Este es un artículo sobre la paz y la educación para la paz, en tiempos de colonización; que no se extinguieron con las declaraciones de independencia de los países del Sur; sino que la colonialidad perpetuo la soslayación. Nuestro principal objetivo es introducir el complejo binomio paz-justicia como necesario para la vida planetaria, ya que no hay paz sin justicia y viceversa. El escrito se construye sobre el pensamiento complejo de Edgar Morin, Raimón Panikkar y el arte de vivir en paz de Pierre Weil, buscando tejer el binomio paz-justicia. Es una escritura rizomática, sustentada en estudios transparadigmáticos. Argumentamos que los predicados del binomio paz-justicia sólo se logran con una educación que provoque la ruptura con la colonialidad global que se impone a las personas, convirtiéndolos en opresores u oprimidos, alimentando el sistema y fortaleciendo el statu quo

En ésta investigación transmetódica cumplimos con el objetivo complejo: analizar al sujeto complejo y la voluntad como necesidades ecosóficas urgentes. Se trata de una investigación transparadigmática compleja, en las líneas de investigación: transepistemologías de los conocimientos-saberes y transmetodologías transcomplejas, y decolonialidad planetaria-complejidad en re-ligaje. Se trata de una antigenealogía de las indagaciones modernistas-postmodernistas-coloniales con el transmétodo: la hermenéutica comprensiva, ecosófica y diatópica entramada rizomáticamente en los momentos analíticos, empíricos y propositivos. Las Sagradas Escrituras y el legado de cristianos como Raimón Panikkar hacen escena en los momentos analíticos – empíricos; entre otros como su prosélito complejo Edgar Morín.

En las líneas de investigación: Decolonialidad planetaria-complejidad en re-ligaje; transepistemologías de los conocimientos-saberes y transmetodologías transcomplejas y educación-transepistemologías transcomplejas, se cumple con objetivo complejo: analizar el cero (0) no tan neutro en la Educación Decolonialidad Planetaria Compleja; con el transmétodo hermenéutica comprensiva, ecosófica y diatópica, en sus momentos: analítico, empírico y propositivo. En el momento propositivo, las conclusiones interpelamos: ¿Qué hacemos con las crueles competencias que nos avasallan en la educación y conforman el centro de dicho proceso? El ser humano necesita formarse y las competencias deben ser complejizadas y humanizadas.

Impact of stabilisers on the catalytic performance of supported Au colloidal nanoparticles for the liquid phase oxidation of glucose to glucaric acid.

La imbricada unión: naturaleza-cuerpo-mente-alma-espíritu-Dios; denota una cualidad sentipensante de la concepción del ser humano que lo hace privilegiado de los demás animales. En la presente investigación se sustentaron transepistemes de la concepción compleja de ser humano: naturaleza-cuerpo-mente-alma-espíritu-Dios como objetivo de investigación. Ubicada en las líneas de investigación: Decolonialidad planetaria-complejidad en re-ligaje y transepistemologías de los conocimientos-saberes y transmetodologías transcomplejas. Marco donde imbricamos una complejidad exquisita del humano ser que debe develarse en la decolonialidad planetaria. Pasamos por los momentos: analíticos, empíricos y propositivos. En este último la antropoética es nuestro accionar en la recivilización de la humanidad como uno de los ejes transepistémicos de lo que es ser individuo humano; individuo en autonomía y dependencia. La esencia compasiva como otro eje transepistémico que transgrede atacarnos a nosotros mismos en tanto al sentirnos naturaleza esa esencia la defendemos de nuestras propias demencias. La transcendencia, ese es un transepisteme que emite respuestas a la pregunta: ¿cómo quiere transcender el ser humano?

