Introduction. Microlearning is one of the modern trends in education, providing high motivation and involvement of students, fast and solid assimilation of the specified educational results, the connection of theory and practice, accessibility and flexibility of learning. At the same time, microlearning also has a number of serious drawbacks, primarily related to its content limitations, fragmentation and unsystematic nature, which prevents its widespread use at various levels of education. According to the authors of the article, these shortcomings are due to the peculiarities of a specific organisational and didactic model of microlearning used in corporate education. Attempts to directly transfer this approach to the system of general, secondary vocational, higher education are unproductive. The authors believe that it is necessary to develop a different didactic model of microlearning, adequate to the goals and approaches of the education system. The article presents a range of issues that are subject to priority resolution for the development of the theory of education, didactics and methodology in the context of the idea of microlearning. Aim. The present research aims to present a scientific approach to assess the possibilities of adapting the accumulated experience of using microlearning in mass educational practice.Methodology and research methods. The current research is based on methodological approaches of foreign and Russian scientists to the problems of digital transformation of education, primarily based on the idea of counter transformation of the educational process and digital tools, cultivation of systematic didactic knowledge. The leading method is the generalisation of scientific provisions, the analysis of practical experience and the creation of prerequisites for the creation of new knowledge.Results and scientific novelty. As a result of the research, the key directions of pedagogical scientific research are identified, ensuring continuity in the development of microlearning during its transfer from corporate education to use in mass educational practice. The novelty of the research is determined by the high demand for the development of didactic tools for organising the educational process, selecting and structuring the content of education.Practical significance. The research results can be applied in the process of training teachers for the broad implementation of microlearning during the digital transformation of education, as well as directly in the design of new digital tools.

The article reveals the life and activities of the Talantsevs merchants family in the pre-revolutionary Chuvashia. At the end of the 19th — beginning of the 20th century, along with entrepreneurship and commercial activities, the Talantsev brothers were also engaged in charity. Unlike many other merchants, who proved themselves primarily as benefactors in the development of temple construction and public charity facilities of religious type, members of the Talantsev family distinguished themselves in the enlightenment direction — in the construction of schools, hospitals, libraries, cultural and educational institutions in multinational Chuvashia, in the organization of agriculture and household life, in preservation and further development of the ethnoculture of Russian and Chuvash peoples. The Talantsevs were particularly active in promoting secular culture and books among the population. That was clearly expressed in their participation in the development of public, factory and private family libraries. The partially preserved library of the Talantsev brothers, located since 1954 in the structure of the National Library of the Chuvash Republic, is a rare example of the noble service of representatives of the merchants of Chuvashia to book culture, enlightenment and preservation of the ethnoculture of the peoples of the Middle Volga region. The historical experience and traditions of merchant families in organizing charity in the field of culture, interrupted in October 1917 and partially revived in the practice of modern entrepreneurs, can now be used in the educational process in all types of educational institutions, taking into account modern realities.

