【目的】探讨卵巢癌患者新辅助化疗效果以及在卵巢癌常用化疗方案筛选中应用 HDST 技术的可行性。【方法】回顾性分析于中山大学孙逸仙纪念医院妇科肿瘤专科收治的 11 例按照 NCCN 指南推荐的行新辅助化疗的晚期卵巢癌患者，采集其社会人口学、临床资料及组织标本，并应用HDST 技术进行药敏检测。按照HDST 药敏结果进行分组，比较组间CA125、HE4 与Fagotti 评分指标总下降率的差异，探讨HDST 结果与患者肿瘤指标下降率的关系。【结果】HDST 结果显示6 例患者对选用的铂类药（顺铂或卡铂）不敏感，1 例患者对紫杉醇不敏感；经3次新辅助化疗后患者肿瘤指标均呈现明显的逐次下降趋势。HDST 结果与临床疗效间一致性较好（Kappa=1.000，P0.05）。【结论】HDST 技术在卵巢癌的新辅助化疗方案筛选上具有快速方便、经济实惠，结果与临床疗效一致的优势，有望为卵巢癌患者提供精准的个体化化疗方案。

同心配置双管形成管中管混凝土可以改善大径管带来的问题。共设计了7根管中管混凝土短柱试件,以内管配钢率和屈服强度为主要变化参数,对其进行轴压试验研究,分析了试件的破坏特征、承载力和荷载-位移曲线,给出了变形和承载力的试验结果,研究了各影响参数对试件变形和承载力的影响。试验结果表明:较普通钢管混凝土柱,内配钢管的钢管混凝土柱改变了其破坏形态,提高了其承载能力,改善了试件的延性;内外钢管在试件达到承载力时都已经屈服,充分发挥了钢材的作用;采用有限元软件ABAQUS进行了数值计算分析,并将计算结果与试验结果进行了比较,吻合良好。

Objective To investigate the interaction between overweight/obesity and dyslipidemia on hyperuricemia (HUA), and to provide scientific basis for the prevention and treatment of HUA. Methods The data were derived from the physical examination records of workers at the Xishan Coal and Electricity Group in 2018, encompassing a total of 38 615 study participants. SAS 9. 3 software was used to conduct 咒2 test and multivariate logistic regression analyses. Additive interaction model was used to analyze the interaction between overweight/obesity and dyslipidemia on HUA. Results The prevalence rate of HUA was 18.28%. After adjusting for gender, age, smoking, drinkings systolic blood pressure, diastolic blood pressure, fasting blood glucose and blood creatinine, the results of multivariate logistic regression analyses showed that overweight/obesity (OR - 2. 756, 95% Cl: 2, 584 - 2. 939) and dyslipidemia (OR = 2, 221, 95% Cl: 2. 100 - 2. 349) were associated with a higher risk of HUA, respectively. Interaction analyses showed a synergistic additive interaction between overweight/obesity and dyslipidemia on the prevalence of HUA, with those who were overweight/obese and dyslipidemic having a higher risk of HUA than those who had neither (OR =4. 840,95% CI: 4.452 - 5. 261), and the relative excess risk of interaction (RERI) was 0.4, the attributable proportion of interaction (API) was 0.083, and the synergy index (SI) was 1. 116. Conclusion Overweight/obesity and dyslipidemia both increase the risk of HUA, and there is synergistic additive interaction between them. Objective: To explore the correlation between the dynamic changes of early immune cells in acute respiratory distress syndrome( ARDS) caused by multiple injuries and lung infection. Methods: Multiple injury patients with ARDS( 235 cases) admitted to the First Affiliated Hospital of Hunan University of Chinese Medicine from October 2017 to February 2023 were selected as the research subjects and included them in the training set; according to simple clinical pulmonary infection scores (sCPIS), they were divided into a pulmonary infection group (94 cases, >6 points) and an uninfected group (141 cases, ≤6 points). Another multiple injury patients with ARDS (78 cases) were selected to be included in the validation set to verify the effectiveness of the prediction model. Dynamic detection of lymphocytes were used flow cytometry. Compared and analyzed the clinical data of two groups of patients. Constructed a joint model and used Cox regression to analyze the relationship. Multi factor Logistic regression analysis of risk factors, construction of a simple risk scoring model and evaluation. Results: As the patient progresses, CD4+ and CD8+ first decrease and then increased, and the lowest stage was 3~15 d after onset; CD19+ was gradually increasing; CD16+ gradually decreased and fluctuated within a certain range. The joint model showed that for every 1 piece/μl longitudinal decrease in CD4+, CD8+, CD19+, and CD16+, the risk of pulmonary infection increased by 5.6%, 4.1%, 3.4% and 1.3%, respectively( P<0.05). Injury severity score( ISS), chest injury as the main factor, emergency surgery, acutephysiology and chronic health evaluation Ⅱ (APACHE Ⅱ), broncho-alveolar lavage fluid( BALF) sTREM-1 level, and CD4+, CD8+, and CD19+ level on the 15th day after onset all were independent influencing factors for the occurrence of pulmonary infection( P<0.05). The score of the simple risk scoring model is 0~36.7 points, which could be divided into three risk levels: low (<16 points), medium (16~22 points), and high risk (>22 points); there was no significant difference in the incidence of pulmonary infection between the two episodes of patients (P>0.05). The evaluation results showed that the predictive model has good discrimination. Conclusion: The fluctuation of immune cells will increase the risk of pulmonary infection in patients with multiple injuries ARDS; Baseline CD4+, CD8+, and CD19+ levels are independent influencing factors for the occurrence of pulmonary infection; controlling high-level immune cells and maintaining stability is crucial for improving prognosis. Obejective: To explore the diagnostic and therapeutic effect of ultra-fine bronchoscopic interventional in bronchiolitis obliterans syndrome (BOS) after allogeneic hematopoietic stem cell transplantation (Allo-HSCT). Methods: Retrospective analysis of clinical data of 2 patients with BOS diagnosed 3 months after transplantation were collected, and the characteristics of diagnosis and therapeutics of BOS were analyzed. Results: The time from Allo-HSCT to diagnosed of BOS in 2 patients in our hospital was within 3 months after transplantation, the clinical manifestations were chest tightness and shortness of breath after activity, and mixed ventilation dysfunction in lung function, Under the direct vision of ultra-fine bronchoscopy, the