23 p. ill., Vele vraagstukken in de gezondheidszorg worden niet in klinische studies onderzocht door de farmaceutische industrie, terwijl ze toch een groot maatschappelijk belang hebben. Zo zijn er weinig studies waarbij een geneesmiddel wordt vergeleken met een ander type behandeling (bijvoorbeeld een vergelijking tussen de werking van antidepressiva en van psychotherapie). In een nieuw onderzoeksrapport komt het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) tot de conclusie dat het financieren van zulke studies met publieke middelen een zeer nuttige investering zou zijn. Het pleit er dus voor om het voorbeeld van andere landen zoals het Verenigd Koninkrijk en Nederland te volgen, waar dit al jaren gebeurt. Wel moeten de onderzoeksonderwerpen goed worden geselecteerd, en zijn een professionele infrastructuur en netwerk nodig. Ook de implementatie van de resultaten in de dagelijkse praktijk is cruciaal. Onder die voorwaarden kunnen programma’s van publiek gefinancierd onderzoek leiden tot een betere zorg en een meer efficiënt gezondheidszorgsysteem. 24 p. ill., In Europa zijn de regels om implantaten en medische hulpmiddelen op de markt te mogen brengen veel minder streng dan voor geneesmiddelen, zelfs als het gaat om implantaten met een hoog risico, zoals heupprothesen, pacemakers en stents. Om ze op de markt te mogen brengen, moet een fabrikant immers niet bewijzen dat ze echt een meerwaarde bieden voor de patiënt. Vaak zijn ze maar heel beperkt op mensen getest, en hun veiligheid laat soms ook te wensen over. In afwachting van een strengere Europese regelgeving doet het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) een aantal aanbevelingen om de Belgische burgers beter te beschermen tegen de mogelijke gevaren en nadelen van onvoldoende geteste medische hulpmiddelen, zonder daarbij de bestaande Europese wetgeving te overtreden. 23 p. ill., Beaucoup de questions de soins de santé importantes pour la société ne trouvent pas de réponse dans les essais cliniques menés par l’industrie pharmaceutique. Ainsi, il n’y a que peu d’études qui comparent un traitement médicamenteux et une autre approche (par ex. une comparaison entre des antidépresseurs et une psychothérapie). Dans une nouvelle étude, le Centre fédéral d’Expertise des Soins de Santé (KCE) arrive à la conclusion que le financement de tels essais cliniques par des moyens publics serait un excellent investissement. Il plaide donc pour que nous prenions exemple sur d’autres pays comme le Royaume-Uni ou les Pays-Bas, où cela se fait depuis des années. Le KCE souligne l’importance de la sélection des questions prioritaires à investiguer et de l’existence d’infrastructures professionnelles et de réseaux d’expertise. Il est également important que les résultats de ces études soient mis en pratique au quotidien sur le terrain. 40 p. ill., Il est fréquent que des médicaments soient utilisés pour d’autres indications que celles annoncées dans la notice, par exemple pour une autre maladie, à un autre dosage, ou pour d’autres types de patients. Il s’agit donc d’usages pour lesquels le médicament n’a pas obtenu d’autorisation ; on appelle cela l’utilisation off-label. Il n’existe pas de chiffres précis sur l’importance de cette pratique en Belgique, mais on estime qu’elle concerne jusque 80% des médicaments prescrits en pédiatrie, et au moins la moitié de ceux prescrits en oncologie. L’utilisation off-label fait partie de la liberté thérapeutique du médecin. Mais celui-ci doit en soupeser les bénéfices et les risques, et en informer son patient, car il n’y a en général pas de garanties que cet usage soit efficace et sans risques. Le Centre fédéral d’Expertise des Soins de Santé (KCE) a examiné les mesures que notre pays pourrait mettre en place pour que cet usage soit mieux encadré. Background: Economic evaluations of companion diagnostics often fail to include the impact that tests have on the overall economic value of test–drug combinations. Methods: To illustrate the importance of test accuracy on the cost–effectiveness of companion diagnostics by means of examples. Data were extracted from the literature. Results: The accuracy of a test and in particularly its specificity, is often more influential on the overall cost–effectiveness results than the price of the test. Specificity becomes more crucial when prevalence of the biomarker is low. Multiple, simultaneous testing faces specific challenges regarding its overall specificity. Conclusion: This article opens a discussion on some fundamental points linked to economic evaluations of test–therapy combinations.

