的　观察后路病灶清除植骨融合内固定术治疗经皮椎体成形术(PVP）术后椎体感染的临床效果. 方法 PVP术后椎体感染的患者13例，均接受后路病灶清除植骨融合内固定术治疗，观察并比较患者术前, 末次随 访时的感染指标(ESR, CRP）, 疼痛评分(VAS）, 神经功能分级(ASIA）. 结果 13例患者均顺利完成后路病灶清除 植骨融合内固定术治疗，无切口感染, 窦道形成, 神经症状加重或死亡等严重并发症. 1例患者切口延迟愈合，对症 处理后痊愈. 随访期间未发现内固定松动, 断裂, 神经功能障碍加重等情况. 13例患者术前ESR, CRP和VAS评分 分别为(48. 9 ± 21. 4）mm/h, (40. 2 ± 33. 9）mg/L, (8. 1 ± 1. 2）分，末次随访时 ESR, CRP 和 VAS 评分分别为(14. 2 ± 2. 9）mm/h, (2. 8 ± 1. 2）mg/L, (2. 2 ± 0. 7）分，末次随访时ESR, CRP和VAS评分与术前相比，P均<0. 05. 13例患者 中，有神经损害症状的 5例患者的 ASIA分级由治疗前的 D级恢复至 E级，无神经损害症状的 8例患者的 ASIA分级 未发生变化. 结论　后路病灶清除植骨融合内固定术治疗PVP术后椎体感染的临床效果较好，术后椎体感染指标 得到控制, 疼痛评分下降, 神经功能恢复. Combined liver and kidney transplantation has been steadily applied in major transplantation centers, saving the lives of many patients with end-stage liver and renal failure. However, there are still multiple unresolved problems in the clinical diagnosis, treatment and long-term prognosis of combined liver and kidney transplantation. By referring to "Technical Operating Standards for Combined Liver and Kidney Transplantation (2019 Edition)" and the latest published literature and guidelines at home and abroad, "Guidelines for Clinical Diagnosis and Treatment of Combined Liver and Kidney Transplantation" was formulated. Recommendations and suggestions were delivered regarding the surgical indications and contraindications, preoperative preparation and evaluation, the timing of transplantation for endstage liver and kidney diseases, the selection of surgical approaches and postoperative follow-up, specific pathophysiology, surgical techniques, complication management and immunosuppressive treatment of combined live and kidney transplantation, aiming to enhance the utilization rate of grafts and improve the survival and prognosis of combined liver and kidney transplant recipients. BACKGROUND: The basic principle of the design of the robot system used for total knee arthroplasty is to combine three-dimensional surgical planning, early warning of danger areas during surgery, real-time data feedback, robot arm assisted osteotomy and other technologies to achieve precision and personalization of total knee arthroplasty, which is exactly its biggest advantage. Therefore, it has become a hot topic in the field of joint surgery in recent years and attracted much attention. OBJECTIVE: To summarize the development status of robotic-assisted total knee arthroplasty in the field of joint surgery and the comparison of advantages and disadvantages with conventional total knee arthroplasty and to prospect the future development of robotic-assisted total knee arthroplasty. METHODS: Relevant articles were searched from PubMed, CNKI, Wanfang and VIP databases by computer. English key words were “robot OR robotic OR robotics OR robotically OR computer, total knee arthroplasty OR total knee replacement, TKA OR TKR”. Chinese key words were “robotic-assisted, computer navigation, total knee arthroplasty”. Finally, 64 articles were included for review and analysis. RESULTS AND CONCLUSION: (1) The robot system used to assist total knee arthroplasty is divided into active, semi-active and passive according to its degree of freedom. The semi-active system, currently widely used in robotic systems, effectively enhances the accuracy and personalization of total knee arthroplasty. However, its high implementation cost and relatively steep learning curve remain key factors to be balanced when promoting its adoption in joint surgery field. (2) Robotic-assisted total knee arthroplasty can achieve precise osteotomy and correct placement of prosthesis in local three-dimensional space of the knee joint. It has been widely proven that it can provide better accuracy of prosthesis implantation, reduce imaging abnormalities, obtain good soft tissue balance during the operation, and ultimately improve the motion and functional status of the knee joint after the operation. (3) However, the current robotic-assisted system still has objective shortcomings, including the problem of learning curve between different robot devices and operators, additional installation and maintenance costs, and potential complications related to robot surgery. Therefore, whether it can truly benefit the medical system and patients still needs to be proven by long-term research, and the robotic-assisted system also needs to be further improved substantially. (4) Robotic-assisted total knee arthroplasty technique is still in the preliminary research stage in clinical practice and has not been widely applied. To better define the usage of robotic-assisted total knee arthroplasty and enhance its clinical procedural standards and safety, refining these aspects will become a focal point of future research on robotic-assisted total knee arthroplasty. Objective To investigate the risk factors of postoperative complications in patients with spinal tuberculosis and analyze the value of prognostic nutritional index （PNI） in predicting these complications. Methods The clinical data of 156 patients with spinal tuberculosis who underwent surgery in the Affiliated Hospital of Zunyi Medical University from January 2018 to July 2022 were retrospectively analyzed. The patients were divided into a complication group and a non-complication group based on the presence or absence of postoperative complications. Baseline data, laboratory indicators, and surgery-related indicators were compared between the two groups. The risk factors for postoperative complications in spinal tuberculosis were analyzed, and receiver operating characteristic （ROC） curves were plotted to evaluate the predictive value of PNI for postoperative complications in the patients. Results Among all of 156 patients, 68 contracted a total of 82 instances of postoperative complications, with an incidence of 43.59%. Coinfection with pulmonary tuberculosis, preoperative anti-tuberculosis treatment duration more than 4 weeks, surgical operation duration, and