201. Prospective multicentre observational study assessing the tolerance and perception of patients using the Liquick Base catheter with an Ergothan tip

Abstract

Summary Introduction Intermittent self-catheterisation has revolutionised the management of neurogenic bladder-sphincter dysfunctions. The Liquick Base catheter is characterised by a streamlined Ergothan tip. The purpose of this study is to assess the tolerance and perception of patients using this catheter. Materials and methods A French prospective multicentre observational study was conducted on patients with neurogenic bladder-sphincter dysfunctions. Upon inclusion in the study, the doctor completed a questionnaire on the patient's pathology. After 3 and 6 months, the doctor checked for neurogenic developments or observations and looked for any complications relating to intermittent self-catheterisation. The patient completed a questionnaire to assess his or her perception of using the catheter. Results Out of 42 patients included in the study, two were excluded. Out of the 40 assessed patients (30 males, 10 females) with an average age of 50.1 ± 14.9 years, there were no reported cases of false passage. Bleeding occurred at least once in 10 patients (25%) in the first three months and in three out of 20 patients (15%) between 3 and 6 months. Two (5%) patients sought medical attention in the first three months for complications related to the catheter and 4 patients sought medical attention (10%) between 3 and 6 months. After 3 months 90% of patients were still using the catheter and after 6 months 90% of patients were still using the catheter. Conclusion The Liquick Base catheter is well tolerated. Patient perception is positive for all parameters being examined, leading to the continued use of the catheter in 90% of cases. Level of evidence 2.

Abstract

Background: The selection of women with hormone receptor-positive (HR+) early breast cancer (EBC) at high risk of relapse after five years (yrs.) of adjuvant aromatase inhibitors (AIs) is crucial, as the benefit of extending AIs is counterbalanced by toxicity. We developed a clinicopathological tool to estimate the residual risk of relapse after five years of adjuvant AIs. Methods: The Institut de Canc&eacute, rologie de l&rsquo, Ouest (ICO) database was used to determine a prognostic score of post-five-year AI relapse. Cox regression models estimated our score&rsquo, s prognostic performance. Results: In total, 1105 women were included. Median follow-up was 44 months (IQR = 21&ndash, 70) post-AI treatment. From the Cox models, we designed a dichotomous prognostic score including the number of macrometastases, age (&gt, 70 yrs. vs. &le, 70 yrs.), tumor size (&ge, T2 vs. not), and mitotic activity (&ge, 2 vs. not). Overall, 77.5% of patients were classified as being at low risk and 22.5% at high risk of late recurrence. Low-risk patients had a five- to ten-year local or distant recurrence risk of 7.6% (95% CI, 5.4% to 10.6%) as compared with 26.9% (95% CI, 19.9% to 35.7%) for the high-risk roup. Conclusion: In this study, we developed a simple tool to identify women at high risk of relapse despite completing five years of AIs.

Abstract

Introduction The anal tone allows the maintenance of anorectal continence. Its regulation depends on spinal segmental mechanisms under supra-sacral control. Material and methods A systematic review was performed using Medline database, according to PRISMA methodology, using following keywords anal tone ; anal sphincter ; anorectal function ; reflex ; digital rectal examination. Results Anal hypertonia is an increase in the muscle's resistance to passive stretching. Muscular hypotonia is a decrease in muscle tone. It is associated with a decrease in resistance to passive mobilization. It is not possible to quantify the prevalence of anal tone alterations in the general population and in specific pathological conditions (urinary disorders, neurogenic or non-neurogenic anorectal disorders). In case of hypotonia, most often due to a lower motor neuron lesion, fecal incontinence may occur. Hypertonia (anal sphincter overactivity) is not always due to perineal spasticity. Indeed, in the majority of the cases, the cause of this anal hypertonia in a neurologic context, can be secondary to an upper motor neuron disease due to spinal or encephalic lesion, leading to recto-anal dyssynergia, giving distal constipation. In another way, this anal hypertonia can be purely behavioral, with no direct pathological significance. The evaluation of anal tone is clinical with validated scores but whose sensitivity is not absolute, and instrumental with, on the one hand, the measurement of anal pressure in manometry and, on the other hand, electrophysiological testing which still require validation in this indication. Conclusion Anal tone assessment is of interest in clinical practice because it gives diagnostic arguments for the neurological lesion and its level, in the presence of urinary or anorectal symptoms.