p. 21 Modelling of NIPT Figure 3 presents an overview of the current screening strategy in Belgium. In Figure 4, the current first trimester biochemistry screening and second trimester screening is replaced by NIPT at week 12. In the next part of this supplementary file, we present and explain the three models in detail (current screening, NIPT 2nd line and NIPT 1st line) with inclusion of the number of pregnant women and T21 pregnancies at different moments in the model.

p. 1-10 The first- and second-trimester screening for trisomy 21 (T21) are reimbursed for all pregnant women in Belgium. Using a cut-off risk of 1:300 for T21, about 5% of all pregnant women are referred for definitive prenatal diagnosis using an invasive test, at a sensitivity of (only) 72.5%. The sensitivity and specificity of the non-invasive prenatal test (NIPT) are over 99% but come at a cost of €460 (£373) per test. The objective is to estimate the consequences of introducing NIPT for the detection of T21. METHODS: A cost-consequences analysis was performed presenting the impact on benefits, harms and costs. Context-specific real-world information was available to set up a model reflecting the current screening situation in Belgium. This model was used to construct the second and first line NIPT screening scenarios applying information from the literature on NIPT's test accuracy. RESULTS: Introducing NIPT in the first or second line reduces harm by decreasing the number of procedure-related miscarriages after invasive testing. In contrast with NIPT in the second line, offering NIPT in the first line additionally will miss fewer cases of T21 due to less false-negative test results. The introduction of NIPT in the second line results in cost savings, which is not true for NIPT at the current price in the first line. If NIPT is offered to all pregnant women, the price should be lowered to about €150 to keep the screening cost per T21 diagnosis constant. CONCLUSIONS: In Belgium, the introduction and reimbursement of NIPT as a second line triage test significantly reduces procedure-related miscarriages without increasing the short-term screening costs. Offering and reimbursing NIPT in the first line to all pregnant women is preferred in the long term, as it would, in addition, miss fewer cases of T21. However, taking into account the government's limited resources for universal reimbursement, the price of NIPT should first be lowered substantially before this can be realised.

105-110 Background Innovative high-risk medical devices, such as new types of heart valves or hip prostheses, become available on the European market more rapidly than in USA. This is due to the European legislation allowing early marketing of innovative high-risk medical devices before high-quality clinical evidence is obtained from randomized controlled trials. Methods We studied the premarket clinical evaluation of innovative high-risk medical devices in Europe compared with the USA. We also discussed patient safety and the transparency of information. The literature and regulatory documents were checked. Representatives from industry, competent authorities, notified bodies, ethics committees, and health technology assessment agencies were consulted. Results In contrast to the US, there is no requirement in Europe to demonstrate the clinical efficacy of high-risk devices in the premarket phase. For the patient, this implies earlier access to innovative technology, but at the risk of potential safety issues. At this moment, European requirements for clinical studies are lower for medical devices than for drugs, and data from premarket clinical trials are scarce or remain unpublished. The European Medical Device Directives are currently being reworked. Conclusions For innovative high-risk devices, and while awaiting a reworked Medical Device Directive, patient risk should be minimized by limiting the market introduction of novel high-risk devices with minimal clinical data to physicians with the necessary training and expertise. The new European legislation should require the premarket demonstration of clinical efficacy and safety, using a randomized controlled trial if possible, and a transparent clinical review, preferably centralized.

Vrij recent, in de zomer van 2012, nam de minister van Volksgezondheid maatregelen om het gebruik van biosimilaire geneesmiddelen aan te moedigen, zonder aanwijsbaar succes tot nu toe. Op vraag van de minister ging het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) na waarom biosimilairen in ons land alsnog niet doorbreken. De kortingen (tot 75% voor sommige producten) en andere voordelen van de fabrikanten van de originelen aan de ziekenhuizen maken dat de biosimilairen uiteindelijk voor hen niet concurrentieel zijn. Tegelijkertijd betaalt het RIZIV de ziekenhuizen immers nog steeds terug op basis van de officiële prijs van het origineel. Daarnaast twijfelen veel artsen aan de doeltreffendheid en veiligheid van biosimilairen, en dit is zeker mee te wijten aan het gebrek aan informatie en klinische gegevens.

