3. Medicina regenerativa de cartílago: efecto del tratamiento con ácido hialurónico reticulado bifásico en lesiones osteocondrales

Objetivo: Evaluar si el tratamiento con ácido hialurónico reticulado bifásico de lesiones osteocondrales promovería la regeneración del tejido cartilaginoso, favoreciendo así la reparación de la lesión. Materiales y Métodos: Quince conejos hembra adultos fueron divididos aleatoriamente en tres grupos: grupo 1, de control; grupo 2 y grupo 3, sometidos a una estrategia quirúrgica de lesión osteocondral en la rodilla derecha (4 mm de diámetro, 5 mm de profundidad), el grupo 3 recibió tratamiento con 0,2 ml de ácido hialurónico por vía intrarticular después de la cirugía. Se realizaron controles clínicos, bioquímicos, histopatológicos y estudios por imágenes. Resultados: Se detectaron menos casos de dolor a la palpación en el grupo 3 que en el grupo 2 a partir de los 45 días. En la resonancia magnética, casi todas las muestras del grupo 3 tenían signos de regeneración del tejido cartilaginoso en el sitio de la lesión, sin edema óseo, ni derrame articular significativo. Los estudios histopatológicos de las muestras del grupo 3 indicaron un aumento de la matriz extracelular propia de tejido cartilaginoso, comparada con la del grupo 2, con hipercelularidad, dada por condrocitos, los que formaban grupos isogénicos axiales y coronales. Conclusiones: Este estudio brinda evidencias de que el tratamiento con ácido hialurónico reticulado bifásico en unidades experimentales de conejos con lesión osteocondral no tuvieron dolor en etapas tempranas después de la lesión, a diferencia de las unidades intervenidas y sin dicho tratamiento. A su vez, los estudios por imágenes e histopatológico mostraron la reparación del tejido dañado.

6. Complicaciones en aplicación estética de ácido hialurónico en el territorio facial: revisión de alcance.

Introduction: Dentists have increasingly focused on aesthetic treatments using HA, leading to a rise in associated complications and a greater need to understand its effects and management. Objective: To describe the different complications associated with the use of HA in non-surgical aesthetic procedures, as well as their management and prevention. Materials and Methods: We conducted an exploratory systematic review of existing literature on complications related to HA use in the facial area and their treatments during August 2023. PubMed, Ebsco, and Scopus were utilized for this purpose. Studies reporting complications and HA management were included, while in vitro, ex vivo, and animal studies were excluded. Results: A total of 31 articles were obtained. After screening based on titles and abstracts, 14 met the criteria for inclusion. For filler migration and lesions resembling xanthelasma, small volumes of HA are recommended. In cases of facial paralysis, hialuronidase combined with systemic or topical corticosteroids is suggested. Treatment options for late infection include laser surgery, hyperbaric oxygen therapy, drainage, broad-spectrum antibiotics, and hialuronidase, which is also used for immediate vascular complications. However, specific treatment protocols for ecchymosis, the Tyndall effect, contour irregularities, diplopia, nodules, and certain ocular complications are not well-documented in the literature. Conclusions: The most prevalent complications reported include hematoma, edema, xanthelasma-like lesions, nodules, filler migration, and erythema. Among the more complex complications, contour irregularities, ocular complications, infections, immune reactions, persistent inflammation, and vascular involvement are described. - Abstract
The temporomandibular joint (TMJ) is a complex joint, and temporomandibular dysfunction (TMD) occurs when there is na imbalance between its components, resulting in symptoms such as pain, restricted movement, and clicking or popping sounds when opening and closing the mouth. Viscosupplementation, a therapeutic technique for TMD, involves injecting hyaluronic acid into the TMJ to improve joint function and alleviate symptoms. Viscosupplementation aims to restore the biomechanics and homeostasis of the TMJ. The injected hyaluronic acid has anti-inflammatory and lubricating properties, improving joint mobility and reducing pain. While studies suggest short-term symptomatic relief, its long-term effectiveness and safety profile are still topics of debate. The concerns include rare complications. Viscosupplementation is considered in cases of symptomatic internal TMJ alterations, but the lack of conclusive evidence highlights the need for further research to elucidate its long- term efficacy and safety before being widely recommended as a treatment for TMD. 8. Impacto a corto plazo del ácido hialurónico y plasma rico en plaquetas en lesiones condrales de rodilla: un estudio de cohorte prospectivo

