OBJECTIVE: To analyze the medication characteristics and rules of traditional Chinese medicine patent compound in the treatment of nephrotic syndrome by using data mining technology, so as to provide reference for clinical treatment and new drug development. METHODS: Traditional Chinese medicine patent compound in the treatment of nephrotic syndrome in Announcement of Chinese Patent Publication of China National Intellectual Property Administration (http:/ / epub. cnipa. gov. cn) from Sept. 10th, 1985 to Nov. 1st, 2023 were retrieved. The drug frequency, theory of four natures, five flavors and meridian tropism, association rule analysis and systematic cluster analysis of traditional Chinese medicine patent compound that met the inclusion criteria were conducted by using the Ancient And Modern Medical Record Cloud Platform V2. 3. 7 and SPSS Statistics 26. 0 software. RESULTS: A total of 88 kinds of traditional Chinese medicine patent compound were enrolled, including 249 traditional Chinese medicines. In terms of drug frequency, in descending order, Hedysarum multijugum maxim, Atractylodes macrocephala koidz, Alisma orientale (Sam.) juz, Poria cocos (Schw.) wolf, and Rhizoma dioscoreae. In terms of four natures, five flavors and meridian tropism, the four natures were flat, the five flavors were sweet, and the meridian tropism was liver. In terms of association rules, the confidence levels from high to low were Atractylodes macrocephala koidz-Hedysarum multijugum maxim, Poria cocos (Schw.) Wolf-Hedysarum multijugum maxim, Alisma orientale (Sam.) juz-Hedysarum multijugum maxim. In terms of systematic clustering, the obtained drug combinations included Rehmanniae Radix Praeparata, Cornus Officinalis Sieb. Et Zucc, Rhizoma dioscoreae, Euryales semen, Leonuri herba, Imperatae rhizoma and Hedysarum multijugum maxim, Codonopsis Radix, and licorice. CONCLUSIONS: The treatment of nephrotic syndrome by traditional Chinese medicine patent compound focuses on spleen and kidney co-regulation, Qi, blood and water treatment, and takes strengthening the spleen and tonifying the kidney, inducing diuresis and eliminating dampness, activating blood circulation and removing blood stasis as the basic treatment, which can develop ideas and provide references for the clinical treatment of the disease and the research and development of new drugs. - Abstract
BACKGROUND: In clinical application, simple interspinous fixation without additional interbody fusion has similar fixation effects to pedicle screw and rod fusion internal fixation, and can effectively reduce the range of motion of the responsible segment and the stress of the articular process. However, after simple placement of the new interspinous fusion fixation device BacFuse, the stress at the root of the spinous process is relatively concentrated, and the spinous fracture is prone to occur. If an intervertebral fusion cage is inserted in conjunction with interspinous fixation, Von Mises stress can theoretically be dispersed to reduce the risk of spinous fracture. However, there are few studies on biomechanics and finite element analysis. OBJECTIVE: To observe the biomechanical stability of interspinous fixation-assisted endoscopic interbody fusion in the treatment of severe lumbar spinal stenosis. METHODS: The normal finite element model M0 of the L4-L5 segment of the lumbar spine was established by Mimics, Geomagic, Solidworks, and ANSYS software based on the lumbar CT images of a 26-year-old adult male volunteer excluding spinal diseases. On the basis of M0, the immediate model M1 after endoscopic decompression combined with interbody fusion, the interspinous fixation device (BacFuse) model M2 after endoscopic decompression, and the interspinous fixation (BacFuse) model M3 after endoscopic-assisted interbody fusion were established. The same stress was applied to the upper surface of the L4 vertebral body in the four groups, and the lower surface of the L5 vertebral body was fixed and supported. The range of motion and the extreme Von Mises stress of the endplate bone and the posterior ligament complex of the vertebral body were analyzed under six working conditions of flexion, extension, left/ right bending, and left/right rotation. RESULTS AND CONCLUSION: (1) Compared with model M0, the range of motion value of model M1 increased significantly under six working conditions. Model M2 and model M3 had a significant reduction in range of motion. (2) Compared with model M0, the maximum stress of the vertebral body in model M1 did not change significantly under the six working conditions. The maximum stress at the rear of the M2 vertebral body increased significantly. (3) Compared with model M1, the maximum stress of model M3 did not change significantly under the six working conditions. Compared with model M2, the maximum stress of model M3 decreased significantly. (4) Compared with the model M0, the extreme Von Mises stress of the L4 and L5 endplates of the model M1 was significantly increased. The extreme Von Mises stress in L4 and L5 endplates of models M2 and M3 decreased slightly. Compared with model M1, the Von Mises stress of the bone under the L4 and L5 endplate of models M2 and M3 was significantly reduced. (5) It is concluded that the implantation of BacFuse can effectively reduce the bone stress under the endplate during simple interbody fusion, decrease the risk of cage subsidence, diminish the risk of facet joint fracture on the decompression side, and provide a good stable environment for interbody fusion. The placement of an intervertebral fusion cage can reduce the stress of the root of the spinous process, which is beneficial to decrease the risk of fracture of the root of the spinous process. - Abstract
