This study examines the relationships among oil prices, gold prices, and the USD real exchange rate. It adopts the wavelet approach as a nonlinear causality technique to decompose the data into various scales over time. Higher-order coherence and partial coherence were used to identify the lead-lag effect and mutual coherence function among the variables. The results show that changes in the USD exchange rate influence the prices of oil and gold negatively in the short- and medium-term. While in the long-term, the oil price has a negative impact on the value of the USD. Oil and gold are significantly linked and correlated because their prices are determined in USD. The findings of this paper have significant implications, particularly for risk management.

In2S3 films have been deposited by the successive ionic adsorption and reaction technique (SILAR) at room temperature. The films have been examined to evaluate the structural and optical properties. X-ray diffraction spectra have revealed the presence of both the α-In2S3 (cubic) and β-In2S3 (tetragonal) phases. The presence of the α-In2S3 phase at room temperature is attributed to the richness of In in the deposited materials. The presence of both phases is also supported by FESEM observations. The crystallinity of the material has been observed to improve with increasing thickness. Direct band gap of the deposits decreased from 2.89 to 2.37 eV with increasing thickness. The main object of this study is to investigate a fluid—structure interaction problem using Newton—Krylov techniques. As a test problem, a bimetallic slab exposed to a non-isothermal flow field is selected. If the metals on the body have different coefficients of thermal expansion, then the structure deflects in the case of a temperature change. In this study, natural convection is the reason for the temperature gradient on the slab. Solution of incompressible Navier—Stokes equations are weakly coupled with the linearized quasi-static thermoelasticity problem. The Stream function—vorticity approach is used to analyse the governing equations of the flow problem and finite differences are used for discretization. The finite-element method is preferred to solve the deflection of the solid which is modelled with plane strain assumption. Matrix-free methodology is utilized in the Newton—Krylov solver. The inexact Newton method is employed along with preconditioned Generalized Minimum RESidual as the linear solver. Domain decomposition is utilized both to ease the solution of the deformed flow geometry and to separate both physics. In this study, non-symmetric flow problems are modeled by selecting subdomains and shifting them in such a way that the symmetry is recovered. As a result, the domains are made of simple grid structures and re-generation of mesh is avoided. Three test problems with various decomposition characteristics, namely, translation, rotation and deformation are selected, and they are analyzed in different flow regimes. To study the internal flow between eccentric cylinders, two cylindrical concentric subdomains are considered, one translated relative to the other. Hence, a simple polar-coordinates mesh can be utilized instead of generating a mesh for the solution domain between the eccentric cylinders of the original problem. External flow around a curvature tube is studied shifting the subdomain around the object in rotation, relative to the outer domain thus avoiding a re-generation of the mesh as the angle-of-attack changes. A third example involves deformation of an object exposed to natural convection, and the shifting of the domain facilitates the iteration process as the object deflects. Systems of nonlinear equations are solved within Newton-Krylov framework using the matrix-free approach. Geometrical and physical parameters are used to improve the solution process. 115. New delirium rating scale for ICU.

- Abstract
Objective: The New Delirium Rating Scale (NDRS) makes use of verbal assessments to evaluate the cardinal features of delirium and is an observer-rated, 10-item symptom rating scale based on both DSM-IV and the findings of the previous clinical research. In this study, we tested the validity of the NDRS for measuring the severity of delirium in intensive care units. Methods: Thirty consecutive non-intubated patients admitted to the ICU for more than 24 hours were included in the study. Two intensivists were trained to rate delirium according to NDRS and they daily carried out structured interviews to evaluate the patients. All patients were interviewed by a psychiatrist for rating according to NDRS, and the data collected by the psychiatrist and the intensivists were compared. Results: The cut-off score for NDRS was 11.3 patients (10%), who were rated as delirious. The inter-rater reliability of the intensivists and the psychiatrist was found to be 0.84 and 0.90, respectively (p<0.0001). Conclusion: Intensivists easily used NDRS, a detailed delirium assessing scale, and rated delirium successfully. [ABSTRACT FROM AUTHOR]

