- Abstract
Adolescent dating and relationship violence is associated with health harms and is an important topic for sex education. School-based interventions addressing this have been effective in the USA, but schools in England confront pressures that might hinder implementation. We assessed the feasibility of, and contextual enablers/barriers to implementing Project Respect, a whole-school intervention. We conducted a pilot trial with process evaluation in six English secondary schools. Intervention comprised: training; policy-review; mapping and patrolling 'hotspots'; parent information; help-seeking app; and a curriculum (including student-led campaigns) targeting dating violence. Process evaluation included assessments of fidelity and interviews with the trainer and school staff. Schools delivered training and lessons partially or completely and made parent and app information available. Two schools conducted policy reviews; none patrolled hotspots or implemented campaigns. Implementation was strengthened where staff saw dating violence as a priority. Delivery was undermined where staff were insufficiently involved, lacked time for planning or struggled to timetable lessons, and where new school challenges undermined engagement. School-based health interventions must work to build staff buy-in and ensure they do not overburden schools. Dating and relationship violence might best be addressed in this context as a broader aspect of sex education.

Abstract\nPLAIN LANGUAGE SUMMARYThis article proposes practical steps for those new to collaborative research or those seeking to improve the nature of collaborative research partnerships. Questioning academia’s exaltation of publications and authorship as coveted assets, the authors critically analyze the often complex, contentious, and ambiguous environment of collaborative research and negotiating authorship. With respect to academic publishing and ethics of authorship, this article grapples with two critical questions: (1) What factors create ethical dilemmas in authorship in academia? (2) How can academics and researchers better navigate the complexities of collaborative research to sustain ethical authorship practices? A seven-phase process model is proposed to guide the collaborative research process to mitigate ambiguities that reside in the black box. The seven phases encompass problem settings and pre-negotiations, direction setting: action planning, social capital, re-negotiations, equity and inclusivity, implementation- the submission-revision cycle, and reflexivity. This theoretical analysis aims to offer a useful resource to promote enhanced collaborative practices, and the authors take an explicit advocacy and visionary perspective to promote transparency, accountability, and reciprocal trust, to shatter overarching issues such as oppressive, deceitful, and exploitative norms in academic authorship practices.Our main aim in this article is to translate the collaborative process into practical steps, whether this is a new experience or you are seeking to improve the nature of collaborative research partnerships. It combines research on authorship ethics and academic collaboration to examine how authors are credited in academic work. It questions the emphasis on publishing and authorship while also analyzing the complexities and challenges of collaborative research. The authors offer guidance for improving collaboration and ensuring fair recognition of author contributions. The authors present a seven-step model to guide more equitable authorship practices, emphasizing the importance of transparency, accountability, and trust in academic collaborations. It sought to use the TOBOGM trial findings to scope the issues involved with early screening, to inform future discussions over possible approaches for diagnosing gestational diabetes mellitus (GDM) in early pregnancy. Most delegates supported testing for early GDM using a one‐step 75 g oral glucose tolerance test approach with Canadian Diabetes Association criteria preferred, but highlighted the importance of considering resources, cost, consumer perspectives and equity in translating TOBOGM results into a clinical approach to screening for, and diagnosing, early GDM. 120. General Population Mortality Adjustment in Survival Extrapolation of Cancer Trials: Exploring Plausibility and Implications for Cost-Effectiveness Analyses in HER2-Positive Breast Cancer in Sweden.

