Abstract

A miniature endomicroscope is presented that combines a large field-of-view (up to 1.7 mm) for OCT-imaging and a high-resolution mode with 360 μm field-of-view (NA = 0.5) for multi-photon fluorescence or OCT imaging. The 4.7x magnification variation is achieved by the axial positioning of an inner micro-optical lens group using an integrated electro-magnetic z-actuator. A reverse fiber-optic piezotube-scanner with minimized length is employed for the image acquisition by resonant spiral scanning. With the probe diameter of 2.7 mm and a rigid length of about 60 mm, the approach may pave the way to clinical applications of these two modalities in a single probe.

Abstract

Plant transpiration links physiological responses of vegetation to water supply and demand with hydrological, energy, and carbon budgets at the land–atmosphere interface. However, despite being the main land evaporative flux at the global scale, transpiration and its response to environmental drivers are currently not well constrained by observations. Here we introduce the first global compilation of whole-plant transpiration data from sap flow measurements (SAPFLUXNET, https://sapfluxnet.creaf.cat/, last access: 8 June 2021). We harmonized and quality-controlled individual datasets supplied by contributors worldwide in a semi-automatic data workflow implemented in the R programming language. Datasets include sub-daily time series of sap flow and hydrometeorological drivers for one or more growing seasons, as well as metadata on the stand characteristics, plant attributes, and technical details of the measurements. SAPFLUXNET contains 202 globally distributed datasets with sap flow time series for 2714 plants, mostly trees, of 174 species. SAPFLUXNET has a broad bioclimatic coverage, with woodland/shrubland and temperate forest biomes especially well represented (80 % of the datasets). The measurements cover a wide variety of stand structural characteristics and plant sizes. The datasets encompass the period between 1995 and 2018, with 50 % of the datasets being at least 3 years long. Accompanying radiation and vapour pressure deficit data are available for most of the datasets, while on-site soil water content is available for 56 % of the datasets. Many datasets contain data for species that make up 90 % or more of the total stand basal area, allowing the estimation of stand transpiration in diverse ecological settings. SAPFLUXNET adds to existing plant trait datasets, ecosystem flux networks, and remote sensing products to help increase our understanding of plant water use, plant responses to drought, and ecohydrological processes. Abstract

A new periodontitis classification scheme has been adopted, in which forms of the disease previously recognized as "chronic" or "aggressive" are now grouped under a single category ("periodontitis") and are further characterized based on a multi-dimensional staging and grading system. Staging is largely dependent upon the severity of disease at presentation as well as on the complexity of disease management, while grading provides supplemental information about biological features of the disease including a history-based analysis of the rate of periodontitis progression; assessment of the risk for further progression; analysis of possible poor outcomes of treatment; and assessment of the risk that the disease or its treatment may negatively affect the general health of the patient. Necrotizing periodontal diseases, whose characteristic clinical phenotype includes typical features (papilla necrosis, bleeding, and pain) and are associated with host immune response impairments, remain a distinct periodontitis category. Endodontic-periodontal lesions, defined by a pathological communication between the pulpal and periodontal tissues at a given tooth, occur in either an acute or a chronic form, and are classified according to signs and symptoms that have direct impact on their prognosis and treatment. Periodontal abscesses are defined as acute lesions characterized by localized accumulation of pus within the gingival wall of the periodontal pocket/sulcus, rapid tissue destruction and are associated with risk for systemic dissemination.

Abstract

The bidentate aromatic adsorbate, 5-(octadecyloxy)-1,3-benzenedimethanethiol (R1ArmDT), with a specific design of extended S-S distance and a geometric constraint to resist cyclic disulfide formation was synthesized. The film formation and thermal stability of self-assembled monolayers (SAMs) derived from R1ArmDT were investigated and compared to those of SAMs derived from an analogous bidentate dithiol 2-(4-(octadecyloxy)-phenyl)propane-1,3-dithiol (R1ArDT), in which the two sulfur atoms can readily form a cyclic disulfide upon reductive elimination from the surface. Although the SAMs derived from R1ArmDT were less densely packed than those derived from R1ArDT, as judged by the data obtained by X-ray photoelectron spectroscopy and polarization modulation infrared reflection absorption spectroscopy, the SAMs derived from R1ArmDT were markedly more thermally stable than those derived from R1ArDT. The greater thermal stability of the R1ArmDT SAMs can be rationalized on the basis of the structure of the bidentate R1ArmDT headgroup, in which the two pendant sulfur atoms cannot access each other intramolecularly to form a cyclic disulfide upon reductive elimination from the surface.

