To study the prevalence of headache in patients with systemic lupus erythematosus (SLE) and normal population as well as associated conditions.Forty nine SLE patients and 50 controls were analyzed for presence of headaches (tensional and migraine). In the SLE group, we studied the occurrence of Raynaud, teleangiectasis, cutaneous vasculitis, convulsions and antiphospholipid antibodies.Among SLE patients, 42 had headaches (85.7%), 29 with migraine and 13 tensional; on the control group, 28 had headaches (57.1%), 18 migraine and 10 tension type with p=0.0026 for migraine. In SLE patients we did not find any association between migraine and Raynaud (p=0.34), teleangiectasis (p=0.77), cutaneous vasculitis (p=0.63), seizures (p=0.13), aCl IgG (p=0.45), IgM (=0.07) and LAC (p=0.59).Migraine is more prevalent in the SLE population. However, it has no relationship with Raynaud, teleangiectasis, seizures, cutaneous vasculitis and antiphospholipid antibodies.

A granulomatose de Wegener, doença conhecida há aproximadamente 65 anos, continua sendo um dilema para a classe médica, talvez pelo seu polimorfismo clínico, talvez pela carência de métodos diagnósticos. Seu diagnóstico laboratorial repousa no binômio: dosagem do anticorpo anti-neutrófilo em sua fração citoplasmática (ANCA-c) e na obtenção de material para análise anatomopatológica. Descrevemos aqui, o caso de um paciente, em cuja evolução clínica pôde ser observada todo aspecto proteiforme desta doença, chamando atenção para o envolvimento cardíaco. Este último considerado por muitos como pouco usual, manifestou-se clinicamente sob a forma de miocardite, pericardite e de uma massa intracardíaca.

Descreve-se um caso de sindrome de Sneddon associada a presenca de anticorpos antifosfolipides em uma paciente jovem com endometriose. Os autores analisam a associacao destas duas enfermidades.

The aim of the present study was to evaluate the prevalence of anti-cyclic citrullinated peptide (CCP) antibodies in patients with Down's syndrome (DS) previously tested for IgM rheumatoid factor (RF) and to correlate the results with clinical findings. Eighty-eight patients with DS previously tested for IgM-RF were divided into two groups matched for sex and age. Group A consists of 42 RF positive patients and group B of 44 RF negative patients. The presence of anti-CCP antibody was determined using a second-generation enzyme-linked immunosorbent assay. A total of 52.3% (45/86) of DS patients were positive for anti-CCP antibodies. Twenty-four patients (57.1%) of the RF positive group and 21 (47.7%) of the RF negative group presented anti-CCP circulating antibodies. The concordance between both tests was 54.6%. None of the patients had clinical evidence of rheumatoid arthritis or juvenile idiopathic arthritis. Although a high prevalence of anti-CCP antibodies was observed in DS patients, no association has been found presently with clinical disease. Careful follow-up of these patients will be necessary to clarify the real significance of these findings.

To evaluate intra ocular pressure (IOP) among rheumatoid arthritis (RA) patients who are chronic oral glucocorticoid users in low to moderate doses.We have studied 125 subjects: 72 glucocorticoid users and 53 controls. The glucocorticoid users were RA patients treated with 5 to 40 mg/day of prednisone or equivalent. Controls were patients with osteoarthritis or with soft tissue rheumatic syndromes who had never used glucocorticoid orally or locally. The IOP was measured three times with Perkins tonometer in both eyes and the mean value was compared between groups. For statistical analysis we used the mean value between the IOP of both eyes.Among RA glucocorticoid users the mean dose was 9.7 mg of prednisone daily during a mean period of 71.1 months. The IOP of glucocorticoid users was 5.8% higher than controls. This difference did not reach statistical significance. The rise in IOP was not affected by the duration of glucocorticoid treatment or by the dose. No RA patient using oral glucocorticoids was found to have abnormal IOP in this study.Low dose glucocorticoids causes a small (5.8%), non significant increase in intraocular pressure.

INTRODUÇÃO: as manifestações articulares do lúpus eritematoso sistêmico (LES) são extremamente variadas, embora esta variabilidade nem sempre seja devidamente apreciada pelo reumatologista. OBJETIVO: este estudo tem por objetivo estudar a freqüência de aparecimento das diferentes formas de comprometimento articular no LES. Objetivou-se, também, procurar associações entre os diferentes padrões articulares com fator reumatóide (FR) e presença de hiperelasticidade. MATERIAL E MÉTODOS: foram estudados 147 pacientes com LES para padrão de envolvimento articular, presença de FR e grau de hiperelasticidade calculado pelo índice de Beighton. RESULTADOS: na população estudada, 53,1% dos pacientes tinham queixas articulares na apresentação da doença. No decorrer da mesma, esta foi encontrada em 80,3%. Artralgia isolada estava presente em 31,3%, artrite aditiva em 36,1% e artrite migratória em 12,9%. Em 4,8% dos casos, existia artrite deformante sendo rhupus em 2,1 % e artropatia de Jaccoud em 2,7%. Encontrou-se associação positiva entre presença de FR e artrite aditiva (p

FUNDAMENTOS: A esclerose sistêmica é colagenose pouco comum e muito rica em manifestações cutâneas. OBJETIVO: Estudar a prevalência das manifestações cutâneas na esclerose sistêmica em geral e nos seus diferentes subtipos (formas limitada, generalizada e mista). MÉTODOS: Analisaram-se 32 pacientes de esclerose sistêmica (20 com forma limitada, oito com generalizada e quatro com forma mista) quanto à esclerose de pele, fenômeno de Raynaud, cicatrizes estelares, telangiectasias, leucomelanodermia, microstomia, calcinose e prurido. RESULTADOS: Encontraram-se esclerose de pele e fenômeno de Raynaud em 100% dos pacientes; cicatrizes estelares em 65,6%; telangiectasias em 43,7%; leucomelanodermia em 43,7%; microstomia em 31,25%; prurido em 28,1% e calcinose em 12,5%. Não se observaram diferenças entre as formas localizada e difusa da doença, sendo p = 1 para cicatrizes estelares; p = 0,69 para telangiectasias; p = 0,22 para microstomia, p = 1 para calcinose e prurido. A forma mista de doença não diferiu das formas isoladas (limitada e difusa) quanto aos mesmos achados. CONCLUSÕES: As manifestações mais comuns na esclerose sistêmica são a esclerose de pele e o fenômeno de Raynaud, e a mais rara é a calcinose. As três formas apresentam freqüências semelhantes de Raynaud, cicatrizes estelares, microstomia, telangiectasia, calcinose e prurido.

