25 páginas, 6 figuras, 2 tablas, Characterization of the genetic landscape of Alzheimer's disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/'proxy' AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOE ε4 allele., This work was funded by a grant (EADB) from the EU Joint Programme – Neurodegenerative Disease Research. Introduction: A bulla is defined as an intraparenchymal air-filled space of more than 2 cm in diameter in distended state within the lung which has developed because of emphysematous destruction of the lung parenchyma and loss of alveolar structural tissue. Indications for surgical resection of bullae include incapacitating dyspnea, a large bulla that fills >30% of the hemithorax, as seen in this case. Patients with Lung emphysematous bulla are at increased perioperative risk of rupture & barotrauma resulting in tension pneumothorax & compromised respiratory status. After a meticulous clinical and radiological assessment, patients with bullae can be taken up for surgeries like local excision/bullectomy, lobectomy, segmental resection, and minimal access surgeries. Case Report: We present an atypical case of a 26-year-old female with a large bulla measuring 16 x 21 x17cm arising from left lower lobe with centrilobular emphysema with passive collapse of left upper lobe and gross mediastinal shift to the right, managed successfully using lung isolation technique and peri-operative pain management. Conclusion: Anaesthetic management of a patient with lung bulla is challenging. Anesthesiologist should deliberate over an extensive preoperative assessment to identify the high-risk factors, reversibility of underlying lung pathology, need and dependency on mechanical ventilation, and establish a successful perioperative management plan for a smooth, pain free post-operative recovery in such patients. In this case, we describe the anaesthetic considerations of a patient with multiple large lung bullae underwent elective bullectomy under general anaesthesia. This will help visually impaired person to navigate independently without any external assistance only by using this real-time object detection system. The app uses image processing and machine learning techniques to identify objects in real time via camera and inform visually impaired people about the object and its location via audio output. The inability to differentiate between products leads to many limitations of current methods, including lower accuracy and poor results. The main objective of this proposed work is to provide effective accuracy, the best performance, and a viable option for visually impaired person to make the world a better place for them.

170. Damage detection in hybrid metal-composite plates using ultrasonic guided waves based on outliers estimate

- Abstract
The present research focusses on the development of a robust data-driven damage diagnosis technique to detect different types of damages in a hybrid metal-composite (HMC) plate specimen resulting from manufacturing processes, loading conditions, and ambient environmental conditions. These defects over a course of time deteriorate the load-bearing capacity of the HMC’s and in turn, their reliability in terms of safe operation. In this work, ultrasonic guided waves (UGW) are used for non-destructive evaluation (NDE) of the HMC. The use of UGW for NDE offers advantages such as long-range inspection and sensitivity to small-sized surface and sub-surface damages. The ultrasonic tests are simulated using a pitch-catch active sensing technique at a typical frequency-mode pair best suited to detect and classify damages in the HMCs. The damage-sensitive feature is extracted from the received UGW using Hilbert transform-based feature extraction method. The damage indicator is classified in the damage-sensitive feature space using the root mean square technique identified as outliers, which is further used to classify the detected damages. The achieved results manifest the ability of the proposed technique to be a part of the industrial structural integrity inspection process typically for HMCs in detecting and classifying embedded damages with high accuracy.

171. Estimating the probability of detection of cracks in metal plates using lamb waves

- Abstract
This paper focusses on the development of a data-driven damage detection method to quantify fatigue crack in metal plates using Lamb waves and its reliability using a probability of detection (POD) technique. The guided Lamb waves are generated and sensed using an array of direct-write (DW) polyvinylidene fluoride (PVDF) annular comb shaped transducers designed to explicitly generate a desired guided wave mode in the test specimen. The annular comb design helps generate a single desired wave mode in the specimen thereby suppressing the energy of other wave modes that can be generated simultaneously. The guided wave responses are obtained through a simulation study and are recorded at different progressions of crack. A damage index (DI) is constructed as a function of crack size that can effectively track the change in ultrasonic response variations and for diagnosing fatigue crack in the metallic specimens. This DI is then further used in the POD model to estimate the crack detection probability. The POD curves can be helpful to check the reliability of the proposed inspection system as well as identify the critical experimental parameters that can significantly influence the crack detection results.