Objetivo: evaluar los efectos del uso de plasma rico en plaquetas (PRP), ácido hialurónico (AH), o su combinación, en la analgesia y funcionalidad en pacientes femeninas operadas por artroscopía por lesiones condrales en la rodilla. Materiales y métodos: estudio de cohorte prospectivo, participaron dieciséis pacientes femeninas con una edad promedio de 52.7 años (DE = 2.84). Las exposiciones evaluadas fueron inyección intraarticular de plasma rico en plaquetas (12.5%), ácido hialurónico (37.5%) y su combinación (50.0%). Todas las pacientes fueron seguidas durante cuatro semanas postquirúrgicas. Se evaluaron el dolor, la funcionalidad en términos de distancia caminada y capacidad para subir y bajar escaleras, y los resultados de maniobras clínicas (Appley, McMurray, cajón anterior y posterior). Resultados: se observó que el dolor promedio disminuyó significativamente de 3 (DE = 1) a 2 (DE = 1) (p = 0.015). En la escala funcional de subir y bajar escaleras, se constató una mejoría clínica significativa (p = 0.004), con un aumento del 12.5% al 62.5% en las pacientes que realizaban esta actividad sin asistencia. Las maniobras de Appley y McMurray positivas al dolor mostraron disminución, siendo significativa la segunda (p = 0.072 y p = 0.023). Conclusiones: a la fecha, la evidencia que apoya o rechaza la inyección de ortobiológicos es escasa y controversial. Los resultados de este estudio sugieren que estas intervenciones pueden ser efectivas para la rehabilitación postquirúrgica en pacientes con lesiones condrales en la rodilla, aunque se necesitan trabajos con mayores muestras y seguimiento a largo plazo para reforzar estos hallazgos.

Introduction: Penile size and circumference are extremely important issues for many men, dissatisfaction with these parameters makes many patients seek medical help every year. Method: We performed penile filling with hyaluronic acid on 52 men and described the technique applied. Results: Patients initially had 9.71 cm (+ SD 1.06) of penile circumference and after hyaluronic acid application, 12.61 cm (+ SD 1.03) of penile caliber with p<0.001 and 95% CI. Hyaluronic acid average volume was 15.55 mL (+ SD 3.31). Discussion: When performing penile enlargement with hyaluronic acid, safety must be considered. Penile enlargement with HA main limitations, are difficulties associated with injection technique and beginners' learning curve. Conclusion: Hyaluronic acid proved to be safe and effective way to perform these procedures and new studies will be necessary for long-term evaluations. [ABSTRACT FROM AUTHOR]

Introduction: Hyaluronic acid injections are a common non-surgical treatment for knee arthritis, despite a lack of clinical evidence and agreement among medical organizations regarding its efficacy. According to previous research, the role that media plays in popularizing this therapy is a topic that needs to be analyzed. Objectives: To examine the information published by the Spanish media between 2012 and 2022 on this treatment in order to identify any bias that could justify the growing interest expressed by citizens concerning this treatment. Methodology: The study examines 103 news items obtained by MyNews and coded them by 10 criteria utilised by previous studies to analyse the information relate to emerging therapies. Results: The study confirms that the information published on this treatment is on the increase throughout the examined period. It also accounts for the most relevant trends in the news items according to the established criteria. Conclusion: The study concludes that the pieces of news examined about intra-articular injections of hyaluronic acid show information gaps. Finally, the need for citizens to have accurate information about this therapy when making decisions is highlighted. [ABSTRACT FROM AUTHOR]

Introduction: knee osteoarthritis (KOA) is known as the most common form of osteoarthrosis with a 6% prevalence in people over 30 years old, and more than 40% in the population over 70 years old. The use of PRP led to diverse results and this disparity can be attributed to the dissimilar methods of PRP preparation. This study aims to assess the functional effects of intraosseous (IO) and intraarticular (IA) injections of platelet rich plasma (PRP) followed by IA injections of hyaluronic acid (HA). Objectives: this study aimed to assess the functional effects of intraosseous (IO) and intraarticular (IA) injections of platelet rich plasma (PRP) followed by IA injections of hyaluronic acid (HA), administered 3 and 4 weeks after the initiation of treatment in 33 patients with grade II-III (Ahlback scale) knee osteoarthritis (KOA). Material and methods: retrospectively, 33 patients were assessed using the Western Ontario and McMaster Universities (WOMAC) osteoarthritis index and visual analogue scale (VAS) score. They were followed-up for 12.92 months on average. Patients were divided into three groups based on age and four groups based on the follow-up period. Results: the pre-operative mean of the WOMAC index was 44.35 ± 20.20 and the post-operative mean was 22.81 ± 17.25 (p < 0.001). The pre-operative and post-operative mean of the VAS scores were 5.79 ± 2.01 and 2.41 ± 1.43 (p < 0.001), respectively. The largest improvement in WOMAC (from 42.86 to 13.69) was observed in the youngest patients (44 to 55 years old) and the largest reduction in VAS (from 6.89 to 2.22) was seen in patients aged 56 to 70 years. Conclusion: the combination of IO and IA plasma rich in growth factor (PRGF) treatment with the IA-HA treatment yielded excellent results, diminishing pain and improving motor functionality in patients with KOA. Pseudoseptic arthritis is a rare complication of hyaluronic acid injections that often is difficult to differentiate from septic arthritis. Patients present acute pain, swelling and joint effusion normally around 24 h after the second or third infiltration. We describe a female patient with seropositive rheumatoid arthritis and flare-ups of knee arthritis with pseudoseptic features in the past, who develops pseudoseptic arthritis of the knee following her first injection of hyaluronic acid. Objetivo: Descrever os efeitos adversos, intercorrências e complicações do preenchimento com ácido hialurônico na região de nariz, indicando possíveis causas para os problemas apresentados após o procedimento. Metodologia: Trata-se de uma revisão integrativa realizada nas seguintes bases de dados: Medline via PubMed, SciELO e Lilacs, com artigos incluídos no período 2013 a 2023, utilizando os descritores, Ácido hialurônico; Necrose; Preenchedores dérmicos; Nariz. A busca integrada foi realizada unindo os descritores com o operador booleano “AND”. Foram analisadas 9 publicações. Resultados: Foi realizada a identificação dos trabalhos selecionados, elencando-se os dados relacionados à título, autor, ano de publicação, tipo de estudo, metodologia, principais intercorrências encontradas e a conclusão do estudo. Após análise dos estudos, observou-se que a ocorrência de complicações após a aplicação de ácido hialurônico não é frequente. Para que não ocorra o agravamento, é necessário que os primeiros sinais e sintomas sejam analisados para assim seguir com o protocolo mais adequado a fim de prevenir ou reverter. Conclusão: Seguindo passos simples de segurança e tendo um conhecimento anatômico refinado, os preenchedores podem ser uma boa ferramenta para um aprimoramento seguro e abrangente da rinomodelação.