Установление советской власти на территории Российской империи привело к новой системе ценностей, активизации населения во всех областях общественной жизни. В сфере музейного строительства в 1920 гг. заметную роль сыграли такие общественные научно-просветительские объединения, как Общество изучения местного края в Чебоксарах (1921–1931) и Общество изучения чувашской культуры в Москве (1927–1931). Основной целью статьи является раскрытие истории возникновения и работы этих организаций. При изложении материала применялся хронологический подход. Установив тесные контакты с научными учреждениями по всей России и с известными учеными, краеведческие организации с широкой сетью своих филиалов и ячеек способствовали углубленному изучению и развитию экономики и культуры Чувашского края и духовной культуры чувашей, половина численности которых проживала вне территории автономии. Особое внимание эти общества обращали на развитие родного языка и культуры чувашского народа, что позволяло чувашам осознать свою национальную идентичность. В первое десятилетие чувашской советской автономии были достигнуты определенные успехи в становлении краеведения как науки. На территории Чувашии краеведческие учреждения и организации внесли значительный вклад в изучение природы, экономики, производительных сил края, а также духовной жизни народа. В указанный период именно Общество изучения местного края в Чебоксарах и Общество изучения чувашской культуры в Москве способствовали сохранению и развитию чувашского языка., The establishment of the Soviet power on the territory of the Russian empire led to a new system of values, activation of the population in all fields of the social life. In the sphere of museum building in the 1920s a noticeable role was played by such public scientific and educational associations as the Society for the Study of the Local Territory in Cheboksary (1921-1931) and the Society for the Study of Chuvash Culture in Moscow (1927-1931). The main aim of the article is to reveal the history of the formation and work of these organizations. The material is presented chronologically. Having established close contacts with scientific institutions throughout Russia and with famous scientists, the local history organizations with a wide network of branches and cells promoted an in-depth study and development of the economy and culture of the Chuvash region and the spiritual culture of the Chuvash, half of whose population lived outside the territory of the autonomy. The special attention these societies paid to the development of the native language and culture of the Chuvash people that allowed the Chuvash people to realize their national identity. During the first decade of the Chuvash Soviet autonomy some successes in the formation of local lore as a science were achieved. On the territory of Chuvashia local history institutions and organizations made a significant contribution to the study of nature, economy, productive forces of the region, as well as the spiritual life of the people. In this period it was the Society for the Study of the Local Territory in Cheboksary and the Society for the Study of Chuvash Culture in Moscow that promoted the preservation and development of the Chuvash language.

This article presents a case report of hereditary leiomyomatosis and renal cell cancer (HLRCC) with new mutation in a 25-year-old female patient admitted to the clinic for diagnosis and treatment due to multiple skin and uterus leiomyomas. The patient has a history of surgery to remove adrenal pheochromocytoma and papillary kidney cancer. Clinical and laboratory examination as well as medical genetic counseling of the patient were performed. We have detected the heterozygous c.395_399del (p.L132*) germline nonsense mutation in exon 4 of the FH gene using polymerase chain reaction/Sanger sequencing of exons 1–10 of this gene and confirmed the diagnosis of HLRCC. The mutation c.395_399del in a patient with HLRCC was described for the first time. The identical mutation was also found in the mother and sister of the patient. Based on the obtained results, medical genetic counseling was carried out in this family, recommendations were given for further oncological monitoring. The case report could be useful for geneticists, oncologists and other specialists to interpretate the clinical heterogeneity of HLRCC and improve the genetic diagnosis of this rare hereditary oncological syndrome.

The article is dedicated to the role of a native of Chuvashia, an outstanding learned monk, corresponding member of the Russian Academy of Sciences, founder of Russian sinology N.Ya. Bichurin (1777–1853) in the study and popularization of the ethnoculture of the Mongols in the XIX century. Mongol studies date back to the XIII century, the time of Genghis Khan, who created a vast and powerful Mongol Empire. It was a memoir of ambassadors and travelers, Western European Christian missionaries. In the XIX century, the center of world Mongolian studies was concentrated in Russia, where A.L. Leontiev, I.K. Rossokhin, Ya.I. Schmidt, O.A. Kovalevsky, A.V. Popov, I.P. Voytsekhovsky, P.Ya. Petrov and other orientalists were focusing on its problems. They studied languages, history, ethnography, geography of the peoples of China, Mongolia, Tibet, India, Kalmykia, etc. A significant trace in the study of the Mongolian language and ethnoculture was left by Professor of Kazan University O.M. Kovalevsky, a participant of expeditions to Transbaikalia and Mongolia (1828–1833), the author of grammar, anthology, and dictionaries of the Mongolian language. The Kazan school of orientalists included N.Ya. Bichurin (monk Hyacinth), the head of the IX Russian Theological mission in China (1807–1821), and a participant of ethnographic expeditions (1830–1831, 1835–1837). During 1828–1853 he published a number of fundamental works on Oriental studies, including on the ethnoculture of the Mongols and their neighbors: a translation of the “History of the Mongols”, the works “Notes on Mongolia”, “The History of the Four Khans from the Family of Genghis Khan”, “Historical Review of the Oirats or Kalmyks from the XV century to the present time”. He published more than a dozen articles in well-known Russian popular journals. Not all the judgments and conclusions of N.Ya. Bichurin on the ethnogenesis of some Eastern peoples are shared by modern scientists. Nevertheless, his works on China, Mongolia, Tibet, Kalmykia, the Uyghur territories to a certain extent helped the Russian state to learn about the internal and external problems of the Eastern countries and peoples, their original ethnic culture. The learned monk was sympathetic to the peoples of the East and predicted our rapprochement in the historical future. Laureate of four Demidov Prizes, Corresponding Member of the Russian Academy of Sciences N.Ya. Bichurin received well-deserved recognition during his lifetime. Our contemporaries, residents of Chuvashia, with respect and pride treat the heritage of the outstanding countryman N.Ya. Bichurin. Settlements, streets, museums are named after him, monuments and busts are built in his honor, state and public awards (order, medal, pennant, prizes) are established.