membrane-like hyperplasia of the lumen orifice of the bronchioles of grade 6 and above in the left and right sides could block the lumen was observed, and the occluded airway could be opened by mechanical expansion or freezing with biopsy forceps, no examination-related complications occurred during and after the operation, the symptoms of the patients were significantly improved compared with the previous ones after treatment, One patient was followed up and the original occluded bronchiole lumen was continuously open after treatment under bronchoscopy. Conclusion: Ultra-fine bronchoscopy can effectively diagnose BOS after Allo-HSCT, and can also open bronchioles through respiratory interventional therapy to improve short-term lung function. The introduction of tobacco has always been the focus of tobacco history research, but the previous studies have mainly focused on Nicotiana tabacum, and there were obvious deficiencies in the study of Nicotiana rustica. In fact, Nicotiana rustica was also an important part of tobacco species for human planting and use, its introduction had a profound impact on the society of that time and even later generations. Based on the collection and arrangement of the annals and anthology, this paper shows that the Nicotiana rustica had already been introduced in the period Emperor Chongzhen’s reign, no later than the period of Emperor Kangxi’s reign. It was introduced into China mainly through smuggling, normal trade and the diplomatic envoys’ spread. OBJECTIVE: To probe into the efficacy of low-dose sitagliptin combined with canagliflozin in the treatment of diabetes nephropathy and its effects on inflammatory indicators and oxidative stress reaction. METHODS: A total of 118 patients with diabetes nephropathy admitted into Danzhou People’ s Hospital from Mar. 2021 to Aug. 2022 were selected to be divided into 59 cases in the control group ( treated with cagelin) and 59 cases in the observation group ( treated with cagelin + low-dose sitagliptin) via the random number table method. The clinical efficacy, renal function, oxidative stress reaction, glycosylated hemoglobin ( HbA1 c), inflammatory indicators and adverse drug reactions were compared between two groups. RESULTS: The clinical total effective rate of the observation group was 88. 14% ( 52 / 59), significantly higher than 72. 88% ( 43 / 59) of the control group, with statistically significant difference ( P < 0. 05). After 12 weeks of treatment, the serum creatinine, HbA1 c, urea nitrogen, cystatin C, 24 h urinary microalbumin excretion rate, interleukin-6, transforming growth factor β1, interleukin-18 and malondialdehyde levels of the observation group were lower than those of the control group at the same time; the glutathione peroxidase and superoxide dismutase levels of the observation group were higher than those of the control group at the same time, with statistically significant differences ( P < 0. 05). CONCLUSIONS: The combination of low-dose sitagliptin with canagliflozin can improve the renal function of patients with diabetes nephropathy, reduce body inflammation and oxidative stress reaction, with high safety. High-voltage electrical penetrations are safety-level equipment installed on the shielding wall of the hot chamber to ensure the electrical continuity, sealing integrity and biological shielding function inside and outside the shielding wall of the hot chamber. The maturity evaluation of high-voltage electrical penetrations was studied. Firstly, the definition and connotation of technical maturity in the nuclear field were expounded, and the internal relationship between technical maturity and quality management was revealed. Based on this, the project breakdown structure of high-voltage electrical penetrations was established, the special evaluation level rules and comprehensive evaluation process were formulated, and an modified analytic hierarchy process (MAHP)-entropy weight was proposed. The assessment results show that the technology readiness of high-voltage electrical penetrations has reached TRL8, which provides an effective basis and important support for its decision to shift to mass production, reduce project subsequent projects. The stage progress and the excellent quality control performance have won unanimous praise from supply risks, and change quality management measure, so that the high-voltage electrical penetrations can be supplied in owners, providing a guarantee for the construction of the project on schedule. Diabetic kidney disease (DKD) is the primary cause of end-stage renal disease, however, specific treatments are still lacking at present. Vitamin D exerts various biological effects in the human body through binding to cellular receptors following metabolism, and has broad clinical applications. In recent years, the results of clinical trials have demonstrated the significant reduction of proteinuria level in DKD patients with favorable safety profiles through using vitamin D or its analogs. However, there is still no consensus on the recommended medication type, dosage, administration route, or course of treatment currently. In addition, fundamental research has suggested that vitamin D may exert its effects on the pathogenesis of DKD through multiple pathways. This review comprehensively reviews the clinical applications of vitamin D and its analogs in the treatment of DKD both domestically and internationally, and also briefly summarizing the achievements in basic research, so as to provide theoretical support for future studies and clinical applications of this type.