drainage days were identified as independent risk factors for postoperative complications in spinal tuberculosis （P < 0.05）. On the other hand, a higher PNI was found to be a protective factor against postoperative complications of the spinal tuberculosis （P < 0.05）. The area under the ROC curve for PNI predicting postoperative complications of the spinal tuberculosis was 0.805. Conclusion The risk of postoperative complications in patients with spinal tuberculosis is subject to such factors ascoexistence of pulmonary tuberculosis, preoperative anti-tuberculosis treatment duration, surgery duration, drainage duration, and preoperative PNI. The incidence of postoperative multi-system complications, such as pulmonary complications and sepsis, as well as mortality of patients with COVID-19. Therefore, patients with COVID-19 should be fully recovered before elective surgery and a minimum of 7 weeks recovery time is routinely recommended. At the same time, the occurrence of adverse outcomes such as postoperative complications in patients with COVID-19 should be considered in a multidimensional manner, taking into account the related risk factors such as age, severity of the disease, type of surgery, anesthesia method, underlying comorbidities, type of viral strain, vaccination status and other related risk factors. Situs inversus totalis (SIT) is a rare congenital condition, with an extremely low incidence. There is no difference between SIT individuals without onset of diseases and healthy counterparts. However, when SIT individuals suffer from diseases, the diagnosis and treatment are highly challenging due to insufficient understanding of SIT populations, especially for those complicated with end-stage liver disease and requiring liver transplantation. It is a huge challenge for surgeons whether SIT individuals serve as donors or recipients of liver transplantation. In this article, recent case reports related to liver transplantation in SIT patients were summarized, and the development, key procedures, clinical prognosis and postoperative complications of liver transplantation in SIT patients were reviewed. Objective： This study aims to review the magnetic resonance imaging (MRI)features of normal grafts at different stages of signal characteristics and complications after anterior cruciate ligament reconstruction (ACLR). Methods： The imaging data of 299 patients who underwent MRI for reexamination after the ACLR at the First Affiliated Hospital of Nanjing Medical University from January 2012 to March 2023 were reviewed, and were classified according to the different manifestations of graft complications on MRI. Their signal on T1WI and T2WI was described and combined with their clinical symptoms. Results： In 299 patients, signal was highest at 4-6 months postoperatively. Post⁃transplantation complications were observed in 66 (22.07%), of which 5 (1.67%)had an anterior displacement of the tibial tunnel, and 1 (0.33%) had a slightly posterior position of the tibial tunnel. Cyclops lesion was seen in 16 (5.35%), re⁃rupture of the graft occurred in 10 (3.34%), and 1 (0.33%)had a postoperative infection. Cystic signal in the tunnel were present in 33 (11.04%) at various times postoperatively. There are 2 patients who underlying anterior displacement of the tibial tunnel had both re⁃rupture and Cyclops lesion of the graft. Conclusion： MRI plays an important role as a major tool for postoperative follow ⁃ up of patients after ACLR. It can clearly reflect the changes of the graft at different stages and the manifestation of complications, providing early detection and warning, so that clinicians can take early interventions to improve the quality of life for patients after surgery. Objective To investigate the preventive effect of sivelestat sodium on postoperative delirium(POD) in patients with acute Stanford type A aortic dissection(ATAAD) and its mechanisms. Methods Eighty patients undergoing emergency surgery for ATAAD were included. They were randomly divided into two groups: the sivelestat sodium group(n=40) and the control group(n=40). In the sivelestat sodium group, sivelestat sodium was intravenously infused at a rate of 10 minutes before the induction of anesthesia until the end of the surgery. In the control group, an equal volume of normal saline was intravenously infused at the same rate until the end of the surgery. The occurrence of postoperative delirium(POD) was assessed using the 3-minute Diagnostic Assessment for Confusion Assessment Method(3D-CAM). Peripheral venous blood samples were collected at 24 h after surgery to extract peripheral blood mononuclear cells(PBMCs) using a modified Ficoll density gradient centrifugation method. The concentration of iron ions(Fe2+ ), malondialdehyde(MDA) content, and superoxide dismutase(SOD) activity in PBMCs were measured using colorimetric assay. The protein levels of long-chain acyl-CoA synthetase 4(ACSL4) and glutathione peroxidase 4(GPX4) in PBMCs were assessed using Western blotting. Perioperative adverse events were recorded. Results The incidence of POD in the cilostazol sodium group was lower than that in the control group(P<0. 05), but there were no significant differences in the severity score or duration of POD between the two groups. The cilostazol sodium group had lower Fe2+ concentration and MDA content, and higher SOD activity in PBMCs than the control group(all P<0. 05). The expression of ACSL4 protein in PBMCs was lower, while the expression of GPX4 protein was higher in the cilostazol sodium group than in the control group(all P<0. 05). Compared with the control group, the cilostazol sodium group had shorter ICU stay and postoperative hospitalization(both P<0. 