This study aimed to understand how users from Family Health Program (FHP) make their decision about health-illness. It was used the Stages of Change Model from Prochaska and DiClemente to identify the level of change of FHP users. It was a qualitative research developed with users and FHP health professionals from Crato, Ceará State, Brazil. The data were collected by semi-structured interviews, focal group and observation from May to September in 2005. They were organized by means of content analysis and analyzed in the light of theoretical reference. The changes of the FHP users are depicted as following: in the pre-contemplation stage the emphasis is on disease and care itself. In the contemplation stage, the care is on the family. It is centred in the cure and treatment of diseases. In the determination stage, we have some improvement concerning prevention in the family, with registers of looking around. In the action stage, behaviour from the user to reach autonomy and emancipation is registered. These results seem to indicate ascendency in FHP users' learning and participation towards health promotion.

Este estudo retrospectivo teve como objetivo analisar os óbitos maternos notificados no Ceará de 1998 a 2002 (n=621). Os dados foram fornecidos pela Vigilância Epidemiológica da Secretaria da Saúde do Ceará. Para análise de associação entre variáveis utilizou-se o teste de Qui-quadrado ou a Razão de Máxima Verossimilhança para tabelas RxS. A maioria dos óbitos foi de mulheres entre 20 e 34 anos, de baixa renda, intervalo gestacional maior que dois anos e paridade de até quatro filhos. A análise do número de consultas ficou prejudicada pela subinformação e pelo fato de que não se analisou a idade gestacional em que ocorrera o óbito. A maioria dos óbitos ocorrera no hospital, assistidos por clínicos e obstetras. O óbito materno merece ser visto sob a qualidade da assistência ao pré-natal, parto e puerpério, de maneira a se estabelecer rigor técnico e compromisso com o atendimento prestado e sua notificação.

Free access to prenatal care and HIV tests represent the first step in the prevention of vertical transmission and guarantees early prophylactic measures. This study aims at analyzing the coverage of HIV testing in pregnant women in a Basic Family Health Unit in the city of Fortaleza, Ceará. The theoretical-methodological basis of the study was the grounded theory, which proposes generation of theory from data derived from the social reality. Data were collected through participant observation, open interviews with physicians, nurses and pregnant women and in focal groups with pregnant women and health agents. The women faced institutional barriers to prenatal care and HIV testing. Difficulties in making the appointments for prenatal care hampered early identification of pregnant women and delayed their assistance. With respect to HIV testing, the study found that the Basic Health Unit does not collect the biologic material; there are few laboratories performing this kind of exam and those who do are very far from the pregnant women's residences. This study concludes that even in the Family Health Unit, the access to prenatal care for pregnant women remains difficult, and that the prevention of vertical HIV transmission depends on political, institutional and structural changes.

No processo de evolução, a humanidade acumulou saberes que foram sistematizados como conhecimentos. A Filosofia nos auxilia nas funções teóricas e práticas a chegar a uma concepção do universo por meio da auto-reflexão.O senso comum contribui para que a ciência progrida. A partir de problemas do cotidiano das pessoas, surge a necessidade de pesquisar, de aprofundar interpretações dos achados e propor soluções para superar as dificuldades enfrentadas pela população. A ciência existe para esclarecer aspectos problemáticos do senso comum, fornecer respaldo aos questionamentos e fundamentar cada conhecimento produzido em resposta às demandas. Assim, os conhecimentos envolvidos nesta reflexão pretendem beneficiar uma articulação entre as formas básicas de conhecimento e desenvolver uma compreensão satisfatória da promoção da saúde, numa visão compartilhada e conscientizadora da mudança de paradigmas no sistema de saúde. Compreendemos que a promoção da saúde constitui um componente indispensável neste processo, tendo como foco central de suas intervenções o indivíduo pertencente a uma comunidade nas suas múltiplas relações, especialmente entre o contexto comunitário e a dimensão subjetiva, propiciando-lhe um resgate da cidadania.

Pesquisa avaliativa realizada em oito municípios do Ceará, Brasil, de julho a setembro de 2003. Os dados foram coletados por entrevistas realizadas com 29 enfermeiros e 50 usuárias do Programa Saúde da Família (PSF) e observações nas unidades de saúde. Teve por objetivos identificar a dinâmica do atendimento em planejamento familiar e verificar barreiras voltadas ao atendimento e entrega dos métodos anticoncepcionais, na perspectiva de uma rede apropriada de serviços. Identificaram-se cinco dinâmicas de atendimentos, porém nenhuma seguia uma rotina formal, o que ocasiona dilema legal e ético sobre a prescrição de métodos anticoncepcionais pelos enfermeiros; a entrega de métodos exige retorno mensal das usuárias, por determinações técnicas excedentes e desnecessárias que são barreiras ao acesso dos usuários aos métodos; e inexiste uma rede apropriada de serviços, com atendimento centralizado no enfermeiro e no médico, inexistindo parceria com serviços outros de saúde reprodutiva ou espaços comunitários. Estudos futuros devem ser formatados com vistas a identificar dinâmicas distintas no PSF que inovem a atenção ao planejamento familiar, bem como definir o aspecto legal e ético da prescrição de métodos pelos enfermeiros. This evaluative study was performed in eight counties in Ceará State, Brazil, from July to September 2003. The aim was to identify the nature of family planning services and verify the existence of barriers to services and provision of contraceptives with a view towards ensuring an appropriate services network. Five styles of services were identified, although none followed a formal protocol, which raises a legal and ethical dilemma regarding prescription of contraceptives by nurses; delivery of contraceptive methods requires a monthly return visit by users due to excessive and unnecessary technical requirements that create barriers to access by users; and there is a lack of appropriate services, with nurse- and doctor-centered treatment and lack of partnership with other reproductive health services or community groups. Future studies should be designed to identify distinct dynamics in the FHP that innovate in family planning, as well as to define the legal and ethical framework for nurses to prescribe the methods.

