Importance: Reducing the use of coercion in mental health care is crucial from a human rights and public health perspective. Psychiatric advance directives (PADs) are promising tools that may reduce compulsory admissions. Assessments of PADs have included facilitation by health care agents but not facilitation by peer workers.Objective: To determine the efficacy of PADs facilitated by peer workers (PW-PAD) in people with mental disorders.Design, Setting, and Participants: A multicenter randomized clinical trial was conducted in 7 French mental health facilities. Adults with a DSM-5 diagnosis of schizophrenia, bipolar I disorder, or schizoaffective disorder who had a compulsory admission in the past 12 months and the capacity to consent were enrolled between January 2019 and June 2020 and followed up for 12 months.Interventions: The PW-PAD group was invited to fill out a PAD form and meet a peer worker who was trained to assist in completing and sharing the form with relatives and psychiatrists.Main Outcomes and Measures: The primary outcome was the rate of compulsory admission at 12 months after randomization. The overall psychiatric admission rate, therapeutic alliance, quality of life, mental health symptoms, empowerment, and recovery outcomes were also investigated.Results: Among 394 allocated participants (median age, 39 years; 39.3% female; 45% with schizophrenia, 36% bipolar I disorder, and 19% schizoaffective disorder), 196 were assigned to the PW-PAD group and 198 to the control group. In the PW-PAD group, 27.0% had compulsory admissions compared with 39.9% in the control group (risk difference, -0.13; 95% CI, -0.22 to -0.04; P = .007). No significant differences were found in the rate of overall admissions, therapeutic alliance score, and quality of life. Participants in the PW-PAD group exhibited fewer symptoms (effect size, -0.20; 95% CI, -0.40 to 0.00), greater empowerment (effect size, 0.30; 95% CI, 0.10 to 0.50), and a higher recovery score (effect size, 0.44; 95% CI, 0.24 to 0.65), compared with those in the control group.Conclusions and Relevance: Peer worker-facilitated PADs are effective in decreasing compulsory hospital admissions and increasing some mental health outcomes (self-perceived symptoms, empowerment, and recovery). Involving peer workers in the completion of PADs supports the current shift of mental health care from substitute decision-making to supported decision-making.Trial Registration: ClinicalTrials.gov Identifier: NCT03630822. Hospital professional violence is defined as hostile and aggressive behavior exerted by health professionals on other health professionals. No quantitative study has been carried out to date on French hospital professional violence among young physicians, while recent qualitative studies have suggested a potential high frequency. The main objective was to determine the prevalence of exposure of young doctors to hospital violence. The secondary objective was to determine their characteristics and consequences as well as to determine if students and young physicians (resident and young MD) differed.The study was a national cross-sectional observational epidemiological study that included 4th-year medical students and young physicians (MD for less than 2 years). Thirty-seven French faculties of medicine were contacted for email recruitment of participants. Social networks were used to increase the visibility of the study. The questionnaire was developed after exhaustive review of the international literature dealing with professional violence in hospitals, its characteristics and its consequences in terms of mental health, addiction, personal and professional life. The report of these events was also explored.In total, 2003 participants have been included. More than nine out of ten participants were exposed to hospital violence at least once and nearly 42% to moral harassment as defined by the French law. This violence does not differ between the students and the residents/young MDs, suggesting that working time in the hospital does not seem to affect this risk. Nearly 80 % of interns and young MDs reported working more than the legal time. The perpetrators of violence include in almost all cases at least one man, often a senior doctor, but students reported the presence of at least one woman among the perpetrators in ¾ of cases. The victims are as often men as women. Compared to the undergraduate medical students, residents and young MDs more frequently reported poor outcomes on their mental health, addictive behavior, personal and professional lives. The majority of victims reported the event to a peer but fewer than 10% to the head of the department, a professor or an instance that could have acted. In almost all cases, participants reported the continuation of abusive behavior after the event. In total, 42% of students think that this is simple part of medical studies that they must endure.These results suggest the need to develop specific information and prevention programs for professional hospital violence in France.