Of all the diseases of the musculoskeletal system, osteoarthritis (OA) is the most common arthritis, leading to enormous costs to society and great morbidity for sufferers. It is characterized by involvement of the entire joint, including cartilage degradation, bone remodeling, osteophyte formation and inflammation of the synovial membrane. This results in pain, stiffness, swelling and loss of joint function with marked functional limitation. Management of this entity involves non-pharmacological therapy, the use of drugs for pain relief and surgery when previous measures have failed. Non-pharmacological measures include education, muscle strengthening exercises, physical therapy, weight reduction and orthoses or devices to assist ambulation. As for recommended drugs, non-steroidal anti-inflammatory drugs, both topical and oral, and intra-articular glucocorticoids are recommended. Finally, partial meniscectomy by arthroscopy has only been recommended when there is knee locking. In individuals suffering from excruciating pain or severe functional limitation due to advanced disease, total joint replacement should be considered. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: The application of Hyaluronic Acid is an alternative treatment for facial aging and has been used for over a decade to fill soft tissues, correct depressions, wrinkles, and furrows. Although considered safe, hyaluronic acid facial fillers are not without risks or side effects. Objective: To evaluate the main side effects and complications associated with the use of HA in facial fillers and emphasize the approaches used in the treatment of these complications. Method: An integrative literature review was carried out with articles published between 2013 and 2023, in Portuguese and English. Bibliographic searches were carried out in the PUBMED/MEDLINE, SCIELO and LILACS databases. Development: The adverse effects of using HA are divided into immediate and late and can range from edema, mild pain, severe pain, ecchymosis, ischemia, mild erythema to necrosis. However, most complications arising from the use of HA are related to the inexperience of the applicator, incorrect technique or are inherent to the product itself. Conclusion: The use of HA is safe, and the complications and side effects associated with its use in most cases are not serious, they can be avoided, in addition to being reversible. 23. Arthrodilation: a treatment algorithm as a conservative management option for adhesive capsulitis.

Introduction: the management of adhesive capsulitis (AC) remains a topic of debate among orthopedic surgeons, with a wide variation in the literature. Conservative treatment relies as the first-line option as clinical studies report positive outcomes. However, there is variability in the effectiveness of different treatment modalities. Material and methods: this study aimed to analyzed functional and clinical outcomes of patients with AC who underwent the arthrodilation protocol, including three ultrasound-guided injections administered on a weekly basis: two corticosteroid injections and one injection of hyaluronic acid combined with corticosteroids. Additionally, patients received a specific rehabilitation therapy. Visual analogue scale (VAS), the university of California-Los Angeles shoulder score (UCLA) and Constant-Murley score were assessed before treatment and after 3-month followup period. Results: 23 patients were included, receiving the same treatment protocol with a mean onset of symptoms of 4.9 ± 1.7 months. Among these patients, there was a clear predominance of females (65.2%). Age distribution ranged from 39 to 74 years (mean = 56) indicating that individuals in their mid-50s were more susceptible to developing this condition. Furthermore, a slight majority (52.2%) exhibited AC in their right shoulder. VAS significantly decreased (-6.09 ± 1.9 [p ≤ 0.05]). Similarly, UCLA score (10.9 ± 2.9 to 31.7 ± 2.2) and Constant-Murley score (22.3 ± 6.1 to 62.0 ± 6.2) improved significantly. Pre-to-post treatment evaluation showed improvement in both UCLA (mean = 20.8 ± 2.9 [p ≤ 0.05]) and Constant-Murley (mean = 39.7 ± 9 [p ≤ 0.05]). Conclusion: arthrodilation protocol demonstrated promising results, with patients achieving good to excellent outcomes and safely resuming their regular daily activities within a short-term follow-up period. These findings provide support for arthrodilation as a viable conservative management option and contribute valuable insights to the ongoing research aimed at identifying optimal treatment approaches for adhesive capsulitis. Introducción: El ácido hialurónico se ha convertido en el tratamiento preferido y mejor aceptado por los pacientes para corregir imperfecciones en el rostro, o armonizar el mismo. Son múltiples efectos adversos descritos en su aplicación como tratamiento de relleno. Con la ecografía como prueba diagnóstica se logra caracterizar el tipo de sustancia empleada. En nuestro caso se llevó a cabo una investigación donde el objetivo principal fue identificar los cambios ecográficos con el uso de hialuronidasa en una paciente con complicaciones tardías post aplicación de ácido hialurónico como material de relleno. Materiales y métodos: El diseño de la investigación es observacional, prospectivo, realizado en base a un caso clínico. Discusión: Si bien es cierto que el ácido hilaurónico es bien tolerado por un gran número de pacientes, aun no se ha determinado el material de relleno ideal. Con el uso de la ecografía se caracterizan los signos sugestivos del material de relleno administrado y su evolución en el tiempo. Las complicaciones van desde el dolor, deformidad hasta la necrosis, por lo que la hialuronidasa ha cobrado un papel preponderante en el tratamiento de dichas complicaciones. Conclusión: La ecografía como método diagnóstico en la caracterización de las sustancias de relleno constituye una herramienta esencial para caracterizar la sustancia aplicada, sus posibles complicaciones, así como el empleo de la hialuronidasa en la corrección de estos efectos no deseados por el uso de ácido hialurónico, apreciando los cambios a nivel de los tejidos tratados.

