Abstract

Liver surgery is the first line treatment for hepatocarcinoma. Hepatocarcinoma Recurrence on the Liver Study (HERCOLES) Group was established in 2018 with the goal to create a network of Italian centres sharing data and promoting scientific research on hepatocellular carcinoma (HCC) in the surgical field. This is the first national report that analyses the trends in surgical and oncological outcomes. Register data were collected by 22 Italian centres between 2008 and 2018. One hundred sixty-four variables were collected, regarding liver functional status, tumour burden, radiological, intraoperative and perioperative data, histological features and oncological follow-up. 2381 Patients were enrolled. Median age was 70 (IQR 63–75) years old. Cirrhosis was present in 1491 patients (62.6%), and Child-A were 89.9% of cases. HCC was staged as BCLC0-A in almost 50% of cases, while BCLC B and C were 20.7% and 17.9% respectively. Major liver resections were 481 (20.2%), and laparoscopy was employed in 753 (31.6%) cases. Severe complications occurred only in 5%. Postoperative ascites was recorded in 10.5% of patients, while posthepatectomy liver failure was observed in 4.9%. Ninety-day mortality was 2.5%. At 5 years, overall survival was 66.1% and disease-free survival was 40.9%. Recurrence was intrahepatic in 74.6% of cases. Redo-surgery and thermoablation for recurrence were performed up to 32% of cases. This is the most updated Italian report of the national experience in surgical treatment for HCC. This dataset is consistently allowing the participating centres in creating multicentric analysis which are already running with a very large sample size and strong power.

Abstract

Background: This study aimed to assess the best achievable outcomes in laparoscopic liver resection (LLR) after risk adjustment based on surgical technical difficulty using a national registry. Methods: LLRs registered in the Italian Group of Minimally Invasive Liver Surgery registry from November 2014 to March 2018 were considered. Benchmarks were calculated according to the Achievable Benchmark of Care (ABCTM). LLRs at each centre were divided into three clusters (groups I, II and III) based on the Kawaguchi classification. ABCs for overall and major morbidity were calculated in each cluster. Multivariable analysis was used to identify independent risk factors for overall and major morbidity. Significant variables were used in further risk adjustment. Results: A total of 1752 of 2263 patients fulfilled the inclusion criteria: 1096 (62·6 per cent) in group I, 435 (24·8 per cent) in group II and 221 (12·6 per cent) in group III. The ABCs for overall morbidity (7·8, 14·2 and 26·4 per cent for grades I, II and II respectively) and major morbidity (1·4, 2·2 and 5·7 per cent) increased with the difficulty of LLR. Multivariable analysis showed an increased risk of overall morbidity associated with multiple LLRs (odds ratio (OR) 1·35), simultaneous intestinal resection (OR 3·76) and cirrhosis (OR 1·83), and an increased risk of major morbidity with intestinal resection (OR 4·61). ABCs for overall and major morbidity were 14·4 and 3·2 per cent respectively for multiple LLRs, 30 and 11·1 per cent for intestinal resection, and 14·9 and 4·8 per cent for cirrhosis. Conclusion: Overall morbidity benchmarks for LLR ranged from 7·8 to 26·4 per cent, and those for major morbidity from 1·4 to 5·7 per cent, depending on complexity. Benchmark values should be adjusted according to multiple LLRs or simultaneous intestinal resection and cirrhosis.

Abstract

Background: We aimed to assess the ability of comprehensive complication index (CCI) and Clavien‐Dindo complication (CDC) scale to predict excessive length of hospital stay (e‐LOS) in patients undergoing liver resection for hepatocellular carcinoma. Methods: Patients were identified from an Italian multi‐institutional database and randomly selected to be included in either a derivation or validation set. Multivariate logistic regression models and ROC curve analysis including either CCI or CDC as predictors of e‐LOS were fitted to compare predictive performance. E‐LOS was defined as a LOS longer than the 75th percentile among patients with at least one complication. Results: A total of 2669 patients were analyzed (1345 for derivation and 1324 for validation). The odds ratio (OR) was 5.590 (95%CI 4.201; 7.438) for CCI and 5.507 (4.152; 7.304) for CDC. The AUC was 0.964 for CCI and 0.893 for CDC in the derivation set and 0.962 vs. 0.890 in the validation set, respectively. In patients with at least two complications, the OR was 2.793 (1.896; 4.115) for CCI and 2.439 (1.666; 3.570) for CDC with an AUC of 0.850 and 0.673, respectively in the derivation cohort. The AUC was 0.806 for CCI and 0.658 for CDC in the validation set. Conclusions: When reporting postoperative morbidity in liver surgery, CCI is a preferable scale.

