AMAÇ: İnsan sağlığı için önemli bir yere sahip olan et ve süt ürünlerinin yapısındaki maddeler nedeniyle pek çok mikroorganizmanın gelişmesi için çok iyi bir ortam oluşturmaktadır. Bu durumdan yola çıkarak, Afyonkarahisar ilinde satışa sunulan süt, yoğurt, peynir, sucuk, pastırma ve etin mikrobiyolojik özelliklerinin ve standartlara uygunluğunun araştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Her bir gıda çeşidine ait örneklerden ayrı ayrı 30 süt ürünü ve 30 et ürünü olmak üzere 60 adet ürün soğuk zincir kuralına uygun olarak taşınmış ve ürünler mikrobiyolojik açıdan incelenmiştir. Bu çalışmaya, incelenmek üzere, çiğ süt, yoğurt, taze peynir, çiğ et, sucuk ve pastırma örnekleri dahil edilmiştir. Toplam aerobik mezofilik bakteri, maya-küf, S. aureus, E. coli, Salmonella ve Koliform grubu mikroorganizmaların varlığı incelenecek olan gıda örneklerinin her birinden 10’ar g/ml alınarak, 10-1, 10-2, 10-3, 10-⁴, 10-⁵ olmak üzere bir seri dilüsyonları hazırlanmıştır. Uygun besiyerleri ve optimum inkübasyon şartlarında, aerobik mezofilik bakteri, maya-küf, S. aureus, E. coli, Salmonella ve Koliform grubu mikroorganizmaların sayısı belirlenmiştir. BULGULAR: Sütteki en yüksek aerobik mezofilik bakteri ve koliform bakteri sayısı 5,5x104 kob/g ve 1,5x103 kob/g; Yoğurttaki en yüksek aerobik mezofilik bakteri, koliform bakteri ve küf-maya sayısı 2,7x103 kob/g, 3,0x10⁶ kob/g ve 2,2x103 adet/g; Peynirdeki en yüksek aerobik mezofilik bakteri, koliform bakteri ve küf-maya sayısı 2,9x103 kob/g, 3,7x103 kob/g ve 7,8x101 adet/g olarak belirlenmiştir. Sucuktaki en yüksek küf-maya ve stafilokok sayısı 2,8x102 adet/g ve 6,0x102 kob/g; Pastırmadaki en yüksek küf-maya ve stafilokok sayısı 3,9x103 adet/g ve 9,8x10⁴ kob/g; Çiğ etteki en yüksek mezofilik aerobik bakteri ve stafikokok bakteri 7,5x102 kob/g ve 3,8x105 kob/g olarak belirlenmiştir. Bu bilgilerle birlikte, süt ve ürünleri için, aerobik mezofilik bakteri, koliform bakteri, küf-maya ve S. aureus’un bulunma yüzdeleri sırasıyla, %57, %53, %33, %23 olarak belirlenmiştir. Et ve ürünleri için, aerobik mezofilik bakteri, küf-maya, S. aureus’un bulunma yüzdeleri sırasıyla, %13, %40, %70 olarak belirlenmiştir. SONUÇ: Hiçbir örnekte, Salmonella ve E. coli’ye rastlanmamıştır. Gıda örneklerinin çoğunun Türk Gıda Kodeksi Et ve Süt Ürünleri Tebliğine uygun olduğu ve mikrobiyolojik açıdan yeterli bir kalitede olduğu belirlenmiştir. Ancak, bazı örneklerde ürünlerin ürün işleme, taşıma ve saklama koşullarına bağlı belirli düzeylerde kontaminasyon olabileceği düşünülmüştür. Sonuç olarak veriler, süt sağımında hijyenin, açıkta süt satılmamasının ve çiğ sütten peynir üretiminin tercih edilmemesinin önemini ortaya koymaktadır. Et ürünlerinin kontaminasyonlarını ve mikroorganizma üremesini önlemek amacıyla kesim ve satış noktalarındaki mikrobiyolojik denetimin artırılması gerektiği görüşüne varılmıştır. OBJECTIVE: Meat and dairy products, which have an important place for human health, create a very good media for the development of many microorganisms due to the substances in their structure. Based on this situation, it was aimed to investigate the microbiological characteristics and compliance with the standards of milk, yoghurt, cheese, sausage, pastrami and meat offered for sale in Afyonkarahisar. MATERIAL AND METHODS: A total of 60 products from samples of each food type, including 30 dairy products and 30 meat products, were transported in accordance with the cold chain rule and the products were examined microbiologically. Raw milk, yoghurt, fresh cheese, raw meat, sausage and bacon samples were included in this study for analysis. Total Aerobic mesophilic bacteria, yeast-mold, coliform bacteria, S. aureus, E. coli, Salmonella and Coliform group microorganisms were taken 10 g / ml from each of the food samples to be examined, and serial dilutions were prepared as 10-1, 10-2, 10-3, 10-⁴, 10-⁵. The numbers of aerobic mesophilic bacteria, yeast-mold, S. aureus, E. coli, Salmonella and Coliform group microorganisms were determined under suitable media and optimum incubation conditions. RESULTS: The highest number of aerobic mesophilic bacteria and coliform bacteria in milk is 5.5x10⁴ cfu/g and 1.5x103 cfu/g; The highest number of aerobic mesophilic bacteria, coliform bacteria and fungus in yogurt was 2.7x103 cfu/g, 3.0x10⁶ cfu/g and 2.2x103 units/g; The highest number of aerobic mesophilic bacteria, coliform bacteria and fungus in cheese were determined as 2.9x103 cfu/g, 3.7x103 cfu/g and 7.8x101 units/g. The highest number of fungus and staphylococci in sausage is 2.8x102 units/g and 6.0x102 cfu/g; The highest number of fungus and staphylococcus bacteria in pastrami was 3.9x103 units/g and 9.8x10⁴ cfu/g; The highest mesophilic aerobic bacteria and staphylococcus bacteria in raw meat were determined as 7.5x102 cfu/g and 3.8x10⁵ cfu/g. For milk and milk products, the percentages of the presence of aerobic mesophilic bacteria, coliform bacteria, mold-yeast and S. aureus were determined as 57%, 53%, 33%, 23%, respectively. For meat and its products, the percentages of the presence of aerobic mesophilic bacteria, mold-yeast, S. aureus were determined as 13%, 40% and 70%, respectively. CONCLUSIONS: Salmonella and E. coli were not found in any sample. It has been determined that most of the food samples comply with the Turkish Food Codex Meat and Dairy Products Communiqué and are of microbiologically sufficient quality. However, in some instances, it was thought that there might be certain levels of contamination of the products due to product handling, transportation and storage conditions. As a result, the data revealed the importance of hygiene in milking, not selling milk in the open and not choosing cheese production from raw milk. It was concluded that the microbiological control at slaughtering and sales points should be increased in order to prevent contamination of meat products and microorganism growth.

