The results of the metagenomic analysis revealed a high phylogenetic and taxonomic diversity of the microbial community of both luminal and mucosal microflora of the bird intestinal microbiome. Analysis of the taxonomic structure of the compared microbiomes showed both similarities and differences in the taxonomic diversity and relative abundance (%) of bacteria of the compared microbiomes at the level of order, class, genus.At the type level, 24 taxonomic categories were identified in the luminal and mucosal intestinal microbiome of birds, at the class level 35, at the order level 75, at the family level 168, at the genus level 350.The main share of the microbial community of the luminal and mucosal microbiome was made up of microorganisms belonging to the Firmicutes, Proteobacteria, Actinobacteria Cyanobacteria, and Bacteroidetes phyla. Comparison of the biological diversity of the colonic and mucosal microbiomes at the phylum level showed that the relative value of Proteobacteria in relation to Firmicutes in the mucosal microbiome was 3.25 times higher, and the ratio of Firmicutes to Bacteroidetes in the colonic microbiome was 3.98 times higher. than in mucosal.At the genus level, differences in the composition of microbiomes were manifested by a greater relative share (in decreasing order) in the luminal microbiome of representatives of the genus Lactobacillus, Candidatus Arthromitus, Romboutsia, Gallibacterium, Campylobacter, Enterococcus, and representatives of the genus Acinetobacter, Staphylococcus, Bacillus and Bradyrhizobium in the mucosal microbiome. An in-depth study of the intestinal microbiota using cultivation techniques and high-tech methods for identifying microorganisms, taking into account the results of metagenomic analysis, can significantly expand the understanding of the species diversity of microbiomes and the functions of representatives of various taxa in maintaining bird health, as well as to form a census of representatives of normal bird microflora.

Titin-truncating variants (TTNtv) are the single largest genetic cause of dilated cardiomyopathy (DCM). The per acre yield of chili in Pakistan is quite low as compared to large producers of this vegetable in the world and can be attributed to an assortment of factors both biotic as well as abiotic. Among biotic factors, chili is susceptible to damping off caused by Pythium spp. and causes lots of damage in Pakistan. There is scantly information on the incidence and severity of Pythium associated with chili. Therefore, the current studies were carried out to determine the incidence and severity of Pythium in the Pothwar region of Pakistan. An overall incidence of 14.08% of Pythium damping off was recorded on chili in Pothwar. Attock showed the maximum incidence of 19.86% followed by Rawalpindi and Chakwal districts having 16.22% and 10.22% disease incidences respectively. Jhelum had the minimum disease incidence of 10.05%. Disease severity was the maximum in district Attock (11.03%) followed by Rawalpindi (9.11%), and was the minimum in Chakwal (5.42%) followed by Jhelum (5.67%). The finding of these studies will be helpful for the farmers in designing control strategies for this pathogen accordingly.

Context To achieve maximum growth and decrease fattening periods farmers tend to use a high concentrate diet in the total mixed ration (TMR). A larger particle size of straw may increase the concentrate level in the TMR and improve the performance without affecting the rumen health of lambs. Aims This study aimed to evaluate the effects of physical form (PF) and wheat straw levels (SL) in the diet on growth performance, rumen health, and carcass characteristics of lambs. Methods Thirty-two male Lohi lambs (31.5 ± 2.5 kg, age 6–7 months) were randomly assigned to four treatments in a 2 × 2 factorial arrangement with two physical forms [pelleted, conventional mashed] and two levels of straw (15% and 25%). The experiment lasted for 16 weeks including 2 weeks for dietary adaptation, 13 weeks for growth data collection, and 1 week for digestibility. Key results Lambs fed pelleted and 15% straw diets had greater dry matter intake than those offered mashed and 25% straw diets. Lambs fed 15% pelleted straw TMR displayed a greater final bodyweight, average daily gain, and feed efficiency than the other treatments. Dry matter and organic matter digestibility were higher in lambs fed on the pelleted and 15% straw diets compared to those fed on the mashed and the 25% straw diets. Aim Patients with heart failure with reduced ejection fraction and low systolic blood pressure (SBP) have high mortality, hospitalizations, and poorly tolerate evidence-based medical treatment. Omecamtiv mecarbil may be particularly helpful in such patients. This study examined its efficacy and tolerability in patients with SBP ≤100 mmHg enrolled in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC-HF). Methods and results The GALACTIC-HF enrolled patients with baseline SBP ≥85 mmHg with a primary outcome of time to cardiovascular death or first heart failure event. In this analysis, patients were divided according to their baseline SBP (≤100 vs. >100 mmHg). Among the 8232 analysed patients, 1473 (17.9%) had baseline SBP ≤100 mmHg and 6759 (82.1%) had SBP >100 mmHg. The primary outcome occurred in 715 (48.5%) and 2415 (35.7%) patients with SBP ≤100 and >100 mmHg, respectively. Patients with lower SBP were at higher risk of adverse outcomes. Omecamtiv mecarbil, compared with placebo, appeared to be more effective in reducing the primary composite endpoint in patients with SBP ≤100 mmHg [hazard ratio (HR), 0.81; 95% confidence interval (CI), 0.70–0.94] compared with those with SBP >100 mmHg (HR, 0.95; 95% CI, 0.88–1.03; P-value for interaction = 0.051). In both groups, omecamtiv mecarbil did not change SBP values over time and did not increase the risk of adverse events, when compared with placebo. Conclusion In GALACTIC-HF, risk reduction of heart failure outcomes with omecamtiv mecarbil compared with placebo was large and significant in patients with low SBP. Omecamtiv mecarbil did not affect SBP and was well tolerated independent of SBP values.

