ÖZ GİRİŞ ve AMAÇ: Ankilozan spondilit (AS) olan bireylerde tıkayıcı uyku apne sendromu (TUAS) normalden sık görülebilir. Çalışmamızın amacı AS tanısı bulunan hastalarda TUAS varlığını Müller manevrası ve Epworth Uykululuk Skalası ile araştırmaktır. YÖNTEM ve GEREÇLER: Çalışmaya 22 AS hastası ile benzer cinsiyet ve yaş dağılımında bulunan 18 sağlıklı kontrol dahil edildi. Her iki grubun baş ve boyun muayenesi yapıldı ve palatin tonsil büyüklüğünün derecesi ile mallampati derecesinin büyüklüğü kaydedildi. Buna ek olarak, fleksibl fiberoptik muayenede Müller manevrası ile hipofarenks ve dil kökü seviyelerinin obstrüksiyon dereceleri kaydedildi. TUAS varlığını belirlemek amacı ile tüm katılımcılara Epworth Uykululuk Skalası uygulandı. Hasta ve kontrol grubunun palatin tonsil büyüklüğü, mallampati derecesi, Müller manevrası değerleri ile Epworth Uykululuk Skalası değerleri karşılaştırıldı. Ayrıca AS hasta grubunda Epworth Uykululuk Skalası ile hastalık süresi ve Bath Ankilozan Spondilit Hastalık Aktivite İndeksi (BASDAI) skorları ile ölçülen hastalık aktivitesi arasındaki ilişki değerlendirildi. BULGULAR: AS tanısı bulunan hastaların fizik muayene bulguları ve Müller manevrası değerleri kontrol grubu ile istatistiksel benzer olarak bulunmuştur. AS tanısı bulunan hasta grubunda Epworth Uykululuk Skalası değeri 8.45±3.73 iken, kontrol grubunda 3.72±2.34 olarak bulunmuştur ve istatistiksel olarak anlamlı fark saptanmıştır (p< 0.001). Epworth Uykululuk Skalası ile hastalık süresi ve BASDAI skorları arasında istatistiksel olarak anlamlı fark saptanmamıştır. TARTIŞMA ve SONUÇ: AS hastalarında, TUAS açısından yüksek risk olabileceği söylenebilir. Daha güvenilir sonuçlar için, objektif değerlendirme olan polisomnografi ile yapılan geniş hasta serili çalışmalara ihtiyaç vardır. ABSTRACT INTRODUCTION: OSAS can be seen more frequently in individuals with ankylosing spondylitis than in normal. Aim of the study is to investigate the presence of OSAS with Müller maneuver and Epworth Sleepiness Scale in patients with AS. METHODS: Patients with AS (n= 22) and healthy control subjects (n= 18) with similar sex and age distribution were evaluated. Head-neck examination was performed in both groups and the degree of palatine tonsil size, the degree of mallampati were recorded. In addition, Müller maneuver and the degree of obstruction of the hypopharynx and tongue root levels were recorded in the flexible fiberoptic examination. Epworth Sleepiness Scale was applied to all participants to determine the presence of OSAS. The values of palatine tonsil size, mallampati degree, Müller maneuver and Epworth Sleepiness Scale were compared between patient and control groups. In addition, the relation between Epworth Sleepiness Scale and disease duration and disease activity measured by BASDAI scores were evaluated in AS patient group. RESULTS: The results show that AS and healthy controls do not differ statistically in physical examination findings and the Müller maneuver values. Objective: To investigate theeffect of ageing on voice by comparing geriatric subjects with a control group.Materialsand methods: The geriatric group (> 65 years of age)included 34, and the control group included 35 subjects. Objective andsubjective analysis of voice were performed in both groups. Acoustic voiceanalysis (F0, jitter % and shimmer %) was done for objectiveanalysis, and VHI -30 questionnaire was used for subjective analysis. Thegroups were compared