118. Evaluation of renal function in Alzheimer's disease and geriatric patients: Results from a Turkish two-center study

Background: Alzheimer's disease (AD) is a severe multifactorial neurodegenerative proteopathy associated with advanced age. Discrepancies in the renal function of these patients compared to geriatric patients with dementia have rarely been reported. In this study, we aimed to disclose the importance of associated renal changes for the pathogenesis of AD. Methods: Patients with AD (n = 107) and geriatric patients with dementia and without dementia (n = 124) (231 patients in total) from Dokuz Eylul and Cukurova University Hospitals were enrolled in the study. We measured serum Na, K, Cl, Ca, BUN, creatinine, total protein levels and MDRD [eGFR] in all groups. Results: From Izmir Center, the first study arm consisted of patients with AD dementia (n= 74), and the second arm included geriatric patients with dementia (n = 79). From Adana, 78 patients were recruited to the study, of which 33 were with AD and 45 were geriatric patients without dementia. When we analyzed comparatively the AD and geriatric dementia patients study arms, a statistically significant difference was observed both in the median age (p< 0.001), as well as in the biochemical parameters from Izmir Center: Na (p< 0.001), K (p< 0.001), Cl (p< 0.05), Ca (p< 0.001), BUN (p< 0.05), creatinine (p< 0.001), total protein (p< 0.001) and MDRD [eGFR] (p< 0.001). However, these were not significantly different between AD and geriatric patients without dementia in the Adana group. Conclusions: Our results indicate that renal function is prone to alterations in different age groups of patients with AD. However, there is no conclusive evidence that renal function is one of the risk factors in AD.

Objective: In this study, we aimed to determine periimplantal tissue regeneration in the use of platelet-rich fibrin in periimplantal defects. Methods:Twelve male New Zealand white rabbits were used for this study. each of the right and left tibias of rabbits Using a trephine bur (diameter 7.0 mm), monocortical defects were prepared (7 mm width,4 mm depth).Subsequently, dental implants(AL-Technology dental implants sharkimplants) were installed into the left and right tibia (diameter 3.0 mm, length 8 mm). In the experimental groups, the peri-implant defect was filled respectively 1-autogenous grafts, 2-PRF 3-synthetic grafts (MIS 4BONE ). The control was left in an unfilled state. İmplant stability was evaluated with the resonance frequency analysis method (RFA)Results: The Ostell ISQ for the experimental groups respectively 1 -autogenous graft group 76.00±3.89,2-PRF group 75.00±3.84,3- sentetic graft group 67.00±5.72 while control group 56,33±5,12. The difference between the 2 groups (autogenous graft and prf groups) was statistically not significant while all other differences between groups are statistically significant.Conclusion: Within the limits of this study, it was found that periimplantal defects in the regeneration of Platelet-Rich Fibrin is effective as the autogenous graft

Globalization has become a major issue of focus as rapid urban populations and urbanization effects are on the rise. A critical need arises for effective urban planning for Istanbul in relation to the use of a hybrid approach integrating AHP-Entropy and GIS for emergency facility planning. In this paper, the combination of AHP and Entropy methods was used for evaluating criterion weights subjectively and objectively. These techniques were utilized with regard to the assessment of suitable areas for planning new urban emergency facilities for Istanbul province which experiences increasing urban fire-related emergencies. AHP and Entropy have been used to evaluate the weights of determined criteria from expert preference judgments and GIS for processing, analysis and visualization of the model result in the form of a suitability map for new urban emergency facilities. Validation of the model was performed on the criteria with the strongest influence in the decision outcome and spatially visualized using the sensitivity analysis (SA) method of one-at-a-time (OAT). From the findings, it was estimated that 28.1% of the project area, accounting for a third of it, is likely to be exposed to the risk of urban fires and therefore immediate planning of new urban emergency facilities is recommended for adequate fire service coverage and protection.

122. Combining AHP and ROC with GIS for Airport Site Selection: A Case Study in Libya

Choosing airport locations requires thorough and comprehensive decisions to be made. To do so in a professional and logical manner is crucial for the social, economic, and logistic settings intended for any region. The present research takes place in Libya, where airports are just as vital for the economy in terms of tourism and investment by allowing for improved transportation throughout the developing market and supplier locations as well as trading between the industrial and financial sectors. For this reason, using the geographic information system (GIS) to determine the appropriate airport site, twenty-three criteria were considered. In addition, two different methods—analytic hierarchy process (AHP) and rank order centroid (ROC)—were utilized to derive the related weights. The comparison of the output maps from these two distinctive approaches shows that both approaches provide identical results. Finally, a sensitivity analysis was carried out to evaluate the reliability of the method used and select the best site among the proposed ones based on the result of the highest suitability index for each candidate site. This research provides a siting approach and substantial support for decision-makers in the issue of airport locations selection in Libya and other developing countries.

