154. Divergent HIV-1 strains (CRF92_C2U and CRF93_cpx) co-circulating in the Democratic Republic of the Congo: Phylogenetic insights on the early evolutionary history of subtype C

Abstract

Molecular epidemiological studies revealed that the epicenter of the HIV pandemic was Kinshasa, the capital city of the Democratic Republic of the Congo (DRC) in Central Africa. All known subtypes and numerous complex recombinant strains co-circulate in the DRC. Moreover, high intra-subtype diversity has been also documented. During two previous surveys on HIV-1 antiretroviral drug resistance in the DRC, we identified two divergent subtype C lineages in the protease and partial reverse transcriptase gene regions. We sequenced eight near full-length genomes and classified them using bootscanning and likelihood-based phylogenetic analyses. Four strains are more closely related to subtype C although within the range of inter sub-subtype distances. However, these strains also have small unclassified fragments and thus were named CRF92_C2U. Another strain is a unique recombinant of CRF92_C2U with an additional small unclassified fragment and a small divergent subtype A fragment. The three remaining strains represent a complex mosaic named CRF93_cpx. CRF93_cpx have two fragments of divergent subtype C sequences, which are not conventional subtype C nor the above described C2, and multiple divergent subtype A-like fragments. We then inferred the time-scaled evolutionary history of subtype C following a Bayesian approach and a partitioned analysis using major genomic regions. CRF92_C2U and CRF93_cpx had the most recent common ancestor with conventional subtype C around 1932 and 1928, respectively. A Bayesian demographic reconstruction corroborated that the subtype C transition to a faster phase of exponential growth occurred during the 1950s. Our analysis showed considerable differences between the newly discovered early-divergent strains and the conventional subtype C and therefore suggested that this virus has been diverging in humans for several decades before the HIV/M diversity boom in the 1950s.

Abstract

Introduction: The reservoir(s) and ecology of Ebola viruses (EBV) remains largely unknown, but previous detection of viral RNA and anti-EBV antibodies in bats suggests that they may play a role in zoonotic transmission. Objectives: Gain insight into the circulation of EBV in bat populations in West and Central Africa by testing for the presence of antibodies against different EBV, using high throughput technology. Materials and methods: Bats were captured across 7 regions in Cameroon and 4 in Guinea, and released immediately after collection of dried blood spots and biological data. Here we used a multiplex immunoassay with Luminex® technology for antibody detection against NP, GP and VP40 antigens for Zaire (EBOV), Sudan (SUDV), Bundibugyo (BDBV) and Reston (RESTV) EBV. In the absence of positive controls, cut-off values were determined using the change-point analysis method with bootstrapping (10 000 times). A sample was considered positive if the detected antibodies level was over the estimated cut-off for both NP and GP antigens. Results: We studied 1796 bats (Cameroon, n=1365 and Guinea, n=431) belonging to 10 genera of the frugivorous family Pteropodidae (n=641) and 12 genera of 6 insectivorous families (n=1155). Based on the change-point analysis, 0,2% (3/1796) of bats were positive for EBOV (E. helvum, n=1; M. angolensis, n=1 and Mops sp., n=1) and 0,1% (1/1796) for SUDV (R. aegyptiacus). A total of 7,9% (142/1796) reacted to at least one EBV antigen, mainly GP. These bats belonged mainly (97%) to 8 frugivorous species and one insectivorous genus (Mops). Conclusion: we confirm the presence of antibodies in 2 frugivorous bat species and 1 insectivorous genus previously found to be seropositive, as well as for the first time in M. angolensis, a frugivorous species. Using a stringent method of interpretation (change-point analysis), prevalence of EBV antibodies can be underestimated. More studies are needed to evaluate the extend of EBV in bats in areas at risk for EBV outbreaks in Africa and complementary less conservative methods to define cut-offs could be used for comparison in order to reflect natural circulation or exposure to Filoviruses.

Abstract

Background: Two pemphigus severity scores, Autoimmune Bullous Skin Disorder Intensity Score (ABSIS) and Pemphigus Disease Area Index (PDAI), have been proposed to provide an objective measure of disease activity. However, the use of these scores in clinical practice is limited by the absence of cut-off values that allow differentiation between moderate, significant and extensive types of pemphigus. Objectives: To calculate cut-off values defining moderate, significant and extensive pemphigus based on the ABSIS and PDAI scores. Methods: In 31 dermatology departments in six countries, consecutive patients with newly diagnosed pemphigus were assessed for pemphigus severity, using ABSIS, PDAI, Physician's Global Assessment (PGA) and Dermatology Life Quality Index (DLQI) scores. Cut-off values defining moderate, significant and extensive subgroups were calculated based on the 25th and 75th percentiles of the ABSIS and PDAI scores. The median ABSIS, PDAI, PGA and DLQI scores of the three severity subgroups were compared in order to validate these subgroups. Results: Ninety-six patients with pemphigus vulgaris (n = 77) or pemphigus foliaceus (n = 19) were included. The median PDAI activity and ABSIS total scores were 27·5 (range 3-84) and 34·8 points (range 0·5-90·5), respectively. The respective cut-off values corresponding to the first and third quartiles of the scores were 15 and 45 for the PDAI, and 17 and 53 for ABSIS. The moderate, significant and extensive subgroups were thus defined, and had distinguishing median ABSIS (P < 0·001), PDAI (P < 0·001), PGA (P < 0·001) and DLQI (P = 0·03) scores. Conclusions: This study suggests cut-off values of 15 and 45 for PDAI and 17 and 53 for ABSIS, to distinguish moderate, significant and extensive pemphigus forms. Identifying these pemphigus activity subgroups should help physicians to classify and manage patients with pemphigus.

