The purpose of this study is to investigate the moderating role that can play ethical training in increasing auditors’ level of ethics. Therefore, this research will test any significant differences between the two groups through a multi-group moderation analysis. The first group represents auditors who took an ethics course before, and the other group represents auditors who did not previously take any course. This comparison will be performed based on the relationships between the variables in our model, namely ethical judgment, ethical intention, moral identity centrality, and perceived moral intensity. The sample of this study is mainly external senior auditors operating in audit firms in Morocco. We could obtain 125 usable responses, and we processed data with SmartPLS software. The results show that auditors’ moral identity impacts their ethical judgment positively and significantly, and the effect is more substantial for the group of auditors who took an ethics course before. Moreover, taking an ethics course seems to moderate also the positive and significant relationship between auditors’ ethical judgment and their ethical intention. However, the results were opposite to what we expected concerning the relationship between perceived moral intensity and auditors’ ethical judgment. Thus, ethical training seemed to moderate the positive relationship negatively.

This article aims to test and provide empirical evidence of the moderated mediating role that plays moral identity in explaining auditor’s ethical decision making based on Jones' model of moral intensity. Therefore, this research proposes a moderated mediation model where moral identity accessibility mediates the relationship between perceived moral intensity and auditors’ moral judgment. Moreover, Moral identity centrality is tested as a moderator variable for this socio-cognitive model. This study used random sampling methods for external senior auditors operating in audit firms in Morocco. Data obtained by 125 respondents and processed with SmartPLS. The results show that the impact of an auditor’s perceived moral intensity on his moral judgment is fully mediated by moral identity accessibility and moderated by moral identity centrality. Auditor’s moral intensity perception seems to trigger the access to moral identity which in turn affects positively the moral judgment based on how central is moral identity to the individual.

Cholesteatomatous otitis is a chronic dangerous condition of the middle earbecause of its potential intra and extra cranial complications.These complications have become exceptional since the systematic use of Antibiotics and the early management of the cholestatomatous disease. However, they are still there, especially in the developing countries. We report an original observation of a 29-year-old female with a Bezold?s abscess, sigmoid sinus thrombosis and purulent meningitis following a neglected right ear othorrhea. Very few cases associating these 3 complications have been described in the literature. They may be life-threatening. Therefore, an otorrhea must not be neglected by the patient and its very important to diagnose and treat early all cases of chronic otitis media to prevent them.

Auto immune pancreatitis (AIP) is an uncommon form of chronic pancreatitis that has been divided into type 1 and type 2 which have distinct histopathology and clinical features. Type 1 AIP seems to be the pancreatic manifestation of an IgG4-related systemic disease, characterized by elevated IgG4 serum levels, infiltration of IgG4-positive plasma cells and extrapancreatic lesions. When manifesting as a focal disease in the pancreas, it can be challenging to differentiate it from pancreatic cancer (PC). Because AIP is typically responsive to steroid therapy without the need for resection, differentiation between these two diseases is critical. We report the case of a 56-year-old man with initial suspect of PC, and final diagnosis of type 1 AIP according to the International Consensus Diagnostic Criteria (ICDC) without elevated levels of serum IgG4 or need for a histology sample. We take a review of literature in order to improve the diagnostic accuracy of AIP and describe clinical and imaging features to differentiate it from PC and avoid unnecessary surgery due to misdiagnosis.