Objective: To compare the application effect of the posterior tibial artery perforator flap and the pedicled skin flap anastomosing subcutaneous veins on skin and soft tissue defects in the limbs. Methods: 68 patients with limb skin and soft tissue defects admitted to our hospital from March 2017 to March 2022 were selected as the study subjects, with 34 cases in each group. All patients underwent flap repair surgery, while the matched group received treatment with the posterior tibial artery perforator flap, while the observation group received treatment with a pedicle skin flap anastomosing the subcutaneous vein. Compare the clinical efficacy, intraoperative bleeding volume, surgical time, hospital stay, wound healing time, postoperative pain level, and sensory function recovery between the two groups of patients. Finally, conduct a one-year follow-up of the two groups of patients to compare their incidence of complications and secondary repair within one year. Results: The total response rate was higher in the observation group than in the matched group (P<0.05); There was no difference in intraoperative bleeding volume, surgical time, and postoperative VAS score between the two groups of patients. The observation group had lower hospital stay and wound healing time than the matched group(P<0.05); There was no difference in the number of patients with postoperative sensory function recovery at S3, S2, S1, and SO levels between the two groups (P>0.05). The number of patients with postoperative sensory function recovery at S4 level in the observation group was higher than that in the matched group (P<0.05); The rate of postoperative complications and the rate of postoperative secondary repair in the observation group were significantly lower than that in the matched group (P<0.05). Conclusion: Compared with the repair of skin and soft tissue defects in the limbs using the posterior tibial artery perforator flap, the repair of pedicle skin flap with subcutaneous vein anastomosis can further improve the treatment effect, shorten the hospitalization time of patients, promote wound healing, improve postoperative sensory function recovery level, reduce the incidence of long-term complications and secondary repair rate, and is worthy of clinical application and promotion [ABSTRACT FROM AUTHOR]

Slurry concentration has characteristics of hysteresis and time varying in conventional PID control, which is easily affected by the properties of tailings and particle size. Therefore, a multi objective optimization control strategy for slurry concentration is proposed. By analyzing the filling process flow, a multi objective control model for slurry concentration is established according to setting multi objective decision variables and establishing corresponding objective functions and constraints. The slurry concentration is optimized by underflow concentration, filling quality and filling cost, the model was solved by genetic algorithm and simulated in MATLAB. Through the simulation curve, the multi objective optimization algorithm meets the control requirements of qualified quality and low cost in terms of evaluation indicators* Compared with the traditional PID control algorithm, the multi objective optimization control algorithm has faster response speed and stronger robustness* Since the operation of the filling system, the slurry concentration has stabilized at 70%士1%, and the filling cost has decreased by 15%, which meets the requirements of industrial production. BACKGROUND: Seed cells are seeded on three-dimensional scaffold materials, and three-dimensional culture in bioreactors is a common in vitro tissue engineering culture method, but the changes in cell proliferation and metabolic patterns in bioengineered blood vessel construction are still unclear. OBJECTIVE: To explore the metabolic changes of cells such as oxygen consumption and their causes during the whole process of biological vascular tissue construction by in vitro bioreactor. METHODS: The self-built vascular bioreactor system was used as the platform; bovine vascular smooth muscle cells were used as the seed cells, and a conventional CO2 incubator provided the external gas environment for the cultivation process. Seed cells were seeded on a tubular porous polyglycolic acid scaffold material for three-dimensional culture, and the whole