In order to effectively describe land cover, scientists, academics, and researchers developed machine learning classification algorithms. According to studies, these classification techniques perform better than more tried-and-true conventional techniques. The primary aim of this project is to determine the most effective strategy for categorizing land cover in order to retrieve data from Wasit. The Maximum Likelihood Classifier (MLC), which is based on the neighborhood function, and the Support Vector Machine (SVM), which is based on the ideal hyper-plane function, are two supervised classification techniques that are contrasted using Sentinel-2 data. Four land cover classes have been chosen for this optimization. Four spatial layers of the research region were surveyed with the aim of collecting and providing field-based training samples. The error matrix and kappa statistics have been used to evaluate the accuracy of each classifier. Results demonstrated that SVM performs superior to MLC. SVM and MLC have overall accuracies of 99.79 and 99.60%, respectively, and kappa coefficients of 0.997 and 0.994.

IntroductionChromium (Cr) is an essential mineral that has been demonstrated to enhance milk production in dairy cows. This study aims to evaluate the effects of dietary Cr supplementation on dry matter intake (DMI), milk production and composition using a meta-analysis based on existing literature.MethodsA random effects meta-analysis was performed to investigate the effects of dietary Cr supplementation on DMI, milk production and composition. The heterogeneity was assessed using the I2 statistic and Q test, while Egger's test was used to evaluate publication bias.ResultsThe meta-analysis discovered that Cr-supplemented cows had a significantly higher DMI compared to those not supplemented, with an increase of 0.72 kg/day [95% confidence interval (CI), 0.46–0.97]. The regression model indicated that DMI significantly increased by 0.9 g/kg of body weight (BW) and by 80.5 g for an increase of 1 mg of Cr supplement. The supplementation phase was associated with an increase in DMI, with an increase of 0.4582 kg/day for BFP (before parturition) and 0.853 kg/day for AFP (after parturition). The methionine and yeast forms of Cr increased DMI by 0.714 and 1.137 kg/day, respectively. The DMI was increased by 2.137 and 0.620 kg/day for multiparous (MP)+ primiparous (PP) cows and MP cows, respectively. Milk production was also increased by Cr supplementation, with an increase of 1.20 kg/day (95% CI, 0.65–1.76). The regression model indicated that milk production increased by 2.3 g/day for an increase of 1 kg of BW and by 122.4 g/day for an increase of 1 mg of Cr supplement. Milk production also increased with the duration of the experiment and days in milk. The amino acid and methionine forms of Cr complexes increased milk production by 1.645 and 1.448 kg/day, respectively. Milk production increased by 1.087 and 1.920 kg/day for MP and PP cows, respectively. Milk composition was not significantly affected by Cr supplementation. Egger's test for publication biases was not significant for all responses of interest.DiscussionThe meta-analysis showed that Cr supplementation improves DMI and milk production in dairy cows. The results suggest that the supplementation phase, form of Cr, and parity should be considered when supplementing dairy cows with Cr. The results have important implications for the dairy industry and can contribute to the development of more effective feeding strategies for dairy cows.