Introduction: Chronomodulated pulsatile drug delivery system is basically time-controlled drug delivery system in which the release of drug in the body is allowed to match with circadian rhythm with controlled lag time. Aim and Objective: The objective of this work was to develop and evaluate oral pulsatile tablet of Pantoprazole sodium for the treatment of nocturnal hyperacidity. Pulsatile drug delivery system for Pantoprazole sodium was formulated initially as a core tablet followed by press coated. Five different compositions of the core tablet were prepared by using cross carmellose super disintegrant. Base on drug content and dissolution time F5 formulation optimised and proceeds for press coating. Five different compositions of press coating were prepared and evaluated for in vitro drug release. Result: The formulation C4 achieved a maximum of 99.65% cumulative drug release over a period of 3hr. 30 min. Lag time for C4 formulation was found to be 2 hr. It means that the system was found to be satisfactory in terms of drug release after a lag time. Conclusion: Since the developed formulation when taken at bedtime, the tablet would be expected to release the drug at midnight which provides better control with the nocturnal hyperacidity.

The present research focuses on evaluating the dislocation-induced material nonlinearity in rails caused by falling of parapets or large rocks during an earthquake or land-slide and consequently appraising their health status using an amplitude-based nonlinear parameter based on Rayleigh wave propagation. The falling of parapets or large rocks onto the rail is simulated through a laboratory experiment. Several cases are considered here with impacts of varying intensities to demonstrate the capability of the nonlinear parameter in diagnosing such damages in the rails. It is found that for a pristine specimen, the first peak of the amplitude-based nonlinear parameter matches well with the physics-based nonlinear parameter, whereas there is a considerable deviation between the two for the specimen with impact damage. Moreover, the linear ultrasonic parameter is also evaluated for different states of damage for studying its effectiveness in diagnosing impact damages in rails. The results reveal that the nonlinear ultrasonic-based method proposed here is very sensitive and helps characterize the health status of the rails in a baseline-free manner.

Laparoscopic splenectomy is a gold standard for management of planned benign splenic pathologies. Spontaneous rupture of the spleen (SRS) leading to acute abdomen occurs in only 1% of all splenic ruptures. Laparoscopic splenectomy in traumatic and atraumatic rupture due to intra-splenic pathology is reported. We present the first reported case of laparoscopic splenectomy in a 23-year-old male who presented with hemoperitoneum due to idiopathic or SRS. The procedure was safely accomplished with slight modified technique and minimum usage of advanced gadgets.

Neurological complications among hospitalized COVID-19 patients may be associated with elevated neurodegenerative biomarkers.Among hospitalized COVID-19 patients without a history of dementia (N = 251), we compared serum total tau (t-tau), phosphorylated tau-181 (p-tau181), glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), ubiquitin carboxy-terminal hydrolase L1 (UCHL1), and amyloid beta (Aβ40,42) between patients with or without encephalopathy, in-hospital death versus survival, and discharge home versus other dispositions. COVID-19 patient biomarker levels were also compared to non-COVID cognitively normal, mild cognitive impairment (MCI), and Alzheimer's disease (AD) dementia controls (N = 161).Admission t-tau, p-tau181, GFAP, and NfL were significantly elevated in patients with encephalopathy and in those who died in-hospital, while t-tau, GFAP, and NfL were significantly lower in those discharged home. These markers correlated with severity of COVID illness. NfL, GFAP, and UCHL1 were higher in COVID patients than in non-COVID controls with MCI or AD.Neurodegenerative biomarkers were elevated to levels observed in AD dementia and associated with encephalopathy and worse outcomes among hospitalized COVID-19 patients.