OBJETIVO: Verificar a prevalência de tendinites e/ou entesopatia em pacientes portadores de diabetes mellitus (DM) tipos 1 e 2 e relacionar a presença das mesmas com a idade dos pacientes, tipo de diabetes e níveis de glicemia. MÉTODOS: Foram estudados 149 pacientes com DM (64 homens e 85 mulheres), sendo 12 portadores de DM tipo 1 e 137 de DM tipo 2, e 55 pessoas normais (23 homens e 32 mulheres). Os indivíduos foram submetidos a exame físico para tendinites e entesopatias, dosagem de glicemia de jejum e hemoglobina (Hb) A1C. RESULTADOS: Achou-se tendinite e/ou entesopatia em 3,6% do grupo-controle contra 32,21% nos diabéticos. A freqüência de tendinites e/ou entesopatia no diabético era semelhante nos dois sexos, e não se notou correlação entre a presença dessas e a idade, o tempo de doença e o tipo de DM. No que diz respeito a níveis glicêmicos, apesar de os diabéticos com tendinites apresentarem uma média das glicemias levemente superior à dos sem tendinites, o estudo não demonstrou diferença estatisticamente significativa, acontecendo o mesmo em relação à Hb A1C. CONCLUSÕES: Concluiu-se que pacientes diabéticos apresentam uma maior prevalência de tendinites e/ou entesopatias que a população normal e que estas não dependem da duração da doença nem do tipo de diabetes. A glicemia de jejum e Hb A1C não refletem adequadamente as alterações no metabolismo do colágeno, responsáveis pelas entesopatias e tendinites nesses pacientes. OBJECTIVE: To verify the prevalence of tendinitis and/or enthesopathy in diabetic patients and its relationship to with age, type and duration of diabetes as well as glycemic control. METHODS: We studied 55 normal controls (23 males and 32 females) and 149 diabetic patients (64 males and 85 females); 12 of them with type 1 diabetes and 137 with type 2. We performed physical examination for tendinitis and determination of fasting glycemic levels and hemoglobin (Hb) A1C. RESULTS: We found that 3.6% (n = 2) of normal persons, and 32.31% of diabetic patients (n = 48) had tendinitis and/or enthesopathy. The frequency of tendinitis and/or enthesopathy in diabetic patients was similar in both gender and we could not find a relationship with age, disease duration and diabetes type. Concerning glycemic control, we found that patients with tendinitis had a glycemic medium level higher than those without it, but no significant statistical difference could be demonstrated, happening the same with HbA1C. CONCLUSIONS: We concluded that diabetic patients have a high prevalence of tendinitis and/or enthesopathy, which does not depend on disease duration or diabetes type. Fasting glycemia and HbA1C do not reflect collagen metabolism changes that favor enthesophaties and tendinitis in these patients.

OBJETIVO: O presente estudo teve por finalidade estudar a freqüência de catarata em pacientes reumáticos e usuários crônicos de corticóide, procurando-se correlacionar o seu aparecimento com tempo de uso, dose cumulativa total e doença de fundo. MÉTODOS: Foram estudados 27 pacientes reumáticos usuários crônicos de corticóide, com exame de lâmpada de fenda e calculado o tempo de uso e doses cumulativas de prednisona ou equivalente. RESULTADO: Encontrou-se freqüência de 18,52% de cataratas e não foi possível demonstrar correlação com dose cumulativa do medicamento (p=0,231) ou com o tipo de doença de fundo. Apesar do tempo médio de uso de corticóide ter sido maior entre os pacientes que apresentaram catarata, esta correlação não demostrou significância estatística (p=0,694). CONCLUSÃO: Os autores concluem que esta complicação é relativamente comum e que mais estudos são necessários para melhor entender o processo fisiopatológico implicado na sua formação. PURPOSE: To study cataract prevalence in rheumatic patients who used corticosteroid chronically and to correlate it with time and total corticosteroid dose as well as with the disease that required such treatment. METHODS: We studied 27 rheumatic patients regarding cataract using slit lamp and calculated the total dose of prednisone or equivalent steroids as well as the treatment time. RESULTS: We found an 18.52% prevalence and no correlation with cumulative dose (p=0.231) or with the underlying disease. Although the time of treatment was longer in the group with cataract, no statistically significant difference could be found (p=0.694). CONCLUSION: The authors conclude that cataract is a relativelly common complication of steroid use and that further studies are needed to understand the pathophysiologic process of its formation.

