Orofacial pain (OFP) disorders affect the maxillofacial complex and typically involve other locations in the head and neck. Common etiologies of OFP disorders include odontogenic sources, temporomandibular disorders, neuropathic pain, chemosensory disorders, and headaches. Less commonly, benign and malignant neoplasms can be associated with OFP complaints. Patients with OFP symptoms require a thorough evaluation and are often referred to additional healthcare providers for interprofessional management. In those OFP patients whose symptoms do not respond to conventional treatment(s), whose symptoms change over time, or who report positive responses to review of systems questions that suggest more extensive involvement, further investigation is warranted. To the best of our knowledge, this is the first case report of nasopharyngeal extramedullary plasmacytoma, a localized malignant neoplasm, associated with persistent OFP.

Commercial intraoral rectangular collimators are available for collimating to size 2 image receptor. The benefits of reducing the x-ray beam to match the area of the image detector in adult intraoral radiography are endorsed internationally. However, in pediatric dentistry the image receptor can be further decreased to size 1 and 0.For this study size 1 and 0 rectangular collimators were fabricated using 1.65-mm lead sheets (Rotometals). The custom-fabricated collimators were fixed to the plastic body of a Rinn (Dentsply) Universal Collimator attachment. Aperture sizes were extrapolated based on the active imaging area of size 1 and 0 digital image receptors. A dose area product (DAP) measuring device was used to determine the change in radiation absorbed dose as a function of the imaging field of view.DAP measurements were evaluated in the 31.7 cm2 conventional round collimation, Rinn 12.0 cm2 Universal rectangular collimator, and in the manufactured size 1 (8.25 cm2) and size 0 (5.72 cm2) rectangular collimators. The size 1 collimator had a 32% DAP reduction from the size 2, and a 53% reduction for the size 0.Size 1 and size 0 rectangular collimators can be independently manufactured and utilized in pediatric dentistry. This study suggests that a considerable radiation dose reduction is possible in pediatric intraoral imaging when using the size 1 and 0 matched collimation. Since the pediatric population is vulnerable to radiation exposure, any measurable reduction has a potential for long-term health benefits and is therefore clinically significant.

Replacing conventional round intraoral collimators with rectangular collimators provides a considerable radiation dose reduction in adult patients. This study aimed to determine the radiation dose reduction via mathematical phantom when converting from round to appropriately sized rectangular collimation in children ages 5 to 15 years.Virtual full mouth series (FMX) were simulated using a commercially available radiation dose software. This software is designed to calculate patient radiation doses from x-ray exams for various age pediatric and adult mathematical phantoms. For this pediatric study an 18-image FMX was simulated for the 15-year-old and a 12-image FMX was simulated for the 5-year-old and 10-year-old pediatric phantoms. An area of 12.0 to 16.8 cm2 represented rectangular collimation, while a 20.4 to 31.7 cm2 area represented typical round collimation.Effective doses decreased in all ages by nearly 60% when switching from 31.7 cm2 round to 12.0 cm2 rectangular collimation. Reduction in absorbed doses to the thyroid (70% to 73%), salivary glands (62% to 78%), and active bone marrow (60% to 62%) were also noted when switching from the largest to smallest collimation.This study suggests the use of rectangular collimators provides clinically relevant dose reduction for pediatric patients, even when altering from smaller round to rectangular collimation with equivalent beam quality, and this information can be utilized in all dental practices.

Integrating transition metal dichalcogenide (TMD) monolayers with dielectric nanostructures exhibiting Mie-like resonances opens a plethora of opportunities in manipulating their excitonic emission in the near-field and far-field regimes, yielding interactions spanning from weak to strong coupling regimes, and routing valley polarized chiral emission, to name just a few. Moreover, there is a completely new path unraveled recently by realizing dielectric resonators directly from TMDs, thus combining scatterer and emitter into a single entity. This hybrid configuration is promising to realize integrated active meta-optical devices in a facile manner. In this review article, we provide an overview of the state of the art on integrating monolayers of TMDs with Mie-resonant photonic nanostructures.

Nanopatterning of monolayers of transition metal dichalcogenides offers a new avenue for the creation of nanoscale light sources and their integration into hybrid nanophotonic systems. Here, focused gallium ion-beam milling is employed as a resist-free and simple nanofabrication approach to pattern MoS2 monolayers grown by chemical vapor deposition into nanoribbons. Using photoluminescence (PL), Raman, and valley polarization spectroscopy, it is investigated how the optoelectronic properties of the MoS2 monolayers are affected by the nanopatterning as a function of the ion fluence used for milling. Characteristic spectral shifts in the Raman and PL peaks are observed, which are indicative of a release of strain in patterned MoS2 monolayers as compared to the as-grown monolayers. Furthermore, while the total PL signal is reduced in the patterned monolayers, using circular-polarization-resolved cryogenic PL spectroscopy it is shown that the valley polarization is well preserved for monolayers patterned within an optimal ion-fluence range. The results and in particular the observed robustness of the valley polarization indicate that focused ion-beam nanopatterned MoS2 monolayers are interesting candidates for their integration into hybrid quantum systems consisting of designed photonic nanostructures and precisely placed nanoscale excitonic light sources.