—The analysis of the local earthquake catalog revealed fine features in the behavior of seasonal components of induced seismicity in the Koyna–Warna region, western India. The seasonal variations in seismicity associated with annual fluctuations in water levels in the Koyna and Warna reservoirs have local maxima during the year, corresponding to the known mechanisms of immediate and delayed responses of reservoir-triggered seismicity (RTS). The superposed epoch analysis combined with the stochastic modeling of random earthquake catalogs revealed regular changes in the b-value of the frequency–magnitude relationship of the earthquakes within the annual cycle of seasonal seismicity fluctuations. The minimum b-values fall in the intervals of the maxima of both the immediate and delayed seismic responses. The maximum b-values are observed during the rise and fall phases of water levels in the reservoirs. The pattern of changes in the seismicity together with the b-value behavior at the activation phase of the immediate RTS response is consistent with the scenario of gradual redistribution of the failure process from the lower to higher scales, characteristic of the activation of transient seismicity. A pilot laboratory experiment on cyclic initiation of fracture process by pore pressure in a granite sample extracted from a well in the Koyna–Warna region of reservoir-triggered seismicity demonstrated the patterns of changes in the acoustic regime similar to the seasonal features of the Koyna–Warna region seismicity.

Objective: to analyze histological structure and identify predictors of detecting malignant non-seminoma in residual tumor masses obtained from patients with testicular non-seminomatous germ cell tumors (TNSGCTs) who had not achieved complete serological and radiological response to chemotherapy (CT).Materials and methods. This study included 96 out of 703 patients with TNSGCTs (13.7 %) operated on in N.N. Blokhin National Medical Research Center of Oncology. The inclusion criteria were as follows: verified advanced TNSGCT, elevated levels of alpha-fetoprotein and/or chorionic gonadotropin at the moment of CT initiation, at least 3 completed courses of first-line or second-line platinum-based CT, residual tumor foci after CT visualized with radiological methods, alpha-fetoprotein >7.29 IU/mL or chorionic gonadotropin >5 mIU/mL 3 weeks after the initiation of the last CT course, and surgery after CT. Histological examination of the primary tumors demonstrated that they contained elements of seminoma (n = 14; 14.6 %), teratoma (n = 29; 30.2 %), choriocarcinoma in (n = 23; 23.9 %), embryonal carcinoma (n = 45; 46.9 %), and yolk sac (n = 18; 18.8 %). All study participants received first-line CT; 58 of them (60.4 %) also received second-line CT. All patients underwent surgery after CT, including retroperitoneal lymph node dissection (RPLND) (n = 96; 100 %) and excision of extra-retroperitoneal lesions (n = 8; 8.3 %).Results. Histological examination of excised retroperitoneal masses showed that they contained areas of necrosis and fibrosis (n = 25; 26.0 %), teratoma (n = 29; 30.2 %), and viable malignant non-seminoma (n = 42; 43.8 %). There was a strong positive correlation between the existence of residual malignant non-seminomatous components in retroperitoneal masses and presence of choriocarcinoma (r = 0.300; р = 0.004), as well as the absence of embryonal carcinoma in the primary tumor (r = –0.300; р = 0.004), invasion of retroperitoneal metastases into major vessels and/or adjacent organs (r = 0.243; р = 0.017), and second-line CT prior to RPLND (r = 0.413; р Conclusion. Malignant non-seminomas were detected in 43.8 % of retroperitoneal and 62.5 % of extra-retroperitoneal residual tumorsremoved after CT in patients with advanced TNSGCTs and incomplete serological and radiological response. Discordant structure of metastases at different locations was observed in 50 % of patients. Our finding can be used to select candidates for surgical excision of residual tumors among these patients.

