Background: The authors report the results of a prospective, multicenter, multidisciplinary study of central venous catheters (CVCs) in pediatric oncology patients analyzing factors involved in early failure., Methods: Information was collected from parent-held records on the fate of 824 devices inserted over a 20-month period, 415 of which were no longer in situ., Results: Within the first 7 weeks after insertion, there were 66 failures, all occurring in external lines. Accidental dislodgement was the principal reason for CVC failure (44 of 66, 67%). Detailed analysis of the reason for failure of this large subgroup showed 11 factors individually associated with early dislodgement, of which, 4 were independently associated with failure by multivariate analysis. These 4 variables were the use of multilumen catheters, the absence of a skin exit site suture, platelet transfusion at the time of insertion, and patient age less than 2 years., Conclusions: This study confirms the multiple influences on successful CVC usage. Our analysis supports the principle of only using multilumen lines when clinically essential. The findings also support the inception of randomized studies of fixation, particularly in infants.

The Malone antegrade enema (MACE) and the caecostomy button (CB) are two methods of achieving colonic lavage in constipated children with faecal soiling. We reviewed our experience with the MACE and CB, aiming to compare results, complications, and outcomes. Between June 1998 and August 2002, 37 children (15 boys) underwent MACE and 12 children (9 boys) underwent CB for idiopathic constipation that had failed conventional treatment. Rectal biopsy was ganglionic in all cases. Mean age at surgery was 9.9 years for the MACE patients and 9.8 years for the CB patients. All children are under continuous review, and mean follow-up is 18 months. Statistical analysis of proportions used Fisher's exact test. Soiling stopped completely in 30 children with MACE and in 9 with CB. Occasional soiling is still present in two children with a CB and in one with MACE. One child with a CB had resumed regular bowel activity, and the CB was removed. MACE failed in 5 (14%) patients because of ineffective colonic lavage, and in one patient (3%) the appendix was replaced by a CB because of perforation of the appendicostomy. CB failed in one patient (8%) because of faecal leak around the button; the child was subsequently converted to MACE (P = >0.5). Complications requiring operative intervention were seen in 9 (24%) of the 37 patients who underwent MACE and none of the 12 patients who underwent CB (P = 0.09). The main complication requiring surgical intervention was stoma stenosis (11%). Complications not requiring operative intervention were seen in 7 (19%) patients after MACE and 11 (92%) of the 12 patients who underwent CB (P < 0.001). The MACE and CB procedures are reliable and effective with high success rates. The MACE has a higher incidence of complications requiring operative intervention. Conversely, complications not requiring operative intervention are more frequent with CB. CB is a safe and effective alternative to MACE in children with faecal soiling.

Monocular patching has been advocated as a treatment for spatial neglect. Eye patching, however, is also reported to increase spatial bias. Thus, patching brain injury patients for ocular conditions may also impair their attention. To learn if an eye patch may induce asymmetric attention in a patient without spatial neglect, we tested a woman with diplopia after a left thalamic-intraventricular hemorrhage. She had no spatial neglect on bedside attention tasks. Using a laser pointer, she bisected a blank space at near (279 mm) and far (914 mm) distances under 3 conditions: unpatched and wearing a right or left eye patch. Unpatched, she showed no bias. She erred further leftward with right patching at both distances. Right eye patching appeared to induce neglect of right space when the patient performed this task. With left patching, leftward error tended to increase in near space only. Clinicians prescribing eye patches to patients with brain injury even for purely ocular indications, should evaluate them for spatial neglect once the patch is in place.

Deficits in visual-spatial ability can be associated with Parkinson's disease (PD), and there are several possible reasons for these deficits. Dysfunction in frontal-striatal and/or frontal-parietal systems, associated with dopamine deficiency, might disrupt cognitive processes either supporting (e.g., working memory) or subserving visual-spatial computations. The goal of this study was to assess visual-spatial orientation ability in individuals with PD using the Mental Rotations Test (MRT), along with other measures of cognitive function. Non-demented men with PD were significantly less accurate on this test than matched control men. In contrast, women with PD performed similarly to matched control women, but both groups of women did not perform much better than chance. Further, mental rotation accuracy in men correlated with their executive skills involving mental processing and psychomotor speed. In women with PD, however, mental rotation accuracy correlated negatively with verbal memory, indicating that higher mental rotation performance was associated with lower ability in verbal memory. These results indicate that PD is associated with visual-spatial orientation deficits in men. Women with PD and control women both performed poorly on the MRT, possibly reflecting a floor effect. Although men and women with PD appear to engage different cognitive processes in this task, the reason for the sex difference remains to be elucidated.

Inflammatory pseudotumor is an uncommon benign tumor found in various sites throughout the body. The etiology is unclear with evidence to support both neoplastic and infective processes. The authors present a case with several unique features. It represents the first reported case detected antenatally. The tumor was found separately within the abdomen and thorax. The thoracic component of the tumor was managed without resection or ablative treatment but showed spontaneous regression.