28 p. Ill. Les biosimilaires sont des « quasi-copies » de médicaments biologiques, mais dont le prix est de 20 à 34 % inférieur à celui de l’original. La ministre de la Santé publique a récemment – dans le courant de l’été 2012 – pris un certain nombre de mesures afin d’en promouvoir l’usage, mais jusqu’ici sans succès démontré. A sa demande, le Centre Fédéral d'Expertise des Soins de Santé (KCE) a examiné les raisons pour lesquelles les biosimilaires n’ont toujours pas percé dans notre pays. On constate d’abord que lorsque l’on prend en compte les ristournes (jusqu’à 75 % pour certaines spécialités) et les autres avantages offerts aux hôpitaux par les fabricants des produits originaux, les biosimilaires ne sont pas concurrentiels pour les hôpitaux d’autant que le montant que l’INAMI rembourse aux établissements de soins reste en effet toujours basé sur le prix officiel du produit original. On constate également que les médecins éprouvent une certaine méfiance quant à l’efficacité et la sécurité des biosimilaires, une attitude certainement liée au manque d’informations et de données cliniques.

p. 126-136 Assessing the overall burden of disease which can be attributed to hospital-acquired infections (HAIs) remains a challenge. A matched cohort study was performed to estimate excess mortality, length of stay and costs attributable to HAIs in Belgian acute-care hospitals, using six matching factors (hospital, diagnosis-related group, age, ward, Charlson score, estimated length of stay prior to infection). Information was combined from different sources on the epidemiology and burden of HAIs to estimate the impact at national level. The total number of patients affected by a HAI each year was 125 000 (per 10·9 million inhabitants). The excess mortality was 2·8% and excess length of stay was 7·3 days, corresponding to a public healthcare cost of €290 million. A large burden was observed outside the intensive-care unit setting (87% of patients infected and extra costs, 73% of excess deaths).

A409 BACKGROUND: Orphan medicinal products are designed to diagnose or treat rare diseases that are serious, life threatening or chronically debilitating and that affect 50 or fewer people in every 100 000 in the EU. In Belgium, the Drug Reimbursement Committee (DRC) evaluates reimbursement requests for orphan drugs based on multiple criteria: the therapeutic value, price and proposed reimbursement tariff; the importance of the drug in clinical practice; and the budget impact of the drug. OBJECTIVES: This study aimed to assess reimbursement dossiers of orphan drugs in Belgium and to compare them with the clinical evidence submitted to the European Medicines Agency (EMA). METHODS: A qualitative analysis examined all reimbursement dossiers of orphan drugs that were submitted in Belgium between January 2002 and June 2008. The following information was extracted from each dossier: description of the orphan drug; indication; reimbursement status; therapeutic value and needs; budget impact; and number of registered indications. For selected orphan drugs, an in-depth analysis extracted and compared information about the clinical trials, their primary endpoints and results from EMA documents (i.e. the marketing authorization application file, European public assessment report and summary of product characteristics) and the Belgian reimbursement dossiers. RESULTS: Reimbursement was awarded to the majority of orphan drugs. In addition to the official criteria, other negotiable factors, such as price adjustments, employment incentives, patient population restrictions and funding of diagnostic tests by the company, seemed to play a role in the reimbursement decision. Despite the low number of patients, randomized controlled trials were conducted for many orphan drugs. Budget-impact analyses were simplistic and did not consider the impact across multiple indications. Some differences were also observed between the clinical evidence submitted to the EMA and that submitted to the Belgian DRC. CONCLUSIONS: In addition to the official criteria, other negotiable factors, such as price adjustments and employment incentives, may play a role in Belgian reimbursement decisions of orphan drugs. Some differences have also been noted between the clinical evidence reported in EMA documents and the evidence included in Belgian reimbursement dossiers of orphan drugs. There appears to be a need for further standardization of Belgian reimbursement applications and for European cooperation in sharing clinical evidence of orphan drugs.