Prothrombotic phenomena are described in leg ulcers of different etiologies. This work attempts to verify the prevalence of anticardiolipin antibodies in such patients. For this purpose, 151 patients with leg ulcers and 150 control patients were studied and it was obtained clinical data and anticardiolipin antibodies titers. Anticardiolipin antibodies were detected in 7,2 % of the patients from the ulcer group against 1.3% of the patients in the control group (p=0.01). Both celiac disease and scleroderma have autoimmune etiology and affect the bowel causing diarrhea. As an association of autoimmune disease in a single individual is not rare, it is important to know if a patient with scleroderma may also have celiac disease. To analyze this we studied 105 scleroderma patients and 97 volunteers for IgA-EmA by indirect immunofluorescence assay. We could not find a higher prevalence of this autoantibody in scleroderma patients. The authors conclude that there is no need to screen scleroderma patients with diarrhea for celiac disease unless there is a clear clinical indication for this. Tanto a esclerodermia como a doença celíaca são doenças de autoimunidade que causam diarreia. Como o agrupamento de doenças autoimunes em único indivíduo não é raro, é importante saber se um individuo com esclerodermia tem maiores chances de ter ou não doença celíaca. Para isso, estudaram-se 105 pacientes com esclerodermia e 97 controles saudáveis para o anticorpo EmA IgA. Não foi possível detectar presença aumentada de EmA IgA em pacientes com esclerodermia. Conclui-se que não existe necessidade de busca ativa de doença celíaca em pacientes com esclerodermia e diarreia, a menos que existam evidências clínicas claras da doença.

BackgroundMannan-binding lectin-associated serine protease 2 (MASP-2) is a key protein of the lectin pathway of complement. MASP-2 levels have been associated with different polymorphisms within MASP2 gene as well as with the risk for inflammatory disorders and infections. Despite its clinical importance, MASP-2 remains poorly investigated in rheumatoid arthritis (RA).MethodsIn this case-control study, we measured MASP-2 serum levels in 156 RA patients, 44 patient relatives, and 100 controls from Southern Brazil, associating the results with nine MASP2 polymorphisms in all patients, 111 relatives, and 230 controls genotyped with multiplex SSP-PCR.ResultsMASP-2 levels were lower in patients than in controls and relatives (medians 181 vs. 340 or 285 ng/ml, respectively, PConclusionsIn this study, we found the first evidence that MASP-2 deficiency might play an important role in the development of RA and articular symptoms among relatives of RA patients.

Background The spondyloarthritides (SpA) represent a group of chronic diseases that affect around 1% of the population. Enthesitis is a frequent clinical complaint in SpA patients that need to be better investigated Objectives To analyze the impact of the entheseal inflammatory involvement in a large Brazilian cohort of SpA patients. Methods A common protocol of investigation was prospectively applied to 1505 SpA patients in 29 centers distributed through the main geographical regions in Brazil. Clinical and demographic variables and disease indexes (BASDAI, BASFI, BASRI, ASQoL) were investigated. MASES (Maastricht Ankylosing Spondylitis Enthesitis Score) was used to investigate the entheseal component. Ankylosing spondylitis (AS) was the most frequent disease (65.4%) that also included psoriatic arthritis (18.4%), undifferentiated SpA (6.7%), reactive arthritis (3.3%), and enteropathic arthritis (3.2%). Results Enthesitis was referred by 54% of the SpA patients, with a mean of 2.12±2.98 enthesis affected. It was present in 53.3% of the AS patients, 54.4% in psoriatic arthritis, and 70.4% in undifferentiated SpA. Background Rheumatic diseases such as ankylosing spondylitis (AS) may be associated with sexual dysfunction. Aim To study erectile function of a group of Brazilian AS patients comparing them with controls. Materials and Methods This was a cross sectional study approved by the local Committee of Ethics in Research. The questionnaire IIEF (International Index of Erectile Function) was applied to 40 AS patients and 40 healthy controls. Objetivo: analisar as variáveis demográficas e clínicas associadas à diminuição da qualidade de vida em uma grande coorte brasileira de pacientes com espondiloartrite (EpA). Métodos: Foi aplicado um protocolo de pesquisa único a 1.465 pacientes brasileiros classificados como tendo EpA de acordo com os critérios do European Spondyloarthropaties Study Group (ESSG), atendidos em 29 centros de referência em reumatologia do Brasil. Foram registradas as variáveis clínicas e demográficas. A qualidade de vida foi analisada por meio do questionário Ankylosing Spondylitis Quality of Life (ASQoL). Resultados: A pontuação média do ASQoL foi de 7,74 (+ 5,39). Ao analisar doenças específicas no grupo de EpA, as pontuações do ASQoL não apresentaram diferença estatisticamente significativa. Os dados demográficos mostraram piores escores de ASQoL associados ao gênero feminino (p = 0,014) e etnia negra (p < 0,001). Quanto aos sintomas clínicos, a dor na região glútea (p = 0,032), a dor cervical (p < 0,001) e a dor no quadril (p = 0,001), estiveram estatisticamente associadas a piores escores no ASQoL. O uso contínuo de fármacos anti-inflamatórios não esteroides (p < 0,001) e agentes biológicos (p = 0,044) esteve associado a escores mais elevados de ASQoL, enquanto outros medicamentos não interferiram nos escores do ASQoL. Conclusão: Nesta grande série de pacientes com EpA, o sexo feminino e a etnia negra, bem como sintomas predominantemente axiais, estiveram associados a uma qualidade de vida reduzida. Regarding clinical symptoms, buttock pain (p = 0.032), cervical pain (p < 0.001) and hip pain (p = 0.001), were statistically associated with worse scores of ASQoL. Continuous use of nonsteroidal anti-inflammatory drugs (p < 0.001) and biologic agents (p = 0.044) were associated with higher scores of ASQoL, while the other medications did not interfere with the ASQoL scores. Conclusion: In this large series of patients with SpA, female gender and African-Brazilian ethnicity, as well as predominant axial symptoms, were associated with impaired quality of life.