05). There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion Sivelestat sodium may prevent the occurrence of POD in patients with ATAAD, and its mechanism may be related to the inhibition of ferroptosis, thereby reducing neurological damage. Objective To investigate the effect of quality control circle (QCC) activities in reducing the incidence of postoperative complications in patients with orbital wall fracture. Methods The problem-solving QCC method was adopted to investigate the current status and factors of postoperative complications in patients with orbital wall fracture. Targeted nursing interventions were carried out as follows: A modified method ophthalmic pressure bandaging had been used; the three-dimensional teaching management model consisting of precision teaching, precision management; fine nursing and exquisite technique; A multipurpose ice pack had been designed to relieve the swelling; A multipurpose semi-reclining position non-slip pad had been designed. Results The incidence of postoperative complications was decreased after implementation of QCC activities. Conclusion The QCC activity is effective to reduce the incidence of postoperative complications in patients with orbital wall fracture. Objective To investigate the application of finger-pressing on Tiantu acupoint to induce cough in postoperative patients with early-stage lung cancer. Methods Totally 100 patients with peak expiratory flow (PEF) ≤5. 33L/s who were admitted to the Hospital from January 2023 to March 2023 were randomly divided into observation group and control group, with 50 cases in each group. The control group received conventional nursing measures, while the observation group received finger-pressing therapy on Tiantu acupoint based on the basis of the control group. The time taken to cough up the first sputum after nebulization on the first postoperative day, the difference of Peak Expiratory Flow (PEF) on first day of hospital admission and three days after surgery, postoperative pulmonary complications and intervention effect on coughing were compared between the two groups. Results The rate of effective cough-inducing in the observation group was significantly higher than that of the control group(P＜0. 01). The time (h) to cough up the first sputum after nebulization on the first postoperative day was shorter in the observation group (1. 73±0. 81) than in the control group (3. 02±0. 88), with a significant difference(P＜0. 01). The incidence of postoperative pulmonary complications in the observation group was lower than that in the control group (P＜0. 01). The rank of the difference of PEF on first day of hospital admission and three days after surgery was lower in the observation group than that in the control gorup(P＜0. 05). Neither of the groups reported adverse reactions. Conclusion Finger-pressing therapy on Tiantu acupoint to induce coughing is effective in coughing and sputum evacuation in early-stage lung cancer patients with PEF ≤ 5. 33 L/s, and reduces pulmonary complications. (目的 探讨指压天突穴引咳法在早期肺癌术后患者中的应用。方法 便利抽样法选取天津市某三级甲等肿瘤专科医院肺部肿瘤科2023年1月—3月收治的100例呼气峰值流速(PEF)≤5. 33L/s的早期肺癌切除术的患者, 随机分为观察组和对照组, 各50例。对照组接受常规护理措施, 观察组在对照组基础上实施指压天突穴引咳法干预。比较两组患者术后第1天雾化后咳出第一口痰的用时、术后3 d晨起PEF与入院PEF差值、肺部并发症发生情况和咳痰效果。结果 观察组咳痰效果评价优于对照组, 差异有统计学意义(P＜0. 01)。观察组术后第1天雾化后咳出第一口痰用时(1. 73±0. 81)h, 较对照组(3. 02±0. 88)h缩短, 差异有统计学意义(t=3. 447, P＜0. 01)。观察组肺部并发症发生率低于对照组, 差异有统计学意义(P＜0. 01)。观察组入院及术后第3天晨起的PEF差值的秩均值小于对照组(P＜0. 05)。两组均未出现严重或者影响机体状态的不良事件。结论 指压天突穴引咳法在PEF≤5. 33L/s的早期肺癌术后患者咳嗽排痰中应用效果明显。)

Objective To investigate the effect of f fine nursing management for patients undergoing simultaneous bilateral lobectomy. Methods Totally 52 patients under simultaneous bilateral lobectomy were randomly divided into the control group and observation group, with 26 cases in each group. The control group received routine nursing interventions during perioperative period. The observation group was given fine nursing management. The indwelling time of catheter, length of postoperative hospital stay, hospitalization costs and postoperative complications were recorded. Results The indwelling time of drainage tube and length of postoperative hospital stay in the observation group were shorter than those in the control group(P＜0. 05). There was a case of pneumothorax, 1 case of stage I pressure injury, 5 cases of arrhythmia, 2 cases of breathing difficulties and 2 cases of incision infection in the observation group. There was a case of pneumothorax, 3 cases of stage I pressure injury, 5 cases of arrhythmia, 1 case of breathing difficulties, 5 cases of incision infection and 1 case of death in the control group. Conclusion Implementation of fine nursing management is effective to shorten indwelling time of drainage tube and length of postoperative hospital stay of patients undergoing simultaneous bilateral lobectomy. (目的 探讨同期双侧肺叶切除围手术期实施精细化护理干预的效果。方法 选取2015年1月—2021年12月医院收治的行同期双侧肺叶切除患者52例, 随机分为对照组和观察组, 各26例。对照组围手术期采取常规护理, 观察组围术期采取精细化护理干预。记录和分析两组患者引流管留置时间、术后平均住院日、住院费用和术后并发症。结果 观察组患者引流管留置时间、术后平均住院日较对照组缩短, 差异有统计学意义(P＜0. 05)。观察组术后发生气胸1例, I期压疮1例, 心律失常5例, 呼吸困难2例, 切口感染2例; 对照组术后气胸1例, I期压疮3例, 心律失常5例, 呼吸困难1例, 切口感染5例, 死亡1例。结论 同期双侧肺叶切除围手术期实施精细化护理干预, 能缩短患者引流管留置时间和术后平均住院日, 对促进患者术后康复具有积极意义。)

目的　探讨多普勒超声辅助跟骨外侧动脉定位方法在跟骨骨折患者扩大L形切口切开复位内固定手术 中的应用效果。方法　选择跟骨骨折患者24例，随机分为超声组和对照组，每组12例。两组均行扩大L形切口切 开复位内固定手术治疗，超声组术前通过多普勒超声确定跟骨外侧动脉体表定位点。记录两组手术时间、术中C臂 透视次数、术中出血量、骨折愈合时间，两组术前及术后 3、12个月行跟骨侧位 X线片检查并测量 Bohler角、Gissane 角；比较两组术后1、3、12个月的疼痛视觉模拟评分 (VAS)，术后3、12个月的美国足踝外科学会 (AOFAS)评分，以及 术后并发症发生率。结果　超声组手术时间短于对照组 (P<0. 05)。两组术中透视次数、术中出血量、骨折愈合时 间比较差异均无统计学意义 (P 均>0. 05)。与同组术前比较，两组术后 3、12个月 Bohler角、Gissane 角均升高 (P 均 <0. 05)；两组术后3、12个月Bohler角、Gissane角比较差异均无统计学意义 (P均>0. 05)。两组术后1、3、12个月VAS 及术后 3、12 个月 AOFAS 评分比较差异均无统计学意义 (P 均>0. 05)。超声组术后发生皮缘坏死 1 例 (8. 3%)；对 照组术后发生皮缘坏死 3 例、感染 1 例，并发症发生率为 33. 3%；超声组术后并发症发生率低于对照组 (P<0. 05)。 结论　跟骨骨折患者扩大L形切口切开复位内固定手术治疗前通过多普勒超声定位标记跟骨外侧动脉有助于缩短 手术时间、减少术后并发症的发生。 [ABSTRACT FROM AUTHOR]