Bu çalışmada P.eurycarpa'dan soyutlanan farklı özütlerin ve bazı vitaminlerin M.tuberculosis üreme hızı üzerine etkileri incelenmiştir. P.eurycarpa meyva ve kabuğundaki lipidler soxhlet cihazı ile ekstrakte edilmiş ve gaz kromatografisi-kütle spektrometrisi ile analizleri yapılmıştır. Meyva ve kabuğun % 50'den fazlasının lipitlerden oluştuğu anlaşılmıştır. gaz kromatografisi-kütle spektrometrisi yöntemi ile kabuk ve meyvada 9 yağ asidi bulunmuştur. En çok saptanan yağ asitleri oleik asit, palmitik asit, ve linolenoik asit olmuştur.Löwenstein Jensen ve Middle Brook Agara meyva ve kabuktan soyutlanan su ile çözünen özütler, lipidler, A,D,E,K vitaminleri ayrıca gaz kromatografisi-kütle spektrometrisi yöntemi ile lipitlerde belirlenen yağ asitleri ayrı ayrı ve birlikte konularak 40 M.tuberculosis suşunun üreme hızına etkileri incelenmiştir.LJ besiyerinde 40 suşun ortalama üreme zamanı 15.9 gün, suda çözünen meyva kabuk özütü varlığında 9.8, meyva özütünde 10.4 ve meyva ve kabuk özütleri varlığında 9.8 gün olarak bulunmuştur. Aynı değerler MBA besiyerinde 12.3, 7.6, 8.5 ve 7.8 gün olarak belirlenmiş ve iki besiyerindede bu özütlerin bakteri üremesini istatitistiksel açıdan anlamlı düzeylerde hızlandırdığı görülmüştür.Diğer yağ asitlerinin, ayrıca A,D,E ve K vitaminlerinin tek tek ve karışımlarının üreme hızına değişik oranlarda olumlu yada olumsuz etki yaptıkları ancak tatmin edici bir zaman kazancı sağlamadıkları görülmüştür.Sonuç olarak P.eurycarpa meyva ve meyva kabuğunun suda çözünmüş özütlerinin M.tuberculosis üreme zamanını en az üçte bir oranında kısalttığı bulunmuştur. Bu özütlerin içindeki etkili maddelerin araştırılması ve besiyerlerine eklenmesinin M. tuberculosis üreme zamanını kısaltacağı, tüberkülozun daha erken tanımlanmasını ve spesifik tedavisine daha hızlı başlanmasını sağlayacağı sonucuna varılmıştır. In this study various the extracts obtained from P.eurycarpa and certain vitamins on the growth time of M.tuberculosis have been analysed.The lipids of P.eurycarpa have been extracted with a soxhlet device and the composition of the fatty acids in the extracted lipid substance was analysed with gas chromatography ? mass spectrometry techniques.The lipids and the water soluble extracts obtained from the nuts and its shells of P.eurycarpa, the vitamins A, D, E and K and also the fatty acids that were found in these lipids determined by gas chromatography ? mass spectrometry techniques were added to Löwenstein Jensen and Middle Brook Agar media.We observed that the average growth time of the 40 strains in the LJ media in the presence of the nut shell soluble water extracts was 9.8 days, in the presence of the nut extracts 10.4 days, and in the presence of the combination of the nut and shell extracts 9.8 days. The average growth time of the 40 strains in the LJ media without any of these additives was 15.9 days. The same values have been detected in MBA medium. We found that all of water soluble extracts accelerated the growth of the mycobacteria in a satistically significant manner in both media.The extracts of P.eurycarpa shortened the growth time of M. tuberculosis at least by a third. It has been concluded that, if these extracts will add to the media used for the cultivation of M. tuberculosis will accelarate the growth of these bacteria. 145

ÖZET PROCION YELLOW BAĞLI pHEMA/KÎTOSAN MEMBRANLARININ HAZIRLANMASI VE LÎZOZÎM ADSORPSİYONUNDA KULLANILMASI ŞİMŞEK, MERİH Kırıkkale Üniversitesi Fen Bilimleri Enstitüsü Biyoloji Anabilim Dalı, Yüksek Lisans Tezi Danışman : Prof. Dr. M. Dr. Gülay BAYRAMOĞLU Ocak 2004, 41 sayfa Procion Yellow 2 bağlı yeni gözenekli hidrojel yapıda poly(2- hidroksietilmetakrilat), p(HEMA/kitosan), membranları afinite destek malzemesi olarak hazırlandı ve model protein olarak seçilen lkozimin adsorpsiyonu çalışmalarında kullanıldı. Afinite pHEMA/kitosan membranları, 2-HEMA monomeri ve kitosan polimer çözeltisi kullanılarak UV ışığı ve AIBN başlatıcısı varlığında fotopolirnerizasyon yöntemi ile sentezlendi. Afinite pHEMA/kitosan membranlar yaklaşık 60 dakikada içerisinde şişme dengesine ulaştı. Boya tutuklanmış membranların denge su miktarı %69 olarak belirlendi ve membrana tutuklanmış Procion Yellow 2 miktarı 189 umol g`1 olarak bulundu. pHEMA/kitosan ve Boyaligand bağlı pHEMA/kitosan membranların lizozim adsorpsiyon çalışmaları manyetik olarak karıştırmalı bir hücre içerisinde yapıldı. Bu membranların adsorpsiyon kapasiteleri adsorpsiyon ortamındaki lizozim başlangıç konsantrasyonu, sıcaklık, tuz ve pH değerleri değiştirilerek belirlendi. PHEMA/kitosan ve boya ligand bağlı pHEMA/kitosan membranların lizozim adsorpsiyon kapasiteleri sırasıyla 7.2 mg g'1 ve 38.6 mg g`1 olarak bulundu. Adsorplanan lizozimin %90'nı pH 8.0'de 1.0 M KSCN içeren desorpsiyon ortamında 60 dakikada içerisinde geri kazanıldı. Procion Yellow 2 was covalently attached onto pHEMA/chitosan membrane as a dye»ligand. The water content of the dye-ligand immobilized membrane was about 69%. The amount of immobilized dye on the membrane surface was found to be as 189 umol g`'. The lysozyme adsorption test on the pHEMA/chitosan and dye ligand immobilized pHEMA/chitosan was performed in a continuous stirrer cell. The optimum lysozyme conditions were mdetermined by changing the effective adsorption parameters such as pH, ionic strength, temperature and initial lysozyme concentration. Lysozyme adsorption capacity of dye-ligand attached membrane was 38.6 mg ml`1. Adsorbed lysozyme was desorbed up to 90% by using 1.0 M KSCN as the desorption agent It was possible to