Despite clues indicating high Bullying at the Work Place (BWP) rates in French hospitals, there has been no quantitative study so far. To determine the prevalence of repeated BWP in a national sample of French young physicians; its risk factors, and the mental health consequences of BWP. The study is a cross-sectional observational epidemiological national study addressed to young physicians. The online internet anonymous questionnaire was elaborated according to previous studies exploring BWP. In addition, we explored the quality of initial training. BWP was defined according to the French legal definition. Mental health was assessed by Hamilton Anxiety and Depression scale, psychotropic drug consumption and psychotherapy follow-up. A Structured Equation Modeling (SEM) was carried out to confirm our theoretical model. 2003 participants of the 37 French medical faculties were included. At least one history of BWP was identified in 41.7% of the participants. The SEM model showed good fit (RMSEA = 0.025, CFI = 0.93, TLI = 0.92, WRMR = 1.285). In the SEM model, BWP was associated with age and number of monthly night shifts and weekly worked hours. Obstetric gynecology, psychiatry, surgery, and medical specialties and low-quality initial training were associated with higher risk of BWP. BWP was associated with increased anxiety and depressive symptoms, daily antidepressant and anxiolytic consumption, and psychotherapy follow-up. Decreasing worked hours and night shifts and improving the quality of the initial training may help preventing BWP among medical students and young physicians. Obstetric gynecology, surgical and medical specialties, and psychiatry should be targeted with a focus on developing prevention programs.

115. Dissociation between the clinical course and chest imaging in severe COVID-19 pneumonia: A series of five cases.

• Radiographic and clinical course can diverge in a number of Covid-19 patients. • Hypoxemia during Covid-19 is not only imputable to lung parenchyma lesions. • Covid-19 induces vascular disorders that may result in an intrapulmonary shunt. Although an RT-PCR test is the "gold standard" tool for diagnosing an infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), chest imaging can be used to support a diagnosis of coronavirus disease 2019 (COVID-19) – albeit with fairly low specificity. However, if the chest imaging findings do not faithfully reflect the patient's clinical course, one can question the rationale for relying on these imaging data in the diagnosis of COVID-19. To compare clinical courses with changes over time in chest imaging findings among patients admitted to an ICU for severe COVID-19 pneumonia. We retrospectively reviewed the medical charts of all adult patients admitted to our intensive care unit (ICU) between March 1, 2020, and April 15, 2020, for a severe COVID-19 lung infection and who had a positive RT-PCR test. Changes in clinical, laboratory and radiological variables were compared, and patients with discordant changes over time (e.g. a clinical improvement with stable or worse radiological findings) were analyzed further. Of the 46 included patients, 5 showed an improvement in their clinical status but not in their chest imaging findings. On admission to the ICU, three of the five were mechanically ventilated and the two others received high-flow oxygen therapy or a non-rebreather mask. Even though the five patients' radiological findings worsened or remained stable, the mean ± standard deviation partial pressure of arterial oxygen to the fraction of inspired oxygen (PaO 2 :FiO 2) ratio increased significantly in all cases (from 113.2 ± 59.7 mmHg at admission to 259.8 ± 59.7 mmHg at a follow-up evaluation; p=0.043). Our results suggest that in cases of clinical improvement with worsened or stable chest imaging variables, the PaO2:FiO2 ratio might be a good marker of the resolution of COVID-19-specific pulmonary vascular insult. Importance: Heterogeneous evidence exists for the association between COVID-19 and the clinical outcomes of patients with mental health disorders. It remains unknown whether patients with COVID-19 and mental health disorders are at increased risk of mortality and should thus be targeted as a high-risk population for severe forms of COVID-19.Objective: To determine whether patients with mental health disorders were at increased risk of COVID-19 mortality compared with patients without mental health disorders.Data Sources: For this systematic review and meta-analysis, MEDLINE, Web of Science, and Google Scholar were searched from inception to February 12, 2021. Bibliographies were also searched, and the corresponding authors were directly contacted. The search paradigm was based on the following combination: (mental, major[MeSH terms]) AND (COVID-19 mortality[MeSH terms]). To ensure exhaustivity, the term mental was replaced by psychiatric, schizophrenia, psychotic, bipolar disorder, mood disorders, major depressive disorder, anxiety disorder, personality disorder, eating disorder, alcohol abuse, alcohol misuse, substance abuse, and substance misuse.Study Selection: Eligible studies were population-based cohort studies of all patients with identified COVID-19 exploring the association between mental health disorders and mortality.Data Extraction and Synthesis: Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) reporting guideline was used for abstracting data and assessing data quality and validity. This systematic review is registered with PROSPERO.Main Outcomes and Measures: Pooled crude and adjusted odds ratios (ORs) for the association of mental health disorders with mortality were calculated using a 3-level random-effects (study/country) approach with a hierarchical structure to assess effect size dependency.Results: In total, 16 population-based cohort studies (data from medico-administrative health or electronic/medical records databases) across 7 countries (1 from Denmark, 2 from France, 1 from Israel, 3 from South Korea, 1 from Spain, 1 from the UK, and 7 from the US) and 19 086 patients with mental health disorders were included. The studies covered December 2019 to July 2020, were of good quality, and no publication bias was identified. COVID-19 mortality was associated with an increased risk among patients with mental health disorders compared with patients without mental health disorders according to both pooled crude OR (1.75 [95% CI, 1.40-2.20]; P < .05) and adjusted OR (1.38 [95% CI, 1.15-1.65]; P < .05). The patients with severe mental health disorders had the highest ORs for risk of mortality (crude OR: 2.26 [95% CI, 1.18-4.31]; adjusted OR: 1.67 [95% CI, 1.02-2.73]).Conclusions and Relevance: In this systematic review and meta-analysis of 16 observational studies in 7 countries, mental health disorders were associated with increased COVID-19-related mortality. Thus, patients with mental health disorders should have been targeted as a high-risk population for severe forms of COVID-19, requiring enhanced preventive and disease management strategies. Future studies should more accurately evaluate the risk for patients with each mental health disorder. The goal of the treatment of a disease has moved from treating organs and diseases through symptoms, biological parameters and imaging towards treating a human being as a whole. The treatments should deliver benefits that patients can personally perceive. However, the patient's perspective does not always match the one of those surrounding them. Illustratively, patients' symptom assessments are more predictable for daily health status, whereas clinicians' symptom measurements are more related to clinical outcomes. The term, patient-reported outcomes (PROs), includes any data that are reported directly by the patient without an intermediary, such as a family member or a healthcare professional. The use of PROs in oncology trials is increasing and the U.S. Food and Drug Administration has published guidelines on the review and evaluation of PROs. However, while PROs are increasingly used in clinical trials, they are rarely used in daily clinical practice. Further, healthcare payers are concerned with issues related to relevance, quality, and interpretability of these outcomes.

