en el diagnóstico y tratamiento de lesiones vesicales utilizando la cistoscopia de fluorescencia con hexaminolevulinato (Hexvix®) en nuestro centro. Métodos: Entre septiembre 2006 y septiembre 2007, a 39 pacientes consecutivos con sospecha ó confirmación de tumor vesical se les administró una instilación endovesical de Hexvix® una hora antes de la cirugía. Se realizó en primer lugar una cistoscopia con luz blanca (CLB) y posteriormente con luz azul (CLA). Se resecaron todas las lesiones papilares y las sospechosas para análisis histológico. La edad media fue de 70.1 años (50-86). 30 hombres (76.9%) y 9 mujeres (23.1%). Las características de los tumores fueron: 18% tumores primarios, 51% recurrencias y 30% controles cistoscópicos. 24 pacientes habían recibido con anterioridad algún tratamiento (9 mitomicina C y 15 BCG). 7 pacientes demostraron citologías urinarias positivas previas. Resultados: Todas las lesiones papilares visualizadas con luz blanca se confirmaron con luz azul (18 pacientes). De éstos, 17 tuvieron biopsias positivas (6 pTaG1, 9 pT1G1-3, 1 pT2, 1 CIS). En 15 pacientes (38.4%) se objetivó al menos 1 lesión más solo visualizada con la luz azul. En este grupo, 8 pacientes (20.5%) tuvieron un diagnóstico histológico positivo (3 pTaG1, 2 pT1G3, 3 CIS). En 5 de estos casos (13%) el manejo terapéutico posterior cambió al introducir la luz azul (BCG vs MMC). Todos los casos de CIS (4) fueron diagnosticados mediante luz azul. En el postoperatorio no se demostraron efectos secundarios (locales ó sistémicos) debidos a la administración de Hexvix® Conclusiones: Nuestros resultados sugieren un incremento en el diagnóstico de lesiones tumorales vesicales, papilares y planas, con el uso de la cistoscopia bajo fluorescencia. Esto ha determinado un cambio de actitud terapéutica relevante en un 13% de los pacientes. Obviamente, se necesita mayor experiencia para consolidar estos resultados y un seguimiento a largo plazo para valorar el impacto a nivel de recurrencia y progresión tumoral. Objectives: White light cystoscopy is the current standard for the diagnosis of bladder cancer and monitorization for recurrence. Recent studies suggest that porphyrin based fluorescence cystoscopy may improve endoscopic detection of bladder tumors. We aimed to evaluate the improvement that hexaminolevulinate fluorescence cystoscopy could lead in bladder cancer detection and treatment at one single centre. Methods: Between September 2006 and September 2007 a total of 39 patients with known or suspected bladder cancer underwent bladder instillation with 50 ml 8 mM hexaminolevulinate (HAL) for 1 hour. The bladder was inspected using white light cystoscopy (WLC), followed by blue light (fluorescence) cystoscopy (BLC). Papillary and suspicious lesions were resected for histological examination. Mean age was 70.1 years (50-86). Thirty patients were male (76.9%) and 9 female (23.1%). The tumor characteristics were: 18% primary tumors, 51% recurrences and 30% control cystoscopies. 24 patients had previously received some treatment (9 MMC and 15 BCG). Only 7 patients had previous positive urine cytology. Results: All papillary lesions visualized with WLC were confirmed by BLC (18 patients). From these, 17 have positive biopsies (6 pTaG1, 9 pT1G1-3, 1 pT2, 1 CIS). In 15 patients (38.4%) we found at least 1 lesion more with BLC. In this group 8 cases (20.5%) had positive histological diagnosis (3 pTaG1, 2 pT1G3, 3 CIS). In five patients (13%) post-TUR therapeutic management has changed by using BLC (BCG vs MMC). All four patients with CIS were diagnosed by BLC. There was no evidence of local or systemic side effects due to HAL in the postoperative time. Conclusions: Our results suggest there is an improvement in the diagnosis of papillary and flat lesions in bladder cancer by using HAL fluorescence cystoscopy. This has changed the management in the postoperative period (MMC vs BCG) in 13% of the patients. Obviously, we need more patients to assess our data and long term follow-up to analyze the impact in terms of tumor recurrence and progression.

