In two phase 3 trials for first-line therapy in adults with HIV-1, doravirine showed non-inferior efficacy, a favourable safety profile, and a superior lipid profile to darunavir and efavirenz through to 48 and 96 weeks. Here we report 192-week results from both studies.

Abstract

Background Physical activity (PA) has been associated with benefits for cognitive function (CF), but previous estimates of the strength of this relationship may have been biased due to limitations in statistical modelling practices that are common among observational studies. We aimed to address this by using a rigorously constructed conceptual causal model to guide an empirical analysis estimating the effect of PA on CF in the UK Biobank cohort of middle-aged and older adults. Methods This study analysed a subsample of 334 227 adults from the UK Biobank prospective cohort study. PA was measured subjectively by self-report and by device using accelerometry, and CF was measured using objective cognitive tests. Composite CF measures were derived to represent general and domain-specific performance. Effect coefficients were estimated using regression models, adjusting for a wide range of confounders specified by the assumed causal model, including genetic risk factors, and relevant health, sociodemographic and behavioural variables from across the lifespan. Results Results indicated very small effect sizes (standardized mean difference estimates all <0.01) of inconsistent direction, for both cross-sectional and longitudinal analyses. Conclusions The expected protective effect of PA on CF was not observed. This may reflect selection bias within UK Biobank, or the relatively young age of the sample at follow-up. 161. A Network Analysis of the PART-O at 1 and 2 Years After TBI: A Veterans Affairs Model Systems Study.

Abstract

Objective: The construct of participation after traumatic brain injury (TBI) can be difficult to operationalize. Psychometric network analysis offers an empirical approach to visualizing and quantifying the associations between activities that comprise participation, elucidating the relations among the construct's components without assuming the presence of a latent common cause and generating a model to inform future measurement methods. The current research applied psychometric network analysis to the Participation Assessment with Recombined Tools-Objective (PART-O) within a sample of service members and veterans (SM/Vs) with a history of TBI at 1 and 2 years (T1 and T2) postinjury. Participants: Participants (N = 663) were SM/Vs with a history of TBI who completed comprehensive inpatient rehabilitation services at a Department of Veterans Affairs (VA) Polytrauma Rehabilitation Center (PRC). Setting: Five VA PRCs. Design: Cross-sectional, retrospective analysis of data from the VA TBI Model Systems study. Main Measures: PART-O. Results: Network analysis demonstrated that the PART-O structure was generally consistent over time, but some differences emerged. The greatest difference observed was the association between "spending time with friends" and "giving emotional support" to others. This association was more than twice as strong at T2 as at T1. The "out of the house" item was most central, as demonstrated by dense connections within its own subscale (Out and About) and items in other subscales (ie, Social Relations and Productivity).When examining items connecting the 3 subscales, the items related to giving emotional support, internet use, and getting out of the house emerged as the strongest connectors at T1, and the internet was the strongest connector at T2. Conclusion: Providing emotional support to others is associated with greater participation across multiple domains and is an important indicator of recovery. Being out and about, internet use, and engagement in productive activities such as school and work shared strong associations with Social Relations. Network analysis permits visual conceptualization of the dynamic constructs that comprise participation and has the potential to inform approaches to measurement and treatment. 166. Salivary MicroRNA as a Concussion Biomarker and the Implications for Athletic Trainers' Practices

Abstract

Many concussions are sustained globally each year; however, research suggests that a large number may go undiagnosed due to a reliance on subjective information from patients and limitations of current objective measures. Recently, efforts have been focused on identifying clinical biomarkers of concussions, including salivary microRNA (miRNA), to improve healthcare professionals’ concussion management practices. Even if salivary miRNA were shown to be a valid and reliable measure for managing concussions, healthcare professionals, such as athletic trainers (ATs), must be familiar with the tool and have positive attitudes toward the implementation into clinical practice. Therefore, the purpose of this dissertation was to better understand salivary miRNA as a concussion biomarker through three studies. In our first study, we performed a scoping review to identify specific salivary miRNA that have demonstrate potential in acting as a diagnostic or prognostic indicator of concussions. Overall, we found forty-nine salivary miRNA across nine studies throughout the literature. Of those forty-nine, thirty-four show potential for being a concussion diagnostic marker and twenty-one correlated to multiple concussions or symptom trends. Of those identified, five were identified in multiple studies. As previously stated, even if identified as a valid and reliable tool for concussion management, healthcare professionals such as ATs should demonstrate familiarity with these tools and show positive attitudes toward the implementation into clinical practice. Therefore, the second study of this dissertation aimed to evaluate ATs familiarity with biomarkers of concussion and their attitudes toward the future implementation into clinical practice. Overall, the results of this study indicated that ATs self-reported a lack of familiarity with current literature surrounding the use of biomarkers for concussion management but a large majority self-reported positive attitudes towards the future implementation of such a tool into clinical practice. Lastly, to address a limitation for the application of salivary miRNA as a concussion biomarker, we aimed to evaluate the effects of one NCAA Division I football season. In this study, we found that although simple inferential statistics revealed no significant differences between pre- and post-season expression of salivary miRNA, further analysis through intraclass correlation coefficient statistics revealed a lack of reliability in each of the six target salivary miRNA. Although these target miRNA did not demonstrate reliability from pre- to post-season, sub-analyses revealed a ratio of two miRNA that showed high sensitivity when identifying concussed individuals. Overall, research should continue to evaluate the potential for salivary miRNA to act as a concussion biomarker by evaluating other target miRNA.