Objective To systematically evaluate the effect of accelerated rehabilitation nursing on reducing postoperative complications in children undergoing day surgery by searching domestic literature.Methods PubMed, CNKI,Wanfang and other databases were searched for literatures related to accelerated rehabilitation nursing in children′s day surgery mode from the establishment of the database to June 2022.The full text and its references were carefully read and judged to determine the included studies.After extracting the data, RevMan5.3 was used for meta-analysis.Results A total of 2 059 cases were included in the six studies, including 979 cases of accelerated rehabilitation nursing and 1 080 cases of routine nursing.Compared with the postoperative complications, there were significant differences in the postoperative recovery time [odds ratio（OR）=-8.62,95%CI-9.50--7.73,P＜0.000 01],the incidence of postoperative nausea and vomiting（OR=0.26,95%CI 0.14-0.48,P＜0.000 01） and fever（OR=0.47,95%CI 0.25-0.91,P=0.02）.Conclusion The application of accelerated rehabilitation nursing in children′s day surgery has significant advantages in early postoperative recovery, reducing the incidence of postoperative nausea and vomiting and postoperative fever. Objective To investigate the application of nurse-led chain management in extended care of children undergoing surgical treatment for Hirschsprung’s disease. Methods Totally 60 children with Hirschsprung’s disease were divided into control group and observation group according to the sequence of hospital admission, with 30 cases in each group. The control group received routine perioperative nursing management, and a nurse-led chain management model was adopted in perioperative nursing management and extended care of children in the observation group. Intervention efficacy was evaluated via incidence of postoperative complications and satisfaction of children’s parents. Results The incidence of postoperative complications in the observation group was lower than that in the control group(P＜0. 05). Parents of children in the observation group were more satisfied with nursing service compared with those in the control group(P＜0. 01). Conclusion The nurse-led chain management in extended care is potentially effective to reduce the risk of postoperative complications and improve the quality of life of children with Hirschsprung’s disease. OBJECTIVE: Metabolic syndrome has been shown to negatively affect postoperative outcomes across a range of surgical procedures. However, it is unclear whether metabolic syndrome is also associated with complications following spinal fusion surgery. METHODS: CNKI, Wanfang, VIP, PubMed, OVID and Web of Science databases were comprehensively searched. Studies on postoperative complications of spinal fusion in patients with metabolic syndrome were selected. Relevant outcome measures included all-cause complications, cardiovascular complications, pulmonary complications, surgical site infection, sepsis, venous thromboembolic events (including deep vein thrombosis and pulmonary embolism), urinary tract infection, 30-day postoperative readmission rate, 30-day postoperative reoperation rate, 30-day postoperative mortality and hospitalization costs. Data were extracted and meta-analyzed or qualitatively synthesized for 11 kinds of outcomes. RESULTS: (1) Eight studies, containing 1 prospective cohort study and 7 retrospective cohort studies, involving a total of 283 593 patients, were included. The overall quality of the included studies was good. (2) Meta-analysis results showed that metabolic syndrome increased the risk of all-cause complications [RR= 1.53, 95%CI(1.22, 1.93), P=0.000 3], pulmonary complications [RR=1.68, 95%CI(1.29, 2.17), P < 0.000 1], cardiac complications [RR=2.05, 95%CI(1.47, 2.87), P < 0.000 1], surgical site infection [RR=1.73, 95%CI(1.37, 2.20), P < 0.000 01], and 30-day readmission [RR=1.44, 95%CI(1.16, 1.79), P=0.001] compared with the non-metabolic syndrome. (3) Qualitative analysis suggested that metabolic syndrome was associated with increased medical costs. CONCLUSION: Metabolic syndrome patients are at increased risk of all-cause complications, pulmonary complications, cardiovascular complications, surgical site infection, and 30-day readmission after spinal fusion surgery. This paper summarized the nursing cooperation in transurethral resection of prostate under comprehensive body temperature control. The clinical data of 80 patients who received transurethral resection of prostate were retrospectively analyzed. The comprehensive body temperature control strategy was carried, including adequate preoperative preparation and targeted nursing interventions during operation. There was no intraoperative hypothermia or massive hemorrhage reported, and patient’s blood pressure was stable during the surgery. Based on perioperative nursing cooperation, adopting comprehensive body temperature control showed a positive effect on reducing the risk of intraoperative hypothermia and postoperative complications. Objective: To discuss the clinical efficacy of minimally invasive circumcision, traditional open surgery, and combination surgery in the treatment of multiple breast tumors complicated with larger lesions, and to provide the clinical the basis for the treatment of the patients with multiple breast tumors. Methods: A total of 167 patients with multiple breast tumors were divided into minimally invasive surgery group (n=83, treated with minimally invasive circumcision), traditional surgery group (n=42, treated with traditional open surgery), and combination surgery group (n=42, treated with minimally invasive circumcision and traditional open surgery). The surgical time, intraoperative bleeding vomule, healing time of the incision, ratios of incision number to tumor number, and postoperative incidences of ecchymosis, infection, hematoma, obvious scar, and residual lesion, and the satisfaction rates of the patients in three groups were observed. Results: Compared with traditional surgery group, the ages of the patients in minimally invasive surgery group and combination surgery group were younger (P< 0. 