Abstract

Background: Civilian studies suggest that trending Shock-Index Pediatric Adjusted(SIPA) values can prove useful in the prediction of trauma outcomes. The purpose of this study was to evaluate the relationship between trends in SIPA and outcomes in pediatric warzone trauma. Methods: Retrospective review of the Department of Defense Trauma Registry from 2008 to 2015, including all patients age ≤17years. SIPA was calculated both pre-hospital and upon arrival, then classified as “normal” or “abnormal” based upon previously validated thresholds. Patients were stratified into groups based on the trend of their SIPA (1-normal to normal, 2-normal to abnormal, 3-abnormal to normal, 4-abnormal to abnormal). Key outcomes including ICU admission, severe injury, mechanical ventilation, and mortality were then compared between groups. Results: 669 patients were included, mean ISS 12 ± 10. The most common mechanism of injury was blast (46.5%). Overall, 43% were stratified into Group 1, 13.9% into Group 2, 14.8% into Group 3, and 28.0% into Group 4. Those patients with a persistently abnormal SIPA (Group 4) had significantly increased incidence of severe injury, ICU admission, need for mechanical ventilation, and mortality. Conclusion: Trends in SIPA may be used to predict trauma outcomes for children injured in warzones, with persistently abnormal values associated with worse outcomes overall.

Abstract

Purpose: The aim of this study was to estimate a metastasis prediction image factor in non-small cell lung cancer by correlation next generation sequence gene expression level and fluorine-18-2-fluoro-2-deoxy-D-glucose positron emission tomography image features.Methods: RNA-sequencing data and 18F-FDG PET images of 63 patients with NSCLC (29 metastasis and 34 non-metastasis) from The Cancer Imaging Archive and The Cancer Genome Atlas Program databases were used in a combined analysis. Weighted correlation network analysis was performed to identify gene groups were related metastasis. Module was selected with high module significance. Genes selection was performed by gene function related metastasis and high AUC (AUC > 0.6). A total of 47 image features were extracted from PET images as radiomics. The relationship of Gene expression and image features were calculated by using a hypergeometric distribution test with the Pearson correlation method. Metastasis prediction model was validated by random forest algorithm using image texture features related gene expression.Results: 36 modules were identified by gene expression pattern with WGCNA assay. The modules had highest module significance was selected assay. 7 genes from selected module were identified to involve in the epithelial mesenchymal transition pathway that have important role in the cancer metastasis and had high AUC. Also, expression of these genes was related to quantitative of image feature (GLCM_contrast, -log10 P-value: 2.45~3.89). The AUC value (accuracy: 0.856 ± 0.06, AUC: 0.868 ± 0.05) was shown from the EMT-related gene and GLCM_contrast model and AUC value (accuracy: 0.842 ± 0.06, AUC: 0.838 ± 0.09) was shown from GLCM_contrast image texture model. Conclusion: GLCM_contrast image texture feature shows relationship with EMT related gene expression. We developed a model for predicting metastasis of non-small cell lung cancer using 18F-FDG PET image feature and evaluated its accuracy.