Although several scores exist to assess psoriasis severity, most have marked limitations that rule out their use in routine clinical practice. A new score, the Simplified Psoriasis Index (SPI), has recently been developed and validated in adults in Britain for such use. It has separate components for current severity (SPI-s), psychosocial impact (SPI-p) and past history and interventions (SPI-p), and it is suitable for either professional assessment or patient self-assessment. The aim of this work was to produce a validated translation of SPI into French (as spoken in France).The index was translated and validated using a strict methodology comprising respectively five and eight phases for the professional (proSPI) and self-administered instruments (saSPI). Translation of the saSPI instrument also involved a cognitive debriefing with five psoriasis patients.Linguistic discrepancies and subtle differences of meaning arising during the process were closely examined. The developer of the instrument ensured conceptual accuracy. A panel of health experts guaranteed that medical terms were correctly translated. Five patients with plaque psoriasis (two female and three male of median age 45 years [range: 31-78]) tested the SPI-p version during cognitive interviews and found the questionnaire clear and easy to understand.Validated French translations of both SPI instruments are now available for use in routine clinical practice. Further investigations are currently underway to validate the psychometric properties of the instrument.

Dans le cadre de l’évaluation des technologies de santé, les mesures rapportées par les patients, les PROMs (patient-reported outcome measures), sont désormais des critères d’évaluation attendus par les agences d’évaluation au même titre que des critères cliniques habituels évalués par un observateur. Les instruments PROMs mesurent tous les domaines de perception des patients liés à leur état de santé : symptômes, activités quotidiennes (fonction physique, sommeil…), différents aspects de la qualité de vie liée à l’état de santé (QdV), observance, évaluation globale d’un changement d’état. Les PROMs servent à la fois comme : 1/critère d’efficacité principal ou secondaire ; 2/comme critère de tolérance pour compléter les données de vigilance rapportées par les cliniciens. Leur mesure doit faire l’objet d’une rigueur méthodologique identique à celle des autres critères d’évaluation mesurés par un observateur. Les échelles doivent être validées, adaptées à l’objectif et préférentiellement spécifiques. Les PROMs, en plus des mesures usuelles de qualité de vie, sont pris en compte dans les évaluations de la HAS, même si leur impact sur les conclusions est difficile à individualiser. Le CEPS tiendra compte indirectement des PROMs dans la fixation du prix ou du tarif uniquement s’ils ont contribué à la cotation de l’ASA/ASMR par la Commission ad hoc de la HAS. Le groupe de travail a formulé trois recommandations qui ont pour vocation de favoriser la mise en œuvre des mesures rapportées par les patients : (1) mieux informer l’ensemble des parties prenantes d’un dossier d’évaluation technologique, (2) systématiser le recueil des PROMs pour l’évaluation des produits de santé, (3) améliorer la qualité des dossiers en ayant recours à des outils pertinents et validés.