Background: In economic evaluations of novel therapies, assessing lifetime effects based on trial data often necessitates survival extrapolation, with the choice of model affecting outcomes. The aim of this study was to assess accuracy and variability between alternative approaches to survival extrapolation. Methods: Data on HER2-positive breast cancer patients from the Swedish National Breast Cancer Register were used to fit standard parametric distribution (SPD) models and excess hazard (EH) models adjusting the survival projections based on general population mortality (GPM). Models were fitted using 6-y data for stage I and II, 4-y data for stage III, and 2-y data for stage IV cancer reflecting an early data cutoff while maintaining sufficient events for comparison of model estimates with actual long-term outcomes. We compared model projections of 15-y survival and restricted mean survival time (RMST) to 15-y registry data and explored the variability between models in extrapolations of long-term survival. Results: Among 11,224 patients compared with the observed registry 15-y RMST estimates across the disease stages, EH cure models provided the most accurate estimates in patients with stage I to III cancer, whereas EH models without cure most closely matched survival in patients with stage IV cancer, in which cure assumption was less plausible. The Akaike information criterion–averaged model projections varied as follows: −8.2% to +5.3% for SPD models, −4.9% to +5.2% for the EH model without a cure assumption, and −19.3% to −0.2% for the EH model with a cure assumption. EH models significantly reduced between-model variance in the predicted RMSTs over a 50-y time horizon compared with SPD models. Conclusions: EH models may be considered as alternatives to SPD models to produce more accurate and plausible survival extrapolation that accounts for general population mortality. Highlights: Excess hazard (EH) methods have been suggested as an approach to incorporate background mortality rates in economic evaluation using survival extrapolation. We highlight that EH models with or without a cure assumption can produce more accurate survival projections and significantly reduce between-model variability in comparison with standard parametric distribution models across cancer stages. EH models may be a preferred modeling method to reduce model uncertainty in health economic modeling since models that would otherwise have produced implausible extrapolations are constrained by the EH framework. Fishing has significant trophodynamic impacts on marine communities, including reductions in the mean trophic position (TP) of the ecosystem resulting from a decrease in the abundance and size of species and individuals with high TPs. This study demonstrates the erosion of fish TP, an additional process that results in lower TP of individuals of a given size, which may exacerbate the effects of fishing on the food web. A stable isotope approach based on the tRophicPosition Bayesian method was used to quantify the TP of 12 target marine species at a given length, and compare their TP between fishery-restricted areas and trawled areas. The results show a difference in the TP of six benthic and apical nekto-benthic predators, which feed in the median at about 0.5 TP lower in trawled areas. It appears that current 'fishing down marine food webs' analyses may underestimate the trophic effects of fishing. Accounting for changes in trophodynamics of individuals at a given size is important to detect indirect effects through food web interactions. The application of a trawling ban may lead to the restoration of lost trophic structure; however, trophic changes may occur more slowly than changes in biomass. When a mother is supported to breastfeed, the benefits for her mental health are significant. However, if pressured or unsupported, the opposite is true. This research examines mothers' breastfeeding experiences, exploring how perceived pressure can impact perinatal mental health. A sample of 501 respondents to a research questionnaire was explored using Reflexive Thematic Analysis. Three main themes identified were perceived pressure to breastfeed, perceived pressure not to breastfeed and mental health impact. The main findings were that mothers received conflicting advice from healthcare professionals, and pressures to feed in a certain way came from their support networks, as well as from their internal beliefs. Perceived pressures negatively impacted maternal mental health, while positive breastfeeding experiences benefitted mental health outcomes. [ABSTRACT FROM AUTHOR]

BackgroundThe erosion of the early mortality advantage of elective endovascular aneurysm repair (EVAR) compared with open repair of abdominal aortic aneurysm remains without a satisfactory explanation.MethodsAn individual-patient data meta-analysis of four multicentre randomized trials of EVAR versus open repair was conducted to a prespecified analysis plan, reporting on mortality, aneurysm-related mortality and reintervention.ResultsThe analysis included 2783 patients, with 14 245 person-years of follow-up (median 5·5 years). Early (0-6 months after randomization) mortality was lower in the EVAR groups (46 of 1393 versus 73 of 1390 deaths; pooled hazard ratio 0·61, 95 per cent c.i. 0·42 to 0·89; P = 0·010), primarily because 30-day operative mortality was lower in the EVAR groups (16 deaths versus 40 for open repair; pooled odds ratio 0·40, 95 per cent c.i. 0·22 to 0·74). Later (within 3 years) the survival curves converged, remaining converged to 8 years. Beyond 3 years, aneurysm-related mortality