The article presents a review of the literature on the clinical aspects of assessing vitamin D deficiency in young children by the concentration of 25(OH)D (hydroxycalciferol) in blood serum. The purpose of the review was to familiarize pediatric specialists with the real state of affairs in assessing the clinical significance of diagnosing vitamin D status, its relationship with the prevention of deficient rickets, ways of correcting and choosing the dose of calciferol. A daily dose of 400 IU of vitamin D for young children is effective and safe in preventing deficient rickets. Higher subsidized doses of calciferol have not been shown to be more effective. In addition, they can potentially lead to toxic levels of vitamin D metabolites in the blood. When using lower daily doses (less than 400 IU), an adequate prophylactic effect may not be achieved. Determination of the level of circulating serum hydroxycalciferol, which characterizes the status of vitamin D in the body, is not recommended for routine examination and as a standard for diagnosing deficient rickets in young children. Calciferol has multilateral effects, modulates not only phosphorus-calcium metabolism, but also affects other systems and functions of the body, in particular, ontogenesis and the immune system. According to foreign literature, all infants should receive vitamin D for the prevention of rickets, treatment from the age of one month. This is most reliably identified for children, probably at risk. Convincing data indicating a positive protective effect on diabetes mellitus D on unforeseen pathology, for example, the frequency of exclusion of pneumonia, infectious diarrhea, atopic dermatitis in infancy, has not yet been obtained.

Background: Recurrent ovarian cancer (OC) patients with platinum‑free interval (PFI) < 6 mo. are usually considered platinum‑resistant and treated with non‑platinum based chemotherapy. There is a lack of evidence to compare various non‑platinum therapeutic options in patients with platinum‑resistant ovarian cancer (PROC). We conducted meta‑analysis to assess efficacy of various therapeutic options in PROC patients. This article aimed to compare non‑platinum therapeutic regimens.Methods: We queried the PubMed database for full‑text articles and abstracts on the treatment of PROC patients (01/01/2000–01/06/2019 timeframe). Inclusion criteria were: 1) epithelial ovarian cancer; 2) recurrent disease ≤ 6 mo. after platinum‑based chemotherapy; 3) standard therapy with platinum‑ or non‑platinum agents; 4) no targeted therapy, investigational agents or non‑platinum doublets; 5) reported response rate (RR) and assessment criteria. Proportion meta‑analysis (random‑effect model) and beta‑regression were conducted to assess efficacy of various options. Statistical analysis was dose with meta and betareg packages of R software.Results: We identified 7156 articles and screened them for title and abstract, 157 studies were left for further analysis. Efficacy of non‑platinum‑ and platinum‑based therapy was assessed in 113 (n = 5272) and 44 (n = 1055) trials respectively, only the latter were included in this analysis. In meta‑proportion random‑ effect model RR among patients treated with taxanes, etoposide, doxorubicin/PLD, topotecan and gemcitabine were 27 % (95 % CI 0.21–0.34), 19 % (95 % ДИ 0.13–0.27), 15 % (95 % CI 0.11–0.19), 13 % (95 % CI 0.10–0.18) and 10 % (95 % CI 0.07–0.14), respectively. Multiple beta‑regression model accounting for response assessment method, number of prior therapy lines and other factors confirmed the results.Conclusions: Taxanes monotherapy may be the most effective therapeutic option for patients who are not candidates for platinum‑based chemotherapy.

