Flavonoids are the most widespread polyphenols in the plant kingdom. They are also associated to many of the beneficial effects of plant-based food and diet, especially in cardiovascular health. Among them, epicatechin, a flavonoid from the flavan-3-ols family is known for its antioxidant or anti-inflammatory properties and capacity to protect from a range of dysfunctions related to chronic diseases. Hemostasis, which contributes to the pathophysiology of cardiometabolic diseases, could also be modulated by flavonoids. The first part of this thesis was to study the impact of epicatechin on the three main stages of hemostasis which are primary hemostasis, plasma coagulation and fibrinolysis, using global tools to identify the hemostatic phenotype induced by epicatechin. We have shown that epicatechin at high concentration inhibits platelet function and modulates the coagulant and fibrinolytic profile. In the second part, we tried to determine the effect of epicatechin on microparticles and their procoagulant functions. While the overall microparticle concentration is not impacted by epicatechin, the latter appears to attenuate their procoagulant potential when detected by the formation of thrombin. Finally in the third part, we studied the final process of fibrin clot formation. Using microscopy, spectrophotometric and functional techniques, we were able to determine that the clot formed in the presence of epicatechin is less dense and more permeable. These in vitro data have yet to be confirmed in vivo to objectify the impact of epicatechin and its metabolites at concentrations reachable through dietary intakes., Les flavonoïdes sont les polyphénols les plus largement répandus dans le règne végétal. Ils sont également à l’origine de nombreux effets bénéfiques liés à l’alimentation notamment dans la santé cardiovasculaire. Parmi eux, l’épicatéchine, un flavonoïde de la famille des flavan-3-ols est connu pour ses propriétés antioxydantes ou anti-inflammatoires. L’hémostase, qui participe à la physiopathologie des maladies cardiométaboliques pourrait également être modulée par les flavonoïdes. La première partie de ces travaux de thèse a consisté à étudier l’impact de l’épicatéchine sur les trois principales étapes de l’hémostase que sont l’hémostase primaire, la coagulation plasmatique et la fibrinolyse, grâce à des outils globaux, afin de tenter de dégager le phénotype hémostatique induit par l’épicatéchine. Nous avons pu montrer qu’à forte concentration l’épicatéchine inhibe la fonction plaquettaire et module le profil coagulant et fibrinolytique. Dans la deuxième partie, nous avons tenté de déterminer l’effet de l’épicatéchine sur les microparticules et leurs fonctions procoagulantes. Alors que la concentration globale en microparticules n’est pas impactée par l’épicatéchine, cette dernière semble atténuer leur potentiel procoagulant lorsqu’il est détecté par la formation de thrombine. Enfin dans la troisième partie, nous avons étudié le processus final de formation du caillot de fibrine. Par des techniques de microscopie, spectrophotométrique et fonctionnelle nous avons pu déterminer que le caillot formé en présence d’épicatéchine est moins dense et plus perméable. Ces données in vitro restent encore à être confirmées in vivo afin d’objectiver l’impact de l’épicatéchine et de ses métabolites à des concentrations compatibles avec l’alimentation.

UHPLC-LTQ-Orbitrap-high resolution mass spectrometry (HRMS) was applied to investigate complex polymeric polyphenols, before and after acid-catalysed depolymerisation in the presence of a nucleophile (phloroglucinol). Reaction products of (−)-epicatechin with acetaldehyde formed in model solution were selected for a proof-of concept experiment. The complexity of the UHPLC-HRMS dataset obtained after 4 h incubation was reduced with petroleomics-inspired strategies using Van Krevelen diagrams and modified Kendrick mass defect filtering targeting ethyl-epicatechin (C17H16O6) units. Combining these approaches with mass fragmentation and phloroglucinolysis allowed us to describe reaction of epicatechin and acetaldehyde. More than 65 compounds were found, including the homogeneous bridged derivatives (up to the undecamer), vinyl and ethanol adducts, and xanthene and xanthylium salt derivatives which were identified for the first time.

The aim of this study was to characterize the potential endothelium-protective effects of epicatechin metabolites and to define underlying mechanisms of action by an integrated systems biology approach. Upon dietary epicatechin supplementation in a mouse model of inflammation, reduced leukocyte rolling over vascular endothelium was observed. Pathway enrichment analysis of transcriptome, epigenome and miRNome profiles of endothelial cells exposed to plasma concentrations of epicatechin metabolites suggested significant changes in interactions with immune cell as well as permeability regarding immune cell transmigration. In line with our in-silico network analysis, in-vitro experiments confirmed that epicatechin metabolites reduce monocyte adhesion to endothelial cells and their transendothelial migration. Altogether, our in-vivo and in-vitro results support the outcome of a systems biology based network analysis of epigenome, transcriptome and miRNome profiles, which suggests that epicatechin metabolites mediate their vasculoprotective effects through dynamic regulation of endothelial cell monocyte adhesion and permeability. This study illustrates complex and multimodal mechanisms of action by which epicatechin modulate endothelial cell integrity.

Two series of compounds showing mass signals at m/z 605 and 893 (negative ionization mode) have been detected in fermented cocoa beans. This study objective is to identify these mass signals and characterize their structure in fermented cocoa samples. Our hypothesis is that these signals may correspond to ethyl-bridged flavan-3-ols resulting from flavan-3-ol condensation with acetaldehyde which is a microbial metabolite. Mass spectrometry was used to compare the retention times and mass fragmentation patterns between a model solution using epicatechin and procyanidin dimer B2, the major flavan-3-ols of cocoa, as precursors and extracts of fermented cocoa. Their identification was confirmed: four isomers of ethyl-linked epicatechin as well as several isomers of epicatechin-ethyl-procyanidin B2, in which B2 was mostly linked through its upper unit, were characterized in cocoa. This study demonstrates the presence of flavan-3-ol acetaldehyde condensation products in fermented cocoa beans and provides the first report of epicatechin-ethyl-procyanidin B2.