reuse dye-ligand affinity membranes without significant reduction in the protein adsorption capacity. These features make the dye-ligand very good candidate for use in metal-affinity chelate separation of proteins and would be effective in processing large volume of biological fluid containing a target protein. Amaç: Afyon bölgesinde yaşayan gebelerde Toxoplasma gondii TOXO , Rubella ve Cytomegalovirus CMV için IgM ve IgG antikor varlığı ve IgG avidite düzeyleri ile Herpes simpleks virüs 2 HSV ’a karşı IgM ve IG antikor varlığının retrospektif olarak incelenmesi ve seroprevalanslarının belirlenmesi amaçlanmıştır. Yöntem ve Gereçler: Bu çalışmaya Afyon Kocatepe Üniversitesi Tıp Fakültesi Hastanesinde takip edilen gebe kadınların serumlarında Toksoplazma, Rubella ve CMV’a karşı IgG ve IgM antikor varlığı Enzyme Immunoassay EIA metodu ile VIDAS,Biomerieux /France çalışılmıştır. HSV Tip 2 antikorlarına EIA metodu ile MiniVIDAS Biomerieux /France bakılmıştır. Ayrıca, CMV IgG avidite testi VIDAS / Biomerieux France, TOXO IgG avidite testi VIDAS/ Biomerieux SA/69280 France, RUB IgG avidite testi ise Dia.Pro Diagnostic Bioprobes Srl./Via G. Italy ile çalışılmıştır.Bulgular: Toplam 1284 gebe kadının serum örneğinde Toksoplazma IgG, Rubella IgG, CMV IgG ve HSV Tip 2 IgG seropozitifliği sırasıyla; % 23.4, % 94.5, % 96.0 ve % 7.0 olarak bulunmuştur. Toksoplazma, rubella, sitomegalovirus ve HSV-2’ye karşı IgM seropozitiflik oranları ise sırasıyla % 1.5, % 2.5, % 2.3, ve % 1.7 olarak belirlenmiştir. Çalışmaya alınan gebe kadınların yaş, gravide ve parite sayıları değerlendirildiğinde yaş ortalaması 29.6±7.65 1645 , gebelik sayısı ortalaması 2.18±0.33 , doğum sayısı ortalaması ise 1.2±0.78 , olarak hesaplanmıştır.Sonuç: Afyon ili ve çevresinde yaşayan gebelerde toksoplazma, rubella, sitomegolvirus ve herpes tip 2 virusları için bulunan seropozitiflik oranları ülkemizde daha önce belirlenmiş oranlarla genel olarak uyumludur. Test sonuçlarına göre en yüksek seropozitiflik oranı rubella, en yüksek seronegatiflik oranı ise Toxoplasma gondii için bulunmuştur. Bu taramaların gebelerde sadece IgM takibi şeklinde gerçekleştirilmesi önerilebilir. Ancak konjenital enfeksiyonların ciddi sonuçlar doğurabilmeleri nedeniyle tüm gebeliklerde bu taramaların yapılması riskli doğumların vakit geçirilmeden sonlandırılması açısından büyük önem taşımaktadır. CMV seropozitifliğinin yüksek bulunmuş olması bölgede düşük sosyoekonomik düzeyi işaret etmektedir. HSV-2 için daha önceki çalışma sonuçlarına göre daha düşük bir seropozitiflik oranı belirlenmiştir, Aim: To determine seroprevalence and presence of IgM and IgG antibodies and IgG avidity levels for Toxoplasma gondii, Cytomegalovirus CMV , Rubella, Herpes simpleks virus HSV 2 and presence of IgM and IgG antibodies against Herpes simplex virus in pregnant women who live in Afyon region.Material and Methods: Toxoplasma gondii IgG- IgM, Cytomegalovirus CMV IgG-IgM, Rubella IgG-IgM Herpes simpleks virus2 IgG-IgM antibodies in serum of one 1284 pregnant women who followed by Afyon Kocatepe University medical faculty hospital were detected by Enzyme Immunoassay EIA method using VIDAS Biomerieux / France . HSV type 2 antibodies were detected by EIA method using MiniVIDAS Biomerieux /France . In addition, CMV IgG avidity tests were measured using VIDAS / Biomerieux France, TOXO IgG avidity tests were measured using VIDAS/ Biomerieux SA/69280 France, RUB IgG avidity tests were measured using Dia.Pro Diagnostic Bioprobes Srl./Via G. Italy.Results: SSeropozitivity of toxoplasma IgG, rubella IgG, CMV IgG, HSV type 2 IgG were detected % 23.4, % 94.5, % 96.0 ve % 7.0 respectively. Seropozitivity of toxoplasma IgM, rubella IgM, CMV IgM, HSV type 2 IgM were detected % 1.5, % 2.5, % 2.3, ve % 1.7 respectively. Furthermore, the values of age, gravida and parity of the pregnant women in the study were calculated as 29.6±7.65 , 2.18±0.33 , 1.2±0.78 respectively.Conclusion: In this study, TORCH group seropositivity rates are similar Turkey data in pregnant women living in Afyon and around. According to test results, the highest seropositivity rate was found for rubella. The highest seronegativity rate was found to Toxoplasma gondii. IgM follow-up is only advisable for high risk pregnancies due to economic reasons.However, making these screening in pregnancy has a great importance in terms of ending the risky birth. CMV seropositivity was found to be quite high. This status indicates low socioeconomic level in the region. The low seropositivity rates of HSV-2 is an indicator that polygamy is low in region

AMAÇ: Üriner sistem enfeksiyonlarının ve nozokomiyal enfeksiyonların en önemli etkenlerinden biri olan enterokok cinsi bakterilerin mortalitesi yüksek enfeksiyonlara sebep olabildiği bilinmektedir. Çalışmada, Afyonkarahisar Sağlık Bilimleri Üniversite hastanesinin çeşitli poliklinik ve servislerden Tıbbi Mikrobiyoloji Laboratuvarı’na gönderilen idrar örneklerinden izole edilen enterokokların tür dağılımı ve kliniklere göre dağılımı ile antibiyotik duyarlılıklarının araştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Ocak 2014 - Aralık 2018 tarihleri arasında laboratuvarımıza gelen idrar örneklerinden bakterilerin izolasyonu için standart mikrobiyolojik yöntemler kullanılmıştır. İdentifikasyon ve antibiyotik duyarlılık profili için VITEK 2 (bioMerieux, France) tam otomatize identifikasyon ve antibiyotik duyarlılık sistemi kullanılmıştır. Kültür sonuçları retrospektif olarak taranmıştır. BULGULAR: Bu çalışmada en sık Enterococcus faecalis (%69.1) üremesi