The use of anti-TNF-α has been associated with several changes in lipid profile, although some study results are conflicting. The knowledge of this fact is of great importance when one observes at the association between rheumatic diseases and accelerated atherogenesis. The aim of this analysis was search for changes in lipid profile in anti TNF-α users in the population of Southern Brazil and its association with duration of use, indications, patient gender and type of anti-TNF. For this purpose, we studied the profiles of total cholesterol (TC), HDL cholesterol (HDLc), LDL cholesterol (LDLc), atherogenic index (ATI) and triglycerides (TGs) of 58 patients (42 with rheumatoid arthritis and 16 with spondyloarthritis) before and after using this drug for a median of 16.0 months. There were no changes in the levels of TC, HDLc, LDLc and ATI (P=NS). However, there was a significant increase in TG levels (P=0.03). The median difference between first and second TG measurements was 16mg/dL and this increase was not associated with gender, time of use, use indication or type of anti TNF-α (P=NS). It was concluded that the use of anti TNF-α is associated with increased values of TG.

CONTEXT AND OBJECTIVE Accelerated atherosclerosis has become a major problem in rheumatic inflammatory disease. The aim here was to analyze carotid intima-media thickness (IMT) in spondyloarthritis (SpA) patients and correlate this with clinical parameters and inflammatory markers. DESIGN AND SETTING Cross-sectional analytical study at Rheumatology Outpatient Clinic, Evangelical University Hospital, Curitiba. METHODS IMTs (measured using Doppler ultrasonography) of 36 SpA patients were compared with controls. The IMT in SpA patients was associated with inflammatory markers, like erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI); and with clinical parameters, like axial or peripheral involvement, dactylitis, HLA B27, uveitis occurrence, Bath Ankylosing Spondylitis Functional Index (BASFI) and lipid profile. RESULTS The mean IMT in SpA patients was 0.72 ± 0.21 mm; in controls, 0.57 ± 0.13 mm (P = 0.0007). There were no associations with ESR, CRP, BASDAI or clinical data. In univariate analysis, greater IMT was seen in patients with longer disease duration (P = 0.014; Pearson R = 0.40; 95% confidence interval, CI = 0.06 to 0.65); higher triglycerides (P = 0.02; Spearman R = 0.37; 95% CI = 0.03 to 0.64); and older age (P = 0.0014; Pearson R 0.51; 95% CI = 0.21 to 0.72). CONCLUSION SpA patients have a higher degree of subclinical atherosclerosis than in controls, thus supporting clinical evidence of increased cardiovascular risk in rheumatic patients. CONTEXTO E OBJETIVO A aterogênese acelerada tem se tornado um grande problema nas doenças reumáticas inflamatórias. O objetivo foi analisar a espessura da camada íntima-média (ECIM) da carótida em pacientes com espondiloartrite (ES) e relacioná-la com parâmetros clínicos e marcadores inflamatórios. TIPO DE ESTUDO E LOCAL Estudo transversal analítico no Ambulatório de Reumatologia do Hospital Universitário Evangélico de Curitiba. MÉTODOS A ECIM (medida por Doppler) de 36 pacientes com ES foi comparada com controles. A ECIM de pacientes com ES foi associada com marcadores inflamatórios, como velocidade de hemossedimentação (VHS), proteína C-reativa (PCR), Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), e com parâmetros clínicos, como envolvimento axial e periférico, dactilite, HLA B27, ocorrência de uveíte, Bath Ankylosing Spondylitis Functional Index (BASFI) e perfil lipídico. RESULTADOS A ECIM média em pacientes com ES foi de 0,72 ± 0,21 mm, enquanto nos controles foi de 0,57 ± 0,13 mm (P = 0,0007). Não se encontrou associação com VHS, PCR, BASDAI e dados clínicos. Em análise univariada, maior ECIM foi encontrado nos indivíduos com maior duração de doença (P = 0,014; R Pearson = 0,40; 95% intervalo de confiança, IC = 0,06 to 0,65), aumento nos triglicerídeos (P = 0,02; R Spearman = 0,37; 95% IC = 0,03 to 0,64) e maior idade (P = 0,0014; R Pearson 0,51; 95% IC = 0,21 to 0,72). CONCLUSÃO Pacientes com ES têm maior grau de aterosclerose subclínica do que controles, dando suporte às evidências clínicas de aumento de risco cardiovascular em pacientes com doenças reumáticas.

OBJETIVO: Avaliar a espessura da camada médio-intimal da artéria carótida comum em pacientes com e sem esclerodermia e verificar possível associação com sua gravidade. MÉTODOS: Em estudo caso-controle, foram selecionados 30 pacientes com esclerodermia e 30 sem a doença e pareados de acordo com a idade, sexo, hipertensão arterial sistêmica, diabete melito e hipercolesterolemia. Todos os pacientes foram submetidos à avaliação das artérias carótidas pela ultrassonografia vascular de alta resolução e realizada a medida do espessamento da camada médio-intimal das carótidas comuns a 2cm da bifurcação carotídea. Em toda a análise foi considerado o maior valor da camada médio-intimal nas artérias carótidas direita e esquerda. RESULTADOS: A amostra foi composta de 30 pacientes estudados, sendo 29 (96,67%) mulheres e um homem (3,3%) com idade de 17 a 79 anos (média de 48 anos). Nesta amostra existiam 11/30 (36,67%) com hipertensão arterial, 5/30 (16,67%) com diabete melito, 6/30 (20%) com dislipidemia e 2/30 (6,67%) fumantes. Ao comparar a medida do maior risco (espessura máxima entre o lado esquerdo e o lado direito), obteve-se média de 0,77mm para o grupo esclerodermia e valor de 0,70mm para o grupo controle (p=0,212). Ao avaliar a associação entre gravidade da doença e a camada médio-intimal da carótida, não se encontrou associação significativa (p=0,925). CONCLUSÃO: Encontra-se discreto aumento do espessamento da camada médio-intimal da artéria carótida comum em pacientes com esclerodermia, mas sem significância estatística. Com relação à gravidade da doença e o espessamento da camada médio-intimal da carótida comum, não foi verificada diferença. OBJECTIVE: To evaluate the intima-media thickness of the common carotid artery in patients with and without scleroderma; to verify a possible association with disease severity; to assess the relationship of intima-media thickness with known cardiovascular risk factors. METHODS: In a case-control study, were selected 30 patients with scleroderma and 30 without the disease and matched according to age, sex and cardiovascular risk factors such as hypertension, diabetes mellitus and hypercholesterolemia. The age ranged from 17 to 79 years (mean 49). All patients underwent carotid artery evaluation by high-resolution vascular Doppler in order to measure the intima-medial thickness of the carotid 2 cm from the bifurcation. In all the analysis was considered the greatest value of intima-media thickness in right and left carotid arteries. RESULTS:The sample consisted of 30 patients, being 29 (96.67%) women and one man (3.3%). In this sample, 11/30 (36.67%) had high blood pressure, 5/30 (16.67%) had diabetes mellitus, 6/30 (20%) had dyslipidemia and 2/30 (6.67%) were smokers. Comparing the measure of the increased risk (maximum intima-media thickness between the left and right side), was obtained an average of 0.77 mm for group scleroderma and a value of 0.70 mm for the control group (p = 0.21). In assessing the association between disease severity and carotid intima-media thickness, was found no significant association (p= 0.925). CONCLUSION: Was found a slight increase in intima-medial thickness of common carotid artery in patients with scleroderma but without statistical significance. Regarding the severity of the disease and intima-medial thickness of common carotid artery, there was no significant difference.