process included a one-week resting period and a seven-week pulsating tensile stress stimulation loading period. A non-invasive monitoring system was built, and the optical dissolved oxygen patch method was used to monitor the changes of dissolved oxygen in the culture solution in the reactor, and the glucose consumption and lactic acid production were measured by regular sampling. CCK-8 assay was used to determine the proliferation of smooth muscle cells on polyglycolic acid three-dimensional scaffold materials. Nicotinamide adenine dinucleotide oxidation state and reduction state ratio (NAD+ /NADH) was utilized to understand cell proliferation and metabolism in the early stage of culture. RT-qPCR and western blot assay were applied to detect the expression of proliferation-related genes (Ki67) and glycolysis-related genes (GLUT-1, LDHA). RESULTS AND CONCLUSION: (1) The dissolved oxygen level in the culture solution was (4.314±0.380) mg/L from the cell injection to the end of the resting period (the first week), and gradually stabilized at (1.960±0.866) mg/L after the tensile stress stimulation (the last seven weeks); the two had significant changes (P < 0.05). (2) The ratio of glucose consumption to lactic acid production in the cell culture medium YL/G increased rapidly after the cells were injected, and the highest value was above 1 on the fifth day, and then slow down to 0.5 (The mean value of YL/G in the resting period was 0.89 and the mean value in the pressurized period was 0.57, P < 0.05). (3) CCK-8 assay results showed that A450 value gradually increased after the cells were injected, and reached the highest value on the fifth day, reaching 3.17, and then slowly decreased. At the same time, it was found that Ki67 mRNA was up-regulated on the third day of culture, and then declined. The expression level of Ki67 protein was higher from the third day to the fifth day. (4) The detection of NAD+ / NADH showed that the increase was obvious from the fifth to the seventh day after the injection of cells, and the expression of glycolysis-related genes (GLUT-1 and LDHA) was up-regulated and changed synchronously, and the relative expression was higher in the first five days. (5) The results showed that the tissue-engineered blood vessels were constructed using the vascular bioreactor and the smooth muscle cells in the early stage mainly proliferated and exhibited a metabolic feature of low oxygen consumption. - Abstract
Objective To investigate the correlation between urinary triclosan (TCS) levels in early pregnancy and glucose homeostasis at 28 weeks of gestation. Methods A total of 241 pregnant women undergoing prenatal examinations at the Second Central Hospital of Baoding from January 2019 to June 2020 were recruited. The primary outcome was gestational diabetes mellitus (GDM) screened by a 1 hour non-fasting, 50 g glucose load test (GLT) at a median of 28 weeks of pregnancy. Urine samples were collected in early and mid pregnancy to measure urine specific gravity (Sg) and quantify phenolic compounds (bisphenol A, benzophenone-3, and TCS) using online solid phase extraction combined with isotope dilution high performance liquid chromatography tandem mass spectrometry. Results According to GLT results, 199 women had normal GLT, while the remaining 42 had abnormal results. Women with abnormal GLT had higher BMI and lower total exercise time compared to those with normal GLT (P<0.05). Positive associations between TCS levels and gestational glucose levels were observed in early pregnancy and mid pregnancy (comparing the 2nd, 3rd, and 4th quartiles with the 1st quartile of TCS: adj. OR = 0.41, 95% CI: 0.191.05; adj. OR 0.43, 95% CI: -0.171.02; adj. OR = 0.45, 95% CI: 0.141.05) and mid pregnancy (adj. OR0.76, 95% CI: 0.191.32; adj. OR0.66, 95% CI: 0.941.22; adj. OR0.56, 95% CI: 0.041.12). Bayesian kernel machine regression (BKMR) results indicated a positive association between TCS and glucose levels in both early and mid pregnancy (P<0.05). Conclusion Urinary TCS concentration during pregnancy is positively correlated with glucose levels in late pregnancy GDM screening tests. [ABSTRACT FROM AUTHOR]

- Abstract
Radiation-induced liver disease (RILD), also known as radiation hepatitis, is subacute liver injury induced by radiation. As the focus of senescence-related studies, the deacetylase family Sirtuins (SIRTs) have the molecular functions including DNA repair and chromatin regulation, which makes SIRTs a hub for regulating genome and epigenome stability. Radiation-induced hepatic DNA damage and reaction is the primary physiological and pathological process of RILD, which is similar to the function of SIRTs. This article briefly introduces the structure and function of the SIRTs protein family, elaborates on the basic concepts and progress of the physical physiology of radiation therapy, discusses the internal relationship between SIRTs and RILD from the perspective of radiobiology, and points out the possibility of SIRTs as a target for the prevention and treatment of RILD.