Abstract

Resume Objectifs Les potentiels evoques somesthesiques corticaux (PES) sont habituellement utilises pour tester l’integrite des voies lemniscales et donner ainsi des arguments en faveur de l’etiologie neurogene de symptomes sensoriels. Ainsi, les PES par stimulation perineale (PES-P), ont ete demontres alteres dans l’incontinence ou les troubles sexuels neurogenes. Nous avons voulu verifier integrite, structuration et amplitude des reponses en champ lointain des PES-P dans deux conditions, la premiere sans sensation de besoin d’uriner (B0), la seconde avec besoin imperieux (BI). Methodes Les PES-P ont ete enregistres chez dix patients sans pathologie neurologique dans les deux conditions B0 et BI apres stimulation du nerf dorsal du penis/clitoris. Trois reponses consecutives moyennees chacune sur 1000 passages a une frequence 3 Hz ont ete enregistrees. Resultats Sept hommes, 3 femmes, d’âge moyen 53,8, (sd 16,8) ont ete inclus. Tous les patients avaient des PES-P normaux en termes d’amplitude et de latence du complexe P40 et totalement reproductibles notamment pour les reponses tardives. Ces reponses precoces P40 etaient identiques dans les deux etats BO et BI. Inversement, les potentiels de champ lointain, c’est-a-dire, les reponses tardives, etaient differentes dans les deux etats, avec une importante diminution des amplitudes moyennes et maximales des reponses corticales dans l’etat BI (p Conclusion Nous avons observe que les composantes tardives des PES-P sont alterees lors du besoin urgent d’uriner accompagne d’une attention soutenue et selective sur ce besoin. Ces composantes tardives du PES-P pourraient etre utiles pour mieux specifier les mecanismes attentionnels impliques dans le cycle continence-miction et pour preciser les dysfonctionnements sensoriels pathologiques (urgence, vessie douloureuse, frequence…). Abstract

Urodynamic (UD) is an exam intended to explore the mechanisms underlying lower urinary tract symptoms (LUTS) or urinary incontinence (UI). It involves the measurement of bladder and sphincter pressures using uretrovesical and rectal catheters with pressure transducers, but also the measurement of urinary flow and bladder sensation during filling. UD is far from being systematic in the assessment of LUTS or UI and must seek to tackle a specific clinical or therapeutic question. Thus, history taking, physical examination, voiding diary and questionnaires are essential prerequisites to UD per se. UD steps include a free (unintubated) uroflowmetry, a cystometry, post-void residual measurement±an urethral profilometry, a pressure-flow study or sensitization tests. The pressures are set to zero before to start the study and the validity of the equipment is tested. This control is continued throughout the procedure to ensure the quality of the recording. Any event (e.g., urine leakage, change of position, urgency) is noted during the study. A final report is made by the doctor. The competence of the nurse ensures the reliability, reproducibility and interpretability of the UD study and the nurse's humanity guarantee f an atmosphere as favourable as possible for this uncomfortable and invasive test.

Abstract

Management of pregnancy and delivery in women with lower urinary tract reconstruction is challenging and the currently available literature is insufficient to guide clinical practice. We report pregnancy and delivery outcomes in this specific population.We conducted a national multicenter retrospective study (16 centers) including 68 women with 96 deliveries between 1998 and 2019. These women had at least 1 successful pregnancy and delivery after augmentation enterocystoplasty, catheterizable channel creation and/or artificial urinary sphincter implantation. Maternal and fetal complications during pregnancy and delivery were reported, as well as postpartum functional outcomes, according to the delivery mode. The chi-square test and Student's t-test were used to compare categorical and continuous variables, respectively.Overall 32% of reported pregnancies were complicated by febrile urinary tract infections, 13.5% by renal colic and 14.6% required upper urinary tract diversion. In addition, 10% of patients reported transient self-catheterization difficulties and 13.5% reported de novo or increased urinary incontinence. The preterm delivery rate was 35.3%. Elective C-section was performed in 61% of pregnancies. Twenty complications occurred during delivery (20%), including 19 during elective C-section. Urinary continence at 1 year was unchanged for 93.5% of deliveries. Delivery mode (p=0.293) and multiparity (p=0.572) had no impact on urinary continence.In this population C-section appeared to be associated with a high risk of complications. In the absence of any obstetric or neurological contraindications, vaginal delivery should be proposed as the first line option to the majority of these women.

