Abstract

Levetiracetam and zonisamide are licensed as monotherapy for patients with focal epilepsy, but there is uncertainty as to whether they should be recommended as first-line treatments because of insufficient evidence of clinical effectiveness and cost-effectiveness. We aimed to assess the long-term clinical effectiveness and cost-effectiveness of levetiracetam and zonisamide compared with lamotrigine in people with newly diagnosed focal epilepsy.

Abstract

Valproate is a first-line treatment for patients with newly diagnosed idiopathic generalised or difficult to classify epilepsy, but not for women of child-bearing potential because of teratogenicity. Levetiracetam is increasingly prescribed for these patient populations despite scarcity of evidence of clinical effectiveness or cost-effectiveness. We aimed to compare the long-term clinical effectiveness and cost-effectiveness of levetiracetam compared with valproate in participants with newly diagnosed generalised or unclassifiable epilepsy.

Abstract

ABSTRACTThis paper asks about the practices and forms of critique afforded at the intersection between Science-and-Technology Studies and the critical study of security politics, by drawing on the work of Isabelle Stengers. Can engagements with practice generate effective forms of critique? How does the attention to materialities and the fine-grained analysis of technical practices, that typically accompany STS-inspired research, feed into ways of practicing critique? How can the analytical attention to ‘little security nothings’ be translated into critical agendas?In addressing these questions, Belgian philosopher Isabelle Stengers serves as an inspiration, not just for her philosophical writing on the situated nature of critical thinking, but also for her own experimental politics and engagement. A key message of the paper is that critical practice is “engagement all the way down.” The article thinks through the ways in which the work of Stengers and her companions can be put into practice in qualitative critical security research. It builds on Foucault’s rejection of the separation between theoretical, universalist knowledge on the one hand, and practical knowledge on the other. It emphasises the processual, uncertain and practical aspects of critical thinking. It identifies the notions of following, leveraging, joined risks and paying attention as key, concrete pathways of engaged critique.

Abstract

Whispering gallery mode resonator lasers hold the promise of an ultralow intrinsic limit of detection. However, the widespread use of these devices for biosensing applications has been hindered by the complexity and lack of robustness of the proposed configurations. In this work, we demonstrate biosensing with an integrated microdisk laser. Al_2O_3doped with Yb^3+ was utilized because of its low optical losses as well as its emission in the range 1020–1050 nm, outside the absorption band of water. Single-mode laser emission was obtained at a wavelength of 1024 nm with a linewidth of 250 kHz while the microdisk cavity was submerged in water. A limit of detection of 300 pM (3.6 ng/ml) of the protein rhS100A4 in urine was experimentally demonstrated, showing the potential of the proposed devices for biosensing.

Abstract

Background and Aims: Population-based studies often use plasma fatty acids (FAs) as objective indicators of FA intake, especially for n-3 FA and linoleic acid (LA). The relation between dietary and circulating FA in cardiometabolic patients is largely unknown. We examined whether dietary n-3 FA and LA were reflected in plasma lipid pools in post-myocardial infarction (MI) patients.Methods and Results: Patients in Alpha Omega Cohort filled out a 203-item food-frequency questionnaire from which eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), alpha-linolenic acid (ALA), and LA intake were calculated. Circulating individual FA (% total FA) were assessed in cholesteryl esters (CE; n = 4066), phospholipids (PL; n = 838), and additionally in total plasma for DHA and LA (n = 739). Spearman correlation coefficients (rs) were calculated for dietary vs. circulating FA. Circulating FA were also compared across dietary FA quintiles, overall and in subgroups by sex, obesity, diabetes, statin use, and high alcohol intake. Patients were on average 69 years old and 79% was male. Moderate correlations between dietary and circulating levels were observed for EPA (rs∼0.4 in CE and PL) and DHA (rs ∼0.5 in CE and PL, ∼0.4 in total plasma), but not for ALA (rs ∼0.0). Weak correlations were observed for LA (rs 0.1 to 0.2). Plasma LA was significantly lower in statin users and in patients with a high alcohol intake.Conclusions: In post-MI patients, dietary EPA and DHA were well reflected in circulating levels. Abstract