Introducción: la enfermedad de ojo seco (EOS) es una patología común y un motivo de consulta frecuente en farmacia comunitaria. Esta tiene un gran impacto en la calidad de vida (QoL) de los pacientes; por ello, un tratamiento adecuado debería mejorarla. Las lágrimas artificiales constituyen la base para el manejo de la EOS y el principal tratamiento de indicación farmacéutica. Objetivo: estudiar el efecto de unas gotas oculares a base de ácido hialurónico, Centella asiática y Aloe vera en la QoL de pacientes con EOS. Paralelamente, la adherencia al tratamiento, la tolerancia y seguridad del producto, fueron evaluados. Metodología: estudio clínico posautorización, abierto, prospectivo y multicéntrico. Los pacientes recibieron el tratamiento durante 60 días. Para estudiar el efecto sobre la QoL, los pacientes cumplimentaron el cuestionario OSDI© antes de iniciar el tratamiento y a los 30 y 60 días de tratamiento. La adherencia se evaluó mediante la reconciliación de los envases y la seguridad mediante un seguimiento de los eventos adversos. Resultados: los pacientes tratados con las gotas oculares, población por intención de tratar (ITT, n=44), obtuvieron mejoras clínicamente importantes en la QoL, sintomatología y función visual, pues la puntuación OSDI© disminuyó de 32,80 (DE=23,00) (basal, EOS severo) a 12.64 (DE=15,32) (estado normal, p

El ácido hialurónico es un polisacárido de la familia de los glicosaminoglicanos naturales no sulfatado, que puede provocar diferentes respuestas en el organismo; el de alto peso molecular presenta efectos antiinflamatorios, inmunosupresores y cicatrizantes; mientras que el de bajo peso molecular presenta propiedades proinflamatorias. Se realiza esta revisión para recopilar información sobre las propiedades de ácido hialurónico y su empleo como coadyuvante en la terapia de gingivitis, periodontitis crónica y la cicatrización periodontal para constatar su efecto terapéutico y sus beneficios en el campo odontológico. Se realizó una búsqueda de acuerdo a las directrices PRISMA en las bases de datos Pubmed, Cochrane Library, Mendeley y Google Scholar incluyendo un total de 22 estudios, tres estuvieron relacionados con la gingivitis, catorce referentes a periodontitis crónica y cinco relacionados a la cicatrización periodontal. Las aplicaciones de ácido hialurónico ya sea en gel o en espray, tienen un resultado efectivo en el empleo para el tratamiento de enfermedades periodontales con una significancia alta, baja o media tanto en los parámetros clínicos y microbiológicos.

Introdução: A perda de volume é a principal causa da aparência de envelhecimento. Assim, a demanda por materiais de preenchimento que sejam seguros, duradouros e biocompatíveis tem crescido, com ênfase na lipoenxertia. Porém, algumas questões relativas à padronização ainda não estão claras, como áreas a serem injetadas e volumes. Assim, busca-se analisar os resultados de uma série de casos e sistematizar os volumes de preenchimento a serem utilizados, bem como correlacionar a linguagem usada na lipoenxertia facial com a do MD Codes®. Métodos: Revisão dos prontuários de pacientes que realizaram lipoenxertia facial. A seleção da área doadora foi procedida com base em critérios de conveniência, como facilidade de posicionamento, abundância de material e o contorno corporal do paciente. A colheita foi por aspiração manual. A preparação foi feita pela técnica de Coleman. Áreas de aplicação: para traçar um paralelo entre os conhecimentos adquiridos com a lipoenxertia facial e o preenchimento com material sintético, foi utilizada a nomenclatura utilizada nos MD Codes®. Resultados: Foram incluídos 54 lipoenxertos faciais (11% homens / 88,88% mulheres). Idade média de 43,94 anos (20 a 71, desvio padrão 11,21). O tempo médio de acompanhamento foi 155,61 dias (variação de 7-543, desvio padrão 156,05). Não houve complicações relacionadas ao método. O volume médio injetado foi de 29,83ml (intervalo 6-53,9, desvio padrão 12,07). Conclusão: A lipoenxertia autóloga é um procedimento factível para alguns casos de rejuvenescimento facial. A linguagem MD Codes® pode ser usada em paralelo com a descrição anatômica das regiões injetadas neste estudo.