gözlenirken bunu Enterococcus faecium (%27.6) türü takip etmektedir. Enterokok türlerinin sırasıyla en sık Fizik Tedavi ve Rehabilitasyon (%23), Çocuk Sağlığı ve Hastalıkları (%22.6) ve Üroloji (%11.9) birimlerinden gelen örneklerden izole edildiği gözlenmiştir. Vankomisin direnci Enterococcus faecalis için %1.9, Enterococcus faecium için %6.3 olarak belirlenmiştir. Enterococcus faecium için ampisillin (%83.9) en yüksek direnç oranına sahipken Enterococcus faecalis için bu oran %10.6 olarak belirlenmiştir. Yüksek düzey gentamisin ve streptomisin direnç oranları sırasıyla, Enterococcus faecalis için %14.8-%6.2 ve Enterococcus faecium için %36.4-%10.5 olarak belirlenmiştir. Penisilin direnci ise Enterococcus faecalis için %4.8, Enterococcus faecium için %20.3 olarak belirlenmiştir. SONUÇ: Bu çalışmada, Enterococcus faecium ve Enterococcus faecalis’ in bazı antibiyotiklere karşı direnç göstermesi, bu bakterilere bağlı olarak ortaya çıkan üriner sistem enfeksiyonlarında yaşanan tedavi zorluklarını açıklamaktadır. Diğer çalışmalara ve çalışmamıza ait veriler incelendiğinde enterokoklar da antibiyotik direnç oranları ülkemizde yöresel ve dünyada bölgesel olarak farklılık göstermektedir. Enterokok enfeksiyonların da yöresel olarak direnç profillerinin belirlenmesi tedavi başarısı açısından ayrıca bu enfeksiyonların yayılmasını önlemek açısından büyük bir öneme sahiptir OBJECTIVE: Enterococcal bacterial infections, which are the main cause of urinary tract infections and nosocomial infections, are known to cause serious and high mortality infections. The aim of this study was to investigate the antibiotic susceptibility and distribution of enterococci isolated from urine samples sent to the Medical Microbiology Laboratory from various outpatient clinics and services of Afyonkarahisar University of Health Sciences Hospital. MATERIAL AND METHODS: Between January 2014 and December 2018, microbiological methods were used for the isolation of bacteria from urine samples. VITEK 2 (bioMerieux, France) fully automated identification and antibiotic susceptibility system were used for identification and antibiotic susceptibility profile. Culture results were evaluated retrospectively. RESULTS: In our study, Enterococcus faecalis (69.1%) was the most common in this study. Enterococcus faecium (27.6%) was the second. Enterococci were most frequently observed in urine specimens sent from Physical Therapy and Rehabilitation (23%), Child Health and Disease (22.6%) and Urology Units (11.9%). Vancomycin resistance was 1.9% for Enterococcus faecalis and 6.3% for Enterococcus faecium. Ampisillin resistance was 83.9% for Enterococcus faecium, whereas this rate was determined to be 10.6% for Enterococcus faecalis. The rates of high-level gentamicin and streptomycin resistance were found to be 14.8%-6.2% for Enterococcus faecalis and 36.4%-10.5% for Enterococcus faecium, respectively. Penicillin resistance was found to be 4.8% for Enterococcus faecalis and 20.3% for Enterococcus faecium. CONCLUSIONS: In our study, antibiotics resistance of Enterococcus faecium and Enterococcus faecalis explains the treatment di?iculties experienced in urinary tract infections. When the data from other studies and our study are examined, the rates of antibiotic resistance of enterococci di?er according to the local in our country and regional in the world. Determination of local resistance profiles in enterococcal infections has a great importance in terms of treatment success and also to prevent the spread of these infections

Hence, the aim of this study was to investigate, by using different methods, the resistance profiles of A. baumannii strains isolated from different clinical specimens; from colistin and tigecycline antibiotics, and also the distribution of this resistance according to the clinical samples. Material and Methods: For this study, 1,265 clinical samples (a samples from each patient) were obtained from various clinics, between; January 2015/December 2018. Identification was conducted by VITEK® 2 compact (bioMerieux, USA) and conventional biochemical tests. Antibiotic susceptibility tests were performed by VITEK 2, and the results of colistin and tigecycline were confirmed by E test and the broth microdilution method. Results: A. baumannii strains (1,265) were most frequently isolated from tracheal aspirate, sputum and blood samples. At the same time, strains were obtained from intensive care units (70.4%) as well as other clinics (29.6%). The rates of colistin and tigecycline-resistant strains were determined using VITEK 2, E test and the broth microdilution methods as: 3.0%, 5.7%, 9.0% and 21.7%, 24.5%, 33.0%, respectively. Conclusion: The determination of appropriate antibioticis are important for empirical treatment. Colistin and tigecycline have become prominent as an important, alternative agent in the treatment of A. baumannii-related infections. The results of this study show that colistin and tigecycline resistance rates in intensive care units have been increasing gradually over the years. Monitoring of resistance patterns of nonfermentative bacteria, isolated from intensive care units, is important for the immediate initiation of appropriate empirical treatment. The aim of this study is to determine the antimicrobial effects of eight different canal sealants used in dentistry against five different standard strains and five clinical isolates by direct exposure test and agar diffusion test. In vitro antimicrobial effects of EndoSequence BC, Well Root ST, MTA-Fillapex, Ortho-MTA, AH