12. MP妊高征监测系统联合PLGF和PI对子痫前期的预测价值.

- Abstract
Objective To establish a clinical prediction model for preeclampsia by monitoring risk rating of MP gestation and levels of placental growth factor (PLGF) combined with uterine artery pulsatility index (PI) measured during examination of fetal nuchal translucency (NT). Methods Twenty-four patients with preeclampsia who met the inclusion criteria were selected as the case group, and 95 healthy pregnant women during the same period were randomly selected as the control group. Serum concentrations of PLGF, uterine artery PI values measured by quantitative immunofluorescence assay at 11-14 weeks of gestation, risk ratings for MP hypertension monitoring at 11-20 weeks of gestation, and other relevant data, BMI, age, gestation, mode of delivery, neonatal birth weight and Apgar score were collected in the two groups. Results Results of univariate regression analysis showed that BMI, age, high risk of PI, MP and PLGF<12 were influencing factors for adverse outcomes. Results of multivariate regression analysis showed that high PI, medium high risk in MP and PLGF<12 were independent risk factors for adverse outcomes. The prediction model of PE established was logit (P) = -15.767 + 0.020 × PI + 0.072 × MP risk (medium-high risk = 1, low risk = 0) + 0.181 × PLGF classification (<12 = 1, ≥12 = 0), with an AUC area of 0.883, specificity of 0.816 and sensitivity of 0.846. Conclusion The combination of PI, MP risk and PLGF to establish a clinical predictive model for preeclampsia has certain value, and its combined predictive value is higher than that of single application. - Abstract
Objective To investigate the anatomical structure of Sanjian acupoint. Methods ① The ancient people's descriptions of Sanjian were summarized by searching the Duxiu Academic and Chinese Medical Canon database and ancient Chinese medical books. ② The modern people's descriptions of Sanjian were summarized by searching China National Knowledge Infrastructure (CNKI) and The People's Republic of China National Standards·Nomenclature and Loca⁃ tion of Meridian Points. ③ The "acupoint sensation" location was determined by fingertip pressing on thirty volunteers' body surfaces based on the summarization of ancient and modern people's descriptions. ④ By referring to the acupoint positioning summarized in ancient and modern literature, the exact match of the Sanjian "acupoint sensation" location was defined by acupuncturing the different points and depths in the neck region of the second metacarpal bones of ten volunteers. ⑤The anatomical structure of Sanjian was determined by microdissection on ten sides of fresh adult upper limb anatomical specimens. ⑥ The location of the needle tip was demarcated by B-ultrasound when the needle tip touched the "acupoint sensation" point on ten volunteers. ⑦ We used ultrasonography to find the physical structure of Sanjian discovered by anatomy, and then acupuncture was conducted under the guidance of B-ultrasound to verify the position of Sanjian acupoint on ten volunteers. Results ① The ancient books recorded the location of Sanjian was in the depression on the hand and the second finger of the posterior inner part of the node. ② Modern literature described Sanjian was in the depression of the proximal radial side of the second metacarpophalangeal joint. ③ Through finger press, "acupoint sensation" appeared in the second metacarpophalangeal joint proximal end, radial volar depression. ④ "Acupoint sensation" was the strongest when acupuncture was performed at the second metacarpal and phalangeal joint proximal radial volar depression. ⑤ Microdissection revealed that this point was the 'hilum of bone', where the bone-nourishing vessels entered the radial volar side of the second metacarpal neck. ⑥ When we acupunctured Sanjian, and it appeared to have an "acupoint sensation", the needle tip was located in the second metacarpal neck radial volar side of the 'hilum of bone'. ⑦ B-ultrasound confirmed that the 'hilum of bone' on the radial volar side of the neck of the second metacarpal was the anatomical structure of Sanjian.Conclusion The anatomical structure of the Sanjian acupoint is the 'hilum of bone' at the radial volar side of the second metacarpal neck. 核苷酸结合寡聚化结构域样受体3(NLRP3) 炎症小体是核苷酸结合寡聚结构域样受体(NLRs) 家族中研 究最为广泛的一类,研究发现 NLRP3炎症小体及其激活产物白细胞介素 1β(IL-1β) 、白细胞介素 18(IL-18) 与多种 关节炎症疾病的发病密切相关。研究显示,NLRP3炎症小体在非感染性关节炎[退行性关节炎(骨关节炎) 、内分泌 代谢性关节炎(痛风性关节炎、骨质疏松) 、脊柱关节病、自身免疫性关节炎(类风湿关节炎、系统性红斑狼疮、干燥综 合征) ]及感染性关节炎(化脓性、结核性、布鲁氏菌性关节炎) 等关节炎症疾病的发生发展中具有重要作用,其可能 成为关节炎症疾病的治疗靶点,对其进行深入研究可为关节炎症疾病的诊治提供新的思路。 [ABSTRACT FROM AUTHOR]