p. 158-162 This matched cohort study estimates the effect of hospital-acquired bloodstream infection (HA-BSI) on length of stay (LOS) and costs during hospitalisation of 1839 patients (age range 80 years) gathered from 19 acute hospitals in Belgium. A second objective was to evaluate the impact of the choice of matching criteria. Data from national surveillance of HA-BSI were linked to hospital administrative discharge data, with respect for the patients' right to confidentiality of their health record. Controls were identified based on a set of matching factors: hospital, All-Patient Refined Diagnosis Related Groups, age, principal diagnosis, Charlson Comorbidity Index and time to infection. The results showed that, depending on the choice of matching factors, the estimation of additional LOS decreased from 26 to 10 days, with the most critical factor being the time to infection. The additional LOS attributable to HA-BSI was 9.9 days [95% confidence interval (CI): 7.8, 11.9]. The additional cost per infection was euro4900 [95% CI: euro4035, euro5750]; 58% of those costs were due to LOS, 10% were due to antibiotics, 10% due to other pharmaceutical products and 15% were due to billed medical acts. The main conclusion is that laboratory-confirmed HA-BSIs increase the LOS by 10 days for patients surviving the infection, and that the time to infection plays a crucial role in this estimation.

En collaboration avec des médecins spécialistes des maladies du foie, le Centre fédéral d’expertise des soins de santé (KCE) a réalisé une étude sur l’évolution et les possibilités de traitement de l’hépatite B chronique. Ces dernières années, de nouveaux antiviraux ont fait leur apparition sur le marché et sont remboursés depuis peu. Souvent, ces traitements doivent être pris à vie. Leur but est d’empêcher le développement de complications à long terme, tels la cirrhose ou le cancer du foie. 109-124 Background: Several European countries have set up population screening programmes for cancer detection. By definition, screening tests are performed in a population that is not at increased risk. The benefits and harms both before and after programme implementation are to be balanced versus programme costs. Methods: The Belgian Health Care Knowledge Centre (KCE) has performed four systematic reviews on respectively colorectal, breast, prostate, and cervical cancer screening. This paper presents an overview and discussion of results. Results: For the guaiac Faecal Occult Blood Test there is high quality evidence from randomised controlled trials that screening reduces colorectal cancer mortality. Its implementation in a real-world setting, however, is not easy. For breast cancer screening, recent reviews raise questions about the balance between benefits and risks and the European recommendations are far from being applied in reality. The Prostate Specific Antigen test is often performed, despite the absence of evidence for its value in population screening. For cervical cancer, the Pap smear is overused in a subgroup of women, whereas 4 out of 10 women in the Belgian target population are not regularly tested. Conclusions: Current screening programmes are often set up to improve life expectancy and quality of life. However, benefits should be proven before the implementation of widespread screening programmes. Screening programmes without a positive benefit/risk balance or that do not offer value for money should not be organised by governments and paid from public funds. If screening programmes are implemented, setting up a register is of paramount importance to collect information on participation, test results, subsequent management, harms, and outcomes to evaluate and fine-tune the programmes.

161-170 OBJECTIVES: The cost-effectiveness of adding a human papillomavirus (HPV) vaccination program in 12-year-old females to the recommended cervical cancer screening in Belgium is examined. Moreover, the health and economic consequences of a potential decline in screening uptake after initiation of a HPV vaccination program are investigated. METHODS: A static Markov model is developed to estimate the direct effect of vaccination on precancerous lesions and cervical cancers. RESULTS: Vaccination is estimated to avoid 20 percent of the cervical cancers occurring in a 12-year-old girls' cohort and to cost 32,665 euro per quality-adjusted life-year (QALY) gained (95 percent credibility interval [CrI]: 17,447 euro to 68,078 euro), assuming a booster injection after 10 years, a limited duration of protection and discounting costs and effects at 3 percent and 1.5 percent, respectively. Assuming lifelong protection, HPV vaccination is estimated to cost 14,382 euro (95 percent CrI: 9,238 euro to 25,644 euro) per QALY gained, while avoiding 50 percent of the cervical cancer cases. In the base-case, a 10 percent reduction in screening compliance after vaccination obliterates the effect of vaccination on cervical cancer cases avoided, whereas further declines in the level of screening compliance even turned out to be detrimental for the cohort's health, inducing a mean loss in QALYs and life-year gained compared with the situation prevaccination. CONCLUSIONS: An HPV vaccination program should only be considered if the level of screening after vaccination can be maintained.

We estimated the cost-effectiveness and budget impact of reimbursing trastuzumab in this indication from a payer's perspective.