Resume Les patients qui survivent a cancer de l’enfance sont exposes a des effets secondaires tardifs. Dans ce contexte, la structuration de cohortes prospectives de grande envergure est un enjeu essentiel pour ameliorer a la fois la prise en charge medicale des patients et nos connaissances sur le sujet. Le programme L.E.A. (pour leucemie enfants adolescents), dedie aux patients gueris d’une leucemie de l’enfance, repose sur la constitution d’une cohorte prospective multicentrique dans 13 centres de cancerologie pediatrique francais. Les donnees sont recueillies au cours de consultations medicales specifiques et comportent des informations sur la maladie initiale et ses traitements, les sequelles physiques, la qualite de vie, l’insertion socioprofessionnelle et les relations au systeme de soins. A la fin 2012, le programme avait permis de prendre en charge sur le long terme 2162 patients. La moitie d’entre eux avaient ete evalues au moins deux fois et le nombre total d’evaluations etait de 3519. Le recul moyen par rapport au diagnostic etait de 12 ans. Introduction Le psoriasis en plaques s’accompagne souvent de lesions dans des localisations critiques aggravant le retentissement de la maladie notamment sur la qualite de vie des patients. Parmi elles, les localisations genitales, du cuir chevelu, des ongles et palmoplantaire sont frequentes et difficiles a traiter. Nous rapportons ici les resultats d’une etude utilisant la version francaise de l’Indice Simplifie du Psoriasis (SPI), outil permettant l’evaluation de la reponse a un traitement, pour mesurer l’evolution de ces localisations critiques sous secukinumab (SK) Materiel et methodes L’etude IPSI-PSO (EUDRACT 2014-003666-25) etait prospective, non comparative, multicentrique. Les patients devaient avoir un psoriasis modere a severe (PASI ≥ 12) depuis au moins 6 mois. Le secukinumab (SK 300 mg) etait administre toutes les semaines jusqu’a la 4e semaine (S4) puis toutes les 4 semaines jusqu’a S48. L’outil SPI donne un score global de severite du psoriasis reposant en partie sur 10 questions portant chacune sur une region. Nous presentons ici l’evolution sous SK des patients ayant plus specifiquement une atteinte de 4 regions : « anogenitale », « cuir chevelu », « mains et ongles » et « pieds et ongles ». Resultats Sur 120 patients inclus, 73 avaient une atteinte anogenitale, 90 du cuir chevelu, 93 des mains et ongles et 91 une atteinte des ongles et pieds evolutive a l’inclusion. Un patient pouvait declarer plusieurs atteintes associees a son psoriasis. A S16, le pourcentage de patients ayant une atteinte de la region « anogenitale » diminuait de 79 % par rapport a l’inclusion, celui des patients concernes par la region « cuir chevelu » diminuait de 83 %, ceux des patients avec une atteinte « mains et ongles » et « pieds et ongles » diminuait de 79 % et de 74 % respectivement. Les taux de reponse pour chacune de ces 4 localisations anatomiques se maintenaient jusqu’a S52. L’evenement indesirable le plus frequent etait la rhinopharyngite (10,8 %). Deux deces sont survenus (traumatisme crânien et lymphome cutane) non lies au traitement de l’etude ( Fig. 1 ). Discussion On note une reponse similaire au traitement par SK dans les 4 localisations : genitale, cuir chevelu, mains et pieds, qui se maintient tout au long de l’etude. Ceci corrobore les resultats obtenus dans les etudes precedentes du SK dediees aux localisations particulieres du psoriasis qui utilisaient des questionnaires specifiques pour chacune de ces localisations (GESTURE NCT01806597 , TRANSFIGURE NCT01807520 et SCALP NCT02267135 ). Conclusion Dans cette etude, le SK montre un benefice dans le traitement des localisations particulieres du psoriasis. La qualite du questionnaire SPI, qui en outre prend en compte et discrimine les localisations particulieres du psoriasis dans sa composante severite, en fait un outil de choix en pratique courante pour les dermatologues. Il merite d’etre diffuse en pratique courante.

