160. From Fetal to Neonatal Neurobehavior.

Abstract

There is also a continuity of fetal and neonatal movements, which are important indicators of developmental processes of the brain. The aim of the paper is to present neurological prenatal and postnatal assessment of behavior, which is affected by the continuity of general and other movements from prenatal to postnatal life. A prenatal neurological test has been developed using four-dimensional ultrasound (4D US) to assess isolated head anteflexion, eye blinking, facial and mouth movements, leg, hand and finger movements, cranial sutures, and general movement (GM) gestalt perception, all included in the Kurjak antenatal neurodevelopmental test (KANET). Neurological assessment should be continued postnatally as clinical investigation and assessment of Prechtl's GMs. Continuity of assessments relates to the continuity of fetal to neonatal movements, understanding which gives clinicians the opportunity for earlier detection of neurological disability. The diagnosis of cerebral palsy (CP) as the most severe neurological disability is retrospective, and it is exceptionally made before the age of 6 months in only the most severely affected infants; the specificity of the diagnosis will improve as the child ages and the nature of the disability evolves. Interest in the diagnosis of neurological impairment among experts using 4D US has recently shifted toward the prenatal period. Are we approaching the era of the development of diagnostic tests to detect nonreassuring fetal neurological status in its intrauterine life to intervene at appropriate times in order to decrease the CP rate? Abstract

Background: While studying fetal behavior using four-dimensional ultrasound (4D US), we have been speculating about the fetal awareness and fetal cognitive function, trying to become familiarized with fetal emotional life and its readiness to separate from the intrauterine environment and begin independent life as a new individual. Aim and objective: The aim and objective to see whether by observing the fetus by 4D US we are able to enter fetal behavior, emotions, mental status, consciousness, awareness, and other states connected with fetal mind and ability of self- regulation. Results: After 24–26 weeks, the fetus has the necessary connections to sense pain. Somatosensory evoked potentials can be registered from the cortex at 29 weeks, and they may provide evidence of pain processing in the somatosensory cortex. According to recent findings, the cortical pain response has been recorded by near- infrared spectroscopy from about 25 weeks. Fetal facial expressions like those of children sustaining pain have been noticed by 4D US. The fetus can alter the frequency, patterning, and coordination of movement in response to sensory challenges, while retention of information from motor experience and motor learning may contribute to normal prenatal motor development. Conclusion: We have learned from the research by 4D US that the fetus is capable of action planning and learning, meaning that probably the capability of being aware and conscious should proceed. Fetal life in utero is dramatic and rich in different experiences, which probably would not be possible without development of fetal awareness and consciousness.

Abstract

Objectives To evaluate the strength of association between maternal and pregnancy characteristics and the risk of adverse perinatal outcomes in pregnancies with laboratory confirmed COVID-19. Methods Secondary analysis of a multinational, cohort study on all consecutive pregnant women with laboratory-confirmed COVID-19 from February 1, 2020 to April 30, 2020 from 73 centers from 22 different countries. A confirmed case of COVID-19 was defined as a positive result on real-time reverse-transcriptase-polymerase-chain-reaction (RT-PCR) assay of nasal and pharyngeal swab specimens. The primary outcome was a composite adverse fetal outcome, defined as the presence of either abortion (pregnancy loss before 22 weeks of gestations), stillbirth (intrauterine fetal death after 22 weeks of gestation), neonatal death (death of a live-born infant within the first 28 days of life), and perinatal death (either stillbirth or neonatal death). Logistic regression analysis was performed to evaluate parameters independently associated with the primary outcome. Logistic regression was reported as odds ratio (OR) with 95% confidence interval (CI). Results Mean gestational age at diagnosis was 30.6±9.5 weeks, with 8.0% of women being diagnosed in the first, 22.2% in the second and 69.8% in the third trimester of pregnancy. There were six miscarriage (2.3%), six intrauterine device (IUD) (2.3) and 5 (2.0%) neonatal deaths, with an overall rate of perinatal death of 4.2% (11/265), thus resulting into 17 cases experiencing and 226 not experiencing composite adverse fetal outcome. Neither stillbirths nor neonatal deaths had congenital anomalies found at antenatal or postnatal evaluation. Furthermore, none of the cases experiencing IUD had signs of impending demise at arterial or venous Doppler. Neonatal deaths were all considered as prematurity-related adverse events. Of the 250 live-born neonates, one (0.4%) was found positive at RT-PCR pharyngeal swabs performed after delivery. The mother was tested positive during the third trimester of pregnancy. The newborn was asymptomatic and had negative RT-PCR test after 14 days of life. At logistic regression analysis, gestational age at diagnosis (OR: 0.85, 95% CI 0.8–0.9 per week increase; p Conclusions Early gestational age at infection, maternal ventilatory supports and low birthweight are the main determinants of adverse perinatal outcomes in fetuses with maternal COVID-19 infection. Conversely, the risk of vertical transmission seems negligible.