Plus, 2 Seal, Sealapex, Adseal sealers to S. aureus, P. aeruginosa, E. faecalis, C. albicans and S. mutans (5 clinical isolates and 5 standard strains), were investigated by the direct exposure test and the agar diffusion test. In periods of 24h, 48h and 72 hours, zone of growth inhibition were measured. Results observed by agar diffusion test exhibited that Adseal sealer occurred the strongest antimicrobial activity to S. aureus, P. aeruginosa, S. mutans. In addition to this, 2 Seal sealer presented strongest antimicrobial activity to E. faecalis and C. albicans. All root canal sealers have been shown to have antimicrobial activity after 48 hours at the latest by direct exposure test. All sealers showed an antimicrobial effect by creating inhibition zones of different degrees against all microorganisms. All sealers have higher antimicrobial activity against clinical isolates than standard strains. In order for the root canal sealers to be effective against these microorganisms, they must be provided with a certain incubation period. In addition, reference studies should be made globally and periodically by the world dental authorities to determine the antimicrobial activities of these sealers. In this study, the presence of blaNDM-1 gene in carbapenem-resistant Enterobacterales obtained from various clinical specimen of a University Hospital in Turkey between the years 2014-2019, has been investigated. Carbapenem-resistant Enterobacterales were identified using Vitek 2. The presence of Metallo beta lactamase in these bacteria was investigated by phenotypically modified hodge test and genotypically by PCR. Of the 110 carbapenem-resistant Enterobacterales isolates, six (5.5%), 17 (15.5%) and 87 (79.0%) were identified as E. cloacae, E. coli and K. pneumoniae , respectively. While blaNDM-1gene was found in 35 (31.8%) K. pneumoniae and three (2.7%) E. coli strains, all 38 strains were found positive for blaNDM-1gene by the modified hodge test. It was determined that six E. cloacae strains did not contain the blaNDM-1 gene. Multiple antibiotic resistances were detected in carbapenem-resistant Enterobacterales strains positive for blaNDM-1. However, 29 K. pneumoniae and two E. coli strains were identified as resistant to tigecycline. Turkey also risks of the world along with other resistance enzymes and bacteria NDM-1 producing the rates must be determined globally and periodically. The fast spread of such resistant through transferable ways should not be overlooked, because enzymatic resistance genes can lead to high mortality hospital outbreaks worldwide.₀ [Med-Science 2021; 10(2.000): 571-6]

Metallo beta-laktamaz (MBL)'lar aztreonam dışında karbapenemler de dahil olmak üzere tüm betalaktam grubu antibiyotikleri hidrolize ederler. Antibiyotiklerdeki direncin yayılmasını sınırlamak ve tedaviye yön vermek amacıyla karbapenem direnç tiplerinin tespit edilmesi gerekmektedir. Bu çalışmanın amacı, giderek artan imipenem ve meropenem dirençli Pseudomonas aeruginosa ve Acinetobacter baumannii klinik izolatlarında MBL üretimini iki farklı fenotipik yöntem kullanarak incelenmesidir. Karbapenem dirençli 54 A.baumannii ve 40 P.aeruginosa klinik suşlarına MBL tespiti için Kombine disk difüzyon testi(KDDT) ve Modifiye Hodge testi(MHT) uygulanmıştır. KDDT: 0.5 McFarland bakteri süspansiyonu müller hinton agara ekildi. İki imipenem diski (10μg) aralarında en az 22 mm olacak şekilde besiyeri yüzeyine yerleştirildi, imipenem disklerinden bir tanesinin üzerine 10 μl EDTA eklendi. Aynı yöntem meropenem/EDTA için de uygulandı. İmipenem ve meropenem diskleri zon çapları, EDTA'lı disklerin zon çaplarından 7 mm'den daha az ise pozitif kabul edildi. MHT: Müller Hinton Agar besiyerine E.coli ATCC 25922 standart suşu yayıldı, meropenem 10 μg diski plağın ortasına yerleştirildi. İzolatlar imipenem diskinden perifere doğru ekildi. Duyarlılık zon çapının yonca yaprağı şeklinde bozulması durumunda test pozitif kabul edildi. Combined disk diffusion (CDT) test and Modified Hodge test (MHT) were performed to detect MBL of 51 A.baumannii and 40 P.aeruginosa clinical strains with resistant to carbapenem.The CDT:0.5 McFarland bacteria suspension was plated into the Muller Hinton Agar.The two imipenem disks (10μg) were placed on the medium surface with a minimum of 22 mm between them, 10 μl of EDTA was added onto one of the imipenem disks. The same method was applied for meropenem/EDTA.The results were considered to be positive if zone diameters of imipenem and meropenem disks were less than 7 mm from the zone diameters of EDTA disks.MHT: E.coli ATCC 25922 standard strain was spread on Müller Hinton Agar medium,10 μg of meropenem was placed in the center of the plate.Isolates were plated from the imipenem disk to the periphery. The deterioration of sensitivity zone clover leaf accepted as positive. In this study, 5 (12.5%) of 40 strains of P.aeruginosa were positive by MHT,and 21 (52.5%) were positive by imipenem/EDTA disk test and 23 (57.5%) by meropenem/EDTA disk test.43(84.3%) of 54 A.baumannii strains were detected by MHT, 35 (68.6%) were by imipenem/EDTA disk test and 46 (90.2%) were positive by meropenem/EDTA disk test. Amaç: Bu çalışmada; kliniklerde yatan hastalardan izole edilen Gram negatif basillerin indüklenebilir beta-laktamaz aktivitelerini belirlemek ve antibiyotik direnç profilini ortaya çıkarmak amaçlanmıştır.Gereç ve Yöntem: Bu çalışmada Afyonkarahisar Sağlık Bilimleri Üniversitesi Tıp Fakültesi Hastanesi' nin farklı kliniklerinde tedavi olan hastalara ait idrar, kan, balgam, beyin