Bacterial canker disease caused by the pathogen Pseudomonas syringae pv. syrinagae (Pss) has been spread widely in Pakistan, in Punjab and KPK provinces specially in strone fruits like apricot, Peach and Plum. During the 2014–2015 growing period, diseased samples were obtained from peach, plum and apricot trees showing bacterial canker symptoms in Punjab and KPK provinces and 48 Pseudomonas syringae isolates were obtained. The main aim of this research study was to find out host resistance in the cultivars of Apricot, Peach and Plum against bacterial canker caused by Pss in Peach, Plum and Apricot cultivars grown in Punjab and KPK province of Pakistan. In a pathogenicity test, three Pss isolates (PS3, PS9 and PS17) were found to be highly virulent in peach, plum and apricot and were tested for cultivar resistance to Pss. Leaves and shoots of five varieties of Peach Early grand, Florida king, 4 ½, 5 ½, 6 ½, Four varieties of Plum Red beauty, Fazal-e- manani, Stanley, Producer and Two varieties of Apricot Chinese apricot and golden amber were foliar sprayed with a mixed culture of Pss having concentration of 10-8 cfu ml−1 . Sprayed cultivars were covered with plastic bags for 3 days for retaining moisture and kept in glass house and were keenly observed for appearance of symptoms. 5 ½ of peach, Fazal-e-manani of plum and Chinese apricot of apricot were found resistant while 6 ½ of peach, Stanley of plum were found susceptible and Golden amber of apricot was found moderately susceptible to Pss. This is the first report of host resistance response of apricot, peach and plum cultivars grown in Pakistan to Pss.

Background: HPV 16 is the primary etiologic agent of cervical cancer and the presence of L1 and E6 oncoproteins are largely responsible for its virulence. It was the objective of this study to identify HPV16 isolates from suspected cases of cervical cancer at Specialist Hospital Sokoto and Sir Yahaya Memorail Hospiatal Birnin Kebbi, Nigeria and also to identifypotent HPV16’s L1 protein inhibitor using in silico analysis. Methods: A total of 144 cervical samples consisting of 21 low grade squamous intraepithelial lesion, 6 high grade lesion and 117 negative pap smears were collected. The samples were subjected for molecular detection using PCR targeting E6 gene of the virus. Data generated for the molecular prevalence was statistically analyzed using Chi-square method. AutoDock Vina was used to carry out the molecular docking between 2hr5 and Chicoric acid, curcumin and Echinacoside. Results: Out of the 144 samples, 24 samples were positive for the PCR representing 16.9% molecular prevalence rate. There is statistically significant association between cyto-diagnoses and presence of HPV16 (P < 0.05). Docking analysis showed that the Chicoric acid components of Echinacea purpurae have strong binding affinity (-8.7 kcal/mol) to the L1 protein of the HPV. Conclusion: This study provides data on HPV 16 epidemiology in northern Nigeria, and also provides novel evidence for consideration on certain interacting residues, when synthesizing Anti-HPV compounds in the wet lab. Relevance. The use of antibiotics to treat infections of the reproductive tract in cows can cause bacterial resistance to antibiotics and endanger human health. Potentially, preparations from representatives of the lactic acid vaginal microflora (probiotics) can be used as a biological control to reduce the use of antibiotics in animal husbandry.However, the specific composition of the vaginal lactic acid microflora of healthy dry cows and its probiotic properties remain poorly documented.Methods. The species identification of catalase-negative gram-positive isolates of bacteria forming colonies on MRS agar isolated from utero-cervical mucus of healthy lactating cows was carried out in accordance with standard procedures using a culturedependent approach, biochemical tests and tests for sugar fermentation using API50 CHL strips (bioMеrieux, France). For testing for antagonistic activity by the method of delayed antagonism against test cultures of staphylococci and Escherichia, 14 isolates were selected in accordance with typing assigned to 5 species of the genus Lactobacillus.Results. The results obtained indicate that the utero-vaginal biocenosis of healthy lactating cows is represented by microorganisms of the Lactobacillaceae family, including lactic acid enterococci and bacteria of the Leuconostocaceae family. The presence of representatives of the genera Aerococcus, Peptostreptococcus and Gardnerella vaginalis was found in the composition of the vaginal biocenosis in cows without signs of pathology of the birth canal. According to the results obtained, the vaginal population of lactic acid bacteria of the genus Lactobacillus has low or medium antagonistic activity against test cultures of indicatory strains of staphylococci and Escherichia.

