The equivalent mixture of cis-3-hexenol and trans-2-hexenal (hexenol/hexenal), 'green odor', is known to have a healing effect on the psychological damage caused by stress. Behavioral studies in humans and monkeys have revealed that hexenol/hexenal prevents the prolongation of reaction time caused by fatigue. In the present study, we investigated which brain regions are activated by the odor of hexenol/hexenal using positron emission tomography with alert monkeys. Regional cerebral blood flow (rCBF) in the prepyriform area (the primary olfactory cortex) was commonly increased by the passive application of odor: acetic acid, isoamylacetate or hexenol/hexenal. We observed rCBF increases in the orbitofrontal cortex (the secondary olfactory cortex) by these olfactory stimuli in two of three monkeys, and found no predominance of laterality of the activated hemisphere. Furthermore, rCBF increase in the cerebellum was observed in two of three monkeys, and the odor of acetic acid increased rCBF in the substantia innominata in all monkeys. In addition to these olfactory related regions, the anterior cingulate gyrus was activated by the odor of hexenol/hexenal. These findings suggest that the increase of rCBF in the anterior cingulate gyrus by the odor of hexenol/hexenal may contribute the healing effects of this mixture observed in the monkey.

Osteochondroma is a benign neoplasm that usually develops in long bones and very rarely occurs in craniofacial bones. Nearly all reported mandibular osteochondromas have arisen in the condyle and the coronoid process, and occurrence in other locations is extremely rare. We describe a case of osteochondroma arising from the inferior border of the mandibular symphysis.

Development of neural circuits generating fetal oral-motor activity was characterized in an in vitro isolated brainstem block preparation. Rhythmical trigeminal activity (RTA) at E20-E21 resembled either the pattern or rhythm of neonatal RTA. Conversely, at E18-E19, RTA displayed a different pattern of discharge from neonatal RTA, and output was not regular but intermittent with another slow rhythm.

To correct the nasal deformity in cleft lip patients, a new procedure of open rhinoplasty using a "flying-bird" incision in the nostril tip with a vestibule "tornado"-shaped incision in the cleft side is presented. The newly designed vestibular incision produces effective vestibular advancement with the freed lower lateral cartilage. The flying-bird incision makes it possible to produce a suitable nostril tip appearance with symmetrical external nostril vestibules. If the vestibular defect after flap advancement is wide, a full-thickness skin graft is used to give priority for making a good external nostril shape. This procedure is useful for most cleft lip noses, particularly in cases of moderate to severe deformity.

To clarify the aspects and role of oropharyngeal closure, soft palate movements during sucking were observed and then assessed by electromyographic and fluoroscopic analyses. Three patterns of sucking movements by the palatoglossus and levator veli palatini muscles were analyzed with electromyographic means in seven healthy adults. Furthermore, a forced sucking maneuver, which required special effort to produce a strong sucking pressure by opening the jaw, was analyzed using lateral fluoroscopy in three of the subjects. During all tested sucking movements, the palatoglossus muscle showed continuous activity. The levator veli palatini muscle did not show remarkable activity during regular air sucking or water sucking when water was held in the oral cavity. However, its participation increased when intraoral sucking pressure was increased by opening the jaw, and lateral fluoroscopic examinations revealed that the soft palate was pulled toward the oral cavity to form a tight contact between the rostral portion of the soft palate and the retrotongue, causing the soft palate to make an “L” shape. The palatoglossus muscle plays a major role in palatal movement during sucking. Furthermore, the levator veli palatini muscle, whose activity was observed only during high-pressure sucking with an open jaw, may act to maintain tension in the soft palate against strong negative pressure in the oral cavity.

