Background: Prostate cancer remains the most frequently diagnosed cancer among men. High-Intensity Focused Ultrasound (HIFU) has emerged as a thermal ablative technique for partial-gland-ablation (PGA), aiming to minimize collateral damage while maximizing tumor control. Monitoring after HIFU PGA relies on serial PSA testing, multiparametric-MRI, and biopsies. The diagnostic accuracy of MRI for clinically-significant cancer(csPCa) recurrence is challenging. Objective: This systematic review and meta-analysis aim to evaluate the accuracy of MRI in detecting early recurrence of localized prostate cancer following HIFU PGA. Methods: Adhering to PRISMA guidelines, a comprehensive literature search was conducted until May 8th2024 using MEDLINE and Scopus. The inclusion criteria encompassed randomized controlled trials and cohort studies involving men diagnosed with localized prostate cancer who had as primary treatment HIFU PGA. The primary outcome measures included the sensitivity, specificity, positive-predictive value (PPV), and negative-predictive value (NPV) of MRI for csPCa(ISUP ≥ 2) based on biopsy results. We pooled data from studies with sufficient csPCa and csPCa-free patients (≥5) post HIFU for statistical analysis. Results: Fifteen studies meet the inclusion criteria, encompassing 1093 patients and 12 studies were eligible for meta-analysis. MRI sensitivity in detecting clinically-significant prostate cancer (csPCa) recurrence post HIFU PGA varied widely (0–89%), with a pooled sensitivity of 0.52 (95% CI:0.36–0.68). Specificity ranged from 44% to 100%, with a pooled specificity of 0.81 (95% CI:0.68–0.91). The pooled NPV was 0.82 (95% CI:0.72–0.90), and the pooled PPV was 0.50 (95% CI:0.35–0.65). Three studies reported in-field diagnostic performance with sensitivities ranging from 0.42 to 0.80 and specificities from 0.45 to 0.97. Conclusion: MRI accuracy for clinically-significant recurrence after partial gland ablation with HIFU for localized prostate cancer shows low diagnostic performance in the treated lobe with pooled sensitivity of 0.52 (95% CI:0.36–0.68) and specificity of 0.81 (95% CI:0.68–0.91). Limits of this review include the low number of studies reporting about site of recurrence in or out of the treated lobe.

Abstract: Objects: We present computer-assisted diagnosis (CAD) software designed to improve prostate cancer detection using perfusion MRI data. Methods: In addition to standard visualization features, this software allows for the 2D and multislice 2D contouring of suspicious areas based on a seeded region growing algorithm, and area labeling based on zonal anatomy. Tumor volume assessment and the semiquantitative analysis of DCE-MRI sequences can both be performed. We retrospectively analyzed DCE-MRI examinations of 100 patients and found 121 lesions showing a suspiciously high intensity with early enhancement in 84 of them. Seventy-one patients turned out to be malignant, whereas 50 were benign. Based on an analysis of the median wash-in and wash-out values of these foci, we designed a standardized 5-level cancer suspicion score (ranging from “probably benign” to “highly suspicious”). This comprehensive score provides a scaled likelihood of malignancy in the region of interest taking account of its location in relation to prostate zonal anatomy. We compared its accuracy with that of visual assessments of time–intensity curves performed by specialist and non-specialist radiologists. Results: Parameters of the scoring algorithm were designed to provide the greatest possible sensitivity in our sample population. A re-substitution evaluation provided an Se/Sp of 100/45% for peripheral zone cancer, and 100/40% for transition zone cancer characterization. When identifying malignant areas using time–intensity curves data, this simple algorithm performed significantly better (AUC = 0.77) than a non-specialist (AUC = 0.57, P < 0.0001) radiologist, and better than a trained (AUC = 0.70) radiologist, although this difference was not significant. Conclusion: Our new prostate MRI CAD software provides a standardized cancer suspicion score for suspicious foci detected in DCE-MRI T1-w images. Our results suggest that it may improve radiologists’ performances in prostate cancer identification, especially when they are not specialized in prostate imaging.