Summary Purpose To assess the capabilities of a velocity ratio > 3 for the diagnosis of Budd-Chiari syndrome (BCS) in children after split liver transplantation using Doppler ultrasonography (DUS). Materials and methods A total of 28 children who underwent liver transplantation using a split procedure were included. There were 11 boys and 17 girls with a mean age of 3.8 years (range: 0.7–12 years). Velocity ratio between blood velocity upstream of the anastomosis and that at the level of the inferior vena cava anastomosis was calculated. Sensitivity, specificity and accuracy of DUS for the diagnosis of BCS were estimated using a velocity ratio > 3. Results Eight children (8/28; 29%) had BCS and 20 (20/28; 71%) did not have BCS using the standard of reference. A velocity ratio > 3 on DUS yielded 88% sensitivity (95% CI: 53–98%), 80% specificity (95% CI: 58–92%) and 82% accuracy (95% CI: 64–92%) for the diagnosis of BCS. Conclusion A velocity ratio > 3 on DUS is a reliable finding for the diagnosis of BCS in children after split liver transplantation.

Resume Introduction La Nouvelle-Caledonie est situee dans le Pacifique Sud-ouest, a 20 000 km de la France. Dans ce pays d’outre-mer (POM), la prevalence du tabagisme est tres elevee, estimee a 30 % chez les hommes et 34 % chez les femmes. L’experimentation du cannabis est aussi tres repandue. L’incidence des pathologies respiratoires chroniques et des cancers bronchopulmonaires reste tres elevee sur le territoire. Les lois les plus recentes de protection des non-fumeurs n’existent pas encore sur le territoire. Cette etude vise a decrire les comportements de sante des jeunes de 14 a 18 ans, scolarises en secteur public a Noumea, en matiere de consommation de tabac et cannabis. Cette enquete examine la maniere dont ces conduites different selon la communaute, l’âge, le sexe, le lieu de residence et les conditions socioeconomiques afin de cerner les groupes les plus exposes. Enfin, on etudie le retentissement de ces consommations sur l’etat respiratoire des jeunes scolarises. Methodes L’enquete de type declaratif a ete realisee du 04 au 15 mai 2009 dans 16 classes tirees au sort parmi les colleges et lycees de Noumea (439 eleves). L’enquete s’est appuyee sur un questionnaire anonyme comportant 46 questions regroupees en cinq themes : description sociodemographique du sujet et de sa famille, vie familiale, consommation de tabac, consommation de cannabis et autoquestionnaire respiratoire. La passation des questionnaires d’une duree moyenne de 25 minutes s’est deroulee en classe en presence du medecin realisant l’enquete. Les donnees ont ete analysees avec le logiciel Ethnos-4®. L’analyse s’est faite en deux etapes : etude descriptive apres analyse uni- et bivariee et etude analytique pour identifier les facteurs de risque des conduites addictives. Les tests statistiques utilises sont le test du Khi2 de Pearson et le test d’Hosmer-Lemeshow pour le modele logistique. Resultats Le taux de participation a ete de 95,2 % (n = 415). L’echantillon etait majoritairement feminin (56,6 %) et 31 % des eleves avaient moins de 16 ans. Le nombre de fumeurs de tabac etait important (41,1 %), 27,3 % etant des consommateurs quotidiens parmi lesquels 38,5 % fumaient plus de six cigarettes par jour. Les resultats etaient dependants du sexe (predominance feminine) mais independants de l’origine ethnique et des donnees sociodemographiques. Pour le cannabis, le nombre de fumeurs etait de 48 % dont 32 % de consommateurs reguliers. Les fumeurs quotidiens etaient uniquement des garcons et l’usage plus frequent chez les Melanesiens. A peine 11 % des eleves etaient preoccupes par leur consommation et parmi eux, seulement 14 % desiraient arreter. On ne retrouvait pas plus d’asthmatiques dans les groupes fumeurs et seuls les fumeurs reguliers etaient plus a risque de bronchite. Conclusions En Nouvelle-Caledonie, les niveaux de consommation tabagique sont eleves par rapport a ceux des pays europeens. On ne retrouve pas de tendance a la baisse si on les compare aux donnees anterieures. L’usage du cannabis reste tres repandu et de facon precoce avec une predominance masculine pour la consommation quotidienne. Pour ces deux addictions, la perception du risque et le desir d’arreter sont faibles. La population etudiee dans cette etude est representative des adolescents scolarises dans le secteur public sur l’agglomeration du Grand-Noumea.