This study investigated the correlation between clinical and histopathological findings in 171 cases clinically diagnosed as oral leukoplakia. Histopathological examination revealed squamous cell carcinoma in 14 cases, leukoplakia with epithelial dysplasia in 68 cases, and hyperkeratosis without epithelial dysplasia in 89 cases. According to the grade of epithelial dysplasia, the leukoplakias were classified into mild, moderate, severe epithelial dysplasia and carcinoma in situ, which included 49, 14, 3 and 2 cases, respectively. High grades of epithelial dysplasia were mainly observed on the lateral edge of the tongue or the oral floor. There was a close correlation between the clinical appearance of lesions and a high grade of dysplasia; the majority of the cases with high grades of dysplasia or with early invasive carcinoma clinically showed erosive-type lesions.

The purpose of this study was to induce swallowing in an in vitro neonatal rat brainstem preparation and to analyze the circuit. When we applied GABA(A) receptor antagonist (bicuculine methiodide, BIC) into the the nucleus tractus solitarius (NTS) in the organ attached brainstem preparation of neonatal (0-3 days after birth) rats, jaw closing movement, palatal lifting, and tongue peristalsis-like movement were seen, subsequent to elevation of the tip of the tongue and anterior movement of the larynx (closure of the trachea). The NTS has been proposed to be a critical locus for swallowing pattern generation in mammals. Electrical stimulation into the NTS or the vagal afferent nerve (X) following an application of BIC (10 microM) to the recording chamber initiated the same organ movement. This movement caused temporary inhibition of respiratory activity that was simultaneously recorded from the fourth cervical ventral nerve (C4). We were also able to elicit this activity in a whole organ (from lip to stomach, midline intact) preparation, whose oral cavity was filled with dye (pontamine sky-blue 3 mM, 50 microl), using each of the three types of stimulation. The esophagus, which was never stained by spontaneous respiratory movements, was stained only after the experimental stimulation. We concluded that the activity elicited was swallowing-like activity and the smallest circuit for swallowing pattern generation exists in this preparation.

Desmoplastic ameloblastoma (DA) is a relatively rare odontogenic neoplasm. DA presents with clinical features that vary significantly from the more common multicystic ameloblastoma, such as a predilection for the anterior jaw and a mixed radiolucent/radiopaque appearance, to a multilocular radiographic appearance of the lesions most common in the posterior mandible. On gross examination, DA appeares as a solid mass, unlike the multicystic ameloblastoma that contains fluid-filled spaces. Histologically, DA is characterized by small nests and strands of “compressed” odontogenic epithelium supported by desmoplastic stroma, the density of which accounts for the radiographic appearance. DA with a significant cystic component has not been reported. In this case report, we present a rare case of DA with significant cystic change in a lesion with extension into the maxillary sinus.

We used rat isolated brainstem block preparations to analyze the functional roles of serotonin receptors in the generation of trigeminal rhythmic activities. We previously reported that trigeminal rhythmic activities could be induced by some pharmacological applications in an isolated brainstem preparation with a rostral boundary at the border between the inferior and superior colliculus, and a caudal border at the level of the rostral facial nucleus. However, the same stimulation did not induce trigeminal rhythmic activities in a whole brainstem block preparation with the same rostral boundary and a caudal border at the obex level. In the present study, both the 5-HT1Aphthalimido-butyl-piperazine, and the 5-HT2Cagonist, 1-2,5-dimethoxy-4-iodophenyl-2-aminopropane, combined with N-methyl-D,L-aspartate and bicuculline, elicited trigeminal rhythmic activities in a whole brainstem block preparation. Our results suggest that serotonin has both facilitation and inhibition effects on the generation of trigeminal rhythmic activities in an isolated brainstem block preparation in vitro.