omurilik sıvısı (BOS), yara yeri ve abse lavajından alınan kültür örnekleri steril şartlarda hastadan alınıp, laboratuvar şartlarında çalışma gününe kadar -80 °C'de muhafaza edilen izolatlar sıvı buyyon besi yerinde 37°C etüvde 48 saat enkübe edilerek canlandırıldı. Canlandırılan bakteriler Gram boyama yöntemi uygulandı. Gram negatif görünümündeki örnekler kanlı agar, çikolata agar, EMB agar ve saboroud dekstroz agar besiyerlerine ekimleri yapılan bakteriler, 37°C etüvde 16-24 saat enkübe edildi. EMB agar besiyerinde üreyen bakteriler çalışmaya dahil edildi. EMB agar besiyerinde üreyen suşların, VITEK-2 tam otomatize cihazı ileri biyokimyasal testlerini 4-6 saatte yaparak identifikasyon sağlandı.Aynı cihaz 16-24 saat içerisinde çalışmaya dâhil edilen P. aeruginosa, E.cloacae complex, P. mirabilis, S.marcescens, M.morganii, S.liquefaciens group ve P.rettgeri türü basillerin antibiyotik duyarlılık sonuçlarını otomatik olarak vermiştir. İndüklenebilir Beta-laktamaz araştırması için ise Müeller-Hinton agara ayrı ayrı ekimleri yapılmıştır. İndükleyici ajan olarak imipenem (10 μg) kullanılmış ve disklerin aralarındaki uzaklık 20 mm olacak şekilde besiyerinin yüzeyine yerleştirilmiştir.Bulgular: Çalışmaya dâhil edilen 104 izolatın 92 tanesinde indüklenebilir beta-laktamaz aktivitesi tespit edilmiştir. Beta-laktamaz aktivitesi en fazla olan bakteri türü P.aeruginosa, en azbeta-laktamaz aktivitesi olan ise S.marcescens olarak tespit edilmiştir. P.aeruginosa türü bakteri en çok PIP/Tazo'ya, S.marcescens türü bakteri kolistin' e, P.mirabilis türü bakteri ampisilin, sefrazidim, seftriakson, sefuroksim ve sefuroksim-aksetil' e, E.cloacae complex türü bakteri ampisilin, sefoksitin ve amoxicillin' e, diğer türler de ampisilin, kolistin, amoxicillin, sefuroksim ve sefuroksim-aksetil' e direnç göstermiştir. P.aeruginosa türü bakteri en çok cerrahi kliniklerde, S.marcescens ve P.mirabilis türü bakteriler dâhili kliniklerde, E.cloacae complex türü bakteri pediatri kliniğinde yatan hastalardan izole edilmiştir. Aim: In this study; The aim of this study was to determine the inducible beta-lactamase activity of Gramnegative bacilli isolated from inpatients and to reveal the antibiotic resistance profiles.Methodology: In this study, the samples of urine, blood, sputum, cerebrospinal fluid (CSF), wound and abscess lavage taken from patients who were treated at different clinics of Afyonkarahisar University of Health Sciences Hospital were taken from the patient under sterile conditions, Isolates were incubated in a liquid broth at 37°C for 48 hours. Gram staining method was applied tothis bacteria. Samples with Gram negative appearance were cultured on blood agar, chocolate agar, EMB agar and saboroud dextrose agar media and incubated at 37°C for 16-24 hours. The bacteria breeding on EMB agar were included in the study. EMB agar in breeding strains, VITEK-2 fully automated device identification was achieved by advanced biochemical tests 4-6 hours. The same device gave results of antibiotic susceptibility results of P. aeruginosa, E.cloacae complex, P. mirabilis, S.marcescens, M.morganii, S.liquefaciens group and P.rettgeri species in 16-24 hours. For the inducible beta-lactamase research, we planted the Müeller-Hinton agar separately. Imipenem (10 μg) was used as the inducing agent and the distance between the discs was placed on the surface of the medium to be 20 mm.Findings: Inducible beta-lactamase activity was determined in 92 of 104 isolates included in the study.The bacteria with the highest beta-lactamase activity were P.aeruginosa, whereas the least beta-lactamase activity was determined as S.marcescens.P.aeruginosa species bacteria most commonly to PIP/Tazo, S. marcescens type bacteria colistin, P.mirabilis type bacteria ampicillin, cefrazidime, ceftriaxone, cefuroxime and cefuroxime-aksetil, E.cloacae complex type bacteria ampicillin, cefoxitin and amoxicillin, and other species showed resistance to ampicillin, colistin, amoxicillin, cefuroxime and cefuroxime-axetil. P. aeruginosa bacteria of the species most surgical clinics, S.marcescens and P. mirabilis bacteria of the species in internal departments, E.cloaca complexe type bacteria were isolated from patients in clinical pediatrics. 114

Hastane kökenli ve de toplum kökenli en önemli infeksiyon etkenlerinden biri de bilinmektedir ki Staphylococcus aureus bakterisidir. Ciddi infeksiyonlara neden olan bu bakteri, deri ve yumuşak doku infeksiyonlarına, bakteriyemiye, solunum sistemi infeksiyonlarına, üriner infeksiyonlara neden olmaktadır. MRSA infeksiyonlarında uzun yıllar vankomisin antibiyotiği başarılı bir şekilde kullanılmıştır. 1996 senesinde ise ilk defa vankomisine orta duyarlılık gözlenmiştir. Bir yıl sonra da Japonya'da ilk vankomisin dirençli S.aureus'un heterojen izolatı tanıtılmıştır. MRSA suşları bilinmektedir ki bütün β-laktam grubu antibiyotiklere dirençlidir. Aminoglikozid, tetrasiklin, linkozamid, kinolon gibi antibiyotiklere de direnç gösterirler. Son yıllarda glikopeptidlere de direnç geliştiği görülmektedir. Dolayısıyla daptomisin, linezolid, tigesiklin gibi antibiyotiklerin de tedavide kullanımı başlamıştır. Görülmektedir ki S.aureus bakterisi kazanmış olduğu dirençler ile yeni yeni birçok antibiyotik arayışına götürmektedir. Biz de bu çalışmamızda, Afyonkarahisar Sağlık Bilimleri Üniversitesi Tıp Fakültesi Hastanesinde 2012/2015 yılları arasında görülen MRSA suşlarının daptomisin ve linezolid antibiyotiklerine olan direnç durumlarını retrospektif olarak değerlendirilmesini amaçladık.Geçmişe