Abstract

In 2009, the National Academy of Sciences (NAS) called for more interdisciplinary and communityengaged approaches to teaching and learning in the agricultural and life sciences to better respond to the food system challenges of the 21st century. As a result, institutions from across the nation have responded with a number of experiential learning and service-learning frameworks and practices aimed to enhance the academic experience for both student and community stakeholders. Sustainable agriculture education, with its explicit focus on experiential learning, interdisciplinarity, and values-based programming, has emerged as a promising approach to strengthen the fabric of agriculture and life sciences education. The purpose of this paper is to illustrate the complex role of service learning as a central approach to undergraduate teaching and learning where interdisciplinary teaching, experiential learning, and community engagement are core goals. Specifically, we conducted a single embedded case study of a sustainable agriculture education program at a land grant university to explore how this triad was organized and possible service learning outcomes. Our case study was informed by semi-structured interviews of faculty and community partner stakeholders, participant observations of faculty and students, and secondary data analysis of course syllabi and other programmatic artifacts. Despite different understandings and practices of service learning by faculty within this, we found a common core of best practices. 159. Neighbourhood deprivation and small-for-gestational-age term births in the United States

Abstract

Residential context has received increased attention as a possible contributing factor to race/ethnic and socioeconomic disparities in birth outcomes in the United States. Utilizing vital statistics birth record data, this study examined the association between neighbourhood deprivation and the risk of a term small-for-gestational-age (SGA) birth among non-Hispanic whites and non-Hispanic blacks in eight geographic areas: Baltimore City, Baltimore County, Montgomery County and Prince Georges County in Maryland, 16 pooled cities in Michigan, Durham County and Wake County in North Carolina, and Philadelphia, Pennsylvania. Multilevel random intercept logistic regression models were employed and statistical tests were performed to examine if the association between neighbourhood deprivation and SGA varied by race/ethnicity and study site. The risk of term SGA was higher among non-Hispanic blacks (range: 10.8%–17.5%) than non-Hispanic whites (range: 5.1%–9.2%) in all areas and it was higher in cities than in suburban locations. In all areas, non-Hispanic blacks lived in more deprived neighbourhoods than non-Hispanic whites. However, the adjusted associations between neighbourhood deprivation and term SGA did not vary significantly by race/ethnicity or study site. The summary fully-adjusted pooled odds ratios, indicating the effect of one standard deviation increase in the deprivation score, were 1.15 [95% CI: 1.08–1.22] for non-Hispanic whites and 1.09 [95% CI: 1.05–1.14] for non-Hispanic blacks. Thus, neighbourhood deprivation was weakly associated with term SGA among both non-Hispanic whites and non-Hispanic blacks.

Abstract

Census data are widely used for assessing neighborhood socioeconomic context. Research using census data has been inconsistent in variable choice and usually limited to single geographic areas. This paper seeks to a) outline a process for developing a neighborhood deprivation index using principal components analysis and b) demonstrate an example of its utility for identifying contextual variables that are associated with perinatal health outcomes across diverse geographic areas. Year 2000 U.S. Census and vital records birth data (1998-2001) were merged at the census tract level for 19 cities (located in three states) and five suburban counties (located in three states), which were used to create eight study areas within four states. Census variables representing five socio-demographic domains previously associated with health outcomes, including income/poverty, education, employment, housing, and occupation, were empirically summarized using principal components analysis. The resulting first principal component, hereafter referred to as neighborhood deprivation, accounted for 51 to 73% of the total variability across eight study areas. Component loadings were consistent both within and across study areas (0.2-0.4), suggesting that each variable contributes approximately equally to "deprivation" across diverse geographies. The deprivation index was associated with the unadjusted prevalence of preterm birth and low birth weight for white non-Hispanic and to a lesser extent for black non-Hispanic women across the eight sites. The high correlations between census variables, the inherent multidimensionality of constructs like neighborhood deprivation, and the observed associations with birth outcomes suggest the utility of using a deprivation, index for research into neighborhood effects on adverse birth outcomes.

