Based on the finite element theory, a joint-plane modeling method is employed to connect the corresponding nodes at the joint surface of the woodworking computer numerical control (CNC) machining center bed with a 2-node 12-degree-of-freedom unit. A spatial element model is established, which can show the state of the nodes between joint surfaces when they are stretched, compressed, or twisted; and it can help build a woodworking CNC machining center on a finite element model of bed with the characteristics of the joint surface. The simulated analysis is performed on the model and is compared with the result of simulated analysis on the bed model that ignores the characteristics of the joint surface and modal experiment. The comparison verifies the effectiveness of the modeling method based on the characteristics of the joint surface. The weak link of the machine bed structure is analyzed and optimized. The natural frequency of the bed is improved by2.55% ~ 11.3%. The displacement is reduced by a maximum of 19.4%, and dynamic performance of the bed is improved.

Objective To analysis the safety and outcome of intravenous thrombolysis with recombinant tissue plasminogen activator (rt⁃PA) in patients with acute mild ischemic stroke. Methods From November 2017 to October 2018, 252 patients with acute mild ischemic stroke who were admitted into our hospital within 4.50 h after onset (84 patients in intravenous thrombolysis group, 108 patients in aspirin and clopidogrel group, and 60 patients in aspirin or clopidogrel group). The general condition, the level of high⁃sensitivity C⁃reactive protein (hs⁃CRP) and erythrocyte sedimentation (ESR) were compared among 3 groups. The modified Rankin Scale (mRS) were evaluated on 7 d after admission or at earlier discharge, and 21 and 90 d. Symptomatic intracranial hemorrhage rates were compared among 3 groups. Results There were no significant difference in general clinical data, the initial level of hs⁃CRP and ESR between the intravenous thrombolytic group and the 2 non⁃thrombolysis groups. At 7 d after admission or earlier discharge, the descending range of hs⁃CRP and ESR in 3 groups were significant (P = 0.000, for all); there were significant differences among the 3 groups (P = 0.000, for all); pairwise comparison of 3 groups showed that the levels of hs⁃CRP and ESR in intravenous thrombolytic group at 7 d were significantly lower than aspirin and/or clopidogrel group (P = 0.000, 0.000; P = 0.000, 0.010). There was no significant difference in the rate of intracranial hemorrhage between thrombolytic group and other 2 groups at 24 h after treatment [1.19% (1/84) vs. 0 (0/168); χ 2discharge, the intravenous thrombolytic group had better prognosis than aspirin and/or dopidogrel group (P = 0.043, 0.012); At 21 and 90 d, the intravenous thrombolytic group had better prognosis than aspirin or clopidogrel group (P = 0.021, 0.037); but compared with aspirin and clopidogrel group, there was no significant difference (P = 0.070, 0.053). Conclusions Patients with mild ischemic stroke, especially that may induce disability should be treated with rt⁃PA intravenous thrombolysis timely. A inducible fluorescent ligand 2-(2-(2-amino-ethylamino) ethyl)-3′,6′- bis (ethylamino)-2′, 7′-dimethy-lspiro[isoindoline-1,9′-xanthen]-3-one was synthesized and used as a fluorescent probe to detect Er3+. Er3+ could induce the structural transformation of the fluorescent ligand, resulting in a sharp fluorescence emission in a buffered solution. The fluorescence intensity of the fluorescent ligand was enhanced quantitatively with an increase in the concentration of erbium ion. The detection limit of Er3+ was 3.0 × 10−10 mol L−1 (50 ng L−1) under optimized conditions. The method applied for the determination of Er3+ in four alloy samples had achieved satisfactory results. Acute pancreatitis (AP) is a common serious acute condition of the digestive system that remains a clinical challenge. Severe acute pancreatitis (SAP) in particular is characterized by high morbidity and mortality. The present study was designed to investigate the protective effect of Galangin (Gal), a natural flavonol obtained from lesser galangal, on L-arginine-induced SAP in mice and in AR42J cells. Amylase and lipase activities were measured and the histopathology of the pancreas, lung, and kidney was evaluated. Inflammation and oxidative stress were assessed using ELISA, western blotting, RT-PCR, and immunohistochemistry. Gal was shown to reduce proinflammatory cytokine production and reactive oxygen species (ROS) generation in vivo and in vitro. L-arginine treatment reduced the expression of components of the nuclear factor E2-related factor 2 (Nrf2) signaling pathway and the downstream protein heme oxygenase-1 (HO-1) in mice, whereas Gal increased their expression. Furthermore, the Nrf2/HO-1 pathway inhibitor brusatol prevented the anti-inflammatory and antioxidant effects of Gal in mice with SAP. Taken together, our results imply that Gal has protective effects in L-arginine-induced SAP that are induced by the upregulation of the Nrf2/HO-1 pathway, which has anti-inflammatory and antioxidant effects. Thus, Gal may represent a promising treatment for SAP.

Influenza virus infection in pregnant women may put the fetus at higher risk; however, to date, there has been no detailed research about the expression of influenza virus receptors in the human placenta. We employed the lectin staining technique, which is a classic influenza virus receptor research method for studying the distribution of viral receptors in the human placenta. In addition, we examined the susceptibility of the human placenta to H1N1/09, by detecting viral proteins and RNA at different time points post-infection. We found that the human placenta expressed both avian and human influenza A virus receptors (α-2, 3-linked sialic acid and α-2, 6-linked sialic acid). In addition, H1N1/09 did not only infect the human placenta, but also replicated and was released into the culture media. We concluded that the human placenta is susceptible to the 2009 influenza A virus (H1N1/09) infection, and that particular attention should be paid to shielding pregnant women from infection during influenza season.