The aim of this study was to analyze how women with back pain perceived their quality of life after having attended educational workshops on back pain directed to self-care and prevention. This qualitative study was conducted in a Higher Education Institution in Fortaleza, Ceará, Brazil, during the months April and May 2005, using a sample of nine women who had concluded eight workshops. The focal group method was selected for data collection, and the sessions were filmed and recorded. Data were grouped into categories and sequentially analyzed. All women thought they had a good quality of life, independently of having a poor health, no money and no a job. All this was overcome by their faith in God. All women reported that, before the workshops, the pain had interfered in their daily and social lives and that, after the intervention, the pain had calmed down or disappeared. The awareness of self-care provided physical, intellectual and emotional well being, improved mobility and decision-making and allowed them to return to their activities. Besides, it helped to eliminate or reduce pain medication. The study made us realize the importance of evaluating the quality of life in order to guide us in the choice of the most effective therapeutic measures.

This study approached the practice of self care lived by the women that suffer with high blood pressure. Descriptive-exploratory study which was inspired on the Orem's self-care theory and developed with 200 women who presented high blood pressure at a Public Health Agency, located in Fortaleza, Ceará, Brazil. The data were collected during nursing encounters, organized in categories, and analyzed according to the fundaments of the referred Theory. In this surveyed sample, about 172 (86%) women found out that they had high blood pressure problems by means of signs and symptoms related to the "climateric". It was evidenced on the women interviewed, that they presented knowledge knowledge about this syndrome, partial adherence to the treatment, and consequently inefficient practice of self-care, related to this deficit. In this way, it is possible to conclude that the self-care practice was influenced by the age, by disease stage development, and by the social, economic and health problems.

OBJETIVO: avaliar a qualidade da assistência em planejamento familiar a partir de opiniões de usuárias do Programa Saúde da Família (PSF). MÉTODOS: estudo avaliativo cujos dados foram coletados em oito municípios do Ceará, por meio de entrevista realizada com 50 usuárias e observação de campo. Os dados foram organizados pela técnica de análise categorial e analisados a luz da estrutura conceitual de Bruce. RESULTADOS: a qualidade da assistência em planejamento familiar foi determinada por: orientação/informação aos clientes, envolvendo o casal; divulgação; oferta regular dos anticoncepcionais; fácil acesso; atendimento cordial; vínculo com a equipe do PSF; e cuidado diferenciado às mulheres com risco reprodutivo. CONCLUSÃO: a escuta ao cliente mostrou-se fundamental à avaliação da qualidade da assistência em planejamento familiar com enfoque nas necessidades dos usuários. OBJECTIVE: to evaluate the quality of assisted family planning based on opinion of users of a Family Health Program. METHODS: an exploratory design was used to conduct this study. Data were collected in eight Ceara State cities through face-to-face interviews and field observations with 50 females. Data analysis consisted of categorical analysis technique in the light of Bruce's conceptual structure. RESULTS: quality of Assisted family planning was determined by orientation/instruction to users, couple involvement, divulgation, regular offering of contraceptive practices, easy access to care, friendly customer service, access to the Family Health Program team; and special care to women with reproductive risk. CONCLUSION: user comments suggested that it is fundamental to evaluate the quality of assisted family planning based on the users' needs. OBJETIVO: evaluar la calidad de la asistencia en planificación familiar a partir de la opinión de usuarias del Programa Salud de la Familia. MÉTODOS: estudio evaluativo cuyos datos fueron recolectados en ocho municipios de Ceará, por medio de una entrevista realizada a 50 usuarias y observación de campo. Los resultados fueron organizados por la técnica de análisis de categorías y analizados a la luz de la estructura conceptual de Bruce. RESULTADOS: la calidad de la asistencia en planificación familiar fue determinada por: orientación/información a los clientes, incluyendo a la pareja; divulgación; oferta regular de los anticonceptivos; fácil acceso; atención cordial; vínculo del equipo del PSF; y cuidado diferenciado a las mujeres con riesgo reproductivo. CONCLUSION: el escuchar al cliente se mostró fundamental para la evaluación de la calidad de la asistencia en planificación familiar con enfoque en las necesidades de los usuarios.

A investigação abordou a prática do autocuidado vivenciada pela mulher hipertensa. Estudo exploratório-descritivo, fundamentado na teoria do autocuidado de Orem e desenvolvido com 200 mulheres hipertensas em uma instituição pública de saúde de Fortaleza-CE. Os dados foram coletados durante as consultas de enfermagem, organizados em categorias e analisados conforme os fundamentos da teoria escolhida. Na amostra pesquisada, cerca de 172 (86%) mulheres descobriram sua hipertensão mediante sinais e sintomas relacionados ao climatério. Evidenciou-se ainda nas mulheres, déficit de conhecimento e aderência parcial ao tratamento, consequentemente prática ineficaz do autocuidado, em virtude desse déficit. Desta forma, conclui-se que a prática do autocuidado era influenciada pela idade, pelo estágio de desenvolvimento e por problemas sociais, financeiros e de saúde. La investigación abordó la práctica del autocuidado vivenciada por la mujer hipertensa. Estudio exploratorio-descriptivo, fundamentado en la teoría del autocuidado de Orem y desarrollado con 200 mujeres hipertensas en una institución pública de salud de Fortaleza-CE. Los datos fueron recogidos durante las consultas de enfermería, organizados en categorías y analizados de acuerdo a los fundamentos de la teoría escogida. En la muestra pesquisada, cerca de 172 (86%) mujeres descubrieron su hipertensión mediante señales e síntomas relacionados al climaterio. Se evidenció además en las mujeres un déficit de conocimiento, adhesión parcial al tratamiento y una práctica ineficaz de autocuidado en virtud de ese déficit. De esa forma, conclúyese que la práctica del autocuidado era influenciada por la edad, por el estadio de desarrollo y por problemas sociales, financieros y de salud.