Este estudo explorou a associação do barbatimão (Stryphnodendron adstringens) com ácido hialurônico para o rejuvenescimento facial. Em indivíduos entre 40 a 70 anos, o estudo de caso adotou análises qualitativas e quantitativas, utilizando fotografias pré e pós-tratamento e avaliações individuais de satisfação. Notavelmente, além da eficácia prevista na mitigação de rugas e linhas de expressão, observou-se um rejuvenescimento cutâneo significativo, bem como melhorias na cor, textura e firmeza da pele. Esses resultados reforçam a promessa desta combinação para o tratamento de diversas condições dermatológicas.

Introduction and objective: Vocal cord paralysis is a prevalent and disabling pathology. The main objective is to compare the recovery of movility and functionality of vocal cords, as well as the requirement of definitive surgery in two groups of patients: non-infiltrated group (NIG) versus early infiltration group with hyaluronic acid (EIG). Method: Retrospective observational study. To set up the NIG, we sorted the database of 715 videos labeled as “laryngeal paralysis” and selected n=33 patients who met the inclusion criteria of the study. To create the EIG, we selected patients injected with hyaluronic acid in the last 3 years (n=22). In the NIG, the following were analyzed as outcome measures: recovery of mobility and functionality of the cord and the need for thyroplasty. In the EIG, the date of infiltration was also measured. Results: The results of the EIG are significantly better (p=0,001) in terms of vocal cord mobility recovery. A non-significant reduction (p=0.14) was observed in the need for thyroplasty in injected patients. There are significant differences between patients with unilateral paralysis of the NIG vs EIG (p=0.009), while the bilateral groups are not comparable, due to the compromise of the airway suffered by patients with the cord in a medial position. Discussion: Early infiltration with hyaluronic acid provides temporary treatment of the glottic closure defect with a low rate of complications, prolonging the maximum phonation time and improving the vocal movement, which may reduce the need for subsequent treatments. It has been proposed as a treatment that can improve the patient’s quality of life. However, it is important to properly select patients, since not all may be candidates. Conclusions: Early infiltration with hyaluronic acid generates a significant improvement in the recovery of vocal cords mobility, supporting the early indication of this therapy. Dry Eye Disease (DED) is a pathology that can be caused by genetic and environmental factors. It is characterized by the loss of homeostasis of the tear film, triggering symptoms such as pain, burning, redness, blurred vision, tearing, and photophobia. It is emphasized that the treatments are aimed at restoring the ocular surface and reducing tear film instability. Hyaluronic acid (HA) has emerged as a promising treatment for this disease. The aim of this study was to seek rationale for this condition and to evaluate the efficacy of the use of HA in its treatment. The methodology used was a literature search based on the descriptor: "{(Hyaluronic Acid)} AND {(Dry Eye)}" in the PubMed and Science Direct databases. Of the 4652 articles selected, 38 that related DED and HA. In this perspective, there are several mechanisms that are part of the pathophysiology of DED, which run with the loss of tear film homeostasis. Thus, HA, with its various applicability, has become a therapeutic option due to its different modes of application and its very few side effects. Skin aging is influenced by intrinsic and extrinsic factors and involves multiple pathogenic mechanisms. The most widely used treatments are topical products and minimally invasive procedures. Evidence on the benefits of systemic therapy is limited for several reasons: reliance on mostly small and predominantly female samples, short study durations, methodologic heterogeneity, and a lack of consensus on which outcome measures are clinically relevant. Furthermore, systemic drugs and oral supplements are not without adverse effects. Oral hydrolyzed collagen and oral hyaluronic acid are well tolerated, and numerous clinical trials show they can mitigate some signs of skin aging. Low-dose oral isotretinoin is another option, but it has a higher risk of adverse effects. Evidence is lacking on the effects of the many dietary supplements on offer, such as vitamins, flavonoids, plant extracts, and trace elements. The future of skin aging management would appear to lie in the use of senolytic and senomorphic agents targeting senescent cells in the skin. Resumen: El envejecimiento cutáneo está influido por factores intrínsecos y extrínsecos y múltiples mecanismos patogénicos están involucrados. Los tratamientos utilizados en la actualidad son sobre todo tópicos o son procedimientos mínimamente invasivos. La evidencia sobre la utilidad de la terapia sistémica es limitada: los estudios son en su mayoría de pequeño tamaño, de reducida duración, incluyen a mujeres de manera mayoritaria, la metodología de evaluación es heterogénea y no hay parámetros consensuados de respuesta clínica relevante. Además, los suplementos o fármacos sistémicos no están exentos de efectos adversos. El colágeno hidrolizado oral y el ácido hialurónico oral son bien tolerados y múltiples ensayos clínicos muestran que pueden mitigar algunos signos de envejecimiento cutáneo. La isotretinoína oral en dosis bajas es otra alternativa, pero con un mayor potencial de efectos adversos. Múltiples suplementos, como vitaminas, flavonoides, diversos extractos de plantas y oligoelementos presentan escasa evidencia clínica. El futuro del manejo del envejecimiento cutáneo parece ser el tratamiento con agentes senolíticos o senomórficos dirigidos específicamente contra células cutáneas senescentes.