Colorectal cancer (CRC) screening programs produce risks, including those derived from colorectal surgeries. The objective of this analysis is to evaluate the complications associated with the surgery.Retrospective analysis including patients who required colorectal surgery within the population-based CRC screening program in Galicia (May 2013-June 2019). We analyzed the indication for surgery and the rate of in-hospital (mildI-II, severeIII-V, Clavien-Dindo classification) and at discharge complications. We performed a multivariate analysis to determine the variables independently associated.In the analyzed period, 1092 patients underwent surgery (benign lesion 16.5%, pT1 CRC 18.2%, rest of CRC 64.6%) laparoscopic approach in 69.8% of the cases. In-hospital complications were detected in 19.2% of patients (mild: 13.4%; severe: 5.9%; deaths: 0.2%) and at discharge in 159 (14.6%) patients. Male sex was associated with in-hospital complications (OR: 2.0; 95%CI: 1.3-3.0). The variables associated with severe complications were: male sex (OR: 2.6; 95%CI: 1.2-5.5), tertiary hospital (OR: 0.5; 95%CI: 0.2-0.9) and ECOGI (OR: 0.2; 95%CI: 0.05-0.6). The factors associated with complications after discharge were age ≥60years (OR: 1.5; 95%CI: 1.0-2.3), rectal location (OR: 1.6; 95%CI: 1.1-2.3) and in-hospital complications (OR: 2.2; 95%CI: 1.5-3.2).Surgery is the main cause of morbidity and mortality associated with a CRC screening program. These results must be taken into account in the decision making of lesions that are candidates for endoscopic resection.

Background Post-transplant prediabetes (PreDM) and diabetes (PTDM) are common and have an impact on cardiovascular events. We sought to investigate the pathogenesis and best approach for prediction. Methods We prospectively studied 115 waitlisted patients from a single center without manifest diabetes. An oral glucose tolerance test (OGTT) was performed yearly until transplantation and 12 months later. Insulin secretion, insulin sensitivity (IS) and disposition index (DI) were derived from the OGTT. Results PreDM and PTDM were observed in 27% and 28.6% of patients, respectively. Pretransplant age, body mass index (BMI), 120 min glucose, IS, DI, and prediabetes or undiagnosed diabetes were significantly associated with these alterations. In multivariate analysis, pretransplant age [odds ratio (OR) 1.5; 95% confidence interval (CI) 1.04–2.1], BMI (OR 1.16; 95% CI 1.04–1.3) and cumulative steroids (OR 1.5; 95% CI 1.02–2.2) were predictors of PreDM or PTDM. Receiver operating characteristic curve analysis showed that pretransplant BMI and 120 min glucose had the highest area under the curve (0.72; 95% CI 0.62–0.8; and 0.69; 95% CI 0.59–0.79, respectively). The highest discrimination cut-off for BMI (≥28.5 kg/m2) and 120 min glucose (≥123.5 mg/dL) yielded a similar number needed to diagnose (2.5). Conclusions PreDM or PTDM develops in waitlisted patients with an ineffective insulin secretion and BMI shows a similar diagnostic capacity to OGTT. Pretransplant interventions may reduce post-transplant glucose alterations.

Pese a grandes avances en la didáctica de la Matemática, el des-ligaje y re-ligaje hacia una Matemática decolonial planetaria está lejos de la realidad en la enseñanza. En la línea de investigación Educación Matemática Decolonial Transcompleja (EMDT), vamos a sustentar la trans-didáctica en la EMDT como objetivo de la indagación. Lo realizamos en el transparadigma complejo con la deconstrucción rizomática como transmétodo. En la reconstrucción construimos estrategias complejas que salvaguardan la etnomatemática-aula mente social-espíritu. La trans-didáctica apertura esencias diatópicas que permiten la conjunción en la Educación Matemática de: abstracto-concreto, teoría-ejemplo, global-local, conocimientos científicos-saberes legos, saberes de las civilizaciones en el Sur- saberes de las civilizaciones en el Occidente-Norte, saberes de las civilizaciones en el Norte — saberes de las civilizaciones en el Sur, mente-alma, razón-emoción, aula física-aula mente social espíritu, entre otros, que en la enseñanza tradicional de la matemática había sido separados.