CONTEXT AND OBJECTIVE: Systemic lupus erythematosus (SLE) is an autoimmune disease with a cyclical clinical course. Evaluation of the clinical activity of this disease is important for choosing the correct treatment. The objective of this study was to analyze the value of beta-2 microglobulin (β2M) serum levels in determining SLE clinical activity.DESIGN AND SETTING: Cross-sectional analytical study conducted at the rheumatology outpatient clinic of a private university hospital.METHODS: 129 SLE patients were studied regarding disease activity using SLEDAI (SLE Disease Activity Index) and cumulative damage using SLICC ACR (SLE International Collaborating Clinics/American College of Rheumatology Damage Index for SLE). At the same time, the β2M serum level, ESR (erythrocyte sedimentation rate), anti-dsDNA (anti-double-stranded DNA) and C3 and C4 complement fractions were determined.RESULTS: β2M levels correlated positively with SLEDAI (P = 0.02) and ESR (P = 0.0009) and negatively with C3 (P = 0.007). Patients who were positive for anti-dsDNA had higher β2M serum levels (P = 0.009).CONCLUSION: β2M levels are elevated in SLE patients with active disease.

Objectives: This study aims to verify the new-onset diabetes after kidney transplant (NODAT) incidence in recipients within 1 year after kidney transplantation from a single center in Southern Brazil and to assess the associated conditions. Subjects and methods: A retrospective study of 258 post-renal transplant patients was performed. Demographic (gender, age, ethnic background) and clinical (origin of graft, associated infections, body mass index (BMI) at transplant time and 6 and 12 months after, causes of renal failure, and comorbidities) data were analyzed. All patients were on tacrolimus, mycophenolate mofetil, and prednisone treatment. Patients with and without NODAT were compared. Results: A NODAT incidence of 31.2% was noted 1 year post transplantation. In the univariate analysis, patients with NODAT were older (p = 0.001), mostly had African–American ethnic background (p = 0.02), and had renal failure secondary to high blood pressure (HBP) (p = 0.001). The group of patients with NODAT also had more incidence of post-transplant HBP (p = 0.01), heart failure (p = 0.02), and dyslipidemia (p = 0.001). Logistic regression showed that African–American ethnic background, post-transplant HBP, and dyslipidemia were independently associated with NODAT. Conclusion: This study shows a NODAT incidence that is greater in patients with African–American ethnic background and that is associated with HBP and dyslipidemia.

Objective To determine the incidence of infections in a population of systemic lupus erythematosus individuals and the characteristics of infections regarding original site, as well as to study the possible associations between infections and treatment. Methods An analytical retrospective study using data from medical charts of systemic lupus erythematosus patients from a single university hospital. A total of 144 patients followed up for five years were included. Data collected comprised age of patients and age at onset of lupus, sex and ethnicity, disease duration before the study period, medications, cumulative dose of prednisone, occurrence of infections and their original site. Inflammatory bowel diseases (Crohn's disease and ulcerative rectocolitis) have extraint- estinal manifestations 25% of the patients, with the most common one being the entero- pathic arthritis.MethodsProspective, observational, multicenter study with patients from 29 reference cen- ters participating in the Brazilian Registry of Spondyloarthritis (RBE), which incorporates the RESPONDIA (Ibero-American Registry of Spondyloarthritis) group. Demographic and clinical data were collected from 1472 patients and standardized questionnaires for the as- sessment of axial mobility, quality of life, enthesitic involvement, disease activity and func- tional capacity were applied. Laboratory and radiographic examinations were performed. The aim of this study is to compare the clinical, epidemiological, genetic, imaging, treat- ment and prognosis characteristics of patients with enteropathic arthritis with other types of spondyloarthritis in a large Brazilian cohort.ResultsA total of 3.2% of patients were classified as having enteroarthritis, 2.5% had spon- dylitis and 0.7%, arthritis (peripheral predominance). The subgroup of individuals with en- teroarthritis had a higher prevalence in women (P < 0.001), lower incidence of inflammatory axial pain (P < 0.001) and enthesitis (P = 0.004). HLA-B27 was less frequent in the group with enteroarthritis (P = 0.001), even when considering only those with the pure axial form. There was a lower prevalence of radiographic sacroiliitis (P = 0.009) and lower radiographic score (BASRI) (P = 0.006) when compared to patients with other types of spondyloarthritis. They also used more corticosteroids (P < 0.001) and sulfasalazine (P < 0.001) and less non- steroidal anti-inflammatory drugs (P < 0.001) and methotrexate (P = 0.001).ConclusionThere were differences between patients with enteroarthritis and other types of spondyloarthritis, especially higher prevalence of females, lower frequency of HLA-B27, associated with less severe axial involvement.

