目的　探讨环泊酚复合中/小剂量艾司氯胺酮静脉麻醉在老年无痛胃肠镜检查术中的应用效果及安全 性。方法　收集行无痛胃肠镜检查术的老年患者195例，采用随机数字表法分为对照组、小剂量艾司氯胺酮组、中 剂量艾司氯胺酮组各65例。对照组、小剂量艾司氯胺酮组及中剂量艾司氯胺酮组分别静脉推注0. 10 mL/kg生理盐 水、0. 15 mg/kg艾司氯胺酮及 0. 30 mg/kg艾司氯胺酮复合 0. 30 mg/kg环泊酚进行麻醉诱导，术中根据情况追加环 泊酚麻醉。记录各组环泊酚消耗量，包括诱导消耗量及内镜检查期间追加消耗量，计算环泊酚总消耗量（诱导消耗 量+追加消耗量）；分别于麻醉诱导前5 min（T0 ）、麻醉诱导后即刻（T1 ）、胃镜插入即刻（T2 ）、结肠镜检查前即刻（T3 ）、 结肠镜检查后（T4 ）、患者苏醒期首次睁眼时（T5 ）记录各组血流动力学参数收缩压（SBP）、舒张压（DBP）、心率（HR） 及血氧饱和度（SpO2 ）；记录各组麻醉诱导时间、胃镜操作时间及患者苏醒时间；记录术中不良事件（低氧血症、低血 压、咳嗽、体动）及术后不良反应（恶心、呕吐、头晕及流泪、激动、噩梦、幻觉、谵妄）发生情况。结果　与对照组比较， 小、中剂量艾司氯胺酮组环泊酚诱导消耗量及总消耗量均降低，中剂量艾司氯胺酮组环泊酚追加消耗量降低；与小 剂量艾司氯胺酮组比较，中剂量艾司氯胺酮组环泊酚诱导消耗量及总消耗量均降低（P均<0. 05）。与同组T0时点比 较，对照组在 T1～T5时点 SBP及 DBP均降低、SpO2均升高，在 T2～T5时点 HR 均降低；小剂量艾司氯胺酮组在 T1～T5 时点 SBP均降低、SpO2均升高，在 T2～T4时点 DBP均降低，在 T3～T5时点 HR 均降低；中剂量艾司氯胺酮组在 T2～T5 时点 SBP均降低、SpO2均升高，在 T2～T4时点 DBP均降低，在 T3～T5时点 HR 均降低（P均<0. 05）。与对照组同时点 比较，小剂量艾司氯胺酮组 T1～T2时点 SBP、DBP 均升高，中剂量艾司氯胺酮组 T1～T2时点 SBP 升高，T1～T3时点 DBP、HR 均升高（P均<0. 05）。各组麻醉诱导时间、胃镜操作时间及患者苏醒时间比较差异均无统计学意义（P均 >0. 05）。小剂量艾司氯胺酮组术中体动发生率低于对照组，中剂量艾司氯胺酮组术中低氧血症、低血压、咳嗽、体 动发生率均低于对照组（P均<0. 05）；各组术后不良反应发生率比较差异均无统计学意义（P均>0. 05）。结论　环 泊酚复合中/小剂量艾司氯胺酮静脉麻醉在老年无痛胃肠镜检查术中具有较好的麻醉效果及安全性，当复合小剂量 艾司氯胺酮时患者血流动力学更稳定、术中不良反应更少。 [ABSTRACT FROM AUTHOR]