Different series of Au on carbon catalysts were prepared via sol-immobilization to investigate the role of polymers (polyvinylpyrrolidone PVP, polyethylene glycol PEG and polyvinyl alcohol PVA), employed as gold nanoparticle (NP) stabilizers, on catalyst properties and on catalytic activity. The synthesized materials were widely characterized with several techniques (DLS, XRD, TEM and XPS) and used as catalysts in the 5-hydroxymethylfurfural (HMF) oxidation to produce 2,5-furandicarboxylic acid (FDCA). The obtained results clearly demonstrated the PVA leading to the formation of smaller and more active NPs. On the contrary, polyethylene glycol was shown to affect gold exposure and, as a consequence, to reduce the catalyst activity due to steric effects while PVP-based catalysts presented bigger and more covered Au NPs. The investigation on the reusability of the catalysts demonstrated the presence of a significant deactivation on all prepared materials, but the presence of higher amounts of polymer seems to have a positive effect on catalyst stability even if associated with lower reactivity. Computational studies have provided interesting information on the NP-polymer interactions and consequently on the catalytic activities.

En la presente indagación rizomática transmetódica, con el transparadigma complejo, se analizan re-ligajes en la bioética compleja como aportes en la decolonialidad planetaria; ese el objetivo de la investigación. Ubicada en las líneas de investigación: educación-transepistemologías transcomplejas y transepistemologías de los conocimientos-saberes y transmetodologías transcomplejas. La indagación se realiza con la hermenéutica comprensiva, ecosófica y diatópica en los momentos: analíticos, empíricos y propositivos. En los momentos propositivos cobra sentido el re-ligaje y des-ligaje del pensamiento y con ello, las categorías: ecosofía, diatopia, condición humana, antropoética y antropolítica a favor de una bioética revitalizada.

O currículo universitário venezuelano é analisado como um objetivo complexo como projeto antropolítico de justiça entre os saberes matemáticos. A investigação está localizada na linha de pesquisa intitulada: Educação Matemática Descolonial Transcomplexa. Utiliza-se o transmétodo, a desconstrução rizomática transmoderna como imersão na transmetodologia; neles interferimos como sujeito autor resgatado da pesquisa modernista-colonizadora, na crise venezuelana para dar pisos epistêmicos da crise educacional universitária; e com ela a reconstrução esperançosa para o projeto antropolítico na salvaguarda do conhecimento matemático. Percorremos vários cenários de desvinculação e religação para romper com as disposições rigorosamente impostas de inúmeras esferas de poder nas instituições educacionais e com elas do conhecimento que perece nas mentes dos atores educacionais. O entrelaçamento ideológico, político e epistêmico no papel da Educação Matemática na Venezuela e sua agência colonizadora e contornada justificam a intervenção da decolonialidade planetária como um cenário possível da justiça libertadora do conhecimento matemático.