Epidemiological studies showed that the consumption of flavonoid-rich foods such as cocoa and tea is associated with a lower risk of cardiovascular disease (CVD). Randomised controlled trials (RCTs) showed that cocoa and tea improved markers of cardiometabolic health including blood pressure, endothelial function, insulin resistance, arterial stiffness and inflammation. Cocoa is particularly rich in the flavan-3-ol epicatechin and tea is the main dietary source of epicatechin and of the major flavonol quercetin. However, evidence on the individual roles of epicatechin and quercetin in the health effects of cocoa and tea is still scarce. Therefore, we estimated the strength of the association between epicatechin intake and CVD mortality in a prospective cohort study. Furthermore, we also investigated the effects of epicatechin and quercetin on markers of cardiometabolic health and gene expression, by means of two RCTs. In Chapter 2, the association between epicatechin intake and CVD mortality was studied using data from the Zutphen Elderly Study, a cohort of 744 elderly Dutch men. During 25 years of follow-up, 329 men died from CVD and 148 from coronary heart disease (CHD). Results from this study showed that men in the highest tertile of epicatechin intake had a 38% lower risk of CHD mortality compared to men in the lowest tertile. For men with prevalent CVD, the risk of CVD mortality was 46% lower for men in the highest tertile of intake, compared to men in the lowest tertile. This is the first epidemiological study to have investigated the association between epicatechin intake and CVD mortality. Hence, more and larger cohort studies are required to confirm this association, possibly with a focus on populations with a high risk of CVD. In Chapter 3, the chronic effects of pure epicatechin and quercetin on markers of cardiometabolic health were investigated by means of a RCT. Thirty-seven apparently healthy men and women aged 40–80 years consumed (-)-epicatechin (100 mg/d), quercetin-3-glucoside (160 mg/d), or placebo capsules for 4 weeks, in random order. Markers of cardiometabolic health were measured before and after each 4-week intervention. The results of this study showed that epicatechin improved insulin resistance and had a borderline significant effect on endothelial function. This suggests that epicatechin contributes to the cardioprotective effects of cocoa and tea, however, larger long-term RCTs are required to confirm these effects. Pure quercetin supplementation did not affect any of these markers of cardiometabolic health. Using data from the same study, we investigated the effects of supplementation of pure epicatechin and quercetin on a comprehensive set of biomarkers of endothelial dysfunction and inflammation (Chapter 4). With the exception of sE-selectin (a biomarker of endothelial dysfunction), epicatechin supplementation did not beneficially influence any of the biomarkers, suggesting a lack of evidence for a role of epicatechin in inflammation. Quercetin also lowered sE-selectin as well as the inflammatory biomarker IL-1β and the overall z-score for inflammation. This suggests that quercetin may contribute to the cardioprotective effects of tea by reducing inflammation and possibly by improving endothelial function. In the same study, the effects of pure epicatechin supplementation on whole genome gene expression profiles of circulating immune cells were also assessed (Chapter 5). Pure epicatechin supplementation modestly reduced gene expression related to inflammation signalling routes in circulating immune cells – routes which are known to play a role in cardiovascular health. However, there was no evidence that epicatechin affected pathways related to insulin resistance or endothelial function. To directly compare the acute effects of pure epicatechin and epicatechin from dark chocolate on vascular function, we carried out an acute RCT in 20 apparently healthy men aged 40-80 years (Chapter 6). On three separate occasions, subjects consumed: 1) 70g dark chocolate (150 mg epicatechin) with two placebo capsules; 2) two pure epicatechin capsules (100 mg epicatechin) with 75g white chocolate and 3) two placebo capsules with 75g white chocolate (0 mg epicatechin). Endothelial function and arterial stiffness were measured before and two hours after each intervention. To determine epicatechin bioavailability, epicatechin metabolites were measured in blood samples taken at repeated intervals over a period of 8 hours. There was no significant difference in improvement in endothelial function or arterial stiffness between pure epicatechin and dark chocolate. There was also no difference in bioavailability of pure epicatechin and epicatechin from dark chocolate, when standardised per 100 mg of epicatechin. This suggests that epicatechin may contribute to the vascular effects of cocoa and that the bioavailability of pure epicatechin and epicatechin from dark chocolate is similar. In the general discussion, the main findings of this thesis were first summarised. Methodological considerations related to cohort studies, such as the assessment of flavonoid intake and the possibility of residual confounding were also discussed. Issues related to the relevance of cardiometabolic markers in RCTs and the effect of cocoa flavan-3-ol bioavailability were addressed. Finally, suggestions for future research were put forward. In conclusion, the results of this thesis suggest that epicatechin contributes to the cardioprotective effects of cocoa and tea. Epicatechin intake was inversely related to CHD mortality in elderly men, and to CVD mortality in men with prevalent CVD. The cardioprotective effects of epicatechin are likely mediated through improvements in insulin resistance and possibly endothelial function. In contrast, quercetin is unlikely to play a major role in the cardioprotective effects of tea. Results for quercetin from cohort studies are inconclusive, and based on the results of our chronic RCT, quercetin did not affect vascular function or insulin resistance, but may help to lower inflammation. Evidence of the role that individual flavonoids play in the aetiology of CVD is still limited. More studies with pure flavonoids are required to elucidate their role.