Objective: To learn whether young and aged subjects exhibit different recall biases for internally derived (Internal) versus externally supplied (External) material., Background: Internally derived knowledge, prized by educators and therapists, can bring about dramatic behavioral change. Such information, seldom assessed on formal memory testing, may be preferentially recalled compared with external-origin material. Under some circumstances, however, subjects may demonstrate a recall advantage for externally supplied over internally generated material., Method: We compared Internal and External word recall in young and aged subjects with and without explicit intent to remember., Results: Although overall the young and aged subjects recalled the same number of words, we did find a word-origin recall bias. This recall bias differed by age group (P = 0.005). When not instructed to remember words, the young subjects tended to remember more External words, while aged subjects remembered more Internal words., Conclusions: The differences in the brain mechanisms mediating Internal versus External recall bias are unknown. However, aging may modify an Internal-External memory bias.

Hemianopic patients may make line bisection errors in the direction of contralesional space, which could represent functional compensation or might be due to ipsilateral neglect. The authors report a patient with stroke with left homonymous hemianopsia and contralesional (leftward) bias on the line bisection task while upright, lying on his left side, and sitting with head bent 90 degrees leftward. Because hemianopic compensation should map retinotopically and the patient consistently erred body-leftward, the authors conclude that his contralesional errors are consistent with ipsilateral neglect.

Normal subjects usually err distally when bisecting radial lines below eye level in midsagittal space (far bias). To learn if a radial line bisection bias may be characterized as primarily perceptual-attentional or primarily motor-intentional, we had subjects bisect lines while watching their hand and the line under two video monitoring conditions. In the DIRECT condition, proximal (near) and distal (far) on the video monitor corresponded with the workspace where subjects bisected lines. In the INDIRECT condition, we reversed proximal and distal as they appeared on the monitor. Thus, in the indirect condition, distal hand movement appeared proximal, and vice versa. In the DIRECT condition, subjects erred toward far space (mean 2.075 mm). In the INDIRECT condition, however, subjects erred proximally (mean 1.59 mm, near bias). Distal-proximal reversal of errors over the group of subjects is consistent with a primarily perceptual-attentional bias. In lower body space, normal visual-perceptual-attentional systems may be more biased toward far space than are kinesthetic-motor systems.

Background: It remains unclear from lesion studies whether the four signs of the Gerstmann syndrome (finger agnosia, acalculia, agraphia, and right-left confusion) cluster because the neuronal nets that mediate these activities have anatomical proximity, or because these four functions share a common network. If there is a common network, with degeneration, as may occur in Alzheimer's disease, each of the signs associated with Gerstmann's syndrome should correlate with the other three signs more closely than they correlate with other cognitive deficits., Methods: Thirty eight patients with probable Alzheimer's disease were included in a retrospective analysis of neuropsychological functions., Results: The four Gerstmann's syndrome signs did not cluster together. Finger naming and calculations were not significantly correlated. Right-left knowledge and calculations also did not correlate., Conclusions: The four cognitive functions impaired in Gerstmann's syndrome do not share a common neuronal network, and their co-occurrence with dominant parietal lobe injuries may be related to the anatomical proximity of the different networks mediating these functions.

The facial feedback hypothesis suggests that facial expressions are either necessary or sufficient to produce emotional experience. Researchers have noted that the ideal test of the necessity aspect of this hypothesis would be an evaluation of emotional experience in a patient suffering from a bilateral facial paralysis; however, this condition is rare and no such report has been documented. We examined the role of facial expressions in the determination of emotion by studying a patient (F.P.) suffering from a bilateral facial paralysis. Despite her inability to convey emotions through facial expressions, F.P. reported normal emotional experience. When F.P. viewed emotionally evocative slides her reactions were not dampened relative to the normative sample. F.P. retained her ability to detect, discriminate, and image emotional expressions. These findings are not consistent with theories stating that feedback from an active face is necessary to experience emotion, or to process emotional facial expressions.

Spatial bias may occur in subjects performing a number of cognitive and visual-motor tasks. These include coordinate visuospatial computations (e.g. bisecting a line) and categorical representations of syntactic information (e.g. drawing a picture depicting the action of a sentence). Readers of European languages scan from left-to-right and this learned visual scanning may contribute to leftward spatial bias. In 30 subjects who first learned to read in a top-to-bottom, right-to-left direction (right-left vertical readers, RL), we tested spatial-syntactic bias by reading sentences to subjects, who drew pictures depicting the actions. We noted whether the subject of the sentence was located leftward or rightward of the object. We assessed visual-spatial bias by measuring subjects' accuracy at bisecting lines, and by measuring how closely their drawings on the house-tree-person test were centered on the page. On the spatial-syntactic task, the RL were not right- or left-biased (P=0.581). Korean controls (left-right horizontal readers, LR) also showed no significant spatial-syntactic bias. RL only tended to bisect lines leftward, but displaced house-tree-person drawings left of page center (P<0.001). LR erred leftward on line bisection, and had a smaller magnitude leftward bias on drawing tasks. We conclude that a leftward spatial-syntactic bias may not be innate and does not appear to be influenced by learned reading direction. In contrast, the leftward visual-spatial bias may occur in subjects whose cultural and reading background is neither western nor left-to-right.