01). Compared with traditional surgery group and minimally invasive surgery group, the surgical time of the patients in combination surgery group were significantly increased (P<0. 01)；compared with traditional surgery group, the intraoperative bleeding vomule of the patients in combination surgery group was decreased (P<0. 01)；compared with minimally invasive group, and the intraoperative bleeding vomule of the patients in combination surgery group was increased (P<0. 01). 目的 调查接受围手术期标准加速康复管理的颅内前循环动脉瘤患者营养状态、营养支持与显微外科夹闭术后并发症的关系。 方法 前瞻性连续入组2018年1月-2019年3月接受显微外科夹闭手术治疗的颅内前循环动脉瘤患者，围手术期采用标准加速康复管理。收集患者的人口学资料，动脉瘤特点（部位、直径、是否破裂），既往病史（高血压和糖尿病），手术前后营养相关指标（营养风险、人体测量、实验室检查、营养支持方式等），术后并发症包括肺部感染、颅内感染、下肢深静脉血栓、卒中（缺血性卒中和颅内出血）等。根据术后至出院是否出现上述1项及以上并发症，分为并发症组和无并发症组，比较2组患者临床指标的差异，并通过多元logistic回归分析营养相关指标对颅内动脉瘤术后并发症的影响。 结果 共纳入颅内前循环动脉瘤患者97例，其中33例（34.02%）发生术后并发症，64例（65.98%）无并发症。并发症组术前动脉瘤破裂（12.1% vs. 0，P=0.009），高血压（72.7% vs. 50.0%，P=0.005）和糖尿病比例（9.0% vs. 3.1%，P=0.032）高于无并发症组；营养相关指标中术前BMI（26.37±6.58 kg/m2 vs. 24.61±3.32 kg/m2，P=0.015）、三头肌皮褶厚度（24.92±8.57 mm vs. 18.61±8.09 mm，P=0.001）、小腿围（37.60±3.067 cm vs. 36.06±3.44 cm，P=0.039）高于无并发症组，血红蛋白水平（136.06±13.93 g/L vs. 143.27±12.08 g/L，P=0.010）低于无并发症组；术后营养干预的比例（84.8% vs. 40.6%，P=0.030）以及口服营养补充（33.3% vs. 26.5%，P＜0.001）、肠内营养（12.1% vs. 0，P＜0.001）、肠外营养（39.3% vs. 14.0%，P＜0.001）支持的比例均高于无并发症组，肠外营养持续时间长于无并发症组[5.0（3.0～11.5）d vs. 1.0（1.0～3.5）d，P=0.009]。多元logistic分析显示术前高TSF（OR 1.230，95%CI 1.056～1.363，P=0.008）是动脉瘤患者术后并发症的危险因素，术后1 d白蛋白水平高（OR 0.734，95%CI 0.561～0.961，P=0.025）是术后并发症的保护因素。 结论 皮下脂肪增多可能会增加颅内动脉瘤患者显微外科夹闭术后至出院期间并发症的风险，而术后1 d时的白蛋白水平高可能减少术后并发症风险。 Abstract: Objective To investigate the effect of nutritional status on postoperative complications in patients who received microsurgical clipping of intracranial aneurysm and perioperative standard enhanced recovery. Methods This study prospectively enrolled consecutive patients who received microsurgical clipping of intracranial aneurysms and perioperative standard enhanced recovery at Beijing Tiantan Hospital from January 2018 to March 2019. The following data were collected including demographic information, aneurysm characteristics (location, diameter, rupture or not), medical history (hypertension, diabetes), perioperative nutritional parameters (nutritional risk, anthropometric measurements, laboratory test results, nutritional support, etc.), postoperative complications including pneumonia, intracranial infection, deep vein thrombosis of lower extremity and stroke (ischemic stroke and intracranial hemorrhage), etc. The patients were divided into complication group and non-complication group according to having at least one complication or not from the end of operation to discharge. The above indexes were compared between the two groups. Multivariate logistic regression analysis were used to evaluate the effects of perioperative nutritional indexes on the complications after microsurgical clipping of intracranial aneurysms. Results A total of 97 patients with intracranial aneurysm were recruited and underwent microsurgical clipping. Postoperative complications occurred in 33 patients (34.02%). The proportion of preoperative aneurysm rupture (12.1% vs. 0, P=0.009), hypertension (72.7% vs. 50.0%, P=0.005) and diabetes (9.0% vs. 3.1%, P=0.032) in complication group were higher than those in non-complication group, and nutritional parameters such as the preoperative BMI (26.37±6.58 kg/m2 vs. 24.61±3.32 kg/m2, P=0.015), triceps skin fold thickness (TSF) (24.92±8.57 mm vs. 18.61±8.09 mm, P=0.001), and calf circumference (37.60±3.067 cm vs. 36.06±3.44 cm, P=0.039) in complication group were higher than those in non-complication group, and the hemoglobin level (136.06±13.93 g/L vs. 143.27±12.08 g/L, P=0.010) was lower than that in non-complication group. BACKGROUND: Postoperative symptomatic intervertebral disc pseudocyst is a cystic lesion in the area of intervertebral disc surgery. It is usually found in patients undergoing MRI examination in the early postoperative period. At present, there is insufficient data on its clinical characteristics and pathogenesis, and its treatment method is still controversial. OBJECTIVE: To investigate the clinical characteristics, pathogenesis and treatment strategy of symptomatic intervertebral disc pseudocyst after discectomy. METHODS: Seven patients with symptomatic intervertebral disc pseudocysts after intervertebral discectomy in the Yongchuan Hospital Affiliated to Chongqing Medical University from January 2019 to September 2021 were enrolled in this study. There were 5 males and 2 females, at the age of 35-60 years. Clinical features and treatments were retrospectively analyzed. RESULTS AND CONCLUSION: (1) Among seven patients, five patients underwent percutaneous endoscopic lumbar discectomy and two underwent posterior microscopic endoscopic discectomy. Their symptoms were significantly relieved after surgery. However, the symptoms recurred after the operation. The postoperative MRI showed low signal on T1WI and high signal on T2WI in the original operation area, and it communicated with the intervertebral disc, which was diagnosed as symptomatic intervertebral disc pseudocyst. The average time for MRI to find the pseudocyst was about 32 days. (2) For seven patients with symptomatic intervertebral disc pseudocysts, four patients received analgesia and detumescence, bed rest and other symptomatic treatments; one patient received percutaneous endoscopic lumbar discectomy; two patients received microscopic endoscopic discectomy. Waist and leg pains were significantly relieved. At 1 month after operation, MRI showed the original pseudocyst was significantly reduced. (3) The results indicate that symptomatic intervertebral disc pseudocyst usually occurs in the early postoperative period. MRI can be the first choice for examination. Objective