Osteocytes express multiple genes involved in mineral metabolism including PHEX, FGF23, DMP1 and FAM20C. In Hyp mice, a murine model for X-linked hypophosphatemia (XLH), Phex deficiency results in the overproduction of FGF23 in osteocytes, which leads to hypophosphatemia and impaired vitamin D metabolism. In this study, to further clarify the abnormality in osteocytes of Hyp mice, we obtained detailed gene expression profiles in osteoblasts and osteocytes isolated from the long bones of 20-week-old Hyp mice and wild-type (WT) control mice. The expression of Fgf23, Dmp1, and Fam20c was higher in osteocytic cells than in osteoblastic cells in both genotypes, and was up-regulated in Hyp cells. Interestingly, the up-regulation of these genes in Hyp bones began before birth. On the other hand, the expression of Slc20a1 encoding the sodium/phosphate (Na+/Pi) co-transporter Pit1 was increased in osteoblasts and osteocytes from adult Hyp mice, but not in Hyp fetal bones. The direct effects of extracellular Pi and 1,25-dihydroxyvitamin D3 [1,25(OH)2D3] on isolated osteoblastic and osteocytic cells were also investigated. Twenty-four-hour treatment with 10-8 M 1,25(OH)2D3 increased the expression of Fgf23 in WT osteoblastic cells but not in osteocytic cells. Dmp1 expression in osteocytic cells was increased due to the 24-hour treatment with 10 mM Pi and was suppressed by 10-8 M 1,25(OH)2D3 in WT osteocytic cells. We also found the up-regulation of the genes for FGF1, FGF2, their receptors, and Egr-1 which is a target of FGF signaling, in Hyp osteocytic cells, suggesting the activation of FGF/FGFR signaling. These results implicate the complex gene dysregulation in osteoblasts and osteocytes of Hyp mice, which might contribute to the pathogenesis.

The chemokine receptor CCR5 has been implicated in the recruitment of T cells to inflammatory sites. However, the regulation of CCR5 induction on T cells and its contribution to T cell adhesiveness are poorly understood. Using a Th1 clone, 2D6, that can be maintained with interleukin (IL)-12 or IL-2 alone (designated 2D6IL-12 or 2D6IL-2, respectively), we investigated how CCR5 is induced on T cells and whether CCR5 is responsible for up-regulating the function of adhesion molecules. 2D6IL-12 grew, forming cell aggregates, in culture containing IL-12. This was due to lymphocyte function-associated antigen (LFA)-1–intercellular adhesion molecule (ICAM)-1 interaction, because 2D6IL-12 expressed both LFA-1 and ICAM-1 and cell aggregation was inhibited by anti-ICAM-1 monoclonal antibody. Despite comparable levels of LFA-1 and ICAM-1 expression, 2D6IL-2 cells did not aggregate in culture with IL-2. It is important that there was a critical difference in CCR5 expression between 2D6IL-12 and 2D6IL-2; the former expressed high levels of CCR5, and the latter expressed only marginal levels. Both types of cells expressed detectable albeit low levels of RANTES (regulated on activation, normal T expressed and secreted) mRNA. Unlike IL-12 or IL-2, IL-18 induced high levels of RANTES mRNA expression without modulating CCR5 expression. Therefore, combined stimulation with IL-12 and IL-18 strikingly up-regulated 2D6 cell aggregation. Notably, LFA-1-mediated aggregation of 2D6IL-12 cells was suppressed by anti-CCR5 antibody. These results indicate that IL-12 plays a critical role in CCR5 expression on Th1 cells and consequently contributes to CCR5-mediated activation of LFA-1 molecules.

To investigate the incidence of cleft lip or palate or both (CLP) in Japan, 303738 babies born in 1532 institutions between 1994 and 1995 were examined and 437 (0.14%) were found to have abnormalities. Of these babies, 32.1% had cleft lip, 43.3% had cleft lip and palate, and 24.8% had cleft palate (Table 2). These results show that the incidence of cleft lip and palate has declined compared with the period from 1981 to 1982.

This article reports a rare case of a displaced dental implant that migrated into the maxillary sinus.