Objective: To examine the prevalence of congenital heart disease in babies with oesophageal atresia and its influence on outcome., Design: Retrospective analysis., Setting: The resident population of one health region., Results: A total of 153 babies with oesophageal atresia were identified from 509 975 live births (0.30 per 1000); 26 (17%) had cardiac defects. Survival of babies with normal hearts was 97%, 97%, and 95% at one week, one month, and one year. Survival of babies with congenital heart disease was 85%, 85%, and 67% at one week, one month, and one year, but only one of ten deaths was the result of the congenital heart disease. The remaining deaths were due to other congenital malformations, respiratory disease, or chromosome abnormalities., Conclusions: There is a high prevalence of congenital heart disease in babies with oesophageal atresia. Congenital heart disease is associated with a higher mortality in oesophageal atresia but it is not the cause of it.

To determine whether monocular patching influences the performance of a patient with primarily sensory-attentional bias on the line bisection task, we present a case study of a 49-year-old woman who had right cortical infarction affecting temporal, parietal, and occipital regions. She had primarily sensory-attentional bias when performing the line bisection task on a video apparatus. In hospital, she was tested with monocular eye patching of the left or the right eye or unpatched. Paradoxically, the right-eye patching significantly worsened and the left patch significantly improved performance. The eye may have some input to the ipsilateral as well as the contralateral superior colliculus. Alternatively, the patch-a novel tactile stimulus-may induce orienting to its side via noncollicular mechanisms. When using a monocular patch for any reason, clinicians should be aware that increased spatial bias may occur.

Objective: To learn if Parkinson's disease (PD) is associated with a restricted attentional "floodlight.", Background: Different visual tasks may have different attentional requirements. Focused attention may be needed for some tasks; other tasks demand spatially distributed attention. Neglect after right cortical injury and dopamine depletion may limit the area over which attention can be spread. Although subjects with PD have dopamine depletion and can perform poorly on tests of visuospatial function, it is unclear if their attentional floodlight is restricted., Methods: Eleven subjects with PD and 11 control subjects viewed different-sized letters on five printed stimulus sheets, 43 x 56 cm. On each sheet, four different large letters (14 cm2) were composed of four different medium-sized letters (2.5 cm2), which in turn were composed of four different small letters (0.4 cm2). Stimulus sheets were presented at 30- and 75-cm viewing distances. Subjects named "all the letters they could see.", Results: Subjects with PD named small- and medium-sized letters comparably to control subjects, but PD subjects named fewer large letters than control subjects (control = 65.68%, PD = 24.55%; group-by-letter-size interaction, p < 0.05). Subjects with PD who had undergone stereotactic pallidotomy named more letters than prepallidotomy PD subjects (p = 0.05)., Conclusions: PD may affect the patient's ability to perceive large spatial configurations. As global configurations in subjects may be perceived preferentially over local patterns, it is possible that DA depletion induces an aberrant perceptual-attentional bias, such that patients have a narrowed attentional floodlight.

Objective: To learn how PD influences verbal description of emotional events., Background: Individuals with PD exhibit emotional processing deficits. Emotional experience likely involves several dimensions (e.g., valence, arousal, motor activation) subserved by a distributed modular network involving cortical, limbic, basal ganglia, diencephalic, and mesencephalic regions. Although the neurodegeneration in PD likely affects components in this network, little is known about how PD influences emotional processing. Because PD is associated with activation deficits, one could predict that the discourse of emotional experiences involving high activation would be reduced in patients with PD compared to control subjects. Alternatively, because patients with PD exhibit paradoxical sensitivity to externally evoked motor activation (kinesia paradoxica), it is possible that emotional stimuli may facilitate verbal emotional expression more so in patients with PD than in control subjects., Methods: The authors measured verbal descriptions of personal emotional experiences in subjects with PD and normal controls., Results: Compared with control subjects, individuals with PD showed a relative increase in the number of words spoken and in discourse duration when talking about emotional experiences that are usually associated with high levels of arousal and motor activation. Although the authors did not measure arousal or activation, prior research has shown that, when asked to recall an emotional experience, people will often re-experience the emotion previously experienced during that episode., Conclusions: Recalling emotional episodes induces verbal kinesia paradoxica in patients with PD. Although recall of these emotional episodes may have been associated with increased arousal and activation, the mechanism underlying emotional verbal kinesia paradoxica is unclear.

Objective: Patients who misperceive that they are moving their paralysed arm (phantom movements) may not recognise its weakness. Therefore, the relation between phantom limb movements and anosognosia for hemiplegia during selective right hemispheric anaesthesia (the Wada test) was examined., Methods: Nine patients with intractable epilepsy underwent the Wada test. During the right hemispheric injection, after the onset of hemiparesis, anosognosia was assessed by asking patients if they were weak. The patient's vision was limited such that they could not see the position of their limbs. Phantom movements were tested for by asking patients to attempt to lift their left upper limb, and to demonstrate their left limb's position by placing their right limb in the same position as their left. Proprioception was tested by lifting the patient's paretic upper limb and having patients demonstrate this position by lifting their right limb to the same position., Results: Three patients experienced left phantom limb movements, and five were anosognosic for their hemiplegia. However, phantom movement occurred in only one patient with anosognosia. The other two patients with phantom movement were without anosognosia. The patient with phantom movement and anosognosia had impaired proprioception. The two patients with phantom movement but without anosognosia had intact proprioception., Conclusions: Phantom movement in the presence of a proprioceptive deficit could contribute to anosognosia. However, anosognosia and phantom movement are dissociable; therefore phantom movement cannot alone account for anosognosia. Because phantom movement occurred with and without proprioceptive deficits, proprioceptive loss is not a prerequisite for phantom movement.

