111. Evaluation of Unusual and Highly Aggressive Variant of Endometrium Cancer: Nonendometrioid Endometrium Carcinoma of the Uterus

- Abstract
Aims To evaluate the survival and treatment outcomes of patients with nonendometrioid endometrium carcinoma after postoperative radiotherapy.Methods The records of 94 patients treated with postoperative radiotherapy (RT) between January 2005 and December 2011 were retrospectively reviewed. Postoperative RT was delivered with a dose of 45-50.4 Gy with 1.8 Gy daily fractions and brachytherapy was added to external RT for 62 patients with a dose of 3 × 6 Gy. Median follow-up time was 35 months (range 6-95 months).Results Median age was 63 years (range 43-83 years) and lymph node metastasis (LNM) was positive in 15 (16.0%) patients. The stage distribution of the patients was as follows: stage I, 58 (61.7%); stage II, 16 (17.0%); stage III, 18 (19.1%); stage IV, 2 (2.2%). Five-year locoregional control (LRC), disease-free survival (DFS), and overall survival (OS) rates were 92.3%, 68.2%, and 78.6%, respectively. In univariable analysis, it was determined that the factors affecting OS rates were stage (p = 0.003), presence of LNM (p = 0.003), and presence of lymphovascular space invasion (LVSI) (p = 0.007); factors affecting DFS rates were stage (p = 0.019), presence of LVSI (p = 0.002), and having LNM (p = 0.049); and the factor affecting LRC rates was tumor size (>5 cm) (p = 0.011). In subgroup analyses, among all stage I patients, the DFS rates were lower for those with histologic grade 3 tumors and more than ½ myometrial invasion.Conclusions Due to its rarity, the prognostic factors and outcomes of nonendometioid endometrium carcinoma are not fully understood. In our analysis, stage, LNM, and presence of LVSI were found to be the most important prognostic factors. In order to tailor the optimal treatment strategy, prospective studies are needed.

Amaç: T1N0 glottik larenks kanseri tedavisinde çeşitli radyoterapi teknikleri arasında karotis arterlerinin maruz kaldıkları dozlar karşılaştırıldı.Hastalar ve Yöntemler: Bilgisayarlı tomografi BT kullanılarak beş hasta simüle edildi. T1N0 glottik larenks kanseri için klinik KHV ve planlanan hedef volümler PHV oluşturuldu. Karotis arterleri ve spinal kord için planlanan risk volümleri PRV çizildi. Her hasta için iki boyutlu radyoterapi 2DRT , üç boyutlu konformal radyoterapi 3DCRT ve basit yoğunluk ayarlı radyoterapi IMRT şeklinde üç planlama yapıldı. Planlanan hedef volümlerin %95’den fazlası tanımlanan dozu 2.25 Gy günlük fraksiyon dozundan 62.25 Gy toplam doz aldı.Bulgular: Üçü sağ ikisi sol vokal kord yerleşimli beş T1 glottik kanserli olgunun tedavi planları karşılaştırıldığında, karotislerin koruması en iyi basit karotis koruyucu IMRT tekniği ile sağlandı. 2DRT, 3DCRT ve IMRT tekniklerine ait karotis ortalama V35, V50 ve V63 değerleri sırasıyla; %70, %47, %35; %55, %15, %5 ve %28, %6, %0 bulundu. V35, V50 ve V63 arasında yapılan istatistiksel karşılaştırmada, 2DRT ve IMRT için anlamlı değerler saptandı. Spinal kord dozu hiçbir planda 45 Gy’i aşmadı. Her üç planlama tekniğinde uygunluk indeksi arasında anlamlı fark saptanmadı ve homojen doz dağılımı elde edildi.Sonuç: Erken evre glottik larenks kanserli hastalarda karotis arterlerinin maruz kaldığı radyasyon dozunu IMRT’nin azaltabildiğine şüphe yoktur, Objectives: This study aims to compare the carotid artery doses applied with various radiotherapy techniques for the treatment of T1N0 glottic laryngeal cancer. Patients and Methods: Five patients were simulated with using computed tomography CT . Clinical CTV and planning target volumes PTV were created for T1N0 glottic laryngeal cancer. Planning risk volumes PRV were constructed for carotid arteries and spinal cord. Three irradiation planning, two dimension radiation therapy 2DRT , three dimension conformal radiation therapy 3DCRT , intensity modulated radiation therapy IMRT were done for each patient. Over 95% of planned target volumes were loaded with predetermined dose a total of 62.25 Gy of 2.25 Gy daily dose . Results: The comparison of the treatment planning of five T1 glottic laryngeal cancer, three involving the right vocal cord and two involving the left vocal cord, the technique of IMRT planning was provided the best carotid-sparing doses. Mean carotid V35, V50, and V63 values including 2DRT, 3DCRT, and IMRT were 70%, 47%, 35%; 55%, 15%, 5% and 28%, 6%, 0%, respectively. The statistical comparison of V35, V50 and V63 revealed significant values for 2DRT and IMRT. Dose of spinal cord did not exceed 45 Gy for any of radiation treatment planning. Between the three techniques, there was no significant difference in terms of conformity index and dose distribution was homogenous with all techniques. Conclusion: It is obvious that IMRT planning technique can decrease the carotid artery radiation doses in early stage glottic laryngeal cancer.