Alkali metal (M = Na, K) salts of the [CB11H12]− anion have attracted increasing attention in many fields such as catalysis and all-solid-state batteries. However, tedious and low-yielding synthetic methods have seriously limited their application in these fields. We developed a facile method for the direct synthesis of the unsolvated potassium and sodium salts of the [CB11H12]− anion in 66% and 68% yields, respectively, by reactions of Na[B11H14] with NaH/NaHMDS (sodium bis(trimethylsilyl)amide), CF3SiMe3 in THF and K[B11H14] with KH, CF3SiMe3 in DME. This method avoids the exchange of cation and significantly simplifies the reaction setup, thus enabling convenient large-scale synthesis. It is believed that this method will support further application of Na[CB11H12] and K[CB11H12] as solid electrolytes for an all-solid-state battery.

BACKGROUND. Adoptive cell therapy (ACT) with tumor-infiltrating lymphocytes (TILs) has achieved remarkable clinical efficacy in metastatic cancers such as melanoma and cervical cancer (CC). Here, we explored the safety, feasibility, and preliminary tumor response and performed translational investigations of adjuvant immunotherapy using infusion of autogenous TILs (auto-TILs) following concurrent chemoradiotherapy (CCRT) in patients with CC who had locally advanced disease. METHODS. Twenty-seven patients with CC with stage III-IV disease were recruited in this single-center, phase I study. TILs were isolated from lesions in the uterine cervix and generated under good manufacturing practice (GMP) conditions and then infused after CCRT plus i.m. IL-2 injections. RESULTS. TILs from 20 of the 27 patients were successfully expanded, with a feasibility of 74.1%. Twelve patients received TILs following CCRT. Adverse events (AEs) were primarily attributable to CCRT. Only 1 (8.3%) patient experienced severe toxicity with a grade 3 hypersensitivity reaction after TIL infusion. No autoimmune AEs, such as pneumonitis, hepatitis, or myocarditis, occurred, and there were no treatment-related mortalities. Nine of 12 patients (75.0%) attained a complete response, with a disease control duration of 9-22 months. Translational investigation showed that the transcriptomic characteristics of the infused TIL products and some immune biomarkers in the tumor microenvironment and serum of patients with CC at baseline were correlated with the clinical response. CONCLUSION. TIL-based ACT following CCRT was safe in an academic center setting, with potentially effective responses in patients with locally advanced CC. 'Hot' inflammatory immune environments were beneficial to the clinical efficacy of TIL-based ACT as adjuvant therapy. TRIAL REGISTRATION. ClinicalTrials.gov NCT04443296. FUNDING. Object: The fluid management strategy in ARDS is not very clear. A secondary analysis of RCT data was conducted to identify patients with ARDS benefitting from a conservative strategy of fluid management.Methods: The data of this study were downloaded from the ARDS network series of randomized controlled trials (Conservative Strategy vs. Liberal Strategy in 2006). Based on the clinical feature of patients, within the first 24 h after admission, clustering was performed using the k-means clustering algorithm to identify the phenotypes of ARDS. Survival was analyzed using the Kaplan-Meier survival analysis to assess the effect of the two fluid management strategies on the 90-day cumulative mortality. Categorical/dichotomic variables were analyzed by the chi-square test. Continuous variables were expressed as the mean and standard deviation and evaluated through a one-way ANOVA. A P-value < 0.05 was defined as the statistically significant cut-off value.Results: A total of 1,000 ARDS patients were enrolled in this unsupervised clustering research study, of which 503 patients were treated with a conservative fluid-management strategy, and 497 patients were treated with a liberal fluid-management strategy. The first 7-day cumulative fluid balance in patients with the conservative strategy and liberal strategy were −136 ± 491 ml and 6,992 ± 502 ml, respectively (P < 0.001). Four phenotypes were found, and the conservative fluid-management strategy significantly improved the 90-day cumulative mortality compared with the liberal fluid-management strategy (HR = 0.532, P = 0.024) in patients classified as “hyperinflammatory anasarca” phenotype (phenotype II). The characteristics of this phenotype exhibited a higher WBC count (20487.51 ± 7223.86/mm3) with a higher incidence of anasarca (8.3%) and incidence of shock (26.6%) at baseline. The furthermore analysis found that the conservative fluid management strategy was superior to the liberal fluid management strategy in avoiding superinfection (10.10 vs. 14.40%, P = 0.037) and returned to assisted breathing (4.60 vs. 16.20%, P = 0.030) in patients classified as “hyperinflammatory anasarca” phenotype. In addition, patients with other phenotypes given the different fluid management strategies did not show significant differences in clinical outcomes.Conclusion: Patients exhibiting a “hyperinflammatory anasarca” phenotype could benefit from a conservative fluid management strategy.

Although the use of biologics has led to great improvement in psoriasis patients, the treatment of psoriasis during pregnancy still faces many challenges. We herein report on a 29-year-old pregnant woman treated with ustekinumab for generalized pustular psoriasis. Upon becoming pregnant, the woman underwent continued treatment with ustekinumab in the first trimester. We also considered the need for neonatal vaccination. The patient discontinued ustekinumab therapy in the second trimester, and during the period of drug discontinuation we noted a slight rash recurrence. The patient was treated with ultraviolet B phototherapy and topical corticosteroids, and the rash was localized to the abdomen. However, in the 27th week of pregnancy, the patient was infected with COVID-19, which made the condition worse. The rash erupted rapidly and spread throughout her body, and she experienced a high fever with her blood count showing augmented numbers of white blood cells. The patients self-administered 0.3 g of acetaminophen three times per day, and after four days her core body temperature was 38.0°C; the rash, however, did not diminish. We diagnosed an outbreak of generalized pustular psoriasis and treated the patient with ustekinumab. The rash resolved quickly, and a healthy newborn was delivered by caesarean section at 39 weeks.