Postdilation is frequently used during coronary interventions to prevent stent malapposition. Currently there are contradictory findings regarding the benefits of postdilation for both intraprocedural and long-term outcomes. We evaluated the impact of postdilation among patients who presented with acute coronary syndromes (ACS) and underwent percutaneous coronary interventions (PCI). A total of 258 consecutive patients who presented with ACS and underwent PCI were included in the study. The patients were followed up for 25±1.7 months for the occurrence of major adverse cardiovascular events (MACE). During follow-up, 65 patients (25.2%) had MACE. This prospective randomized study investigates the possible toxic effects of interferon (IFN) @a2A on the mouse cochlea. Thirty-six albino Swiss mice that were randomly assigned to 3 groups underwent baseline auditory brain stem response testing bilaterally to objectively assess baseline hearing levels. The first group received a single dose of 50,000 units, and the second group received 100,000 units of IFN-@a2A intraperitoneally, whereas the third group was given no medication. Repeat auditory brain stem response testing revealed a significant rise in mean baseline peak equivalent sound pressure level thresholds in the groups that received IFN (P < 0.001). Histologicallly, the cochleae of mice that received IFN had a decreased number of fibroblasts in the spiral limbus, as well as prominent cytoplasmic vacuolation of these cells, compared with control animals. Loss of hair cells was not observed. It is possible that reversible biochemical and metabolic changes in the cochlea, rather than morphologic abnormalities, manifest IFN ototoxicity. This study investigated the influence of EAT on QT and P wave dispersions (QTd, PWd), as simple, non-invasive tools of proarrhythmia on surface ECG. Objective: We planned to investigate the effect of mean platelet volume (MPV) on in-hospital mortality and coronary risk factors in geriatric patients with ST segment elevation myocardial infarction (STEMI) who underwent primary percutaneous coronary intervention (PCI). Methods: We enrolled 194 consecutive STEMI patients. The study population was divided into two groups on the basis of admission MPVs. The high-MPV group ( n =49) included patients in the highest tertile (>8.9 fL), and the low-MPV group ( n =145) included patients with a value in the lower two tertiles (≤8.9 fL). Clinical characteristics, in-hospital mortality, cardiovascular risk factors, and outcomes of primary PCI were analyzed. Results: The patients in the high-MPV group were older, more of them had three-vessel disease, and they had higher in-hospital mortality. Patients with in-hospital death were older, had higher Gensini score, creatinine concentration, and MPV, and had lower HDL cholesterol concentration. MPV, age, HDL cholesterol concentration, creatinine concentration, and Gensini score were found to be independent predictors of in-hospital death. Conclusion: These results suggest that high admission MPV levels are associated with increased in-hospital mortality in geriatric patients with STEMI undergoing primary PCI.

129. Clinical and histopathological characteristics of cutaneous Leishmaniasis in Sanliurfa City of Turkey including Syrian refugees

Background: The aim of our study was to investigate the clinical and histopathological characteristics of cutaneous leishmaniasis (CL) in the city of Sanliurfa in Turkey, where Syrian refugees also reside. Materials and Methods: At the Harran University Hospital outpatient clinics between 2012 and 2013, 54 CL cases, including 24 Syrian patients, underwent punch biopsy of the skin and/or a touch imprint. Patients in whom leishmania parasites were detected were included in the study. The clinical and histopathological data of the patients were obtained by a review of the patients′ medical records. All the slides of each patient were re-evaluated histopathologically. Results: Fifty-four cases (mean age; 17 ± 12 years), consisting of 32 males (59.3%) and 22 females (40.7%), were examined. The most common site of involvement was the face (63%). The most common presentation was noduloulcerative lesions (57.4%). Histopathologically, the majority of the cases exhibited hyperkeratosis, follicular plugging of the epidermis, chronic inflammatory infiltration, leishmania amastigotes and non-caseating granulomatous inflammation in the dermis. Conclusion: CL presents with a wide spectrum of expression, both clinically and histologically, and may mimic other inflammatory and neoplastic diseases. The diagnosis of CL relies on the identification of leishmania amastigotes in either a direct smear of the lesion or in a tissue section.

Amaç: Kronik böbrek yetmezliği bulunan, renal transplant yapılamayan hastalar hemodiyaliz bağımlı olarak yaşamaktadır. Hemodiyalize giriş yolu olarak arteriovenöz şant (AV) altın standart yöntemlerdir. AV şant açılamayan ya da AV şant mevcut olup henüz olgunlaşmamış hastalarda hemodiyaliz"e girebilmek için hemodiyaliz kateterleri kullanılmaktadır. Hemodiyaliz kateterleri sıklıkla femoral, subklaviyan ve juguler venlere uygulanmakla beraber nadir olarak torakolomber yaklaşımla inferior vena kavaya da takılmaktadır. Çalışmamızda hemodiyaliz amacıyla kullanılan kateterlerin hasta memnuniyeti ve yaşam kalitesine etkilerini incelemeyi amaçladık. Yöntem: Kliniğimizde ocak 2011- mayıs 2012 yılları arasında hemodiyaliz amacıyla arteriyovenöz şant açılan ve katater takılan topla 139 hasta çalışmaya alındı. Hastalardan 91 erkek 48 kadın, yaş ortalaması 60,85 ±14,6 (min: 54, max:83) idi. Hastalar arasında demografik olarak anlamlı fark yoktu. Bu hastalar ile tek tek görülerek takılan kateter ve bölgelere göre menuniyet durumları araştırıldı. Bulgular: Takılan kataterlere bağlı olarak olguların %69.06"da herhangi bir şikayet gözlenmedi. En sık gözlenen şikayet ağrı olup olguların %17.9"da bu şikayet mevcut idi. Diğer şikayetler sırası ile %9.3 olguda görünümünden rahatsız olma ve %3.5 olguda yaşam kalitesinin olumsuz etkilenme olarak belirlendi. Komplikasyon olarak %10.7 oranında katater yeri enfeksiyonu ve %9.3 oranında katater oklüzyonu tespit edildi. Femoral bölgeye takılan kataterlerin enfeksiyon oranı %85 oranında tespit edildi. Sonuç: Kronik böbrek yetmezliği bulunan olgularda hemodiyaliz amacıyla katater takılacağı durumlarda öncelikli olarak juguler bölgenin tercih edilmesi gerektiği, juguler bölgenin uygun olmadığı durumlarda yüksek enfeksiyon oranları sebebiyle subklaviyan bölgenin ve en son seçenek olarak femoral bölgenin tercih edilmesi gerektiğini düşünüyoruz. Katater takılacak bölge ve oluşabilecek şikayetler hakkında hastaların bilgilendirilmesinin hasta memnuniyetini arttıracağı kanaatindeyiz