In order to ensure the safety of assembly building hoisting construction process,this paper will use the combined weighting two‐dimensional cloud model evaluation method to evaluate the safety problems existing in the hoisting construction process of a prefabricated project in Lanzhou City. The risk evaluation system of the whole project is established from 5 aspects of human,material,technical scheme,environment and site management, and 17 grade II risk assessment indexes,such as technical ability of construction personnel and quality defects of components,are established. Based on the improved G2 method and the inverse entropy weight method,the linear optimal combination of subjective and objective weights is used to determine the weight. The risk grade is acceptable and accords with the actual risk management of the project. Flood in small and medium-sized catchments can be characterized by the short concentration time, high flow velocity, fierce attack, and violent stage change. Mountain torrents also occur frequently in these small and medium-sized catchments. But, it is very difficult to carry out flood forecasting and early warning, due mainly to those located in the ungauged areas. Therefore, it is a high demand to improve the forecast accuracy in the operational hydrology, especially in the areas without enough measured data. In this study, an energy model was proposed to estimate the velocity of overland flow for the high accuracy in the simulation of flow concentration. Firstly, the spatial distribution of the energy was determined for the water particles in the basin. The gravitational potential energy was gradually transformed into kinetic energy using the iterative computation from the upstream to the downstream, according to the flow direction. Secondly, the spatial energy field was constructed considering the energy loss, and then the overland flow velocity was estimated to generate the spatial velocity field. Thirdly, the concentration time was calculated to count the number of grids, when the water particles on each grid reached the outlet of the watershed. Finally, the geomorphic unit hydrograph was generated to determine the relationship between the catchment area and concentration time. The study area was set as the Zhuxipo basin in Yiyang City, Hunan Province, China, located at the source of Yixi, a tributary of the Zishui River. The Zhuxipo basin was divided into 57 sub watersheds using Digital Elevation Model (DEM) data with a resolution of 30 m×30 m. A distributed model was then constructed to simulate 36 floods in the study area from 1984 to 2020. A Xinanjiang model, geomorphic unit hydrograph model, and Muskingum Routing were used to calculate the runoff generation, overland flow concentration, and river network flow concentration, respectively. The geomorphic unit hydrograph was also extracted by the Energy Conversion Method (EC-GUH) and Slope Rain Intensity Method (SR-GUH). At the same time, the EC-GUH and SR-GUH were also used to compute the overland flow concentration for the evaluation. The average flow velocity of the hydrological section was then calculated to estimate the range of energy residual coefficient (the only parameter of EC-GUH), according to the total water volume and kinetic energy of 36 floods. The results show that the EC-GUH method performed better than the SR-GUH method, where the proportion of floods with a peak time error no more than 1 h increased from 30.5% to 83.3%, the number of floods with Nash-Sutcliffe efficiency coefficient no less than 0.9 increased from 9 to 17, and the average Nash-Sutcliffe efficiency coefficient increased from 0.82 to 0.89, indicating a significantly improved simulation accuracy. It was estimated that the range of energy residual coefficient was [0.008, 0.014] under the flow velocity. In this case, the proportion of flood simulation with the Nash-Sutcliffe efficiency coefficient not less than 0.9 was 44%-50%, which was close to the calibration. It infers that the parameter can be estimated indirectly using the average velocity of the outlet section. Consequently, the concentration model presented a clear physical meaning, whose parameters were determined by the calibration or measurement for the cross and vertical section of the channel. As such, the obtained velocity can be used to simulate the flow concentration of overland and river networks.