194. CAVIN1-Mediated Endocytosis: A Novel Mechanism Underlying The Interindividual Variability In Drug-Induced Long QT

BackgroundDrug-induced QT prolongation (diLQT) is a feared side-effect as exposing susceptible individuals to fatal arrhythmias. The occurrence of diLQT is primarily attributed to unintended drug interactions with cardiac ion channels, notably the hERG channels that generate the repolarizing current (IKr) and thereby regulate the late repolarization phase. There is an important inter-individual susceptibility to develop diLQT which is of unknown origin but can be reproduced in patient-specific iPSC-derived cardiomyocytes (iPS-CMs).ObjectiveWe aimed to investigate the dynamics of hERG channels in response to sotalol and to identify regulators of the susceptibility to developing diLQT.MethodsWe measured electrophysiological activity and cellular distribution of hERG channels after hERG blocker treatment in iPS-CMs derived from patients with highest or lowest sensitivity (HS or LS) to sotalol administrationin vivo(i.e., based on the measure of the maximal change in QT interval 3 hours after administration). Specific small-interfering RNAs (siRNA) and CAVIN1-T2A-GFP adenovirus were used to manipulateCAVIN1expression.ResultsWhile HS and LS iPS-CMs showed similar electrophysiological characteristics at the baseline, the late repolarization phase was prolonged, and IKrsignificantly decreased after exposure of HS iPS-CMs to low sotalol concentrations. IKrreduction was caused by a rapid translocation of hERG channel from the plasma membrane to the cytoskeleton upon sotalol application. This phenomenon was suppressed by blocking active endocytosis using dynasore.CAVIN1, essential for caveolae biogenesis, was two-times more expressed in HS iPS-CMs and its knockdown using siRNA decreased their sensitivity to sotalol.CAVIN1overexpression in LS iPS-CMs using adenovirus showed reciprocal effects. Mechanistically, we found that treatment with sotalol promoted trafficking of the hERG channel from the plasma membrane to the cytoskeleton through caveolae and in a manner dependent on CAVIN1 expression.CAVIN1silencing reduced the number of caveolae at the membrane and abrogated the internalization of hERG channel in sotalol-treated HS iPS-CMs. CAVIN1 also controlled cardiomyocyte responses to other hERG blockers such as E4031, vandetanib, and clarithromycin.ConclusionsOur study identifies unbridled turnover of the potassium channel hERG as a mechanism supporting the inter-individual susceptibility underlying diLQT development and demonstrates how this phenomenon is finely tuned by CAVIN1.CLINICAL PERSPECTIVEWhat is new?-The inter-individual susceptibility underlying diLQT development involves unbridled turnover of cardiac ion channels from the plasma membrane.-This phenomenon is finely tuned by CAVIN1, a protein that is essential for essential for caveolae biogenesis.-Treatment with hERG blocker promoted trafficking of the hERG channel from the plasma membrane to the cytoskeleton through caveolae and in a manner dependent on CAVIN1 expression.What are the clinical implications?-While congenital long QT is primarily from a genetic origin, the development of drug-induced long QT involves differences in the trafficking machinery of cardiac ion channels.-The prediction of CAVIN1 expression levels could help preventing drug-induced cardiotoxicity.

Phosphine poisoning is responsible for hundreds of thousands of deaths per year in countries where access to this pesticide is unrestricted. Metal phosphides release phosphine gas on contact with moisture, and ingestion of these tablets most often results in death despite intensive support. A 36-year-old woman presented to a regional hospital after ingesting multiple aluminium phosphide pesticide tablets and rapidly developed severe cardiogenic shock. In this case, serendipitous access to an untested Extracorporeal Membrane Oxygenation (ECMO) service of a regional hospital effected a successful rescue and prevented the predicted death. We discuss the toxicology, management and the evidence for and against using ECMO in this acute poisoning.

BACKGROUND Although cholinesterase inhibitors (ChEIs) are the primary treatment for dementia, they are associated with overactive bladder (OAB), necessitating antimuscarinic use. Limited data exist regarding the risk of OAB across individual ChEIs in dementia. This study evaluated the risk of OAB associated with individual ChEIs in older adults with dementia. METHODS This was a new user retrospective cohort study using Medicare claims data involving Parts A, B, and D claims dataset from 2013 to 2015. The study included older adults (aged 65 and older) with a diagnosis of dementia using donepezil, galantamine, or rivastigmine. New ChEI claims were identified with a 6-month baseline washout period. Patients with OAB diagnosis or any antimuscarinic or mirabegron use in the baseline period were excluded. The primary outcome of interest was OAB diagnosis or prescription of antimuscarinics or mirabegron within 6 months of ChEI initiation. Multivariable cox proportional hazards regression with propensity scores (PS) as covariates and inverse probability of treatment weighting generated using generalized boosted models was used to assess the risk of OAB among donepezil, galantamine, and rivastigmine users. RESULTS The study included 524,975 older adults with dementia who were incident users of ChEIs (donepezil 80.72%, rivastigmine 16.41%, galantamine 2.87%). Overall, OAB diagnosis/antimuscarinic/mirabegron prescription was observed in 5.07% of the cohort within 6 months of ChEIs prescription.
