Purpose: There is some debate over the necessity of ureteral stenting after laparoscopic ureterolithotomy. We evaluated the need for ureteral stenting after retroperitoneal laparoscopic ureterolithotomy (RLU). Materials and Methods: Between January 2009 and January 2013, 41 patients underwent RLU to remove upper ureteral stones. The retroperitoneal approach was used in all patients by a single surgeon. A double J (D-J) stent was placed in the first 17 patients after the procedure but not in any of the next 24 patients. Results: The mean patient age, serum creatinine levels, and stone size were not significantly different between the two groups. The stone-free rate was 100%. The mean operative time was significantly shorter in the stentless group than in the stent group (59.48 minutes vs. 77.88 minutes, p<0.001). Parenteral analgesic use and anticholinergic medication use were observed in the stent group only. The blood loss, drain removal day, and hospital stay were not significantly different between the two groups. No other significant complications occurred during or after the operation in any patients. Conclusions: RLU is a safe and effective treatment modality for large impacted ureteral stones. In this study, D-J stent placement was not necessary after RLU. In the future, large-scale studies of RLU without D-J stenting, especially on the frequency of the development of complications according to the surgical technique, may be needed. Purpose: The effects of an anticholinergic or antidiuretic agent as add-on therapy to an alpha- blocker for lower urinary tract symptoms (LUTS) according to a voiding diary in 3 days are unknown. We prospectively investigated the efficacy of an anticholinergic or antidiuretic agent as add-on therapy for nocturia in men previously treated with an alpha-blocker for LUTS. Subjects and methods: Patients were randomly subdivided into two groups. All patients had a 4-week washout. Group A had alpha-blocker for 4 weeks, then an alpha-blocker plus an anticholinergic agent for 4 weeks, and, finally, 4 weeks of an alpha-blocker plus an antidiuretic agent. Group B had an alpha-blocker for 4 weeks, then an alpha-blocker plus an antidiuretic agent for 4 weeks, and, finally, 4 weeks of an alpha-blocker plus an anticholinergic agent. In both groups, patients were subdivided into nocturnal polyuria, decreased nocturnal bladder capacity (NBC), or nocturia by both causes subgroups. A 3-day voiding diary, total International Prostate Symptom Score (IPSS), IPSS sub-scores, Overactive Bladder Symptom Score, uroflowmetry, and post-void residual urine volume, were assessed at baseline, and at 4, 8, and 12 weeks. Results: A total of 405 patients completed the study. During treatment, the changes from baseline in total IPSS and IPSS sub-scores were significantly decreased at 4 weeks and were maintained for 12 weeks. In the nocturnal polyuria subgroup of Groups A and B, the number of episodes of nocturia in 3 days, nocturnal urine volume, and nocturnal index were significantly decreased using an alpha-blocker plus an antidiuretic agent. In the decreased NBC subgroup of Groups A and B, IPSS storage sub-score, Overactive Bladder Symptom Score, number of episodes of nocturia in 3 days, number of episodes of urgency in 3 days, and NBC index were all significantly decreased using an alpha-blocker plus an anticholinergic agent. Conclusion: An anticholinergic agent or antidiuretic agent as an add-on therapy in men previously treated with an alpha-blocker improves nocturia including LUTS. [ABSTRACT FROM AUTHOR]

Purpose: Prostate cancer is rare in men younger than 50 years. Digital rectal examination (DRE) and measurement of prostate-specific antigen (PSA) concentrations are standard screening methods for detecting prostate cancer. We retrospectively investigated the risks and benefits of repeated transrectal ultrasonography-guided prostate needle biopsies in relation to the follow-up status of men younger than 50 years with a consistently high PSA concentration (>3.0 ng/mL). Materials and Methods: During the period from January 2000 through February 2013, we reviewed patient's ages, dates of procedures, DRE results, frequencies of biopsies, results of the biopsies, periods of follow-up, PSA concentrations, and prostate volumes in Chonbuk National University Hospital records. We conducted telephone interviews in patients who did not undergo regular follow-up. Results: The mean age of the patients was 44.7 years, and the mean PSA concentration was 8.59 ng/mL (range, 3.04-131 ng/mL) before biopsy. The PSA concentration was significantly different (p<0.001) between the patients with prostate cancer and those with benign prostatic hyperplasia (BPH). Nineteen