Araştırmada Leonardo da Vinci'ye ait sanatsal ve bilimsel veriler araştırılmış, sanatçının bilimsel çalışmalarının sanatına ne gibi etkiler yaptığı incelenmeye çalışılmıştır. Araştırma sonucunda Leonardo da Vinci'nin sanatsal çalışmalarının yanı sıra matematik, geometri, botanik, mühendislik, anatomi gibi pek çok bilimsel alanda çalışmalar yaptığı ve bu çalışmaların resimlerinde etkileri görülmektedir. Anatomi alanında ki bilgiler sağlam temelli figürler çizmesine, matematik ve geometri gibi alandaki çalışmaları ise uzamsal ve perspektif olarak derin çalışmalar yapmasına temel hazırlamıştır., The general purpose of this research is how Leonardo da Vinci's curiosity towards science affected his art. The research has been prepared using literature scanning techniqueIn the research, artistic and scientific data of Leonardo da Vinci were investigated, and the effects of the artist's scientific works on his art were tried to be examined. As a result of the research, besides the artistic works of Leonardo da Vinci, he worked in many scientific fields such as mathematics, geometry, botany, engineering, and anatomy, and the effects of these studies are seen in his paintings. The knowledge in the field of anatomy has laid the basis for drawing solid-based figures, and studies in the field such as mathematics and geometry have provided the basis for deep studies in spatial and perspective.

