- Abstract
PURPOSE: Sperm DNA fragmentation (SDF) has been associated with male infertility and poor outcomes of assisted reproductive technology (ART). The purpose of this study was to investigate global practices related to the management of elevated SDF in infertile men, summarize the relevant professional society recommendations, and provide expert recommendations for managing this condition. MATERIALS AND METHODS: An online global survey on clinical practices related to SDF was disseminated to reproductive clinicians, according to the CHERRIES checklist criteria. Management protocols for various conditions associated with SDF were captured and compared to the relevant recommendations in professional society guidelines and the appropriate available evidence. Expert recommendations and consensus on the management of infertile men with elevated SDF were then formulated and adapted using the Delphi method. RESULTS: A total of 436 experts from 55 different countries submitted responses. As an initial approach, 79.1% of reproductive experts recommend lifestyle modifications for infertile men with elevated SDF, and 76.9% prescribe empiric antioxidants. Regarding antioxidant duration, 39.3% recommend 4-6 months and 38.1% recommend 3 months. For men with unexplained or idiopathic infertility, and couples experiencing recurrent miscarriages associated with elevated SDF, most respondents refer to ART 6 months after failure of conservative and empiric medical management. Infertile men with clinical varicocele, normal conventional semen parameters, and elevated SDF are offered varicocele repair immediately after diagnosis by 31.4%, and after failure of antioxidants and conservative measures by 40.9%. Sperm selection techniques and testicular sperm extraction are also management options for couples undergoing ART. For most questions, heterogenous practices were demonstrated. CONCLUSIONS: This paper presents the results of a large global survey on the management of infertile men with elevated SDF and reveals a lack of consensus among clinicians. Furthermore, it demonstrates the scarcity of professional society guidelines in this regard and attempts to highlight the relevant evidence. Expert recommendations are proposed to help guide clinicians.

- Abstract
The development of innovative technologies and the advances in the genetics and genomics, have offered new opportunities for personalized treatment in oncology. Although the selection of the patient based on the molecular characteristics of the neoplasm has the potential to revolutionize the therapeutic scenario of oncology, this approach is extremely challenging. The access, homogeneity, and economic sustainability of the required genomic tests should be warranted in the clinical practice, as well as the specific scientific and clinical expertise for the choice of medical therapies. All these elements make essential the collaboration of different specialists within the Molecular Tumor Boards (MTBs). In this position paper, based on experts' opinion, the AIOM-SIAPEC/IAP-SIBioC-SIC-SIF-SIGU-SIRM Italian Scientific Societies critically discuss the available molecular profiling technologies, the proposed criteria for the selection of patients candidate for evaluation by the MTB, the criteria for the selection and analysis of biological samples, and the regulatory and pharmaco-economic issues.

Todos los músicos de esta producción, al igual que Justo, hacen parte de la comunidad cristiana. Como hemos contado, desde 1981 que abrazó dicha religión, esta ha estado en el centro de la vida de Justo, por lo que ha sido frecuente que otros músicos con los que ha realizado grabaciones y presentaciones hagan parte de la misma comunidad. Sin embargo, Justo no encuentra en la religión una limitante, sino, por el contrario, un motivo para acercarse y trabajar con diferentes personas. Para entender un poco su comprensión de su práctica religiosa, es muy bello y significativo su testimonio cuando le pregunté si uno de los músicos con los cuales había realizado un disco era también cristiano. Human mediator complex ,MED11 ,MEDopathies - Abstract
Purpose: The mediator (MED) multisubunit-complex modulates the activity of the transcriptional machinery, and genetic defects in different MED subunits (17, 20, 27) have been implicated in neurologic diseases. In this study, we identified a recurrent homozygous variant in MED11 (c.325C>T; p.Arg109Ter) in 7 affected individuals from 5 unrelated families. Methods: To investigate the genetic cause of the disease, exome or genome sequencing were performed in 5 unrelated families identified via different research networks and Matchmaker Exchange. Deep clinical and brain imaging evaluations were performed by clinical pediatric neurologists and neuroradiologists. The functional effect of the candidate variant on both MED11 RNA and protein was assessed using reverse transcriptase polymerase chain reaction and western blotting using fibroblast cell lines derived from 1 affected individual and controls and through computational approaches. Knockouts in zebrafish were generated using clustered regularly interspaced short palindromic repeats/Cas9. Results: The disease was characterized by microcephaly, profound neurodevelopmental impairment, exaggerated startle response, myoclonic seizures, progressive widespread neurodegeneration, and premature death. Functional studies on patient-derived fibroblasts did not show a loss of protein function but rather disruption of the C-terminal of MED11, likely impairing binding to other MED subunits. A zebrafish knockout model recapitulates key clinical phenotypes. Conclusion: Loss of the C-terminal of MED subunit 11 may affect its binding efficiency to other MED subunits, thus implicating the MED-complex stability in brain development and neurodegeneration.

Since our first release we have received additional data from our new collaborators, allowing this resource to grow to its current size. Dedicated data gathering started from March 11th yielding over 4 million tweets a day. We have added additional data provided by our new collaborators from January 27th to March 27th, to provide extra longitudinal coverage. Version 22 added ~1.5 million tweets in the Russian language collected between January 1st and May 8th, gracefully provided to us by: Katya Artemova (NRU HSE) and Elena Tutubalina (KFU). From version 12 we have included daily hashtags, mentions and emoijis and their frequencies the respective zip files. From version 14 we have included the tweet identifiers and their respective language for the clean version of the dataset. Since version 20 we have included language and place location for all tweets. The data collected from the stream captures all languages, but the higher prevalence are: English, Spanish, and French. We release all tweets and retweets on the full_dataset.tsv file (1,371,22223,2242 unique tweets), and a cleaned version with no retweets on the full_dataset-clean.tsv file (355,6221,266 unique tweets). There are several practical reasons for us to leave the retweets, tracing important tweets and their dissemination is one of them. For NLP tasks we provide the top 2200 frequent terms in frequent_terms.csv, the top 2200 bigrams in frequent_bigrams.csv, and the top 2200 trigrams in frequent_trigrams.csv. Some general statistics per day are included for both datasets in the full_dataset-statistics.tsv and full_dataset-clean-statistics.tsv files.