Functional diversity can be defined as the distribution of trait values within a community. Hence, functional diversity can be an indicator of habitat filtering and a reliable environmental predictor of ecosystem functioning. However, there is a serious lack of studies that test how functional diversity indices change depending on the environmental conditions. The aim of this study is to provide such evidence by analyzing the distribution and variation of continuous body-mass values (i.e. functional diversity) and related shifts in body length and width in a nematode community. We used a large online dataset on nematode traits to analyze: (i) the distribution of body mass using three functional diversity indices, i.e. functional richness, functional divergence and functional evenness; (ii) the shifts in body-size traits (length and width); and (iii) the body-mass distributions of five trophic groups and of the entire nematode community. Managed grasslands exhibited the widest range of body-mass values while body-mass distribution in arable fields covered the greatest area in comparison to the other ecosystem types. The shift in body size revealed environmental filters that could not have been identified by the study of functional diversity indices per se. We found low values of functional evenness to be associated with high values of functional richness. We provide novel empirical evidence that body-mass distribution within a trophic group mirrors the effects of habitat filtering more than the distribution in the community as a whole. 230. Watchful Waiting Versus Surgery of Mildly Symptomatic or Asymptomatic Inguinal Hernia in Men Aged 50 Years and Older: A Randomized Controlled Trial.

Abstract

Objective: To compare if watchful waiting is noninferior to elective repair in men aged 50 years and older with mildly symptomatic or asymptomatic inguinal hernia. Background: The role of watchful waiting in older male patients with mildly symptomatic or asymptomatic inguinal hernia is still not well-established. Methods: In this noninferiority trial, we randomly assigned men aged 50 years and older with mildly symptomatic or asymptomatic inguinal hernia to either elective inguinal hernia repair or watchful waiting. Primary endpoint was the mean difference in a 4-point pain/discomfort score at 24 months of follow-up. Using a 0.20-point difference as a clinically relevant margin, it was hypothesized that watchful waiting was noninferior to elective repair. Secondary endpoints included quality of life, event-free survival, and crossover rates. Results: Between January 2006 and August 2012, 528 patients were enrolled, of whom 496 met the inclusion criteria: 234 were assigned to elective repair and 262 to watchful waiting. The mean pain/discomfort score at 24 months was 0.35 [95% confidence interval (CI) 0.28-0.41)] in the elective repair group and 0.58 (95% CI 0.52-0.64) in the watchful waiting group. The difference of these means (MD) was -0.23 (95% CI -0.32 to -0.14). In the watchful waiting group, 93 patients (35·4%) eventually underwent elective surgery and 6 patients (2·3%) received emergent surgery for strangulation/ incarceration. Postoperative complication rates and recurrence rates in these 99 operated individuals were comparable with individuals originally assigned to the elective repair group (8.1% vs 15.0%; P = 0.106, 7.1% vs 8.9%; P = 0.668, respectively). Conclusions: Our data could not rule out a relevant difference in favor of elective repair with regard to the primary endpoint. Nevertheless, in viewof all other findings, we feel that our results justify watchful waiting as a reasonable alternative compared with surgery in men aged 50 years and older. [ABSTRACT FROM AUTHOR]

Abstract

This study used tree data from field plots in urban areas to describe forest structure in urban areas throughout California. The plot data were used with numerical models to calculate several ecosystem services produced by trees. A series of transfer functions were calculated to scale-up results from the plots to the landscape using urban tree canopy (UTC) mapped at 1-m resolution for each combination of 6 land use classes and climate zones. California's UTC covered 15% of the urban area and contained 173.2 million trees, five per city resident. UTC per capita was lowest among U.S. states (90.8 m 2 ), indicating ample opportunity for tree planting. Oaks were the most abundant taxon (22%) and overall plantings were youthful. The annual value of ecosystem services was estimated at $8.3 billion and the urban forests asset value was $181 billion. Assuming an average annual per tree management cost of $19 and benefit of $47.83, $2.52 in benefit was returned for every dollar spent. The threat posed by Invasive Shot Hole Borer ( Euwallacea sp.) illustrates that urban forests are a relatively fragile resource whose contributions to human health and well-being can be suddenly jeopardized. One scenario projected that should Southern California cities lose 50% (11.6 million) of all susceptible trees, the value of ecoservices foregone over 10 years was $616.6 million. The approximate cost of removing and replacing the trees was $15.9 billion. Strategies to reduce the risk of catastrophic loss by increasing the resilience of California’s urban forests are discussed. [ABSTRACT FROM AUTHOR]

Abstract

Infantile spasms constitute a severe form of epileptic encephalopathy. In the International Collaborative Infantile Spasms Study (ICISS), we showed that combining vigabatrin with hormonal therapy was more effective than hormonal therapy alone at stopping spasms between days 14 and 42 of treatment. In this planned follow-up, we aimed to assess whether combination therapy was associated with improved developmental and epilepsy outcomes at 18 months of age.