N-methyl-D,L-aspartate acid and bicuculline are required to enhance the trigeminal rhythmic activities in an in vitro isolated brainstem block preparation. In this study, we analyzed the effect of norepinephrine on the trigeminal neural circuit underlying rhythmic jaw movements. Rhythmic trigeminal activity is observed in brainstem preparations (inferior colliculus to obex) only following blockade of alpha(2)-adrenoceptors with idazoxan. This observation, combined with the inhibition of rhythm by alpha(2)-adrenoceptor agonists suggests endogenous alpha(2)-adrenoceptor mediated inhibition of trigeminal networks. A complex noradrenergic modulation of trigeminal systems is further supported by the prazosin-sensitive potentiation of rhythm by bath application of the alpha(1)-adrenoceptor agonist phenylephrine.

Objective Respiratory-related electromyographic (EMG) activity of the superior pharyngeal constrictor (SPC) muscle was analyzed during the early stage of forced breathing. Design Four adult dogs anesthetized with sodium pentobarbital were used. In the first part of the study, oral and nasal breathing tubes were placed into the respective cavities, and a tracheotomy tube was placed in the second part of the study. Two conditions, the presence (oral-nasal tube breathing) and absence (tracheotomy breathing) of airflow in the upper airway, were achieved in each dog. Following quiet breathing, animals were connected to a closed breathing system, first by an oral-nasal tube and then by a tracheotomy tube. We proposed to induce a forced breathing condition mechanically by using this system for 1 minute. We increased resistance to airflow during forced breathing by means of connecting tubes and a bag. Our aim was not to produce chemical drive but to produce a forced respiration by increasing the resistance to airflow. Tidal volume, breathing frequency, minute volume, chest wall movement, and EMG activity of the SPC muscle were measured and analyzed. Results During quiet breathing through an oral-nasal or tracheotomy tube, low-amplitude EMG activity of the SPC muscle corresponding to the expiratory cycle of the respiration was observed. In both study conditions, phasic expiratory EMG activity increased immediately after the advent of the breathing from the closed system. Tidal volumes and frequencies also increased rapidly during forced breathing. Conclusions An increase in the resistance to airflow increased the activity of the SPC muscle. This augmented respiratory activity probably assists the patency of the upper airway. The augmented respiratory activity was independent of the local reflex pathways. Respiratory-related activity of the SPC muscle may help dilate and stiffen the pharyngeal airway, promoting airway patency.

Respiratory activity in trigeminal (V) motoneurons was studied in rhythmically active en bloc brainstem-spinal cord preparations isolated from neonatal rats (P0-P3). In the majority of preparations (83%), the temporal pattern of V activity consisted of spontaneous inspiratory phasic discharge with onset delayed or coincident with onset of phrenic motoneuron discharge. Blockade of alpha-2 noradrenergic receptor activation shifted onset of V respiratory discharges earlier than phrenic discharges, while elevation of extracellular potassium concentration or blockade of GABAergic and glycinergic inhibitory synaptic transmission had little effect on temporary pattern of V respiratory discharges. We conclude V motoneurons in the in vitro preparation generate respiratory activity during inspiratory phase, and their temporal patterns are modulated by inhibitory noradrenergic synaptic transmission.

We present a rare case of angiomyolipoma of the tongue in a 23-year-old man. The clinical appearance was of a small, solitary, well-demarcated, painless mass. The oral angiomyolipoma followed a benign course with no recurrence after excision.

Objective The mechanical contraction property of the superior pharyngeal constrictor (PC) muscle was analyzed and compared with the levator veli palatini (LVP) muscle. Design Twenty adult dogs anesthetized with sodium pentobarbital were used. Results The isometric contraction curves of LVP and PC were recorded after direct electric stimulation to each muscle. The contraction time of isometric twitch contraction of the superior PC muscles was 30.3 ± 5.25 msec, and the half relaxation time was 22.97 ± 5.44 msec. The summation curve was observed at a stimulus frequency above 20 Hz of a repetitive pulse stimulus frequency of 85 Hz. Conclusions The mechanical contraction properties of the PC muscle are similar to those of the LVP, but with more aspects that are characteristic of a fast twitch muscle.