Objetivo: Estudar o perfil nutricional de pacientes com artrite reumatoide (AR). Métodos: Estudo transversal observacional de 102 pacientes com AR. Os pacientes foram estudados para dados clínicos, demográficos, sorológicos, atividade de doença e perfil nutricional. Neste último incluiu-se a medida do índice de massa corporal (IMC), relação quadril/cintura, pregas cutâneas bicipitais (PCB) e sua adequação; prega cutânea tricipital (PCT) e sua adequação e circunferência muscular do braço (CMB) e sua adequação. Estudos de associação foram feitos usando os testes de Fisher e qui-quadrado para dados nominais e teste t não pareado e Mann Whitney para dados numéricos. Para cálculo de correlação usou-se o teste de Spearman. Resultado: Na amostra existiam 14/102 homens e 88/102 mulheres com idade média de 52,1 ± 11,5 anos e duração média de doença de 10,6 ± 7,4 anos. A relação cintura/quadril média era de 0,92 ± 0,07. De acordo com IMC, 30,3% tinham peso normal e 65,5% tinham peso acima do normal. De acordo com PCB, 74,5% eram normais e 25,5% tinham depleção; de acordo com a PCT, 83,3% eram normais e 16,7% tinham depleção. Associação de variáveis nutricionais com gênero, fator reumatoide, nódulos e atividade da doença não mostraram diferenças (p = NS) exceto, por uma relação cintura/quadril menor em indivíduos com nódulos (p = 0,02) e uma correlação modesta da PCT com a duração de doença (p = 0,02; R = 0,22; 95% IC = 0,01-0,40). Conclusão: Existe uma alta prevalência de sobrepeso e obesidade em artrite reumatoide e uma pequena frequência de depleção muscular. Background: Atherosclerosis in Rheumatoid Arthritis (RA) patients may be aggravated by obesity. Objective: To study the nutritional status of patients with RA. Methods: Observational cross sectional study of 102 RA. Patients were studied for clinical, demographic, serologic, activity and nutritional profile. In the latter we included: measurement of body mass index (BMI), waist-hip ratio; bicipital skinfold (BSF) and their adequacy; triceps skinfold measure (TSF) and its adequacy and arm muscle circumference (AMC) and its adequacy. Association studies of nominal data were done using Fisher and chi-square tests and the Mann Whitney and unpaired Student t tests for numerical data. For correlation calculations the Spearman test was used. Results: In the sample there were 14/102 men, 88/102 women with mean age of 52.1 ± 11.5 years and mean disease duration of 10.6 ± 7.47 years. The mean waist-hip ratio was 0.92 ± 0.07. According to BMI 30.3% had normal weight and 65.5% a total weight above normal. According to BSF, 74.5% were normal and 25.5% had depletion of muscular mass; according to TSF, 83.3% were normal and 16.7% depleted. Association of nutritional variables with gender, rheumatoid factor, age, nodules, and disease activity showed no differences (p = NS) except for a lower waist/hip ratio in individuals with nodules (p = 0.02) and a modest correlation of TSF with disease duration (p = 0.02; R = 0.22; 95% CI = 0.01 to 0.40). Conclusion: We found a high prevalence of overweight and obesity in patients with RA and a small frequency of muscle depletion.

OBJECTIVE: To study the relationship of the presence of secondary SS with disease activity, duration in RA. METHODS: Eighty two patients with RA were submitted to Schirmer test, minor salivary gland biopsy, questionnaire on sicca symptoms, DAS-28 4v determination. RESULTS: In this population, 20 (24.3%) patients fulfilled the American-European classification criteria for secondary SS. No relation could be found between the presence of secondary SS and disease activity (p = 0.31) and RA duration (p = 0.95). CONCLUSION: Appearance of Secondary SS in RA patients is independent of RA duration or activity. OBJETIVO: Estudar a associação entre presença de SS secundária e atividade e duração da artrite reumatoide. MÉTODOS: Oitenta e dois pacientes com artrite reumatoide foram submetidos ao teste de Schirmer, biópsia de glândula salivar menor, questionários acerca de sintomas de secura e determinação do DAS28 4v. RESULTADOS: Nesta população, 20 (24,3%) dos pacientes preenchiam os Critérios Americanos Europeus para classificação de SS Secundário. Nenhuma associação foi encontrada entre presençade SS secundário e atividade da doença (p = 0.31) e duração da doença (p = 0.95). CONCLUSÃO: O aparecimento de SS secundário em AR é independente da duração e atividade da AR.