Abstract

Background: While dolutegravir has been added by WHO as a preferred second-line option for the treatment of HIV infection, boosted protease inhibitor (bPI)-based regimens are still needed as alternative second-line options. Identifying optimal bPI-based second-line combinations is essential, given associated high costs and funding constraints in low- and middle-income countries. We assessed the cost-effectiveness of three alternative bPI-based second-line regimens in Burkina Faso, Cameroon and Senegal. Methods: We used data collected over 2010–2015 in the 2LADY trial/post-trial cohort. Patients with first-line antiretroviral therapy (ART) failure were randomly assigned to tenofovir/emtricitabine + lopinavir/ritonavir (TDF/FTC LPV/r; arm A), abacavir + didanosine + lopinavir/ritonavir (arm B), or tenofovir/emtricitabine + darunavir/ritonavir (arm C). Costs (US dollars, 2016), quality-adjusted life-years (QALYs) and incremental cost-effectiveness ratios were computed for each country over 24 months of follow-up and extrapolated to 5 years using a simulated patient-level Markov model. We assessed uncertainty using cost-effectiveness acceptability curves, scenarios and prices threshold analysis. Results: In each country, over 24 months, arm A was significantly less costly than arms B and C (incremental costs ranging from US$410–$US721 and US$468–US$546 for B and C vs A, respectively) and offered similar health benefits (incremental QALY: − 0.138 to 0.023 and − 0.179 to 0.028, respectively). Over 5 years, arm A remained the least costly, health benefits not being significantly different between arms. Compared with arms B and C, in each study country, Arm A had a ≥ 95% probability of being cost-effective for a large range of cost-effectiveness thresholds, irrespective of the scenario considered. Conclusions: Using TDF/FTC LPV/r as a bPI-based second-line regimen provided the best economic value in the three study countries. Trial Registration: ClinicalTrials.gov Identifier: NCT00928187.

Abstract

Objectives: In terms of HIV infection, western and central Africa is the second most affected region world‐wide, and the gap between the regional figures for the testing and treatment cascade and the Joint United Nations Programme on HIV/AIDS (UNAIDS) 90–90–90 targets is particularly worrying. We assessed the prevalence of virological suppression in patients routinely treated in 19 hospitals in Cameroon. Methods: A cross‐sectional survey was performed in adult patients receiving antiretroviral therapy (ART) in the Centre and Littoral regions. The prevalences of virological suppression (<1000 HIV‐1 RNA copies/mL) were compared among all 19 hospitals using the χ2 test. Potential individual and health care‐related determinants of virological suppression were assessed using multivariate logistic regression models. Results: A total of 1700 patients (74% women; median age 41 years; median time on ART 3.7 years) were included in the study. The prevalence of virological suppression was 82.4% overall (95% confidence interval 80.5–84.2%). It ranged from 57.1 to 97.4% according to the individual hospital (P < 0.001). After adjustment, virological suppression was associated with age, CD4 cell count at ART initiation, disclosure of HIV status to family members, interruption of ART for more than two consecutive days, and location of patient's residence and hospital (rural/urban). These factors did not explain the heterogeneity of virological suppression between the study hospitals (P < 0.001). Conclusions: The overall prevalence of virological suppression was reassuring. Nevertheless, the heterogeneity of virological suppression among hospitals highlights that, in addition to programme‐level data, health facility‐level data are crucial in order to tailor the national AIDS programme's interventions with a view to achieving the third UNAIDS 90 target. Abstract

Retroviruses include the human immunodeficiency virus (HIV), which causes AIDS, and the human T-cell lymphotropic virus type I (HTLV-I), which may cause leukemia and other diseases. Infection with these viruses is increasing in populations throughout the world, particularly the poorer countries in Africa. Cultural practices in this area of the world can inhibit defining populations at high risk for these infections. Infertility rates are also very high in this part of the world, mainly due to sexually transmitted diseases. This study examined retroviral infection and fertility status in women in Gabon (Western Equatorial Africa) to see if a relation existed between infection and infertility, and if a high risk group could be identified. A random sample of women from the towns and villages of Gabon who were of reproductive age were tested for HIV and HTLV-I infections; their fertility status was also estimated. Results found that 9 of the 654 women (1.4 percent) were HIV-positive and 44 (6.8 percent) were HTLV-I positive. (That is, they had been infected with HIV and HTLV-I.) HIV infection rates did not increase with age, but they did for HTLV-I. HIV infection rates were much higher in women who had primary infertility (9.3 percent) than in those with secondary infertility (2.3 percent) or those who were fertile (0.7 percent). (Primary infertility means that a woman has never been able to conceive, whereas secondary infertility means that a woman who has been pregnant is not able to conceive again.) HTLV-I infection rates did not differ with fertility status. Only two women tested positive for both infections. These results indicate that women in Gabon who have primary infertility are a high risk group for HIV infection, but not HTLV-I infection.