Resume Introduction Les actes chirurgicaux pratiques par les dermatologues occupent une place croissante dans le traitement des tumeurs cutanees. Objectif de l’etude Le but etait de decrire cette activite afin de la valoriser aupres des autorites et de la faire connaitre des autres specialites et du grand public. Patients et methodes Nous avons etudie prospectivement l’activite des centres hospitaliers et hospitalo-universitaires de France, ainsi que celle des dermatologues du groupe chirurgical de la Societe francaise de dermatologie, durant une periode d’un mois. Les donnees etaient recueillies a partir d’un questionnaire commun preetabli. Elles etaient saisies sur les logiciels EpiData 3.1 et Excel et analysees sur le logiciel SPSS 15.0. Resultats Le taux de participation des services de dermatologie etait de 66,7 % ; celui des dermatologues liberaux de 43,1 %. Deux mille trois cent huit actes ont ete recenses. La moyenne d’âge des patients operes etait de 55,5 ± 20,7 ans. Trois cent trente-six actes (14,6 %) avaient ete effectues sous antithrombotiques. Pour 71,3 % des tumeurs malignes (685 cas), l’exerese avait ete realisee sans biopsie prealable, avec des marges adaptees a la suspicion clinique. L’exerese avec reconstruction en un temps etait la plus frequente (92,2 %), avec dans la majorite des cas une fermeture par suture directe (83,4 %). Les taux d’exerese complete d’emblee et de correlation entre le diagnostic evoque et l’analyse anatomopathologique etaient excellents (95,2 et 89,5 % respectivement). Le taux global de complications etait de 3 %, majore sous traitement antithrombotique. Conclusion Il s’agit, a notre connaissance, de la premiere etude prospective analysant l’activite de chirurgie des dermatologues en France. Le fort taux de participation met en avant leur implication dans une prise en charge optimale des cancers cutanes.

Introduction Le secukinumab (SK), anticorps monoclonal humain neutralisant selectivement l’IL-17A, est efficace dans le traitement du psoriasis en plaques modere a severe et du rhumatisme psoriasique, avec une action rapide et durable et un profil de tolerance favorable. Nous rapportons ici les resultats post-hoc d’une etude francaise evaluant les effets de SK chez des patients ayant un psoriasis modere a severe traites en premiere ligne par le SK. Materiel et methodes L’etude IPSI-PSO (EUDRACT 2014-003666-25) etait prospective, non comparative, multicentrique. Les patients devaient avoir un psoriasis en plaques modere a severe (PASI ≥ 12) depuis au moins 6 mois et etre candidats a un traitement systemique. SK (300 mg) etait administre selon l’AMM jusqu’a la semaine 48 (S48). L’efficacite sur le blanchiment des lesions etait evaluee d’apres les reponses PASI 90 et PASI absolu ≤ 3 (selon la valeur seuil de l’algorithme de traitement des recommandations francaises sur la prise en charge du psoriasis). L’impact sur la qualite de vie etait mesure a l’aide du questionnaire DLQI (indice de qualite de vie en dermatologie). Resultats Parmi les 120 patients inclus n = 63 etaient naifs de biotherapies et 57 etaient non-naifs. Chez ces patients naifs, la proportion de repondeurs PASI 90 etait de 81 % a la S16 versus 54,9 % chez les non-naifs. Le taux de reponse etait maintenu jusqu’a S52 pour 80 % vs 47,6 % respectivement. Le pourcentage de patients avec un PASI absolu ≤ 3 a S16 etait de 86,2 % chez les naifs versus 73,1 % chez les non-naifs. Les taux de reponse respectifs a S52 etaient de 86,0 % versus 65,1 %. L’amelioration des scores DLQI etait en moyenne de −89 % chez les naifs versus −76,9 % chez les non-naifs a S16 (−11,47 vs −10,74, reduction absolue) et maintenue jusqu’a S52 (−88,5 % vs −72,6 %). Discussion Les 2 sous-groupes de patients naifs et non naifs ont montre une meme cinetique de reponse au traitement par SK. Cependant, les taux de repondeurs PASI 90, la proportion de patients avec un PASI absolu faible ainsi que la diminution du score DLQI etaient numeriquement superieurs dans le sous-groupe de patients naifs jusqu’a la semaine 52. La limite de cette analyse reside cependant dans son caractere post-hoc et l’absence, a ce stade, de tests statistiques. D’autres etudes concues pour mesurer l’absence d’impact de l’exposition anterieure a des biotherapies sur la qualite de reponse au SK sont necessaires pour confirmer ces resultats. Conclusion Le traitement par SK entraine une reponse efficace et maintenue en termes de blanchiment des lesions de psoriasis et d’amelioration de la qualite de vie, a la fois chez des patients naifs et non naifs de biotherapies.