Objective To observe the clinical efficacy and safety of sacubitril-valsartan in patients with reduced ejection fraction and hypotensive heart failure. Methods A total of 100 patients with decreased ejection fraction combined with hypotension and heart failure who were admitted to the Department of Cardiology of Kunming First People’ s Hospital from January 2020 to December 2021 were included in the study. The patients were randomly divided into an observation group and a control group, with 50 cases in each group. The patients in the two groups were given standard treatment for heart failure including diuresis and β -receptor blockers. The patients were given sacubatril valsartan at a small dose of 25–50 mg twice a day. According to the patient’s tolerance, the dose was doubled and increased once every 2–4 weeks until the maintenance dose was reached. After 12 months of treatment, the LVEF, LVDd and LVDs of cardiac function in two groups were compared before and after treatment, and the SBP, DBP, blood K+, blood Na+, CTNT, NT-proBNP were observed. Results After treatment, the LVEF of patients in the two groups was increased, while the LVDd and LVDs were decreased. There was statistical difference in LVEF and LVDs between the two groups (P < 0.05), but no statistical difference in LVDd (P > 0.05). After treatment, the levels of blood K+, blood Na+, and CTNT increased in the two groups, there was no significant difference between the two groups after treatment (P > 0.05); NT proBNP decreased significantly after treatment (P < 0.05). After treatment, the systolic blood pressure in the experimental group increased compared with that before treatment, while there was no significant change in the control group (P > 0.05), and there was no significant difference in diastolic blood pressure between the two groups (P > 0.05). Conclusion The treatment of HFrEF combined with hypotension in patients taking sacubatril valsartan can improve heart function, it has no effect on blood pressure, and is safe. : The principle of fast spin echo ( FSE ) producing stimulated echo and the relationship between stimulated echo and the phase angle of radio frequency pulse were investigated. For fast spin echo pulse sequence, a stimulated echo artifact correction method was proposed. The spin echo and stimulated echo were separated by two times of excitations, then the area of dispersion gradient was adjusted to make the center of stimulated echo coincide with that of the spin echo, and the phase angle of 180° refocusing pulse was modified to make the phase of stimulated echo and spin echo consistent. The correction method was tested and validated on a 1. 5 T superconducting MRI scanner. After stimulated echo artifact was corrected, the signal to noise ratio of the FSE image was improved by 57. 08%, while the image artifact was reduced by 76. 12% . The experimental results showed that the proposed method can effectively suppress stimulated echo artifacts on FSE image and improve the image quality. [ABSTRACT FROM AUTHOR]

The effect of eddy currents on diffusion-weighted echo-planar imaging(EPI) was investigated for permanent magnetic resonance imaging(MRI) scanner. An automatic eddy currents calibration algorithm was presented， and a compensation method for phase errors caused by residual eddy currents in diffusion-weighted EPI was proposed. The method was tested and validated on a 0.35T permanent magnet MRI scanner.For three different gradient directions after pre-emphasis compensation， the eddy currents reduced to at least 1/15 of that before compensation. The diffusion-weighted EPI images with and without eddy currents compensation were compared. After two times of compensations， the geometric distortion was reduced by 5% and artifacts was reduced by 1/3. The experimental results showed that the proposed algorithm can effectively suppressed the diffusion-weighted EPI artifacts and distortions caused by eddy currents. [ABSTRACT FROM AUTHOR]

After large-scale electric vehicle is connected to the distribution network，the load of the distribution network will increase，and then the power quality of the distribution network will be affected． Therefore，a reliability analysis method of power grid based on component failure rate change is proposed in combination with the actual change of component failure rate． According to various factors related to charging power demand，the charging power of the electric vehicle is modeled，and the load variation of the original distribution network caused by the access of electric vehicle to the distribution network in different scales is simulated． Considering the change of component failure rate，the Weibull distribution is used to model the bathtub curve of component failure rate． The reliability of power grid is evaluated by Monte Carlo method． In reliability analysis，the real-time failure rate of components is taken as the criterion． Taking the IEEE － RBTS Bus2 distribution system as an example，the reliability indexes of each load point change with time are analyzed and calculated，and the relationship between the reliability indexes and the time of the system under different scale electric vehicles connected to the distribution network is studied． The reliability index calculated by this method is closer to reality． [ABSTRACT FROM AUTHOR]

Objective: To explore the role of miR-1290 in epithelial-mesenchymal transition of cervical cancer Hela cell. Methods: The radioresistant cervical cancer Hela cells and Siha cells were established by applying fractionated radiation consisted of a fraction dose of around 2 Gy of μ-ray. The differential expression patterns of miRNAs between radioresistant cells and parental cells were evaluated using miRNA microarray. The expression level of miR-1290 was detected in Hela cells treated under different conditions of hypoxia and radiation. The lentivirus of miR-1290 and miR-1290-inhibition were used to modulate the expression of miR-1290 in Hela cells. The wound healing and Transwell were used to compare the abilities of migration and invasion of Hela cells with different expression level of miR-1290. Western Blot was used to detect the expression level of E-cadherin and N-cadherin. Results: MiR-1290 expression were significantly upregulated in radioresistant cervical cancer cells (P<0.05). The radiation and hypoxia induced the expression of miR-1290 in cervical cancer Hela cells (P<0.05). Up-regulating the expression of miR-1290 enhanced abilities of migration and invasion of Hela cells and Hela cells presented the mesenchymal cell phenotypes (P<0.05). The miR-1290 overexpression decreased the expression of E-cadherin and increased the expression of N-cadherin in Hela cells (P<0.05). Conclusions: These results demonstrated that hypoxia and radiation up-regulated the expression of miR-1290 in cervical cancer Hela cells and miR-1290 promoted the EMT of Hela cells by modulating the expression of E-cadherin and N-cadherin.