Amaç: Derin venöz sistemde trombüs oluşumu ve venöz akımın kesilmesi derin ven trombozu (DVT) olarak adlandırılmaktadır. Çalışmada DVT tanısı alan hastalarda tedavi sonuçlarını değerlendirmeyi amaçladık. Yöntem: Kliniğimizde kasım 2010 - mayıs 2012 tarihleri arasında doppler ultrasonografi ile derin ven trombozu tanısı almış toplam 73 hasta çalışmaya alındı. Hastaların yaş ortalaması 55.205 ± 18.12 (Min: 22, Max: 84) idi. Hastalara standart DVT tedavisi uygulandı. Olgular tarafımızdan 6. ay"da doppler ultrasonografi ile venöz sistemleri tekrar değerlendirildi. Hastalar etyolojik faktörlere göre incelendi. Bulgular: Otuz hasta cerrahi sonrası yetersiz profilaksi, 9 hasta malignite, 34 hasta ise idiyopatik idi. 39 olguda sol alt ekstremite, 21 olguda sağ alt ekstremite ve 13 olguda ise her iki alt ekstremitede derin ven trombozu mevcuttu. 6 ay sonunda doppler ultrasonografi ile tüm hastaların tromboze ven segmenti değerlerdirildi. 54 olguda (%73) tromboza ait radyolojik bulgu kalmamış olup, 12 olguda (%16.4) rekanalize tromboze segment, 7 olguda (%8.5) ise trombozun devam ettiği görülmüştür. Sonuç: Derin ven trombozu tedavisi ve profilaksisinin yeterli yapılması rekürrensi azaltırken rekanalizasyon ve tromboz lizisini arttırmaktadır. Olguların mortalite ve morbiditesinin azalmasını sağlamaktadır.

132. Patient Satisfaction with Hemodialysis Catheter in Patients with Chronic Renal Failure

Purpose: Patients with chronic renal failure, for whom renal transplantation is not an option, depend on hemodialysis for life. An Arteriovenous shunt is the gold standart access site for hemodialysis. In patients without an AV shunt or with an immature shunt, a hemodialysis catheter is used. Subclavian, femoral and juguler veins are common sites for hemodiaylsis catheter. Patients may have different complaints with the catheter according to the insertion site. In our study, we aimed to investigate the impact of the site of the catheter on patients' comfort and social lives. Method: Between January 2011 and May 2012, 48 women and 91 men (139 patients) who underwent AV shunt operation in our clinic. The mean age of the patients was 60.85±14.6(min:54, max:83) years. Every patient was interviewed individually about his comfort and satisfaction with the catheter.There were no statistically significant demographic differences among patients. Results: When insertion of a hemodialysis catheter is planned in patients with chronic renal failure, internal juguler vein is the optimum site and should be preferred in the first line. In cases which juguler vein is not available, subclavian vein is the next preferred site and he femoral vein, due to its high rates of infection, is the last place to choose for the insertion of a hemodialysis catheter. Conclusion: We believe that informing patients about the site of the catheter and the possible disadvantages will improve the patient satisfaction. [Cukurova Med J 2012; 37(4.000): 211-214]