The relationship between autonomic-visceral arousal and emotional experience is unclear. The attribution or cognitive-arousal theory of emotional experience posits that emotional experience is dependent on both visceral-autonomic nervous system feedback and the cognitive interpretation of the stimulus that induced this visceral activation. The finding that false cardiac feedback can alter emotional experience suggests that it may be the conscious perception that one is aroused, together with the cognitive interpretation of the stimulus that are important in developing emotional experience. Because the right hemisphere appears to play a special role in modulating arousal and interpreting emotional stimuli, it is possible that right hemisphere damage may interfere with developing the computations needed for emotional experience. To test this hypothesis we exposed men, both neurologically intact and those with right and left hemisphere lesions, to emotionally provocative pictures that were paired with false cardiac feedback, and examined the effects of this false feedback on their ratings of attractiveness of these pictures and their cardiac reactivity to this information. Subjects with left hemisphere damage, but not right hemisphere damage, showed significant changes in their emotional rating whereas control subjects showed marginal reactivity in their emotional ratings. Subjects with left hemisphere damage also showed significant changes in their cardiac reactivity. This finding is consistent with prior reports that indicate, when compared to right hemisphere damaged patients and normal controls, patients with left hemisphere lesions have an increased visceral-autonomic response to stimuli. These findings further provide support for the postulate that it is the cognitive interpretation of perceived physiological arousal together with the cognitive interpretation of the stimulus that is important in the development of emotional judgment and experience. These results do not support the approach-left hemisphere/avoidance-right hemisphere dichotomy, but instead suggest that left hemisphere damage increases reactivity to false feedback, and that the intact right hemisphere function integrates the cognitive interpretation of the emotional information and perceived arousal that lead to that emotional judgment. That these subjects showed no consistent relationship between their measures of cardiac reactivity and their ratings of attractiveness detracts from the James-Lange and attribution theories. These subjects also showed no consistent relationship between their knowledge of affective physiological reactivity and their ratings of attractiveness, or between their knowledge of physiological reactivity and actual measures of cardiac reactivity, suggesting that other neuropsychological factors are involved in making an emotional judgment.

Holmes noted that with hemispheric injuries proprioceptive disturbances were more marked in the distal than proximal limb segments and proposed that this difference was related to the size of cortical sensory representations. An alternative hypothesis is that sensation from distal segments projects to the contralateral hemisphere and sensation from proximal segments projects to both hemispheres. Selective hemispheric anesthesia was used to test these alternative hypotheses and revealed a decrement in distal but not proximal proprioception with hemispheric anesthesia, thereby supporting the bilateral projection hypothesis.

Individuals with autism spectrum disorder (ASD) have impaired ability to use context, which may manifest as alterations of relatedness within the semantic network. However, impairment in context use may be more difficult to detect in high-functioning adults with ASD. To test context use in this population, we examined the influence of context on memory by using the "false memory" test. In the false memory task, lists of words were presented to high-functioning subjects with ASD and matched controls. Each list consists of words highly related to an index word not on the list. Subjects are then given a recognition test. Positive responses to the index words represent false memories. We found that individuals with ASD are able to discriminate false memory items from true items significantly better than are control subjects. Memory in patients with ASD may be more accurate than in normal individuals under certain conditions. These results also suggest that semantic representations comprise a less distributed network in high-functioning adults with ASD. Furthermore, these results may be related to the unusually high memory capacities found in some individuals with ASD. Research directed at defining the range of tasks performed superiorly by high-functioning individuals with ASD will be important for optimal vocational rehabilitation.

The objective of this study was to develop an immunoassay that would be capable of detecting flunitrazepam and/or cross-reacting metabolites in urine and comparing the results with those obtained by gas chromatography-mass spectrometry. Doses of Rohypnol varying between 0.5 and 4 mg were given to volunteers, and urine was collected for up to two weeks postingestion. These samples were analyzed by an ELISA that was developed using an antibody raised to flunitrazepam and a drug-enzyme conjugate prepared by attaching 7-aminoflunitrazepam to horseradish peroxidase. Significant levels of flunitrazepam and/or cross-reacting metabolites were detected in urine for up to one week after ingestion. The immunoassay is selective with only diazepam cross-reacting at a level of 1000 microg/L.

Whereas the ventral cortical visual stream is important in object recognition, the dorsal stream is specialized for spatial localization. In humans there are also right and left hemisphere asymmetries in visual processing: the left hemisphere being more important in object recognition and the right in specifying spatial locations. Based on these dorsal-ventral and right-left where-what dichotomies, one would expect that the dorsal right hemisphere systems would be most activated during spatial localization tasks, and this activation may induce a leftward spatial bias in lower space. To determine if visual stimuli in upper and lower body space evoke different hemispheric activation, we had 12 normal participants bisect horizontal lines above and below eye level. Participants erred leftward in lower body space relative to upper body space (M = 1.3345 mm and 0.4225 mm, respectively; p = .011). In upper body space, bisection errors did not differ from zero, but in lower body space, errors tended to deviate leftward (M = 1.3345 mm, differs from null hypotheses at p = .0755). Our results are consistent with dorsal stream/right hemisphere activation when performing a spatial localization task in lower versus upper body space.