Abstract

WOS:000613461600006 PubMed ID: 32926494 Objectives To evaluate the maternal and perinatal outcomes of pregnancies affected by SARS-CoV-2 infection. Methods This was a multinational retrospective cohort study including women with a singleton pregnancy and laboratory-confirmed SARS-CoV-2 infection, conducted in 72 centers in 22 different countries in Europe, the USA, South America, Asia and Australia, between 1 February 2020 and 30 April 2020. Confirmed SARS-CoV-2 infection was defined as a positive result on real-time reverse-transcription polymerase chain reaction (RT-PCR) assay of nasopharyngeal swab specimens. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit (ICU), use of mechanical ventilation and death. Results In total, 388 women with a singleton pregnancy tested positive for SARS-CoV-2 on RT-PCR of a nasopharyngeal swab and were included in the study. Composite adverse maternal outcome was observed in 47/388 (12.1%) women; 43 (11.1%) women were admitted to the ICU, 36 (9.3%) required mechanical ventilation and three (0.8%) died. Of the 388 women included in the study, 122 (31.4%) were still pregnant at the time of data analysis. Among the other 266 women, six (19.4% of the 31 women with first-trimester infection) had miscarriage, three (1.1%) had termination of pregnancy, six (2.3%) had stillbirth and 251 (94.4%) delivered a liveborn infant. The rate of preterm birth before 37 weeks' gestation was 26.3% (70/266). Of the 251 liveborn infants, 69/251(27.5%) were admitted to the neonatal ICU, and there were five (2.0%) neonatal deaths. The overall rate of perinatal death was 4.1% (11/266). Only one (1/251, 0.4%) infant, born to a mother who tested positive during the third trimester, was found to be positive for SARS-CoV-2 on RT-PCR. Conclusions SARS-CoV-2 infection in pregnant women is associated with a 0.8% rate of maternal mortality, but an 11.1% rate of admission to the ICU. Abstract

Hypertensive syndrome in pregnancy complicates up to 15% of pregnancies, and preeclampsia (PE) occurs in about 3–10% of pregnant women. Inadequate prenatal care is associated with higher mortality from PE, possibly due to reduced monitoring, detection, and missed opportunities for early intervention. The imperative of the clinician’s work is to monitor the symptoms and clinical signs of PE, and stratification of patients in relation to the risk of PE is essential. PE represents a multisystem inflammatory response, and the consequences can be expected in all organs. The question of the effect of PE on long-term maternal health is raised. The aim of the paper is to present the effect of PE on the patient’s health through the prism of low-income countries.

Abstract

Norway spruce is an ecologically and commercially important species, which currently suffers from climate change and is expected to decline because of heat and drought stress. A manipulative experiment with drought-stressed and well-watered (control) seedlings was established, comprising five populations of Norway spruce coming from a small territory but distributed along a steep elevational transect from 550 to 1280 m.a.s.l. Plants were phenotyped at physiological traits (content of phytohormones and monoterpenes, chlorophyll a fluorescence, gas exchange and hyperspectral indices) and genotyped using ddRAD sequencing. After filtering, 34 127 polymorphisms (SNPs) were retained and used for analysis. Association tests revealed 42 SNP-variable pairs located across 29 scaffolds exhibiting significant associations, among them 20 were found for drought-stressed seedlings and 22 for control (watered) plants. However, there was no consistency between drought-stressed and control plants, no SNP-trait pair was common for both treatments. The study demonstrated that the identified genotype–phenotype associations can only be interpreted in the context of environmental setup preceding the assessment of phenotypic traits.

