目的　探讨支气管哮喘患儿外周血Toll样受体4 (TLR4) 基因多态性及其与哮喘发病的关系。方法 172 例哮喘患儿 (观察组) ，根据哮喘急性发作严重程度分为轻度34例、中度35例、重度45例和危重58例，另选择136例 健康体检儿童为对照组。抽取观察组病情发作期、对照组体检时外周静脉血，采用聚合酶链式反应-限制性内切酶 片段长度多态性法检测TLR4基因，采用二元Logistic回归分析不同TLR4基因位点基因型儿童哮喘发病的危险性。 结果　观察组患儿 TLR4 基因 Thr399Ile 位点 CC、CT、TT 基因型分别为 31、73、68 例，等位基因 C、T 分别为 135、209 例，对照组分别为 42、60、34、144及 128例；与对照组比较，观察组 TLR4基因 Thr399Ile位点 T等位基因分布频率高 (P<0. 05) 。与 CC 基 因 型 比 较，TLR4 基 因 Thr399Ile 位 点 TT 型 人 群 哮 喘 发 病 的 危 险 性 高［OR=2. 353 (95% CI 1. 575～4. 663) ；P<0. 05］。结论　哮喘患儿TLR4基因Thr399Ile位点的基因型以TT为主，等位基因T分布频率高。 TLR4基因Thr399Ile位点多态性可能与支气管哮喘的发病有关。 [ABSTRACT FROM AUTHOR]
