「アルツハイマー病者からみた世界-私はだれになって行くの-」の著者クリスティーン・ボーデンは、その著書で痴呆の初期段階に、より信頼できる診断がより早期になされると同時に、患者の機能をより長く維持させるための抗痴呆薬を利用し、前向きな生活態度を維持することが重要であることを主張している。1)これに対し我が国の現実は、早期に適切な診断を受け治療をしている患者よりも、痴呆を疑われながらも診断未確定の高齢者が多いことが指摘されている。この現状をもたらしている要因とその弊害について概観した。

老人保健施設の痴呆性専門療養棟において,ケア・スタッフが痴呆性高齢者の訴えや行動に対応した場面を,参加観察法で収集した。収集した場面を質的に分析し,ケア・スタッフのケア提供へ影響しているものについて検討した。その結果,以下の点が明らかになった。1.ケア提供へ影響するものとして,【痴呆性高齢者の日常生活支援のあり方】【痴呆性高齢者の思いとのすれ違い】【否定的感情の表面化】の3つのコアカテゴリーが抽出された。2.【痴呆性高齢者の日常生活支援のあり方】には,〔基盤となる姿勢〕〔創造的な支援〕の2つのカテゴリーが抽出され,〔基盤となる姿勢〕には,『入所高齢者への気遣い』『信頼を壊さない』『安全の保障』『入所高齢者の感情への寄り添い』の4つのサブカテゴリーが含まれ,〔創造的な支援〕には,『探索』『入所高齢者の力を生かす』の2つのサブカテゴリーが含まれた。3.【痴呆性高齢者の思いとのすれ違い】には,〔個人的体験の優位さ〕〔個人の意識の差〕〔その場をしのぐ〕の3つのカテゴリーが抽出され,〔個人的体験の優位さ〕には,『体験的ルーティン化』『強引さ』の2つのサブカテゴリーが含まれ,〔個人の意識の差〕には,『優先順位』『見過ごし』『無意識』の3つのサブカテゴリーが,〔その場をしのぐ〕には,『説得』『関心を寄せない』の2つのサブカテゴリーが含まれた。4.【否定的感情の表面化】には,〔混乱〕の1つのカテゴリーが抽出され,〔混乱〕には,『困難感』『答えが出ない』『引きずられ』の3つのサブカテゴリーが含まれた。

痴呆対応型共同生活介護事業(以下、グループホームとする)は、介護保険法施行後急速に増加している.グループホームの利用者は、介護保険施行当初は軽度から中等度の痴呆性高齢者が中心と考えられてきたが、年数を重ねるにつれて痴呆が進行し、重度化する状況にある.そのため、グループホームにおけるターミナルケアについて考えていくことが重要となっている.そこで岩手県におけるグループホームでのターミナルケアの取り組みについて現状を把握し、課題を明らかにすることを目的として調査を行った.調査協力の得られた23ホームのホーム長に対して半構成的面接法による調査を行った結果、グループホームでターミナルケアを行うにあたって、以下の4つの課題が見出された.1.スタッフの質的問題 2.医療面での問題 3.建物の構造上の問題 4.家族の理解の問題また、グループホームにおけるターミナルケアの必然性について、議論が不足し論点が整理されないままの現状においては、今回の調査で見出された課題への対応を含め、ホーム利用者のターミナル期について、個々のグループホームにおける考えにより決定されていた.グループホームにおけるターミナルケアについて、グループホーム全体でもっと真剣に論議され、全体的な合意がなされるような方策が早急にとられなければならないことが示唆された., The number of group homes for dementia patients has increased rapidly after the enactment of the nursing-care insurance law. At first, most group home users were elderly patients with mild to moderate dementia. However, over time, the severity of dementia in group home users has increased. Terminal care in group homes is thus starting to become a relevant issue. The present study was conducted to ascertain the current state of terminal care in group homes in Iwate Prefecture, and to clarify problems associated with terminal care in group homes. Semi-structured interviews were held at 23 group homes, and the following 4 problems with terminal care in group homes were identified : 1. Problems with substance of care staff 2. Problems with medical care 3. Problems with building structures 4. Problems with family understanding Currently, little discussion about the necessity of terminal care in group homes has been initiated, and related issues have not been identified and organized. As a result, terminal care for group home users, including countermeasures for problems identified by the present study, is determined based on the individual approaches of each group home. Group homes need to actively initiate wider discussion of terminal care and promptly establish consistent and uniform measures that can be adopted by all group homes.