Abstract

Dynamic Velocity Prediction Programs are taking an increasingly prominent role in high performance yacht design, as they allow to deal with seakeeping abilities and stability issues. Their validation is however often neglected for lack of time and data. This paper presents an experimental campaign carried out in the towing tank of the Ecole Centrale de Nantes, France, to validate the hull modeling in use in a previously presented Dynamic Velocity Prediction Program. Even though with foils, hulls are less frequently immersed, a reliable hull modeling is necessary to properly simulate the critical transient phases such as touchdowns and takeoffs. The model is a multihull float with a waterline length of 2:5 m. Measurements were made in head waves in both captive and semi-captive conditions (free to heave and pitch), with the model towed at constant yaw and speed. To get as close as possible to real sailing conditions, experiments were made at both zero and non-zero leeway angles, sweeping a wide range of speed values, with Froude numbers up to 1:2. Both linear and nonlinear wave conditions were studied in order to test the limits of the modeling approach, with wave steepness reaching up to 7% in captive conditions and 3:5% in semicaptive ones. The paper presents the design and methodology of the experiments, as well as comparisons of measured loads and motions with simulations. Loads are shown to be consistent, with a good representation of the sustained non-linearities. Pitch and heave motions depict an encouraging correlation which confirms that the modeling approach is valid.

Abstract

The anal tone allows the maintenance of anorectal continence. Its regulation depends on spinal segmental mechanisms under supra-sacral control.A systematic review was performed using Medline database, according to PRISMA methodology, using following keywords anal tone ; anal sphincter ; anorectal function ; reflex ; digital rectal examination.Anal hypertonia is an increase in the muscle's resistance to passive stretching. Muscular hypotonia is a decrease in muscle tone. It is associated with a decrease in resistance to passive mobilization. It is not possible to quantify the prevalence of anal tone alterations in the general population and in specific pathological conditions (urinary disorders, neurogenic or non-neurogenic anorectal disorders). In case of hypotonia, most often due to a lower motor neuron lesion, fecal incontinence may occur. Hypertonia (anal sphincter overactivity) is not always due to perineal spasticity. Indeed, in the majority of the cases, the cause of this anal hypertonia in a neurologic context, can be secondary to an upper motor neuron disease due to spinal or encephalic lesion, leading to recto-anal dyssynergia, giving distal constipation. In another way, this anal hypertonia can be purely behavioral, with no direct pathological significance. The evaluation of anal tone is clinical with validated scores but whose sensitivity is not absolute, and instrumental with, on the one hand, the measurement of anal pressure in manometry and, on the other hand, electrophysiological testing which still require validation in this indication.Anal tone assessment is of interest in clinical practice because it gives diagnostic arguments for the neurological lesion and its level, in the presence of urinary or anorectal symptoms.

Abstract

Objective To analyze and quantify sacral spinal excitability through bulbocavernosus reflex (BCR) stimulus-response curves. Methods Thirty subjects with upper motor neuron lesions (UMN) and nine controls were included in this prospective, monocentric study. Sacral spinal excitability was assessed using stimulus-response curves of the BCR, modeled at different bladder filling volumes relative to the desire to void (as defined by the International Continence Society) during a cystometry. Variations in α (i.e. the slope of the stimulus-response curve) were considered as an indicator of the modulation of sacral spinal excitability. Results In all subjects, α increased during bladder filling suggesting the modulation of spinal sacral excitability during the filling phase. This increase was over 30% in 96.7% of neurological subjects and 88.9% of controls. The increase was higher before the first sensation to void in the neurological population (163.15%), compared to controls, (29.91%), p Conclusions We showed the possibility of using BCR stimulus-response curves to characterize sacral spinal response with an amplification of this response during bladder filling as well as a difference in this response amplification in patients with UMN in comparison with a control group. Significance BCR, through stimulus-response curves, might be an indicator of pelvic-perineal exaggerated reflex response and possibly a tool for evaluating treatment effectiveness.