Temporomandibular disorder (TMD) is prevalent in special needs patients. Clinical examination of the temporomandibular joint (TMJ) complex and imaging in this patient population can be challenging due to patient disposition and cooperation. We report a case of a 29-year-old male with neurologic and behavioral special needs who required advanced maxillofacial imaging for a suspected TMD under general anesthesia, which has not been reported previously. This article intends to serve as a resource for health care providers who may encounter similar clinical situations.

Historically, Eagle syndrome is a term that has been used to describe radiating pain in the orofacial region, foreign body sensation, and/or dysphagia due to a unilateral or bilateral elongated styloid process impinging upon the tonsillar region. Because elongated styloid processes-with or without associated Eagle syndrome-can present with various symptoms and radiographic findings, it can be challenging for healthcare practitioners to formulate an accurate diagnosis. Abnormal styloid anatomy can lead to a multitude of symptoms, including chronic orofacial/neck pain, thus masquerading as more commonly diagnosed conditions. In this report, we describe a patient who presented to our department with styloid process elongation and fracture. A careful history, physical examination, and a conebeam computed tomography (CBCT) investigation led to the diagnosis. The patient was then referred for appropriate care. This case report demonstrates the utilization of CBCT in differentiating a fracture site from a pseudo-joint that might mimic a fracture.

Although the American Joint Committee on Cancer developed its first cancer-specific staging system in 1959, the TNM classification, as it has become known, has undergone many revisions mainly due to the advancements in both diagnosis and management of cancer, Although the basic purpose of the cancer staging system has remained fundamentally unchanged, the ease with which the cancer can be staged has evolved with newer methods. This article reviews cancer evaluation for staging, as well as the introduction of a new staging method effective as of 2018.

Orofacial pain can often be the chief complaint of many systemic disorders. Cysticercosis involving the lateral pterygoids may cause limitation of mouth opening and may mimic clinical symptoms of a temporomandibular disorder. A 37-year-old female presented with 1-month-old complaint of limited mandibular range of motion. She reported a similar episode a year earlier and was diagnosed with a temporomandibular joint disorder by her primary dentist. Comprehensive intra- and extra-oral examinations were performed, which revealed a limitation of mouth opening accompanied by mild limitation of contralateral excursion. A magnetic resonance imaging revealed a ring-enhancing lesion within the left pterygoid muscle suggestive of cysticercosis. The patient was referred to her primary care physician for further treatment and given physical therapy (stretching exercises) to improve mouth opening. One week later, she developed lesions in the arm and trunk. Further ultrasound imaging of the abdomen and the forearms confirmed the diagnosis of cysticercosis. She was treated with albendazole, physiotherapy, joint stabilization appliance, and had eventual complete recovery. This case emphasizes the importance of diagnosis of a systemic condition that may have serious implications, if untreated, and the importance of a comprehensive evaluation, workup, and multidisciplinary management.

In a time when orthodontists are getting away from paper charts and going digital with their patient data and imaging, practitioners need to be prepared for a potential hardware failure in their data infrastructure. Although a backup plan in accordance with the Security Rule of the Health Insurance Portability and Accountability Act (HIPAA) of 1996 may prevent data loss in case of a disaster or hard drive failure, it does little to ensure business and practice continuity. Through the implementation of a common technique used in information technology, the redundant array of inexpensive disks, a practice may continue normal operations without interruption if a hard drive fails.

Skin cancer is extremely common, and melanoma causes about 80% of skin cancer deaths. In fact, melanoma kills over 50 thousand people around the world each year, and these numbers are rising. Clearly, standard treatments are not effectively treating melanoma, and alternative therapies are needed to address this problem. Hibiscus tea has been noted to have medicinal properties, including anticancer effects. Extracts from Hibiscus have been shown to inhibit the growth of a variety of cancer cells. In particular, recent studies found that polyphenols extracted from Hibiscus sabdariffa by organic solvents can inhibit melanoma cell growth. However, effects of aqueous extracts from Hibiscus rosa-sinesis flowers, which are commonly used to make traditional medicinal beverages, have not been examined on melanoma cells. Here, we report that aqueous H. rosa-sinesis flower extract contains compounds that inhibit melanoma cell growth in a dose dependent manner at concentrations that did not affect the growth of nontransformed cells. In addition, these extracts contain low molecular weight growth inhibitory compounds below 3 kD in size that combine with larger compounds to more effectively inhibit melanoma cell growth. Future work should identify these compounds, and evaluate their potential to prevent and treat melanoma and other cancers.