This paper presents the results of theoretical and experimental studies of a surface-wave oscillator (SWO) of planar geometry excited by a ribbon high-current relativistic electron beam. Within the framework of the quasi-optical approach and direct three-dimensional particle-in-cell modeling, we demonstrate the advantages of open transverse edged configuration against the closed one for effective mode selection at a fairly large oversize factor. In the experiments carried out on the basis of the SINUKI accelerator (Institute of Applied Physics of the Russian Academy of Sciences, Nizhny Novgorod, 1 kA/650 keV/17 ns), we form a magnetically guided ribbon electron beam with a cross-section of 0.3 × 20 mm2, which moves parallel to a slow-wave structure with a period of 1.75 mm. A planar W-band SWO both of open and closed edged configuration has been experimentally tested. In full agreement with modeling, the open configuration exhibits much more stable operation, where at a frequency of 75 GHz, we observe pulse generation with duration of about 7 ns. The output power measured by the calorimetric method reaches 25 MW.

Introduction: local treatment of metastases is an integral part of colon cancer treatment. However, there is not enough data on the efficacy of surgical resection of metastases in patients with a BRAF gene mutation to recom‑mend this approach in routine practice. We initiated a retrospective multicenter study to assess the incidence of BRAF gene mutations in patients with metastatic colon cancer and to study the efficacy of metastasectomy in this group of patients.Materials and methods: we selected all patients who underwent surgical resection of metastases in various sites from the database of patients with BRAF gene mutations created as a result of a multicenter retrospective study with participation of 7 clinics in the Russian Federation. All 57 patients with RAS gene mutations and 43 patients with wild‑type RAS and BRAF genes who also underwent surgical resection of metastases at any stage of treatment were selected from the register of the Chemotherapy Department No. 2 of the NMRC of Oncology named after N. N. Blokhin for comparative analysis. Disease‑free survival and overall survival were used as primary efficacy criteria.Results: we found 26 patients with BRAF gene mutations who underwent surgical resection of metastases. When comparing disease‑free survival, the worst median was achieved in the group of patients with BRAF gene mutations: 7 months versus 14 months in patients with RAS gene mutations (HR 0.4, 94 % CI 0.23–0.7, P = 0.006); median disease‑free survival was not achieved in the wild‑type RAS and BRAF group (HR 0.2, 95 % CI 0.11–0.45, P The median overall survival in the BRAF gene mutation group was 26 months versus 38 months in the RAS gene mutations group (HR 0.8, 95 % CI 0.33–1.98, P = 0.6) and 49 months in the wtRAS/wtBRAF group (RR 0.46, 95 % CI 0.17–1.24, P = 0.1). Resection of recurrent tumors in patients with metastases in retroperitoneal lymph nodes was associated with extremely low disease‑free survival (2 months); at the same time, disease‑free survival was 7 months after resection of isolated metastases in the liver and 8 months for metastases in the peritoneum.Conclusion: prognosis of patients with a BRAF gene mutation after surgical resection of metastases is worse than in patients with a different mutation phenotype. Nevertheless, literature data, as well as the results of our study, confirm the possibility of performing metastasectomy with careful selection of patients.