was significantly higher in the EVAR groups (19 deaths versus 3 for open repair; pooled hazard ratio 5·16, 1·49 to 17·89; P = 0·010). Patients with moderate renal dysfunction or previous coronary artery disease had no early survival advantage under EVAR. Those with peripheral artery disease had lower mortality under open repair (39 deaths versus 62 for EVAR; P = 0·022) in the period from 6 months to 4 years after randomization.ConclusionThe early survival advantage in the EVAR group, and its subsequent erosion, were confirmed. Over 5 years, patients of marginal fitness had no early survival advantage from EVAR compared with open repair. Aneurysm-related mortality and patients with low ankle : brachial pressure index contributed to the erosion of the early survival advantage for the EVAR group. Studying communities impacted by traumatic events is often costly, requires swift action to enter the field when disaster strikes, and may be invasive for some traumatized respondents. Typically, individuals are studied after the traumatic event with no baseline data against which to compare their postdisaster responses. Given these challenges, we used longitudinal Twitter data across 3 case studies to examine the impact of violence near or on college campuses in the communities of Isla Vista, CA, Flagstaff, AZ, and Roseburg, OR, compared with control communities, between 2014 and 2015. To identify users likely to live in each community, we sought Twitter accounts local to those communities and downloaded tweets of their respective followers. Tweets were then coded for the presence of event-related negative emotion words using a computerized text analysis method (Linguistic Inquiry and Word Count, LIWC). In Case Study 1, we observed an increase in postevent negative emotion expression among sampled followers after mass violence, and show how patterns of response appear differently based on the timeframe under scrutiny. In Case Study 2, we replicate the pattern of results among users in the control group from Case Study 1 after a campus shooting in that community killed 1 student. In Case Study 3, we replicate this pattern in another group of Twitter users likely to live in a community affected by a mass shooting. We discuss conducting trauma-related research using Twitter data and provide guidance to researchers interested in using Twitter to answer their own research questions in this domain. The study published in Diabetes Care explores the association between immediate treatment of early gestational diabetes mellitus (GDM) and breastfeeding outcomes. The research found that early diagnosis and management of GDM led to higher rates of breastfeeding initiation compared to deferred treatment. However, there was no significant difference in breastfeeding initiation within 1 hour of birth between the groups. The study suggests that early diagnosis and treatment of GDM may positively impact breastfeeding outcomes, but further research is needed to understand the long-term effects on both mothers and offspring. Objectives Acute myocardial infarction (AMI) case ascertainment improves for the UK general population using linked health data sets. Because care pathways for people with chronic kidney disease (CKD) change based on disease severity, AMI case ascertainment for these people may differ compared with the general population. We aimed to determine the association between CKD severity and AMI case ascertainment in two secondary care data sets, and the agreement in estimated glomerular filtration rate (eGFR) between the same data sets.Methods We used a cohort study design. Primary care records for people with CKD or risk factors for CKD, identified using the National CKD Audit (2015–2017), were linked to the Myocardial Ischaemia National Audit Project (MINAP, 2007–2017) and Hospital Episode Statistics (HES, 2007–2017) secondary care registries. People with an AMI recorded in either MINAP, HES or both were included in the study cohort. - Abstract
OBJECTIVE: To identify factors associated with neonatal respiratory distress (NRD) in early Gestational diabetes mellitus (eGDM). DESIGN: Nested case-control analysis of the TOBOGM trial. SETTING: Seventeen hospitals: Australia, Sweden, Austria and India. POPULATION: Pregnant women, <20 weeks' gestation, singleton, GDM risk factors. METHODS: Women with GDM risk factors completed an oral glucose tolerance test (OGTT) before 20 weeks: those with eGDM (WHO-2013 criteria) were randomised to immediate or deferred GDM treatment. Logistic regression compared pregnancies with/without NRD, and in pregnancies with NRD, those with/without high-dependency nursery admission for ≤24 h with those admitted for >24 h. Comparisons were adjusted for age, pre-pregnancy body mass index, ethnicity, smoking, primigravity, education and site. Adjusted odds ratios (95% CI) are reported. MAIN OUTCOME MEASURES: NRD definition: ≥4 h of respiratory support (supplemental oxygen or supported ventilation) postpartum. Respiratory distress syndrome (RDS): Supported ventilation and ≥24 h nursery stay. RESULTS: Ninety-nine (12.5%) of 793 infants had NRD; incidence halved (0.50, 0.31-0.79) if GDM treatment was started early. NRD was associated with Caesarean section (2.31, 1.42-3.76), large for gestational age (LGA) (1.83, 1.09-3.08) and shorter gestation (0.95, 0.93-0.97 per day longer). Among NRD infants, >24 h nursery-stay was associated with higher OGTT 1-h glucose (1.38, 1.08-1.76 per mmol/L). Fifteen (2.0%) infants had RDS. CONCLUSIONS: Identifying and treating eGDM reduces NRD risk. NRD is more likely with Caesarean section, LGA and shorter gestation.