ÖZETTarihi Orta Asya Türklerine kadar dayanan halı, kilim ve dokuma ürünleri, arkeologlar tarafından yapılan kazı çalışmalarında, çoğunlukla Türklerin bulunduğu yerleşim yerlerinde ortaya çıkarılmıştır. Dünyada bilinen en eski halı, Orta Asya'da Altay Dağlarında bulunan Pazırık halısıdır. St. Petersburg'taki Hermitage Müzesi'nde orijinal haliyle korunan halı, Türklerin Kavimler Göçü yıllarındaki göçebe alışkanlıklarını, yaşam şekillerini simgeleyen önemli bir kaynak olmuştur. Pazırık kurganının bulunmasıyla gün yüzüne çıkan halı ve dokuma kültürü, yüzyıllar boyunca insan hayatını tanıtmıştır. Halı ve kilimler dönemin ihtiyaçlarını karşılamakla kalmamış, insana ait birçok duygunun sembol ifadesi olmuştur. Türk halı ve kilimleri ilk defa Avrupalı ressamların resimlerinde görüldükten sonra yabancı sanat tarihçileri tarafından tarihsel olarak sınıflandırılmış, incelenmeye başlanmıştır.Tanzimat ve I. Meşrutiyet döneminde eğitim alanında yapılan yenilikler ve çalışmalar Sanayi-i Nefise'nin kurulmasını sağlamıştır. Bu dönemlerde sanat eğitimi Batı eğitim anlayışıyla verilir olmuş, yurt dışından ressamlar getirtilmiş, yurt dışına resim eğitimi almaları için öğrenci gönderilmiştir. Gelişen siyasi olaylar, dönemin sosyal ve ekonomik yapısı, Türk resim sanatına yön verecek olan yeni ressam kuşağının etkin olacağı bir süreç başlatmıştır. Çağdaş Türk resminde milli değerlerden hareket ederek yeni sanat düşünceleri oluşmuş, 1940'lardan itibaren bu düşünce ve uygulamalar yaygınlaşmış, 1950'li yıllarda milli değerlerden, kültürel varlıklardan, geleneklerden yararlanma fikri önem kazanmıştır. Osman Hamdi, Şeref Akdik, Malik Aksel, Turgut Zaim, Bedri Rahmi Eyüboğlu, Fahrelnisa Zeid, Nurullah Berk, Sabri Fettah Berkel vb. ressamların eserlerinde geleneksel öğelerin çağdaş bir yorumla biçimsel olarak, kimi resimlerde ise gerçekçi betimlemeler ile bu fikirleri yansıtmışlardır. The oldest known carpet in the world is the Pazyryk Carpet found in Altay Mountains in Central Asia. This carpet, which is still kept in its original form in the Hermitage Museum in St. Petersburg, is an important source symbolising nomadic habits and life style in the time of the migration of Turkic tribes. Carpet and weaving culture, which came to light with the discovery of Pazyryk Cairn, has introduced human life for centuries. Carpets and rugs not only met the demands of the era, but also expressed so many human feelings. Only after Turkish carpets and rugs had been depicted in the works of European artists for the first time did foreign art historians begin to study and classify them. With the foundation of Sanayi-i Nefise, one of the breakthroughs in educational field, they started art training in Western style and foreign artists were invited and trainees were sent abroad. The emerging political developments and the socio-economic factors of the period started a new process, in which a new generation of artists would dominate the art of painting. New artistic approaches in modern Turkish painting were formed, stemming from national values. From 1940s onwards, these ideas and practices became widespread, and benefiting from national values, cultural assets and traditions became popular in 1950s. Osman Hamdi, Şeref Akdik, Malik Aksel, Turgut Zaim, Bedri Rahmi Eyüboğlu, Fahrelnisa Zeid, Nurullah Berk, Sabri Fettah Berkel are among the artists who reflected traditional elements in a modern style and sometimes depicted these ideas in realistic ways. Mevlâna, bu sevgiyle yoğrulmuş, ulaştığı cevapları paylaşarak, insanlığa nefis yolculuklarında ışık olmuş, öğretileriyle yol göstermiş, engin hoşgörüsü ile herkesi kucaklamış ve bu sayede Mevlevilik felsefesinin de kaynağı olmuştur. Bu sistemi bütün olarak ele alacak olursak, baş hareketlerinden ayak duruşlarına, ellerin konumundan dönüş şekline kısacası her hareketin, giyilen her kıyafetin sembolize edilmiş derin anlamları bulunmaktadır. Temelinde sevgi olan, merakları cezbeden bu felsefe, dilden dile, kulaktan kulağa, gönülden gönüle ve kültürden kültüre geçerek başka diyarlara da yayılmıştır. Gerek kişisel meraktan, gerekse ülkeleri tarafından çeşitli görevlendirmelerle keşfe gelen batılı sanatçıların Osmanlı kültürünü, tarihe ışık tutacak önemli olayları, doğu mistisizmini, yapıları, doğal güzellikleri, antik kentleri, gündelik yaşamı ve her tabakaya mensup insanları resmettikleri çalışmaları önemli bir çok kaynakta kullanılmıştır. Mevlâna has been an important guide for the ages to people who have lived their inner journeys with his wisdom, eternal love for the creator, unconditional surrender, world and beyond vision and tolerance. To love the creature is because of the creator, to see, to hear and to feel the creator in every creature. The basis is Allah, love of Allah. Hz. Mevlana has formed by this love, shared the answers he reached, become a light in their inner journeys to mankind, guided them by his teachings, embraced everyone with his wide tolerance and thus became the source of the Mevlevi philosophy. After his death, by those who followed him, the Mevlevi philosophy has been based on a systematic structure with procedures and rules, music and ceremony. If we consider this system as a whole, there are symbolized deep meanings of every movement, every dress that is worn, from the head movements to the footsteps, from the position of the hands to the turns. This philosophy which is the love on its basis, which attracts curiosities, has spread to other lands from tongue to tongue, ear to ear, heart to heart and culture to culture. Western artists 'who came to explore the Ottoman culture with both the personal curiosity and various entrusting by their countries' used important events that will shed light on the history, eastern mysticism, structures, natural beauties, ancient cities, daily life and people in their artworks which were used on lots of important sources. During their travels, they were also greatly influenced by the Sema Rituals, which many of them described the ritual as a relaxing and peaceful ceremony. They also included Mevlevi philosophy to their memoirs, travel books, books, paintings, and much more. Thanks to Western artists and travelers, Mevlâna and Mevlevi have spread all over the world and will continue to be.