Objective: analyze the counseling services for pregnant women with HIV/Aids performed by an interdisciplinary team in a Special Care Unit. Methodology: this is a qualitative research conducted with health professionals who worked in a public maternity hospital in Fortaleza, Ceara. Data collection occurred between the months of March and May, 2009, through a semi-structured interview and a check-list based on the Ministry of Health guidelines for counseling. The analysis considered the pre-established categories: reception and counseling; vulnerability/risk assessment and development of prevention alternatives. We interviewed seven professionals from the Special Care Unit. Results: the team performed the reception, considered the importance of listening to patients and provided emotional support for pregnant women with HIV/Aids. However, the counseling was rather prescriptive and professionals did not develop prevention strategies together with pregnant women. Conclusion: it was identified that the counseling services did not meet all the criteria established by the Ministry of Health. Descriptors: Counseling. HIV. Pregnancy. RESUMO Objetivo: analisar o aconselhamento para gestantes com HIV/Aids realizado por uma equipe interdisciplinar em um Servico de Assistencia Especializada. Metodologia : trata-se de uma pesquisa qualitativa desenvolvida com profissionais de saude que trabalhavam em uma maternidade publica de Fortaleza, Ceara. A coleta de dados ocorreu entre os meses de marco e maio de 2009, atraves de uma entrevista semiestruturada e de um check-list baseados nas diretrizes preconizadas pelo Ministerio da Saude para a realizacao do aconselhamento. A analise ocorreu considerando as categorias pre-estabelecidas: receptividade e acolhimento; avaliacao de vulnerabilidade/risco e elaboracao de alternativas de prevencao. Foram entrevistadas sete profissionais do Servico de Assistencia Especializada. Resultados: a equipe desenvolvia o acolhimento, valorizava a escuta e prestava apoio emocional as gestantes com HIV/Aids. Entretanto, as orientacoes eram prescritivas e os profissionais nao elaboravam estrategias de prevencao conjuntamente com as gestantes. Conclusao: identificou-se que as acoes do aconselhamento nao atendiam todos os criterios recomendados pelo Ministerio da Saude. Descritores : Aconselhamento. HIV. Gravidez. RESUMEN Objetivo: analizar el consejo para embarazadas con HIV/Sida realizado por un equipo interdisciplinar en un Servicio de Asistencia Especializada. Metodologia: tratase de un estudio cualitativo desenvuelto con profesionales de salud que trabajaban en una maternidad publica de Fortaleza, Ceara. Los datos fueron colectados entre los meses de marzo y mayo, 2009, por medio de una entrevista semiestructurada y de una lista de verificacion basados en las directrices recomendadas por el Ministerio de Salud para la realizacion del consejo. El analisis considero las categorias preestablecidas: receptividad y recepcion; evaluacion de vulnerabilidad/riesgo e elaboracion de alternativas de prevencion. Fueron entrevistadas siete profesionales del Servicio de Asistencia Especializada. Resultados: El equipo desenvolvia la recepcion, valoraba escuchar y apoyaba emocionalmente las embarazadas con HIV/Sida. Entretanto, las orientaciones eran prescriptivas y los profesionales no elaboraban estrategias de prevencion conjuntamente con las embarazadas. To evaluate the technical competence of professionals carrying out activities related to contraceptive care.Evaluative research in the field of health was conducted in eight districts of the State of Ceara from July to September 2003. Data was collected by means of interviews with 29 nurses working in the Family Health Care Program within these districts and 50 people being attended by this program. Observations of the Family Health Care Units were a complementary source of data within this study.The majority of nurses had received some form of training regarding contraception and the technical norms regulating their use. However, professional barriers were reported by the nurses and others were identified by lay persons being attended by the program, that indicate the need to provide better training for professionals engaged in this area of care. The nurses recognized they had deficits in information and communication skills as well as technical deficits in dealing with contraception.There are gaps in professional competence with regard to contraceptive care that, when associated to the lack of systematization of team work, generates distortions in the quality of care. Team work was characterized by the lack of definition of team members' specific attributions and tasks.

OBJETIVO: Avaliar a competência técnica de profissionais que atuam no cuidado em anticoncepção. MÉTODOS: Realizou-se pesquisa do tipo avaliativa, na área da saúde, em oito municípios do Estado do Ceará, de julho a setembro de 2003. Os dados foram coletados mediante entrevistas com 29 enfermeiros e 50 usuários do Programa Saúde da Família e observações feitas nas unidades de saúde. RESULTADOS: A maioria dos enfermeiros havia participado de alguma capacitação sobre anticoncepção e normas técnicas. No entanto, foi relatada a existência de barreiras profissionais pelos enfermeiros e outras foram identificadas pelos usuários, os quais mostraram a necessidade de melhor capacitação dos profissionais que atuam nessa área. Os enfermeiros reconheceram dificuldades, tanto na comunicação e informação quanto técnicas, no manejo dos anticoncepcionais. CONCLUSÕES: Há lacunas na competência profissional em anticoncepção que associadas a falta de sistematização do trabalho em equipe, geram distorções na qualidade da atenção. O trabalho em equipe foi caracterizado pela indefinição de atribuições e tarefas dos seus membros. Estudo descritivo realizado com o objetivo de analisar o relacionamento familiar após a mastectomia, conhecer as relações de interdependência e identificar alterações surgidas no dia-a-dia. Foram entrevistados 15 familiares entre os meses de junho a agosto de 2002, com consentimento declarado pela mulher e familiares. Usamos a abordagem da Teoria de Sister Callista Roy, especificando o modo de interdependência, para fundamentação da análise dos dados, os quais foram organizados com base na análise de conteúdo. Os resultados mostraram que a doença traz melhoria no relacionamento para a maioria das famílias, transmitindo-lhe confiança e afetividade. As relações de interdependência cresceram na maioria dos membros da família e as mudanças ocorridas foram favoráveis para a aproximação familiar e a melhoria da saúde de todos. Concluímos que a existência de uma pessoa doente mobiliza as emoções da maioria dos familiares e estes tomam decisões favoráveis à recuperação e ao convívio saudável.

Este artigo busca promover uma reflexão acerca dos critérios adotados pelo sistema de avaliação da Pós-graduação em curso no país e seu impacto no âmbito dos diferentes programas, conferindo destaque na análise à área de Saúde Coletiva. Ressalta-se, por um lado, a importância que os processos avaliativos têm na consolidação do sistema de ensino e pesquisa, contribuindo, de modo decisivo, para a promoção e monitoramento da sua qualidade. Em contrapartida, os autores convidam a uma reflexão sobre a concepção de ciência subjacente aos critérios adotados e seu impacto no conjunto dos cursos que compõem a pós-graduação em Saúde Coletiva. Para tanto, apontam obstáculos situados em planos distintos e as desigualdades, não apenas regionais, mas entre instituições geograficamente próximas mas que desenvolvem suas práticas em contextos distintos. O texto finaliza indagando sobre a correspondência entre o percurso histórico e os compromissos que marcam a trajetória da Saúde Coletiva e os rumos atuais da avaliação da pós-graduação nesse campo de saberes e práticas. This article aims to reflect on the criteria used in the current evaluation of graduate studies in Brazil and their impact on different programs, highlighting the analysis of the Collective Health field. The article emphasizes, on the one hand, the importance of evaluation processes for consolidating the teaching and research system, contributing decisively to the promotion and monitoring of its quality. On the other, the authors invite readers to reflect on the underlying concept of science in these criteria and their impact on the set of courses comprising Collective Health graduate studies in the country. The authors thus point to the obstacles on various levels and the inequalities between regions and between institutions that are geographically close, but which develop their practices in distinct contexts. The article concludes by inquiring as to the correspondence between the historical background and the commitments marking the history of Collective health and the current directions in the evaluation of graduate studies in this field of knowledge and practice.