Hypertrophic cardiomyopathy (HCM) is the most prevalent inherited cardiac disease in humans and cats and lacks efficacious pharmacologic interventions in the preclinical phase of disease. LV outflow tract obstruction (LVOTO) is commonly observed in HCM-affected patients and is a primary driver of heart failure symptoms and reduced quality of life. Novel small-molecule cardiac myosin inhibitors target actin-myosin interactions to alleviate overactive protein interactions. A prospective, randomized, controlled cross-over study was performed to evaluate pharmacodynamic effects of two doses (0.3 and 1 mg/kg) of a next-in-class cardiac myosin inhibitor, aficamten (CK-3773274, CK-274), on cardiac function in cats with the A31P MYBPC3 mutation and oHCM. Dose-dependent reductions in LV systolic function, LVOT pressure gradient, and isovolumetric relaxation times compared to baseline were observed. Promising beneficial effects of reduced systolic function warrant further studies of this next-in-class therapeutic to evaluate the benefit of long-term administration in this patient population.

State-owned investors (SOIs), including sovereign wealth funds and public pension funds, have $27 trillion in assets under management in 2020, making these funds the third largest group of asset owners globally. SOIs have become the largest and are among the most important private equity investors, and they are key investors in other alternative asset investments such as real estate, infrastructure, and hedge funds. SOIs are also leaders in promoting environmental, social, and governance policies and corporate social responsibility policies in investee companies. We document the rise of SOIs, assess their current investment policies, and describe how their state ownership both constrains and enhances their investment opportunity sets. We survey the most impactful recent academic research on sovereign wealth funds, public pension funds, and their closest financial analogs, private pension funds. We also introduce a new Governance-Sustainability-Resilience Scoreboard for SOIs and survey research examining their role in promoting good corporate governance.

Introduction Arterial stiffness, measured by estimated pulse-wave velocity is a known predictor of major adverse cardiovascular events, however its predictive value in patients requiring extracorporeal membrane oxygenation (ECMO) is unknown. Methods A retrospective cohort study was performed at the London Health Science Centre in London, Canada between 1996–2021, totaling 255 patients requiring ECMO. Estimated pulse-wave velocity (ePWV) was calculated using an algorithm from the Reference Values for Arterial Stiffness Collaboration. Recorded outcomes included in-hospital death, ischemic stroke, hemorrhagic stroke, renal failure and need for renal replacement therapy (RRT). For adjusted analysis, survival-to-discharge was used. Multivariate logistic regression and propensity-score matching were utilized to control for confounding. Results On univariate analysis, higher ePWV was significantly predictive of ischemic stroke (OR 1.676, p = 0.0002) and in-hospital death (OR 1.20, p = 0.006), but insignificant for predicting hemorrhagic stroke (OR 1.07, p = 0.710), and appeared protective for renal failure (OR 0.88 [0.78–0.99], p = 0.034) and RRT (OR 0.87, p = 0.027). On multivariate analysis and propensity-score matching, five of six models demonstrated ePWV as an independent predictor of survival-to-discharge. (OR 0.70, p = 0.00,021; OR 0.72, p = 0.0002; OR 0.87, p = 0.045; OR 0.85, p = 0.013; OR 0.57, p = 0.012) Conclusions ePWV is a promising marker for risk-stratification in ECMO patients. Further investigation is required to better delineate the role of arterial health assessment in disease trajectory and strengthen the validity of AS as a marker of interest in medical and surgical management.

The discovery of reldesemtiv, a second-generation fast skeletal muscle troponin activator (FSTA) that increases force production at submaximal stimulation frequencies, is reported. Property-based optimization of high throughput screening hit 1 led to compounds with improved free exposure and in vivo muscle activation potency compared to the first-generation FSTA, tirasemtiv. Reldesemtiv demonstrated increased muscle force generation in a phase 1 clinical trial and is currently being evaluated in clinical trials for the treatment of amyotrophic lateral sclerosis.