patients underwent repeated prostate biopsy; however, in only one patient did the pathologic findings indicate a change from BPH to prostate cancer. We identified several complications after transrectal biopsy through an evaluation of follow-up data. Conclusions: All patients with benign prostatic disease based on their first biopsy were shown to have benign disease based on all repeated biopsies (15.83%), except for one patient; however, several complications were noted after biopsy. Therefore, the risks and benefits of repeated biopsy in young patients should be considered because of the low rate of change from benign to malignant disease despite continuously high PSA concentrations (>3.0 ng/mL). A design method of an ultra-wideband transition from double-sided parallel stripline (DSPSL) to coplanar waveguide (CPW) is proposed based on analytical expressions of characteristic impedance. The conformal mapping is used to obtain the characteristic impedance for each section of the transition within 3.7% accuracy as compared with the EM simulation results. An efficient and clear guideline for the design of the transition is proposed. The implemented transition performs less than 0.6 dB insertion loss per transition for frequencies from 40 MHz to 12 GHz and less than 1.2 dB insertion loss to 27 GHz, which well exceeds the previous results in the literature. A clear and efficient design method for ultra-wideband microstrip-to-suspended stripline transition, which is based on the analytical expressions of the whole transitional structure, is presented. The conformal mapping is applied to obtain the characteristic impedance of the transitional structure within 2.85% accuracy as compared with the EM-simulation results. The transition is designed to provide broadband impedance matching and smooth field conversion. In this study, a systematic and simplified design procedure for quasi-Yagi antennas is presented. The design is based on the simple field and impedance matching among antenna components. This new antenna design is possible due to the newly developed ultra-wideband balun. Two types of wideband quasi-Yagi antennas are successfully designed and implemented in Ku- and Ka-band with frequency bandwidths of 53.2% and 29.1%, respectively. © 2008 Wiley Periodicals, Inc. This paper presents a method for the selection of test sequences from statecharts. It is shown that a statechart can be transformed into a flow graph modelling the flow of both control and data in the statechart. The transformation enables the application of conventional control and data flow analysis techniques to test sequence selection from statecharts. The resulting set of test sequences provides the capability of determining whether an implementation establishes the desired flow of control and data expressed in statecharts. A 49-year-old man was referred to our department with profuse serous fluid discharge from a Penrose drain after undergoing internal fixation with metal screws for multiple pelvic bone fractures. A definite ureteral penetration was identified that was orientated from the lateral to the medial aspect of the right distal ureter. The patient was surgically treated with excision of the 2-cm injured ureteral segment, end-to-end ureteroureterostomy, and double J ureteral stent placement. To our knowledge, a penetrating ureteral injury caused by bone drilling has not been reported previously in the published literature. This case shows that surgeons who do pelvic surgery, including orthopedic surgeons, should be familiar with the anatomical relationship of the ureter and its potential injuries. This study purposed to analyze inter-influences among the administrators’ profiles of characteristics, their value systems, and their public policies in the situations of local governments in South Korea. The characteristic profiles are attributed to behavior, emotion, and cognition. The value systems are referred to as economics and job ethics. The public policies were divided into distribution and regulation. The research methods were based on a questionnaire that constructed items for the above three variables and requested Likert scales. The sample size was 1061, recruited from all eight Korean local governments. The results of the structure analysis suggested that the administrators’ decisions about their public policy were influenced by their value systems attributed to their characteristics profiles. Further studies required the application of this study’s paradigm to the diverse values and policies that were limited in this study and to the world’s nations, where the governmental processes are different in economic ideology and political culture from Korean structures.