In its evolution, humanity has accumulated data which were systematized as knowledge. Philosophy through self examination helps us in its practical and theoretical functions to reach a concept of the universe. Common sense helps science evolve. People's daily difficulties stir up the need for research, for deepening data interpretation and to propose solutions to overcome the population's problems. Science exists to explain difficult aspects of common sense, to support questions, as well as to substantiate knowledge produced as a response to demands. Thus, knowledge involved in this reflection sets out to foster an articulation between basic forms of knowledge and to develop a satisfactory understanding of the health care process, through a shared and critically consciousness view of the changes in the health system's paradigm. We understand that health education is an essential component within this process, provided that it is focused primarily on an individual belonging to a community with its multiple relationships, especially between the community context and the subjective dimension, which can provide citizenship empowerment redemption.

This study analyses the usage of Roy's Adaptation Model in the understanding of behaviorial responses experienced by woman during the parturition process, applies the description phase of The Meleis's analysis model and stops in the structural components, as follows: suppositions, concepts and propositions. These elements were explored in the model proposed by Roy and analysed according to their evidence in obstetric nursing situation. One remarks that that the usage of Roy's adaptation model was appropriated, which allowed an explicit analysis of most structural elements of the study.

Trata-se de pesquisa de avaliação com abordagens quantitativa e qualitativa que teve por objetivo descrever as barreiras relacionadas à oferta de informações em planejamento familiar (PF) / métodos anticoncepcionais (MACs); identificar os atores responsáveis por esta atividade; levantar o conhecimento da clientela sobre os MACs e a noção que esta tem a respeito da importância de planejar a família. Os dados foram coletados por meio de entrevista e observação livre em uma região do Ceará. Os resultados descreveram uma prática de dar informação em PF centrada na orientação individual e atividades grupais ou coletivas sendo realizadas esporadicamente, o que a maioria dos enfermeiros atribuiu à sobrecarga de tarefas e ao elevado número de pessoas vinculado a uma Equipe de Saúde da Família (ESF); a ausência de médicos em algumas ESF, sobrecarregando o enfermeiro; e a escassez de matérias de apoio e de um espaço físico apropriado. A pílula, o preservativo masculino e o injetável foram os MACs mais conhecidos das mulheres entrevistadas, seguidos pelo DIU, a tabela e o aleitamento materno. Mesmo assim, ficou constatada a necessidade de maior divulgação dessas informações. Ressaltamos as percepções singulares descritas pelo grupo de mulheres, guardando informações de sentido bastante amplo sobre os benefícios do PF.

Changes brought about by the diagnosis and treatment of cancer always cause psychological, social, cultural, economic and spiritual disorders. We have tried to understand the significance of breast cancer for women and to find out what they do in order to detect it in an early stage. We have randomly chosen thirty-four women who went to a cancer prevention centre in the town of Fortaleza. We have carried out clinical examinations of the breasts, taught the circular self-examination technique and observed how the technique was learnt. We have analysed the results by means of an interactionist approach, and they have been organised according to their significance and to the development of early detection actions. We have concluded that women perceive cancer as a possibility of loss and death and make associations between people or past events and real or imaginary causes. Some of them perform early detection procedures, but the lack of information on the disease is evident, which makes it unfeasible to control breast cancer.

This work aimed at the application of Nursing appointments to an hypertensive woman based on Orem's Theory, and at the identification of her degree of satisfaction in committing herself to self-care. The appointments were applied monthly for six months to fifty women. At every appointment, an evaluation was made on self-care deficit and on the abilities women showed to develop self-care activities. Most women overcame their self-care deficits, and acquired technical and cognitive abilities to perform self-care activities, falling into a profile that is desirable for improving quality of life. These changes were intensified at each appointment. Evidence showed that the application of this new technology brings highly positive influences to clients and assures quality in the Nursing services provided.

This objective of this study was to observe ethics and solidarity in the teaching-care process in women during prenatal care and childbirth. A centralized case study was carried out using critical-reflexive pedagogy. Thirty-nine nursing students and six teachers who taught theoretical-practice classes were interviewed at the Federal University of Ceará. The results showed that students and teachers are aware of the need for ethics competency and its applicability in care and that there is a need for new learning strategies to enable the student to develop interactive and human actions and solidarity. Updated care methodologies are needed that involve women as process participants and enable respect to their dignity.

The aim of this work is to understand woman's experiences and reality during the climacteric phase and its influence on their quality of life. The sample consisted of 25 women from 35 to 64 years old. They are all participants in the group of Self-Counseling for woman in this phase, developed in the Nucleus of Integrated Medical Attention - NAMI / Unifor , from April to May 2003. The data were obtained through a semi-structured interview. Results showed the women's characteristics, the symptoms and the more frequent psycho-physiological problems can be related to the lack of information, to the life history and to the woman's family and cultural context. The relevance of the Self-Counseling group was evidenced as support and incentive to the adaptation and in agreement with family and social coexistence. Also, the need of an attendance of committed nursery with information, education and promotion of health was evidenced. This is intended to the well-being and to the quality of woman's life in that phase of her life.