133. Evaluation of Treatment Activity in Patients With Deep Venous Thrombosis

Purpose: Deep vein thrombosis (DVT) refers to thrombi formation and blood stasis in the deep veins of the extremities. This study was to determine the effectiveness of DVT treatment in our clinic. Method: All of 73 patients who diagnosed Deep Venous Thromboses with dupplex ultrasoun between december 2010 and may 2012, at our institution were enrolled in this study. The mean age of the patients was 55.205 ± 18.12 (Min: 22, Max: 84). Standart DVT treatment was applied. We re-evaluated the change in deep venous system using follow-up Dupplex ultrasonography after 6 months.The patients were observed according to etiology. Results: Etiologic factors were insufficient prophylaxis after surgery (30 patients), malignancy (9 patients) and idiopathic (34 patients). Deep venous thrombosis existed left lower extremity (n=39), right lower extremity (n=21) and bilateral lower extremity (n=13). Thrombosis vein segment of all patients were evaluated with duplex ultrasonography after 6 months. Radiologic findings of thrombosis disappeared in 54 patients (73%). Thrombosis segment were recanalization in 12 patients (16.4%). Thrombosis was not recovery in 7 patients (8.5%). Conclusion: If treatment and prophylaxis of deep venous thrombosis perform enough, recurrence decreases and recanalization and lysis of thrombosis increase. Leser-Trélat sign is a rare cutaneous disorder that manifests with sudden emergence of seborrheic keratoses and increase in their numbers and size within weeks or months. Its concurrence with many types of malignancies, especially adenocarcinoma and lymphoma, has been reported in the literature. In this case report, a patient having Leser-Trélat sign, without any detected malignancy in spite of detailed examination and 18 months of follow up, is presented. Current literature is reviewed in relation to the debate whether Leser-Trélat is a paraneoplastic syndrome or not.

Hepatocellular carcinoma (HCC) is the most common primary tumor of the liver.1,2 The most common extra-hepatic metastatic sites are lung, abdominal lymph nodes and bone, while its cardiac metastasis is rare.2,3 Metastasis of HCC into the cardiac cavity is mostly caused by direct tumor invasion of vena cava inferior with continuous extension into the right cardiac cavity.4,5 Right heart metastasis without invasion of inferior vena cava, which may be caused by hematogenous spread of cancer cells, is rarely reported.6,7 This paper announces an unusual case of isolated involvement of left ventricle (LV) together with myocardial invasion of HCC. Our patient is known to be the first case with isolated HCC metastasis to the left ventricle. Strikingly, the patient was young and non-cirrhotic with negative serum HBsAg, and anti-HCV results.

138. The Evaluation of In Vitro Effects of Dipyridamole on Coronary Artery Bypass Grafts

Objective: The aim of this study is to evaluate the efficacy of dipyridamole in relieving the vasospasm of coronary artery bypass grafts. Material and Methods: Twenty two patients who underwent elective coronary artery bypass grafting (CABG) were admitted to the study (Men/Women:14/8, mean age: 62.4±8.8 years). Ten samples from left internal mammary(LIMA), radial arterial (RA) and saphenous vein (SV) grafts were collected for each. The samples were transported to the vascular laboratory in 4°C Krebs solution. Submaximal smooth muscle contraction was achieved first by 10-7M of phenylephrine solution. Dipyridamole was then added, starting from a concentration of 10-9M to a concentration of 10-3.5M in two minutes intervals and half logarithmic dose increments. The concentration-response curves were obtained of the vasodilatation response relative to the begining.Results: In the LIMA graft samples, the vasodilation response to dipyridamole was 43.2±1.6% and 97.6±4.1% at concentrations of 10-6M and 10-3.5M respectively. In RA graft samples, the vasodilation response to dipyridamole was 36.3±1.8% and 95.3± 2.7% at concentrations of 10-6M and 10-3.5M respectively. In SV graft samples, however, the vasodilation response to dipyridamole was 43.2±1.4 % and 96.6± 2.2% at concentrations of 10-6M and 10-3.5M respectively.Conclusions: The amplitude of relaxation response to dipyridamole of all grafts samples were similar, without statistically significant difference among the IMA, RA and SV grafts in the in vitro tissue bath system. These results prove that dipyridamole has a potential use as a vasodilatatory drug in all graft types.

Background: Various adverse effects of homologous blood transfusion detected particularly in open heart surgery, in which it is frequently used, lead researchers to study on autologous blood use and to evaluate the patient′s blood better. Due to the complications of homologous blood transfusion, development of techniques that utilize less transfusion has become inevitable. We aimed to evaluate the effects of acute normovolemic hemodilution (ANH) in patients undergoing open heart surgery. Materials and Methods: In this study, 120 patients who underwent open heart surgery were included. Patients were grouped into three: Autologous transfusion group (Group 1), homologous transfusion group (Group 2), and those received autologous blood and homologous blood products (Group 3). Patient data regarding preoperative characteristics, biochemical parameters, drainage, extubation time, duration of stay at intensive care, atrial fibrillation (AF) development, and hospital stay were recorded. Results: A statistically significant difference (P < 0.005) was found in favor of autologous group (Group 1) with respect to gender, body surface area, European System for Cardiac Operative Risk Evaluation, smoking, hematocrit levels, platelet counts, urea, C-reactive protein levels, protamine use, postoperative drainage, frequency of AF development, intubation period, stay at intensive care and hospital stay, and amount of used blood products. Conclusion: The use of autologous blood rather than homologous transfusion is not only attenuates side effects and complications of transfusion but also positively affects postoperative recovery process. Therefore, ANH can be considered as an easy, effective, and cheap technique during open heart surgery.