for F0, jitter %, shimmer % and VHI-30 scores. Results:Mean F0 was 186.14±43.13 in the geriatricgroup, and 160.74±25.76 in the control group, with a statistically significantdifference in between (p= 0.004). Mean jitter % were 0.61±0.34 and 0.68±0.34 inthe geriatric and the control groups, respectively (p= 0.417). Mean shimmer %was found as 0.70±0.27 in geriatric, and 1.03±0.47 in the control groups, andthe difference between two groups was statistically significant (p= 0.001).Mean VHI -30 score was 10.85±4.11 in geriatric subjects, and 1.37±2.45 in thecontrols, and the difference was statistically significant (p< 0.001).Conclusion: Subjective voice analysis showed that voice performance was deterioratedin geriatric population compared to the controls. However, objective parametersof the geriatric patients were better than the controls. Objective: Inflammation has an important place in chronic otitis media with effusion (COME) etiology. Neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) are simple and cheap tests that show inflammation and can be calculated by all physicians. In our study, we aimed to investigate the relationship between COME and NLR and PLR parameters.Methods: The study was performed with the pediatric patient group consisting of 77 patients who got diagnosis of COME and the control group consisting of 62 healthy children whose age, gender and demographical characteristics were concordant with the patient group. research Amaç: Bu çalışmada tıkayıcı uyku apnesi sendromu (TUAS) hastalarında hastalık şiddetinin ve sürekli pozitif havayolu basıncı (SPHB) titrasyonunun evre 3 ve REM (rapid eye movement) uykusu üzerine etkisi araştırıldı.Hastalar ve Yöntemler: Çalışmaya polisomnografi (PSG) ile orta veya ağır TUAS tanısı konulan 114 hasta (87 erkek, 27 kadın, ort. yaş 51.1±10.5 yıl; dağılım 34-66 yıl) dahil edildi. Apne hipopne indeksi ve uyku evrelerinin yüzde değerleri ayrı ayrı istatistiksel olarak karşılaştırıldı. Hypopharyngeal injury due to trauma is extremely rare. Hypopharyngeal trauma is divided in two groups; blunt and penetrating trauma. Patients usually present to the hospital with swallowing disorders, pain and dysphonia. Clinical findings on physical examination are mucosal edema, discoloration, hemorrhagic areas, hematoma, stridor and subcutaneous emphysema. The airway should be secured in patients with hypopharyngeal trauma and surgical interventions should be performed if necessary. In this article; we present a 56 year old schizophrenic female patient who presented to our clinic with dysphagia , the patient had tried to commit suicide the previous week and was suspected of having a hypopharyngeal tumor., Hipofarenks travması oldukça nadir görülür. Hipofarenks travmaları künt ve penetran travma olarak ikiye ayrılır. Hastalar genellikle yutma güçlüğü, ağrı ve ses değişiklikleri ile hastaneye başvururlar. Fizik muayenede mukozada ödem, renk değişikliği, kanamalı alanlar ve hematom tespit edilebilir. Ayrıca stridor ve subkutanöz amfizem de görülebilir. Hipofarenks travması olan hastaların takibinde solunum yolu güvenliği sağlanmalı ve gerekirse cerrahi müdahaleler yapılmalıdır. Hastamızda cerrahiye gereksinim duyulmadı. Bu yazıda psikiyatri kliniğince şizofreni tanısı ile takip edilen ve 1 hafta önce özkıyım girişimi sonrasında yutma güçlüğü şikayeti gelişen, hipofarenks malignitesi şüphesi ile konsülte edilen 56 yaşında kadın hasta literatür eşliğinde sunulmuştur.