En la presente investigación analizamos las diatopías en la decolonialidad planetaria como reconciliación en tiempos de dolor. El transparadigma complejo y el transmétodo la hermenéutica comprensiva, ecosófica y diatópica, siguiendo los momentos: analítico, empírico y proposicional. Ubicada en las líneas de investigación tituladas: conocimiento-saber transepistemologías y transmetodologías transcomplejas, y decolonialidad-complejidad planetaria en reconexión. En el momento propuesto, la decolonialidad planetaria tiene un lugar profundo en la medida en que la inclusión es una profunda reconexión en busca de exclusiones a nivel planetario. La reconciliación en tiempos de dolor es fundamental en la medida en que alcancemos la alteridad como comprensión compleja del otro, en medio de su diversidad. la fraternidad como necesidad para la salvación de la Patria; Unidos por un propósito o perecemos. ¿De qué constricción y reconciliación con nuestra esencia espiritual estamos hablando si negamos la fe y Dios como parte de la ciencia y la educación, como anatema a la humanidad, y a la complejidad formativa del ser: somos cuerpo-mente-alma-espíritu-naturaleza? Background Post-transplant diabetes mellitus (PTDM) beyond 12 months (late PTDM) is a severe complication after renal transplantation. Late PTDM develops mostly in subjects with prediabetes. Although exercise may have a potential role in preventing late PTDM, there are no previous data on the effect of exercise in patients with prediabetes. Material and Methods The design was a 12-month exploratory study to test the capacity of exercise in reverting prediabetes in order to prevent late-PTDM. The outcome was the reversibility of prediabetes, assessed every 3 months with oral glucose tolerance tests (OGTT). The protocol included an incremental plan of aerobic and/or strength training as well as an active plan for promoting adherence (telephone calls, digital technology, and visits). A priori, a sample size cannot be calculated which makes this an exploratory analysis. Based on previous studies, the spontaneous reversibility of prediabetes was 30% and the reversibility induced by exercise will account for another 30%, a total reversibility of 60% (p value Results The study was interrupted early due to efficacy after the evaluation of the follow-up of 27 patients. At the end of follow-up, 16 (60%) patients reverted to normal glucose levels at fasting (from 102.13 mg/dL ± 11 to 86.75 ± 6.9, p = 0.006) and at 120 min after the OGTTs (154.44 mg/dL ± 30 to 113.0 ± 13.1, p = 0.002) and 11 patients had persistent prediabetes (40%). Also, insulin sensitivity improved with the reversibility of prediabetes, compared to those with persistent prediabetes: 0.09 [0.08–0.11] versus 0.04 [0.01–0.07], p = 0.001 (Stumvoll index). Most needed at least one increment in the prescription of exercise and compliance. Finally, measures aimed at the improvement of compliance were successful in 22 (80%) patients. Conclusion Exercise training was effective to improve glucose metabolism in renal transplant patients with prediabetes. Exercise prescription must be conducted considering both the clinical characteristics of the patients and pre-defined strategy to promote adherence. Obesity is a global health issue, in which modifications in gut microbiota composition have a key role. Different therapeutic strategies are being developed in combination with diet and exercise, including the use of plant extracts, such as those obtained from Morus alba L. leaves. Recent studies have revealed their anti-inflammatory and antioxidant properties. The aim of the present work was to evaluate whether the beneficial effects of M. alba L. leaf extract in high-fat diet-induced obesity in mice is correlated with its impact on gut microbiota. The extract reduced body weight gain and attenuated lipid accumulation, as well as increased glucose sensitivity. These effects were associated with an amelioration of the obesity-associated inflammatory status, most probably due to the described antioxidant properties of the extract. Moreover, M. alba L. leaf extract mitigated gut dysbiosis, which was evidenced by the restoration of the Firmicutes/Bacteroidota ratio and the decrease in plasma lipopolysaccharide (LPS) levels. Specifically, the extract administration reduced Alistipes and increased Faecalibaculum abundance, these effects being correlated with the beneficial effects exerted by the extract on the obesity-associated inflammation. Obesity is a worldwide public health problem whose prevalence rate has increased steadily over the last few years. Therefore, it is urgent to improve the management of obesity and its comorbidities, and plant-based treatments are receiving increasing attention worldwide. In this regard, the present study aimed to investigate a well-characterized extract of Lavandula multifida (LME) in an experimental model of obesity in mice and explore the underlying mechanisms. Interestingly, the daily administration of LME reduced weight gain as well as improved insulin sensitivity and glucose tolerance. Additionally, LME ameliorated the inflammatory state in both liver and adipose tissue by decreasing the expression of various proinflammatory mediators (Il-6, Tnf-α, Il-1β, Jnk-1, Pparα, Pparγ, and Ampk) and prevented increased gut permeability by regulating the expression of mucins (Muc-1, Muc-2, and Muc-3) and proteins implicated in epithelial barrier integrity maintenance (Ocln, Tjp1, and Tff-3). In addition, LME showed the ability to reduce oxidative stress by inhibiting nitrite production on macrophages and lipid peroxidation. These results suggest that LME may represent a promising complementary approach for the management of obesity and its comorbidities.

