Abstract

Purpose: To our knowledge, no study specifically assessed anxiety and depression in Moroccan cancer population undergoing radiotherapy. The objective of this study was to assess the prevalence of anxiety and depression in patients with cancer prior to the first session of radiotherapy and to identify predictor factors. Material and methods: It is a cross-sectional single center study. An interview was conducted using the Hospital Anxiety and Depression Scale. Results: 150 patients filled the study inclusion criteria. The mean age was 54.9 ± 12.8 years, 12% of patients were in current employment. 42% of the participants had a positive family history of cancer. Out of study participants, 58% scored abnormal on the anxiety scale and 52% on the depression scale. Statistically significant predictors of anxiety and depression were similar to those reported in published studies, such as the presence of family history of cancer, received radiotherapy in family, employment status and treatment intention. Conclusions: The results of this study suggest that a large proportion of cancer patients have psychiatric comorbidities. There is an urgent need for psychosocial support programs and psychological screening for patients diagnosed with cancer and candidate for radiation therapy.

7. Radiothérapie hypofractionnée du cancer du sein avec envahissement ganglionnaire massif : résultats thérapeutiques et toxicité

Abstract

Objectif de l’etude Evaluer la radiotherapie hypofractionnee adjuvante en cas de cancer du sein avec envahissement ganglionnaire massif (de stade N2-N3), aussi bien en termes d’efficacite (controle locoregional, survies globale et sans rechute) que de toxicite. Patientes et methodes Les dossiers de 113 patientes, traitees entre octobre 2009 et juin 2011, d’âge moyen de 48,2ans ± 10,9 ans ont ete analyses. Toutes les patientes ont recu une radiotherapie hypo fractionnee (42 Gy, soit 2,8 Gy/fraction, cinq fractions/semaine) dans le sein (20,4 %, n = 23) ou la paroi thoracique (79,6 %, n = 90) et la region susclaviculaire (99,1 %, n = 112) ou axillo-sus-claviculaire (0,9 %, n = 1). En cas de traitement conservateur, un boost a ete delivre. Toutes les patientes ont recu une chimiotherapie. Le nombre median de ganglions axillaires preleve etait de 15 (7–33), avec un nombre moyen de 8,4 ± 4,6 de ganglions metastatiques. Par ailleurs, 68,1 % des cancers ( n = 77) etaient de stade N2, pour 62,8 % ( n = 71), le ratio ganglions envahis/ganglions preleves etait entre 20 et 65 %, pour 31,9 % ( n = 6) de plus 65 %. Resultats Le suivi median etait de 61 mois. A cinq ans, les probabilites de survie globale, de survie sans recidive, les taux de controle locoregional et de controle a distance etaient respectivement de 81,1 %, 80,9 %, 90,3 % et 77,9 %. En analyse multifactorille, la survie globale et sans rechute a 5 ans des malades atteintes de cancer triple negatif et avec un ratio ganglionnaire superieur a 65 % differaient statistiquement significativement. En ce qui concerne toxicites, 8,4 % des patientes ont souffert d’un lymphœdeme. Il n’a ete observe aucune plexite radique. Conclusion Nos resultats sont encourageants, la radiotherapie hypofractionnee peut etre proposee en cas d’envahissement ganglionnaire de stade N2 ou N3.

10. Lacrimal gland adenoid cystic carcinoma: report of an unusual case with literature review.

Abstract

Lacrimal gland adenoid cystic carcinomas are rare, aggressive orbital tumors characterized by poor overall prognosis, tendency for local recurrence and metastasis despite aggressive treatment. Treatment continues to be controversial. Many authorities today will often initiate surgery (orbital exenteration with or without bone removal vs. globe-sparing resection) and adjuvant radiotherapy (external beam or proton beam therapy). We introduce a case of lacrimal gland adenoid cystic carcinoma treated with orbital exenteration and adjuvant volumetric modulated arc therapy, and discuss the related literature.

11. Intrathyroidal parathyroid carcinoma: a case report and literature review.

Abstract

Parathyroid carcinoma is an uncommon endocrine malignancy comprising 0.5%-2% of patients with primary hyperparathyroidism. The probability of an intrathyroidal location is low (0.2%) and make preoperative suspicion and diagnosis challenging. Less than 20 cases of intrathyroidal parathyroid carcinoma have been reported. We introduce a case of intrathyroidal parathyroid carcinoma mimicking a suspicious thyroid nodule, and review the literature, with a focus on the role of adjuvant radiotherapy.

Abstract

Purpose: To evaluate the effect of hypofractionated radiation therapy (HFRT) of the breast/chest wall and regional nodes on overall survival (OS), disease-free survival (DFS), locoregional control and on treatment-related toxicity in patients with breast cancer and nodal involvement., Methods and Materials: Two hundred fifty-seven patients treated between October 2009 and June 2011 with hypofractionated locoregional radiation therapy (42 Gy in 15 fractions) were retrospectively reviewed, 51 (19.8%) after breast-conserving surgery and 206 (80.2%) after radical surgery. Patients treated with breast-conserving surgery received a boost dose to the tumor bed (delivered by photons, electrons, or interstitial high-dose-rate brachytherapy). Two hundred fifty-six (99.6%) patients underwent chemotherapy, 209 (81.3%) had hormonal treatment, and 65 (25.3%) had anti-HER2 targeted therapy., Results: The median follow-up time was 64 months (range, 11-88 months). The rates of 5-year OS, DFS, locoregional recurrence (LRR)-free survival, and distant metastasis (DM)-free survival were 86.6%, 84.4%, 93.9%, and 83.1%, respectively. In multivariate analysis (MVA), lymph node ratio >65%, lymphovascular invasion, and negative hormone receptor status predicted for OS, DSF, and DM. T3 to 4 stage was also associated with worse DFS and DM. Finally, for LRR the independent prognostic factors on MVA were N2 to 3 stage and grade 3. Hyperpigmentation was observed in 19.2% of patients, telangiectasia in 12.3%, and fibrosis in 30.7%. Grade ≥2 lymphedema was recorded in 5.8% of cases. During the study follow-up, no cardiac or symptomatic pneumonitis was observed, nor were plexopathy or rib fractures., Conclusion: According to the findings from this retrospective study, HFRT seems to be an acceptable alternative for patients with breast cancer who need regional nodal irradiation.