Objective: To learn how pAD (probable Alzheimer's disease), PD+ ("Parkinson's Plus" syndrome), and control subjects remember internally generated material under different conditions., Background: "Self-discovered," or internally generated knowledge, prized by educators and therapists, can bring about considerable behavioral change. Both parietal-temporal-limbic (pAD) and frontal-subcortical dementia (e.g. PD+) cause dysmemory, but may cause different internal-external memory bias. pAD subjects, confusing internal and external information (confabulation) and reporting internal information during memory testing (intrusions), may be biased to remember internal material. PD+ subjects, impaired at generative tests, may be externally biased., Methods: Ten pAD, 5 PD+, and 10 control subjects generated words in a category without instruction to remember (INR), and took a list-learning test of incidental memory for internally and externally generated words. To test how INR influences memory, subjects then generated and attempted to recall four more words., Results: All three subject groups remembered more internally generated than externally provided words without INR. Recall versus recognition of internally generated words differed by group, with PD+ subjects showing greatest improvement with recognition. The pAD subjects performed worse with INR than without INR, had the most intrusion errors, and, rather than demonstrating a release from proactive inhibition, recalled fewer words outside the category. Groups differed in overall recall/recognition improvement (p = 0.015)., Conclusions: Aged subjects preferentially retained internally generated material. However, among demented subjects, memory for internally generated words was influenced by the testing method used. PD+ subjects have poor internal recall, but excellent internal recognition. In pAD, memory for internally generated words may exceed external memory, but only when subjects are not explicitly trying to remember.

Objective: This study was undertaken to determine the rate of prenatal diagnosis and surgical outcome of all cases of esophageal atresia reported to the Northern Region Congenital Anomaly Survey., Study Design: A retrospective review was conducted on maternal and infant case notes of all cases of esophageal atresia in the Northern Region from 1985-1997, inclusive., Results: A total of 176 cases of esophageal atresia was reported, and 158 diagnoses were confirmed after birth. Six cases were excluded because of incomplete data. Among the 32 patients in whom esophageal atresia was suspected antenatally because of an absent stomach bubble and hydramnios, 14 (44%) had esophageal atresia confirmed postnatally. In 10 of the 18 patients with false-positive diagnoses the stomach was subsequently seen. Esophageal atresia should have been suspected prenatally in a further 38 patients with polyhydramnios, 3 of whom also had an absent stomach bubble. There were 12 pregnancy terminations, 1 spontaneous abortion, and 19 perinatal deaths (including 9 stillbirths). Among the patients with esophageal atresia, 63.2% had associated anomalies (including 5.3% with aneuploidy), and 78.4% of these anomalies were missed prenatally. Among the live births 21.5% of the infants had a birth weight below the 5th percentile. One hundred eight (90%) had esophageal atresia with a distal tracheoesophageal fistula, and overall 102 (85%) underwent a primary repair. Among the 120 infants who underwent surgical treatment 11 subsequently died, and 6 of these deaths were related to postoperative complications. Thirty-nine infants (32.5%) had postoperative gastroesophageal reflux, necessitating fundoplication in 21 cases. At 2-year follow-up 23 of 89 infants had dysphagia, for which 7 still required a gastrostomy or jejunostomy. Infants in whom the condition was diagnosed prenatally were more likely to need prolonged mechanical ventilation, to have a longer hospital stay, and to have long-term gastrointestinal problems., Conclusions: Most cases of esophageal atresia are not suspected prenatally. Among fetuses with ultrasonographic features suggestive of esophageal atresia, 50% have the disorder confirmed postnatally. Overall perinatal and infant mortality rate among those with esophageal atresia is high (21.6%), and a further 21% of affected infants have significant morbidity after the age of 2 years.

Studies of animals and humans with focal brain damage suggest that attention in near and far extrapersonal space may be mediated by anatomically separate systems. Thalamic lesions have been associated with spatial neglect, but whether asymmetric attention specific to near or far space occur after thalamic damage has not been explored. It is also unclear if thalamic injury can induce contralesional defective response inhibition. We tested a woman with a left thalamic infarction who reported that, when driving, she had a tendency to veer towards people or objects on the right side of the road. Our patient and four controls performed a line bisection task with a laser pointer in near and far extrapersonal space. The experimenter marked each bisection either from the right of the presented line (right-distractor, RD) or the left (left-distractor, LD). RD and LD trials were pseudo-randomized. Our patient performed similarly to controls (mean -0.7 mm, controls -0.6 mm) on the line bisection task in near space. In far space she erred significantly rightward compared to her performance in near space (p<0.001). Controls performed similarly in near and far space. The experimenter position did not affect our patient's performance on near line bisections, nor did controls demonstrate a distractor effect for the near condition. In the far condition, however, our patient showed a significant distractor effect (LD -3.3 mm, RD 35.3 mm, p<0.001). Controls also demonstrated a distractor effect in the far condition (LD -6.4 mm, RD 0.7 mm, p<0.01), though of much smaller magnitude. Our results suggest that frontal-thalamic systems regulating visual attention may be disrupted by thalamic infarction. Such damage may produce an attentional grasp specific to far extrapersonal space.