OBJETIVOS: Correlacionar os achados da biópsia transcutânea hepática guiada por ultrassonografia com os dados ultrassonográficos modo B e Doppler da veia hepática direita; comparar os padrões de onda entre os grupos de estudo (hepatopatas) e controle (sadios); e avaliar se o Doppler da veia hepática direita serve como marcador de hepatopatia crônica. MÉTODOS: Foram estudados 38 pacientes portadores de hepatopatia crônica comprovada por sorologia e biópsia (grupo de estudo) e dez pacientes sem hepatopatia sorológica (grupo controle), avaliados pela ultrassonografia modo B e Doppler. Os critérios histológicos foram a classificação da Sociedade Brasileira de Patologia de Hepatite Crônica. RESULTADOS: A ultrassonografia modo B e o Doppler diferenciaram os indivíduos portadores de hepatopatia crônica dos normais (p=0,047). Houve diferença significativa entre o grupo de estudo e o controle na comparação entre os achados histopatológicos, ultrassonográficos modo B e o Doppler nos padrões de onda da veia hepática direita (p=0,001). CONCLUSÃO: Foi possível correlacionar a biópsia hepática com a ultrassonografia modo B e o Doppler da veia hepática direita; os hepatopatas apresentaram alteração no fluxo da veia hepática direita e os normais não, sendo que o padrão de onda nos controles saudáveis foi trifásico e nos hepatopatas bifásico ou monofásico; e o Doppler da veia hepática direita serviu como marcador de hepatopatia crônica. OBJECTIVE: To correlate chronic liver disease diagnosed by transcutaneous liver biopsy guided by ultrasound, with ultrasound findings with B-mode and Doppler of the right hepatic vein; 2) to compare the wave patterns between the study group and the control group; 3) to compare the right hepatic vein Doppler findings with histopathology findings as a possible marker of chronic liver disease. METHODS: Were studied 38 patients with chronic liver disease diagnosed by biopsy and serology (study group) and 10 persons without serologic liver disease (control group), assessed only by ultrasound B-mode and Doppler. The criteria were based on histology classification of the Brazilian Society of Pathology for chronic hepatitis. Chi-square, Fisher's exact and Student t tests were used. RESULTS: The B-mode and Doppler ultrasound were useful in inferring the differentiation between individuals with chronic liver disease from normal. There were significant differences between the study group and the controls when comparing the histopathology findings, ultrasound B-mode and Doppler in relationship to the wave patterns of the right hepatic vein. CONCLUSION: The correlation of liver biopsies with ultrasound B-mode and Doppler of hepatic vein was positive; 2) individuals with liver disease showed alterations in the flow of the right hepatic vein and normal subjects not, being the wave pattern in normal triphasic and in patients with chronic liver disease monophasic or biphasic; 3) Doppler of the right hepatic vein is useful marker for chronic liver disease.

Introduction: Patients with Down syndrome (DS) seem to have higher rates of hyperuricemia, although there are very few studies that address this issue. High levels of uric acid (UA) are an independent risk factor for atherosclerotic heart disease. Objective: To study the prevalence of hyperuricemia in DS children and its distribution according to age. Methods: Retrospective study of 139 DS patients aged 1-14 years for serum UA values considering the normal cut-off values for age. Results: Among the 139 included patients, 70 (50.4%) were males, 69 (49.6%) were females, and the mean age was 5.31 ± 3.49 years (range 1-14 years). In this sample, 39 (28.1%) had serum UA levels above the normal value. We did not find differences in the serum UA according to gender, except in patients in the range of 13-15 years, where males had mean UA levels of 5.75 ± 1.37 mg/dl; and females, of 8.02 ± 0.77 mg/dl (p = 0.02). Conclusion: We found a high rate of hyperuricemia in children with DS that equally affects both genders, except in the age range of 13-15 years, in which it was more common among females. RESUMEN Introducción: Pacientes con síndrome de Down (SD) perecen tener mayor grado de hiperuricemia, aunque hay pocos estudios que traten esa cuestión. Niveles elevados de ácido úrico (AU) son un factor de riesgo independiente para enfermedad cardiovascular aterosclerótica. Objetivo: Estudiar la prevalencia de hiperuricemia en niños y adolescentes con SD y su distribución de acuerdo a la edad. Métodos: Estudio retrospectivo de 139 pacientes con SD y edades entre 1 y 14 años para valores de AU, considerando los puntos de corte normales según edad. Resultados: En los 139 pacientes incluidos, 70 (50,4%) eran del sexo masculino y 69 (49,6%) del femenino; la edad media fue 5,31 ± 3,49 años (rango: 1-14 años). En esta muestra, 39 (28,1%) presentaron niveles séricos de AU por encima de lo normal. No fueron encontradas diferencias en la concentración sérica de AU por sexo, salvo en pacientes en la franja entre 13 y 15 años, en la que los niños presentaron niveles medios de AU de 5,75 ± 1,37 mg/dl y las niñas, de 8,02 ± 0,77 mg/dl (p = 0,02). Conclusión: Encontramos alto índice de hiperuricemia en niños con SD, que afecta igualmente a ambos sexos, salvo en la franja etaria entre 13 y 15 años, en la que fue más común en el sexo femenino. Níveis altos de ácido úrico (AU) são um fator de risco independente para doença cardíaca aterosclerótica. Objetivo: Estudar a prevalência de hiperuricemia em crianças e adolescentes com SD e sua distribuição de acordo com a idade. Métodos: Estudo retrospectivo de 139 pacientes com SD entre 1 e 14 anos de idade, para valores de AU, considerando os valores de corte normais para a idade. Resultados: Dos 139 pacientes incluídos, 70 (50,4%) eram do sexo masculino e 69 (49,6%), do feminino; a média de idade foi de 5,31 ± 3,49 anos (variação de 1-14 anos). Nesta amostra, 39 (28,1%) apresentaram níveis séricos de AU acima do valor normal. Background: Sleep disturbances are common in rheumatoid arthritis (RA) patients and contribute to loss of life quality. Objective: To study associations of sleep quality with pain, depression and disease activity in RA. Methods: This is a transversal observational study of 112 RA patients submitted to measurement of DAS-28, Epworth scale for daily sleepiness, index of sleep quality by Pittsburg index, risk of sleep apnea by the Berlin questionnaire and degree of depression by the CES-D (Center for Epidemiologic Studies Depression scale) questionnaire. We also collected epidemiological, clinical, serological and treatment data. Results: Only 18.5% of RA patients had sleep of good quality. In univariate analysis a bad sleep measured by Pittsburg index was associated with daily doses of prednisone (p = 0.03), DAS-28 (p = 0.01), CES-D (p = 0.0005) and showed a tendency to be associated with Berlin sleep apnea questionnaire (p = 0.06). In multivariate analysis only depression (p = 0.008) and Berlin sleep apnea questionnaire (p = 0.004) kept this association. Conclusions: Most of RA patients do not have a good sleep quality. Depression and risk of sleep apnea are independently associated with sleep impairment. RESUMO Antecedentes: Os distúrbios do sono são comuns em pacientes com artrite reumatoide (AR) e contribuem para a perda da qualidade de vida. Objetivo: Estudar as associações entre a qualidade do sono e a dor, depressão e atividade da doença na AR. Métodos: Estudo observacional transversal com 112 pacientes com AR submetidos à avaliação do DAS-28, escala de Epworth para sonolência diurna, qualidade do sono pelo índice de Pittsburg, risco de apneia do sono pelo questionário de Berlim e grau de depressão pelo questionário CES-D (Center for Epidemiologic Studies Depression). Também foram coletados dados epidemiológicos, clínicos, sorológicos e de tratamento. Resultados: Apenas 18,5% dos pacientes com AR tinham uma boa qualidade do sono. Na análise univariada, um sono ruim medido pelo índice de Pittsburg esteve associado à dose diária de prednisona (p = 0,03), DAS-28 (p = 0,01), CES-D (p = 0,0005) e mostrou uma tendência a estar associado à apneia do sono pelo questionário de Berlim (p = 0,06). Na análise multivariada, somente a depressão (p = 0,008) e a apneia do sono pelo questionário de Berlim (p = 0,004) mantiveram essa associação. Conclusões: A maior parte dos pacientes com AR não tem uma boa qualidade de sono. A depressão e o risco de apneia do sono estão independentemente associados ao comprometimento do sono.