OBJETIVO: La braquiterapia prostática con implantes permanentes de semillas de I125 ó 103Pd es una opción terapéutica en el tratamiento del cáncer prostático organoconfinado. Analizamos los resultados preliminares y las complicaciones a cinco años en el grupo de pacientes tratados con braquiterapia de baja tasa (I125) como tratamiento único de intención curativa y evaluamos las diferencias existentes con el tratamiento estándar (cirugía). MÉTODOS: Sobre una casuística que supera los 400 pacientes tratados con braquiterapia como monoterapia y con intención curativa radical por adenocarcinoma de próstata organoconfinado, hemos excluido el grupo de pacientes con un seguimiento menor a 12 meses para el análisis estadístico de resultados y complicaciones, por lo tanto el grupo a estudio consta de 275 pacientes reclutados desde abril de 1999 a diciembre del 2003. El seguimiento medio de la serie es de 31 meses (12-68 m). Se consideró fallo bioquímico cuando los pacientes cumplieron los criterios marcados por la ASTRO. El análisis estadístico de supervivencia se realizó con el paquete estadístico SPSS y el método de Kaplan-Meyer. Las complicaciones urinarias y digestivas fueron evaluadas de acuerdo con los criterios de la RTOG RESULTADOS: La edad media del grupo fue de 68 años con un rango (49-83 años). El 93% de los pacientes presentaron un estadio clínico ≤ T2a y un 7% de estadios T2b con PSA ≤ 10 ng/ml en el 68% de los casos. El Gleason fue ≤ 6 en el 94% de los casos. Un 9% de los casos tratados presentaban un volumen prostático >50cc. la supervivencia global a los 5 años es del 96%, con una supervivencia libre de enfermedad de 97% y una con una supervivencia libre de fallo bioquímico del 99% al mismo tiempo. Las complicaciones de las diferentes series incluida la nuestra están recogidas en la Figura 3 y Tablas II-VI. CONCLUSIÓN: Las múltiples series publicadas en la literatura parecen poner de manifiesto los mismos resultados de control bioquímico de la enfermedad cuando se compara la cirugía con la braquiterapia de baja tasa en enfermedad organoconfinada. La braquiterapia presenta frente a la cirugía la ventaja de tener menor porcentaje de complicaciones en el postoperatorio inmediato, menor índice de incontinencias y una preservación de la función eréctil en mayor número de pacientes. OBJECTIVES: Prostatic brachytherapy by permanent implant of I125 or Pd103 is a therapeutic option in the treatment of organ confined prostate cancer. We analyze preliminary results and complications after five years in the group of patients who received I125 low dose rate brachytherapy as the only intention-tocure treatment and evaluate the differences with the standard treatment (surgery). METHODS: From a case series of more than 400 patients treated with brachytherapy as radical intentionto- cure monotherapy for organ-confined prostate cancer we excluded patients with less than 12 months of follow-up for statistical analysis; the study group includes 275 patients enrolled between april 1999 and December 2003. Mean follow-up is 31 months (12- 68). Biochemical failure was defined in accordance to the ASTRO criteria. Statistical survival analysis was carried out with the SPSS statistical software using the Kaplan Meyer method. Urinary and gastrointestinal complications were evaluated in accordance to the RTOG criteria. RESULTS: Mean age was 68 years (range 49-83 years). 93% of the patients presented a clinical stage ≤ 3dT2a and 7% T2b, with 60 8% of the cases having a PSA ≤3d 10 ng/ml. Gleason score was ≤3d 6 in 94% of the cases. 9% of the cases had a prostate volume > 50 cc. Overall 5 year survival was 96%, with a 97% disease-free survival and a 99% biochemical failure-free survival. Figure 3 and tables II-VI summarize the complications in various series including this. CONCLUSIONS: Multiple published series seem to show similar results on biochemical control of the disease when comparing surgery and low dose rate brachytherapy in organ-confined disease. In comparison to surgery, brachytherapy has the advantage of having a lower percentage of immediate postoperative complications, lower incontinence rate, and a higher number of patients preserving erectile function.