The article presents the results of a study of the immunogenicity and reactogenicity of the vaccine Gam-COVID-Vac (Sputnik V) when used in military personnel undergoing military service on conscription. From 300 military personnel consistently vaccinated with one and two components of Gam-COVID-Vac at the intervals of 21 days, blood serum was obtained and examined three times: before vaccination, and 30 and 60 days after the introduction of the first component of the vaccine. In the blood serums, the content of Class G antibodies to the SARS-CoV-2 was determined by the method of solid-phase enzyme immunoassay. After immunization with the Gam-COVID-Vac vaccine, the average geometric titer of Class G antibodies to SARS-CoV-2 -in the blood serum of a military personnel obtained during the second and third examinations (5.02 log2 and 5.67 log2) increased by 2.4 and 2.7 times, respectively (p 0.05), compared to the same indicator before the vaccination (2.11 log2). Total of 30 days after the introduction of the first component of the vaccine (Nine days after the introduction of the second component of the vaccine), Class G antibodies to the new coronavirus SARS-CoV-2 were detected in the 86.7% of military personnel, and after 60 days in 92% of vaccinated. Studies have revealed moderate reactogenicity of the vaccine. Moreover, the proportion of postvaccination reactions in the first 35 days after the introduction of the second component of the vaccine was less after the introduction of the first component of the vaccine. So, if after the introduction of the first component of the vaccine, an increase in body temperature 37 C was observed in 20% of military personnel, then after the introduction of the second component only in 9%, and the share of local reactions decreased from 94%. There have been no cases of serious adverse events after immunization of military personnel with the Gam-COVID-Vac vaccine.

Study design Retrospective analysis of a single-center cohort of patients with pyogenic vertebral osteomyelitis. The purpose of the study was to review risk factors for perioperative complications and recurrences in patients diagnosed with hematogenous pyogenic vertebral osteomyelitis. Material and methods 141 inpatient records of hematogenous pyogenic vertebral osteomyelitis were reviewed at the Regional Clinical Hospital No. 2, Tyumen. Patients who developed complications, neurological deficits and recurrences were assigned to A group (n = 66), and those who did not included in B group (n = 75). Results Perioperative complications, recurrences, risk factors and neurological disorders that could develop with different treatment methods were explored. There was a greater risk of recurrences and a lower rate of neurological complications seen in chronic cases and drug addicts. There was a lower risk of intra- and postoperative complications seen in thoracic spine involvement, a greater risk of neurological deficit with involved cervical spine and a lower risk in the lumbar spine involvement. There was a lower risk of neurological disorders in type A and type B disorders, and a greater risk in type C disorders as categorized by E. Pola. There were no differences in recurrence rates among different types of involvement. HIV did not increase the risk of complications and relapses. Surgical site infection developed in 8.1 % cases of transpedicular fixation and in 6.9 % following ventral interventions. No correlation was observed between the recurrence and complication rate and the surgical methods used. Overall, recovery or stable remission was achieved in 88.6 % of cases after treatment of relapses. Conclusion Recurrences and complications were primarily caused by delayed diagnosis, co-morbidities and neurological deficits. No correlation was observed between the recurrence and complication rate and the surgical methods used. There was a greater risk of recurrences and a lower rate of neurological complications seen in chronic cases and drug addicts. Neurological disorders are common for cervical spine disorders. The risk of complications was dependent on the type of involvement as categorized by E. Pola. There was a lower risk of neurological disorders in type A and B disorders and a statistically greater risk in type C disorders.

The authors of the article have summarized the experience of multilateral well application, performed an efficiency analysis of multilateral wells vs horizontal wells in Rosneft Oil Company's fields with various subsurface architecture. The algorithm for multilateral well efficiency estimation, compared to other type of well completions, was developed. This algorithm is based on the selection of areas for well locations with similar reservoir properties, reservoir energy conditions, and reservoir development conditions to evaluate production startup parameters, decline rates, cumulative parameters for the areas of over 6 month production. A matrix of multilateral well applicability in various geological conditions was generated, and recommendations for preferable well design were made. This type of analysis was conducted for the first time due to collection of sufficient statistical data, because of a multiple increase in the amount of drilling complex wells in the recent years. The obtained results provide an opportunity to design an efficient field development system for new assets, perform an adjustment of brownfields development systems, select multilateral well design for certain geological conditions based on lessons learned.

The complexity of implementation of a threshold symmetric n-place Boolean function with threshold k = O(1) via circuits over the basis {∨, ∧} is shown not to exceed 2 log2 k ⋅ n + o(n). Moreover, the complexity of a threshold-2 function is proved to be 2n + Θ( $\begin{array}{} \sqrt n \end{array} $ ), and the complexity of a threshold-3 function is shown to be 3n + O(log n), the corresponding lower bounds are put forward.