Abstract

Objectives Partial restoration of aortic flow during resuscitative endovascular balloon occlusion of the aorta (REBOA) is advocated by some to mitigate distal ischemia. Our laboratory has validated the mechanics and optimal partial REBOA (pREBOA) flow rates using a prototype device. We hypothesize that pREBOA will increase survival when compared with full REBOA (fREBOA) in prolonged nonoperative management of hemorrhagic shock. Methods Twenty swine underwent placement of aortic flow probes, zone 1 REBOA placement, and 20% blood volume hemorrhage. They were randomized to either solid organ or abdominal vascular injury. The pREBOA arm (10 swine) underwent full inflation for 10 minutes and then deflation to a flow rate of 0.5 L/min for 2 hours. The fREBOA arm (10 swine) underwent full inflation for 60 minutes, followed by deflation/resuscitation. The primary outcome is survival, and secondary outcomes are serologic/pathologic signs of ischemia-reperfusion injury and quantity of hemorrhage. Results Two of 10 swine survived in the fREBOA group (2/5 solid organ injury; 0/5 abdominal vascular injury), whereas 7 of 10 swine survived in the pREBOA group (3/5 solid organ injury, 4/5 abdominal vascular injury). Survival was increased (p = 0.03) and hemorrhage was higher in the pREBOA group (solid organ injury, 1.36 ± 0.25 kg vs. 0.70 ± 0.33 kg, p = 0.007; 0.86 ± 0.22 kg vs. 0.71 ± 0.28 kg, not significant). Serum evidence of ischemia was greater with fREBOA, but this was not significant (e.g., lactate, 16.91 ± 3.87 mg/dL vs. 12.96 ± 2.48 mg/dL at 120 minutes, not significant). Swine treated with pREBOA that survived demonstrated trends toward lower alanine aminotransferase, lower potassium, and higher calcium. The potassium was significantly lower in survivors at 60 minutes and 90 minutes time points (5.97 ± 0.60 vs. 7.53 ± 0.90, p = 0.011; 6.67 ± 0.66 vs. 8.15 ± 0.78, p = 0.029). Calcium was significantly higher at 30 minutes, 60 minutes, and 90 minutes (8.56 ± 0.66 vs. 7.50 ± 0.40, p = 0.034; 8.63 ± 0.62 vs. 7.15 ± 0.49, p = 0.019; 8.96 ± 0.64 vs. 7.00, p = 0.028). Conclusion Prolonged pREBOA at a moderate distal flow rate provided adequate hemorrhage control, improved survival, and had evidence of decreased ischemic injury versus fREBOA. Prophylactic aggressive calcium supplementation may have utility before and during the reperfusion phase.

Abstract

The spin-structure functions g1 and g2, and the spin-dependent partial cross-section σTT have been extracted from the polarized cross-sections differences, Δσ∥(ν,Q2) and Δσ⊥(ν,Q2) measured for the He3→(e→,e′)X reaction, in the E97-110 experiment at Jefferson Lab. Polarized electrons with energies from 1.147 to 4.404 GeV were scattered at angles of 6∘ and 9∘ from a longitudinally or transversely polarized $^{3}$He target. The data cover the kinematic regions of the quasi-elastic, resonance production and beyond. From the extracted spin-structure functions, the first moments Γ1‾(Q2), Γ2(Q2) and ITT(Q2) are evaluated with high precision for the neutron in the Q2 range from 0.035 to 0.24GeV2. The comparison of the data and the chiral effective field theory predictions reveals the importance of proper treatment of the Δ degree of freedom for spin observables.

171. Insulin suppresses PDK-4 expression in skeletal muscle independently of plasma FFA

Abstract

Starvation and experimental diabetes induce a stable increase in pyruvate dehydrogenase kinase (PDK) activity in skeletal muscle, which is largely due to a selective upregulation of PDK-4 expression. Increased free fatty acid (FFA) level has been suggested to be responsible for the upregulation. Because these metabolic states are also characterized by insulin deficiency, the present study was designed to examine whether insulin has a significant effect to regulate PDK mRNA expression in rat skeletal muscle. In study 1, overnight-fasted rats received an infusion of saline or insulin for 5 h (n = 6 each). During the insulin infusion, plasma glucose was clamped at basal levels (euglycemic hyperinsulinemic clamp). A third group (n = 6) received Intralipid infusion during the clamp to prevent a fall in plasma FFA. PDK-2 mRNA level in gastrocnemius muscle was not altered by insulin or FFA (i.e., Intralipid infusion). In contrast, PDK-4 mRNA level was decreased 72% by insulin (P < 0.05), and Intralipid infusion prevented only 20% of the decrease. PDK-4 protein level was decreased ~20% by insulin (P < 0.05), but this effect was not altered by Intralipid infusion. In study 2, overnight-fasted rats were refed or received an infusion of saline or nicotinic acid (NA, 30 [micro]mol/h) for 5 h (n = 5 each). During the refeeding and NA infusion, plasma FFA levels were similarly (i.e., 60-70% vs. saline control) lowered. Muscle PDK-4 mRNA level decreased 77% after the refeeding (P < 0.05) but not after the NA infusion. In conclusion, the present data indicate that insulin had a profound effect to suppress PDK-4 expression in skeletal muscle and that, contrary to previous suggestions, circulating FFA had little impact on PDK-4 mRNA expression, at least within 5 h. pyruvate dehydrogenase kinase; free fatty acid; insulin resistance; forkhead transcription factor

Research has been conducted on the Brca1 C-terminal domains, protein-protein interaction motif in the proteins involved in the DNA damage response and DNA repair. The structures of the human 53BP1-p53 complex and of the rat Brac1 BRCT repeats are described.