The purpose of this study is to examine the transition and installation criteria of the social system, and to clarify the trend and problems of the past research paper about the living environment in a nursing home for the elderly. First of all, we organized about the change of the social policy, and summarized the research paper for the past of 12 years according to the research division and the research item. Moreover, we tried to review and extract the knowledge or opinion of the planning problem of a nursing home for the elderly indicated in these research papers. (1) The arrangement of the bathroom is related to the traffic line distance of a care staff or a resident. It is a problem to establish the bathroom at each unit in the latest nursing home for the elderly. (2) In order to support the wheelchair user, the building plan which can consciously correspond to the wheelchair must be considered at the beginning of reconstruct or new construction. (3) On the terminal care in a nursing home for the elderly, it is a problem to not carry out the research almost. (4) The conclusion is not coming out on the room division between general elderly and the dementia elderly. (5) It is necessary to take in the opinion of the people whose the standpoint differs in the stage of the design in order to produce the comfortable dwelling environment for resident and nursing staff.

痴呆性高齢者自身の経験や体験に関する先行研究のうち，国内文献６件，海外文献６件，計12件の文献の検討を行った．これらの文献を検討した結果，主な傾向として，国内の研究では，中等度から重度である痴呆性高齢者の日常生活における会話や行動をもとに，彼らの経験や体験の意味を探求していた．一方，海外の研究では，軽度から中等度の痴呆性高齢者自身の語りから，痴呆という病気のexperienceについて探求していた．今後の課題として，１. 未だに明らかになっていない研究領域であり，他の患者に適応できる可能性もあるため，痴呆性高齢者自身が語った経験や体験を，できるだけ詳細に記述してゆく．２. 重度になった段階でも，個々の主観を反映できるように，軽度の時期から研究を開始し，継続的な変化を通して捉える．３. 質的研究を行う場合，研究の前提とする理論的な立場について言及する方が望ましい．と，示唆された．

Until 1975, it would appear that the general view was that the illness of imbecility among aged persons was beyond medical improvement mainly due to a lack of emphasis on the importance of care treatment. However in later years, many people came to express wide interests regarding the disease, and as a result, concrete steps have been advanced on care treatment. Today, although there may be little hope for improvement on the patient's brain structure, experience has shown that with proper care, the patient's general condition could vastly improved. In this paper, I wish to cite some cases of five-to-nine aged-persons involved in the Group Home, where the subjects live and do things together under one roof. It is well-known that their general condition keep improving, through a combination of doing light-household chores, resulting to social acquaintances. This writer also studied reports concerning some women suffering from Alzheimer's Disease who were confined in the hospital, where the subjects did almost nothing by themselves. The same subjects were moved from the hospital to the Group Home, with a vastly different environment. Unlike the hospital, subjects in the Group Home had to do light household work, depending on one's limitations. Performance data shows that the ranking needs of the same subjects came down from "three to one". One glaring example was the case of a male patient diagnosed with cerebral apoplexy. In the hospital, the male patient was totally-cared for including staff changing his diapers. Weeks after moving him to the Group Home, he was able to perform some basic functions like walking with a cane unassisted and going to the toilet unattended on daytime all by himself. His over-all personality also changed due to social exposure, he was seen talking with the staff and other patients.