Hypercontractility of the cardiac sarcomere may be essential for the underlying pathological hypertrophy and fibrosis in genetic hypertrophic cardiomyopathies. Aficamten (CK-274) is a novel cardiac myosin inhibitor that was discovered from the optimization of indoline compound 1. The important advancement of the optimization was discovery of an Indane analogue (12) with a less restrictive structure-activity relationship that allowed for the rapid improvement of drug-like properties. Aficamten was designed to provide a predicted human half-life (t1/2) appropriate for once a day (qd) dosing, to reach steady state within two weeks, to have no substantial cytochrome P450 induction or inhibition, and to have a wide therapeutic window in vivo with a clear pharmacokinetic/pharmacodynamic relationship. In a phase I clinical trial, aficamten demonstrated a human t1/2 similar to predictions and was able to reach steady state concentration within the desired two-week window.

Hypertrophic cardiomyopathy (HCM) is the most prevalent cardiac disease in cats and lacks efficacious preclinical pharmacologic intervention, prompting investigation of novel therapies. Genetic mutations encoding sarcomeric proteins are implicated in the development of HCM and small molecule myosin inhibitors are an emerging class of therapeutics designed to target the interaction of actin and myosin to alleviate the detrimental effects of inappropriate contractile protein interactions. The purpose of this study was to characterize the pharmacodynamic effects of a single oral dose of the novel cardiac myosin inhibitor aficamten (CK-274) on cardiac function in purpose bred cats with naturally occurring A31P MYBPC3 mutation and a clinical diagnosis of HCM with left ventricular outflow tract obstruction (LVOTO). Five purpose bred cats were treated with aficamten (2 mg/kg) or vehicle and echocardiographic evaluations were performed at 0, 6, 24, and 48 h post-dosing. High dose aficamten (2 mg/kg) reduced left ventricular fractional shortening (LVFS%) by increasing the LV systolic internal dimension (LVIDs) and reduced isovolumic relaxation time (IVRT) compared with baseline without significant adverse effects. The marked reduction in systolic function and reduced IVRT coupled with an increased heart rate in treated cats, suggest a lower dose may be optimal. Further studies to determine optimal dosing of aficamten are indicated.

In response to the coronavirus (COVID-19) pandemic, teprotumumab production was temporarily halted with resources diverted toward vaccine production. Many patients who initiated treatment with teprotumumab for thyroid eye disease were forced to deviate from the standard protocol. This study investigates the response of teprotumumab when patients receive fewer than the standard 8-dose regimen.This observational cross-sectional cohort study included patients from 15 institutions with active or minimal to no clinical activity thyroid eye disease treated with the standard teprotumumab infusion protocol. Patients were included if they had completed at least 1 teprotumumab infusion and had not yet completed all 8 planned infusions. Data were collected before teprotumumab initiation, within 3 weeks of last dose before interruption, and at the visit before teprotumumab reinitiation. The primary outcome measure was reduction in proptosis more than 2 mm. Secondary outcome measures included change in clinical activity score (CAS), extraocular motility restriction, margin reflex distance-1 (MRD1), and reported adverse events.The study included 74 patients. Mean age was 57.8 years, and 77% were female. There were 62 active and 12 minimal to no clinical activity patients. Patients completed an average of 4.2 teprotumumab infusions before interruption. A significant mean reduction in proptosis (-2.9 mm in active and -2.8 mm in minimal to no clinical activity patients, P0.01) was noted and maintained during interruption. For active patients, a 3.4-point reduction in CAS (P0.01) and reduction in ocular motility restriction (P0.01) were maintained during interruption.Patients partially treated with teprotumumab achieve significant reduction in proptosis, CAS, and extraocular muscle restriction and maintain these improvements through the period of interruption.