O estudo objetivou evidenciar ética e solidariedade no processo de ensinar-cuidar da mulher no pré-natal e parto. Adotou-se estudo de caso centrado na pedagogia crítica-reflexiva. Foram entrevistados 39 alunos do curso de Enfermagem da Universidade Federal do Ceará e seis docentes que ministraram aulas teórico-práticas. Os resultados mostraram que alunos e professores têm consciência da necessidade da competência ética e sua aplicabilidade no cuidar e que há necessidade de novas estratégias de ensino para o aluno desenvolver ações interativas, humanas e solidárias. O cuidar carece de metodologias atualizadas que envolvam a mulher como participante do processo e possibilitem o respeito à sua dignidade. Objetivou-se analisar a utilização de um vídeo educativo como instrumento de apoio para a orientação do auto-exame de mamas e detecção precoce do câncer de mama. A coleta de dados foi dividida em quatro momentos onde um vídeo educativo foi passado. A maioria das participantes desconhecia a doença, os métodos de detecção precoce e não realizavam o auto-exame das mamas. Após a exposição do vídeo educativo, as participantes mostravam-se mais seguras para falar da doença, referiram três exames para a detecção precoce e responderam corretamente as perguntas feitas sobre o auto-exame de mamas. Constatamos que a utilização do vídeo educativo facilitou o aprendizado das mulheres e que é preciso despertar esta população para a prática periódica do auto-exame de mama.

Observaram-se três aulas e realizaram-se reuniões com docentes e discentes, a fim de levantar dados sobre o processo ensino-aprendizagem. As docentes apresentaram experiência no ensino e pesquisa, e reconheceram que o elevado número de alunos prejudica o ensino-aprendizagem. Os discentes referiram dificuldades na elaboração de diagnóstico de enfermagem e no contato inicial com cliente hospitalizado, ocasionando insegurança e rejeição à prática da disciplina. El objetivo es el análisis de la enseñanza de la semiología a través de los programas de estudio de la disciplina en cinco universidades estatales y el análisis de una universidad publica. Se observaron tres grupos y se realizaron reuniones con docentes y estudiantes con el fin de recolectar los datos sobre el proceso de enseñanza aprendizaje. Los maestros introducen la experiencia en la enseñanza y en la investigación y reconocieron que el elevado numero de estudiantes, perjudica el proceso enseñanza-aprendizaje. Los estudiantes refieren dificultades en la elaboración del diagnostico de enfermería y en el contacto inicial con el cliente hospitalizado, ocasionando inseguridad y rechazo a la práctica de la disciplina. Opresente trabalho enfatiza os aspectos da conduta ética e moral que permeiam o espaço social das famílias e de seus membros. Aborda questões e princípios necessários de serem trabalhados em família, tendo em vista um conviver saudável, melhorando o nível de satisfação pessoal, o que inclusive poderá contribuir para reduzir os eventos de violência própria e com os demais que compartilham o espaço “mundus vivendi” no ambiente familiar e social.

Report a leaning-teaching experience process. The objective was to analyze the leaning-teaching process based on breast cancer control activities and evaluation the lack of knowledge of the team. It was descritive study where they appied three steps from Saul (1988). Was developed – describing reality; reality criticism and collective creation. It was evaluation that the leaning-teaching process improve group participation by considering the own reality as hallmark of discussions. The participants presented knowledge lack in management, technical practices, difficult of dealing with local culture values as well. Relata-se experiência de ensino-aprendizagem tendo como objetivo analisar esse processo nas ações de controle do câncer de mama e avaliar déficit de conhecimento dos participantes acerca dessas ações. Trata-se de um estudo descritivo em que vivenciou-se os momentos de descrição da realidade, crítica da realidade e criação coletiva, propostos por Saul (1988). Constatou-se que quando o processo ensino-aprendizagem envolve os participantes e valoriza a realidade como aspecto norteador das discussões, obtém-se participação efetiva; os sujeitos apresentaram déficit de conhecimento gerencial, técnico e dificuldades para lidar com os valores culturais locais. Se describe una experiencia de enseñanza–aprendizaje con los objetivos de analizar el proceso de enseñanza-aprendizaje en las acciones de control del cáncer de mama y evaluar déficit de conocimiento de los participantes, acerca de estas acciones. Se trata de un estudio descritivo en que se vivenciaron los momentos de descripción y critica de la realidad y creación colectiva, propuestos por Saul (1988). Se constató que cuando el proceso enseñaza-aprendizaje favorece la participación grupal y considera la propia realidad como aspecto orientador de las discusiones, se obtene cambios concretos. Los participantes presentaron déficit de conocimiento gerencial, técnico y dificultades para lidiar con los valores culturales locales.

Potencializar recursos educativos para mediar ações de educação em saúde, na prevenção da AIDS tem se constituído um desafio para educadores de saúde. Pela inegável importância desse tipo de material nos espaços propiciadores de aprendizagem, surgiu a proposta com o objetivo de criar um percurso metodológico para elaboração de jogos educativos, com vistas a mediar processos de educação e prevenção do HIV/AIDS. O estudo se desenvolveu a partir das idéias contidas nos pressupostos da problematização, adaptadas do "método do arco" de Charles Maguerez, com operacionalização em instituições públicas de saúde e educação, no Estado do Ceará, englobando uma população de escolares (180) na faixa etária de 13 a 19 anos, no período de 1995 a 1997. A coleta de dados se deu durante a realização de 04 oficinas, de acordo com o esquema elaborado. Os resultados envolvendo o significado do jogo, ambiente onde se insere e realidade social com abrangência dos sujeitos durante as jogadas, foram problematizados pelos adolescentes e serviram para a formulação de 03 jogos educativos (memorAIDS, baralhAIDS e dominAIDS). As conclusões apontam a proposta como propiciadora do processo participativo, valorizando, em todo o seu percurso, o desenvolvimento da natureza intelectiva e empreendedora, pela mobilização de capacidades humanas e exercício de associações do universo de exigências relacionados à epidemia de AIDS. Por sua clareza, postulou-se, cientificamente, sua aceitação como percurso metodológico apropriado para elaboração de jogos educativos. The development of educational resources to mediate actions in health education and AIDS prevention has been a challenge to health's educators. Due to the undeniable importance of this kind of material in environments favourable to learning, the authors created a methodological proposal to be used in the elaboration of educative games with the purpose of mediating educational measures and the prevention of HIV/AIDS. For theoretical framework, the study relied on the ideas/premises about problem solving, adapted from Charles Manguerez's "arch method", which where put into practice in public schools and health institutions in the state of Ceara, having as study population a group of students (180), aged 13 to 19 years, from the period of 1995 to 1997. Data collection occurred during four workshops, according to a predefined scheme. The results dealing with the meaning of the game, place of action and social environment were obtained from the teenagers while they were engaged in the games and further analysed by them, resulting in the formulation of (03) educative games (memor AIDS, baralhAIDS and dominAIDS). The authors conclude that the proposal is favourable to participatory action, encouraging during its entire process the development of intellectual and creative skills, by way of mobilizing human capacities and exercising associations related to the AIDS epidemic. Due to its clarity, the authors consider the proposal scientifically acceptable as a methodological guideline for the elaboration of educative games. Por la innegable importancia de ese tipo de material en los espacios que proporcionan ese aprendizaje, surgió la propuesta, con el objetivo de crear un recorrido metodológico para elaborar juegos educativos para intermediar procelos de educación y prevención del SIDA. El estudio se desarrolló a partir de los presupuestos de la problematización, que se adaptan al "método del arco" de Charles Maguerez, con operacionalización en instituciones públicas de salud y educación, en el Estado de Ceará, y engloban una población de escolares (180) entre los 13 y 19 años, desde 1995 hasta 1997. La recogida de datos ocurrió durante la realización de 04 oficinas, de acuerdo al esquema elaborado. Los resultados que involucraban el significado del juego, ambiente donde se insiere y realidad social de los sujetos involucrados durante las jugadas, fueron problematizados por los adolescentes y sirvieron para la formulación de 03 juegos educativos (memorSIDA, barajaSIDA y domiSIDA). Las conclusiones apuntan la propuesta como propiciadora del proceso participativo y valora, durante todo su percurso, el desarrollo de la naturaleza intelectiva y emprendedora, la mobilización de capacidades humanas y el ejercicio de asociaciones del universo de exigencias que se relacionan a la epidemia del SIDA. Por su claridad, se ha postulado científicamente su aceptación, como percurso metodológico apropiado para la elaboración de juegos educativos.