143. NORMOCORTISOLEMIA AFTER TEN DAYS OCTREOTIDE TREATMENT IN A PATIENT WITH ECTOPIC CUSHING'S SYNDROME DUE TO BRONCHIAL CARCINOID.

Contex. We report the use of subcutaneous somatostatin injection three times a day to decrease hypercortisolism in a patient who had Cushing's syndrome induced by bronchial carcinoid tumour progressive pneumonia due to immune suppression. Subject and Method. A 46-year-old man with 7-month history of DM type-2, hypertension and cerebrovasculardisease, vertebral compression-fracture was admitted to our clinic. Physical examination was consistent with Cushing's syndrome. Laboratory results revealed hyperglycemia (143 mg/dL; reference range, <100 mg/dL) and hypokalemia (2.29 mEq/L; reference range, 3.5-5.1 mEq/L). His morning serum cortisol was 40 μg/dL (reference range 6.7- 22.6 μg/dL), urine cortisol-excretion was 2245 μg/24 hours (reference range 58-403 μg/24 hours), after 1 mg dexamethasonesuppression test serum cortisol was 28 μg/dL (6.7-22.6 μg/dL) and ACTH 354 pg/mL (reference range 7.9-66 pg/mL). Adrenal CT and hypophyseal MRI were normal. An ectopic source was searched for Cushing's syndrome. Chest CT scan of the right lung showed 12x9 mm nodule. High fever cough occurred on the follow-up. Chest radiograph revealed diffuse pneumonic infiltration. Despite 3-drug antibiotic combination therapy, infection did not improve. Subcutaneous injection of octreotide 3x100 μ/g was initiated to decrease hypercortisolism. The infection improved rapidly after the therapy. The morning serum cortisol, urine cortisol-excretion, ACTH was at the upper normal range (77.1 pg/mL, reference range 7.9-66 pg/mL) on 10th day of treatment. The patient was a consulted for surgery and the nodule was excised. The pathology was consistent carcinoid tumor. Conclusion. Subcutaneous octreotide treatment may be helpful to gain time for exploring the focus in ectopic cushing's syndrome and to control the serious infections due to hypercortisolism. Using an international perspective, the objective of this study is to assess the return on investment of influenza corporate vaccination programs in various countries, industrial sectors and sizes of companies.Considering the employer perspective, a cost–benefit model has been designed in order to provide a probabilistic assessment of the costs and benefits of vaccinating healthy working adults. Costs of the vaccination alternative are thus compared with the benefits, i.e., avoided absenteeism and loss of productivity by preventing the diseases. Companies from Australia, Brazil, China, Korea, Philippines, Thailand and Turkey have been sampled. Their financial data have been collected through international consultants'' databases and epidemiological data were gathered through literature review and national sources, in order to provide an average cost–benefit result by country. Various sensitivity analyses have then been performed to evaluate the impact of key data variations on the final result. By discounting costs and benefits at a rate of 3%, the annual net cost-savings per employee vaccinated ranges from US$6 in Thailand to US$108 in Australia. In this study, assessing the medical and economic benefits of influenza corporate immunization programs, the vaccination coverage rate is clearly identified as one of the key criteria securing vaccination benefits. Çalışmanın temel problemi ilkokul öğrencilerinin dijital oyun oynama tutumları ile saldırganlık düzeyi arasındaki ilişkinin ve dijital oyun oynama tutumları ile saldırganlık düzeylerinin cinsiyete, hafta içi ve hafta sonu oyun oynama sürelerine ve sınıf düzeyine göre farklılaşma durumunun incelenmesidir. Araştırma nicel bir araştırma olup araştırmanın modeli tarama modelidir. Araştırmanın çalışma grubunu 101 erkek, 86 kız olmak üzere İstanbul’da öğrenim gören toplam 187 ilkokul öğrencisi oluşturmaktadır. Araştırmada veri toplama aracı olarak Demir ve Bozkurt, (2019) tarafından geliştirilen Dijital Oyun Oynama Tutumları Ölçeği ve Gültekin (2008) tarafından geliştirilen Saldırganlık Ölçeği kullanılmıştır. Verilerin analiz edilmesi için basit doğrusal korelasyon, ilişkisiz grup t testi ve tek yönlü varyans analizi (ANOVA) kullanılmıştır. Elde edilen bulgulara göre, dijital oyun oynama tutumları ile saldırganlık düzeyi arasında pozitif yönde bir ilişki bulunmuştur. Erkeklerin dijital oyun oynama tutumları ve saldırganlık düzeyleri puanları kızların puanlarına göre daha yüksektir. Dijital oyun oynama tutumları ile saldırganlık düzeyi, hafta içi 0-1 saat dijital oyun oynayan kişilerin puanları ile 3-4 saat dijital oyun oynayan kişilerin puanları arasında anlamlı fark olduğu görülmüştür. Dijital oyun oynama tutumları ile saldırganlık düzeyi, hafta sonu 2-3 saat dijital oyun oynayan öğrenciler ile 0-1 saat ve 1-2 saat dijital oyun oynayan öğrenciler arasında anlamlı fark vardır. Dijital oyun oynama tutumları ile saldırganlık düzeylerinin sınıf seviyesine göre ise farklılaşmadığı tespit edilmiştir. The aim of this study is to examine the relationship between primary school students' digital game playing attitudes and aggression level, and the differentiation of digital game playing attitudes and aggression levels according to gender, weekdays and weekends playing times, and grade level. In the studycorrelational survey model was used. The study group of the research consists of 187 primary school students, 101 boys and 86 girls, studying in Istanbul. Digital Gaming Attitudes Scale developed by Demir and Bozkurt (2019) and Aggression Scale developed by Gültekin (2008) were used as data collection tools in the research. Simple linear correlation, independent samples t-test and one-way analysis of variance (ANOVA) were used to analyze the data. According to the findings, a positive relationship was found between the attitudes of playing digital games and the level of aggression. Boys' digital game playing attitudes and aggression levels scores are higher than girls' scores. It has been observed that there is a significant difference between the attitudes of playing digital games and the level of aggression, the scores of people who play digital games for 0-1 hours on weekdays and the scores of people who play digital games for 3-4 hours. There is a significant difference between the attitudes of playing digital games and the level of aggression, between the students who play digital games for 2-3 hours on the weekend and those who play digital games for 0-1 hours and 1-2 hours. It has been determined that digital game playing attitudes and aggression levels do not differ according to class level.