わが国の人口の高齢化は急速に進み,2000年では全人口の6人に1人であった高齢者(65歳以上)が,2025年には3人に1人になると予測されている。それに伴い,痴呆性高齢者の数も確実に増加し,在宅で痴呆性高齢者を介護する家族も増加することが考えられる。今回著者らは,介護者だけでなく介護を受けている痴呆性高齢者に対しても家庭訪問による聞き取り調査を行い現状を把握した。調査の結果,痴呆性高齢者の問題行動は徘徊が最も多く,そのため介護者は自分の時間が持てないことが一番の悩みであった。しかし訪問したうち多くの家庭では,それぞれ介護負担はあるが,介護サービスを利用して介護状況は安定していた。保健師は専門職として介入する役割が求められていることが明らかになった。, As the population of Japan increases, the population of aged people (more than 65 years of age) increases, from one in six in 2000 to probably one in three in 2025. As a result, it is assumed that the number of family who take care of aged demented people at home also increases. We thus made a survey on care-givers and demented people by visiting their homes and found their present situation. The most serious problem the families suffered from was that care-givers are not able to have their own time due to the roam of the demented. Nevertheless, as far as we have visited, the present condition of the care-givers in the family who take care of demented seems stable by using nursing services. この症例報告は,重度の認知障害により日常生活の基本動作ができない高齢者が,入院という環境の変化に適応していく過程をまとめたものである。本研究の目的は,看護者が実践する基本的な生活援助が,痴呆性高齢者の行動や感情に与えた効果について明らかにすることである。対象は老年期痴呆の80歳代女性Yさんである。生活援助特に食事の摂取状況とアクティビティ活動から分析した。その結果,彼女は食事を分割する介助方法によって,自力摂取することが可能となった。また,塗り絵にも積極的に関わるようになった。彼女は入院によって家族から分離させられ,分離不安を起こしていたが,看護(介護)の援助を通して,彼女は家族の代理として,看護師や他のスタッフと良い人間関係を保つことができるようになった。基本的な生活援助の継続は,痴呆性高齢者に残されている可能性を引き出し,痴呆症の経過の遅延に影響を与えることがこの事例から明らかになった。 (英文抄録：Present study described the process of environmental adaptation during the entering a hospital in the patient with severe cognitive disorder who unable to daily action alone. As the result, it became possible that she took a meal herself by assistance method for dividing the meal, and it would be also engaged in activities such as a line drawing for coloring. Her psychological status became high anxiety when separated from her family. However, she could keep good human relations by humanistic nursing intervention. This study suggested that the continuation of the fundamental life leads to increased ADL and activities in the demented elderly.)

The ultra-high age society is coming and the correspondence of the dementia elderly to the increase is hurried up. The guideline of living environment plan and architectural planning, which reflect behavioral characteristics of the dementia elderly, has been required. Then, we arranged the knowledge of living environment or architectural planning from the published research on the dementia elderly, in order to clarify the problem on the planning of group home for dementia elderly. And, we examined it on the basis of the description of problem in the group home of book or magazine that problem and request are reported by well-informed person or care staff. (1) In the published research, it has been indicated that various elements of the living environment affect the dementia elderly. Especially, though the life in the small-scale space gives the beneficial effect in action and body situation, knowledge on floor planning is not found. (2) The opinions by care staff on the architectural planning for the dementia elderly is mainly viewed, and there are some contrary indications in those opinions. Contrary indication is often the spatial composition and layout of common space, and not opposite opinion often concerns the plan and specification of various rooms. (3) It was possible to clarify the current of the opinion on architectural planning by the care staff for the dementia elderly, by classifying the floor plan of group homes into three basic types and examining those opened years.

本研究では、痴呆性高齢者のQOLに関する国内外の42文献から「１．痴呆性高齢者におけるQOLの概念」、「２．痴呆性高齢者におけるQOLの尺度開発」、「３．痴呆性高齢者のQOL向上に関わる日本におけるケア」の3点について検討した。その結果、痴呆性高齢者におけるQOLの概念は身体的・精神的・社会的well-beingの3つの要素から提案されていたが、QOLの概念化に完全な意見の一致はなかった。また、痴呆性高齢者とその介護者のQOLは互いに影響しあう存在であることが示唆されていた。痴呆性高齢者におけるQOLの尺度は、現在も開発中のものが多数であった。痴呆性高齢者へのケアは、「快適性」、「なじみの人間関係」、「日常生活活動能力」が必要であり、心地よい療養生活に主眼が置かれていた。さらに、回想法、音楽療法などは、彼らに内在する能力を引き出し、QOLを向上させる手段となる可能性が報告されていた。

近年,痴呆性高齢者に効果的なケア環境を提供し得る居住の場として,痴呆性高齢者のグループホームはその重要性が認識され全国的な広まりを見せている。一方,その運営主体をみると,(1)介護保険制度による指定事業が始まる以前から運営してきた,民間ベースの小規模多機能ホーム(宅老所やデイホームなど)と,(2)介護保険制度の指定事業を受けるグループホームの2つに大きく分かれている。立地状況については,(1)施設に併設・隣接,あるいは(2)単独に立地するタイプ,また建物形態に関しては,(1)新築,あるいは(2)住宅・寮・ビルなど既存の建物を改築するタイプなどがあり,種々な形態のホームが創設されている。本研究では,このように多様化するグループホームにおいて,望ましい物的な環境条件を考察することを目的とし,新築および既存の建物を改造したグループホーム計35件を対象に,運営・住まい方について訪問・ヒアリング調査を行った。その結果,以下の基礎的知見が得られた。(1)グループホームの計画においては,公私の生活空間をバランス良くデザインする点に考慮することが重要となる。(2)個の空間については,私的な生活を確保するのに必要なスペースを確保するとともに,プライバシー確保のためには便所を設置することが求められる。(3)共用空間の計画については,大勢で活動できるスペースおよび2,3人でプライベートに利用できるスペースなどが必要とされる。(4)個の空間との共用空間(居室と廊下,居間や食堂など)の間には,緩衝的空間をつくり,プライバシーに充分配慮することが必要である。