Abstract

The outcome of assessments of the biodiversity of a taxonomic group often depend on the sampling method. The choice of an adequate method is especially important for biomonitoring purposes. In this study, the effectiveness of two methods of sampling syrphids (Diptera: Syrphidae) is compared: observation plot method vs. line transect, both sampled by sweep netting. Altogether, 18 meadows were selected in three mountain regions in the Austrian and Swiss Alps. We recorded a significantly higher abundance and richness of syrphids using the observation plot method than the line transect method in 2015. Comparing data for one region recorded in 2015 and 2016, similar results were obtained. Syrphid species assemblages were affected by sampling method in both years. More syrphid species and individuals were recorded using the observation plot method, which makes it more suitable for studies aiming at comparing differences in the numbers of adult syrphids in different grassland habitats.

Abstract

Fisheries development has long been recognized as a means of achieving greater economic independence for Pacific 'microstates.' The Republic of Kiribati is particularly well endowed in marine resources, largely due to its extensive Exclusive Economic Zone (EEZ). However, the extraction of those resources rests mainly with distant-water fishing nations (DWFNs) who pay a license fee. This feature of a 'MIRAB' economy, which encompasses elements of migration, remittances, aid, and bureaucracy, only marginally benefits Kiribati. Government has therefore looked to develop its own fishery industry and begun to focus on inshore fisheries and aquaculture projects to diversify Kiribati's meager exports. In addition, commercial artisanal fishing is seen as a means of creating local employment. With increasing human populations, urban drift, more-efficient extractive technologies, and expanding market opportunities, conflicts over resource ownership may become more acute. Better resource management, together with the implementation of a remodeled marine tenure system, will need to take priority if fisheries development is to be sustainable.

283. Quality Initiative to Improve Postdischarge Care for Patients on Outpatient Parenteral Antimicrobial Therapy.

Abstract

Background Patients discharged from the hospital on outpatient parenteral antimicrobial therapy (OPAT) require close monitoring, including weekly blood tests and an early posthospital follow-up visit. However, because patients often receive OPAT in a separate healthcare system from where they received inpatient care, the OPAT plan often fails, with less than 75% of OPAT patients receiving the recommended laboratory monitoring. We sought to determine whether changing our inpatient OPAT documentation method would improve postdischarge care. Methods As a quality improvement initiative, we conducted 2 Plan-Do-Study-Act interventions on our OPAT documentation. Our first intervention was to create a standardized OPAT Progress Note, and our second was to turn that note into a SmartForm (Epic) with discrete fields for the key information. We examined the effects of these changes on the rate of completion of recommended laboratory monitoring, attendance at outpatient follow-up visits, and 30-day readmission rates. Results Changing our documentation to a standardized Progress Note and then to a SmartForm with discrete fields led to an increase in the proportion of patients with a serum creatinine checked within 10 days of discharge (from 63% to 71% to 73%) and who attended an infectious disease clinic visit within 3 weeks of discharge (from 21% to 36% to 47%). However, the rate of readmissions for OPAT-related problems did not change, nor did a composite outcome of 30-day mortality/unplanned readmission. BackgroundIn pandemic situations, digital contact tracing (DCT) can be an effective way to assess one’s risk of infection and inform others in case of infection. DCT apps can support the information gathering and analysis processes of users aiming to trace contacts. ObjectiveWe pursued three goals: (1) to analyze how automation-related user experience (ie, perceived trustworthiness, traceability, and usefulness) relates to user behavior toward a DCT app, (2) to contextualize these effects with health behavior factors (ie, threat appraisal and moral obligation), and (3) to collect qualitative data on user demands for improved DCT communication. MethodsSurvey data were collected from 317 users of a nationwide-distributed DCT app during the COVID-19 pandemic after it had been in app stores for >1 year using a web-based convenience sample. We assessed automation-related user experience. In addition, we assessed threat appraisal and moral obligation regarding DCT use to estimate a partial least squares structural equation model predicting use intention. To provide practical steps to improve the user experience, we surveyed users’ needs for improved communication of information via the app and analyzed their responses using thematic analysis. Hyperbaric oxygen may be beneficial for patients with carbon monoxide poisoning, according to a study of 228 patients. Hyperbaric oxygen is oxygen that is delivered under high pressure. Three treatments within 24 hours can reduce the risk of intellectual deficits caused by carbon monoxide.