Bilimsel sorgulama, bilimsel okuryazar birey yetiştirmek için etkili bir stratejidir. Eğitimin her aşamasında kaynak olarak başvurulan ders kitaplarında bilimsel sorgulamanın nasıl sunulduğu bu amaca ulaşmada önemli rol oynar. Bu yüzden çalışmanın amacı, 9. sınıf fizik, kimya ve biyoloji ders kitaplarında yer alan etkinlikleri bilimsel sorgulama açısından değerlendirmektir. Çalışmamızda nitel araştırma yöntemlerinden doküman incelemesi kullanılmıştır. Araştırma materyali olarak incelemesi yapılacak olan kaynaklar Millî Eğitim Bakanlığı Talim Terbiye Kurulu Başkanlığı tarafından onaylanan ve 2017-2018 yılı eğitim-öğretim yılında okutulmak üzere genel liseler ve fen liselerindeki 9. sınıflara dağıtılan kitaplardan oluşmaktadır. Çalışmamıza konu olan 6 adet ders kitabındaki etkinlik ve deneyler Yang ve Liu (2016) tarafından geliştirilen sorgulama tabanlı görevler analiz envanterindeki boyutlara ve o boyutların içerdiği 22 alt boyuta göre incelenmiştir. Elde edilen veriler içerik analizi yapılarak değerlendirilmiştir. Tüm kitaplar incelendiğinde etkinliklerde en çok yer alan alt boyutlar; “Dersin amacına uygunluk’’ ve “Ana düşünce’’ boyutlarıdır. En az yer alan alt boyutlar; “Tahmin’’ ve “Hipotez kurma’’ boyutlarıdır. Ayrıca genel liseler için hazırlanan fizik ders kitabındaki etkinliklerin diğer branşlara göre daha çok alt boyut içerdiği görülmüştür. Bilimsel okuryazarlık hedefine ulaşmadaki önemi düşünüldüğünde, ders kitaplarındaki etkinliklerin daha planlı hazırlanması gerektiği sonucuna ulaşılmıştır., Scientific inquiry is an important instrument for increasing scientific literacy in society. How scientific inquiry is presented in textbooks plays an important role in achieving this goal. Therefore, the main objective of this study is to evaluate the nature and the quality of activities in Turkey’s 9th-grade biology, physics, and chemistry textbooks using a qualitative approach. Six textbooks the Republic of Turkey’s Ministry of National Education approved for ninth graders makes up the sample of this study. The Inquiry-based Tasks Analysis Inventory and its 22 sub-scales were developed by Yang and Liu (2016) and used for this study’s analysis. Content analysis was chosen as an appropriate methodological framework for this study. The findings indicate that the activities contain a limited number of sub-dimensions. When examining all the textbooks together, lesson objectives and core ideas are the most common sub-dimensions in the activities, and the least present sub-dimensions are predicting and formulating a hypothesis. The activities in the physics textbook for general high schools were additionally found to contain more sub-dimensions compared to the other textbooks. As one of the fundamental elements of scientific literacy, the representation of scientific inquiry in science textbooks was found to not be at the desired level.