Visual-spatial deficits are often associated with Parkinson's Disease (PD). Recent theories suggest that frontal-basal ganglionic dysfunction affects cognition in PD. Although this hypothesis does not entirely explain spatial deficits in PD, the inappropriate utilization of cues associated with executive dysfunction may induce spatial deficits. Alternatively, the vestibular system is also involved in spatial cognition, and vestibular dysfunction may affect visual-spatial ability in PD. To test these hypotheses, we administered the Water Jar Test, while perturbing vestibulo-proprioceptive input. Non-demented PD patients were significantly less accurate than controls in judging horizontal, and appeared to inappropriately utilize cues. No group effect was found for head tilt. These findings suggest the visual-spatial difficulties seen in PD are related to executive dysfunction that is associated with a disruption of the frontal-basal ganglionic and frontal-parietal systems.

Background: Contralesional hemispatial neglect may be induced by an attentional deficit where patients are inattentive to or unaware of stimuli in contralesional hemispace, an intentional deficit where patients are unable to act in or towards contralesional hemispace, or both. The deficits associated with ipsilesional neglect have not been as well characterized. Because cueing may be used as a rehabilitative assistive device, we wanted to learn whether the efficacy of an attentional or intentional cue was related to the type of bias., Methods: We studied a patient with a right frontotemporal stroke who had ipsilesional neglect by using a video apparatus that dissociates sensory-attentional and motor-intentional systems. We also performed a cueing experiment with primarily sensory-attentional cues (i.e., read the letter at the end of the line) and primarily motor-intentional cues (i.e., touch the end of the line)., Results and Conclusions: Ipsilesional neglect was primarily a motor-intentional deficit with a motor-action bias to the left and a secondary sensory-attentional bias for stimuli to the right. With cueing we found a double dissociation: the rightwards motor-intentional cue improved the primary left-sided intentional bias and the leftwards sensory-attentional cue improved the secondary right-sided attentional bias. Effective rehabilitation strategies need to address both sensory-attentional and motor-intentional deficits in patients with neglect.

Studies in animals and humans report dopamine agonists can improve neglect. Because dopamine deficit reduces intention to act, it has been suspected the dopamine agonist bromocriptine would improve deficient hemispatial intention. Thus, the effect of bromocriptine on line bisection was examined in a patient with neglect and failure of the action-intention system. The 58-year-old patient had left-sided neglect from a right cerebral infarction involving both cortical and subcortical (striatal) structures. It was determined that neglect on a line bisection task was attributable to a motor-intentional bias by testing under congruous and incongruous video monitoring. Testing sessions were held before starting bromocriptine, on 20 mg/d, and after stopping bromocriptine. The patient's ipsilesional bias increased on bromocriptine, and improved when bromocriptine was stopped. Bromocriptine may worsen neglect if putamenal receptors are damaged. Dopamine agonists may activate the normal hemisphere, increasing an intentional bias. Clinicians using dopaminergic pharmacotherapy should assess patients for this possible adverse effect.

Background: Several studies have demonstrated that patients with right hemisphere damage, when compared with left-hemisphere damaged controls, are impaired at comprehending emotional prosody. Critics of these studies, however, note that selection may have been biased because left-hemisphere-damaged subjects had good verbal comprehension., Objective: To learn whether a subject with a large left hemisphere stroke and global aphasia could comprehend emotional prosody in spoken material., Method: The authors formally tested speech and language with the Western Aphasia Battery and comprehension of emotional prosody and emotional facial expression with the Florida Affect Battery., Results: The patient could not perform verbally mediated tests but demonstrated spared ability to match emotional prosody to emotional facial expressions under a variety of conditions., Conclusions: These observations further support the idea that verbal and emotional communication systems are independent and mediated by different hemispheres.

Probable Alzheimer's disease is diagnosed by exclusion when no other disorder--such as stroke, brain infection, or vitamin deficiency--can be found to account for specific cognitive change. Although probable Alzheimer's disease affects both men and women, studies in many different populations show that between 1.5 and 3 times as many women as men suffer from probable Alzheimer's disease. For women with the disease, one needs to consider behavioral and cognitive problems, therapeutic issues, and other gender-related risks. Additionally, social, psychological, and emotional variables play a role in the female caregiver's burden.

Five commercially available immunoassay test kits (SYVA EMIT(R) d.a.u(TM), SYVA EMIT(R) II assay, Abbott FPIA, Cozart Auto-Lyte(R) and Roche Abuscreen(R) Online(TM), all used for the benzodiazepine group of drugs) were evaluated for their ability to detect flunitrazepam, its major urinary metabolite, 7-aminoflunitrazepam, and several other benzodiazepines at serial dilutions (final concentration 25-1000 ng/ml) in drug-free urine and in urines following oral administration of flunitrazepam (1-3 mg). For comparison, gas chromatography/mass spectrometry was used to measure urinary levels of 7-aminoflunitrazepam. Levels of drug detected in the study were compared with the cross-reactivities presented by the manufacturers for each individual kit. One to three mg doses of flunitrazepam were taken by volunteers and levels excreted in urine analysed over several hours. A positive response was obtained in several samples from volunteers who had taken 2 mg or 3 mg doses, but not a 1 mg dose. Thirty-five clinical samples from the individuals suspected of benzodiazepine abuse were also examined. The results were not consistent among the kits evaluated.We conclude that the test kits evaluated in this study do not detect flunitrazepam reliably, due primarily to their poor sensitivities.