A laser driver for a four-channel parallel fibre optic transceiver operating up to 2.5 Gbit/s is designed using 0.35 µm SiGe BiCMOS process. An 850 nm vertical cavity surface emitting diode array is used as a light source. The measurement was performed under the case temperature range 0-80°C at 3.1 to 3.5 V power supply variations. Using a gigabit Ethernet filter, a 45% eye mask margin has been achieved at -4 dBm average optical power and 7 dB extinction ratio at 40°C. The rise/fall time is less than 100 ps. Abstract Scanning electron microscopy (SEM) is a high-resolution imaging technique with subnanometer spatial resolution that is widely used in materials science, basic science, and nanofabrication. However, conducting SEM is rather complex due to the nature of using an electron beam and the many parameters that must be adjusted to acquire high-quality images. Only trained operators can use SEM equipment properly, meaning that the use of SEM is restricted. To broaden the usability of SEM, we propose an autofocus method for a SEM system based on a dual deep learning network, which consists of an autofocusing-evaluation network (AENet) and an autofocusing-control network (ACNet). The AENet was designed to evaluate the quality of given images, with scores ranging from 0 to 9 regardless of the magnification. The ACNet can delicately control the focus of SEM online based on the AENet’s outputs for any lateral sample position and magnification. The results of these dual networks showed successful autofocus performance on three trained samples. Moreover, the robustness of the proposed method was demonstrated by autofocusing on unseen samples. We expect that our autofocusing system will not only contribute to expanding the versatility of SEM but will also be applicable to various microscopes.

Degraded electrodes in a resistance spot welding system should be replaced to ensure that weld quality is maintained. Welding electrodes are subjected to different environmental and operational loading conditions during use. When they are replaced with a fixed interval, replacement may occur too early (raising maintenance costs) or too late (leading to quality issues). This motivates condition monitoring strategies for resistance spot welding electrode tips. Thus, this paper proposes a modified recurrence plot (RP) for robust condition monitoring of welding electrode tips in resistance spot welding systems. The overall procedure for the proposed condition monitoring approach consists of three steps: (1) transformation of a one-dimensional signal to a two-dimensional image, (2) unsupervised feature extraction with LeNet architecture-based convolutional neural networks, and (3) health indicator calculation. RP methods convert dynamic resistance waveforms to RPs. The original RP method provides an image with binary-colored pixels (i.e., black or white) that makes this method insensitive to the change of the waveform signal. The proposed RP method is devised to be sensitive to the change of the waveform signal, while enhancing robustness to external noise. The performance of the proposed RP method is evaluated by examining simulated aperiodic waveform signals with and without external noise. A case study is presented to examine the proposed method’s ability to monitor the condition of resistance spot welding electrodes. The results show that the proposed method outperforms handcrafted, feature-based condition monitoring methods. This study can be used to accurately determine the lifetime of welding electrodes in real time during the spot welding process.

Objective: The cholesterol-lowering effects of garlic as part of a healthy diet remain controversial. The aim of this study was to investigate whether supplementation with aged black garlic (ABG) could improve blood lipid profiles in patients with mild hypercholesterolemia. Methods: We conducted a double-blind, randomized placebo-controlled trial. Sixty participants were randomly assigned to receive either ABG or placebo twice daily (total 6 g/d) before consumption of a meal every morning and evening for 12 wk. During the study, two participants dropped out of the ABG group, and three participants dropped out of the placebo group. Thus, the effects of AGB on fasting blood levels of lipids were evaluated in 28 participants and compared with the placebo group (n = 27). Results: Among lipid components, no significant differences in triglycerides, low-density lipoprotein cholesterol, total cholesterol, or free fatty acid levels were observed between the two groups. However, ABG increased high-density lipoprotein cholesterol levels compared with the placebo group at the end of the study. Moreover, a significant decrease in the levels of alipoprotein B and a significant increase in the ratio of low-density lipoprotein cholesterol/alipoprotein B were observed in the ABG group. No adverse effects were reported in any of the patients. Conclusion: ABG supplementation reduced atherogenic markers and thus may have a cardioprotective effect beyond the gold standard medication in patients with mild hypercholesterolemia.