O livro divulga a experiência da integração graduação e pós-graduação desenvolvida no Programa de pós-graduação em Enfermagem da Universidade Federal do Ceará a partir das disciplinas Estudo Crítico de Programas de Enfermagem e Metodologia do Ensino em Enfermagem oferecidas no primeiro e segundo semestres do Curso de Mestrado.

A necessidade de compreender o significado da nutrição parenteral para o cliente no contexto hospitalar surgiu das nossas inquietações diante do cuidado de enfermagem. Realizaram-se observações participantes, entrevistas e anotações de campo junto a sete clientes hospitalizados, que se encontravam sob nutrição parenteral. O sentido das investigações foi centrado na indagação: como você está se sentindo com a nutrição parenteral? Os dados foram analisados numa abordagem interacionista, mostrando as dificuldades enfrentadas pelos clientes na assistência hospitalar, como percebiam eles a nutrição parenteral e que significados eram atribuídos a essa terapêutica. O estudo revelou os sentimentos, comportamentos e atitudes relacionados aos aspectos biológico, emocional e sócio-cultural do cliente, ligados à nutrição parenteral, demonstrando, também as limitações impostas ao cliente sob esse tratamento, bem como as dificuldades na assistência de enfermagem. The need to understand the meaning of parenteral nutrition to the client in hospital context has come from my personal questioning on nursing care. Participative observations have been made as well as interviews and diligences with seven clients in hospital, who have been having parental nutrition. These information have been analysed through interactive approach, showing the situation faced by patients who have hospital assistance; how they say parental nutrition within this context; and what meaning has been given to it. This study revealed feelings, behaviours and attitudes related to the biological, emotional and social cultural aspects of the patient. All of them have been linked to parental nutrition. The limits faced by parental nutrition and its obstacles in the nursing assistance were finally understood.

Background The lack of information on the care for sexually transmitted infections (STI) associated syndromes may contribute for its non-inclusion as prevention and control strategy for STI in Brazil. This study aims to analyze the cases of STI – Associated Syndromes assisted in primary health care center in a city in Northeast Brazil associating them with socio-demographic and behavioral variables. Methods This is a retrospective study that analyzed 5148 consultation forms and medical records of patients assisted in a primary health care center who presented at least one genital syndrome from 1999 to 2008. Was considered as dependent variables the genital syndromes and serologies for syphilis and HIV and as independent variables the socio-demographic and behavioral aspects. It was used Pearson’s chi-square test to analyze the differences between the categorical variables, with a significance level of 5%. It was performed a multivariate analysis through the multivariate logistic regression model with the variables with p Results The most frequent syndromes were vaginal discharge and/or cervicitis (44%) and genital wart (42.2%). Most people were between 20 and 39 years old (70%) and women (74.2%). Genital ulcer was most prevalent among men (OR = 2.67; CI 95% 1.99-3.58) and people who studied more than eight years (OR = 1.33; CI 95% 1.00-1.75) and wart prevailed among men (OR = 3.92; IC 95% 3.36-4.57), people under 29 years old (OR = 1.81; CI 95% 1.59-2.07) and who studied more than eight years (OR = 1.75; CI 95% 1.54-1.99). The Venereal Disease Research Laboratory (VDRL) was positive in 7.3% of men and in 7.1% of women and the Anti-HIV in 3.1% of men and 0.7% of women. Conclusion Vaginal discharge was the most frequent syndrome assisted in primary health care, followed by genital wart. The high prevalence of genital wart justifies the greater effort for the proper follow-up of these cases. Men presented more genital wart and ulcer and reported having more sexual partners, showing their need for a greater access and inclusion in health activities developed in primary health care in Brazil.

Estudo transversal, de campo, objetivando identificar características demográficas, socioeconômicas e de saúde reprodutiva de população atendida em planejamento familiar na Estratégia Saúde da Família do interior do Ceará e verificar associação entre estas variáveis. Participaram 279 mulheres das Regionais de Saúde de Maracanaú, Canindé, Quixadá e Sobral, abrangendo 16 municípios, e 88 áreas adstritas. O n amostral foi calculado com base na população de mulheres em idade fértil nas referidas regionais (N=403.417), usando nível de significância 5%, erro amostral 6% e proporção P 50%, aplicados na fórmula para populações infinitas. Os dados foram coletados de novembro de 2005 a março de 2006. Utilizou-se entrevista estruturada. Os dados foram processados no Statistical Package for Social Science (SPSS), versão 11.0 for Windows. Realizou-se análise univariada e bivariada pelo teste do x2 (Qui-Quadrado), fixado nível de significância de 5%. Identificou-se baixo percentual de adolescentes frequentando serviço de planejamento familiar (7,2%); escolaridade inversamente proporcional ao número de multíparas, de abortos e de partos em idade precoce; número de filhos foi semelhante para mulheres de áreas urbanas e rurais; baixa renda apresentou associação estatística com idade precoce do primeiro parto (p=0,055), porém não houve associação com número de partos, de abortos e intervalo gestacional menor de dois anos (p>0,05). A atenção em planejamento familiar no interior cearense tem desafios a serem suplantados, necessitando conjugar políticas de saúde, educação e trabalho para minimizar desigualdades socioeconômicas, promovendo perfil equânime de saúde reprodutiva.