We wanted to learn if pupillary changes induced by looking and attending to stimuli on the right and left are asymmetrical. In humans, there are hemispheric asymmetries in the control of attention-arousal systems. Because attention and arousal may influence pupil size, asymmetric pupillary responses may be seen when looking and attending in different directions. Twelve right-handed, healthy volunteers served as subjects. Using infrared pupillography, we recorded changes of pupillary diameter while subjects were looking and attending to the stimuli on the right and left sides of space. For the one second following a saccade, there are three phasic pupillary responses, an initial constriction (C1) then a dilation (D1), followed by constriction (C2). Evaluation of these three responses revealed right-left asymmetries with more pupil dilation (D1) when looking to the stimulus on the right. Our results suggest that subjects are more aroused when looking to the right than when looking to the left.

Anosognosia of hemiplegia is of interest for both pragmatic and theoretical reasons. We discuss several neuropsychological theories that have been proposed to explain this deficit. Although for psychological reasons people might deny deficits, the denial hypothesis cannot account for the hemispheric asymmetries associated with this disorder and cannot explain why some patients might deny one deficit and recognize another equally disabling deficit. There is some evidence that faulty feedback from sensory deficits, spatial neglect and asomatognosia might be responsible for anosognosia in some patients. However, these feedback hypotheses cannot account for anosognosia in all patients. Although the hemispheric disconnection hypothesis is appealing, disconnection is probably only a rare cause of this disorder. The feedforward intentional theory of anosognosia suggests that the discovery of weakness is dependent on attempted action and some patients might have anosognosia because they do not attempt to move. We present evidence that supports this theory. The presence of one mechanism of anosognosia, however, does not preclude the possibility that other mechanisms might also be working to produce this disorder. Although a large population study needs to be performed, we suspect that anosognosia might be caused by several of the mechanisms that we have discussed. On the basis of the studies of impaired corporeal self-awareness that we have reviewed, we can infer that normal self-awareness is dependent on several parallel processes. One must have sensory feedback and the ability to attend to both one's body and the space where parts of the body may be positioned or acting. One must develop a representation of the body, and this representation must be continuously modified by expectations (feedforward) and knowledge of results (feedback).

We present a case study of a 43-year-old woman with chronic and stable pure alexia. Using a multiple baseline design we report the results of two different interventions to improve reading. First, a restitutive treatment approach using an implicit semantic access strategy was attempted. This approach was designed to exploit privileged access to lexical-semantic representations and met with little success. Treatment was then switched to a substitutive treatment strategy, which involved using the patient's finger to pretend to copy the letters in words and sentences. Reading using this motor cross-cuing strategy was 100% accurate and doubled in speed after 4 weeks of intervention. We propose that this patient's inability to benefit from the implicit semantic access treatment approach may be in part related to her inability to suppress the segmental letter identification process of word recognition.

Objective: To learn whether there was a defect in an attentional floodlight. We used a line decision task for which subjects had to decide if two line segments separated by a gap were one line or two parallel lines. We varied the area of the gap and, therefore, the area over which subjects needed to spread attention to perform the task correctly., Background: Visual tasks requiring focused attention use an attentional spotlight. Other visual tasks requiring spatially distributed attention may require a floodlight. Neglect after right hemisphere stroke can be associated with a defect in the attentional spotlight., Results and Conclusions: Two patients with neglect after right hemisphere stroke performed more poorly than normal control subjects and left hemisphere-damaged control subjects as the area of spread in the gap increased. Right hemisphere-damaged patients did not differ from control subjects' performance on another visuospatial parameter--the degree of discontinuity between the line segments. These results support a defective attentional floodlight in neglect.

Limb apraxia, a defect in skilled, learned purposive movement, may be related to impairment of either representational or innervatory components of praxis processing. Innervatory motor patterns, in turn, may involve on-line motor programs (visual feedback-controlled) or prepared movement programs (independent of continuous visual feedback). We evaluated movement abilities ofthe innervatory pattern system in TB,a 26-year-old patient with apraxia from a left dorsolateral frontal stroke. TB and four controls performed nonmeaningful single and multi-joint movements to command, with multi-joint movements combined sequentially (e.g. "open and close your hand and then bend your elbow") or simultaneously (e.g. "open and close your hand; keep doing that while bending your elbow"). TB showed no difference between single-joint (71.5% correct) and multi-joint movements in sequential combinations (68% correct), but she was significantly worse at simultaneous movement combinations (28.6% correct; P < .02). Controls performed consistently at > 90% mean accuracy. TB and four normals also performed the Fitts (1954) task, in which they alternately tapped with a pen between two target circles of varying size. TB was proportionately slower than controls on the larger Fitts circles, which call predominantly on prepared movement programs; her performance on the smaller circles (involving more on-line programs) was comparable to normals. We conclude that functional synchrony of one innervatory pattern subtype, prepared movement programs, may require late-level frontal processing, and that failure at this level can result in both apraxia and defective programming of nonmeaningful movements.