Amaç: Bu çalışmada nazal polip NP ve NP yoğunluğu ile nötrofil lenfosit oranı NLO ve trombosit lenfosit oranı TLO değerleri arasındaki ilişki araştırıldı.Hastalar ve Yöntemler: Çalışmaya NP tanısı konulmuş 105 hasta NP grup 72 erkek, 33 kadın; ort. yaş 41.33±12.85 yıl; dağılım 16-63 yıl ile 83 sağlıklı birey 54 erkek, 29 kadın; ort. yaş 44.01±8.50 yıl; dağılım 18-62 yıl kontrol grubu dahil edildi. Nazal polip yoğunluğu skoru ameliyat öncesi Lund-Mackay bilgisayarlı tomografi skorlama sistemi ile değerlendirildi. NP ve kontrol grubunun NLO ve TLO değerleri hesaplandı ve istatistiksel olarak karşılaştırıldı. NP grubunda NLO ve TLO değerleri NP yoğunluğuna göre istatistiksel olarak karşılaştırıldı.Bulgular: Ortalama NLO değeri NP grubunda 2.26, kontrol grubunda ise 1.75 idi ve bu farklılık istatistiksel olarak anlamlıydı p=0.001 . Ortalama TLO değeri NP grubunda 120.79, kontrol grubunda ise 109.84 idi fakat bu farklılık istatistiksel olarak anlamlı değildi p=0.073 .Sonuç: Nazal polipli hastalarda kolay uygulanabilen ve düşük maliyetli testler ile elde edilen NLO değeri yeni bir belirteç olarak kullanılabilir. Trombosit lenfosit oranı değeri için ise daha geniş hasta serili yeni çalışmalara ihtiyaç vardır, Objectives: This study aims to investigate the correlations between nasal polyposis NP and NP density with neutrophil to lymphocyte ratio NLR and platelet to lymphocyte ratio PLR . Patients and Methods: The study included 105 patients 72 males, 33 females; mean age 41.33±12.85 years; range 16 to 63 years diagnosed as NP NP group and 83 healthy individuals 54 males, 29 females; mean age 44.01±8.50 years; range 18 to 62 years control group . Nasal polyposis density score was calculated with preoperative Lund-Mackay computed tomography grading system. Neutrophil to lymphocyte and PLR ratio values of NP and control groups were calculated and statistically compared. Neutrophil to lymphocyte and PLR values were statistically compared in terms of NP density in the NP group. Results: Mean NLR value was 2.26 in the NP group and 1.75 in the control group with a statistically significant difference p=0.001 . Mean PLR value was 120.79 in the NP group and 109.84 in the control group with a statistically insignificant difference p=0.073 . Conclusion: Neutrophil to lymphocyte ratio value may be used as a novel marker that is easily administered in patients with nasal polyps and obtained with low-cost tests. New studies with larger patient series are needed for the value of PLR.

Burun tıkanıklığı, burun akıntısı ve burun kanaması bu tümörlerin en yaygın semptomlarıdır. Tümörün yerleşim yerine ve büyüklüğüne bağlı olarak, yutma güçlüğü ve ses kısıklığı gibi nadir semptomlara da rastlanabilir. Bu yazıda üst konkadan köken alan, epiglota kadar uzanan ve yutma güçlüğü ve nefes darlığına yol açan sıra dışı dev bir sinonazal inverted papillom olgusu sunuldu, Sinonasal inverted papillomas are benign tumors of the nasal cavity and paranasal sinuses. The basic characteristics of these papillomas are their different localizations in the nasal cavity, local aggressive behavior and potential to transform into malignant tumors. The most common symptoms of those tumors are nasal obstruction, rhinorrhea, and epistaxis. Rare symptoms such as dysphagia and hoarseness may be seen in relation with the localization and the size of the tumor. In this article, we present an unusual case of a huge inverted papilloma originating from the superior turbinate, extending down to epiglottis, and causing dysphagia and dyspnea.

Patients and Methods: This retrospective study included 417 OSAS patients 284 males, 133 females; mean age 47.9±10.0; range 23 to 75 years who were performed polisomnography PSG in our clinic between January 2013 and December 2014.