Objective: This study, using mongrel dogs, attempted to delineate the respiratory activity of two closure muscles of the velopharynx, the levator veli palatini (LVP) and superior pharyngeal constrictor (PC) muscles. Method: Fifteen mongrel dogs were given tracheotomies, with intratracheal tubes being connected to their respiratory circuits. Activities of the PC and LVP muscles were recorded electromyographically, and the effects of Pao2 and Paco2 were analyzed along with the effects of lung inflation intratracheally with O2 and N2. Results: Under hypoxic conditions, expiratory LVP activity was observed while PC activity diminished. The effects of lung inflation on both LVP and PC activities were also studied. The activities of both muscles were enhanced by lung inflation. Conclusion: It was thus suggested that the roles of both muscles during velopha-ryngeal closing movements are not the same.

Purpose Bifocal distraction osteogenesis has been shown to be a reliable method for reconstructing missing bone segments. However, no reports have been published regarding inferior alveolar nerve regeneration during this procedure. We assumed that the nerve could regenerate with the bone regeneration during bifocal distraction, if the nerve had been saved at a mesial site of the transport disc. In the present study, we investigated that possibility in dogs. Materials and Methods Using a bifocal distraction osteogenesis method, we produced a 10-mm mandibular defect, including the nerve defect, and distracted the transport disc at a rate of 1 mm/day in 12 dogs. The nerve was saved at the mesial site of the transport disc. The regenerated nerve was evaluated by a jaw opening reflex examination performed once daily. Histologic examinations with hematoxylin-eosin and immunohistochemical staining with neurofilament and S-100 antibody were also performed on all dogs after death at 3, 6, and 12 months after the first operation. Results The jaw opening reflex had recovered in all dogs. The average period of recovery was 109.5 ± 24.7 days. On histologic examination, although consecutive nerves were observed in all areas, cellular nerve fascicles were seen, consistent with wallerian degeneration at 3 and 6 months in the nerve connection area on the distal side of the transport disc. Conclusions Our results have indicated that inferior alveolar nerve regeneration after bifocal distraction osteogenesis is successful in dogs. Although our research is still at the stage of animal experiments, future application in humans can be considered to be possible.

Backgrounds The optimal method of management of clinical N0 neck in squamous cell carcinoma (SCC) of the oral cavity remains controversial. In 1994 Weiss et al. recommended with decision analysis that when the probability of occult cervical metastasis is more than 20% (treatment threshold), the neck should be electively treated. Recently, we calculated the treatment threshold as 44.4% on patients treated between 1991 and 2006. In order to explore this gross discrepancy, we conducted the decision sensitivity analysis on patients treated between 1987 and 2000 compared with those treated between 2001 and 2009. Results Between 1987 and 2009 we treated 270 consecutive patients with previously untreated SCC of the oral cavity and N0 neck. Among three variables leading to the treatment threshold, the 5-year overall survival rate for patients whose necks were observed and harbored occult metastasis was significantly different between two treatments periods. The rate was 83.3% since 2001, significantly higher than 47.1% until 2000. On the contrary, other variables were constant. The 5-year overall survival rate for the patients who received elective neck dissection (END) with no neck recurrence was 88.2% until 2001 and was the same rate as 88.1% until 2000. The 5-year overall survival rate for the patients whose N0 necks were observed without END and had no neck recurrence was 96.8% since 2001 and the rate was 95.7% until 2000. Thereby, the treatment threshold was calculated to be 22.8% until 2000 and 93.5% since 2001, respectively. Conclusion The survival rate for these observed patients can be improved to the comparable rate for patients who underwent elective neck dissection and were verified to have occult metastatic nodes. The improvement thereby permits wait-and-see policy for patients with N0 neck.