Background STAMPEDE previously reported adding upfront docetaxel improved overall survival for prostate cancer patients starting long-term androgen deprivation therapy. We report long-term results for non-metastatic patients using, as primary outcome, metastatic progression-free survival (mPFS), an externally demonstrated surrogate for overall survival. Methods Standard of care (SOC) was androgen deprivation therapy with or without radical prostate radiotherapy. A total of 460 SOC and 230 SOC plus docetaxel were randomly assigned 2:1. Standard survival methods and intention to treat were used. Treatment effect estimates were summarized from adjusted Cox regression models, switching to restricted mean survival time if non-proportional hazards. mPFS (new metastases, skeletal-related events, or prostate cancer death) had 70% power (α = 0.05) for a hazard ratio (HR) of 0.70. Secondary outcome measures included overall survival, failure-free survival (FFS), and progression-free survival (PFS: mPFS, locoregional progression). Results Median follow-up was 6.5 years with 142 mPFS events on SOC (3 year and 54% increases over previous report). There was no good evidence of an advantage to SOC plus docetaxel on mPFS (HR = 0.89, 95% confidence interval [CI] = 0.66 to 1.19; P = .43); with 5-year mPFS 82% (95% CI = 78% to 87%) SOC plus docetaxel vs 77% (95% CI = 73% to 81%) SOC. Secondary outcomes showed evidence SOC plus docetaxel improved FFS (HR = 0.70, 95% CI = 0.55 to 0.88; P = .002) and PFS (nonproportional P = .03, restricted mean survival time difference = 5.8 months, 95% CI = 0.5 to 11.2; P = .03) but no good evidence of overall survival benefit (125 SOC deaths; HR = 0.88, 95% CI = 0.64 to 1.21; P = .44). There was no evidence SOC plus docetaxel increased late toxicity: post 1 year, 29% SOC and 30% SOC plus docetaxel grade 3-5 toxicity. Conclusions There is robust evidence that SOC plus docetaxel improved FFS and PFS (previously shown to increase quality-adjusted life-years), without excess late toxicity, which did not translate into benefit for longer-term outcomes. This may influence patient management in individual cases.

Omecamtiv mecarbil (OM) is a cardiac myosin activator under development for the treatment of heart failure. The effect of CYP3A4 and CYP2D6 inhibition on OM pharmacokinetics and the potential for OM to induce CYP3A4 was assessed in 2 studies. Study 1, part A, assessed the effect of ketoconazole 200 mg on the pharmacokinetics of OM 10 mg in CYP2D6 extensive metabolizers (EMs; n = 8) or poor metabolizers (PMs; n = 8). Study 1, part B, assessed the effect of diltiazem 240 mg on the pharmacokinetics of OM 10 mg (EM; n = 8). Study 2 assessed the effect of OM 25 mg on the pharmacokinetics of midazolam 5 mg (n = 14). Coadministration with ketoconazole led to 51% and 31% increases in OM AUCinf in EM and PM subjects, respectively, whereas OM Cmax remained similar (3% higher and 14% lower for EM and PM subjects, respectively). No changes in OM pharmacokinetics were observed in EM subjects following coadministration with diltiazem. Midazolam AUCinf and Cmax decreased by 18% and 10%, respectively, when coadministered with OM. In conclusion, CYP3A4 and CYP2D6 inhibitors are unlikely to have a clinically significant effect on the pharmacokinetics of OM. In addition, OM is unlikely to have a clinically relevant effect on the pharmacokinetics of CYP3A4 substrates.

Left ventricular outflow tract (LVOT) obstruction is a major determinant of heart failure symptoms in obstructive hypertrophic cardiomyopathy (oHCM). Aficamten, a next-in-class cardiac myosin inhibitor, may lower gradients and improve symptoms in these patients.This study aims to evaluate the safety and efficacy of aficamten in patients with oHCM.Patients with oHCM and LVOT gradients ≥30 mm Hg at rest or ≥50 mm Hg with Valsalva were randomized 2:1 to receive aficamten (n = 28) or placebo (n = 13) in 2 dose-finding cohorts. Doses were titrated based on gradients and ejection fraction (EF). Safety and changes in gradient, EF, New York Heart Association functional class, and cardiac biomarkers were assessed over a 10-week treatment period and after a 2-week washout.From baseline to 10 weeks, aficamten reduced gradients at rest (mean difference: -40 ± 27 mm Hg, and -43 ± 37 mm Hg in Cohorts 1 and 2, P = 0.0003 and P = 0.0004 vs placebo, respectively) and with Valsalva (-36 ± 27 mm Hg and -53 ± 44 mm Hg, P = 0.001 and 0.0001 vs placebo, respectively). There were modest reductions in EF (-6% ± 7.5% and -12% ± 5.9%, P = 0.007 and P0.0001 vs placebo, respectively). Symptomatic improvement in ≥1 New York Heart Association functional class was observed in 31% on placebo, and 43% and 64% on aficamten in Cohorts 1 and 2, respectively (nonsignificant). With aficamten, N-terminal pro-B-type natriuretic peptide was reduced (62% relative to placebo, P = 0.0002). There were no treatment interruptions and adverse events were similar between treatment arms.Aficamten resulted in substantial reductions in LVOT gradients with most patients experiencing improvement in biomarkers and symptoms. These results highlight the potential of sarcomere-targeted therapy for treatment of oHCM.