Abstract

Cortical somatosensory evoked potentials (SEP) are usually used to test the integrity of lemniscal pathways and thus provide arguments for the neurogenic etiology of sensory symptoms. For example, PES by perineal stimulation (SEP-P) has been shown to be altered in incontinence or neurogenic sexual dysfunction. We wanted to verify the integrity, structure and amplitude of far-field responses of PES-P in two conditions, the first without feeling the need to urinate (S0), the second with urgency (US).SEP-P were recorded in ten patients without neurological pathology in both conditions S0 and US after stimulation of the dorsal nerve of the penis/clitoris. Three consecutive responses each averaged over 1000 passages at a frequency of 3Hz were recorded.Seven men and 3 women were included. All patients had normal SEP-P in terms of amplitude and latency of the P40 complex and fully reproducible especially for late responses. These early P40 responses were identical in both S0 and US states. Conversely, the far-field potentials, i.e. the late responses, were different in the two states, with a significant decrease (P0.008 paired T-test) in the amplitude of cortical responses in the US state.We observe that the late components of SEP-P were altered by the need to urinate urgently with sustained and selective attention to this need. These late components of SEP-P could be useful to better specify the attentional mechanisms involved in the continence-voiding cycle and to specify pathological sensory dysfunctions (urgency, painful bladder, frequency…).4.

Abstract

The solar corona is a highly-structured plasma which reaches temperatures of more than ~2MK. At low radio frequencies (≤ 400 MHz), scattering and refraction of electromagnetic waves are thought to broaden sources to several arcminutes. However, exactly how source size relates to scattering due to turbulence is still subject to investigation. This is mainly due to the lack of high spatial resolution observations of the solar corona at low frequencies. Here, we use the LOw Frequency ARray (LOFAR) to observe the solar corona at 120-180 MHz using baselines of up to ~3.5 km (~1--2’) during a partial solar eclipse of 2015 March 20. We use a lunar de-occultation technique to achieve higher spatial resolution than that attainable via traditional interferometric imaging. This provides a means of studying source sizes in the corona that are smaller than the angular width of the interferometric point spread function.

Abstract

Purpose To develop and measure the uptake of a local guideline for herpes simplex keratitis (HSK) and to standardize initial antiviral therapy in Australia. Methods The Registered Nurses' Association of Ontario Toolkit: "Implementation of Best Practice Guidelines" was used to develop, implement, and evaluate the guideline at Sydney Eye Hospital. An implementation team was established to reach consensus on antiviral therapy guidelines through review of available evidence, identifying stakeholders, facilitators and barriers, creating strategies for implementation, and developing a sustainability plan. An audit of all adult HSK cases during a 6-month postguideline implementation period was conducted, and the results were compared with a preimplementation audit. A web-based survey was created to assess clinician awareness, usage, and level of knowledge of the guideline. Results Clinicians, pharmacists, and administrative staff were identified as stakeholders. Changing clinician's behavior was the major barrier to implementation. Implementation strategies included printed and online materials and lectures to clinicians. A postimplementation audit included 85 patients, and 95 clinicians received a web-based survey. The dose of the prescribed antiviral medication was in alignment with the local guideline in 80% (51/64) of the patients compared with 73% (163/223) before implementation (P = 0.331). Stromal HSK with ulceration and keratouveitis were excluded because there were no recommendations before implementation. Over 70% of clinicians (30/41) were aware of the guideline and accessed them through educational resources. Conclusions Guidelines for the management of HSK may improve standardization of initial antiviral therapy in HSK. In practice, most clinicians were aware of and adhered to the local guideline.