Resumo Analisou-se a utilização de protocolos na atenção à saúde de mulheres em situação de violência sexual sob a ótica de profissionais. Estudo qualitativo com 18 instituições e 140 profissionais entrevistados em Fortaleza e Rio de Janeiro. Diante dos dados organizados os núcleos de sentido apontaram para as potencialidades na utilização de protocolos; limites a serem enfrentados na adoção de protocolos; e necessidade da construção de protocolo e de formação profissional. Os resultados mostraram pontos de convergência e divergência entre os dois municípios em relação à utilização de protocolos na atenção à saúde de mulheres em situação de violência sexual. No Rio de Janeiro é reconhecida a adoção de protocolos nos serviços pela maioria dos entrevistados e a existência de um fluxo estabelecido, envolvendo a atuação das diversas categoriais profissionais. Em Fortaleza, os participantes de apenas uma instituição mencionaram a utilização de protocolos e fluxos internos de atendimento. Nas duas capitais observa-se a fragilidade da qualificação profissional para atuar junto às mulheres em situação de violência sexual decorrente da limitada abordagem durante a graduação das profissões de saúde, agravada pela falta de treinamento nos serviços. Abstract The use of protocols in health care for women suffering from sexual violence was analyzed from the standpoint of health professionals. It involved a qualitative study conducted in 18 institutions with 140 professionals interviewed in Fortaleza and Rio de Janeiro. After organization of the data, the clusters of meaning pointed to the potential benefits of the use of protocols; limits to be faced in the adoption of protocols; and the need for protocol development and professional training. The results revealed points of convergence and divergence between the two municipalities regarding the use of protocols in health care for women suffering from sexual violence. In Rio de Janeiro, the adoption of protocols in the services by the majority of interviewees and the existence of an established reporting system are acknowledged and involve the work of professionals from different areas. In Fortaleza, the participants of only one institution mentioned the use of protocols and an internal reporting system. In both capitals, professional qualifications to work with women suffering from sexual violence is inadequate due to the limited coverage of the issue during the undergraduate years of the healthcare professions and the lack of training in the health services.

Background Congenital syphilis is a major public health concern, even after the implementation of intervention protocols in several countries. This study aimed to analyze the prevalence and socio-demographic, behavioral and institutional factors associated with syphilis in parturient women attending public maternity hospitals in Northeast, Brazil. Methods A cross-sectional study was conducted from June to September 2010 with a proportionate stratified sampling of 222 parturient women using a structured questionnaire. The study analyzed socio-demographic, behavioral and institutional variables. The structured questionnaire was conducted with parturient women and complementary information was obtained through hospitals records, admission forms and prenatal cards. Data were stored using the Statistical Package SPSS version 18. A descriptive statistical analysis was performed using frequency distribution, central tendency and measures of spread for the variables. A bivariate analysis was done using chi square test and Fisher’s exact test, with a significance level of 5% and a 95% confidence interval, in order to analyze the relation between the variables and risk factors for syphilis. The multivariate logistic regression analysis was done in the statistical package STATA, version 11.0. Results The prevalence of syphilis in parturient women was 7.7%. The bivariate analyses showed that the rate was higher among women who: were from Fortaleza (p = 0.019), studied for less than nine years (p = 0.044), had more than one sexual partner in life (p = 0.021), did not live with partner (p = 0.022), used illegal drugs (p

Procuramos nesta pesquisa, trabalhar a questão da violência voltada à mulher, em especial à mulher detenta. Para tanto, sentimos necessidade de buscar referenciais teóricos que nos desse subsídios para compreensão do significado da violência generalizada, a doméstica e a sexual e a vulnerabilidade às IST- aids e sua articulação com as relações de gênero. Isto nos levou a traçar os seguintes objetivos: fazer uma revisão da literatura sobre as questões mais amplas que envolvem a violência generalizada, e particularmente a sexual; investigar qual a percepção que as mulheres detentas têm sobre a violência, detectando suas possíveis experiências nesse sentido e conseqüentes riscos de infecção pelo IST/HIV-aids; trabalhar, conjuntamente com elas, um programa educativo, desenvolvendo ações e intervenções, visando ao atendimento das questões em apreço, incluindo a elaboração de cartilha sobre violência, IST, aids e drogas, destinada a este público alvo. Para tanto, desenvolvemos um estudo exploratório-descritivo através da pesquisa-ação, fundamentada numa visão humanitária. Priorizamos a abordagem qualitativa cuja análise se deu por categorização – valorizando a subjetividade, ao favorecer a compreensão e a interpretação da fala dos sujeitos pesquisados. Trabalhamos com 15 mulheres detentas, de uma Cadeia Pública Feminina do interior paulista. As técnicas utilizadas foram: a entrevista individual, semi-estruturada, tendo como instrumento, questionário com questões norteadoras; e a observação participante. Os resultados evidenciaram que a violência sexual é repudiada e tida como geradora de marcas psíquicas e emocionais à mulher. Sete (7) do total de pesquisadas, sofreram algum tipo de violência sexual; em cinco (5) casos, os ofensores eram conhecidos. Duas (2) engravidaram de seus ofensores sendo que uma delas ainda se infectou pelo HIV. Sentimentos de rejeição, humilhação, revolta, baixa auto-estima e medo, foram descritos pela maioria delas. Muitas mulheres na delinqüência são agredidas sexualmente, por parceiros, tornando-se mais vulneráveis aos riscos de contágio às IST-aids e a gravidez. Fazem-se necessárias, ações educativas preventivas à violência de gênero e as temáticas interligadas a essa questão, merecendo sejam implantados programas especiais educativos, exames e controle da saúde sexual e reprodutiva da mulher detenta, sob a supervisão de profissionais de saúde gabaritados. The purpose of this study was to understand the violence against women that are prisoners. Therefore, the author searched for theoretical references that helped her to understand generalized, domestic and sexual violence as well as the vulnerability regarding STD – AIDS and the relationship of violence and gender. As immediate goals, the author aimed at reviewing the literature on questions involving generalized violence and particularly sexual violence and investigating the perception of women prisoners about violence in order to find out possible risks to STD and AIDS infection. The author also had the goal to work with these women through an educative program, developing actions and interventions, including the elaboration of a folder about violence, STD, AIDS and drugs. Thus, this was an exploratory and descriptive study, through action research based on an humane view. The author prioritized a qualitative approach that occurred through a categorization – valuing the subjectivity and favoring the understanding and interpretation of the subjects’ speech. The author worked with 15 women prisoners of a Public Prison located at the interior of the state of São Paulo, Brazil. The techniques used were: individual semi-structured interviews, with a questionnaire composed by guiding questions; and participant observation. Results showed that sexual violence results in psychical and emotional consequences for women. Seven women were victims of some type of violence; in five cases, the aggressors were known. Two women got pregnant and one of them was infected by HIV. Feelings of rejection, humiliation and low self-esteem were described by the majority of the subjects. These women were sexually harassed by their partners and are vulnerable to the risks of being infected by HIV and getting pregnant. As a result, the author recommends educative actions that will prevent violence and the themes related to this problem, through the implementation of special programs, examinations regarding their sexual and reproductive health of the women prisoners, under de supervision of qualified health professionals.