In voorliggend Rekenkamerrapport vindt u antwoord op deze en meer van zulke vragen. Het rapport is gebaseerd op een onderzoek dat SEO Economisch Onderzoek heeft uitgevoerd aan de hand van 74 gemeentelijke reïntegratie-onderzoeken, waaronder veel rekenkameronderzoeken. Hieruit wordt duidelijk op welke manier gemeenten een slimme mix van instrumenten kunnen kiezen voor hun (divers samengestelde) bijstandspopulatie.

247. Differential effects of diet composition and timing of feeding behavior on rat brown adipose tissue and skeletal muscle peripheral clocks

Abstract

The effects of feeding behavior and diet composition, as well as their possible interactions, on daily (clock) gene expression rhythms have mainly been studied in the liver, and to a lesser degree in white adipose tissue (WAT), but hardly in other metabolic tissues such as skeletal muscle (SM) and brown adipose tissues (BAT). We therefore subjected male Wistar rats to a regular chow or free choice high-fat-high sugar (fcHFHS) diet in combination with time restricted feeding (TRF) to either the light or dark phase. In SM, all tested clock genes lost their rhythmic expression in the chow light fed group. In the fcHFHS light fed group rhythmic expression for some, but not all, clock genes was maintained, but shifted by several hours. In BAT the daily rhythmicity of clock genes was maintained for the light fed groups, but expression patterns were shifted as compared with ad libitumand dark fed groups, whilst the fcHFHS diet made the rhythmicity of clock genes become more pronounced. Most of the metabolic genes in BAT tissue tested did not show any rhythmic expression in either the chow or fcHFHS groups. In SM Pdk4and Ucp3were phase-shifted, but remained rhythmically expressed in the chow light fed groups. Rhythmic expression was lost for Ucp3whilst on the fcHFHS diet during the light phase. In summary, both feeding at the wrong time of day and diet composition disturb the peripheral clocks in SM and BAT, but to different degrees and thereby result in a further desynchronization between metabolically active tissues such as SM, BAT, WAT and liver.

Abstract

This study compiled recent inventory data from 929,823 street trees in 50 cities to determine trends in tree number and density, identify priority investments and create baseline data against which the efficacy of future practices can be evaluated. The number of street trees increased from 5.9 million in 1988 to 9.1 million in 2014, about one for every four residents. Street tree density declined from 65.6 to 46.6 trees per km, nearly a 30% drop. City streets are at 36.3% of full stocking. State-wide, only London planetree ( Platanus × hispanica ) comprises over 10% of the total, suggesting good state-wide species diversity. However, at the city scale, 39 communities were overly reliant on a single species. The state’s street trees remove 567,748 t CO 2 (92,253 t se) annually, equivalent to taking 120,000 cars off the road. Their asset value is $2.49 billion ($75.1 million se). The annual value (USD) of all ecosystem services is $1.0 billion ($58.3 million se), or $110.63 per tree ($29.17 per capita). Given an average annual per tree management cost of $19.00, $5.82 in benefit is returned for every $1 spent. Management implications could include establishing an aggressive program to plant the 16 million vacant sites and replace removed trees, while restricting planting of overabundant species. Given the tree population’s youth there is likely need to invest in pruning young trees for structure and form, which can reduce subsequent costs for treating defects in mature trees. 250. Role of reverse phenotyping in interpretation of next generation sequencing data and a review of INPP5E related disorders.

Abstract

Introduction Next Generation Sequencing (NGS) is a useful tool in diagnosis of rare disorders but the interpretation of data can be challenging in clinical settings. We present results of extended studies on a family of multiple members with global developmental delay and learning disability, where another research group postulated the underlying cause to be a homozygous RABL6 missense variant. Methods and results Using data from the Exome Variant Server, we show that missense RABL6 variants are unlikely to cause early onset rare developmental disorder. Protein structural analysis, cellular functional studies and reverse phenotyping proved that the condition in this family is due to a homozygous INPP5E mutation. An in-depth review of mutational and phenotypic spectrum associated with INPP5E demonstrated that mutations in this gene lead to a range of cilliopathy-phenotypes. Discussion We use this study as an example to demonstrate the importance of careful clinical evaluation of multiple family members, reverse phenotyping, considering the unknown phenotypic variability of rare diseases, utilizing publically available genomic databases and conducting appropriate bioinformatics and functional studies while interpreting results from NGS in uncertain cases. We emphasize that interpretation of NGS data is an iterative process and its dynamic nature should be explained to patients and families. Our study shows that developmental delay, intellectual disability, hypotonia and ocular motor apraxia are common in INPP5E -related disorders and considerable intra-familial phenotypic variability is possible.