Cleft palate results from a mixture of genetic and environmental factors and occurs when the bilateral palatal shelves fail to fuse. The objective of this study was to search for new genes involved in mouse palate formation. Gene expression of murine embryonic palatal tissue was analyzed at various developmental stages before, during, and after palate fusion using GeneChip® microarrays. Ceacam1 was one of the highly up-regulated genes during palate formation, and this was confirmed by quantitative real-time PCR. Immunohistochemical staining showed that CEACAM1 was present in prefusion palatal epithelium and was degraded during fusion. To investigate the developmental role of CEACAM1, function-blocking antibody was added to embryonic mouse palate in organ culture. Palatal fusion was inhibited by this function-blocking antibody. To investigate the subsequent developmental role of CEACAM1, we characterized Ceacam1-deficient (Ceacam1(-/-)) mice. Epithelial cells persisted abnormally at the midline of the embryonic palate even on day E16.0, and palatal fusion was delayed in Ceacam1(-/-) mice. TGFβ3 expression, apoptosis, and cell proliferation in palatal epithelium were not affected in the palate of Ceacam1(-/-)mice. However, CEACAM1 expression was retained in the remaining MEE of TGFβ-deficient mice. These results suggest that CEACAM1 has roles in the initiation of palatal fusion via epithelial cell adhesion.

The present study employed the neonatal rat isolated brainstem preparation to determine whether oral-motor rhythmical activity, a substrate for the complex behaviors of suckling and chewing, could be elicited in vitro by path application of excitatory amino acids (EAAs). Bath application of EAA agonists (kainate [KA], [+/-]-a-amino-3-hydroxy-5-methylisoxazole-4-propionic acid [AMPA], N-methyl-D, L-aspartate [NMA]), in conjunction with the gamma-aminobutyric acid antagonist bicuculline, either failed to induce rhythmic activity (n = 17 preparations) or induced a low-amplitude, low-frequency burst discharge (< 1 Hz, n = 10 preparations) from the motor branches of the trigeminal nerves when the brainstem was contiguous from the spinomedullary junction to the superior colliculus. Burst activity was in most cases bilaterally synchronous. However, when a discrete coronal transection was made at the level of the facial colliculus, between the trigeminal and facial motor nuclei, the rhythmic bursts produced by the resultant 3- to 5-mm block of tissue following bath application of EAA agonists increased in amplitude and frequency (4-8 Hz, n = 35). Application of 6-cyano-7-nitroquinoxaline-2, 3-dione (CNQX), a non-N-methyl-D-aspartate (non-NMDA) receptor antagonist, blocked the rhythm induced by non-NMDA receptor agonist (n = 4) but was less effective in suppressing NMA-induced rhythmicity. In contrast, D, L-2-amino-5-phosphonovaleric acid (APV) blocked by both NMA-induced (n = 5) and, in most cases, KA-induced (n = 5) rhythmicity, suggesting an essential role for NMA receptors in production of EAA-induced rhythmical oral-motor activity in the neonatal rat. The present data demonstrate that a narrow, bilaterally distributed region of brainstem surrounding the trigeminal motor nucleus contains sufficient neuronal circuitry for the production of oral-motor rhythmogenesis.

Oblique facial cleft is a rare congenital anomaly, its incidence ranging from 1 to 5 per 1, 000 common facial clefts.In this paper, we reported a case of bilateral oblique facial cleft, classified into Morian II.These clefts extended from the lateral sides of the cupid's bow, toward the medial canthi.The medial canthi were dislocated downwards and the alae nasi were pulled upwards. In addition, bilateral cleft palate and the obstruction of nasolacrimal ducts were observed.At 6 months of age, the bilateral facial clefts were closed by a plastic operation using small triangular flaps.This operation procedure resulted in a great improvement of the appearance.At 18 months of age, the cleft palate was repaired with the push back method.

A case of an infiltrating lipoma, occurring in the tongue of a 56-year-old Japanese woman, is presented. The relevant literature is discussed.