Abstract

ESA Science Programme Committee (SPC) selected CHEOPS as the first small class science mission in 2012. CHEOPS is considered as a pilot case for the implementation of “small science missions” and its success is key for the continuation of fast-paced, small missions. The mission has been developed and brought into a flight readiness state within 5-6 years from selection, which is about half the time of other ESA missions. This paper focuses on the CHEOPS payload and its predicted capabilities. The 300mm effective aperture Ritchey-Chretien telescope provided by the CHEOPS consortium has been tested and characterized on ground in a 2 months calibration campaign after the qualification for flight. The results have led to performance estimations, which are discussed here. We show that the performance requirements in flight are expected to be met by the instrument. A preview is given towards the 2 months lasting In Orbit Commissioning (IOC) phase of the CHEOPS payload after LEOP and platform check-out. The activities in orbit range from dark current measurements, PSF characterization and parasitic stray light determination to AOCS and instrument performance verifications to science validation using reference transits.

Abstract

The European Space Agency (ESA) Science Programme Committee (SPC) selected CHEOPS (Characterizing Exo-planets Satellite) in October 2012 as the first S-class mission (S1) within the Agency\'s Scientific Programme, targeting launch readiness by the end of 2018. The CHEOPS mission is devoted to the first-step characterization of known exoplanets orbiting bright stars, to be achieved through the precise measurement of exo-planet radii using the technique of transit photometry. It is implemented as a partnership between ESA and a consortium of Member States led by Switzerland. CHEOPS is considered as a pilot case for implementing \"small science missions\" in ESA with the following requirements: science driven missions selected through an open Call for missions (bottom-up process); spacecraft development schedule much shorter than for M (medium) and L (large) missions, in the range of 4 years; and cost-capped missions to ESA with possibly higher Member States involvement than for M or L missions. To reach its scientific goal of accurately characterizing exoplanet sizes, CHEOPS instrument will measure photometric signals with a precision of 20 ppm in 6 hours of integration time for a 9th magnitude star and a precision of 85 ppm in 3 hours integration time for Neptune-size planets orbiting 12th magnitude star. By being able to point at large portion of the sky, with 50% of the whole sky accessible for 50 cumulative days of observations per year, CHEOPS will provide the unique capability of determining accurate radii for a subset of those planets for which the mass has already been estimated from ground-based Doppler surveys. The instrument provided by a consortium led by the University of Bern is an optical Ritchey-Chretien telescope with 300 mm effective aperture diameter, which provides a defocussed image of the target star on a single frame-transfer backside illuminated CCD detector. Straylight is minimized using a large external baffle and a dedicated field stop. AIRBUS DS Spain was awarded with the contract to manufacture and integrate the satellite platform, the integration of the scientific instrument, the realization of the satellite functional and environmental campaigns, the launch campaign and the LEOP (lunch and early operation phase) and IOC (in orbit commissioning) phases. The platform is based on heritage from AS250, which is a compact, high performance and flight proven design. The paper reports the Cheops satellite readiness for flight, including instrument calibration and spacecraft environmental test campaign and the preparation for launch which is planned for November 2019 on a shared Soyuz launcher.

Abstract

Objective: Laparoscopic liver resection (LLR) for Hepatocellular Carcinoma (HCC) is one of the most important indications for the minimally invasive approach. Our study aims to analyze the experience of the Italian Group of Minimally Invasive Liver Surgery with laparoscopic surgical treatment of HCC, with a focus on tumor location and how it affects morbidity and mortality. Methods: 38 centers participated in this study; 372 cases of LLR for HCC were prospectively enrolled. Patients were divided into two groups according to the HCC nodule location. Group 1 favorable location and group 2 unfavorable location. Perioperative outcomes were compared between the two groups before and after a propensity score match (PS) 1:1. Results: Before PS in group 2 surgical time was longer; conversion rate was higher; postoperative transfusion and comprehensive complication index were also higher. PS was performed with a cohort of 298 patients (from 18 centers), with 66 and 232 patients with HCC in unfavorable and favorable locations, respectively. After PS matching, 62 patients from group 1 and group 2 each were compared. Operative and postoperative course were similar in patients with HCC in favorable and unfavorable LLR locations. Surgical margins were found to be identical before and after PS. Conclusions: These results show that LLR in patients with HCC can be safely performed in all segments because of the extensive experience of all surgeons from multiple centers in performing traditional open liver surgery as well as laparoscopic surgery.