Abstract

The article discusses the case of a 60-year-old male patient who received the MMR vaccine less than 4 weeks before an emergent heart transplant. Despite concerns about vaccine-associated disease, successful postexposure prophylaxis against measles was administered. The patient's posttransplant course was uncomplicated, highlighting the challenges of vaccination prior to transplantation and the need for further research on postexposure prophylaxis in vulnerable patient populations. 189. Consonant Accuracy after Severe Pediatric Traumatic Brain Injury: A Prospective Cohort Study

Abstract

Purpose: The authors sought to describe longitudinal changes in Percentage of Consonants Correct--Revised (PCC-R) after severe pediatric traumatic brain injury (TBI), to compare the odds of normal-range PCC-R in children injured at older and younger ages, and to correlate predictor variables and PCC-R outcomes. Method: In 56 children injured between age 1 month and 11 years, PCC-R was calculated over 12 monthly sessions beginning when the child produced greater than or equal to 10 words. At each session, the authors compared odds of normal-range PCC-R in children injured at younger (less than or equal to 60 months) and older (greater than 60 months) ages. Correlations were calculated between final PCC-R and age at injury, injury mechanism, gender, maternal education, residence, treatment, Glasgow Coma Score, and intact brain volume. Results: PCC-Rs varied within and between children. Odds of normal-range PCC-R were significantly higher for the older than for the younger group at all sessions but the first; odds of normal-range PCC-R were 9 to 33 times higher in the older group in sessions 3 to 12. Age at injury was significantly correlated with final PCC-R. Conclusion: Over a 12-month period, severe TBI had more adverse effects for children whose ages placed them in the most intensive phase of PCC-R development than for children injured later. 190. Teacher Supervision and Evaluation: A Case Study of Administrators' and Teachers' Perceptions of Mini Observations

Abstract

This case study will investigate teachers' and administrators' perceptions of the relationship between mini observations and teacher performance to understand what effect, if any, a system of mini observations has on teacher performance, and if mini observations influences a teacher's pedagogical practice differently than a traditional evaluation model. Evaluating public school teachers has been studied for over one hundred years, yet little evidence exists as to the efficacy of mini observations on teacher performance. This study, including ten administrators and thirty seven teachers, will provide a small sample of data to help public schools evaluate and potentially implement this new method. Four questions will guide this research: (1) What is the experience of teachers and administrators who have been exposed to mini observations? (2) How does the mini observation model impact teacher's instructional practices as perceived by both teachers and administrators? (3) What are teachers' perceptions about the reflective feedback and its ability to lead them to modify their practice? (4) Do teachers and administrators perceive any change in their relationship as a result of participating in mini observations? This qualitative study will gather data from six to ten participants from across one elementary school and one middle school through semi-structured interviews, observations of the teachers' and administrators' reflective conversations, and responses to a written survey. 194. FROSTED BRANCH ANGIITIS ASSOCIATED WITH MIXED CONNECTIVE TISSUE DISEASE.

Abstract

To report on a rare case of frosted branch angiitis in a patient presenting with subacute vision loss in the setting of a flare of her mixed connective tissue disease. The patient responded well to immunosuppressive therapy and had full visual recovery at 1-month follow-up. Purpose: To present a case of frosted branch angiitis associated with an exacerbation of mixed connective tissue disease (MCTD). Methods: Single case report. Results: A 31-year-old woman presented with a flare of her long-standing MCTD after a change in her immunosuppressive medications. She developed blurred vision and floaters first in the left eye but eventually in both eyes. Fundoscopy showed patchy perivascular sheathing of tertiary branch venules surrounded by retinal hemorrhages characterized as frosted branch angiitis. The patient's MCTD symptoms and retinal vasculitis improved with continued immunosuppressive therapy. At 1-month follow-up, her visual acuity had improved to 20/20 bilaterally with complete resolution on fundoscopy. Conclusion: To the authors' knowledge, this is the first report of frosted branch angiitis seen in association with MCTD. The improvement in the patient's visual acuity and fundoscopic findings in this case supports the role of immunosuppressive therapy to treat secondary frosted branch angiitis associated with an autoimmune condition such as MCTD. However, it is recommended that a comprehensive medical workup is performed to exclude an infective cause, particularly in immunocompromised patients. [ABSTRACT FROM AUTHOR]