yönelik olarak çalıştığımız bu bakteri suşlarından MRSA olarak tanımlananların, hangi klinik ve yoğun bakımdan, hangi klinik materyalden, hangi cinsiyetteki hastalardan izole edildikleri ve daptomisin ve linezolid antibiyotiklerine olan direnci incelendi. Toplamda 618 hasta çalışılmıştır. Bu 618 hastadan 130 tanesinde MRSA gözlemlenmiştir. Bu MRSA suşları 26 klinik ve yoğun bakımdan gelen numunelerdendir. Diğer 488 hastadan alınan numuneler ise çeşitli stafilokok türleridir. Buna göre MRSA'nın diğer etkenlere göre görülme durumu % 21,035 olarak hesaplanmıştır. Bu numunelerden MRSA durumları ise; 2 apse, 3 balgam, 2 burun, 1 diyabetik ayak, 1 doku, 9 idrar, 53 kan, 1 kulak, 26 trakeal aspirat ve 32 yara şeklindedir. MRSA görülen 130 hastadan 83 tanesi erkek, 47 tanesi kadın hastadır. Buna göre %63,84 erkek, %36,15 kadın hasta profili görülmüştür.130 hastada görülen MRSA üzerinde yapılan çalışmada 4 tanesinde Linezolid dirençliyken 126 tanesinde duyarlı görülmüştür. 77 tanesinde daptomisin dirençliyken 53 tanesinde duyarlı görülmüştür. Daptomisin duyarlı olan 53 hastada aynı zamanda linezolid de duyarlıdır. Linezolid dirençli olan 4 hastada ise aynı zamanda daptomisin de dirençli görülmüştür. Daptomisin ve Linezolid antibiyotiklerinin her ikisine de dirençli olan 4 hastaya ait numunelerden 1 tanesi yara, 3 tanesi de kan numunesidir. Staphylococcus aureus is also known as the most important infectious agent of hospital origin and community origin. This bacterium, which causes serious infections, causes skin and soft tissue infections, bacteremia, respiratory system infections and urinary infections. Vancomycin antibiotic has been successfully used for many years in MRSA infections. In 1996, intermediate sensitivity to vancomycin was observed for the first time. A year later, a heterogeneous isolate of the first vancomycin-resistant S.aureus was introduced in Japan. MRSA strains are known to be resistant to all β-lactam group antibiotics. They also exhibit resistance to antibiotics such as aminoglycoside, tetracycline, linkosamide, quinolone. In recent years, resistance to glycopeptides has been observed. Therefore, the use of antibiotics such as daptomycin, linezolid and tigecycline has also begun in the treatment. It is seen that S.aureus bacterium with the resistance gained leads to a new search for many new antibiotics. In this study, we aimed to retrospectively evaluate the resistance states of daptomycin and linezolid in MRSA strains seen between 2012 and 2015 at Afyonkarahisar Health Sciences University Medical Faculty Hospital.We investigated the resistance of these bacterial strains, which were identified as MRSA, from which clinical and intensive care, which clinical material, from which sex patients were isolated, and their resistance to daptomycin and linezolid antibiotics. A total of 618 patients were studied. MRSA was observed in 130 of these 618 patients. These MRSA strains belong to 26 clinical and intensive care samples. The samples taken from the other 488 patients are various Staphylococcus species. According to the other factors, MRSA was calculated as 21,035%. MRSA cases from these samples; 2 abscesses, 3 sputum, 2 nose, 1 diabetic foot, 1 tissue, 9 urine, 53 blood, 1 ear, 26 tracheal aspirate and 32 wound. Of the 130 patients with MRSA, 83 were male and 47 were female. According to this, 63.84% male and 36.15% female patients were seen.In the study conducted on MRSA in 130 patients, Linezolid was resistant in 4 and linezolid sensitive in 126 cases. Daptomycin was resistant in 77 specimens and sensitive in 53 samples. In 53 patients who are sensitive to daptomycin, linezolid is also sensitive. 4 patients with linezolid resistance were also resistant to daptomycin. Of the 4 patients who were resistant to both Daptomycin and Linezolid antibiotics, one of the samples was wound and 3 were blood samples. 75

569317 Amaç: Bu çalışmada; kliniklerde yatan hastalardan izole edilen Gram negatif basillerin indüklenebilir beta-laktamaz aktivitelerini belirlemek ve antibiyotik direnç profilini ortaya çıkarmak amaçlanmıştır. Gereç ve Yöntem: Bu çalışmada Afyonkarahisar Sağlık Bilimleri Üniversitesi Tıp Fakültesi Hastanesi' nin farklı kliniklerinde tedavi olan hastalara ait idrar, kan, balgam, beyin omurilik sıvısı (BOS), yara yeri ve abse lavajından alınan kültür örnekleri steril şartlarda hastadan alınıp, laboratuvar şartlarında çalışma gününe kadar -80 °C'de muhafaza edilen izolatlar sıvı buyyon besi yerinde 37°C etüvde 48 saat enkübe edilerek canlandırıldı. Canlandırılan bakteriler Gram boyama yöntemi uygulandı. Gram negatif görünümündeki örnekler kanlı agar, çikolata agar, EMB agar ve saboroud dekstroz agar besiyerlerine ekimleri yapılan bakteriler, 37°C etüvde 16-24 saat enkübe edildi. EMB agar besiyerinde üreyen bakteriler çalışmaya dahil edildi. EMB agar besiyerinde üreyen suşların, VITEK-2 tam otomatize cihazı ileri biyokimyasal testlerini 4-6 saatte yaparak identifikasyon sağlandı.Aynı cihaz 16-24 saat içerisinde çalışmaya dâhil edilen P. aeruginosa, E.cloacae complex, P. mirabilis, S.marcescens, M.morganii, S.liquefaciens group ve P.rettgeri türü basillerin antibiyotik duyarlılık sonuçlarını otomatik olarak vermiştir. İndüklenebilir Beta-laktamaz araştırması için ise Müeller-Hinton agara ayrı ayrı ekimleri yapılmıştır. İndükleyici ajan olarak imipenem (10 ?g) kullanılmış ve disklerin aralarındaki uzaklık 20 mm olacak şekilde besiyerinin yüzeyine yerleştirilmiştir. Bulgular: Çalışmaya dâhil edilen 104 izolatın 92 tanesinde indüklenebilir beta-laktamaz