Until 1975, the importance of caring for elderly patients suffering from dementia was not sufll addressed, that is, the possibility of healing was not explored. However, in the recent years, thcr been much interest and concern regarding the illness. Various ways, some in concrete forms hav devised aimed at improving care of patients with different conditions. For example in the imbecility group home eare, it is a well-known fact that group their conditi* ine mli be greatly improved by keeping a mental balance. Doing simple light chores, such as preparing together or shoppiing together stimulate one's conscious concentration by way of recollection. the process leads to rehabilitation remedy. Since care treatment is not involved in this proce subject of care bears no relation to the illness. The group home care concept is ideal simc patients-from five to nine-arc placed in the same living environment, where they take tin nsing domestic chores, drawing from their talents and skills. After moving into the group home, the women patients were required light chores on their own, "the number of women patients in need of care came down from three Some were able to fit themselves with diapers and were able to take a few footsteps. The author will explore this ease study based on the facts made available' in the report. 痴呆性高齢者の生活環境を評価するために、高齢者の感情と行動に着目し、生活環境の影響について行動分析的手法による事例検討を行った。施設に入所する痴呆性高齢者1名を観察対象者とし、ビデオカメラを用いて2時間毎に10分間、5回の観察を行った。10秒間1コマとする時間間隔記録法によって得たデータを分析した。結果は以下のとおりである。1)「喜び」では観察対象者・援助者相互に定位や発語が観察された。「満足感」では頭をゆっくり上下させる行動が観察された。「不安/恐れ」では処置を受ける他の高齢者への定位が観察された。2)観察セッション中に援助を行った援助者は5名であった。「喜び」は3名の援助者が介助した際に観察され、援助者の対象者への定位(出現率88.9〜64.3%)は周囲への定位(出現率0.0〜57.1%)よりも多かった。残る2名では対象者への定位(出現率63.3%、44.1%)は周囲への定位(出現率86.7%、52.9%)よりも少なかった。3)観察対象者では特定の音楽に合わせて頭をゆっくりと上下させる行動が観察された。観察対象者の感情や行動は、援助者や他の高齢者の行動、音楽等の生活環境によって影響されると考えられた。本研究方法は痴呆性高齢者の生活環境の評価に活用できると考えられた。, To evaluate living environment for elderly with dementia, with a focus on the emotions and behavior of the elderly person, a case study of the effects of the living environment was conducted by behavior analysis. The subject was an elderly institutional resident with dementia, who was recorded on video for ten minutes every two hours for five observations in total. Data were collected with interval recording in which ten seconds was taken as one frame, and then analyzed. The following results were obtained : 1) The mutual orientation and language between the subject and carer were observed in "Pleasure". The subject's behavior of slowly nodding the head up and down was observed in "Contentment". The subject's orientation to other elderly persons receiving treatment was observed in 'Anxiety/fear". 2) There were five carers who provided care in the observations. "Pleasure" was observed when assistance was provided by three of the carers. The orientation of these carers toward the subject (ratio 88.9%-64.3%) was greater than their orientation toward the surroundings (ratio 0.0%-57.1%). With the other two carers, the orientation toward the subject (ratio 63.3%, 44.1%) was less than that toward the surroundings (ratio 86.7%, 52.9%). 3) The subject was observed to slowly nod his head up and down when specific music was played. The emotions and behavior of the present subject was thus affected by the living environment, such as the behavior of the carers and other elderly, and music. The present method may be applicable in the evaluation of living environment for elderly with dementia.