Pulque is the most-studied traditional Mexican alcoholic beverage prepared by fermentation of the fresh sap (aguamiel, AM) extracted from different Agave species (maguey) cultivated for pulque production. This beverage has been produced mainly in the Central Mexico Plateau since pre-Columbian times. In this contribution, we report the analysis of the bacterial and fungal diversity through 16S rRNA gene V3–V4 fragment amplicon and ITSR1 sequencing associated with the tissue of the walls (metzal) of the cavity or cajete, where the sap accumulates in producing plants for its daily extraction, in AM, and during four fermentation stages for pulque production. The results led to determining which microorganisms detected in the plant tissue are present in AM and maintained during the fermentation process. The results showed that eight bacterial OTUs (Lactobacillus, Leuconostoc, Weisella, Lactococcus, Acetobacter, Gluconobacter, Zymomonas, and Obesumbacterium) and five fungal OTUs (Kazachstania, Kluyveromyces, Saccharomyces, Hanseniaspora, and the OTU O_Saccharomycetales) were present from metzal to AM and during all the stages of the fermentation analyzed. The detected diversity was considered the microbial core for pulque fermentation, comprising up to ~84% of the total bacterial diversity and up to ~99.6% of the fungal diversity detected in the pulque produced from three plants of A. salmiana from the locality of Huitzilac, Morelos, Mexico. This study provides relevant information on the potential microorganisms responsible for pulque fermentation, demonstrating that the core of microorganisms is preserved throughout the elaboration process and their association with the AM and fermented beverage physiochemical profile.

Background: During the SARS-cov2 pandemic, to evaluate if the clinical protocols in gynecology oncology assistance were changed and, to assess the perception for the management of these disease in the surgeon. Methods: The data were collected by an anonymous and voluntary survey which was sent via email to professionals dedicated to the field of gynaecological oncology in Spain. Results: A total of 75 professionals completed the online survey, 93.2% reported working in a public hospital and 62.5% in a tertiary level hospital. 97.3% belonged to hospitals treating patients with SARS-CoV2 infection. An 85.1% of respondents showed concern about the SARS-CoV2 pandemic and 52.1% indicated the pandemic influenced the diagnostic and therapeutic quality of oncologic patients. SARS-CoV2 PCR on nasopharyngeal exudate was always performed before surgical interventions by 97.3%, being considered best triage method by 94.4%. An 87.5% reported no change in the type of surgical approach during the pandemic. Limitations in accessing special protective equipment against SARS-CoV2 were found by 62.5%. An impact on the follow-up of patients with gynaecological cancers due to pandemic was referred by a 70.4%. Conclusions: Most of Spanish gynecologist oncologist reported that COVD 19 pandemic has affected their clinical practice. The major measures implemented were an increase of telemedicine, a decrease the visits to high-risk or symptomatic patients and the use of SARS-CoV2 screening prior to surgery.

The present research work consists of the description of the work carried out within the framework of the UPMSat-2 project related to its attitude determination and control subsystem, ADCS. The methodologies used for the acceptance and integration of the ADCS sensors and actuators, considering that the means available for a didactic project such as UPMSat-2 are reduced. The UPMSat-2 ADCS is purely magnetic and consists of two magnetometers and three magnetorquers. This work includes methodologies to undertake the sensors and actuators acceptance tests and ADCS robustness tests of the ADCS facing magnetometer and magnetorquer errors. In addition a study to find the simplest and most robust method to calibrate the magnetometers once integrated in the satellite was conducted, considering the electrical noise that could be introduced during calibration tests and the constraints in movement of the satellite. The method that was found to have the best performance in dealing with these constraints is the one used to find the new calibration of the ADCS sensors. RESUMEN El presente trabajo de investigación consiste en la descripción del trabajo realizado en el marco del proyecto UPMSat-2 relacionado con su subsistema de determinación y control de actitud, ADCS. Se describen las metodologías utilizadas para la aceptación e integración de los sensores y actuadores ADCS, considerando que los medios disponibles para un proyecto didáctico como el UPMSat-2 son reducidos. El ADCS del UPMSat-2 es puramente magnético y consta de dos magnetómetros y tres magnetopares. Este trabajo incluye metodologías para llevar a cabo las pruebas de aceptación de sensores y actuadores y las pruebas de robustez del ADCS frente a errores introducidos en las medidas de los magnetómetros y de los magnetopares. Además, se incluye un estudio para encontrar el método más simple y robusto para calibrar los magnetómetros una vez integrados en el satélite, considerando el ruido eléctrico que podría introducirse durante las pruebas de calibración y las limitaciones en el movimiento del satélite. El método que muestra el mejor rendimiento frente a estas limitaciones es el que se usó para realizar la nueva calibración de los sensores del ADCS.