To explore the feasibility of applying indocyanine green angiography (ICGA) before and during the operation to evaluate the blood supply of the flap and adjust the surgical plan in flap repair surgery to increase the survival rate of flap transplantation and reduce complications after flap surgery. Methods From March 2019 to March 2021, the clinical data of 78 patients admitted to the Department of Burn Plastic and Cosmetic Surgery of Xi'an Central Hospital Affiliated to Xi'an Jiaotong University were randomly selected and divided into control group of 40 cases and experimental group of 38 cases according to the order of admission. The control group underwent traditional flap transplantation, and the experimental group underwent ICGA before and during the operation to evaluate the blood supply of the flap and adjust the surgical plan based on the results of the angiography. Results All 38 patients in the experimental group had no angiography-related complications after ICGA. Among them, 3 cases adjusted the operation plan according to the ICGA results and ended the operation smoothly, the remaining 35 cases ended the operation smoothly according to the preoperative design. In the control group, all flap operations were performed by traditional surgical methods. Among them, 8 cases of flap tips had blood flow disorders during the operation, and the operations were terminated after corresponding treatments, the remaining 32 cases were successfully completed according to the preoperative design. All patients were followed up for 6 months. There was no statistically significant difference between the observation group and the control group in the incidence of postoperative flap blood supply abnormalities (X² ＝0.212，P＝0.645) and flap related complications (X² ＝0.749，P＝0.387). Objective To evaluate the detailed nursing management in nosocomial infection control in the operation room. Methods Totally 70 patients undergoing surgical treatment were divided into the observation group (n=35) and control group (n=35) according to the admission sequence number. Patients in both control and observation received routing nursing management during the hospitalization period, and those in the observation group were given detailed nursing management. The rates of nosocomial infection and postoperative complications among patients were observed. The patient statifacion was evaluated by using a self-made questionnaire. The outcomes of theoretical knowledge and practice skills assessment among nurses were analyzed. Results There were three cases of nosocomial infection reported in the control group. The incidence of postoperative complications in the observation group was lower than that in the control group(P＜0. 05). The score of patient satisfaction assessment in the observation group was higher than that in the control group (P＜0. 01). The scores of theoretical knowledge and practice skills assessment among nurses in the observation group were higher than those in the control group(P＜0. 01). Conclusion The detailed nursing management is an effective tool for reducing risk of nosocomial infection and postoperative complications. The detailed nursing management represents a good opportunity for nurses to grow their knowledge and nursing skills, therefore improving the quality of specialized care. BACKGROUND: More and more emerging repair materials are used for the repair of skull defects, and it is not clear which repair material is the best choice. OBJECTIVE: To preliminarily compare the application of three-dimensional digital shaping polyether ether ketone and titanium mesh in skull repair. METHODS: The clinical data of 174 patients who underwent skull repair in the Department of Neurosurgery of First Affiliated Hospital of Zhengzhou University from January 2017 to September 2020 were analyzed retrospectively. There were 126 males and 48 females with an average age of (43.9±12.0) years. Among them, 95 cases were treated with polyether ether ketone repair materials and 79 cases with titanium mesh. The intraoperative data, postoperative complications and follow-up results (including seizures, hematoma which requiring surgical treatment, infection, subcutaneous effusion, cerebrospinal fluid leakage, loosening or depression of repair materials, exposure of implants, and subjective feelings) were obtained. The differences between the two groups were compared. The protocol was in accordance with the requirements of the Ethics Committee of First Affiliated Hospital of Zhengzhou University. RESULTS AND CONCLUSION: (1) The postoperative follow-up time was from 2 to 37 months, and the average follow-up time was (17.2±12.4) months in 174 patients. (2) The total incidence of postoperative complications in the polyether ether ketone group and the titanium mesh group was 20% and 26%, respectively, and there was no significant difference between the two groups (P > 0.05). (3) There was no significant difference in epilepsy, hematoma, surgical site infection, subcutaneous effusion, cerebrospinal fluid leakage, loosening or depression of repair materials between the two groups (P > 0.05). There was no significant difference in proportion of bilateral symmetry of head between the two groups (P > 0.05). Occurrence ratios of exposure of repair materials, vibration sensation, hot and cold experience were lower in the polyether ether ketone group than those in the titanium mesh group (P < 0.05). (4) Both the polyether ether ketone and the titanium mesh have good bioaffinity. The exposure of repair materials and the experience of vibration and hot and cold sensation in the polyether ether ketone group are better than those in the titanium mesh group. The implementation of living donor liver transplantation not only extends the number of donors and effectively alleviates the shortage of organ donation, but also reduces the fatality of patients on the