182. The soil hydrogel improved photosynthetic performance of beech seedlings treated under drought

Abstract

The effect of soil amendment with the STOCKOSORB®500 MICRO hydrophilic polymer on the photosynthetic traits in beech seedlings (Fagus sylvatica L.) during 50 days of dehydration was investigated. Dehydration was detected through osmotic potential (Ψs) in the assimilatory organs of beech seedlings. The addition of Stockosorb positively affected the CO2 assimilation rate (A) and instantaneous water use efficiency (A/T), for severely drought-treated seedlings. In comparison with irrigated plants, the values of A of non-irrigated plants with Stockosorb substrate decreased by 50%, and in non-irrigated plants with common substrate by 88%. The fast kinetics of chlorophyll a fluorescence indicated chronic photoinhibition under drought treatment without Stockosorb, while no significant changes in maximal quantum efficiency (Fv/Fm) were recorded under drought treatment with Stockosorb. The actual quantum efficiency of PSII (ΦPSII) markedly decreased in both treatments - with and without Stockosorb, though significant differences were found only between control treatments and drought treatment without Stockosorb. Moreover, the thermal energy dissipation (NPQ) was strongly limited under severe drought stress. The capacity to down regulate PSII functionality through non-photochemical quenching was maintained under drought treatment with Stockosorb. The results indicate that an amendment with soil conditioner significantly improved the photosynthetic performance of drought-stressed beech seedlings.

Abstract

Background: The key considerations for healthy aging are diversity and inequity. Diversity means that there is no typical older person. Policy should be framed to improve the functional ability of all older people, whether they are robust, care dependent or in between. Objective: The aim of this article is to describe negative influence of Corona pandemic (COVID-19) for realization of the WHO project about Healthy Aging global strategy proposed in the targets "Health for all". Methods: Authors used descriptive model for this cross-sectional study based on facts in analyzed scientific literature deposited in on-line databases about healthy aging concept of the prevention and treatment of the people who will come or already came to the "third trimester of the life". Results and Discussion: Some 80-year-olds have levels of physical and mental capacity that compare favorably with 30-year-olds. Others of the same age may require extensive care and support for basic activities like dressing and eating. Policy should be framed to improve the functional ability of all older people, whether they are robust, care dependent or in between. Inequity reflects a large proportion (approximately 75%) of the diversity in capacity and circumstance observed in older age is the result of the cumulative impact of advantage and disadvantage across people's lives. Importantly, the relationships we have with our environments are shaped by factors such as the family we were born into, our sex, ethnicity, level of education and financial resources. Conclusion: COVID-19 pandemic "celebrated" one year of existing in almost all countries in the world with very difficult consequences for whole population. But in the first risk group are old people who have in average 6 to 7 co-morbidities. WHO recommended some measures to improve prevention and treatment this category of population, but COVID-19 pandemic stopped full realization of Decade of Healthy Aging project.

Abstract

Drought tolerance is becoming an increasingly important criterion for the selection of tree species, especially in urban areas characterized by low water availability. Apart from drought tolerance, the introduction of non-native species should be considered for new planting programs under such conditions to enhance the resilience of urban forests. The present study is aimed at evaluating the in situ physiological responses of Magnolia grandiflora and Magnolia × soulangeana to severe drought that frequently occurs in urban environments in the Southeastern Europe. Transpiration rate, stomatal conductance, intercellular CO2 concentration, water-use efficiency and intrinsic water-use efficiency showed notable differences both between species and between the measured periods (wet and dry). Among the chlorophyll a fluorescence parameters, effective photochemical quantum yield of PS II, quantum yield of light-induced non-photochemical fluorescence quenching, quantum yield of non-regulated heat dissipation, fluorescence emission and index of susceptibility of leaves to light stress revealed significant differences both between the two species and the periods of measurements. The reduction of net photosynthesis in both magnolia species occurs as the result of non-stomatal limitation obtained by the reduction of electron transport rate coupled with simultaneous increase in intercellular CO2 concentration. Moreover, M. grandiflora was the species less vulnerable to water shortage conditions, while M. soulangeana exhibited a photosynthetic capacity sensitive to drought-induced stress. M. grandiflora can therefore be considered as a promising alternative to M. soulangeana for urban sites under the predicted climate change scenarios.