L’élevage, dans ses formes dites « industrielles », est aujourd’hui largement critiqué pour ses impacts environnementaux. Pour autant, d’autres formes d’élevage plus vertueuses existent, et l’élevage peut s’avérer indispensable pour répondre aux enjeux de production alimentaire, de fourniture de services écosystémiques et de dynamiques socioterritoriales, pour autant qu’il s’intègre dans un paysage agricole diversifié. L’agroécologie nous invite à mieux valoriser la diversité cultivée et élevée dans les exploitations et dans les territoires. L’intégration culture-élevage correspond aux pratiques agricoles à même d’exploiter les synergies entre composantes animales et végétales de cette diversité, à travers le bouclage des cycles de nutriments (fertilisation, alimentation) et les complémentarités fonctionnelles (lutte biologique, désherbage). Ces pratiques déployées au sein d’exploitations diversifiées et/ou entre exploitations spécialisées d’un territoire permettent ainsi de réduire la dépendance aux intrants (aliments achetés, fertilisants chimiques, phytosanitaires, carburants pour la mécanisation) et d’améliorer l’efficience d’utilisation des ressources. Elle peut également accroître la résilience en diluant les risques liés aux aléas (climatiques, économique), et améliorer la productivité du milieu par la diversité des produits. Pour autant, ces pratiques ne font pas (encore) partie du modèle agricole dominant et leur développement nécessitera de lever certains freins techniques, territoriaux et socioéconomiques qui, à défaut d’être rédhibitoires, en limitent aujourd’hui le déploiement.

Antibodies effective against the recent Omicron sublineages are missing. By taking advantage of a multi-centric prospective cohort of immunocompromised individuals treated for mild-to-moderate COVID-19, Bruel et al. show that administration of 500 mg of sotrovimab induces serum neutralization and antibody-dependent cellular cytotoxicity of BQ.1.1 and XBB.1.5. Therefore, sotrovimab may remain a therapeutic option against these variants.

Digital papillary adenocarcinoma (DPA) is a rare sweat gland neoplasm that has exceptionally been reported outside acral locations. Recently, human papillomavirus 42 was identified as the main oncogenic driver of DPA. Herein, we report 5 tumors arising in extra-acral locations predominantly in the female anogenital skin. Four patients were female and 1 patient was male. The mean age at the diagnosis time was 65 years (range: 55 to 82 y). Tumors were located on the vulva (n=3), perianal area (n=1), and forearm (n=1). Histologically, all tumors were lobular and mainly solid and composed of sheets of cells with rare focal papillae and frequent glandular structures in a “back-to-back” pattern and lined by atypical basophilic cells. Immunohistochemistry showed diffuse positivity for SOX10. Epithelial membrane antigen and carcinoembryonic antigen highlighted the luminal cells and staining for p63 and p40 revealed a consistent and continuous myoepithelial component around glandular structures. Follow-up was available in 3 cases (mean duration: 12 mo [range: 8 to 16 mo]). One patient developed local recurrence and 1 experienced regional lymph node metastases. HPV Capture Next-generation sequencing revealed the presence of the HPV42 genome in all samples. Viral reads distributions were compatible in the 5 cases with an episomal nature of the viral genome, with a recurrent deletion in the E1and/or E2open reading frames. In conclusion, this study demonstrates that digital DPA may rarely present in nonacral locations mainly in the female anogenital area, usually with a more solid pattern as compared with those cases presenting on the digits and it is also associated with HPV42.

Venous compression syndromes are clinical conditions in which the large veins are compressed by other anatomical structures. Laboratory simulations may help us better understand the hemodynamics in venous compressions by creating situations similar to those seen in vivo. The aim of this study is to produce a model of the caval bifurcation using a polymer with distensibility similar to the human vena cava.