Özellikle doğal kaynakların ve çevrenin sanayi sonrası toplumla birlikte hızla tahrip olması çevreye yönelik bakış açılarının değişmesine sebep olmuştur. Bu süreçle birlikte çevrenin ve doğal kaynakların korunması hususu hem hükümetler hem de sivil inisiyatiflerle desteklenmiştir.Araştırma kapsamında çevre korumaya yönelik yürütülen bir politika türü olarak Özel Çevre Koruma Bölgeleri ele alınmış ve Foça bölgesi örnek olarak seçilmiştir. Çevre koruma konusu Foça bölgesinde icra edilen turizm etkinlikleri ve yerel kalkınma boyutuyla analiz edilmiştir.Çalışma kapsamında alana ilişkin teorik bilgilere değinilmiş ve Foça'da kamu çalışanları, sivil toplum örgütleri ve turizm alanında faaliyet yürüten işletmecilerle görüşülmüştür. Elde edilen bulgular sosyolojik düzeyde değerlendirilmiştir. General perception towards environment have been changed especially in post-industrial society which caused rapid destruction of natural resourses and environment. With in this process, the matter of protection of natural resourses and environment encouraged by both goverments and non-goverment organizations.in scope of this research, Special Protection Zones which is used as a policy of protection of environment are investigated and Foça is choosed as an example region. The subject of protection of environment is analyzed with dimension of tourism activities and local development that is performed in Foça.İn this study theoretical knowledge has been explained and interviewed with public employees, non-goverment organizations and employees who are work in the field of tourism in Foça. Bu tezde, kalkınma politikalarının hidroelektrik santralleri üzerinden uygulanışı ve bu uygulamalar kapsamında insan-doğa ilişkisinde meydana gelen hiyerarşik durum irdelenmiştir. Bu bağlamda, konun ifadesinde çıkış noktası olarak HES'ler temel alınmıştır. Enerji ihtiyacının karşılanması amacıyla yapıldığı ve bu durumun gerek bölgesel gerekse de ulusal kalkınma noktasında zorunluluğa dayandığı şeklinde ifade edilen görüşlerin yanı sıra HES'lerin ekonomik, sosyal, ekolojik etkileri ve toplum nezdinde nasıl anlamlandırıldığı sorgulanmıştır.Bu araştırma, çalışmanın amacına uygun olarak, nitel araştırma tekniklerinin kullanıldığı, bilgilendirici bir araştırmadır. Araştırma kapsamında Tunceli iline gidilmiş ve amaçlı örnekleme ile seçilmiş bölgede yaşayan halk, STK ve kamu kurum ve kuruluşları ile görüşülmüştür. Görüşmelerde her bir grup için ayrı soru formu hazırlanmıştır. Veriler bu doğrultuda elde edilmiştir. Marxist yaklaşımın temel alındığı çalışmada, Marx'ın özellikle yabancılaşma ve metalaştırma kavramları çalışmanın ortaya konuşunda belirleyici özellik taşımaktadır.Verilerin analiz edilmesi sonucunda, HES'lerin gerek amaçları gerekse de kalkınma ile ilişkilendirilme şekillerinde sorunlar olduğu görülmüştür. HES'lerin kurulması ile ilgili karar aşamasında, HES'lerin etki alanlarında bulunan kişi ve kurumlarla yeterli koordinasyonun oluşturulamaması ise bir diğer sorun olarak ortaya çıkmaktadır. Tüm bunların yanında, HES'lerin sebep olduğu çevresel yıkım, durumdan etkilenen kesimlerin sınıfsal kimliklerindeki farklılıkları görünmez kılmış ve ortak bir muhalefet alanı yaratmıştır. Bu muhalefet sürdürülebilir bir kapitalizmin mümkün olmadığını ortaya koymuştur. In this thesis, performance of development policies through hydroelectric plants and hierarchical status occurs in human-nature relationship in that scope are examined. In this sense, hydroelectric plants (HES) are selected as the baseline in the expression of the subject as the starting point. Besides the statements like that they're established in order to meet the need of energy and this situation is based on obligation in the point of both regional and national development, economic, social, ecological effects are questioned along with the question that how these hydroelectric plants make sense of society.This research is an informative research in which qualitative research techniques are used for the purpose of the study. In the scope of the research, the province of Tunceli is visited and the people living in the region, NGO and state institutions and organizations' selected by purpose sampling are interviewed. In the interviews, separate questionnaire forms are prepared for each group. The data are collected in this direction.In the study based on Marxist approach, especially `alienation and `commodification concepts of Marx have a determinant characteristic in the presentation of the study.As a result of the analysis of the data, it's seen that there are problems both in purposes of hydroelectric plants and their association forms with development.In the decision phase related to the establishment of hydroelectric plants, not creating a sufficient coordination with persons and institutions in the influence areas of hydroelectric plants happens to be another problem. Apart from all these, environmental destruction caused by hydroelectric plants have made the differences in the class identities of the people, who are affected by the situation, invisible and created a common opposition area. This opposition shows that a sustainable capitalism is not possible. 255