Objectives: To evaluate the frequency of four serum biomarkers in RA patients and their relatives and identify possible associations with clinical findings of the disease. Methods: This was a transversal analytical study. Anti-cyclic citrullinated peptide (anti-CCP), anti-mutated citrullinated vimentin (anti-MCV) and IgA-rheumatoid factor (RF) were determined by ELISA and IgM-RF by latex agglutination in 210 RA patients, 198 relatives and 92 healthy controls from Southern Brazil. Clinical and demographic data were obtained through charts review and questionnaires. Results: A higher positivity for all antibodies was observed in RA patients when compared to relatives and controls (p < 0.0001). IgA-RF was more frequent in relatives compared to controls (14.6% vs. 5.4%, p = 0.03, OR = 2.98; 95% CI = 1.11-7.98) whereas anti-CCP was the most common biomarker among RA patients (75.6%). Concomitant positivity for the four biomarkers was more common in patients (46.2%, p < 0.0001). Relatives and controls were mostly positive for just one biomarker (20.2%, p < 0.0001 and 15.2%, p = 0.016, respectively). No association was observed between the number of positive biomarkers and age of disease onset, functional class or tobacco exposure. In seronegative patients predominate absence of extra articular manifestations (EAMs) (p = 0.01; OR = 3.25; 95% CI = 1.16-10.66). Arthralgia was present in positive relatives, regardless the type of biomarker. Conclusions: A higher number of biomarkers was present in RA patients with EAMs. Positivity of biomarkers was related to arthralgia in relatives. These findings reinforce the link between distinct biomarkers and the pathophysiologic mechanisms of AR. RESUMO Objetivos: Avaliar a frequência de quatro marcadores sorológicos em pacientes com AR e seus familiares e identificar possíveis associações com achados clínicos da doença. Métodos: Estudo analítico transversal. Determinaram-se os níveis de anticorpos antipeptídeo citrulinado cíclico (anti-CCP), anticorpos antivimentina citrulinada-mutada (anti-MCV) e fator reumatoide (FR) IgA por Elisa e de FR-IgM por aglutinação em látex em 210 pacientes com AR, 198 familiares e 92 controles saudáveis do sul do Brasil. Coletaram-se dados clínicos e demográficos por meio da revisão de prontuários e questionários. Resultados: Observou-se maior positividade para todos os anticorpos em pacientes com AR em comparação com os familiares e controles (p < 0,0001). O FR-IgA era mais frequente em familiares quando comparados com os controles (14,6% versus 5,4%, p = 0,03, OR = 2,98; IC95% = 1,11 a 7,98). O anti-CCP foi o biomarcador mais comum entre pacientes com AR (75,6%). A positividade concomitante para os quatro biomarcadores foi mais comum nos pacientes (46,2%, p < 0,0001). Familiares e controles eram positivos em sua maioria para apenas um biomarcador (20,2%, p < 0,0001 e 15,2%, p = 0,016, respectivamente). Não foi observada associação entre o número de biomarcadores positivos e a idade de início da doença, classe funcional ou exposição ao fumo. Em pacientes soronegativos, predominou a ausência de manifestações extra-articulares (MEA) (p = 0,01; OR = 3,25; IC95% = 1,16 a 10,66). A artralgia estava presente em familiares positivos, independentemente do tipo de biomarcador. Conclusões: Um maior número de biomarcadores estava presente em pacientes com AR com MEA. A positividade dos biomarcadores estava relacionada com a artralgia em familiares. Esses achados reforçam a ligação entre os diferentes biomarcadores e os mecanismos fisiopatológicos da AR.

Objective To investigate the prevalence of electrocardiographic changes in patients with spondyloarthritis and to correlate these changes with use of anti-tumor necrosis factor-alpha (TNF-α) drugs and HLA-B27 positivity. Methods Retrospective study including 100 patients diagnosed with spondyloarthritis according to Assessment of SpondyloArthritis International Society (ASAS) criteria and 50 controls. Epidemiological and clinical features, results of inflammatory activity tests, HLA-B27 positivity, and medication use data were extracted from medical records. Disease activity was assessed using the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI). All participants were submitted to electrocardiogram performed using a 12-lead device; rhythm, heart rate, conduction disorders and QT interval corrected using the Bazett formula were analyzed. Results Of 100 patients with spondyloarthritis, 51 were on anti-TNF-α drugs and 49 were not. HLA-B27 was detected in 53.1% of patients in the sample.