Feinberg et al. proposed that right-hemisphere-damaged stroke patients with anosognosia for hemiplegia (AHP) confabulate seeing stimuli on the left side but those without AHP admit to having inadequate visual information. This study examines the relationship between AHP and confabulation using selective anesthesia of the cerebral hemispheres. Seventeen patients with intractable epilepsy were tested during intracarotid methohexital infusion. For half of the trials, subjects were stimulated on their paretic hand with a material (sandpaper, metal, or cloth), and for the remaining trials they were not stimulated. The subjects were trained to use a pointing response to indicate if they been stimulated and the type of material they had felt. Admission of uncertainty was defined as pointing to a question mark. Confabulation was defined as any material response to a no-touch trial. During anesthesia of either hemisphere, subjects with and without AHP confabulated responses. The AHP and non-AHP groups did not differ in admission of uncertainty. Our results support the postulate that confabulation and AHP are independent disorders, and therefore confabulation cannot fully account for AHP.

A cross sectional audit of central venous catheter (CVC) use was performed in United Kingdom Children's Cancer Study Group oncology centres. There were wide variations in choice of line, insertion technique, aftercare practice, and diagnosis of CVC related sepsis. These variations highlight the difficulty in interpretation of published data on CVC efficacy.

Delayed surgery has become widely accepted in the management of congenital diaphragmatic hernia after comparing outcomes only with historical retrospective controls. It was the aim of this study to compare early and delayed hernia repair in a randomized prospective clinical trial. Fifty-four infants were randomized to receive either early repair (within 4 hours of admission) or delayed repair (more than 24 hours after birth). The survival rate was higher for the delayed group (57% v 46%), but the difference was not significant (difference: -11; 95% confidence limits: -37.5, 15.5). There were no significant differences between the two groups with respect to length of hospital stay, ventilator dependency, or survival time. Recorded preoperative risk factors were similar for the two groups. Eight infants in the delayed repair group died without having undergone surgery. The optimum time for surgery still needs clarification.

Objective: The intrastromal corneal ring (ICR), a new keratorefractive device designed to correct myopia, was evaluated for safety and efficacy as part of a phase I Food and Drug Administration clinical trial., Patients and Methods: Ten patients, each with one nonfunctional eye, underwent surgical implantation of the ICR, which is a split ring composed of polymethyl methacrylate. The eyes were followed up during a 1-year period and were evaluated for wound healing, implant-related inflammation, intraocular pressure, corneal thickness, endothelial cell count, and refractive effects., Results: Wound healing was uncomplicated in all of the patients. No patient experienced implant-associated inflammation or extrusion. Intraocular pressure, corneal thickness, and endothelial cell counts were unaffected by the ICR. Non-sight-threatening intraoperative events included ring decentration and inadequate attachment of a vacuum-centering guide. The ICR reduced keratometrically determined spherical equivalence by an average of -2.5 +/- 1.1 diopters (mean +/- SD). Spherical equivalence, as determined by retinoscopy, was reduced by an average of -2.4 +/- 1.0 D. Results of corneoscopy and computerized videokeratography further confirmed this reduction of myopia., Conclusions: The ICR may be a safe and efficacious device that offers predictable results with few side effects, and it may be a feasible alternative to current keratore-fractive procedures.

Corneal epithelial iron lines commonly occur, and their shape is characteristically influenced by the underlying corneal surface topography. We studied a pattern of iron deposition, observed after implantation of the Intrastromal Corneal Ring (KeraVision, Inc., Santa Clara, California). In five of ten patients undergoing placement of the Intrastromal Corneal Ring, an arcuate pattern of epithelial iron deposition was observed eight to 12 months postoperatively. There was no significant clinical or topographic difference over time among the corneas of patients who developed an iron line vs those who did not. Of the five patients with an arcuate epithelial iron line, three underwent explantation of their Intrastromal Corneal Ring at 12 months according to the study protocol. Iron deposition either reverted to a Hudson-Stähli pattern or disappeared in each of these three patients.

Age differences were examined in 4 aspects of visual mental imagery, namely, image generation, maintenance, scanning, and rotation. The results suggested that one or more distinct processes are used to carry out each aspect of imagery, and that this is true for 5-year-olds, 8-year-olds, 14-year-olds, and adults. There was no evidence that younger children have fewer processing components, which become differentiated into more specialized subsystems over age. In addition, the results suggested that younger children are relatively poor at scanning, rotating, and generating objects in images, but are relatively good at maintaining images.

Performance of the IL 613 automatic blood-gas analyser has been assessed using a group of 100 "analysts" with no previous training or experience in the use of the instrument. Test material consisted of blood equilibrated to a known PO2 and PCO2 in a tonometer: pH estimations were carried out on thawed aliquots of a large batch of frozen serum which were then equilibrated to a known PCO2. Eighty-six per cent of analyses were within acceptable limits of error. The largest proportion of errors was in the measurement of pH. Satisfactory results were obtained in 98% of the analyses of PO2 and PCO2. Eighty-eight per cent of operators were able to use the analyser after instruction lasting less than 1 min. These results were significantly better than those obtained in a regional survey of 16 blood-gas laboratories, staffed by trained technicians.