Litopenaeus vannamei, the Pacific whiteleg shrimp, is one of the most marketable species in aquaculture worldwide. However, it is susceptible to different infections causing considerable losses in production each year. Consequently, using prebiotics that promotes the proliferation of beneficial bacteria and strengthen the immune system is a current strategy for disease control. In this study, we isolated two strains of E. faecium from the gut of L. vannamei fed with agavin-supplemented diets. These isolates showed antibacterial activity against Vibrio parahaemolyticus, Vibrio harveyi and Vibrio alginolyticus , most likely due to peptidoglycan hydrolase (PGH) activity. Furthermore, we sequenced the genome of one isolate. As a result, we observed three proteins related to the production of bacteriocins, a relevant trait for selecting probiotic strains since they can inhibit the invasion of potential pathogens. Additionally, the genome annotation showed genes related to the production of essential nutrients for the host. It lacked two of the most common factors associated with virulence in Enterococcus pathogenic strains (esp and hyl). Thus, this host-probiotic-derived strain has potential application not only in shrimp health but also in alternative aquatic environments, as it is adapted to coexist within the gut shrimp microbiota, independently of the diet.

The SARS-CoV-2 infection has increased the number of patients entering Intensive Care Unit (ICU) facilities and antibiotic treatments. Concurrently, the multi-drug resistant bacteria (MDRB) colonization index has risen. Considering that most of these bacteria are derived from gut microbiota, the study of its composition is essential. Additionally, SARS-CoV-2 infection may promote gut dysbiosis, suggesting an effect on microbiota composition. This pilot study aims to determine bacteria biomarkers to predict MDRB colonization risk in SARS-CoV-2 patients in ICUs. Seventeen adult patients with an ICU stay >48 h and who tested positive for SARS-CoV-2 infection were enrolled in this study. Patients were assigned to two groups according to routine MDRB colonization surveillance: non-colonized and colonized. Stool samples were collected when entering ICUs, and microbiota composition was determined through Next Generation Sequencing techniques. Gut microbiota from colonized patients presented significantly lower bacterial diversity compared with non-colonized patients (p < 0.05). Microbiota in colonized subjects showed higher abundance of Anaerococcus, Dialister and Peptoniphilus, while higher levels of Enterococcus, Ochrobactrum and Staphylococcus were found in non-colonized ones. Moreover, LEfSe analysis suggests an initial detection of Dialister propionicifaciens as a biomarker of MDRB colonization risk. Cuál es la responsabilidad sociopolítica de los docentes hoy, en una escuela encuarentenada, desde la concientización-concienciación freiriana? ¿Cómo el aula mente-espíritu como espacio intersubjetivo de aprendizaje coadyuva a otra escuela hoy? En medio de categorías freirianas en la educación liberadora, en la línea de investigación “Paulo Freire: el andariego de la utopía en las transmetodologías”, se cumple con el objetivo complejo de sustentar la diada: concientización- concienciación freiriana en el aula mente-espíritu como escuela hoy. En la reconstrucción del transmétodo, la deconstrucción y concienciación-concientización en el aula mente-espíritu revitalizan la educación freiriana hoy como una necesidad que debemos atender fuera del aula física, como utopía de los pueblos que se niegan a vivir en contaminadas acciones colonizadoras, las cuales los desmitifican del vivir feliz y dignamente y que son develadas aún más hoy. La educación debe traspasar el aula física, ir a la complejidad del ser y atender las transformaciones de hoy.