- Abstract
Introduction: gonarthrosis is a degenerative disease, whose loss of cartilage causes changes in the adjacent bone and its response produces deformations and attempts at regeneration. In 1934 Meyer and Palmer isolated bovine vitreous humor, a polysaccharide called hyaluronic acid, which has the property of visco-elasticity. Today, multiple procedures performed in orthopedics are known, in order to limit the degenerative process that this entails. Objective: to know the use and efficacy of hyaluronic acid in the patient requiring knee arthroscopy, evaluating the evolution of the patient using the WOMAC scale; as well as the ideal time of application of hyaluronic acid in this study group. Material and methods: clinical, non-randomized, prospective, analytical trial. 48 patients from 45 to 60 years of age, divided into 3 groups with a diagnosis of gonarthrosis grade II-III, who underwent arthroscopy with subsequent hyaluronic acid application, were evaluated; in the postoperative period (group 1), one month after the postoperative period (group 2) and without application (group 3) in a period from September 2015 to June 2016 at the ISSSTE Hospital «Dr. Darío Fernández Fierro». Results: the treatment showed differences in patients regarding reduction of pain and stiffness; and improvement in functional capacity. Conclusion: despite, there are no conclusive results if the treatment is better than the other, certain data suggests that using hyaluronic acid in the postoperative improves rigidity and function, however, it was not statistically significant. Introduction: rhinomodeling fillers represent an important innovation niche in aesthetic medicine. Among the main substances, hyaluronic acid (HA), polymethylmethacrylate (PMMA) and calcium hydroxyapatite (CHA) stand out. Recently, technological advances have allowed the development of dermal fillers with less immunogenicity; however, their use is not free of adverse effects. Objective: To analyze the advances in the last 10 years in terms of research studies related to fillers in rhinomodeling. Material and methods: A bibliographic review was carried out consulting Medline, Web of Science, Google Scholar, LILACS, ProQuest, PLOS ONE, Embase, Ovid, ScienceDirect, Pascal and Francis, Cochrane and Trip Database. A combination of Medical Subject Headings (MeSH terms), descriptors and free terms was used. The terms, in English, included: nonsurgical rhinoplasty, nose, hyaluronic acid, and fillers. In Spanish, the following were included, among others: "rinomodelación", "nariz", "rellenos dérmicos" and "estudio de caso". Results: 36 articles were included. The majority corresponded to case reports (58.3%), followed by prospective studies (22.2%). Articles from South Korea, China and the U.S. predominate. The most frequently described filler was HA and the main thematic cluster corresponded to "complications". Conclusions: Complications, especially ophthalmic, represent an element of great importance within the area of rhinomodelation. The aesthetic medicine specialist must have extensive training and deep knowledge of nasal anatomy. Likewise, it is relevant to establish consensus with regard to the methods and times for evaluating patient satisfaction with both the procedure and the filling. [ABSTRACT FROM AUTHOR]

Hyaluronic acid is a natural non-sulfated polysaccharide of the glycosaminoglycan family, it can cause different responses in the body, the high molecular weight has anti-inflammatory, immunosuppressive and healing effects, while the low molecular weight has proinflammatory properties. Perform a synthesis of information on the properties of hyaluronic acid and its use as an adjuvant in the therapy of gingivitis, chronic periodontitis and periodontal healing to verify its therapeutic effect and benefits in the field of dentistry. A search was carried out according to the PRISMA guidelines in the Pubmed, Cochrane Library, Mendeley and Google Scholar databases, including a total of 22 studies, three related to gingivitis, fourteen related to chronic periodontitis and five related to periodontal healing. Hyaluronic acid applications, either in gel or spray, have an effective result in the use for the treatment of periodontal diseases with a high, low or medium significance both in clinical and microbiological parameters. - Abstract
A 49-year-old female with no pre-morbidities comes in with sudden, painless loss of vision in the left eye (OS) after a facial hyaluronic acid (HA) injection for aesthetic purposes one hour ago. Visual acuity was no light perception (NLP). OS examination revealed a mid-non-reactive mydriasis and complete ophthalmoplegia. Fundus examination of the OS suggested central retinal artery occlusion (CRAO). Systemic evaluation was normal. Given this diagnosis, ocular massage, anterior chamber paracentesis, anticoagulation, systemic antibiotherapy, and subcutaneous hyaluronidase around the injection sites and peribulbar region were performed. Despite these interventions, the patient did not regain vision, exhibiting signs of anterior and posterior ocular ischemia. Subsequently, she developed phthisis bulbi, necessitating evisceration. Introducción: el presente trabajo tuvo como objetivo de comparar y analizar la eficacia de las terapias alternativas en condromalacia patelar (terapia con plasma rico en plaquetas, endoret y ácido hialurónico) y determinar si tendremos un tratamiento óptimo para esta patología y su impacto en la calidad de vida de los pacientes. Materiales y métodos: estudio cuasi-experimental, de tipo prospectivo no contemporáneo, longitudinal realizado en el Centro Médico Docente la Trinidad. desde abril 2022 a octubre 2022. Resultados: después de la colocación de la terapia alternativa en el cartílago hialino de la rodilla de los pacientes, se evidenció que, a los que se les administró Endored® (plasma rico en factores de crecimiento (PRGF)) + ácido hialurónico, obtuvieron un promedio de 84.49% en la escala KOOS, con una desviación estándar de más o menos 10.43%, mientras que, los pacientes que recibieron plasma rico en plaquetas (PRP) + ácido hialurónico, obtuvieron un promedio de 86,02% en la escala KOOS con desviación estándar de más o menos 9,06%. Conclusión: en ambos grupos no hubo una mejoría de la calidad de vida del 100% en el total de los pacientes de la muestra, según los resultados en la escala KOOS.