O diagnóstico do câncer de uma maneira geral, em particular do câncer de mama, tem o potencial de provocar desequilíbrio psicosocial não somente nas pessoas acometidas pela doença, mas também no contexto familiar. Assim, objetivou-se neste estudo identificar as percepções de familiares sobre a dinâmica familiar após o câncer de mama, bem como as diferenças quanto: composição familiar, gênero, idade, nível educacional e ocupação. O estudo seguiu as premissas da pesquisa tipo exploratória-descritiva. Participaram do estudo 23 familiares constituintes de 10 famílias de mulheres com câncer de mama. Utilizou-se como procedimento metodológico para coleta de dados, entrevistas individuais gravadas e transcritas na íntegra. Analisaram-se os dados por meio de análise de conteúdo, buscando identificar unidades temáticas relacionadas às dimensões da dinâmica familiar, conforme descrito por Barnhill. Os resultados revelaram que os familiares independente da idade, nível educacional e ocupação, mostraram aspectos positivos da dinâmica familiar. Quanto ao gênero feminino, este na visão dos maridos e filhos coloca-se em posição de destaque na reorganização da unidade familiar. Os achados mostraram que o câncer de mama em um membro familiar tem a capacidade de provocar alterações na dinâmica familiar; por outro lado, as famílias utilizaram potenciais de cada membro, em particular, na busca da estabilidade familiar. The diagnosis of cancer, in general, and of breast cancer in particular has the potential to cause a psychosocial instability not only in the person with the disease, but also in the family context. Therefore, the author aimed at identifying the perceptions of family members about the family dynamics after breast cancer, as well as the differences regarding: family composition, gender, age, literacy level and occupation. This was an exploratory and descriptive research with 23 family members of 10 families of women with breast cancer. In order to collect data, the author used individual interviews that were recorded and transcribed. Data were analyzed through content analysis, aiming at identifying the thematic units related to the family dynamics as described by Barnhill. The results revealed that the family members, independently of their age, literacy level and occupation, showed positive aspects of the family dynamics. Regarding the feminine gender, this in the view of husband and children was placed in an important position in the reorganization of the family unit. The findings showed that the breast cancer in a family member causes alterations in the family dynamics and, on the other hand, the families use the potentials of each member, in particular, in the search for family stability.

Este é um estudo qualitativo que teve como objetivo conhecer junto a casais como a prática sexual é exercida após a mulher ter tido o câncer de mama, utilizando-se como referencial teórico as relações de gênero e, como metodologia, a Teoria Fundamentada em Dados. Por meio de entrevistas com nove casais, em que a mulher havia tido um diagnóstico de câncer de mama, obtivemos os dados que após análise permitiu a criação de dois diagramas; 1- a mama após o câncer: um olhar - órgão que ficou afetado pela cirurgia, independente de ter ocorrido a mastectomia e que ficou longe das carícias no ato sexual. 2- O exercício da sexualidade dos cônjuges - nos homens o desejo sexual não diminuiu pela doença da esposa, mas as relações sexuais, sim, em razão de a mulher não estar disposta para o ato, principalmente no período de quimioterapia. As relações de gênero estiveram presentes no desenrolar de cada fase da doença e do tratamento, sendo que os papéis sexuais se mantiveram dentro dos padrões de nossa cultura. A desinformação masculina foi maior do que a feminina, mas os casais utilizaram alguma forma de enfrentamento. Os dados permitiram verificar que muitos mitos, como o abandono da mulher pelo homem, a nudez da mulher, o encerramento da vida sexual, não acontecem se o casal já tiver realizado uma construção de vida a dois. This is a qualitative study that had as objective knows close to couples as the sexual practice is exercised after the woman to have had the breast cancer, being used as theoretical reference the gender relationships and, as methodology, the Grounded Theory. Through interviews with nine couples, in that the woman had had a breast cancer diagnosis, we obtained the data that allowed us the creation of two diagrams after analysis; 1 - the mamma after the cancer: a glance - organ that was affected for the surgery, independent of having happened the mastectomy and that it was far away from the caresses in the sexual action. 2 - the exercise of the spouses' sexuality - in the men the sexual desire didn't decrease for the wife's disease, but the sexual relationships, yes, in the woman's reason not to be disposed for the action, mainly in the chemotherapy period. The gender relationships were present in uncoiling of each phase of the disease and of the treatment, and the sexual papers stayed inside of the patterns of our culture. The masculine disinformation was larger than the feminine, but the couples used some form to face it. The data allowed to verify that many myths, as the woman's abandonment for the man, the woman's nakedness, the finishing of the sexual life, does not happen if the couple has already accomplished a life construction for two.