aktivitesi tespit edilmiştir. Beta-laktamaz aktivitesi en fazla olan bakteri türü P.aeruginosa, en azbeta-laktamaz aktivitesi olan ise S.marcescens olarak tespit edilmiştir. P.aeruginosa türü bakteri en çok PIP/Tazo'ya, S.marcescens türü bakteri kolistin' e, P.mirabilis türü bakteri ampisilin, sefrazidim, seftriakson, sefuroksim ve sefuroksim-aksetil' e, E.cloacae complex türü bakteri ampisilin, sefoksitin ve amoxicillin' e, diğer türler de ampisilin, kolistin, amoxicillin, sefuroksim ve sefuroksim-aksetil' e direnç göstermiştir. P.aeruginosa türü bakteri en çok cerrahi kliniklerde, S.marcescens ve P.mirabilis türü bakteriler dâhili kliniklerde, E.cloacae complex türü bakteri pediatri kliniğinde yatan hastalardan izole edilmiştir. Aim: In this study; The aim of this study was to determine the inducible beta-lactamase activity of Gramnegative bacilli isolated from inpatients and to reveal the antibiotic resistance profiles. Methodology: In this study, the samples of urine, blood, sputum, cerebrospinal fluid (CSF), wound and abscess lavage taken from patients who were treated at different clinics of Afyonkarahisar University of Health Sciences Hospital were taken from the patient under sterile conditions, Isolates were incubated in a liquid broth at 37°C for 48 hours. Gram staining method was applied tothis bacteria. Samples with Gram negative appearance were cultured on blood agar, chocolate agar, EMB agar and saboroud dextrose agar media and incubated at 37°C for 16-24 hours. The bacteria breeding on EMB agar were included in the study. EMB agar in breeding strains, VITEK-2 fully automated device identification was achieved by advanced biochemical tests 4-6 hours. The same device gave results of antibiotic susceptibility results of P. aeruginosa, E.cloacae complex, P. mirabilis, S.marcescens, M.morganii, S.liquefaciens group and P.rettgeri species in 16-24 hours. For the inducible beta-lactamase research, we planted the Müeller-Hinton agar separately. Imipenem (10 ?g) was used as the inducing agent and the distance between the discs was placed on the surface of the medium to be 20 mm. Findings: Inducible beta-lactamase activity was determined in 92 of 104 isolates included in the study.The bacteria with the highest beta-lactamase activity were P.aeruginosa, whereas the least beta-lactamase activity was determined as S.marcescens.P.aeruginosa species bacteria most commonly to PIP/Tazo, S. marcescens type bacteria colistin, P.mirabilis type bacteria ampicillin, cefrazidime, ceftriaxone, cefuroxime and cefuroxime-aksetil, E.cloacae complex type bacteria ampicillin, cefoxitin and amoxicillin, and other species showed resistance to ampicillin, colistin, amoxicillin, cefuroxime and cefuroxime-axetil. P. aeruginosa bacteria of the species most surgical clinics, S.marcescens and P. mirabilis bacteria of the species in internal departments, E.cloaca complexe type bacteria were isolated from patients in clinical pediatrics.

569654 E. coli K. pneumoniae suşlarında genişlemiş spektrumlu beta laktamazların farklı Yöntemlerle Araştırılması Amaç: Afyonkarahisar Sağlık Bilimleri Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesinde hastalardan izole idilen E. coli K. pneumoniae suşlarında genişlemiş spektrumlu beta laktamazların Kombine Disk Yöntemi, Çift Disk Sinerji Yöntemi ve Üç Boyutlu Test Yöntemleri ile tespiti amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda hastalardan izole edilen 125 E. coli ve 25 K. pneumoniae suşlarına 3 yöntem uygulanmıştır. Suşlar konvansiyonel yöntemlerle tür düzeyinde tanınmıştır. Çift disk sinerji yöntemi ile GSBL üretimini 125 E. coli izolatı için 85 (%68,00) ve 25 K. pneumoniae izolatı için 9 (%76,00), üç boyutlu test ile E. coli için 125 izolatta 105 (%84,00) K. pneumoniae için de 25 izolatta 17 (%68,00), kombine disk difüzyon yöntemi ile E. coli için 125 izolatta 77 (%61,60) K. pneumoniae için 25 izolatta 17 (%68,00) tespit edilmiştir. Enterobacteriaceae suşlarında farklı direnç genleri sebebiyle fenotipik yöntemlerle GSBL saptanması genellikle yanlış sonuçlara neden olmaktadır. Çalışmamızda E. coli izolatları için fenotipik yöntemlerden, üç boyutlu test , K. pneumoniae izolatı için ise çift disk sinerji yöntemi en yüksek GSBL saptama yöntemi olarak bulunmuştur. Çalışmamızın sonucuna göre moleküler testler ile fenotipik testlerin bir arada çalışılması önerilmektedir. şartlarına uygun olmayan merkezlerde GSBL araştırılmasında en etkili yöntem E. coli için üç boyutlu test , K. pneumoniae için çift disk sinerji testi uygulanmalıdır. Investigation of expanded-spectrum beta-lactamases in E. coli K. pneumoniae strains by different methods Objective: Combined Disc Method, Double Disc Synergy Method and Three Disc Dimensional Testing Methods Materials and Methods: Three methods were applied to 125 E. coli and 25 K. pneumoniae strains isolated from patients. Strains were identified at the species level by conventional methods. Detection of ESBL by phenotypic methods due to different resistance genes in Enterobacteriaceae strains often leads to wrong results. In our study, the three-dimensional test for E. coli isolates and the double disc synergy method for K. pneumoniae isolates were found to be the highest ESBL detection methods. According to the results of our study, it is suggested that molecular and phenotypic tests should be combined. The most effective method to investigate ESBL in centers that do not meet the requirements of E. coli for the three-dimensional test, K. pneumoniae double disc synergy test should be applied.