Objectifs (1) Rapporter le cas d’une maladie cerebrovasculaire hereditaire rare : arteriopathie cerebrale autosomique dominante avec infarctus sous-corticaux et leucoencephalopathie (CADASIL). (2) Expliquer la physiopathologie de cette pathologie. (3) Souligner l’importance de l’IRM pour etablir le diagnostic. Materiel et methodes Homme de 40 ans, ayant comme antecedents une migraine et un accident vasculaire cerebral ischemique (AVCI) droit en 2010 qui s’est presente aux urgences pour un deficit aigu de l’hemicorps droit avec etat de mal epileptique, ce qui a motive une IRM cerebrale avec des sequences angiographique 3D. Resultats L’IRM cerebrale a objective des anomalies de signal intraparenchymateuses sus-tentorielles d’âges differents : une cavite porencephalique parietale droite en rapport avec son ancien AVCI et une lesion lenticulaire gauche en hypersignal T2 Flair avec restriction de diffusion expliquant son hemiplegie droite recente. Il s’y associe des anomalies de signal en plages de la substance blanche peri-ventriculaire en hypersignal T2 et FLAIR sans restriction de diffusion en rapport avec une leucoencephalopathie vasculaire. Les sequences angiographiques 3D TOF ne retrouvent pas d’occlusion ni de stenose des gros vaisseaux du polygone de Willis. Devant ce tableau clinique et la presence de lesions ischemiques d’âge differents associees a une leucopathie vasculaire a l’IRM chez un sujet jeune, le diagnostic de CADASIL est evoque, confirme par un test genetique positif qui a revele une mutation specifique du gene NOTCH3. Conclusion Cadasil est une maladie hereditaire autosomique dominante rare qui touche l’adulte d’âge moyen caracterise par l’association d’infarctus sous-corticaux et d’une leucoencephalopathie. Elle est due a des mutations du gene NOTCH3 sur le chromosome 19, ainsi responsable d’une atteinte des petites arteres cerebrales. L’imagerie repose essentiellement sur l’IRM qui montre des lesions diffuses de la substance banche, associes a de petits infarctus cerebraux et parfois a des micro-saignements d’âge differents. Le diagnostic est confirme par un test genetique ou par une biopsie cutanee.

Objectifs Rapporter le cas d’une malformation congenitale rare representee par l’holoprosencephalie. Decrire la semiologie radiologique de cette entite. Souligner l’importance de l’imagerie pour etablir le diagnostic. Materiel et methodes Nourrisson de 3 mois, admis pour syndrome polymalformatif. L’examen clinique trouve une microcephalie avec microphtalmie. Un examen ophtalmologique et une TDM cerebrale ont ete realises. Resultats La TDM cerebrale a objective une cavite ventriculaire unique avec fusion des cornes frontales des 2 ventricules lateraux, des thalami et agenesie du corps calleux. Le diagnostic d’holoprosencephalie dans sa forme semi-lobaire a ete retenu. Conclusion L’holoprosencephalie est une malformation cerebrale rare qui resulte d’une division incomplete du cerveau anterieur (prosencephale) lors de la 3e a la 4e semaine de gestation. Elle s’associe souvent a des anomalies faciales evocatrices, dominees par les fentes labio-palatines. La TDM cerebrale, ou au mieux, l’IRM definit le sous-type anatomique et identifie les anomalies associees du systeme nerveux central. On distingue trois formes anatomiques selon le degre d’individualisation des hemispheres cerebraux : la forme alobaire, semi-lobaire et lobaire. Le pronostic est reserve. Le diagnostic antenatal peut etre fait par l’echographie fœtale.

Objectifs Rapporter 3 cas du syndrome hemiconvulsion-hemiplegie-epilepsie (HHE). Mettre en evidence le profil clinique, paraclinique, et evolutif du syndrome HHE. Illustrer la semiologie radiologique. Materiel et methodes Trois enfants de 4 et 5 ans, ayant presente des crises convulsives suivies par une hemiplegie, une TDM cerebrale a ete demandee dans 2 cas et une IRM cerebrale dans 1 cas pour une exploration etiologique. Resultats La TDM cerebrale a objective une hemi-atrophie cerebrale dans 2 cas. L’IRM a mis en evidence une atrophie cortico sous corticale occipitale gauche avec attraction et dilatation du VL homolateral. Le diagnostic du syndrome HHE a ete retenu en se basant sur les donnees cliniques, de l’EEG et de l’imagerie. Conclusion Le syndrome hemiconvulsion-hemiplegie-epilepsie fait partie des epilepsies partielles. Il se definit par des convulsions interessant un hemicorps, suivies immediatement par une hemiplegie flasque ipsilaterale aux convulsions, et plus tard par l’epilepsie avec des crises focales. La TDM cerebrale comme l’IRM peut montrer initialement un œdeme, suivi d’une atrophie hemispherique et parfois des images de cavites porencephaliques.

Multiple biliary hamartomas (MBHs) are a rare source of cystic lesions of the liver that radiologic detection is difficult, because of its tiny size. We report the case of a 64-year-old woman, with recurrent abdominal pain. The ultrasonographic configuration in this patient showed multiple cystic hepatic lesions. However, the diagnosis of MBH was made by magnetic resonance imaging (MRI) and magnetic resonance cholangiopancreatography (MRCP). A regular follow-up of patient presenting this benign disease is recommended because of its potential for malignant conversion.