42. A utilização de fitoterápicos no tratamento de feridas diabéticas: Relato de caso

Objetivo: O presente estudo tem como finalidade avaliar a efetividade do ácido hialurônico associado à papaína na cicatrização de feridas em pacientes diabéticos. Método: Trata-se de um estudo descritivo do tipo estudo de caso. Este estudo foi realizado no Município de Duque de Caxias, no período de 17 de junho a 20 de julho de 2022. A coleta de dados foi realizada por meio de visitas domiciliares com anotações diárias da evolução do caso, e acompanhamento fotográfico da evolução da lesão. O paciente assinou o termo de consentimento livre esclarecido, conforme determinação 196/96 do MS. A avaliação da ferida foi realizada pelo enfermeiro responsável pelo caso, e os curativos eram realizados periodicamente pela equipe de enfermagem. Resultados: Paciente do sexo masculino, 51 anos, com Diabetes Mellitus, insulinodependente, sem aderir adequadamente ao tratamento; apresentando lesão crônica, decorrente de amputação em Pé esquerdo, a qual ocorreu em setembro de 2019, estando desde então com curso desfavorável. Considerações finais: foi constatado que após o início do uso de fitoterápicos, houve a aceleração do processo de cicatrização, e que a ação destes, foi de fundamental eficácia para a completa cicatrização da ferida, em um espaço bem curto de tempo. E também é muito importante levarmos em conta o baixo custo dos fitoterápicos em relação aos demais medicamentos, e a melhoria da qualidade de vida e de autoestima do paciente com o desfecho do caso.

Background and objective. The male demand for aesthetic treatment is increasingly relevant. However, there is still little evidence on Aesthetic Medicine in men and although some studies include recommendations on treatment guidelines with toxins and fillers for male patients, not all are valid in our Mediterranean setting. For this reason, the MAPA (Male Aesthetic Patient Approach) consensus has been drawn up, a practical orientation guide to help the aesthetic doctor who is beginning to treat male patients. Methods. A panel of 9 experts in Aesthetic Medicine from different areas of Spain with extensive experience in the use of Abobotulinum toxin and hyaluronic acid fillers met to share their knowledge and usual clinical practices, and develop a series of fundamental recommendations in the field of male Aesthetic Medicine. Results. The topics discussed included questions related to clinical practice, profile of patients treated, treatment planning based on the desired end goal, and main Aesthetic Medicine techniques recommended for men. Finally, a series of recommendations on treatments to be applied in each area of facial treatment were also discussed. Conclusions. This consensus has collected the opinions, advice and experience of aesthetic doctors with extensive experience in the use of Abobotulinum toxin and hyaluronic acid fillers to help professionals achieve the best possible results in terms of efficacy and satisfaction in male patients. Objetivo: verificar o conhecimento da população de um município norte mineiro sobre harmonização orofacial. Materiais e Métodos: trata-se de um estudo de caráter descritivo, quantitativo e transversal. A população foi constituída por moradores da cidade de Montes Claros (MG). A amostra foi composta por 193 pessoas com idade entre 18 e 65 anos. Para a coleta de dados, foi utilizado um formulário criado no Google Formulários, distribuído pela técnica de snowball. Resultados: constatou-se que a maioria da população conhece os procedimentos de harmonização orofacial e que são realizados por cirurgiões-dentistas. As mulheres na faixa etária de 18 a 35 anos são predominantes na busca por esses procedimentos. Entre eles, os mais conhecidos são preenchimento com ácido hialurônico, bichectomia e preenchimento labial. Conclusão: a população conhece os procedimentos de harmonização orofacial e considera os cirurgiões-dentistas aptos a realizá-los.