グループホームで生活している痴呆性高齢者に対し、アクティビティの一環として、下肢の筋力維持・向上に重点をおいた運動を計画、実施し、利用者の行動特性とアクティビティケア評価表からその有効性を評価し、運動介入の効果について検討した。その結果、運動に対する行動特性として、【動きが出る】、【意欲的に行う】、【感情表出する】、【交流する】の4つの概念が創出された。アクティビティケア評価表による評価では、積極的に参加した者に実施中の積極性や集中度、コミュニケーション、表情の点数が高い傾向にあった。以上の結果から、グループホームでの運動の実施は、単にADLの維持・向上だけでなく、痴呆性高齢者の感覚機能の活性化や社会性の向上、痴呆症状の改善に有効であることが示唆された。

It is one of the important problems of the elderly with dementia that consolidate living environment. We need to clarity the principle of housing in order to realize that family take care of the elderly with dementia in the home. As a foundation study, this study aims to understand the life style of the elderly with dementia and their family. The action of the elderly with dementia at home is large different from former action when they had the symptom of dementia. Especially, vascular dementia is taking a passive action because of a paralysis and intellectual disorder. Therefore, the bedroom of the elderly is moved near the living room. On the other hand, Alzheimer disease is taking a passive action as vascular dementia, and often they move around in the home.

Now in our country we try to change the system of social welfare and we have more interest in how we make the user in the service. Especially, it is an important subject how we support the user’s self-determination and self-choice. Traditionally, it had been emphasized as a general rule in practice of social welfare that we paid respect to user’s self-determination. But only it makes it easy for us to cope on the surface, while the population of the elderly people having dementia, for example, have increased. So we need not only to pay respect to user’s self-determination but also to look over what user’s self-determination should be including the point of view that we should support each other. About the people who have difficulty in doing self-determination and the people who have little power, it is important how we should support and also how we should the understanding the support in addition to paying respect to user’s self-determination. We should not restrict supporting user’s self-determination to user’s own development of personality and one side of urging them to adapt. Always we must understand supporting the user’s self-determination in relation between the social support system, right security, right protection and so on. We are asked for the supports that the individual gains own power by and that the individual works on society on the basis of it is requested. For it, we need to put a level of empowerment, and to examine it to support user’s self-determination.

グループホームの生活環境やケアが痴呆性高齢者(利用者)の行動にどのような影響を及ぼしているかを検討することを目的に、平成11年度から13年度にかけて、利用者の行動特性の変化を参加観察法により明らかにした。その結果、利用者がグループホームの生活になじみ、精神的に安定した生活を送っていることや、その人らしさを保持していることがわかった。その反面、利用者の痴呆症状の進行と、老化や痴呆の進行による身体機能の低下がみられた。以上のことから、グループホームケアの効果と意義および課題について考察した。

本研究はグループホームの痴呆性高齢者の行動特性を大集団ケアの場として代表的な介護老人保健施設の痴呆性高齢者の行動特性と比較し、グループホームケアの意義および効果について検討することを目的とした。研究方法は、グループホームに入居している痴呆性高齢者の行動を参加観察法により抽出し、内容分析の手法に基づき質的帰納的に分析した。介護老人保健施設においても同様の方法で行った。その結果、グループホームでは【仲間との交流】や【スタッフとの交流】が老健よりも多く、老健では、【その人らしい日常生活行動】や【単独行動】の個人行動が多かった。また、グループホーム特有の行動として【アタッチメント】、【役割行動】があった。これらの結果からグループホームケアの意義および効果について考察した。

本研究はグループホームにおける痴呆性高齢者の行動特性を明らかにすることを目的とした。グループホームに入居している痴呆性高齢者の行動を参加観察法により抽出し、内容分析の手法に基づき帰納的・質的に分析した。その結果、グループホームにおける痴呆性高齢者の行動特性として、【自由な自己表出】、【家族・訪問者との交流】、【内輪の仲間・スタッフとの交流】、【その人らしい日常生活行動】、【痴呆に伴う行動】、【家事の役割分担】の6つの概念が創出された。以上の結果から痴呆性高齢者のグループホームにおけるケアの効果と意義について考察した。

高齢社会の到来により、痴呆性高齢者および老々介護が増加している。今回、在宅酸素療法を行う痴呆性高齢者の訪問看護を行った。同居の妻も痴呆があり、長男はアルコール依存というキーパーソン不在の対処困難な事例であった。妻の痴呆症状の悪化と長男のアルコール依存の進行により、患者は身体・精神的な負担が増加し、幻覚・妄想を訴え始めた。訪問看護師の役割として、内的な世界を理解し支持的な関わりを持つように努めた。これらの対処法は効果があり、幻覚・妄想を軽減することができた。また、家族の状態が痴呆性高齢者の心身に大きな影響を及ぼすことから、家族全体に視点を置きケアを提供した。さらに、在宅生活を継続するためには社会資源の導入が必要であり、そのためにはヘルパー・ケアマネジャーとの連携が必要であった。
