The artificial island project of the Hongkong-Zhuhai-Macao Bridge is situated in a location prone to year-round exposure to extreme weather events such as tropical cyclones and typhoons, often accompanied by prolonged heavy rainfall when typhoons pass through. To mitigate operational risks of the tunnel under such extreme and adverse conditions, a local physical model test was conducted to verify the water interception capacity of the transverse ditch grate on the open section of the artificial island. The influence of grid angle, opening ratio, road slope, scale effects, and the number of transverse ditch on the water interception capacity of the grate were studied. The results indicate that the water interception capacity of the grate in the design with a 0° grid angle surpasses that of the 90° grid angle. Simultaneously, the arrangement of two transverse ditches demonstrates a significant water interception effect. The findings of this research are of great significance for the accuracy of the design and layout of the transverse ditch grates on the artificial island, the elimination of drainage hazards, risk prevention, and ensuring operational safety. In order to ensure the safety and reliability of a sea -crossing bridge project under more severe extreme weather conditions, and coordinate the design of the artificial island flood control and drainage facilities for the main project, two wave physical model tests were carried out to verify and optimize the design scheme of drainage facilities on artificial island. According to the technical requirements of the test, under the condition of different wave, different top elevations of retaining wall and different width of peak platform, the test is accepted to determine the wave overtopping rate at the top of the north and south revetment in artificial island, the maximum height of upstream, the flood plain situation in artificial island, and check the drainage capacity of the overtopping drainage facilities. Under the initial design conditions and more extreme supplementary review wave conditions, by performing the experiment, the original design scheme of the north and south revetment was verified whether to satisfy the requirement of overtopping flood discharge requirement, and the design scheme was optimized to meet the drainage requirements under more extreme conditions, which provides a scientific basis for the engineering design. The research findings from this study have been successfully applied to the practical implementation of this project. Objective To explore the efficacy of neuroendoscopic transseptal submucosal approach in the surgical treatment of pituitary adenomas. Methods The clinical data of 50 patients with pituitary adenoma were analyzed retrospectively. According to the operation method, they were divided into mucosal approach group（n = 26） and classical approach group（n = 24）. The perioperative observation indexes（operation time, intraoperative bleeding, intraoperative cerebrospinal fluid leakage, postoperative hospital stay）, tumor resection rate,hormone biochemical remission rate and postoperative complication rate were compared and analyzed. Results There was no significant difference in operation time/intraoperative cerebrospinal fluid leakage/operation bleeding between mucosal approach group and classical approach group, P values（0. 089 7/0. 385/1. 000） were greater than 0. 05. In the effect of tumor resection, the total resection rate of the two groups was 88%,and there was no significant difference between the two groups（χ~2= 0. 110,P =0. 917）. The hormone biochemical remission rate was 83% in the mucosal approach group and 63%in the classical approach group. There was no significant difference between the two groups（χ~2=1. 847,P = 0. 174 > 0. 05）. In terms of postoperative complications, there was no significant difference in postoperative transient diabetes insipidus, intracranial infection and decline of pituitary function between the two groups（P > 0. 05,0. 174/1. 000/0. 480/0. 623,respectively）. The P values of postoperative hospital stay and nasal complications were 0. 001/0. 022 and less than0. 05, respectively. Conclusions Compared with the classic neuroendoscopic bilateral transsphenoidal approach for pituitary adenoma, the neuroendoscopic bilateral transseptal submucosal approach can better protect the nasal anatomy and function in pituitary tumor resection, and reduce postoperative hospital stay and nasal complication. BACKGROUND: Adipose-derived stem cells have become one of the popular seed cells in regenerative medicine due to abundant content and easy harvest. Adipose-derived stem cell exosomes are served as a bridge and link among adipose-derived stem cells and other cells and have the effects on regulating immune and inflammatory responses, promoting self-repair of damaged cells, and are considered as an alternative treatment for adipose-derived stem cells. OBJECTIVE: To review research progress and challenges in the adipose-derived stem cell exosomes regulating tissue regeneration. METHODS: CNKI, Wanfang database and PubMed, and Web of Science database were searched for related articles with the search terms of “adipose-derived stem cells; adipose stem cells; exosomes” in English and Chinese. After preliminary screening of articles based on inclusion and exclusion criteria, 70 articles with high relevance and reference value were retained for review. RESULTS AND CONCLUSION: Adipose-derived stem cell exosomes have shown the therapeutic potential in various diseases, such as skin wounds, neurodegenerative diseases, tumors, bone regeneration, kidney diseases, cardiovascular diseases, immune diseases, and fibrotic disease. Objective To explore the related risk factors affecting the prognosis of epidermoid cyst in cerebellopontine angle area (CPA), so as to guide the improvement of the treatment and prognosis of epidermoid cyst in CPA. Methods The clinical data of 23 patients with cerebellar angle epidermoid cyst were analyzed retrospectively. According to the postoperative recovery and follow-up after discharge, the patients were divided into good prognosis group(15 cases) and poor prognosis group (8 cases). The basic data, surgical methods and prognosis of the two groups were compared, and the related factors were analyzed by univariate and multivariate analysis. Results There were significant differences between the two groups in age, recurrence, maximum tumor diameter, whether the tumor crossed the midline, operation method and intraoperative bleeding(P < 0. 05-0. 01). The maximum tumor diameter and operation mode were independent risk factors affecting the prognosis (all P < 0. 05). There was no significant difference in gender, course of disease, whether there was cranial nerve damage before operation, whether combined with other chronic diseases, degree of resection and whether drainage was placed (all P > 0. 05). There were significant differences in intraoperative bleeding, postoperative cranial nerve damage and postoperative hospital stay between patients with simple microscope and patients with neuroendoscope assisted microscope (P < 0. 05-0. 001), but there were no statistical differences in the degree of tumor resection, postoperative fever and preoperative symptom relief (all P > 0. 05). Conclusions In addition to reducing the influence of risk factors such as age, course of disease and tumor volume, the choice of surgical method is also a key factor to improve the prognosis of epidermoid cyst in CPA. The application of endoscope assisted microsurgery can reduce intraoperative injury and postoperative complications, improve the degree of lesion resection, and shorten the hospital stay.