45. Mucinose cutânea em um cão da raça Shar pei: Relato de caso

A mucinose cutânea é uma dermatopatia idiopática primária que ocorre em cães da raça shar pei, caracterizada pela excessiva deposição de mucina na derme que leva a apresentação de dobras cutâneas, e em casos mais graves a formação de vesículas ou bolhas. O ácido hialurônico foi identificado como o principal componente da mucina, contudo a etiopatogenia, prevenção e tratamento desta enfermidade ainda não foram totalmente elucidados. Este trabalho relata um cão da raça shar pei com 3 anos de idade, diagnosticado com mucinose cutânea. Ao exame físico foram observadas múltiplas vesículas contendo fluido transparente e viscoso e hipotricose em região ventral do pescoço, axilas, flanco bilateral, perianal e na porção distal dos membros. Nos membros pélvicos havia também a presença de edema. A pele acometida apresentava-se inchada, espessada e negativa ao sinal de Godet. Foram realizados exame de sangue, raspado e citologia de pele. Os exames de sangue não apresentaram alterações significativas, assim como o raspado e a citologia de pele apresentaram resultados negativos. Mediante estes resultados optou-se por realizar biópsia de pele e análise histopatológica. Foi observado edema mixomatoso intenso e difuso, principalmente na região mais superficial e perianexal, com formação de lagos de mucina, confirmando o diagnóstico. Para o tratamento foi prescrito prednisolona na dose de 1 mg/kg a cada 24 horas, durante 7 dias, reduzindo a dose até a suspensão do medicamento em 30 dias. Após 21 dias de tratamento o paciente apresentou resolução total das lesões e do prurido. Foi possível concluir que a mucinose cutânea é uma dermatopatia exclusiva de cães da raça shar pei devido à intensa seleção genética ao longo da história da raça. O impacto da alta síntese do ácido hialurônico no fenótipo destes cães é apenas parcialmente conhecido. Deste modo, muitos outros locais e efeitos sistêmicos do ácido hialurônico ainda precisam ser compreendidos já que este glicosaminoglicano é sintetizado por diversas células e pode ser encontrado em vários tecidos e fluidos corporais com concentrações variadas.

A study was carried out whose objective was to carry out a review of the clinical literature to compare the utility of hyaluronic acid versus calcium hydroxyapatite in facial volume replacement therapy. A search was carried out in the databases: Sciencedirect, Springer, and Wiley Science with the keywords: "hyaluronic acid facial", "hyaluronic acid filler", obtaining 854 results from clinical studies. Of which were evaluated by a group of experts, reducing the number of articles to 126, later only those studies that had full text access were collected, obtaining a total of 17 studies included in the hyaluronic acid section. For the consultation of studies of calcium hydroxyapatite, the same databases as the hyaluronic acid section were carried out with the keywords: "calcium hydroxylapatite facial", "calcium hydroxylapatite filler", obtaining 154 articles, of which were Selected by the group of experts 68 articles, of which 11 studies were selected to which full text was accessed. For qualitative analysis, the extraction domains included: main author, year of publication, number of patients, assessment method, patient satisfaction, clinical results. We concluded that the use of hyaluronic acid is slightly superior in terms of clinical results of facial volume replacement therapy., Se llevó a cabo un estudio cuyo objetivo fue realizar una revisión de la literatura clínica para comparar la utilidad del ácido hialurónico frente al hidroxiapatito de calcio en la terapia de reemplazo de volumen facial. Se realizó una búsqueda en las bases de datos: Sciencedirect, Springer y Wiley Science con las palabras clave: "ácido hialurónico facial", "relleno de ácido hialurónico", obteniendo 854 resultados de estudios clínicos. De estos, fueron evaluados por un grupo de expertos, reduciendo el número de artículos a 126, posteriormente se recopilaron solo aquellos estudios que tenían acceso al texto completo, obteniendo un total de 17 estudios incluidos en la sección de ácido hialurónico. Para la consulta de estudios de hidroxiapatito de calcio, se realizaron las mismas bases de datos que en la sección de ácido hialurónico con las palabras clave: "hidroxiapatito de calcio facial", "relleno de hidroxiapatito de calcio", obteniendo 154 artículos, de los cuales fueron seleccionados por el grupo de expertos 68 artículos, de los cuales se accedió al texto completo de 11 estudios. Para el análisis cualitativo, los dominios de extracción incluyeron: autor principal, año de publicación, número de pacientes, método de evaluación, satisfacción del paciente, resultados clínicos. Concluimos que el uso del ácido hialurónico es ligeramente superior en términos de resultados clínicos en la terapia de reemplazo de volumen facial.

The search for a beautiful smile has been increasing, not only for beautiful teeth, but also for soft lips. Dry lips might occur due to internal and external factors, such as cold, wind, sun, saliva, acids, pollution, medicine, and even the low consumption of water. Thus, lip hydration becomes necessary. For this study, microneedling associated with hyaluronic acid was selected among the available techniques on the market. This qualitative bibliographical descriptive research aims to analyze the benefits of microneedling with hyaluronic acid for lip hydration. Results demonstrate that microneedling is an aesthetic procedure consisting of microlesions perforating and breaking the skin barrier, causing an inflammatory process and stimulating collagen production. In addition, microperforations open channels that significantly increase the permeability of active compounds, such as hyaluronic acid, a hygroscopic molecule with a high hydration capacity. Its association with microneedling makes it more effective, nurturing a better result in dry lips. Thus, it is possible to conclude that the association of microneedling and hyaluronic acid benefits lip hydration. [ABSTRACT FROM AUTHOR]

A obstrução arterial induzida por materiais de preenchimento pode simular outras condições dermatológicas. Nesta série de casos, os autores descrevem quatro pacientes com obstrução arterial induzida por preenchimento, os quais se apresentaram como erupção cutânea pustulosa que foi diagnosticada erroneamente como infecção herpética. Como o diagnóstico correto e precoce é crucial para a prevenção de cicatrizes, a erupção cutânea pustulosa deve ser reconhecida como uma expressão clínica da obstrução vascular induzida pela injeção cutânea.