La matemática en la Educación Inicial, el primer contacto de los niños y niñas con la ciencia en las instituciones educativas, ha estado supeditada en la mayoría de los casos a la comprensión de la serie numérica y las clasificaciones, entre otros procesos. Generalmente se impone el número como abstracción cuando el estudiante conoce sólo de concreción. Así, la colonialidad y disyunción del proceso educativo impide el desarrollo metacognitivo profundo, como valor intrínseco de la ciencia por excelencia. Desde una transmetodología compleja se analiza la matemática como el arte de pensar en la Educación Inicial y las implicaciones de la crisis de la Educación Matemática en la vida de los seres humanos. El recorrido rizomático permite revisar la crisis a la luz de categorías como condición humana y sentipensar. En la reconstrucción encontramos que la matemática no es sólo cantidad; sino cualidad, crítica, pensar profundo que no reduce el pensamiento sólo a un algoritmo; estos son sólo herramientas, no constituyen el centro y el hacer de la matemática en la Educación Inicial; en todo caso los discentes tienen sus propios conjuntos de pasos, a los que el docente debe pensarse como niño en ese mundo; pensar transversalmente desde los saberes generalizados del ser, los enfoques cualitativoscuantitativos-socio-críticos, y plurales, pues en las fronteras de los conocimientos consiguen esencias complejizadoras para saber cómo el niño puede llegar a sentir y pensar profundamente. El sentipensar como forma matemática de expresarse a la luz de la emotividad del estudiante debe provocarse emerger para interpelar nuestra propia praxis.

The electrocardiogram continues to be essential in the diagnosis of acute myocardial infarction, and a useful tool in arrhythmic risk stratification. We aimed to determine which electrocardiographic variables can successfully predict the occurrence of ventricular arrhythmias (VA) in patients following ST-segment elevation myocardial infarction (STEMI).We performed an observational study including 667 patients with STEMI admitted to the University Hospital in Sancti Spíritus, Cuba. Demographic variables, cardiovascular risk factors, and clinical variables were recorded. Electrocardiographic variables included QT interval duration (measured and corrected) and QT dispersion, QRS duration and dispersion, JT interval duration and ST-segment elevation magnitude. We also determined left ventricular ejection fraction and glomerular filtration rate. A binary statistical regression model and a regression tree were used to determine the variables that successfully predicted VA.VA occurred in 92 (13.8%) patients, within the first 48 hours in 68 (73.9%) and after this period in 24 (26.1%) patients. The variables associated with VA were QT interval duration529 ms and QT dispersion66 ms, QRS dispersion50 ms, and the presence of ST-segment elevation in six or more leads.The main predictor of VA occurring during the initial 48 hours was QT interval duration, while, after this period, it was QRS dispersion.

La pancreatitis crónica (PC) es una enfermedad de incidencia creciente en la población pediátrica, aunque desconocida, y conlleva una importante comorbilidad. La clínica inicial suele ser la propia de un episodio de pancreatitis aguda pero la importancia recae en el desarrollo, al cabo de meses o años, de insuficiencia pancreática. Para su diagnóstico será necesario tener un alto índice de sospecha clínica y realizar pruebas de imagen que demuestren cambios propios de la PC. El manejo de esta entidad deberá ser multidisciplinar y consistirá en controlar de forma adecuada el dolor e iniciar tratamiento sustitutivo cuando se constate la presencia de insuficiencia pancreática exocrina y endocrina. El caso que se reporta a continuación es un buen ejemplo de un caso de PC con presencia de insuficiencia exocrina y endocrina. Se documenta el proceso diagnóstico de la PC, en cuanto a pruebas de imagen y pruebas que demuestren la presencia de insuficiencia pancreática. Se exponen también las diferentes pruebas que se realizan para llegar a la etiología del cuadro. Se intenta reflejar el manejo que se debe llevar a cabo en estos pacientes y el seguimiento estricto que requieren.

Desde la deconstrucción rizomática como transmétodo se cumple con el objetivo complejo: analizar el desarrollo de las tecnologías del yo en la Educación Matemática Decolonial Transcompleja; está es una línea de investigación que va en conjunción con el dialogo en la matemática y su enseñanza en dos sentidos: el dialógico y el dialogo dialéctico. La dialéctica es insuficiente para asumir los retos de la interculturalidad; así en la educación la comunicación es posible y debe ocurrir en un nivel conjuncionado con la dialéctica, el dialogo dialógico. En la reconstrucción de la investigación como apertura, las tecnologías del yo: las matemáticas son virtuosas para el cuerpo, mente y espíritu humano y más ampliamente para la sociedad siempre y cuando consideremos la esencia de la matemática: complejidad y desarrollo no reduccionista; aporte como el poderoso lenguaje que manejan; sustentos en todos los saberes; diálogos dialogantes con la dialéctica en tanto discernir y refutar valores, las innovaciones; colaboración en todas las civilizaciones y avances de la humanidad.