Resume Probleme La bronchiolite aigue du nourrisson a fait l’objet d’une Conference de consensus (CC) en 2000. Objectifs Notre etude propose d’evaluer son application en medecine generale a 5 ans d’intervalle. Materiel et methodes Deux cas cliniques standardises de bronchiolite aigue du nourrisson (forme moderee et grave) ont ete adresses a 762 (2003) et 800 (2008) medecins generalistes tires au sort. On attendait : bon diagnostic et traitement symptomatique exclusif a domicile pour la forme moderee ; bon diagnostic et hospitalisation pour la forme grave. Les traitements inutiles (bronchodilatateurs, corticoides, antibiotiques) ou deleteres (fluidifiants, antitussifs), la connaissance des recommandations, le mode d’acces a ces recommandations etaient aussi evalues. Les donnees 2003–2008 ont ete comparees par test t de Student ou test du Chi2. Les facteurs de bonne adhesion a la CC ont ete evalues en analyse multivariee. Resultats Le taux de reponse etait similaire en 2003 et 2008 (25 % et 23 %). La prise en charge conforme des 2 cas etait de 6 % en 2003 vs 20 % en 2008 (p Discussion La prise en charge de la bronchiolite aigue du nourrisson reste delicate, avec encore sur-traitement des formes moderees. Conclusion Meme a distance de la publication de recommandations, leur application reste difficile.

Resume L’entorse de la cheville chez l’enfant et l’adolescent est un motif frequent de consultation dans les services d’urgences. Cependant, il n’existe pas de consensus sur la realite et le type des lesions anatomiques presentes sur ces chevilles. L’objectif de ce travail a ete de documenter chez un nombre important d’enfants et d’adolescents l’ensemble des lesions osseuses et ligamentaires a la suite d’une entorse de la cheville et ce grâce a l’imagerie par resonance magnetique. Il s’agit d’une etude prospective menee chez 116 enfants se presentant pour un traumatisme isole de la cheville sans fracture sur les radiographies standard. Un interrogatoire et un examen clinique standardise ont ete conduits, puis un examen par resonance magnetique a ete realise. L’ensemble des examens radiologiques a ete etudie par un radiologue ne connaissant pas les donnees de l’examen clinique. Nous avons denombre des lesions ligamentaires mineures chez 20 patients et une rupture ligamentaire chez cinq patients. Parmi ces cinq patients, trois avaient un cartilage de croissance tibial distal ferme. Nous avons denombre 42 lesions osseuses mineures et sept fractures. Aucune de ces fractures n’etait visible sur les radiographies standard. Les garcons ont presente plus de lesions que les filles. Nous avons retrouve un plus grand nombre de lesions lorsque la douleur etait surtout localisee sur la face externe de la cheville et lorsqu’il existait un œdeme de la cheville.