waiting list for liver transplantation. In recent years, with persistent development of laparoscopic techniques and instruments, laparoscopic donor liver hepatectomy has been widely applied in the field of living donor liver transplantation, which is also one of the research hotspots. It has multiple advantages of mild trauma, slight pain, accelerated postoperative recovery for the donors and slight psychological burden, etc. However, due to strict screening criteria for donors, complicated procedures and long learning curve, it has been carried out in a few transplantation centers nationwide. In this article, the development history and current status, specific surgical patterns, the problems and challenges of laparoscopic living donor liver procurement for liver transplantation were introduced, and the development direction of laparoscopic living donor liver procurement was predicted, aiming to provide reference for accelerating the development and enhancing clinical prognosis of living donor liver transplantation. Objective To investigate the practice effect of continuing care with Traditional Chinese Medicine nursing for patients after surgical treatment of mixed hemorrhoids in the specialized outpatient clinic. Methods Totally 80 patients undergoing surgical treatment of mixed hemorrhoids of Damp-Heat type were randomly divided into the observation group and the control group, with 40 cases in each group. All patients received routine perioperative nursing during hospitalization. Patients in the control group received routing telephone follow-up and health education after discharged form hospital, and those in the observation group were given continuing care with Traditional Chinese Medicine nursing in the specialized outpatient clinic, and a three-step integrated management of follow-up was carried out. The occurrence of postoperative complications was assessed, and patient satisfaction was evaluated by using a self-made questionnaire. Results The score of postoperative complication evaluation in the observation group was lower than that in the control group (P＜0. 01). The patient satisfaction rate was 97. 50%(39/40), which was higher than 82. 50%(33/40) in the control group (P＜0. 05). Conclusion The practice of continuing care with Traditional Chinese Medicine nursing in the specialized outpatient clinic is potentially effective to reduce the risk of postoperative complications after surgical treatment of mixed hemorrhoids and improve the patient satisfaction. Objective To explore the effect of pain normative model in postoperative pain control intervention of neurosurgery.Methods A total of 116patients who underwent neurosurgery in the hospital from January 2018to September 2020were selected and divided into two groups,according to the random number table method.The control group was treated with routine nursing and the study group was treated with pain normative model.The mastery of pain related knowledge,postoperative pain score（4h,12hand 24hafter surgery）,sleep quality level,vital signs and nursing satisfaction were compared between the two groups.Results The scores of drug analgesia,pain evaluation and basic knowledge of pain in the study group[（22.16±3.54）,（21.84±2.93）,（19.12±3.13）scores]were higher than those in the control group[（18.92±3.24）,（17.26±3.67）,（15.91±2.44）scores],the differences were statistically significant（P<0.05）.The pain scores of the study group at 4h,12hand 24hafter surgery[（2.36±1.66）,（1.83±0.23）,（1.35±0.21）scores]were lower than those of the control group[（3.82±1.88）,（2.96±1.53）,（2.48±0.38）scores],the differences were statistically significant（P<0.05）.The scores of daytime dysfunction,hypnotic drugs,sleep time,sleep quality,sleep efficiency,sleep disorder and asleep time in the study group[（1.2±0.3）,（1.4±0.1）,（1.3±0.1）,（1.4±0.2）,（1.5±0.2）,（1.1±0.1）,（1.1±0.1）scores]were lower than those in the control group[（1.7±0.4）,（1.7±0.1）,（1.7±0.1）,（1.8±0.3）,（1.7±0.3）,（1.7±0.2）,（1.7±0.2）scores],the differences were statistically significant（P<0.05）.Mean arterial pressure（MAP）[（83.75±12.23）mm Hg（1mm Hg=0.133kPa）]in the study group was higher than that in the control group [（73.21±11.51）mm Hg],respiratory rate（RR）[（86.41±3.36）times/min]and heart rate（HR）[（20.61±0.31）times/min]were lower than those in the control group[（98.22±4.33）,（25.51±0.62）times/min],the differences were statistically significant（P<0.05）.The total satisfaction of the study group（96.6%,20/58）was higher than that of the control group（82.8%,24/58）,the differences were statistically significant（P<0.05）.Conclusion Pain normative model in postoperative pain control of neurosurgery has ideal nursing effect and can improve the effectiveness.It should be further popularized and applied in clinic. This paper summarized the nursing management in the surgical treatment of transposition of the left common carotid artery to left subclavian artery in 85 patients with Stanford type B aortic dissection. Key Points of nursing included preoperative evaluation of non-complex and complex Stanford type B aortic dissection, pain control and target blood pressure management and prevention of rupture and reverse tearing; enhancing the blood pressure monitoring; observation of surgical incisions and prevention of respiratory arrest; enhancing the postoperative anticoagulation to prevent the thrombosis or coagulation disorders; observation on vital signs and chief complaint and prevention of postoperative complication. Objective To explore the effect of perioperative nursing interventions based on fast-track surgery (FTS) for patients undergoing neuroendoscope-assisted microvascular decompression (EMVD) for trigeminal neuralgia. Methods Totally 100 patients undergoing EMVD for trigeminal neuralgia were enrolled and randomly divided into control group and observation group, with 50 cases in each group. The control group received routine care during the operative period, and the observation group received nursing interventions based on FTS theory. The anxiety status was evaluated by Self-rating Anxiety Scale (SAS), and incidence of postoperative complications such as nausea and vomiting was compared between two groups. Results The score of SAS in the observation group was lower than that in the control group before and after surgery (P＜0. 