Na presente investigação rizomática transmetodológica, com o transparadigma complexo analisamos o lema: somos natureza no planeta terra, a partir de visões decoloniais complexo-planetárias, como objetivo complexo de estudo cogitativo. A investigação é realizada com a hermenêutica compreensiva, ecosófica e diatópica nos momentos: analítico, empírico e proposicional; na linha de investigação: transepistemologias do conhecimento-conhecimento e transmetodologias transcomplexas. No momento propositivo, a religação faz sentido e com ela a reforma do pensamento do que é a vida no planeta-terra e com ela a devida consciência-consciência em favor da vida.

Aims The interplay between aortic stenosis (AS), cardiovascular events, and mortality is poorly understood. In addition, how echocardiographic indices compare for predicting outcomes remains unexplored for the full range of AS severity. Methods and results We prospectively calculated peak jet velocity (Vmax) and aortic valve area (AVA) in 5994 adult subjects with and without AS. We linked ultrasound data to 5-year mortality and clinical events obtained from electronic medical records. Proportional-hazard and negative binomial regression models were adjusted for relevant covariables such as age, sex, comorbidities, stroke-volume, LV ejection fraction, left valve regurgitation, aortic valve sclerosis or calcification, and valve replacement. We observed a strong linear relationship between Vmax and all-cause mortality (hazard ratio: 1.26, 95% confidence interval: 1.19–1.33 per 100 cm/s), cardiovascular events, as well as incidental and recurrent heart failure (HF). Adjusted risks were highly significant even at Vmax values in the range of 150–200 cm/s, risk curves separating very early after the index exam. Vmax was not associated with coronary, arrhythmic, cerebrovascular, or non-cardiovascular events. Although risks were confirmed when AVA was entered in place of Vmax, the risks estimated for categories based on the two indices were mismatched, even in patients with normal flow. An external cohort comprising 112 690 patients confirmed augmented risks of all-cause and cardiovascular mortality starting at values of Vmax and AVA in the range of mild AS. Conclusions Aortic stenosis is strongly associated to all-cause mortality, cardiovascular mortality, and cardiac events, specifically HF. Risks increase in parallel to the degree of outflow obstruction but are apparent very early in patients with mild disease. Criteria for grading AS based on Vmax and AVA are mismatched in terms of outcomes.

Background In living kidney transplantation there are two different individuals, a healthy donor and a renal transplant recipient. This is an excellent human model to study factors that influence kidney function in the context of reduced renal mass and the adaptation of two comparable kidneys to different metabolic demands. Methods We analyzed the changes in measured glomerular filtration rate (GFR, iohexol) from pretransplantation to 12 months after transplantation in 30 donor–recipient pairs. Each donor was compared with his/her recipient. We defined a priori three different groups based on GFR differences at 12 months: donor > recipient (Group A; 78 ± 8 versus 57 ± 8 mL/min), donor Results In Group A donors were mostly male and recipients were female (75% each). Donors had a higher baseline weight than their recipients. During follow-up, weight remained stable in donors but increased 7% in recipients. In Group B donors were mostly female (60%) and recipients male. At baseline, donors had a lower weight than recipients. At 12 months, weight was stable in donors but increased in recipients. In Group C donors were mostly (75%) female and recipients male. At baseline, donors had a higher BMI than their recipients. At 12 months, BMI was stable in donors but increased 14% in recipients. In multivariable analysis, higher GFR at 12 months was associated with higher baseline weight and GFR in donors and with male gender and higher baseline weight in recipients. Conclusions Kidneys from living donors are more ‘plastic’ than originally thought and respond to metabolic demands and weight changes of their new host. These changes should be taken into account when assessing GFR outcomes in this population.