This study aims to validate the Multiple Sclerosis (MS) International Quality of Life (MusiQoL) questionnaire, a multi-dimensional, self-administered questionnaire, available in 14 languages, as a disease-specific quality of life scale that can be applied internationally. A total of 1992 patients with different types and severities of MS from 15 countries were recruited. At baseline and day 21 ± 7, each patient completed the MusiQoL, a symptom checklist and the short-form (SF)-36 QoL questionnaire. Neurologists also collected socio-demographic, MS history and outcome data. The database was randomly divided into two subgroups and analysed according to different patient characteristics. For each model, psychometric properties were tested and the number of items was reduced by various statistical methods. Construct validity, internal consistency, reproducibility and external consistency were also tested. Nine dimensions, explaining 71% of the total variance, were isolated. Internal consistency and reproducibility were satisfactory for all the dimensions. External validity testing revealed that dimension scores correlated significantly with all SF-36 scores, but showed discriminant validity by gender, socio-economic and health status. Significant correlations were found between activity in daily life scores and clinical indices. Objective Compare the irradiation delivered in conventional radiography and digital radiography by image intensifier during a scoliosis workup. Patients and methods Our prospective randomized study included 105 patients, all of whom were identified according to sociodemographic parameters as well as criteria evaluating the quality of the full front spinal x-ray at PA incidence. The entry dose at the scapula and the exit dose in interorbital, thyroid, mammary, and hypogastric projection was measured by thermoluminescent dosimeters. Results The results of 71 girls and 28 boys, aged a mean 13.8 years with a mean weight of 47 kg were analyzed. At equal image quality, the entry dose was not significantly different between the two techniques; the mean exit dose reduction was 64% during digital acquisition. This reduction involved the interorbital (162%), mammary (43%), and thyroid (309%) regions. However, this system is more irradiating in the hypogastric region (34%). Conclusion The dosimetric evaluation of the different imaging techniques used to explore the entirety of the spine should be part of radiologists' quality standard used to document their work and their choices.

Resume Justificatif La chimiotherapie adjuvante a la chirurgie ameliore la survie des cancers bronchiques non a petites cellules (CNAPC) completement reseques. Methodes L’objectif principal de cet essai multicentrique, comparatif, ouvert, randomise, est de comparer deux schemas de chimiotherapie (cisplatine/docetaxel versus cisplatine/gemcitabine), dans la prise en charge du CNAPC reseque, en matiere d’impact sur la qualite de vie (QdV) evaluee a la fin du traitement. Les objectifs secondaires sont d’etudier l’impact de ces deux therapeutiques sur la survie, les toxicites hematologiques et extra-hematologiques et les couts. Le critere de jugement principal est represente par la mesure de la QdV a l’aide d’un questionnaire standardise, l’EORTC QLQ-C30. Les criteres de jugement secondaires sont multiples : QdV mesuree par l’EORTC QLQ-LC13 et le SF36, survies globales et sans progression, profil de tolerance et couts. Le nombre de sujets necessaires est de 75 sujets par groupe, 150 au total (hypotheses : alpha 0.05, puissance 80 %, difference attendue en terme de score EORTC QLQ-C30 de 10 points, ecart-type de 20 points). Resultats attendus Cet essai fournira aux cliniciens des donnees sur l’impact sur la qualite de vie, la tolerance et les couts de deux combinaisons de chimiotherapie adjuvante non explorees a ce jour dans les CNAPC.

Resume Objectif. – Analyser les transferts entre les differents etablissements du systeme de sante identifies dans la region Provence–Alpes–Cote-d'Azur (PACA) dans le cadre de la prise en charge de l'infarctus du myocarde. Methode. – Appliquer une methodologie initialement utilisee en sociologie, l'analyse des reseaux sociaux qui permet de decrire et de modeliser la structure relationnelle d'un ensemble d'acteurs, representes dans ce travail par les etablissements de soins. Les flux entre les acteurs sont representes par les transferts de patients. Cette approche est innovante dans le cadre de l'analyse du systeme sanitaire. Resultats. – Le nombre total d'etablissements de sante ayant pris en charge un infarctus du myocarde est de 74. Le nombre de patients hospitalises pour un infarctus du myocarde est de 2049. L'âge moyen est de 68,9 ans. La densite du reseau hospitalier est de 0,12. Le nombre de cliques mis en evidence est de 51. Les principaux facteurs susceptibles de diminuer la survenue de deces hospitaliers sont soit lies aux caracteristiques des etablissements : prestige ; soit lies au patient et a sa prise en charge : âge de moins de 65 ans, l'angioplastie et la thrombolyse associee a l'angioplastie par rapport a l'absence de revascularisation. Conclusion. – L'analyse des reseaux sociaux offre une vue synthetique des relations du systeme de sante. Elle constitue un champ de recherche ouvert en sante publique, pouvant egalement etre appliquee a l'evaluation des reseaux de sante et au fonctionnement d'un etablissement.