05). The incidences of nausea and vomiting, urinary tract infection in the observation group was lower than those in the control group (P＜0. 05). Conclusion Nursing interventions based on FTS concept can effectively relieve the anxiety status of patients undergoing EMVD for trigeminal neuralgia, and reduce the risk of postoperative complications such as nausea and vomiting. Objective To compare and analyze the perioperative clinical effects of minimally invasive Ivor-Lewis esophagectomy ( MIE-Ivor-Lewis ) and minimally invasive McKeown esophagectomy ( M1E-McKeown). Methods A total of 147 patients who underwent endoscopic esophageal cancer surgery from April 2018 to August 2019 were selected, including 85 patients undergoing MIE-McKeown surgery and 62 patients undergoing MIE-Ivor-Lewis surgery. The measurement data were expressed as (z ±s), the comparison of normally distributed measurement data was performed by independent sample t-test, and the comparison of count data was performed by X² test or Fisher's exact test. Results The operation time of McKeown ( M) group and Ivor-Lewis (IL) group were (219.2±72.4) minutes and (225 8±65.3) minutes. The mediastinal lymph node dissection number of M and IL groups were 13.3±4.8 and 11.6±6.5, respectively. The number of left recurrent laryngeal nerve lymph node dissection were 3.5±1.2 and 3.1±1.4, respectively. The intraoperative blood loss were ( 178.3t41.3) ml and ( 163.2t64.1) ml, respectively. The number of patients reoperated for postoperative bleeding were I and 0, respectively. The number of patients with postoperative gastric bleeding were 0 and 1, respectively. The postoperative chest tube retention time were (2.8±1.3) days and (3.1±1.2) days, respectively. The number of patients with anastomotic leakage were 7 and 1, respectively. The number of patients with lung infection were 13 and 5, respectively, and with chylothorax were 2 and 1, respectively, without statistically significant difference (P> 0.05 ). The number of patients with hoarseness were 11 and 3, respectively. The total incidence of complication were 41.2% (35/85) and 17.7% (11/62), and the postoperative hospital stay were ( I4.7±6.5) days and (12.3±2.3) days, with statistical difference ( P<0.05). Conclusion MIE-Ivor-Lewis and MIE-McK n are safe and effective in treating esophageal cancer, but the complication of MIE-Ivor-Lewis is less than that of MIE-Mckeown, and the perioperative clinical effect of MIE-Ivor-Lewis is better than that of MIE-McKeown. Since the 21st century, minimally invasive technique has become a main development direction of surgery, which has been widely applied in all branches of surgery. In the field of kidney transplantation, minimally invasive technique has been mainly applied in the procurement of living donor kidney, kidney transplantation and the management of complications after kidney transplantation. It not only increases the resource of donor kidney, but also reduces the incidence of postoperative complications and enhances the quality of life of the recipients. The application of minimally invasive technique has become one of the research hot spots in the field of kidney transplantation. In this article, research progresses on the application of minimally invasive technique in the procurement of living donor kidney, kidney transplantation and management of complications after kidney transplantation were reviewed, aiming to provide reference for increasing the resource of donor kidney, enhancing the success rate of kidney transplantation and improving clinical prognosis of kidney transplant recipients, thereby promoting the development of minimally invasive technique in surgery. This paper reviewed the nursing management and complication observation after interventional therapy in five patients with acute superior mesenteric artery embolism. Postoperative complications included bowel necrosis, intra-abdominal hemorrhage and hemorrhage at wound site. A comprehensive nursing management, including illness observation, complication identification and predictive nursing, is required to reduce the risk of death and improve the rehabilitation. Objective To evaluate the effects of postoperative pulmonary rehabilitation (PR) training on postoperative complications, exercise tolerance 'fatigue 'negative emotions and quality of life in the patients with lung cancer. Methods The Cochrane Library Joanna Briggs Institute(JBI), PubMed, Embase, China Bi-ology Medicine Literature Service System (CBM), China National Knowledge Infrastructure (CNKI), Wanfang database and VIP Chinese periodical service platform(VIP) were used to search for the randomized controlled trials (RCT) on the effects of postoperative PR on postoperative complications, exercise tolerance, fatigue, negative emotions and quality of life. The retrieval time was from the database establishment to March 2021. The quality of the included literatures was evaluated and data was extracted by two researchers independently. RevMan 5. 4 software was used for data analysis. Descriptive analysis was used to analyze the outcome indicators that could not be applied to meta-analysis. Results A total of 750 subjects were included in the selected 12 articles, including 367 subjects in the intervention group and 383 subjects in the control group. The results showed that the postoperative PR could reduce the incidence of postoperative complications, improve exercise tolerance, reduce fatigue and improve quality of life in patients with lung cancer.