Abstract

Early pregnancy prediction of third trimester glucose intolerance may identify a population of women whose trajectory toward gestational diabetes mellitus (GDM) is modifiable. We assessed whether first trimester glycated hemoglobin (HbA1c) and sex hormone-binding globulin (SHBG), markers of insulin resistance, predicted third trimester glucose intolerance. Nondiabetic women with singleton pregnancies enrolled in a prospective observational study, 11 0/7 to 14 6/7 weeks. At enrollment, maternal characteristics, medical history, and blood samples were collected for HbA1c and SHBG. Two-step GDM screening was performed, 22 0/7 to 33 6/7 weeks. A 50 g oral glucose tolerance test ≥130 mg/dL defined screen positive, or glucose intolerance. Carpenter-Coustan criteria diagnosed GDM. Means HbA1c and SHBG were compared between glucose-intolerant versus normoglycemic women, and GDM versus no GDM women. We report unadjusted and adjusted odds ratios (OR; 95% confidence interval [CI]) of regression analyses. Adjusted models include race, enrollment body mass index, and history of GDM. Among 250 women, 29% were glucose intolerant and 6% had GDM. Among glucose-intolerant women, HbA1c was higher (5.3±0.3 vs. 5.1 ± 0.3, P = .01) and associated with glucose intolerance in unadjusted, but not adjusted, models (OR: 2.9, 95% CI: 1.2-7.1; adjusted odds ratio [aOR]: 2.0, 95% CI: 0.7-5.4). Among GDM women, HbA1c was higher (5.4±0.4 vs 5.2±0.3, P = .002) and SHBG was lower (228±72 vs 288±93 mmol/L, P = .02). The HbA1c predicted GDM in unadjusted (OR: 13.2, 95% CI: 2.6-68.0) but not adjusted (aOR: 6.7, 95% CI: 0.8-55.2) models. Although metabolic alterations may well precede third trimester glucose intolerance, neither HbA1c of SHBG remained an independent predictor of glucose intolerance or GDM in adjusted models. Abstract

The inclusion of service-learning in higher education is an opportunity to enhance experiential curricula to increase student success post-graduation in the current dynamic and challenging job market. However, designing effective assessments to measure learning that takes place outside the classroom may be a difficult aspect for educators interested in including community- based learning opportunities in their curricula. A 2014 Community Nutrition course incorporating a service-learning assignment is used as an example of Assessment for Learning to achieve predetermined learning objectives. The purpose of this paper is to illustrate how educators can implement structured, multi-faceted assessment within service-learning using intentionally designed assignments that include multiple points of feedback to students, providing opportunities for reflection and learning. 175. Parent and Adolescent Interest in Receiving Adolescent Health Communication Information From Primary Care Clinicians.

Abstract

Purpose Patient-centered health care recognizes that adolescents and parents are stakeholders in adolescent health. We investigate adolescent and parent interest in receiving information about health topics and parent-teen communication from clinicians. Methods Ninety-one parent-adolescent dyads in one practice completed individual interviews. Items assessed levels of interest in receiving health and health communication information from the adolescent's doctor about 18 topics, including routine, mental health, sexual health, substance use, and injury prevention issues. Analyses tested differences between parents and adolescents, within-dyad correlations, and associations with adolescent gender and age. Results Most parents were female (84%). Adolescents were evenly divided by gender; 36 were aged 12–13 years, 35 were aged 14–15 years, and 20 were aged 16–17 years. Adolescent race reflected the practice population (60% black; 35% white). The vast majority of parents and adolescents reported moderate or high levels of interest in receiving information about all 18 health issues and information to increase parent-teen communication about these topics. Parents' interest in receiving information varied by adolescent age when the expected salience of topics varied by age (e.g., acne, driving safety), whereas adolescents reported similar interest regardless of age. Adolescent gender influenced parent and adolescent interest. Level of interest in receiving information from doctors within adolescent-parent pairs was not significantly correlated for one-half of topics. Conclusions Parents and adolescents want health care professionals to help them learn and talk about a wide range of adolescent health topics. Feasible primary care interventions that effectively improve parent-teen health communication, and specific adolescent health outcomes are needed. [ABSTRACT FROM AUTHOR]

Abstract

The National Academy of Sciences called for a dynamic approach to teaching and learning in colleges of agriculture. In response, faculty at colleges and universities are implementing innovative frameworks for undergraduate education in the agricultural sciences. This study explored the collaborative and interdisciplinary teaching and learning practices of faculty in sustainable agriculture education curricula at a land grant university as an illustration of this innovation. Drawing upon a sociocultural learning framework, this study specifically emphasizes faculty work as a social practice and the inherently relational learning that occurs with other faculty, their students and community partners. Using an in-depth, qualitative research approach, a single embedded case study design was implemented to illustrate the teaching and learning experiences of an interdisciplinary group of faculty collaborating within an undergraduate minor that fosters community engagement through service-learning and sustainable agriculture curricula. The collaborative teaching structure that is explored is comprised of an instructor of record, collaborating faculty, community-partner and graduate teaching assistant. Faculty teaching in this program of study experience learning in the areas of disciplinary knowledge and pedagogical practice and navigate organizational challenges and barriers to collaborative work.