Abstract

Nowadays, a large area of Norway spruce forest stands in Europe is disturbed by windstorm and, subsequently, bark beetle outbreaks. We investigated the state of three disturbed spruce stands along an altitudinal gradient in Tatra National Park (Slovakia) through various physiological processes. Tree-growth characteristics, the mineral nutrition in the needles, and photosynthetic efficiency were assessed. Two techniques of chlorophyll a fluorescence and analyses of assimilatory pigments were used to detect the changes in photosynthesis functioning. Also, the heat sensitivity of photosystem II was tested. Our results showed that these stands are located in nutrient-poor environments. We recorded similar contents of nitrogen, phosphorus, potassium, sodium, zinc, and iron in all stands. Down the vertical transect, the contents of calcium, magnesium, and manganese significantly decreased and the non-essential aluminium increased. Based on stem circumference measurements, water deficit occurred during the vegetation season in all stands, but with the smallest magnitude highest U–stand. We found some photosynthetic constraints: slightly lower chlorophyll contents in all stands were recorded; however, seasonal dynamics with increasing chlorophyll concentration in the highest U–stand were observed. Moreover, the photochemistry of the lowest D–stand was the most negatively influenced by simulated heat, as the photosynthetic performance index, and the density of the active reactions centres significantly decreased and the values of the K–step and basal fluorescence increased. Therefore, we can conclude the different levels of physiological vitality in these naturally damaged spruce stands, with the best physiological performance of the trees in the highest stand.

195. Pregnancy Success Prediction Using Uterine and SubEndometrial Arteries Doppler Indices Comparison Between the Trigger Day and the Mid Luteal Phase Day When Using Oral Ovarian Induction Medication in Sub Fertile Patients

Abstract

Background: Doppler of uterine and sub-endometrial arteries might be beneficial for the prediction of pregnancy success. None or few studies evaluated them when using oral ovulation induction. Methods: This study is a randomized prospective study. We recruited infertile couples with PCOS or unexplained subfertility from the infertility unit at Al-Moosa Specialist Hospital in eastern province in the Kingdom of Saudi Arabia. We randomize cycles to use clomiphene or letrozole. We study the uterine arteries doppler indices difference between the hCG trigger day and the mid-luteal phase day with positive and negative pregnancy test groups. Results: In the negative pregnancy test group, there was no sufficient evidence suggesting a difference for sub endometrial artery PI and RI between the trigger day and mid-luteal phase day with a p-value of 0.95 and 0.765, respectively. For the uterine artery PI and RI, there was evidence suggesting a difference between the trigger day and the mid-luteal phase day with a p-value of .003 for both of them. In the positive pregnancy test group, there was evidence to suggest a difference between the trigger day and the mid-luteal phase day for the sub endometrial artery RI with a p-value of 0.035. However, there was no sufficient evidence to suggest that there was a difference for PI of the sub endometrial artery and the uterine artery PI and RI between the day of the trigger and mid-luteal phase day with a p-value of 0.137, 0.533 and 0.687 respectively. Conclusions: When uterine artery PI and RI decrease, we can predict failed ovulation treatment. In comparison, we can predict a successful pregnancy with increasing sub-endometrial artery RI.

Abstract

Effects of drought, Al and their possible interaction on physiological characteristics were studied in four-years-old Norway spruce seedlings in a pot experiment. Drought stress was imposed by withholding irrigation. Al was applied to the soil as an AlCl3 solution at a concentration of 1,500 μmol.l-1. Water deficit caused a decrease in needle water potential, net photosynthetic rate (Pn) and an increase in proline accumulation. On the other hand, water potential, Pn and proline concentration in seedlings subjected to Al remained unchanged. During the experiment, no significant variation was registered in the chlorophyll a fluorescence parameters. Chlorophyll content was significantly reduced in the Al presence. Drought led to a lower Al accumulation in needles in comparison with well-watered seedlings. Progressive dehydration influenced the physiological state of spruce seedlings. The presence of Al in soil did not cause any negative changes in the physiological parameters under an optimal water regime. By contrast, the synergic effect of drought and Al induced the most marked changes in measured characteristics, which may indicate a possible enhanced impact of drought and Al interaction in comparison with the single effect of these stress factors.

Abstract

Fetal growth restriction (FGR) is defined as failure to reach full growth potential in utero and in clinical routine, it is translated as estimated fetal weight (EFW) below the 10th percentile, according to the growth charts. FGR affects 7–10% of pregnancies and it is responsible for the vast majority of perinatal mortality and morbidity. Moreover, FGR is considered as a risk factor for long-term infirmities such as motor skills disorders, cognitive, memory, and neuropsychological impairment, immune disorders, and metabolic syndrome. Importantly, there is an increasing pool of data, suggesting that individuals who as fetuses suffered from FGR, are at high risk for cardiovascular disease (CVD) in adulthood, namely hypertension, atherosclerosis, coronary artery disease, and stroke. Changes in cardiac function and cardiac morphology can be assessed prenatally and especially according to the severity of FGR. © The Author(s).