New artistic approaches in modern Turkish painting were formed, stemming from national values. From 1940s onwards, these ideas and practices became widespread, and benefiting from national values, cultural assets and traditions became popular in 1950s. Osman Hamdi, Şeref Akdik, Malik Aksel, Turgut Zaim, Bedri Rahmi Eyüboğlu, Fahrelnisa Zeid, Nurullah Berk, Sabri Fettah Berkel are among the artists who reflected traditional elements in a modern style and sometimes depicted these ideas in realistic ways. Artists who produce new plastic and esthetic solutions in search of national painting style in Turkish art, inspired by ancient Turkish arts, provided the inspiration for contemporary artists.

117. Trends in the incidence and overall survival of multiple primary cancers in Turkey.

- Abstract
Purpose: Multiple primary malignant tumors (MPCs) are defined as at least two histologically distinct malignancies in one individual. We aimed to present the risk of developing multiple primary cancers and to give information about the periods and prediction data regarding survival outcome for MPCs., Methods: All patients with MPCs between 1992-2017 were included in this study. Patients were compared in terms of the primary cancer number, age at diagnosis of first primary cancer, gender, time interval after first cancer detected, cancer types seen mostly together, and the rate of MPCs during the years., Results: The total number of included patients was 117,139. The proportion of patients with MPCs during the follow-up period was 4.95% (n=5,796). Eighteen percent of the cases were synchronous and 82% metachronous. MPCs were most commonly detected in the gastrointestinal tract and were detected more commonly in men than in women (p<0.001). Patients receiving radiotherapy (RT) (p<0.001) and chemotherapy (CT) (p <0.001) had more MPCs than those who never received RT or CT. Survival in MPCs was worse than in single primary cancers. The 5- and 10-year survival rates were 28.1%, and 12.4%, respectively., Conclusion: We identified approximately 5% MPCs among 117,139 patients in our database. MPCs are more common at 60-69 years in males and at 50-59 years in females. In terms of the risk of MPCs, we should be aware of the cancers that have risk factors, habits, and genetic features commonly affecting primary cancers.

118. Can carotid-sparing radiotherapy approaches replace with conventional techniques for the patients with T1 glottic larynx cancer?

- Abstract
Objectives: This study aims to compare the carotid artery doses applied with various radiotherapy techniques for the treatment of T1N0 glottic larynx cancer., Patients and Methods: Five patients were simulated with using computed tomography (CT). Clinical (CTV) and planning target volumes (PTV) were created for T1N0 glottic larynx cancer. Planning risk volumes (PRV) were constructed for carotid arteries and spinal cord. Three irradiation planning, opposed lateral box field (OLBF), three dimension conformal radiation therapy (3DCRT), intensity modulated radiation therapy (IMRT) were done for each patient. Over 95% of planned target volumes were loaded with predetermined dose (a total of 62.25 Gy of 2.25 Gy daily dose)., Results: The comparison of the treatment planning of five T1 glottic larynx cancer, three involving the right vocal cord and two involving the left vocal cord, the technique of IMRT planning was provided the best carotid-sparing doses. Mean carotid V35, V50, and V63 values including OLBF, 3DCRT, and IMRT were 70%, 47%, 35%; 55%, 15%, 5% and 28%, 6%, 0%, respectively. The statistical comparison of V35, V50 and V63 revealed significant values for OLBF and IMRT. Dose of spinal cord did not exceed 45 Gy for any of radiation treatment planning. Between the three techniques, there was no significant difference in terms of conformity index and dose distribution was homogenous with all techniques., Conclusion: It is obvious that IMRT planning technique can decrease the carotid artery radiation doses in early stage glottic larynx cancer.
